OBJECTIVES: Leprosy still represents a public health concern in Brazil. The country is the only one in America not to reach the global goal of leprosy disease control. Hence, this study aimed to assess the temporal, spatial and space-time patterns of leprosy cases in Brazil of the 20-year time series 2001-2020. METHODS: An ecological and population-based analysis was carried out, applying temporal and spatial techniques, and using the detection coefficient of sociodemographic and clinical-epidemiological variables of leprosy new cases in the 5570 municipalities of Brazil. Temporal trends were assessed using a segmented linear regression model. For spatial analysis, global and local Moran indexes were applied, and space-time scan statistics was used to identify risk clusters. RESULTS: The mean detection coefficient was 19.36/100,000 inhabitants, with a higher occurrence among men (21.29/100,000 inhabitants) and in the 60-69 age group (36.31/100,000). A decreasing temporal trend was observed in the country (annual percentage change: -5.20% per year). The North and Midwest regions were the most affected, exhibiting municipalities with a high/high standard, and with the highest annual percentage increase of multibacillary (MB) cases. Leprosy has a heterogeneous distribution throughout Brazil, but with high-risk spatiotemporal clusters, mainly located in the North and Midwest regions. CONCLUSION: Although Brazil has shown a decreasing temporal trend during the past 20 years, the country is still classified as highly endemic for leprosy, showing an increase in the proportion of new MB cases over the years.

Dengue fever is endemic in most part of Pakistan and continues to be a public health concern. Knowledge, attitude and practices can play an important role in management of the disease. Current study was aimed to determine the level of knowledge, attitude and practices regarding dengue fever among health practitioners, to study the level of knowledge and attitude with preventive practices for dengue fever. A cross sectional study was carried out in medical practitioners of the four districts of Malakand region during October to November 2019. A pre-structured questionnaire was used to collect data from medical practitioners. Data was analyzed using Graph Pad version 5. Significant value was considered when less than 0.05 (at 95% confidence of interval). The results revealed that most of participants have seen dengue vector (62%), the media being the most quoted source of information. Nearly 81.2% participants were aware from transmission of dengue fever is by mosquito bite. Practices based upon preventive measures were found to be predominantly focused towards prevention of mosquito bites rather than elimination of breeding places. Although the knowledge regarding DF and mosquito control measure was quite high among the medical practitioners but this knowledge was not put into practice. Further studies are required to aware the people about dengue and its vector in order to get prevention and control.

Mais estudos são necessários para conscientizar a população sobre a dengue e seu vetor, a fim de se obter prevenção e controle.

Resumo Objetivo Traçar o perfil sociodemográfico dos agentes de combate a endemias (ACEs) dos municípios que compõem a microrregional de saúde de Itaúna, Minas Gerais; e apreender a percepção dos profissionais da vigilância entomológica sobre a doença de Chagas e serviços de saúde. Metodologia Aplicou-se um questionário semiestruturado aos ACEs e foi realizado um grupo focal com os coordenadores de endemias e digitadores. Resultados Dentre os ACEs, a maioria era do sexo feminino, entre 21 e 40 anos, com ensino médio, possuindo contrato de trabalho temporário há menos de 5 anos. Foi possível apreender: as más condições de trabalho dos ACEs, destacando-se a alta rotatividade e defasagem salarial; ausência de ações de promoção à saúde relacionadas à doença de Chagas; desarticulação entre a Atenção Primária à Saúde (APS) e Vigilância à Saúde (VS); e críticas às ações de controle da doença pós-processo de descentralização da saúde. Conclusões Há urgência na concretização da legislação inerente à natureza trabalhista e às condições de trabalho dos ACEs, bem como ao planejamento conjunto das ações de APS e VS, garantindo a atenção integral por meio de ações de promoção à saúde voltadas à população, focando especialmente a doença de Chagas, consolidando o SUS.
Abstract Objective To trace the sociodemographic profile of endemic disease combat agents (ACEs) in the municipalities that make up the health microregion of Itaúna, Minas Gerais; and apprehend the perception of entomological surveillance professionals about Chagas disease and health services. Methodology A semi-structured questionnaire was applied to the ACEs and a focus group was held with coordinators of endemic diseases and typists. Results Among the ACEs, the majority were female, between 21 and 40 years old, with high school education, with a temporary work contract for less than 5 years. It was possible to apprehend: the poor working conditions of the ACEs, highlighting the high turnover and wage gap; absence of health promotion actions related to Chagas disease; disarticulation between Primary Health Care (PHC) and Health Surveillance (HS); and criticism of disease control actions after the health decentralization process. Conclusions There is an urgent need to implement the legislation inherent to the labor nature and working conditions of the ACEs, as well as the joint planning of PHC and SV actions, guaranteeing comprehensive care through health promotion actions aimed at the population, focusing especially on Chagas disease, consolidating the SUS.

