Background: This systematic review and meta-analysis aimed to assess the impact of inspiratory muscle training (IMT) on lung function parameters (MIP, MEP, FEV1, and FVC) between both elite and non-elite swimmers. Methods: We searched for controlled clinical trials (CCT) and prospective longitudinal studies (PLS) in elite and non-elite swimmers following an inspiratory muscle training (IMT) protocol with a standardized device, published between 2012 and 2023. The databases used in the search were PubMed, Science Direct, Scopus, Springer, Cochrane Central Register of Controlled Trials, and Google Scholar. The primary outcome assessed was the impact of IMT on lung function parameters, including MIP, MEP, FEV1, and FVC. Results: We selected 13 articles involving 277 subjects aged 11-21 years, with 61.4% being male, and 84.6% being elite swimmers. The most commonly used IMT device was the PowerBreathe®, prescribed for 3-12 weeks, 1-2 sessions per day, 3-6 times per week, with 30 repetitions, starting at 50% of MIP and progressing up to 80%. The meta-analysis showed that IMT was associated with a higher MIP (MD = 29.35â cmH2O, 95% CI: 13.04-45.65â cmH2O, p < 0.01) without affecting FEV1 and FVC. Conclusion: The swimmers that used IMT improved muscle strength, specifically MIP, without changes in MEP, FEV1, and FVC.

Purpose: The aim of this pilot study was to analyze the potential financial loss and a range of potential risk factors for hamstring muscle injuries in elite Brazilian soccer. Methods: Thirty-four male players (age: 25 ± 6 years; stature: 180 ± 8â cm; body mass: 78 ± 9â kg; minutes played in matches: 2243 ± 1423â min) from an elite professional soccer club were monitored during a 12-month season. Muscle injury was identified by magnetic resonance imaging and the severity was defined according to the number of days away: minimal (1-3 days), mild (4-7 days), moderate (8-28 days), severe (>28 days). Potential financial loss due to the team's under achievements was determined. Dorsiflexion range of motion, eccentric knee flexor strength and isokinetic tests were performed during the pre-season. Association between dependent variables and the occurrence of injury was evaluated. Results: Nine hamstring muscle injuries with moderate severity were found in 8 athletes. Recovery time was 22 days off the field on average. Potential financial loss was $-43.2 million USD and earnings on merit money was 21%. Previous injury, increased flexor deficit 60°â /sec and increased flexor fatigue index 300°/sec were all associated with a greater chance of hamstring muscle injury. Ankle dorsiflexion range of motion was significantly lower in the injured group (35.6 ± 3° vs. 39.1 ± 4.9°; p = 0.017, effect size = -0.74). Conclusion: High financial burden was found in elite Brazilian soccer during one full season. Injured athletes had high hamstring fatigue index, knee flexor strength deficit, ankle range of motion restriction and previous hamstring muscle injury when compared to non-injured athletes. Therefore, preventive approaches in professional soccer players with previous hamstring injuries should be a priority.

Objective: To evaluate and compare the sexual function and pelvic floor muscles (PFM) function of women with endometriosis and chronic pelvic pain (CPP) with and without Myofascial Pelvic Pain Syndrome (MPPS). Methods: Cross-sectional study conducted between January 2018 and December 2020. Women with deep endometriosis underwent assessments for trigger points (TP) and PFM function using the PERFECT scale. Electromyographic activity (EMG) and sexual function through Female Sexual Function Index (FSFI) were assessed. Statistical analyses included chi-square and Mann-Whitney tests. Results: There were 46 women. 47% had increased muscle tone and 67% related TP in levator ani muscle (LAM). Weakness in PFM, with P≤2 was noted in 82% and P≥3 in only 17%. Incomplete relaxation of PFM presented in 30%. EMG results were resting 6.0, maximal voluntary isometric contraction (MVIC) 61.9 and Endurance 14.2; FSFI mean total score 24.7. We observed an association between increased muscle tone (P<.001), difficulty in relaxation (P=.019), and lower Endurance on EMG (P=.04) in women with TP in LAM. Participants with TP presented lower total FSFI score (P=.02). TP in the right OIM presented increased muscle tone (P=.01). TP in the left OIM presented lower values to function of PFM by PERFECT (P=.005), and in MVIC (P=.03) on EMG. Conclusion: Trigger points (TP) in pelvic floor muscles (PFM) and obturator internus muscle (OIM) correlates with poorer PFM and sexual function, particularly in left OIM TP cases. Endometriosis and chronic pelvic pain raise muscle tone, weaken muscles, hinder relaxation, elevate resting electrical activity, lower maximum voluntary isometric contraction, and reduce PFM endurance.

