Abstract Chronic ulcers significantly affect the quality of life of patients and impose a high cost on the healthcare system. The therapeutic management should be comprehensive, taking into consideration the etiological diagnosis of the wound and the characteristics of the wound bed when deciding on a therapeutic proposal appropriate to the healing phase, correcting factors that delay healing. During the epithelialization phase, repair techniques with grafts are recommended to shorten re-epithelialization time, improve the quality of scar tissue, and achieve adequate pain management. Currently, due to the reported benefits of skin appendages, the technique of follicular unit auto-grafting obtained with a scalp punch is among the chosen strategies for wound repair. This is a minimally invasive, outpatient practice, whose technique has advantages over the donor site, patients recovery and well-being.

BACKGROUND: Androgenetic alopecia (AGA) is a prevalent genetic condition that can affect both male and female, and is considered the most frequent form of hair loss. Traditional scales and methods of classifying AGA are basically qualitative. OBJECTIVE: This work aims to propose a quantitative scale to classify AGA in order to assist hair transplantation surgery. METHODS: Based on whole hairless and thinning areas that needs to receive follicular units in a hair transplantation procedure, basic equations to support the scale are proposed. Additionally, the study involves simulations that apply the classification system and compare its results with those of qualitative methods. RESULTS: The PRECISE scale utilizes a range of 0-10, using 30 cm2 as the measured standard of a bald area. For hair transplantation, 1500 follicular units (FU) are recommended for each score in the PRECISE scale. Technological and manual methods to measure the hairless and thinning areas are presented and discussed. This new quantitative classification, combined with different and complementary methods of measurement of hairless and thinning areas endorse the understanding of the clinical condition by the patient and the planning of a surgery procedure. CONCLUSION: The developed PRECISE scale brings a different way of classifying Androgenetic alopecia (AGA), through an essentially quantitative evaluation. It can be used to elaborate the best strategy for the hair transplantation surgery and to improve the outcomes. Coronavirus disease 2019 (COVID-19) has impacted our society in numerous ways. So far (November 2022) around 640 million SARS-CoV-2 infections have been confirmed worldwide. It is known that this disease has a main affection at the cardiopulmonary level, however, it affects almost all the organs and systems of the body, including the skin. Post-COVID-19 syndrome occurs in some patients, leaving significant sequelae that impact their quality of life. Among the most common sequelae are: cough, dyspnea, chronic fatigue, hyposmia and dysosmia. In addition to these, other sequelae not related to respiratory syndrome have been reported, such as hair loss, which has been observed in around 20% of post-COVID-19 patients, this could be attributable to acute telogen effluvium (TE). To date, no study has been conducted to determine the relationship of ET in those who have had COVID-19. Acute ET is defined as diffuse, non-scarring hair loss, usually self-limited, lasting for about 6 months. Within its etiology, severe systemic febrile illness, pregnancy, restrictive diets, severe stress and various drugs have been described. It seriously affects the quality of life of patients and is often associated with loss of confidence and low self-esteem.


La enfermedad por coronavirus 2019 (COVID-19) ha impactado a nuestra sociedad en numerosos aspectos. Hasta el momento (noviembre 2022) se han confirmado alrededor de 640 millones de infecciones por SARS-CoV-2 en el mundo. Se sabe que esta enfermedad tiene afección principal a nivel cardiopulmonar, sin embargo, afecta casi todos los órganos y sistemas del cuerpo, entre ellos, la piel. El síndrome post-COVID-19 se presenta en algunos pacientes dejando secuelas significativas que impactan su calidad de vida. Entre las secuelas más comunes se encuentran: tos, disnea, fatiga crónica, hiposmia y disosmia. Además de estas se han presentado otras secuelas no relacionadas con el síndrome respiratorio, como la caída de pelo, que se ha observado en alrededor del 20% de los pacientes post-COVID-19, esto podría ser atribuible al efluvio telógeno (ET) agudo. Hasta la fecha, no se ha realizado ningún estudio para determinar la relación del ET en quienes han tenido COVID-19. El ET agudo se define como una pérdida de pelo difusa, no cicatrizal, generalmente autolimitada, con una duración de alrededor de 6 meses. Dentro de su etiología, se han descrito la enfermedad febril severa sistémica, el embarazo, las dietas restrictivas, el estrés severo y diversos fármacos. Afecta gravemente la calidad de vida de los pacientes y, a menudo, se asocia con pérdida de confianza y baja autoestima.

