Study design: Systematic Review and Meta-analysis. Objective: To compare the complication rates associated with anterior and posterior approaches for the surgical treatment of unstable hangman's fractures. Methods: A systematic review and meta-analysis were performed following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines in PubMed, Web of Science, and Scopus databases to identify comparative studies reporting complications of anterior versus posterior approaches for the treatment of unstable hangman's fractures. Results: The search yielded 1163 papers from which 5 studies were fully included. One hundred fifteen (115) patients were operated on using an anterior approach versus 65 through a posterior approach. The average complication rates for the anterior and posterior approaches were 26.1 % and 13.8 %, respectively. No complications following the anterior approach required pharmacological or surgical intervention (Clavien-Dindo, Grade 1), while 88.9 % of complications following the posterior approach did (Clavien-Dindo, Grade 2). Conclusion: No significant differences in the complication rates were found when comparing anterior versus posterior surgery for treating a C2 traumatic spondylolisthesis. However, most of the complications presented in the posterior surgery group were more severe.

Surgical treatment of high-grade spondylolisthesis is controversial and aims at restoring the spinopelvic sagittal balance through complete or partial reduction of the listhesis. Nerve decompression and interbody fusion are necessary for patients presenting with neurological deficit, severe pain, lower limb asymmetry, or deformities. We present the case and the results of a patient with high-grade spondylolisthesis, in whom minimally invasive management was performed. A narrative review in this topic is also provided. We performed a literature review of high-grade spondylolisthesis to compare our technique to current surgical alternatives. We included articles from PubMed, Embase, Scopus, Ovid, and Science Direct published between 1963 and 2022 that were written in English, German, and Spanish. The terms used were the following: "high grade spondylolisthesis," "spondyloptosis," "surgical management," "interbody fusion," and "arthrodesis." In all, 485 articles were displayed, from which we filtered 112 by title and abstract. At the end, 75 references were selected for the review. Different interbody fusion techniques can be used to correct the lumbosacral kyphosis and restore the spinopelvic parameters. A complete reduction of the listhesis is not always required. The surgical procedure carried out in our patient corresponds to the first known case of minimally invasive circumferential arthrodesis with iliac screws and sacral fixation in a high-grade dysplastic spondylolisthesis. This approach guarantees the correction of the lumbosacral kyphosis and a complete reduction of the listhesis. Further studies are required to determine whether the results of this case can be extrapolated to other patients with high-grade spondylolisthesis.
RESUMEN
PURPOSE: To describe the surgical technique and methodology to successfully plan and execute an endoscopic foraminotomy in patients with isthmic or degenerative spondylolisthesis, according to each patient's unique characteristics. METHODS: Thirty patients with degenerative or isthmic spondylolisthesis (SL) with radicular symptoms were included from March 2019 to September 2022. Treating physician registered patients' baseline and imaging characteristics, as well as preoperative back pain VAS, leg pain VAS and ODI. Subsequently, authors treated the included patients with an endoscopic foraminotomy according to a "patient-specific" tailored approach. RESULTS: Nineteen patients (63.33%) had isthmic SL and 11 patients (36.67%) had degenerative SL. 75.86% of the cases had a Meyerding Grade 1 listhesis. One of the transforaminal foraminotomies with lateral recess decompression in degenerative SL had to be aborted because of intense osseous bleeding. Of the remaining 29 patients, one patient experienced recurrence of the sciatica pain that required subsequent reintervention and fusion. No other intraoperative or post-operative complications were observed. None of the patients developed post-operative dysesthesia. In 86.67% of the patients, the foraminotomy was implemented using a transforaminal approach. In the remaining 13.33% of the cases, an interlaminar contralateral approach was used. Lateral recess decompression was performed in half of the cases. Mean follow-up time was 12.69 months, reaching a maximum of 40 months in some patients. Outcome variables such as VAS for leg and back pain, as well as ODI, showed statistically significant reduction since the 3-month follow-up visit. CONCLUSION: In the presented case series, endoscopic foraminotomy achieved satisfactory outcomes without sacrificing segmental stability. The proposed patient-specific "tailored" approach allowed to successfully design and execute the surgical strategy to perform an endoscopic foraminotomy through transforaminal or interlaminar contralateral approaches.