The synthetic nitro-alcohol 2-nitro-1-phenyl-1-propanol (NPP) has endothelium-independent relaxing properties in isolated preparations of rat aorta and mesenteric artery. In this study, we investigated whether the vasodilator effects occur in coronary vessels and explored whether hyperpolarization is involved in the underlying mechanism of NPP-induced smooth muscle relaxation. The relaxing responses were studied in isolated preparations of the left anterior descending coronary (ADC) and the septal coronary (SC) arteries, which had been previously maintained under sustained contraction induced by the thromboxane A2 analogue U-46619. Administered cumulatively, NPP elicited concentration-dependent vasorelaxation with similar potency in both vessels. The relaxant effect remained unaffected by the nitric oxide synthase inhibitor L-NAME, the protein kinase C inhibitor bisindolylmaleimide IV and the Rho-associated protein kinase inhibitor Y-27632. However, it was significantly diminished by the adenylyl cyclase inhibitor MDL-12,330A, the guanylyl cyclase inhibitor ODQ, as well as the K+ channel inhibitors tetraethylammonium and CsCl. In ADC preparations impaled with intracellular micropipettes, NPP hyperpolarized the vascular preparation. When the isolated preparation was precontracted by 5-hydroxytryptamine or 80 mM KCl, NPP-induced relaxation with lower pharmacological potency compared to the vessels contracted by U-46619. In conclusion, NPP exhibits vasorelaxant effects on rat coronary arteries, likely involving pathways that include cyclic nucleotide production and membrane hyperpolarization.

Obesity is a major public health problem and is a major contributor to the development of insulin resistance. In previous studies we observed that single-wavelength red or infrared photobiomodulation (PBM) improved insulin signaling in adipocytes and skeletal muscle of mice fed a high-fat diet, but information about the combination of different wavelengths, as well as the effect of different light doses (J/cm2) is lacking. Therefore, the aim of this study was to investigate the effects of different doses of dual-wavelength PBM on insulin signaling in muscle cell, and explore potential mechanisms involved. Mouse myoblasts (C2C12) were differentiated into myotubes and cultured in palmitic acid, sodium oleate and l-carnitine (PAL) to induce insulin resistance high or in glucose medium (CTRL). Then, they received SHAM treatment (lights off, 0 J/cm2) or PBM (660 + 850 nm; 2, 4 or 8 J/cm2). PAL induced insulin resistance (assessed by Akt phosphorylation at ser473), attenuated maximal citrate synthase activity, and increased the phosphorylation of c-Jun NH(2) terminal kinase (JNK) (T183/Y185). PBM at doses of 4 or 8 J/cm2 reversed these PAL-induced responses. Furthermore, at doses of 2, 4 or 8 J/cm2, PBM reversed the increase in mitofusin-2 content induced by PAL. In conclusion, the combination of dual-wavelength red and infrared PBM at doses of 4 and 8 J/cm2 improved intracellular insulin signaling in musculoskeletal cells, and this effect appears to involve the modulation of mitochondrial function and the attenuation of the activation of stress kinases.