Nanotechnology has been investigated for treatments of hair follicle disorders mainly because of the natural accumulation of solid nanoparticles in the follicular openings following a topical application, which provides a drug "targeting effect". Despite the promising results regarding the therapeutic efficacy of topically applied nanoparticles, the literature has often presented controversial results regarding the targeting of hair follicle potential of nanoformulations. A closer look at the published works shows that study parameters such as the type of skin model, skin sections analyzed, employed controls, or even the extraction methodologies differ to a great extent among the studies, producing either unreliable results or precluding comparisons altogether. Hence, the present study proposes to review different skin models and methods for quantitative and qualitative analysis of follicular penetration of nano-entrapped drugs and their influence on the obtained results, as a way of providing more coherent study protocols for the intended application.

Resumen El tricofoliculomaes un tumor benigno infrecuente del folículo piloso, de mayor prevalencia entre la segunda y la sexta década de vida. Suele presentarse como una lesión solitaria en forma de nódulo,pápulas o placas del color de la piel,asintomático, con un poro o cavidad central obstruido con queratina de la que emerge un "penacho de pelos".La localización más frecuente suele ser el rostro, seguido del cuero cabelludo y el cuello.1Las características histopatológicas son diagnósticas. Se reporta el caso clínico de un paciente de 44 años de edad con tricofoliculomas múltiples en rostro.


Abstract Trichofolliculoma is an infrequent benign tumor of the hair follicle, with a higher prevalence between the second and sixth decade of life. It usually presents as a solitary lesion in the form of a nodule, papules or asymptomatic skin-colored plaques with a pore or central cavity obstructed with keratin from which a "tuft of hairs" emerges.The most frequent location is usually the face, followed by the scalp and neck.1 Histopathological characteristics are diagnostic. We report a case of a 44-year-old man with multiple trichofolliculomas on his face.

Background: Pilomatricomas are neoplasms of hair follicles, located in the hair bulb, and the incidence is uncommon in the clinical-surgical clinical routine of dogs and cats. It commonly affects adult animals, with no predisposition to sex or race, and is mainly located in the neck, back, and tail region. The diagnosis is made by histopathological examination, where cells are observed in which their nucleus does not stain with hematoxylin and eosin - empty nucleus. The present work aimed to report a case of benign pilomatricoma since it is a rare condition in dogs and cats and, consequently, there is little information in the literature. Case: A 6-year-old male Shih-Tzu dog, not submitted to surgical contraception, weighing 6.9 kg, was treated at the Veterinary School Hospital (HVE) of the North Parana State University (UENP), Bandeirantes, PR, Brazil, with a history of nodules in the tail region, lasting 6 months. On physical examination, no changes were identified in the patient's physiological parameters. However, the presence of neoformations in the distal and medial region of the tail, similar to a nail, adherent, non-ulcerated and non-alopecic, and absence of pruritus or self-mutilation were identified. Vaccination and deworming were updated. Hematological examination, serum biochemicals (urea, creatinine, alanine aminotransferase, alkaline phosphatase, and albumin), and abdominal ultrasound showed no changes. According to the clinical and laboratory signs, it was decided to perform an excisional biopsy, using a caudectomy, for subsequent histopathological examination. The specimens were fixed in 10% formaldehyde and sent for histopathological examination. Histological examination was compatible with benign pilomatricoma. Postoperatively, cephalexin, dipyrone, tramadol hydrochloride, and meloxicam were prescribed, surgical wound cleaning, and the use of an Elizabethan collar until the suture was removed. After 10 days of the surgical procedure, the patient was asked to remove the sutures, and no changes were identified regarding the physiological parameters and blood count. Discussion: Pilomatricomas are commonly benign neoplasms arising from the germ cells of the follicular matrix. They present dermal or subdermal forms, with several cystic structures which are surrounded by keratinocytes, similar to the matrix cells of an anagen hair follicle, more keratinized and firmer areas, corroborating the results of the present report. In the ultrasound examination, the presence of intra-abdominal metastases was not identified, which is consistent with the literature, since in this type of neoplasm it is not common to identify foci of intra-abdominal and thoracic metastases. In the present report, immunohistochemistry was not used, even though it is used to differentiate follicular neoplasms from pilomatricomas. However, histopathological examination is considered the best method for the definitive diagnosis of pilomatricomas in dogs. It was concluded that the surgical treatment through excisional biopsy, with safety margins of 2 cm, was effective as a therapeutic method in the case of benign pilomatricoma, and the definitive diagnosis must be made through histopathological examination.

