Background: Autologous fat transfer (AFT) is gaining popularity in breast surgery, offering a natural-looking and minimally invasive approach for augmentation, reconstruction, and contouring. However, concerns about its impact on breast cancer necessitate an understanding of the interplay between transplanted adipose-derived stem cells (ADSCs) and the breast tissue microenvironment. Renowned for regeneration, ADSCs raise questions about their role in cancer promotion. This systematic review delves into the complex relationship between AFT and breast cancer, exploring how ADSCs may influence development, growth, and metastasis. Methods: A systematic search of electronic databases, including PubMed, Embase, and BVS was conducted to identify relevant studies. The search strategy employed a combination of keywords, including "breast augmentation", "fat grafting", "breast enhancement", "mammoplasty", "cancer", "neoplasm" and related terms. Two reviewers independently screened titles and abstracts. Full-text articles were then retrieved for further evaluation based on their potential contribution to the review objectives. Results: Two hundred and forty records were identified. Among these, 104 duplicates were removed, resulting in 136 reports available for title and abstract screening. Subsequently, 54 papers were deemed potentially eligible for inclusion, and all reports were retrieved. Conclusions: In vitro studies reveal ADSCs dual role in breast cancer, influencing proliferation, migration, and drug resistance through complex signaling pathways. Animal studies highlight distinct ADSC subpopulations impacting tumor growth via direct interactions and extracellular vesicle cargo. In vivo, ADSC-enriched fat grafting is generally safe, showing no increased cancer recurrence risk compared to other methods. Notably, cases of invasive breast carcinoma warrant special attention. ADSC-enriched fat grafts exhibit potential benefits in graft retention and survival rates. Despite promising evidence, further studies are needed to comprehensively understand the intricate relationship between ADSCs and breast cancer for optimized clinical applications and potential therapeutic innovations.

INTRODUCTION: The Brazilian Butt Lift (BBL) is a safe technique that entails body reshaping through buttock augmentation using autologous fat transfer. METHOD: Between 2018 and 2022, 3000 patients underwent surgery. Each procedure commenced with the patient in supine position, starting with abdominal liposuction, followed by fat transfer to hips. Subsequently, the patient was repositioned to a prone stance. Liposuction of the back and waist ensued, concluding the surgery with fat transfer to the buttocks. Stringent precautions were implemented to ensure the safety of the patient throughout the procedure. RESULTS: Eighty-six percent of patients express satisfaction with their outcomes, while seventy percent of the transferred fat calls survive in the buttocks. One serious complication and some minor complications have been encountered and effectively managed through conservative measures. CONCLUSION: BBL proves to be a safe and satisfactory surgery when performed by experienced surgeons who adhere to fundamental principles. Resumen Objetivo: Describir la evolución que presentaron los pacientes con la aplicación de injerto graso autólogo con plasma rico en plaquetas (IGAPRP) en defectos de espesor completo (DET). Método: Durante el periodo del 15 de abril al 30 de septiembre de 2021 se analizaron los DET de pacientes que recibieron IGAPRP. Resultados: Se realizaron de 3 a 4 procedimientos de aplicación de IGAPRP para su integración con un tiempo promedio de estancia hospitalaria de 19.6±0.5 días. El tamaño de la lesión fue de 17.5 a 280 cm2 (95.1±81.6 cm2, profundidad de 1.1±1.1 cm). Volumen de grasa injertada en los DET fue de 14.7±7.9 ml. No se identificaron casos con infección. La comparación de la dimensión de los defectos a través de los procedimientos reveló una diferencia de 95.13 cm2 con hallazgos estadísticamente significativos (p<0.001). Limitaciones del estudio: La población de estudio limitada a solo 6 pacientes no nos permite sacar conclusiones significativas con respecto al efecto del tratamiento. A pesar hubo diferencias significativas en el volumen y área de los DET. Se requiere un estudio multicéntrico para evaluar completamente la eficacia de estos tratamientos en pacientes con DET. Valor: El uso de IGAPRP podría disminuir secuelas y complicaciones del uso de colgajos para la reconstrucción de DET. Conclusiones: La aplicación de IGAPRP demostró ser una opción eficaz y segura para la granulación de DET en nuestros pacientes para su posterior cobertura con un injerto cutáneo de espesor parcial.