ABSTRACT Objective: evaluate the correlation between main sacropelvic alignment measurements and pelvic retroversion reduction and assessing Lamartina Square effectiveness in choosing the proximal instrumentation level. Methods: sample comprising 21 patients with high-grade L5 -S1 spondylolisthesis subjected to arthrodesis with instrumentation from January 2004 to December 2016. Patients' demographic data, surgery type, complications, sagittal alignment parameters, Severity Index and Lamartina Square were recorded before surgery and in the last follow-up. Data processed in SPSS 22.0, with different means were calculated through Student's t test, for paired data. Linear correlation analysis was performed based on Spearman's coefficient. P <0.05 was statically significant. Results: there was significant improvement in the slip and Dubousset's lumbosacral kyphosis angles (> 100° in the last postoperative follow-up). There was significant reduction of slip rate ( and in the L4 and L5 Severity Index, which highlighted strong correlation to pelvic tilt, mismatch and slip angle. Severity index also showed strong inverse correlation between Dubousset's lumbosacral kyphosis angle and sacral slope. Conclusion: L5 Severity Index and Lamartina Square are important variables for preoperative planning of patients with high-grade lumbar spondylolisthesis. Level of Evidence IV; Case Series.
Resumo: Objetivo: avaliar a correlação entre as principais medidas do alinhamento sacropélvico com a retroversão pélvica e avaliar a efetividade do Lamartina Square na escolha do nível proximal de intrumentação. Métodos: amostra com 21 pacientes com espondilolistese de alto grau L5-S1, submetidos à artrodese com instrumentação, de 01/2004 a 12/2016. Os dados demográficos dos pacientes, tipos de cirurgias, complicações, parâmetros do alinhamento sagital, Severity Index e Lamartina Square foram registrados antes da cirurgia e no último acompanhamento. Dados processados no SPSS 22.0 com as diferenças das médias calculadas utilizando-se o teste t de Student para dados emparelhados. A análise da correlação linear foi realizada pelo coeficiente de Spearman. Significantes as análises com p < 0,05. Resultados: melhora significativa do ângulo de deslizamento e de cifose lumbosacral de Dubousset´s (> 100° no ultimo seguimento pósoperatório). Redução significativa da porcentagem de deslizamento ( e do severity index em L4 e L5. Este teve uma forte correlação direta com: tilt pélvico, mismatch, ângulo de deslizamento, porcentagem de deslizamento e uma forte inversa com: ângulo de cifose lumbosacral de Dubousset´s e o slope sacral. Conclusão: O Severity Index de L5 e o Lamartina Square devem ser consideradas variáveis importantes no planejamento pré-operatório de pacientes com espondilolistese lombar de alto grau.

Métodos: muestra compuesta por 21 pacientes con espondilolistesis L5-S1 de alto grado sometidos a artrodesis con instrumentación desde enero de 2004 hasta diciembre de 2016. Se registraron datos demográficos de los pacientes, tipo de cirugía, complicaciones, parámetros de alineación sagital, Índice de Severidad y Cuadrado de Lamartina antes de la cirugía y en el último seguimiento. Los datos procesados en SPSS 22.0, con diferentes medias, se calcularon mediante la prueba t de Student, para datos pareados. Se realizó un análisis de correlación lineal basado en el coeficiente de Spearman. P <0,05 fue estadísticamente significativo. Resultados: hubo mejoría significativa en los ángulos de deslizamiento y cifosis lumbosacra de Dubousset (> 100° en el último control postoperatorio). Hubo una reducción significativa en la tasa de deslizamiento (<50 %) y en el índice de gravedad L4 y L5, que destacó una fuerte correlación con la inclinación pélvica, la falta de coincidencia y el ángulo de deslizamiento. El índice de gravedad también mostró una fuerte correlación inversa entre el ángulo de cifosis lumbosacra de Dubousset y la pendiente sacra. Conclusión: L5 Severity Index y Lamartina Square son variables importantes para la planificación preoperatoria de pacientes con espondilolistesis lumbar de alto grado. Background: Controversy exists in the literature about whether facet effusions and other degeneration parameters are associated with instability. Objective: To assess the association between facet effusions and other lumbar degeneration parameters and segmental instability. Material and Methods: In this study, 207 L4-L5 and L5-S1 levels in 104 patients were assessed. We divided the spinal levels into two groups: the small facet effusions (SFE) group in whom facet effusions were <1.5 mm or non-effusions were found, and the large facet effusions (LFE) group in whom they were ≥1.5 mm. The association between other degeneration parameters and instability was also assessed, such as disc degeneration, Modic changes (MC), spondylolisthesis, facet orientation and tropism, facet subchondral sclerosis, and facet cartilage degeneration. Furthermore, we subdivided the levels into subgroups to evaluate the association of LFE and instability within each one. Results: The main analysis comparing the presence of instability in SFE and LFE groups showed a non-statistically significant association between LFE and instability. The presence of MC type 1 and the existence of L4-L5 spondylolisthesis had a statistically significant association with instability. In the subset of 43 levels with L4-L5 degenerative spondylolisthesis, the presence of LFE and the existence of MC type 1 reached a significant association with instability. Conclusion: The presence of LFE and/or MC type 1 may act as red flags in patients with L4-L5 degenerative spondylolisthesis to suspect segmental instability.