Canine facial eosinophilic furunculosis (FEF) is a hyperacute dermatopathy especially of the nasal bridge of dogs and is probably associated with type I hypersensitivity secondary to arthropod bites. The aim of this study is to report on a FEF case in a four-year-old female free-roaming mixed-breed dog showing papules on the nasal bridge that evolved to an ulcerated plaque. No other clinical, hematological, or biochemical alterations were detected. Cytology revealed eosinophilic and neutrophilic inflammation associated with bacterial infection. Punch biopsies were obtained for histopathological and microbiological analysis. Histopathology revealed marked, acute, multifocal to coalescent granulomatous eosinophilic furunculosis, and mild, acute, multifocal eosinophilic folliculitis. Microbiology revealed growth of coagulase-positive Staphylococcus sp. Clinical and histopathological findings were suggestive of facial eosinophilic folliculitis and furunculosis. Complete remission of the lesions was obtained after treatment. This condition is hyperacute, progressive, with a papular and erosive to ulcerative pattern, good prognosis, and its development is linked to arthropod bites. Furthermore, anti-inflammatory therapy is effective in treating the disease.(AU)

ABSTRACT Introduction: Folliculitis decalvans is a rare skin disease characterized by the presence of painful papules and pustules with an underlying neutrophilic infiltrate, usually on the scalp. Its treatment is lengthy and challenging, and recurrence is relatively common. Although its etiology is unknown, several theories explaining its development have been proposed, including colonization by Staphylococcus aureus. Case description: This is the case of a 26-year-old male healthcare worker who visited the outpatient service after experiencing a 4-year history of painful pustules on the scalp; initially these lesions were located in the occipital region, but then also started to appear in the temporal and parietal regions. After being treated for bacterial folliculitis and having several recurrences, a skin biopsy was performed, which allowed diagnosing him with folliculitis decalvans. Once the diagnosis was made, isotretinoin (20mg) treatment was implemented for a year and a half, achieving complete remission of the lesions. Conclusion: Although this case has some limitations, such as the lack of histopathology images and some control laboratory tests, it clearly shows the difficulties faced when treating this type of skin disorders and presents an overview of the use of isotretinoin, evidencing that although this drug is well tolerated, possible adverse reactions from drug interactions with trimethoprim/sulfamethoxazole may arise. In addition, this case is of great importance since the possible presence of a familial cluster of folliculitis decalvans could be confirmed, if further genetic testing is performed.