RESUMEN Uno de los focos de leishmaniasis más importante de Colombia está en Los Montes de María, Departamento de Bolívar, específicamente en El Carmen de Bolívar. A la fecha están registradas 19 especies del género Lutzomyia, en este departamento, algunas con antecedentes como vectores de Leishmania spp. La presente nota informa la abundancia y la diversidad de la flebotominofauna en la zona rural de El Carmen de Bolívar. Mediante el uso de trampas CDC, se colectaron 1327 hembras del género Lutzomyia entre enero a noviembre de 2018: Lu. evansi (81 %), Lu. panamensis (15,5 %), Lu. gomezi (2,9 %), Lu. dubitans (0,15 %), Lu. aclydifera (0,07 %), Lu. c. cayennensis (0,07 %), Lu. rangeliana (0,07 %) y Lu. serrana (0,07 %). La mayor abundancia de flebotomíneos se presentó en noviembre y la menor en abril, coincidiendo con los meses de lluvia y sequía, respectivamente; mientras que en enero se presentó la mayor diversidad. En esta investigación sobresalen Lu. evansi, Lu. panamensis y Lu. gomezi por sus antecedentes como vectores de Leishmania en el Departamento de Bolívar y otras regiones del país. Se reporta por primera vez para el Caribe Colombiano la especie Lu. aclydifera, la cual deberá ser vigilada en cuanto a su capacidad vectorial para Leishmania.
ABSTRACT One of the most important leishmaniasis foci in Colombia is Los Montes de María, Department of Bolívar, specifically in El Carmen de Bolívar. To date, 19 species of Lutzomyia have been recognized in this department, some with antecedents as vectors of Leishmania spp. This note reports the abundance and diversity of the phlebotomine sandflies fauna in the rural area of El Carmen de Bolívar. Through the use of CDC traps, 1327 females of the genus Lutzomyia were collected between January to November 2018: Lu. evansi (81 %), Lu. panamensis (15.5 %), Lu. gomezi (2.9 %), Lu. dubitans (0.15 %), Lu. aclydifera (0.07 %), Lu. c. cayennensis (0.07 %), Lu. rangeliana (0.07 %) and Lu. serrana (0.07 %). The highest abundance of sandflies occurred in November and the lowest in April, which is consistent with the rainy and dry months, respectively; while in January there was the greatest diversity. In this research, stands out Lu. evansi, Lu. panamensis and Lu. gomezi for their antecedents as vectors of Leishmania in the Department of Bolívar and other regions of the country. We reported the species Lu. aclydifera for the first time for the Caribbean region, hence, this species should be monitored given its vector capacity for Leishmania.

RESUMO Objetivo: analisar o perfil sociodemográfico e clínico dos casos notificados de hanseníase em menores de 15 anos, no município de São Luís - Maranhão - Brasil. Método: estudo descritivo realizado com 826 casos notificados no Sistema de Informação de Agravos de Notificação, em São Luís - Maranhão, no período de 2010 a 2019. Os dados foram analisados por meio de estatística descritiva e foram expressos em frequências absolutas e relativas. Resultados: faixa etária entre 10 e 14 anos (60,29%), masculino (51,09%), cor parda (69,59%), ensino fundamental incompleto (39,58%), multibacilar (62,71%), forma clínica dimorfa (54,24%), até cinco lesões (74,45%), zero a dois nervos afetados (83,23%), sem baciloscopia (46,28%), casos novos (93,70%), demanda espontânea (46,38%) e grau zero de incapacidade no diagnóstico (78,98%). Conclusão: foram demostrados parâmetros hiperendêmicos com elevado risco para a doença. Destaca-se a importância de criar estratégias de controle e prevenção para reduzir a transmissão da hanseníase em crianças.
ABSTRACT Objective: to analyze the sociodemographic and clinical profile of reported cases of Hansen´s disease in children under 15 years old in the municipality of São Luís - Maranhão - Brazil. Method: descriptive study conducted with 826 cases reported in the Notifiable Diseases Information System, in São Luís - Maranhão, in the period from 2010 to 2019. Data were analyzed using descriptive statistics and were expressed in absolute and relative frequencies. Results: age group between 10 and 14 years (60.29%), male (51.09%), brown (69.59%), incomplete elementary education (39.58%), multibacillary (62.71%), dimorphic clinical form (54.24%), up to five lesions (74.45%), zero to two nerves affected (83.23%), no bacilloscopic (46.28%), new cases (93.70%), spontaneous demand (46.38%) and zero degree of disability at diagnosis (78.98%). Conclusion: hyper-endemic parameters with high risk for the disease were demonstrated. It is important to create control and prevention strategies to reduce Hansen´s disease transmission in children.