OBJECTIVE: The purpose of this study was to investigate the feasibility of the use of shear wave elastography (SWE) in comparison to chemical shift encoding (CSE) magnetic resonance imaging (MRI) for the evaluation of multifidus muscle fatty degeneration in patients with chronic low back pain. METHOD: Multifidus muscles were evaluated with the CSE-MRI and SWE examinations in control and patient groups. With the in-phase and out-phase sequences in CSE-MRI, signal intensity index (SII), and signal intensity suppression ratio (SISR) values; with the SWE method, shear wave velocity values were determined. Differences in the mean values of these parameters per level and study group were analyzed by Student's t-test. RESULTS: SWE revealed significantly lower stiffness at the L2-3 level, consistent with the signal index values (SII-SISR) showing increased fatty infiltration on MRI in the patient group. No such relationship was found at the L4-5 level or in control group. CONCLUSIONS: SWE may be a promising method to show muscle fatty infiltration at L2-3 level in patients with chronic low back pain.
OBJETIVO: Investigar la viabilidad del uso de la elastografía de ondas de corte en comparación con la resonancia magnética con codificación de desplazamiento químico (RM-CDQ) para la evaluación de la degeneración grasa del músculo multífido en pacientes con dolor lumbar crónico. MÉTODO: Los músculos multífidos se evaluaron con RM-CDQ y elastografía de ondas de corte en los grupos de control y de pacientes. Se consideraron las secuencias en fase y fuera de fase en RM-CDQ, y los valores del índice de intensidad de señal y del índice de supresión de intensidad de señal; con el método de elastografía de ondas de corte se determinaron los valores de velocidad de onda de corte. Las diferencias en los valores medios de estos parámetros por nivel y por grupo de estudio se analizaron mediante la prueba t de Student. RESULTADOS: La elastografía de ondas de corte reveló una rigidez significativamente menor en el nivel L2-3, consistente con los valores de los índices de señal que muestran una mayor infiltración grasa en la RM en el grupo de pacientes. No se encontró tal relación en el nivel L4-5 ni en el grupo de control. CONCLUSIONES: La elastografía de ondas de corte puede ser un método prometedor para mostrar la infiltración grasa muscular a nivel L2-3 en pacientes con dolor lumbar crónico.

BACKGROUND AND OBJECTIVE: Frailty in older adults is a geriatric syndrome that has gained importance in the last decade. However, there is still no consolidated information regarding diagnostic tools that allow timely identification, and therefore, provide an appropriate therapeutic approach. The objective is to determine the Thickness of the Rectus Femoris Muscle (GMRF) and Thickness of the Vastus Intermedius Muscle (GMVI) in older Mexican adults with frailty and sarcopenia. MATERIAL AND METHODS: Cross-sectional, descriptive and comparative study in patients ≥65 years of age, admitted to the geriatrics service. The selection was according to the FRAIL (frail vs. non-frail) and SARC-F (high risk vs. low risk) classification; evaluating GMRF and GMVI by ultrasound. The data were analyzed through the statistical software Statistical Package for Social Sciences (SPSS) ver. 25. RESULTS: The number of patients evaluated in the study and control group were 136 respectively. Significant differences were found regarding frailty status in age (years) (Frail: 75.06±7.92 vs. Non-frail: 71.60±5.56; P<.001), GMRF (mm) (Frail: 8.41±3.08 vs. Non-frail: 11.03±3.50; P<.001) and GMVI (mm) (Frail: 6.53±2.64 vs. Non-frail: 8.66±2.68; P<.001); Considering sarcopenia, there were differences in age (years) (High risk sarcopenia: 75.17±7.84: vs low risk sarcopenia; 71.49±5.60; P<.001), GMRF (mm) (high risk sarcopenia: 8.45±3.11 vs low risk sarcopenia: 10.98±3.50; P<.001) and GMVI (mm) (High risk Sarcopenia: 6.67±2.63 vs Low risk Sarcopenia: 8.52±2.79; P<.001). CONCLUSIONS: The results found show that there is a significant difference in GMRF and GMVI in Mexican older adults with respect to frailty and sarcopenia. In this way, the present investigation establishes clinical bases for the use of ultrasonography assessments in the geriatric population.

AIM: This study examined the association of low handgrip strength (HGS) for falls in middle-aged adults and older adults every half-decade of life. METHODS: This cross-sectional study was conducted using the public data from the first wave of the Brazilian Longitudinal Study of Aging (ELSI-Brazil). The participants were allocated into seven age groups 50-54, 55-59, 60-64, 65-69, 70-74, 75-79, and ≥ 80 years. Binary logistic regression analysis was performed to identify the odds ratio (OR) of low HGS to the falls regardless of confounding variables such as sex, balance, gait speed, and total number of health conditions. RESULTS: A total of 8,112 participants aged 50-105 years (median = 62.0 years): 3,490 males (median = 60.0 years) and 4,622 females (median = 63.0 years) attended the study. Altogether, 21.5% of participants experienced at least one fall. HGS gradually decreases over each half-decade of life. In addition, low HGS presented a significative OR (p < 0.05) for falls for age groups, until 80 s, even when considering confounding variables. CONCLUSIONS: Low HGS is associated with falls in middle-aged adults over their 50 s and remained a strong measure of falls across each subsequent half-decade of life, until 80 s.