RESUMEN Introducción. La folliculitis decalvans es una enfermedad dermatológica rara caracterizada por la presencia de pápulas y pústulas dolorosas que están acompañadas de un infiltrado de neutrófilos subyacente. Esta condición suele aparecer en el cuero cabelludo, su recurrencia es relativamente común y su tratamiento, largo y difícil. Aunque su etiología es desconocida, se han propuesto muchas teorías que intentan explicar su aparición, siendo la colonización por Staphylococcus aureus una de ellas. Presentación del caso. Hombre de 26 años que se desempeñaba como trabajador de la salud y consultó por un cuadro clínico de 4 años de evolución caracterizado por la aparición de pústulas dolorosas en la región occipital, las cuales posteriormente se extendieron a la región temporal y parietal. Después de tratarlo como una foliculitis infecciosa y tras múltiples recurrencias, se realizó una biopsia de las lesiones que permitió diagnosticarlo con folliculitis decalvans. Se instauró un tratamiento consistente de 20mg de isotretinoina al día por un año y medio, con el cual se logró la resolución de la folicutis. Sin embargo, dos años después tuvo un relapso, pero, según el paciente, esto pudo ocurrir por el consumo de derivados lácteos, ya que, según indicó, cuando suspende el consumo de esta clase de productos no aparecen más lesiones luego de 2-3 semanas. Conclusión. Aunque este caso tiene algunas limitaciones como la ausencia de imágenes histopatológicas y algunos laboratorios de control, muestra las dificultades para tratar este tipo de condiciones dermatológicas y presenta un panorama del uso de la isotretinoina, ya que evidencia que este medicamento tiene una buena tolerancia, pero presenta interacciones medicamentosas adversas con la trimetoprima/sulfametoxazol. Además, este caso es de gran importancia, ya que, si se realizan más pruebas genéticas, podría confirmarse la posible presencia de un grupo familiar de foliculitis decalvante.

Alopecia is a condition associated with different etiologies, ranging from hormonal changes to chemotherapy, that affects over 80 million people in the USA. Nevertheless, there are currently few FDA-approved drugs for topical treatment, and existing formulations still present skin irritation issues, compromising treatment adherence. This work aimed to develop a safe formulation based on nanostructured lipid carriers (NLC) that entrap an association of minoxidil and latanoprost and target drug delivery to the hair follicles. To do so, thermal techniques combined with FTIR were used to assess the chemical compatibility of the proposed drug association. Then, NLC with 393.5 ± 36.0 nm (PdI < 0.4) and +22.5 ± 0.2 mV zeta potential were produced and shown to entrap 86.9% of minoxidil and 99.9% of latanoprost efficiently. In vitro, the free drug combination was indicated to exert positive effects over human primary epidermal keratinocytes, supporting cell proliferation, migration and inducing the mRNA expression of MKI67 proliferation marker and VEGF - a possible effector for minoxidil-mediated hair growth. Interestingly, such a favorable drug combination profile was optimized when delivered using our NLC. Furthermore, according to the HET-CAM and reconstructed human epidermis assays, the nanoformulation was well tolerated. Finally, drug penetration was evaluated in vitro using porcine skin. Such experiments indicated that the NLC could be deposited preferentially into the hair follicles, causing a considerable increase in the penetration of the two drugs in such structures, compared to the control (composed of the free compounds) and generating a target-effect of approximately 50% for both drugs. In summary, present results suggest that hair follicle-targeted delivery of the minoxidil and latanoprost combination is a promising alternative to treat alopecia.

O transplante capilar moderno está em constante evolução, e nossa intenção não é apenas demonstrar nossas técnicas e metodologias, mas, principalmente, descrever a dinâmica de todo o processo cirúrgico da técnica FUE (Follicular Unit Extraction), desde a discussão da estratégia cirúrgica com o paciente e a avaliação da área doadora até extração e implantação. É de extrema importância que o dermatologista compreenda a complexidade do procedimento e a necessidade de um treinamento adequado para que um bom resultado seja alcançado.


Modern hair transplant is constantly evolving. We intend to demonstrate our techniques and methodologies, and especially to describe the dynamics of the entire surgical process of the FUE (Follicular Unit Extraction) technique, from the discussion of the surgical strategy with the patient and evaluation of the donor area to extraction and implantation. The dermatologist must understand the complexity of the procedure and the need for adequate training to achieve a good surgical result

Pilomatricoma é um tumor cutâneo benigno derivado do folículo piloso que pode se desenvolver secundariamente a um trauma local. O tratamento cirúrgico é curativo e, portanto, é o de escolha. Relatamos o caso de um pilomatricoma de apresentação clínica atípica, como um nódulo ulcerado encimado por crosta hemática central, com telangiectasias periféricas desencadeado, muito provavelmente, pela aplicação da vacina de HPV. O exame ultrassonográfico sugeriu um possível granuloma inflamatório crônico, porém o exame histopatológico foi definitivo e confirmou o diagnóstico em questão