Abstract: Objective: To describe the evolution of patients with the application of autologous fat grafting with platelet-rich plasma (IGAPRP) in full-thickness defects (TED). Method: During the period from April 15, 2021 to September 30, 2021, the TEDs of patients who received IGAPRP were analyzed. Results: 3 to 4 IGAPRP application procedures were performed for its integration with an average hospital stay of 19.6±0.5 days. Lesion size ranged from 17.5 to 280 cm2 (95.1±81.6 cm2, Depth 1.1±1.1 cm). Volume of grafted fat in the TEDs was 14.7±7.9 ml. No cases of infection were identified. The comparison of the dimensions of the defects through the procedures revealed a difference of 95.13 cm2 with statistically significant findings (p<0.001). Study limitations: The study population limited to only 6 patients does not allow us to draw significant conclusions regarding the effect of treatment. Despite there were significant differences in the volume and area of ??the TEDs. A multicenter study is required to fully evaluate the efficacy of these treatments in patients with TED. Value: The use of IGAPRP could reduce sequelae and complications of the use of flaps for TED reconstruction. Conclusions: The application of IGAPRP proved to be an effective and safe option for TED granulation in our patients for subsequent coverage with a split-thickness skin graft.

Introducción: El aumento de glúteos tiene como objetivo lograr una apariencia y contorno más joven; así como crear la proporción ideal entre cintura y cadera. Esto se puede lograr mediante la lipoinyección, aunque en este caso existen controversias en cuanto a la viabilidad del tejido injertado y la supervivencia de la grasa. Objetivo: Determinar el porcentaje de supervivencia del injerto de grasa autóloga y su relación con el volumen inyectado en pacientes sometidas a lipotransferencia glútea. Métodos: Se realizó un estudio observacional, descriptivo, de corte longitudinal y prospectivo con una muestra de 44 pacientes sometidas a lipotransferencia glútea en el Servicio de Cirugía Plástica del Hospital Docente Clínico Quirúrgico Dr. Miguel Enríquez en el período comprendido entre marzo de 2018 y junio de 2021. Resultados: El estudio evidencia que, mediante ultrasonido y fórmulas, el volumen promedio y la altura de la de grasa en los glúteos se duplica en el posoperatorio mediato y disminuye en el posoperatorio tardío sin llegar a los valores del preoperatorio. Además, se estableció que cerca de la media del volumen de grasa autóloga injertada en los glúteos sobrevive de manera definitiva en el posoperatorio tardío. Además, quedó establecido que la relación entre el volumen infiltrado y la supervivencia del injerto de grasa autóloga es inversamente proporcional: mientras mayor es el volumen, menor es su supervivencia. Conclusiones: Solo la mitad del volumen de grasa autóloga injertada en los glúteos sobrevive de manera definitiva, y la supervivencia de la grasa es inversamente proporcional al volumen infiltrado(AU)


Introduction: Buttock augmentation aims to achieve a more youthful appearance and contour; as well as creating the ideal proportion between waist and hips. This can be achieved by lipoinjection, although in this case there are controversies regarding the viability of the injected tissue and the survival of the fat. Objective: To determine the percentage of autologous fat graft survival and its relationship with the injected volume in patients undergoing gluteal fat transfer. Methods: An observational, descriptive, longitudinal and prospective study was carried out, with a sample of 44 patients undergoing gluteal lipotransfer, in the plastic surgery service of the Dr. Miguel Enriquez Clinical Surgical Teaching Hospital, in the intermediate period between March 2018 to June 2021. Results: The study shows that through ultrasound and formulas, the average volume and height of fat in the buttocks doubles in the immediate postoperative period, decreasing in the late postoperative period, without reaching preoperative values. Furthermore, it's established that about the mean volume of autologous fat grafted to the buttocks survives definitively in the late postoperative period. In addition, it was established that the relationship between the infiltrated volume and the survival of the autologous fat graft is inversely proportional, the greater the volume, the less its survival. Conclusions: Only half of the volume of autologous fat injected into the buttocks survives definitively, and the survival of the fat is inversely proportional to the volume injected(AU)

Background: Cutis verticis gyrata (CVG) is a condition of excessive skin growth and excessive laxity of the scalp, leading to deep furrows and folds that resemble the gyri and outer surface of the brain. Approaches for the treatment of CVG range from conservative to surgical, the last one being the predominant way of treating the condition, however, the surgery proposed in the recent literature may not be suitable for patients who desire a less invasive approach. Aim: To report the first case of autologous fat injection as a novel treatment option for primary essential CVG. Methods: A 51 -year-old Colombian man, with no medical records was treated with a scalp injection of autologous fat, with previous failed treatment with pressure therapy apply directly on skin is described. Results: A successful cosmetic improvement and a less invasive approach was reached after two sessions of autologous fat grafting. Conclusion: This method allowed excellent cosmetic outcomes while preserving the option of subsequent surgical repair in refractory cases.