Los datos se analizaron mediante estadísticas descriptivas y se expresaron en frecuencias absolutas y relativas. Resultados: edad entre 10 y 14 años (60,29%), sexo masculino (51,09%), color marrón (69,59%), educación elemental incompleta (39,58%), multibacilar (62,71%), forma clínica dimórfica (54,24%), hasta cinco lesiones (74,45%), de cero a dos nervios afectados (83,23%), sin baciloscopia (46,28%), casos nuevos (93,70%), demanda espontánea (46,38%) y cero grado de discapacidad en el momento del diagnóstico (78,98%). Conclusión: se demostraron parámetros hiperendémicos con alto riesgo de enfermedad. Destaca la importancia de crear estrategias de control y prevención para reducir la transmisión de la lepra en los niños.

Background and objective: Nephrolithiasis (NL) is a public health problem in the population of Southeast Mexico because of its high prevalence and recurrence. The evolution of this pathology can result in renal damage and may even cause chronic kidney disease (CKD), leading to a reduced glomerular filtration rate (GFR), decreased kidney function, and kidney loss in advanced stages. However, few studies support this evidence in the population. The present study aimed to determine risk factors associated with CKD in adult patients in an endemic population of Mexico. Materials and methods: A case-control study was carried out with patients diagnosed with NL. Additionally, the clinical information of patients (age, weight, height, blood pressure, comorbidities, and time of progress of NL), characteristics of the stones (number, location, and Hounsfield units), and biochemical parameters were collected. Results: The recurrence of NL was associated with CKD (OR 1.91; 95% CI 1.37−2.27; p = 0.003). In addition, male sex (p = 0.016), surgical history (p = 0.011), bilateral kidney stones (p < 0.001), and urinary tract infections (p = 0.004) were other factors associated with CKD. Interestingly, thirty-two patients younger than 50 years old with >2 surgical events presented a significant decrease in GFR (p < 0.001). Conclusions: The recurrence of NL and the number of surgical events were risk factors associated with CKD in patients with NL treated in our population.

A paracoccidioidomicose não é uma doença de notificação obrigatória apesar de sua relevância na América Latina, por isso as estimativas de prevalência, incidência e morbidade dessa micose são baseadas em relatórios de levantamentos epidemiológicos, séries de casos, registros de hospitalização e dados de mortalidade. O objetivo desse trabalho foi descrever aspectos relacionados com o paciente, evolução da doença, confirmação diagnóstica e tratamento de casos confirmados de paracoccidioidomicose atendidos em um hospital de ensino do sul do Brasil. Foram coletadas informações de prontuários de 27 pacientes com diagnóstico de paracoccidioidomicose confirmado no período de 2010 até 2019. O perfil prevalente foi de um paciente do sexo masculino, com idade média de 53 anos, envolvido com atividades laborais diversas, de procedência urbana, imunocompetente e sem comorbidades, tabagista, mas não etilista. Para a maioria dos casos o acometimento inicial foi pulmonar, com importante envolvimento do sistema linfático no percurso da doença. A observação microscópica das estruturas fúngicas patognomônicas em amostras de biópsia, aspirado linfonodal e escarro foi o método mais utilizado para confirmar a suspeita clínica. O itraconazol foi a primeira opção de tratamento, seguido da anfotericina B. [au]
Paracoccidioidomycosis is not a notifiable disease despite its relevance in Latin America, so estimates of prevalence, incidence and morbidity of this mycosis are based on reports of epidemiological surveys, case series, hospitalization records and mortality data. The objective of this study was to describe aspects related to the patient, disease evolution, diagnostic confirmation and treatment of confirmed cases of paracoccidioidomycosis treated at a teaching hospital in southern Brazil. Information was collected from the medical records of 27 patients diagnosed with paracoccidioidomycosis confirmed in the period from 2010 to 2019. The prevalent profile was a male patient, with a mean age of 53 years, involved in various work activities, of urban origin, immunocompetent and without comorbidities, smoker, but non-alcoholic. For most cases, the initial involvement was pulmonary, with significant involvement of the lymphatic system in the course of the disease. Microscopic observation of pathognomonic fungal structures in biopsy samples, lymph node aspirate and sputum was the most used method to confirm the clinical suspicion. Itraconazole was the first treatment option, followed by amphotericin B. [au]