Pilomatricoma is a benign skin tumor derived from the hair follicle. It has a varied etiology, and the traumatic one is often described as a triggering factor. The surgical treatment is curative and, therefore, the choice. We report a case of an atypical clinical presentation pilomatricoma ­ an ulcerated nodule topped by a central hematic crust, with peripheral telangiectasias probably triggered by the HPV vaccination. The ultrasound examination suggested the possibility of a chronic inflammatory granuloma, although the histopathological examination was definitive and confirmed the diagnosis of pilomatricoma

The position and number of hair whorls have been associated with the behavior, temperament, and laterality of horses. The easy observation of whorls assists in the prediction of reactivity, and thus permits the development of better measures of handling, training, mounting, and riding horses. However, little is known about the genetics involved in the formation of hair whorls. Therefore, the aim of this study was to perform a genome-wide association analysis to identify chromosome regions and candidate genes associated with hair whorl traits. Data from 342 Quarter Horses genotyped for approximately 53,000 SNPs were used in an association study using a single-step procedure. The following traits were analyzed: vertical position of hair whorl on the head, number of whorls on the head, and number of whorls on the left and right sides of the neck. The traits had between one and three genomic windows associated. Each of them explained at least 4% of the additive variance. The windows accounted for 20-80% of additive variance for each trait analyzed. Many of the prospected genes are related to hair follicle growth. Some of these genes exert a pleiotropic effect on neurological and behavioral traits. This is the first indication of biological and physiological activity that might explain the association of hair whorls and temperament.

This study proposes developing a topical formulation based on poly-ε-caprolactone (PCL) or methacrylic acid/methyl methacrylate copolymer (EL100) nanoparticles to enable a safer and more effective therapy of alopecia and acne with spironolactone. The effect of the size of the nanoparticle on follicular-targeted drug delivery is also verified. Compatibility studies based on thermal analyses and complementary techniques showed a small interaction of the drug with excipients, which may not compromise the drug stability. PCL nanoparticles of 180.0 ±â€¯1.6 and 126.8 ±â€¯1.0 nm, and EL100 nanoparticles of 102.7 ±â€¯7.1 nm were then prepared. All nanoparticles entrapped more than 75 % of spironolactone, were physically stable, and stabilized the drug for at least 90 days. They were also non-irritant according to HET-CAM tests. Drug release from the nanoparticles was reduced in aqueous buffer media but fast when in contact with oil. Finally, in vitro skin penetration experiments revealed the largest nanoparticles (of 180 nm) targeted drug delivery to the hair follicles 5-fold (p < 0.05) more than the control solution, 2.1-fold (p < 0.05) more than nanoparticles produced with the same polymer (PCL) but with smaller size (123 nm), and 4.9-fold (p < 0.05) more than the 102-nm E100 nanoparticles. In conclusion, follicular targeting can be adjusted according to nanoparticle size, and this work succeeded in obtaining polymeric nanoparticles adequate to enable topical treatment of acne and alopecia with spironolactone.

Tumor triquilemal proliferante (TTP) é uma rara neoplasia que se desenvolve a partir de lesões císticas foliculares, cuja característica histológica é a presença de queratinização triquilemal. Manifestação mais comum é um nódulo solitário no couro cabeludo de mulheres idosas. Descreve-se caso de tumor triquilemal proliferante, que se apresenta como lesão tumoral cística em couro cabeludo de mulher idosa, com recidiva após remoção sem investigação diagnóstica.


Proliferating trichilemmal tumor (PTT) is a rare neoplasm that develops from cystic follicular lesions, whose histological characteristic is the presence of trichilemmal keratinization. The most common manifestation is a solitary lump on the scalp of older women. RESUMO

Alopecia is a clinical condition related to hair loss that can significantly affect both male and female adults' quality of life. Despite the high market demand, only few drugs are currently approved for alopecia treatment. Topical formulations still bring drawbacks, such as scalp irritation with frequent use, and low drug absorption to the site of action, which limits the efficacy. The most recent research points out that different formulation technology could circumvent the aforementioned flaws. Such technology includes incorporation of drugs in rigid or deformable nanoparticles, strategies involving physical, energetical and mechanical techniques, such as iontophoresis, sonophoresis, microneedling, and the use of solid effervescent granules to be hydrated at the moment of application in the scalp. In this paper, the progress of current research on topical formulations dedicated to the treatment of alopecia is reviewed and discussed.