A 52-year-old female patient developed facial fat necrosis presenting with cutaneous induration three weeks after minimal access cranial suspension (MACS) lift with autologous fat grafting from the abdomen. Given that the patient received the Moderna SARS-CoV-2 vaccine one week after surgery, we hypothesize that the former predisposed her to tissue ischemia leading to fat necrosis. Histological findings after biopsy were consistent with fat necrosis, which included marked dermal fibrosis with areas of focal fat necrosis, lipophages, multinucleated giant cells, and siderophages. It is our hope that documenting this rare development in literature may serve as encouragement for adverse effect reporting after the SARS-CoV-2 vaccine administration and may boost inspection and monitoring of other health consequences by regulating agencies.

OBJECTIVE: To present a systematic review of the literature and a meta-analysis evaluating the oncological safety of autologous fat grafting (AFG). SUMMARY BACKGROUND DATA: AFG for breast reconstruction presents difficulties during follow-up radiological exams, and the oncological potential of grafted fat is uncertain. Previous studies confirmed that the fatty tissue could be transferred under a good condition suitable would not interfere with mammographic follow-up, although the issue of oncological safety remains. METHODS: We reviewed the literature published until 01/18/2021. The outcomes were overall survival (OS), disease-free survival (DFS), and local recurrence (LR). We included studies that evaluated women with breast cancer who undergone surgery followed by reconstruction with AFG. We synthesized data using the inverse variance method on the log-HR (log of the hazard ratio) scale for time-to-event outcomes using RevMan. We assessed heterogeneity using the Chi2 and I2 statistics. RESULTS: Fifteen studies evaluating 8541 participants were included. The hazard ratios (HR) could be extracted from four studies, and there was no difference in OS between the AFG group and control (HR 0.9, 95% CI 0.53 to 1.54, p = 0.71, I2 = 58%, moderate certainty evidence), and publication bias was not detected. The HR for DFS could be extracted from six studies, and there was no difference between the AFG group and control (HR 1.01, 95% CI 0.73 to 1.38, p = 0.96, I2 = 0%, moderate certainty evidence). The HR for LR could be extracted from ten studies, and there was no difference between the AFG group and control (HR 0.86, 95% CI 0.66 to 1.12, p = 0.43, I2 = 1%, moderate certainty evidence). CONCLUSION: According to the current evidence, AFG is a safe technique of breast reconstruction for patients that have undergone BC surgery and did not affect OS, DFS, or LR.

BACKGROUND: The BAAPS advice against Brazilian butt lift (BBL) surgery in the UK was set in October 2018. A Delphi study conducted by BAAPS highlighted the importance of defining current practice and perceptions amongst UK surgeons, as this is currently unknown. OBJECTIVES: To evaluate BAAPS members' current practice and perceptions around BBL surgery to ensure patient safety and propose better recommendations. METHODS: A BAAPS-commissioned survey was emailed to all BAAPS members through an on-line link. The survey collected quantitative and qualitative information in several domains. RESULTS: This survey received a 44% response rate. Of 102 respondents, 32 surgeons undertook BBL surgery before the BAAPS advice to halt it. There was a wide variation in actual fat volumes injected, and in perceptions of what constituted a small or large volume. Virtually all respondents (96.9%) performed only subcutaneous fat injections. There were differences in fat harvest techniques. The majority (66.7%) felt that BAAPS should maintain its recommendation against undertaking BBL surgery until further data became available. Nearly a quarter of 102 respondents (20.6%) had been treated for BBL complications, the majority as a result of surgical tourism. CONCLUSIONS: The survey provides member-reported perception and experience with regard to BBL surgery in the UK. The demand for BBL surgery and its provision should be reassessed in the UK. This information will be analysed with further national and international data to better define and shape the scope of the safety of BBL surgery in the UK. BAAPS will use such data towards the production of future guidance and support for surgeons and patients.