Con el objetivo de identificar los factores de riesgo para la infección por leishmaniasis cutánea en individuos del cantón Montecristi, se realizó un estudio durante enero y diciembre del 2019. La muestra estuvo conformada por 297 individuos, con edades comprendidas entre 15 y 50 años, ambos sexos. Se realizó una encuesta para conocer los factores de riesgo y nivel de conocimiento de la enfermedad. Los factores de riesgo identificados donde se mostró diferencias entre los grupos con y sin leishmaniasis fueron: la edad, el sexo, el nivel educativo, ocupación y conocimiento de la enfermedad. Se concluye que los factores de riesgo más relacionados con la enfermedad en la población de Montecristi resultaron ser sexo masculino sobre el femenino y las ocupaciones agricultores-obreros-oficios del hogar, por lo que se deben mantener promoción de salud con la finalidad de dar a conocer las medidas preventivas y así reducir la enfermedad
In order to identify risk factors for cutaneous leishmaniasis infection in individuals from the Montecristi canton, a study was carried out during January and December 2019. The sample consisted of 297 individuals, aged between 15 and 50 years, both sexes. A survey was conducted to find out the risk factors and level of knowledge of the disease. The risk factors identified where differences were found between the groups with and without leishmaniasis were: age, sex, educational level, occupation and knowledge of the disease. It is concluded that the risk factors most related to the disease in the population of Montecristi turned out to be male over female and farmers-workers-housework occupations, so health promotion should be maintained in order to give know the preventive measures and thus reduce the disease

En países con elevada endemia tuberculosa, las personas están muy cerca a esta enfermedad a lo largo de toda su vida. Sea porque algún familiar o amigo desarrolla la enfermedad o porque en el devenir de la vida experimenta la infección o la enfermedad. En el caso del médico o profesional de la salud, seguramente su cercanía será mayor, y preferentemente si se dedica a actividades de prevención y control en tuberculosis. En este artículo se exponen las experiencias cercanas con la tuberculosis desde nuestra infancia, como estudiante de medicina, médico, investigador, salubrista y docente universitario, donde el profesional pasa situaciones afectivas personales, temas de limitaciones de conocimiento, limitaciones en el diagnóstico, hasta ejecución de investigaciones sobre problemas locales y generando respuestas en la prevención y control. Seguramente es una experiencia común en profesionales de salud que en el día a día enfrentan esta endemia, de la cual cada vez conocemos más, pero se nos hace muy difícil su control.
In countries with high tuberculosis endemicity, people are very close to this disease throughout their lives. This may be because a relative or friend develops the disease, or because in the course of their lives they themselves acquire the infection or illness. In the event, that the person is a doctor or a health professional their contact with tuberculosis will be greater, and even more so if they are involved in activities relating to its prevention and control. This article discusses my experiences with tuberculosis since childhood, as a medical student, doctor, researcher, health professional and university professor, where the professional navigates situations of a personal or emotional nature, knowledge limitations, diagnostic constraints, and even undertaking research on local problems and generating responses in prevention and control. It is surely, a common experience among health professionals who each day face this endemic which we increasingly know more about, yet it remains very difficult to control.