Objective: To evaluate the incidence of injuries to the infrapatellar branch of the saphenous nerve (IPBSN) after anterior cruciate ligament reconstruction (ACLR) with an oblique incision for hamstring graft harvesting. Methods: In total, 59 knees (from 57 patients) were evaluated in the follow-up of ACLR for six months. We drew a horizontal line parallel to the ground, passing through the most medial portion of the surgical incision and another, perpendicular to the first, starting at the tibial tuberosity (TT). We measured the length and angle of the cut, the distances from its most medial point to the perpendicular line, and from the TT to the horizontal line. Skin sensitivity was tested with a brush and the altered sensitivity area was measured. Patients were asked about difficulties in activities daily of living (ADL). Results: A total of 27 knees (45.7%) had sensory disorders, which persisted until the sixth postoperative month in 92.6% of them. The ADL were compromised in one knee (3.7%). No significant differences were found between the groups with and without changes in sensitivity regarding age, affected side, incision angle, or measured distances. The incision size was larger in the group without alteration in sensitivity. Conclusions: An oblique incision did not avoid IPBSN injuries. This condition rarely compromised the ADL. Objetivo: Avaliar a incidência de lesões do ramo infrapatelar do nervo safeno (RIPNS) na reconstrução do ligamento cruzado anterior (RLCA), com incisão oblíqua para a coleta do enxerto dos isquiotibiais. Métodos: 59 joelhos (57 pacientes) foram avaliados no pós-operatório da RCLA, por seis meses. Traçamos uma linha horizontal paralela ao solo, passando pela porção mais medial da incisão cirúrgica, e outra perpendicular à esta, iniciando na tuberosidade tibial (TT). Medimos o comprimento e a angulação do corte, as distâncias do ponto mais medial do corte à linha perpendicular e outra, da TT, à linha horizontal. A sensibilidade da pele foi testada com um pincel, e a área alterada foi mensurada. Os pacientes foram questionados sobre as dificuldades nas atividades diárias da vida (ADV). Resultados: 27 joelhos (45,7%) apresentaram distúrbios sensitivos, persistentes até o sexto mês pós-operatório em 92,6% deles. As ADV foram comprometidas em um joelho (3,7%). Não houve diferença significante entre os grupos com e sem alterações da sensibilidade, relativamente à idade, ao lado comprometido, ao ângulo da incisão ou às distâncias medidas. O tamanho da incisão foi maior no grupo sem alteração de sensibilidade. Conclusões: Uma incisão oblíqua não evitou lesões no RIPNS. Essa condição raramente comprometeu as ADV. Osteosarcopenia is a major driver of functional loss and a risk factor for falls, fractures, disability and mortality in older adults, urgently requiring the development of effective interventions to address it. The hallmarks of aging provide a theoretical and practical framework that allows for the structured organization of current knowledge and the planning of new development lines. This article comprehensively reviews the currently available literature on the role of the hallmarks of aging in the development of osteosarcopenia, thereby offering a panoramic view of the state of the art and knowledge gaps in this field.

Sarcopenia is characterized by a decline in muscle strength, generalized loss of skeletal muscle mass, and impaired physical performance, which are common outcomes used to screen, diagnose, and determine severity of sarcopenia in older adults. These outcomes are associated with poor quality of life, increased risk of falls, hospitalization, and mortality in this population. The development of sarcopenia is underpinned by aging, but other factors can lead to sarcopenia, such as chronic diseases, physical inactivity, inadequate dietary energy intake, and reduced protein intake (nutrition-related sarcopenia), leading to an imbalance between muscle protein synthesis and muscle protein breakdown. Protein digestion and absorption are also modified with age, as well as the reduced capacity of metabolizing protein, hindering older adults from achieving ideal protein consumption (i.e., 1-1.5 g/kg/day). Nutritional supplement strategies, like animal (i.e., whey protein) and plant-based protein, leucine, and creatine have been shown to play a significant role in improving outcomes related to sarcopenia. However, the impact of other supplements (e.g., branched-chain amino acids, isolated amino acids, and omega-3) on sarcopenia and related outcomes remain unclear. This narrative review will discuss the evidence of the impact of these nutritional strategies on sarcopenia outcomes in older adults.