Abstract Purpose: Faecal incontinence (FI) is a frequent condition that can occur due to different causes; with negative impact on self-esteem and quality of life, secondary morbidity, disability and significant costs. For its treatment there is a wide range of options, being medical treatment, hygienic dietary modifications and biofeedback, the first line of treatment; reserving surgery for patients who do not respond or with severe FI; this with variable success rates and high cost. This study has the primary aim to assess the efficacy and describe the Technique of Application of Autologous Fat with Platelet Rich Plasma (AFPRiP) in patients with faecal incontinence as well as secondary endpoints of quality of life, manometric and ultrasound evaluation, safety of implantation, and complications. Methods: A single-centre prospective, experimental study, was conducted from January 2017 to February 2018 in Domingo Luciani Hospital. Wexner and FIQL scores were filled preoperative and compared at follow-up at 3, 6 and 12 months as well as anorectal manometry and endoanal ultrasound were performed before and 6 months after surgery. Results: Twelve patients were operated, mean time 43 min, no major complications. Wexner finding continence improvement from 10.4 pre to 4 in the 3rd month (p = 0.066) 4.74 at the 6th month (p = 0.001) and 5 at one year (p = 0.001); that is, improvement of >50% in 83.4%. FIQL 50.9 prior to 98.6 at 3rd month (p = 0.001) 95.5 to 6th month (p = 0.001) and 91.3 a year (p = 0.066). Conclusions: We conclude that AFPRiP is innovative, safe and with adequate results.


Resumo Objetivo: A incontinência fecal é uma condição frequente que pode ocorrer devido a diferentes causas, com impacto negativo na autoestima e qualidade de vida, morbidade secundária, incapacidade e custos significativos. Existem várias opções para o manejo da incontinência fecal; o tratamento médico, as modificações higiênicas da dieta e o biofeedback, são os de primeira linha. A cirurgia é recomendada apenas para pacientes que não respondem ao tratamento de primeira linha ou aqueles com incontinência fecal grave; as taxas de sucesso são variáveis e o custo do tratamento cirúrgico é elevado. Este estudo teve como objetivo principal avaliar a eficácia e descrever a técnica de aplicação de gordura autóloga com plasma rico em plaquetas (AFPRiP) em pacientes com incontinência fecal; o estudo também avaliou parâmetros secundários de qualidade de vida, manométricos e ultrassonográficos, bem como a segurança da implantação e suas complicações. Métodos: Um estudo experimental prospectivo, de centro único, foi realizado de janeiro de 2017 a fevereiro de 2018 no Hospital Domingo Luciani. A escala de Wexner e o FIQL foram preenchidos no pré-operatório e comparados no seguimento de três, seis e 12 meses; manometria anorretal e ultrassonografia endoanal foram realizadas antes e seis meses após a cirurgia. Resultados: Doze pacientes foram operados; o tempo médio da cirurgia foi de 43 minutos, sem maiores complicações. Na escala de Wexner, observou-se melhora na continência: de 10,4 pré-operatório a 4 no terceiro mês (p = 0,066), 4,74 no sexto mês (p = 0,001) e 5 em um ano (p = 0,001), uma melhoria de 83,4%. Já o FIQL evoluiu de 50,9 no período pré-operatório para 98,6 no terceiro mês (p = 0,001), 95,5 no sexto mês (p = 0,001) e 91,3 em um ano (p = 0,066). Conclusões: A AFPRiP é uma técnica inovadora, segura e que apresenta resultados adequados.