Coccidioides is a fungus that is frequently found in dry places of sparse storms, with warm temperatures during most of the year. Two species are known to infect the human being: C. immitis and C. posadasii.It is endemic in northern Mexico, southern United States, as well as in some regions of Central America and South America. It is a highly contagious organism, but mostly it generates self-limited and asymptomatic diseases. Only 10 percent of cases with pulmonary symptoms are severe and may manifest as lobar pneumonia. Some cases of multiple foci and pleural effusion are diagnosed through biopsy with molecular methods. The treatment in many cases includes the use of azole antifungals for 3-6 months and follow-up with antibody titres
El Coccidioides es un hongo que se encuentra de manera frecuente en lugares secos, con temperaturas cálidas durante la mayoría del año. Son dos especies las que se conoce que infectan al ser humano la C. Immitis y C. Posadasii. Es endémico del norte de México, sur de Estados Unidos, así como algunas pequeñas regiones de centro y Sudamérica. Es un organismo altamente contagioso, pero en su mayoría genera enfermedades autolimitadas y de tipo asintomático, los pacientes con manifestaciones pulmonares solamenteen 10 por ciento son de presentación grave. Esta puede ser como una neumonía lobar, pero en algunos casos de focos múltiples y con derrame pleural, el diagnóstico es mediante biopsia en a fechas recientes con métodos moleculares. Los tratamientos en muchos casos requieren únicamente vigilancia. De requerir tratamiento se usa antimicótico 3-6 meses con azoles y seguimientos con títulos de anticuerpos

La fiebre Hemorrágica Argentina es una enfermedad viral aguda grave, de carácter sistémico, con duración de una a dos semanas, con cuadros clínicos de gravedad variable. Su agente transmisor es el virus Junín cuyo reservorio natural es el llamado ratón maicero y su zona endémica de distribución comprende sur de la provincia de Santa Fe, Córdoba, Noroeste de Buenos Aires y La Pampa, en Argentina. La primera medida preventiva para la enfermedad es la vacuna llamada Candid 1. Se realizó una encuesta poblacional para dimensionar en la zona de Venado Tuerto y localidades vecinas el alcance de la vacunación y estimar el conocimiento de la existencia de dicha vacuna, que no es de aplicación obligatoria.
Population survey on vaccination against Argentine Hemorrhagic fever in endemic area in the Province of Santa Fe. Argentine Hemorrhagic Fever is a serious systemic, acute viral disease, with a duration of one or two weeks and of variable gravity. Its transmitting agent is Junín Virus, whose natural reservoir is the corn mouse. Its endemic zone is the south of the province of Santa Fe, Cordoba, northwest of Buenos Aires and La Pampa; in Argentina. The first preventive measure for the disease is the vaccine called Candid 1. A population survey was carried out to measure the extent of vaccination and the knowledge of the vaccine, that is not of compulsory application.

Abstract: INTRODUCTION: Rhinosporidiosis is a chronic infection of the mucous membrane and is caused by Rhinosporidium seeberi, an aquatic mesomycetozoan. The mode of infection is probably transepithelial penetration. The large number of rivers and lakes and the strong presence of riparian populations in the State of Maranhão are strong predisposing factors for rhinosporidiosis. METHODS: A 5-year retrospective study was conducted in a tertiary medical center situated in Maranhão, Northeast Brazil. Twenty-five Maranhense patients diagnosed with rhinosporidiosis were analyzed. RESULTS: Most of the patients were children, adolescents and young adults (age range: 7-24 years, mean age: 14 years). The majority of the participants were male (84%), brown (76%), and students (92%). All lesions involved the entire nasal cavity and presented with a vascular polypoid mass. All patients were treated by surgical excision of the lesions. CONCLUSIONS: Rhinosporidiosis affects younger age groups, especially students from the countryside and the outskirts of urban areas. This study will aid and guide physicians in diagnosing and treating this infection in endemic areas.