Resumen El folículo piloso es una estructura compleja que presenta diversas características morfológicas macroscópicas, microscópicas e inmunológicas especiales que permiten el adecuado funcionamiento de la misma, en algunasenfermedades estos mecanismos de regulación inmunológica se ven alteradose incluso exacerbados por factores como el estrés emocional. El objetivo de esta revisión es conocer los mecanismos inmunobiológicos específicos del folículo pilosoanalizando el papel que juegan diversos factores como la pérdida delinmunoprivilegio y el estrés emocional en el desarrollo de la alopecia areata.


Abstract The hair follicle is a complex structure that presents diverse morphologicaland immunological characteristics that allow the proper functioning of the unit.In some diseases as alopecia areata these mechanisms of immune regulation are disrupted by external factorssuch as emotional stress preventing the growth of the hair shaft. The objective of this review is to recognize the specific immunobiological mechanisms of the hair follicle, analyzing the role played by the loss of immunoprivilege and emotional stress in the development of alopecia areata.

Latanoprost has recently been used to treat alopecia as it causes an increase in the capillary density of patients. This work presents for the first time the development of polymeric nanocapsules containing latanoprost for the topical treatment of alopecia. Poly-ε-caprolactone nanocapsules loading latanoprost were developed by nanoprecipitation of the polymer on the surface of drug oily nanodroplets. The method encapsulated 93.9 ± 0.4% of the drug into nanocapsules of 197.8 (± 1.2) nm (PdI = 0.15 ± 0.01). The nanosystem presented a zeta potential equal to - 30.1 ± 1.8 mV and was stable for at least 90 days when stored at 6°C. The colloidal aqueous dispersion was non-irritating, according to the in vitro HET-CAM test. The nanocapsules improved latanoprost accumulation into the hair follicles when topically applied on porcine skin, delivering 30% more drug to these skin structures relative to the control solution (P < 0.05). Also, with a simple manual massage, latanoprost accumulation was increased by twofold (P < 0.05). In conclusion, in addition to being a stable and safe formulation, nanocapsules enhanced latanoprost accumulation into the hair follicles, being a nanosystem with high potential for use as a topical formulation for the treatment of androgenic alopecia.

RESUMO

Folliculitis is a common skin disease, usually benign, which causes inflammation and eventual infections of hair follicles. They may have an infectious etiology, mainly due to the bacteria Staphylococcus aureus; it also occurs due to localized irritation, such as in areas of skin friction and for long periods of immersion in water, as in athletes and workers who are continuously exposed to the aquatic environment. Herein, we report on two fishermen, from fluvial and maritime environments, who presented with chronic aseptic folliculitis associated with daily immersion of their lower extremities while exercising the profession and that regressed when there was a decrease in their contact with water.

Abstract Background: The hair follicle is a unique structure, one of the most dynamic structures in mammalians, which can reproduce in every new cycle all the mechanism involved in its fetal development. Although a lot of research has been made about the human hair follicle much less has been discovered about the importance of the cytokeratins (CKs) in its development. Objective: Study the immunohistochemical pattern of epithelial CKs during human hair follicle development. Methods: We performed an immunohistochemical study using fresh post-mortem skin biopsies of human fetuses between 4 and 25 weeks of gestational age to study the expression of cytokeratins (CKs): CK1, CK10, CK13, CK14, CK16 and CK20 during human hair follicle fetal development. Study limitations: Restrospective study with a good number of makers but with a small population. Results/conclusion: We found that, the CKs were expressed in an intermediate time during follicular development. The epithelial CKs (CK1, CK14, CK10, CK13) and the epithelial CKs with a proliferative character such as CK16 were expressed first, as markers of cellular maturation and follicular keratinization. At a later phase, CK20 was expressed in more developed primitive hair follicles as previously discussed in literature.