IMPORTANCE: Autologous fat grafting (AFG), or lipofilling, has been used for immediate reconstruction at the time of breast-conserving surgery in order to achieve a satisfactory cosmetic outcome in patients with breast cancer and an unfavorable tumor-to-breast volume ratio or unfavorable tumor location. However, the oncologic safety of this technique is still unclear. OBJECTIVE: To determine whether AFG performed simultaneously with breast-conserving surgery is associated with differences in local relapse rates and disease-free survival. DESIGN: Matched retrospective cohort study. SETTING: Tertiary referral center. PARTICIPANTS: Patients undergoing breast-conserving surgery with or without AFG between 2004 and 2016 were retrospectively enrolled and matched for age, staging, grade, tumor histology, and tumor immunohistochemical profile. MAIN OUTCOME(S) AND MEASURE(S): The cumulative incidence of locoregional recurrence (LRR) and disease-free survival were the primary end points, while distant recurrence and overall survival were the secondary end points. RESULTS: A total of 320 patients were followed. Cases were matched with controls at a 1:4 ratio. There was no difference in LRR or distant recurrence of breast cancer between the two groups. The annual LRR rate was 0.86% in patients who received immediate AFG vs. 0.7% in patients undergoing breast-conserving surgery alone (p ≥ 0.05). Number of lymph nodes was the sole independent risk factor for local recurrence (p = 0.045). No significant differences in disease-free survival rates were found between the groups. CONCLUSIONS AND RELEVANCE: At a mean follow-up of 5 years, no significant differences in locoregional recurrence rates were found between patients who received immediate AFG and those who underwent breast-conserving surgery alone. These findings corroborate previous research demonstrating the oncological safety of immediate AFG reconstruction, further suggesting that this technique as a safe, effective way to achieve optimal cosmetic outcomes in primary breast cancer surgery without jeopardizing oncologic outcomes.

Introducción. La Lipoinyección de grasa autóloga con fines estéticos es una de las técnicas más jóvenes que permiten resolver problemas relacionados con valores concernientes a las elecciones personales que apuntan hacia la belleza, es un procedimiento quirúrgico sencillo y con resultados rápidos. Es una alternativa segura, real, sencilla y accesible que abarata costos, sobre todo con el uso de equipos y tecnología. Objetivos. Caracterizar los resultados estéticos de la Lipoinyección Labial con Grasa Autóloga en pacientes que asistieron a la consulta externa de Cirugía Plástica del Hospital Universitario "General Calixto García". Material y Método. Se realizó un estudio prospectivo, descriptivo, de corte transversal, en el período comprendido entre octubre de 2018 a septiembre del 2019. Se seleccionaron aquellos que cumplieron los criterios de inclusión y aceptaran a participar en el estudio voluntariamente. La muestra quedó constituida por 23 pacientes. La información obtenida se procesó mediante bases de datos confeccionadas en Excel, lo que permitió la realización de la estadística descriptiva simple para las variables individuales como media, desviación estándar y razón. La disposición a colaborar en el estudio se recogió en el modelo de Consentimiento Informado elaborado para esta investigación. Resultados. Predominio del sexo femenino en el 60.87 % de los operados. El grupo de edad entre 51 a 61 años fue el de mayor número de pacientes. Las complicaciones inmediatas más frecuentes en los labios fueron: la inflamación y los hematomas; las tardías fueron nodulaciones y endurecimiento. El 82.60 % de los pacientes se mostró satisfecho con los resultados. Conclusiones. La Lipoinyección labial con Grasa Autóloga se caracterizó por ser un método capaz de satisfacer el resultado estético. Las variables edad y sexo evidencian que las mujeres se realizaron más Cirugía Estética Labial que los hombres. Las variables clínicas labios finos y volumen evidencian que el aumento de la edad es una condición importante que se asocia a la demanda de esta técnica quirúrgica y que el ser hombre o mujer no establece una diferencia para la solitud de este tipo de técnica quirúrgica. Se presentaron complicaciones inmediatas y tardías. Los niveles de satisfacción expresados por los pacientes fueron altos.


Introduction. Autologous fat lipoinjection for aesthetic purposes is one of the youngest techniques that allow solving problems related to values concerning personal choices that point towards beauty, it is a simple surgical procedure with quick results. It is a safe, real, simple and accessible alternative that lowers costs, especially with the use of equipment and technology,Objectives. To characterize the aesthetic results of Lip Lipoinjectionwith Autologous Fat in patients who attended the Plastic Surgery outpatient clinic at the "General Calixto García" University Hospital. Material and Method. A prospective, descriptive, cross-sectional study was conducted in the period from October 2018 to September 2019. Those who met the inclusion criteria and voluntarily agreed to participate in the study were selected. The sample consisted of 23 patients. The information obtained was processed through databases prepared in Excel, which allowed the performance of simple descriptive statistics for individual variables such as mean, standard deviation and ratio. The willingness to collaborate in the study was included in the Informed Consent model prepared for this research. Results. female predominance in 60.87% of those operated on. The age group between 51 and 61 years was the one with the largest number of patients. The most frequent immediate complications on the lips were: inflammation and bruising; the later ones were nodulations and hardening. 82.60% of the patients were satisfied with the results.Conclusions. Lip Lipoinjection with Autologous Fat was characterized as a method capable of satisfying the aesthetic result. The age and sex variables show that women underwent more cosmetic lip surgery than men. The clinical variables thin lips and volume show that increasing age is ́t a condition that is associated with the demand for this technique and that the Being male or female does not make a difference to the solitude of this type of surgical technique. There were immediate and late complications. The satisfaction levels expressed by the patients were high.