Muscle tone is defined as the resistance to passive stretch, but this definition is often criticized for its ambiguity since some suggest it is related to a state of preparation for movement. Muscle tone is primarily regulated by the central nervous system, and individuals with neurological disorders may lose the ability to control normal tone and can exhibit abnormalities. Currently, these abnormalities are mostly evaluated using subjective scales, highlighting a lack of objective assessment methods in the literature. This study aimed to use surface electromyography (sEMG) and machine learning (ML) for the objective classification and characterization of the full spectrum of muscle tone in the upper limb. Data were collected from thirty-nine individuals, including spastic, healthy, hypotonic and rigid subjects. All of the classifiers applied achieved high accuracy, with the best reaching 96.12%, in differentiating muscle tone. These results underscore the potential of the proposed methodology as a more reliable and quantitative method for evaluating muscle tone abnormalities, aiming to address the limitations of traditional subjective assessments. Additionally, the main features impacting the classifiers' performance were identified, which can be utilized in future research and in the development of devices that can be used in clinical practice.

BACKGROUND/OBJECTIVES: The oral administration of hydrolyzed collagen peptides is a scientifically validated intervention for enhancing skeletal muscle health and performance. This integrative review consolidates the evidence supporting the use of low molecular weight collagen peptides (2000-3500 daltons) for their superior bioavailability and absorption. Our objective was to review the effects of collagen peptide or hydrolyzed collagen supplementation on muscle damage, recovery, and construction related to physical exercise. METHODS: A bibliographic search was conducted in major English-language databases, including PubMed/Medline, using terms like "Peptides Collagen and Damage" and "collagen peptides AND Soreness Muscle". This review followed PRISMA guidelines, with bias risk assessed via the PEDro scale. The inclusion criteria were (a) randomized clinical trials, (b) randomized studies in humans with a control or placebo group, (c) studies assessing muscle damage or delayed onset muscle soreness via physiological markers or strength performance tests, and (d) studies using hydrolyzed collagen or collagen peptides. RESULTS: Initially, 752 articles were identified. After applying the inclusion and exclusion criteria, including duplicate removal, eight articles with 286 participants were included. Of these, 130 participants received collagen peptide supplementation, while 171 received a placebo or control. CONCLUSION: This integrative review supports the potential of collagen peptide supplementation to mitigate muscle stress from acute strenuous resistance training. However, due to the methodological heterogeneity among the studies, further clinical trials are needed to clarify the mechanisms underlying muscle improvement with collagen supplementation.

Systematic reviews and meta-analyses support the benefits of inspiratory muscle training (IMT) for sports and clinical populations. A typical application of "standalone" IMT intervention consists of breathing against an inspiratory load (IRL), twice daily, for five to seven days per week, for four to twelve weeks. However, the application of IRL during aerobic exercise is often seen in a training routine of sports and rehabilitation centres with noevidencebased guide. In this Current Opinion, we will revisit putative mechanisms underlying the established benefits of "standalone" IMT to support our contention that IMT need not and should not be used during aerobic exercise.