Abstract This study analyzed the relationship between rainfall, temperature and occurrence of dengue cases. Ecological study performed with autochthonous dengue cases reported during 2003 to 2010 in São Luís, Maranhão. Data of rainfall and temperature were collected monthly. The monthly incidence of dengue cases was calculated by year/100,000 inhabitants. In order to identify the influence of climate variables and dengue cases different distributed lag models using negative binomial distribution were considered. Model selection was based on the lowest AIC (Akaike Information Criterion). Thirteen thousand, four hundred forty-four cases of dengue between 2003 and 2010 were reported, with peaks in 2005, 2007 and 2010. The correlation between rainfall and the occurrence of dengue cases showed increase in the first months after the rainy months. Occurrence of dengue cases was observed during all the period of study. Only rainfall-lag per three months showed a positive association with the number of cases dengue. Thus, this municipality is considered as an endemic and epidemic site. In addition, the relation between rainfall and dengue cases was significant with a lag of three months. These results should be useful to the future development of politics healthy for dengue prevention and control.
Resumo Este estudo analisou a relação entre a pluviosidade, temperatura e ocorrência de casos de dengue. Estudo ecológico, realizado no município de São Luís (MA), com os casos de dengue autóctone notificados no período de 2003 a 2010, e os dados mensais de pluviosidade e temperatura. A incidência mensal de casos de dengue foi calculada por ano/100.000hab. Para a identificação da influência das variáveis climáticas e casos de dengue foram ajustados diferentes modelos de defasagem distribuída, considerando a distribuição binomial negativa. A seleção dos modelos foi baseada no critério AIC (Akaike Information Criterion). Foram notificados 13.444 casos de dengue entre 2003 e 2010, com picos em 2005, 2007 e 2010. A correlação dos índices pluviométricos com a ocorrência da dengue mostrou aumento de casos nos primeiros meses subsequente aos chuvosos. Houve ocorrência de casos de dengue em todo o período estudado, caracterizando o município como endêmico-epidêmico. Além disso, a relação entre casos de dengue e quantidade de chuva foi significativa com defasagem de três meses. Estes resultados são úteis para o desenvolvimento futuro de políticas de saúde para controle e prevenção da dengue.

La leishmaniasis visceral en Venezuela es de carácter endémico y focal, con patrón zoonótico asociado a determinantes primarios, al ambiente físico y social, los cuales se relacionan con la pobreza y sus consecuencias inmediatas, como desnutrición, desinformación, habitación precaria. Por esta razón, se estimó el nivel de conocimiento de contenidos específicos sobre las medidas dirigidas a la población en riesgo, objetivando la participación activa de las personas en el control y en el desarrollo de hábitos, actitudes y prácticas saludables, para su protección individual y comunitaria. La metodología utilizada fue la combinación de técnicas cualitativas como técnicas cuantitativas modificando la propuesta etnográfica original d el Programa Nacional de Control, aplicando una encuesta estructurada y grupos focales. Se identificó la composición tecnológica del control y nivel de intervención. La población estudio para el primer año, fue de 36 familias, con 105 habitantes, con proporción de participación de 0,63 y para el segundo, de 42 con 132 personas y 0,83, no se evidenciaron diferencias estadísticas significativas por género y grupo de edad, para ambos años. En el primero, una vez puesta en práctica la técnica de los grupos focales se identificaron conocimientos no pertinentes de la población sobre la historia natural de la enfermedad, la transmisión y la prevención. Lo que contribuyo a mejorar significativamente el conocimiento de la comunidad, en los componentes atención, tratamiento y modo de transmisión, influyendo positivamente en el incremento de la proporción de adherencia para prácticas de prevención y control (0,52) y de control de Reservorio (0,26).
Visceral leishmaniasis is endemic in Venezuela and focal character pattern associated with zoonotic primary determinants, physical and social environment, which are associated with poverty and its immediate consequences, such as malnutrition, misinformation, poor room, etc. For this reason, the level of knowledge of specific content on measures aimed at the population at risk was estimated, aiming the active participation of people in the control and the development of habits, attitudes and healthy practices for individual protection and Community. The methodology used was a combination of methods and qualitative and quantitative techniques modifying the original ethnographic techniques proposed National Control Program, using a structured questionnaire and focus groups. Technological composition of control and intervention level was identified. The study population for the first year was 36 families, with 105 inhabitants, with shareholding ratio of 0.63 and the second from 42 to 132 people and 0.83, no statistically significant differences were found by gender and group age, for both years. In the first, once the technical implementation of the focus groups no relevant knowledge of the population on the natural history of disease transmission and prevention were identified. What contributed to significantly improve knowledge of the community in the care components, processing and transmission mode, positively influencing the increase in the proportion of adherence to prevention and control practices (0.52) and control Reservoir (0.26).
RESUMO
We developed a stochastic model for quantitative risk assessment for the Schistosoma mansoni (SM) parasite, which causes an endemic disease of public concern. The model provides answers in a useful format for public health decisions, uses data and expert opinion, and can be applied to any landscape where the snail Biomphalaria glabrata is the main intermediate host (South and Central America, the Caribbean, and Africa). It incorporates several realistic and case-specific features: stage-structured parasite populations, periodic praziquantel (PZQ) drug treatment for humans, density dependence, extreme events (prolonged rainfall), site-specific sanitation quality, environmental stochasticity, monthly rainfall variation, uncertainty in parameters, and spatial dynamics. We parameterize the model through a real-world application in the district of Porto de Galinhas (PG), one of the main touristic destinations in Brazil, where previous studies identified four parasite populations within the metapopulation. The results provide a good approximation of the dynamics of the system and are in agreement with our field observations, i.e., the lack of basic infrastructure (sanitation level and health programs) makes PG a suitable habitat for the persistence and growth of a parasite metapopulation. We quantify the risk of SM metapopulation explosion and quasi-extinction and the time to metapopulation explosion and quasi-extinction. We evaluate the sensitivity of the results under varying scenarios of future periodic PZQ treatment (based on the Brazilian Ministry of Health's plan) and sanitation quality. We conclude that the plan might be useful to slow SM metapopulation growth but not to control it. Additional investments in better sanitation are necessary.

