RESUMO

Mistreatment of medical students is an international problem that has been reported for decades in different countries, but its conceptualization and registration form are very heterogeneous. This review aims to identify the main features of this mistreatment from a systematic analysis of the literature published between 1980 and 2016. Using databases, 118 published papers were obtained under the selected criteria. Most widely accepted definitions are presented and structuring of the following categories: directionality, types, perpetrators, scenarios, vulnerable groups, consequences, complaints, and the way they justify mistreatment. Concluding, in order to bring down abuse in medical students, it is proposed to replace the verticality and submission by teamwork and collaboration among all, and promote strategies of complaint and admonition of the perpetrator.

RESUMO

The structure of the change process on medical professionalization has not being studied enough. The physicians are made aware of the necessity of changing their medical practice in ways that also affect their personal life. A change involves the need to plan, evaluate possibilities and resources with efficacy and outline competencies. The aim is to contribute in the design of educational strategies that promote professional change and an understanding of change, as an evolution; we describe a theoretical-schematic model based on five consecutive ordained scopes: background, intentions, planning and making decisions, carrying out decisions and achieving the goal.

RESUMO

En este ensayo se revisan los conceptos de ética, bioética y deontología que con mayor frecuencia son aceptados. De igual forma se analiza la relación entre ética y medicina, así como algunas situaciones que generan dilemas éticos además de influir en la relación médico paciente, en el trabajo clínico del médico y en las decisiones que se toman tanto en el nivel individual como en el de las instituciones de salud. Se discuten también algunos factores que resultan de la relación entre la ética y la atención de la salud, en particular la necesidad de preservar y acrecentar la dimensión humana del trabajo médico. Se sostiene que para lograr lo anterior se deben superar tres retos fundamentales: vencer el reduccionismo en el análisis de los asuntos éticos médicos; saber manejar la incertidumbre en el trabajo médico; alcanzar niveles adecuados de equidad en la prestación de los servicios de atención a la salud.


In this essay, the concepts of ethics, bioethics, and professional ethics are reviewed. In addition, we analyzed the relationship between ethics and medicine, the conditions that can be source of ethical dilemmas, andthe influence of ethics in the doctor-patient relationship, in the clinical work of the physician, and in an individual and institutional process of decision- making. Some factors that result from the interrelationship between ethics and health care, in particular the need for preserving and increasing the human dimension in medical practice, are discussed. Finally, the idea is supported that to achieve the purpose previously mentioned, three challenges must to be overcome: to defeat reductionism in analysis of medical matters, to accept the uncertainty of medical work, and the attempt to reach adequate levels of equity in the development of health services.

RESUMO

This paper analyzes the limitations of dominant paradigms in education and identifies the necessity of research-situated learning in real environments and how medical education must be involved with knowledge management in real, complex, adaptive systems, and concludes with the need for constructing novel educative paradigms with regard to new educational paradigms.

RESUMO

El presente trabajo cuestiona los paradigmas dominantes en la educación médica y establece la necesidad de participar en la investigación del aprendizaje in situ, para incorporar a la educación médica como elemento indispensable del proceso de gestión del conocimiento, analiza el sistema de salud considerado como sistema complejo adaptativo y concluye en la necesidad de construir nuevos paradigmas educativos.

RESUMO

Se analizan las aplicaciones sociales de la calidad profesional de los médicos y del proceso certificador. La adecuada competencia de los médicos reviste gran importancia en los países caracterizados por amplias diferencias socioeconómicas. Esta diversidad de condiciones, demanda médicos capaces para confrontarse con una variedad de ambientes y lograr la mejor práctica profesional con uso óptimo de los recursos para la atención a la salud. La competencia y la calidad de la atención están articuladas. El acoplamiento entre el conocimiento universal y las condiciones locales crea una diversidad sincrónica de prácticas profesionales. De igual forma, la certificación de los médicos debe ser vista como parte del proceso difusor de las innovaciones. Asimismo, la existencia de capacidades diferenciales para contender con el cambio, contribuye a crear una diversidad diacrónica de prácticas médicas. A la luz del proceso de mejora continua de la calidad, los sistemas de certificación requieren articularse tanto con la educación médica continua, como con el proceso de acreditación de las instituciones de salud. De lo contrario, se corre el riesgo de aumentar la falta de equidad en la práctica médica y terminar culpando a la víctima.