PURPOSE: Implanted rectal spacers (IRS) have been developed to increase the distance between the prostate and the rectum, thus optimizing dose escalation. Cost is a disadvantage and there are still uncertainties as to their durability. We have developed an autologous fat transfer (AFT) technique to use as an IRS. We aim to present the feasibility and durability at 6 months of AFT placed immediately after the implant of the seeds in low-dose-rate brachytherapy (BT). METHODS AND MATERIALS: Thirty-five patients underwent AFT (12 were treated with primary BT, 7 with a combined primary treatment of external beam radiotherapy + BT, 16 with salvage BT). The isodose used for primary BT was 14400 cGy, 11,000 cGy after 4600 cGy of external beam radiotherapy in the combined group, and 14400 cGy for the salvage group. Patients underwent a CT scan at 1, 3, and 6 months to measure the distance between the rectum and the prostate. RESULTS: An average of 32.7 cc (20-40) of fat was transferred successfully in 100% of cases. The mean distance to the rectum at the level of the base, middle, and apex at 1 and 6 months were 11.2, 9.7, and 7.6 mm; 8.3, 8.1, and 5.9 mm, respectively. No rectal toxicity or major complications were reported. CONCLUSIONS: The use of fat as an IRS seems to be a valid alternative to reduce rectal toxicity after BT, achieving equivalent distances to synthetic IRS. It is feasible, safe, and the loss of distance at 6 months is small. Cost is lower than other alternatives.

INTRODUCTION: Autologous fat grafting (AFG; lipofilling, lipografting) has been used in delayed breast reconstruction. Recently, it has also been investigated as an alternative for immediate reconstruction in patients submitted to breast-conserving surgery (BCS). Although good aesthetic results have been reported, the oncologic safety of the procedure remains under investigation. This article aims to assess oncologic outcomes of patients submitted to BCS with immediate AFG reconstruction. METHODS: This study consisted of 65 patients undergoing BCS with AFG between January 2010 and January 2017. They were closely followed after surgery for a median period of 40.8 months. Locoregional and systemic recurrences were the primary endpoints of this study. RESULTS: Ten patients developed cancer recurrence (15.4%). The median time for recurrence was 58.9 months. Only two patients presented locoregional recurrence (LRR) (3.07%). Five patients had systemic recurrence (7.69%), and three had both systemic and LRR (4.61%). Median disease-free survival (DFS) was 42.2 months, and overall survival (OV) was 44.3 months. Recurrences were significantly associated with the number of metastatic axillary lymph nodes detected. CONCLUSION: Oncologic outcomes of immediate AFG are similar to the results previously reported in BCS without AFG. Locoregional and systemic recurrences are associated with the presence of axillary metastases. LEVEL OF EVIDENCE IV: This journal requires that authors assign a level of evidence to each article. PURPOSE: To evaluate the aesthetic and functional outcomes of autologous fat transfer using the SEFFI (superficial enhanced fluid fat injection) technique for reconstruction of the periocular area. METHODS: Autologous fat injections prepared with the 0.5 mL and 0.8 mL SEFFI technique were used in four patients for periocular rehabilitation. RESULTS: Case 1 (C1): A patient with left-sided progressive facial hemiatrophy underwent ipsilateral volumizing with 0.8 SEFFI in the superior, temporal, and inferior periorbital areas, and 0.5 SEFFI in both eyelids. C2: A 21-year-old female with a post trauma frontal scar, left ptosis, and lower eyelid retraction was treated with 0.5 SEFFI applied in the scar area associated with an upper eyelid conjunctivomullerectomy and resection of the lower eyelid retractors. C3: A patient with previous left-eye evisceration and orbital floor and medial wall fractures underwent socket reconstruction with buccal mucosal graft in the lower fornix and 0.5 SEFFI injections in both superior and inferior eyelids. SEFFI was also applied in the intraorbital space for correction of the enophthalmos. C4: A patient with lower lid retraction post blepharoplasty was treated with 0.8 SEFFI injections in lower eyelids and malar areas, complemented with a bilateral lateral cantopexy. CONCLUSIONS: Autologous fat transfer with SEFFI technique is an effective and safe procedure in cases of periocular rehabilitation.