FUNDAMENTOS: A hanseníase é doença infectocontagiosa crônica causada pelo Micobacterium leprae. Caracteriza-se por acometimento dermatoneurológico, variando em espectro entre dois pólos estáveis. Apesar de curável, ainda representa relevante problema de saúde pública, sendo o Brasil o segundo país mais endêmico do mundo. Sua maior morbidade associa-se aos estados reacionais e ao acometimento neural, podendo causar incapacidades físicas permanentes que comprometem significativamente a qualidade de vida dos pacientes, com auto-estigmatização e vergonha. OBJETIVO: Avaliar o grau de comprometimento da qualidade de vida nos pacientes com hanseníase. MÉTODOS: Estudo observacional com correlação entre variáveis clínicas de gravidade da doença e questionário de avaliação da qualidade de vida: Dermatology Life Quality Index. RESULTADOS: Dos 40 pacientes avaliados, a maioria apresentou comprometimento da qualidade de vida de grave a muito grave segundo score obtido no Dermatology Life Quality Index. As variáveis analisadas individualmente também mostraram correlação de gravidade com o número de pontos obtido. CONCLUSÃO: A hanseníase causa sofrimento que ultrapassa a dor e o mal-estar estritamente vinculados ao prejuízo físico, com grande impacto social e psicológico, justificando tanto avanços para abordagem multidisciplinar ao paciente quanto a necessidade de ações de saúde que visem ao controle da doença.
BACKGROUND: Hansens disease is a chronic infecto-contagious illness caused by Mycobacterium leprae. It is characterized by dermato-neurological damage, varying in spectrum between two steady poles. Albeit curable, it still represents an important public health problem, Brazil being the second most endemic country in the world. Its higher morbidity is associated with reactions and neural damage, which is able to cause permanent disabilities that significantly compromising the quality of life of the patients, with self-stigmatization and shame. OBJECTIVE: To evaluate the degree of compromise in the quality of life of patients with Hansens disease. METHODS: Observational study with correlation between clinical indicators of severity and a questionnaire on quality of life: the Dermatology Life Quality Index (DLQI). RESULTS: Of the 40 evaluated patients, the majority presented an impairment in the quality of life, which ranged from the serious to the very serious score in the Dermatology Life Quality Index. Individual analysis of disease indicators also showed a correlation between severity and the Dermatology Life Quality Index. CONCLUSION: Hansens disease causes suffering that exceeds the pain and the malaise strictly related to the physical damage, having a great social and psychological impact. This justifies both improvements to a multidisciplinary health care approach and the need of health actions aiming at disease control.
RESUMO
Epidemias devem ser consideradas não como um tipo especial de doença, mas sim como parte de um complexo processo de distribuição de patologias no tempo e no espaço. Diversas classificações de doenças existem, sendo oficial a CID atualmente em sua décima versão. As cartas de controle, empregadas pelo serviço de saúde, permitem uma aproximação à fase, endêmica ou epidêmica, em que se encontram especialmente doenças sujeitas a vigilância em todo o mundo, as quarentenáveis. O conceito de surto deve ser encarado como um indício de concentração de episódios que merece estudo mais profundo. As mais importantes doenças transmissíveis no mundo em desenvolvimento são chamadas negligenciadas e merecem atenção especial das agências de fomento de pesquisa por não serem tidas como prioritárias pela indústria farmacêutica. No Brasil, a Agenda de Prioridades de Pesquisa em Saúde inclui diversas doenças dessa natureza e tem sido contempladas em editais do CNPq e do Ministério da Saúde.