PURPOSE: Our study aimed to 1) investigate the differences of muscle parameters in relation to each SARC-F component/question; and 2) explore the relationship between SARC-F score with these muscle parameters using various landmarks derived from computed tomography (CT) scans of patients with cancer. METHODS: This study is a cross-sectional analysis of a cohort comprised of consecutive patients with cancer, displaying CT scans. SARC-F questionnaire was utilized as a proxy for muscle functionality, with a score ≥4 indicating a poor status. Muscle assessment via CT measurements was performed using single cross-sectional images at the level of the third lumbar vertebrae (L3) in the abdominal region, the thigh region, and the total gluteal region at the level of the second sacral vertebrae. Skeletal muscle (SM) cross-sectional area, SM index (normalized to height2), and SM radiodensity (SMD) were evaluated for all anatomical landmarks. RESULTS: A total of 128 patients were included in this analysis (53.1% females, 61.7% older adults). Patients with SARC-F scores ≥4 demonstrated significantly lower values of SMD across all landmarks assessed. Those reporting difficulties related to strength (P = 0.039), requiring assistance in walking (P = 0.033), and climbing stairs (P = 0.012) exhibited significantly lower SMD values at the L3 landmark. At gluteus and thigh levels, only patients experiencing difficulty climbing stairs (P = 0.012) showed significantly lower values of SMD. Only SMD at gluteus level was independently associated with SARC-F score (ßadjusted -0.09, 95% CI -0.16 to -0.02). CONCLUSIONS: Our findings suggest that individuals with poor muscle composition may experience a higher risk of sarcopenia/poor muscle functionality.

The distribution and amount of intramuscular fat and fibrous tissue can be influenced by biological sex and impact muscle quality in both the functional (force-generating capacity) and morphological (muscle composition) domains. While ultrasonography (US) has proven effective in assessing age- or sex-related differences in muscle quality, limited information is available on sex differences in children. Quantitative ultrasonographic measurements, such as echo intensity (EI), EI bands (number of pixels across 50-unit intervals) and texture, may offer a comprehensive framework for identifying sex differences in muscle composition. The aim of our study was to examine the effect of sex on the rectus femoris (RF) muscle quality in children. We used EI (mean and bands) and texture as muscle quality estimates derived from B-mode US. We hypothesised that RF muscle quality differs significantly between girls and boys. Additionally, we also hypothesised that there is a significant correlation between EI bands and texture. Forty-four non-active healthy children were recruited (n = 22 girls, 12.8 ± 1.5 years; and n = 22 boys, 13.5 ± 1.2 years). RF was assessed using EI mean, EI bands, and texture analysis (homogeneity and correlation) using the Gray-Level Co-Occurrence Matrix. The results revealed significant (p < 0.05) sex differences in RF EI bands and texture. Boys displayed higher values in the 0-50 EI band and had more homogeneous muscle texture than girls. Conversely, girls displayed greater values in the 51-100 EI band and had less homogenous texture compared to boys (p < 0.05). A positive correlation was observed between the 0-50 EI band and muscle homogeneity. However, the 51-100 EI band correlated negatively with homogeneity (p < 0.05), particularly for girls. In conclusion, our study revealed sex-specific differences in mean EI, EI bands, and texture of the RF muscle in children. The variations in the correlations between the first and second EI bands and texture reveal different levels of homogeneity in each band. This indicates that distinct muscle tissue constituents, such as intramuscular fat and/or connective tissue, may be reflected in EI bands. Overall, the methods used in this study may be useful for examining muscle quality in healthy children and those with medical conditions.

BACKGROUND: This study aimed to determine the threshold of muscle power and strength enhancements that lead to functional gains after exercise intervention in an acute care unit. METHODS: A total of 302 older patients (intervention: 169, control: 133) from two randomized clinical trials were included (mean age 86.7 years). We measured maximal strength (1RM) and muscle power via a velocity transducer during leg press exercise at 30% and 60% of 1RM. A multicomponent exercise program, including power training, balance, and gait exercises performed over 3 to 6 consecutive days, served as the intervention. We used an anchor-based method to correlate muscle function increases with the Short Physical Performance Battery (SPPB) and gait velocity (GVT) to define clinically meaningful improvements (CMI). RESULTS: In the intervention group, marked differences were found in maximal power at 30% of 1RM between SPPB responders and non-responders (relative 83.5% vs. 34.8%; absolute 33.0 vs. 12.8 W; P<0.05) and at 60% of 1RM (relative 61.1% vs. 22.4%; P<0.05). GVT responders demonstrated significantly greater improvements in both relative and absolute maximal power than non-responders at both 30% and 60% of 1RM (P<0.05), as well as greater absolute 1RM gains (21.2 vs. 15.2 kg, P<0.05). CMI for muscle power based on SPPB and GVT ranged from 30.2% to 48.7%, whereas for 1RM, it was 8.2% based on GVT. CONCLUSION: Muscle power gains were most notable in patients with improvements in the SPPB and GVT, highlighting the critical role of muscle power in functional recovery in these patients.