This study explores the technical viability of autologous fat grafting for immediate partial breast reconstruction following the primary conservative surgery for breast cancer; restoring volume, shape, and symmetry to the treated breast. We analyze the impact in relation to mammographic follow-up and the subjective degree of satisfaction with esthetic results and although we didn't have any local failure during the observation period; we understand that is too early for any kind of conclusion. From June of 2010 to July of 2011, 20 patients were selected for breast-conserving surgery (BCS) with partial breast reconstruction with autologous fat grafting performed during the same operation by the same medical team. The fat graft was injected in the subcutaneous and intraglandular breast tissue around the resected area. We performed an overcorrection, predicting a resorbing of about 30-50% of the volume grafted. All patients received the literature-based recommendations for breast cancer treatment with complementary radiotherapy. The follow-up period after the completion of radiotherapy ranged from 13 to 29 months. The average volume grafted was 121 cc and it was 2.1 times larger than the resected volume. The esthetic scores were considered very good or good in the majority of cases. Mammograms were taken every 6 months after radiotherapy and only one patient had a mammogram classified as BI-RADS 3, suggesting fat necrosis. The remaining patients were classified as BI-RADS 2. Immediate autologous fat grafting is a promising technique for partial breast reconstruction after BCS. It is associated with high patient and physician satisfaction. It is an alternative for reconstruction of small- and medium-sized breasts. This technique has the advantage that enables the reconstruction of defects in areas difficult to repair, particularly in the upper inner quadrants. It also may reduce the need for major glandular or myocutaneous flaps mobilization.

BACKGROUND: Capsular contracture (CC) is a common complication after breast augmentation. Autologous fat grafting may be effective for restoring tissue vascularization and function. OBJECTIVE: The authors evaluated the efficacy of autologous fat grafting in a porcine model as a treatment for CC after breast augmentation. METHODS: This prospective study was performed in 20 female 30-day-old pigs. Each animal was implanted with three 30-cc textured silicone implants (stage 1 of the experiment). Group A served as the untreated control group. To induce CC, 2 mL of autologous fibrin glue was applied to the pericapsular space in group B and C animals at implantation. Three months after implantation (stage 2), the CCs of all groups were assessed by Baker classification and applanation tonometry (AT). Liposuction was performed in group B to harvest fat for these animals. Three months after group B underwent fat grafting, all 3 groups were reevaluated. Reassessments included Baker classification, AT, histologic analysis, and tensiometry (stage 3). RESULTS: The deposition of mature and immature collagen was similar for the 3 groups. The amount of fat remaining around the implanted capsules did not differ significantly between the groups. At stage 3, group B exhibited significantly larger tonometry areas than did group C. The CCs in groups B and C were significantly thicker than those of group A, but the difference between groups B and C was not significant. Capsule rupture forces did not differ significantly between groups A and B but were significantly higher in group C compared with the other groups. CONCLUSIONS: Results in this animal model indicate that pericapsular lipoinjection may be a promising treatment for CC in humans.

Autologous fat graft to the breast is a useful tool to correct defects after breast conservative treatment (BCT). Although this procedure gains popularity, little is known about the interaction between the fat graft and the prior oncological environment. Evidences of safety of this procedure in healthy breast and after post-mastectomy reconstruction exist. However, there is paucity of data among patients who underwent BCT which are hypothetically under a higher risk of local recurrence (LR). Fifty-nine patients, with prior BCT, underwent 75 autologous fat graft procedures using the Coleman's technique, between October 2005 and July 2008. Follow-up was made by clinical and radiologic examination at least once, after 6 months of the procedure. Mean age was 50 ± 8.5 years, and mean follow-up was 34.4 ± 15.3 months. Mean time from oncological surgery to the first fat grafting procedure was 76.6 ± 30.9 months. Most of patients were at initial stage 0 (11.8%), I (33.8%), or IIA (23.7%). Immediate complication was observed in three cases (4%). Only three cases of true LR (4%) associated with the procedure were observed during the follow-up. Abnormal breast images were present in 20% of the postoperative mammograms, and in 8% of the cases, biopsy was warranted. Autologous fat graft is a safe procedure to correct breast defects after BCT, with low postoperative complications. Although it was not associated with increased risk of LR in the group of patients studied, prospective trials are needed to certify that it does not interfere in patient's oncological prognosis.

BACKGROUND: Given the wide application of autologous fat grafting, a new emphasis on fat processing techniques has emerged in an effort to limit unpredictable degrees of resorption often seen with this procedure. With the growing interest in regenerative medicine, approaches to supplement fat grafts with adipose-derived stem cells are evolving in hopes of promoting vascularization and neoadipogenesis. OBJECTIVE: The authors evaluated the outcomes of the most common processing techniques for fat grafting--decantation, washing, high-speed centrifugation--and stromal vascular cell-supplemented lipotransfer to determine which method yields a higher percentage of retention and better quality graft. METHODS: A total of 32 subcutaneous injections of processed human lipoaspirate were carried out in 8 athymic rats. Each animal received all 4 processing conditions, with end points at 4, 8, and 12 weeks postinjection. Evaluation of graft survival included serial measurements of volume retention and histologic analysis. RESULTS: At 12 weeks postinjection, cell-supplemented and centrifuged grafts showed the most consistent volume maintenance. Based on histologic analysis, cell-supplemented and washed grafts had higher scores of viability and vascularity, with the former presenting the least cystic necrosis and calcification as well as minimal inflammation. CONCLUSIONS: Cell-supplemented lipotransfer had optimal outcomes for graft retention, viability, and vascularity, while washing resulted in high viability with a less intensive process. High-speed centrifugation resulted in consistent volume retention but lower viability. Each of these approaches is ideal under different circumstances and contributes to the versatility and reliability of fat grafting.

Lipofilling ou enxerto de gordura é uma técnica muito utilizada para corrigir defeitos em qualquer região do corpo humano. O objetivo deste estudo foi analisar a segurança desta técnica para corrigir defeitos de reconstruções mamárias após câncer de mama. Foram analisados 158 pacientes que receberam 194 procedimentos de enxerto de gordura, segundo a técnica de Coleman e realizados por um único cirurgião (MR). 98% das pacientes desta série tinham cirurgia oncológica mamária precedente: conservadora ou mastectomia com reconstrução. A única complicação pós-operatória encontrada nesta série foi a liponecrose associada à mastite em sete casos (3.6%), todos tratados com terapia antibiótica e curativos diários. Nos pacientes com precedente de cirurgia conservadora e tratados com enxerto de gordura, somente quatro (5.9%) apresentaram pequenas alterações na mamografia, simulando alterações benignas. Conclusão: O enxerto gorduroso pode ser uma boa solução para reparar defeitos após reconstrução em pacientes tratados de câncer de mama e pode reduzir indicações de cirurgias maiores, como reconstruções com retalhos miocutâneos. A incidência de complicações pósoperatórias é muito baixa e não ocorrem alterações significativas nas mamografias de seguimento oncológico...


Fat grafting is largely used to correct soft-tissue defects in any region of the human body. This study analysed its safety when the technique is used to correct defects after breast cancer reconstruction. A total of 158 patients who underwent 194 breast fat grafting procedures were analysed. Almost all patients (98%) had a personal history of breast cancer treated with conservative surgery or mastectomy followed by breast reconstruction. In all cases, fat grafting was performed according to the Coleman’s technique by a single surgeon. Immediate complications included liponecrosis and infection in seven cases (3.6%) that required only daily dressings and oral antibiotics administration. In cases of fat grafting after conservative surgery, only four patients (5,9%) showed minor alterations in the postoperative mammograms, consisting of the appearance of benign images. Breast fat grafting can be a good solution to repair defects after breast-cancer treatment and reconstruction, and can reduce the indication for more extensive surgeries such as myocutaneous flaps. Postoperative complication rates are very low and there is little alteration in the follow-up mammograms...