Variable resistance training has been widely used in athletic preparation. OBJECTIVES: To analyze the use of currents (VRT) and the traditional method (TRAD) on speed, power and temperature in a training session. METHODS: Fourteen paralympic powerlifting (PP) athletes took part over three weeks. In week 1, familiarization and 1RM tests took place, and, in weeks 2 and 3, pre- and post-training took place, where the propulsive mean velocity and power and temperatures were assessed before and after, at 24 h and 48 h. RESULTS: There was a difference in the sternal pectoral temperatures before and after VRT (p = 0.040) and at 48 h for TRAD and VRT (p = 0.018); in the clavicular pectoralis before and after VRT and TRAD (p = 0.003); in the anterior deltoid after and at 48 h for TRAD and VRT (p = 0.026 and p = 0.017); and in the triceps after and at 24 h and 48 h between TRAD and VRT (p = 0.005). In the training series, the MPV was significant in TRAD between Set1 and Set5 (p = 0.003), in training (VRT) between Set1 and Set5 (p = 0.001) and in Set5 between the methods (p = 0.047). For power, there was a difference between Set1 and 5 in TRAD (p = 0.016) and VRT (p = 0.002). CONCLUSION: We conclude that training with currents (VRT) promoted greater muscle fatigue when compared to traditional training.

We report on a 14-year-old girl who developed post-transplantation smooth muscle tumours (PTSMT) located in the spleen, lungs, liver, and central nervous system (CNS), 4 years after kidney transplantation. She was asymptomatic, and the disease was detected during the work-up for a urinary tract infection. Diagnosis was performed by the analysis of a tissue specimen, through the biopsy of a lung tumour, which revealed a proliferation of spindle-shaped cells which were positive for actin and vimentin. In situ hybridization studies were positive for Epstein-Barr virus, and her serologic status was negative prior to transplantation. We reduced immunosuppression by stopping mycophenolate and switching tacrolimus for sirolimus. After 18 months of follow-up, she remains asymptomatic, and the CNS tumour reduced its diameter from 24 × 21 mm to 14 × 13 mm. PTSMT should be considered in the differential diagnosis of transplanted patients who develop neoplastic complications associated with immunosuppression.

RATIONALE: Intensive care unit-acquired weakness (ICUAW) is common in critically ill patients, characterized by muscle weakness and physical function loss. Determining risk factors for ICUAW poses challenges due to variations in assessment methods and limited generalizability of results from specific populations, the existing literature on these risk factors lacks a clear and comprehensive synthesis. OBJECTIVE: This overview aimed to synthesize risk factors for ICUAW, categorizing its modifiable and nonmodifiable factors. METHODS: An overview of systematic reviews was conducted. Six relevant databases were searched for systematic reviews. Two pairs of reviewers selected reviews following predefined criteria, where bias was evaluated. Results were qualitatively summarized and an overlap analysis was performed for meta-analyses. RESULTS: Eighteen systematic reviews were included, comprising 24 risk factors for ICUAW. Meta-analyses were performed for 15 factors, while remaining reviews provided qualitative syntheses. Twelve reviews had low risk of bias, 4 reviews were unclear, and 2 reviews exhibited high risk of bias. The extent of overlap ranged from 0 to 23% for the corrected covered area index. Nonmodifiable factors, including advanced age, female gender, and multiple organ failure, were consistently associated with ICUAW. Modifiable factors, including neuromuscular blocking agents, hyperglycemia, and corticosteroids, yielded conflicting results. Aminoglycosides, renal replacement therapy, and norepinephrine were associated with ICUAW but with high heterogeneity. CONCLUSIONS: Multiple risk factors associated with ICUAW were identified, warranting consideration in prevention and treatment strategies. Some risk factors have produced conflicting results, and several remain underexplored, emphasizing the ongoing need for personalized studies encompassing all potential contributors to ICUAW development.