Abstract Objective To assess the role of facet tropism (FT) in intervertebral disc prolapse. Methods A total 98 patients with lower back pain were included in the study. Magnetic resonance imaging scans were performed and analyzed. The angles of the right and left facets were measured on the axial section. Patients without disc prolapse at the L3-L4, L4-L5 and L5-S1 levels act as controls for those with disc prolapse at the same levels. A statistical analysis was also performed. Results The incidence of FT at the L3-L4 level was of 85.2% in patients with disc herniation (n= 27), and of 56.3% in the control group, which was statistically significant (p= 0.008). Similarly, at the L4-L5 level, incidence of FT among cases and controls was of 71.4% (n= 35) and 52.4% respectively (p= 0.066). At the L5-S1 the incidence was of 66% and 51% among cases and controls respectively (p= 0.13). Conclusion We found a positive association between FT and disc herniation at the L3-L4 level, but no association at the L4-L5 and L5-S1 levels.
Resumo Objetivo Avaliar o papel do tropismo facetário (TF) no prolapso discal intervertebral. Métodos Um total de 98 pacientes com dor lombar foram incluídos no estudo. Exames de ressonância magnética foram realizados e analisados, e os ângulos das facetas direita e esquerda foram medidos na seção axial. Os pacientes sem prolapso discal nos níveis L3-L4, L4-L5 e L5-S1 atuam como controles para aqueles com prolapso nos mesmos níveis. Fez-se também uma análise estatística. Resultados A incidência de TF no nível L3-L4 foi de 85,2% em pacientes com hérnia discal (n= 27), e de 56,3% no grupo controle, o que foi estatisticamente significativo (p= 0,008). Da mesma forma, a incidência de TF no nível L4-L5 entre casos e controles foi de 71,4% (n= 35) e 52,4%, respectivamente (p= 0,066). No nível L5-S1, a incidência foi de 66% e 51% nos caso e nos controles, respectivamente (p= 0,13). Conclusão Encontramos associação positiva entre TF e hérnia de disco no nível L3-L4, mas nenhuma associação nos níveis L4-L5 e L5-S1.

Objective The present study evaluates radiographic outcomes and the lumbar lordosis achieved with a transforaminal lumbar interbody fusion (TLIF) arthrodesis technique according to the positioning of an interbody device (cage) in the disc space. Methods This is a retrospective radiographic analysis of single-level surgical patients with degenerative lumbar disease submitted to a TLIF procedure and posterior pedicle instrumentation. We divided patients into two groups according to cage positioning. For the TLIF-A group, the cages were anterior to the disc space; for the TLIF-P group, cages were posterior to the disc space. Considering the superior vertebral plateau of the lower vertebra included in the instrumentation, cages occupying a surface equal to the anterior 50% of the midline were placed in the TLIF-A group, and those in a posterior position were placed in the TLIF-P group. We assessed pre- and postoperative orthostatic lateral radiographs to obtain the following measures: lumbar lordosis (LL) (angle L1-S1), segmental lordosis (LS) (L4-S1), and segmental lordosis of the cage (SLC). Results The present study included 100 patients from 2011 to 2018; 44 were males, and 46 were females. Their mean age was 50.5 years old (range, 27 to 76 years old). In total, 43 cages were "anterior" (TLIF-A) and 57 were "posterior" (TLIF-P). After surgery, the mean findings for the TLIF-A group were the following: LL, 50.7°, SL 34.9°, and SLC 21.6°; in comparison, the findings for the TLIF-P group were the following: LL, 42.3° ( p < 0.01), SL 30.7° ( p < 0.05), and SLC 18.8° ( p > 0.05). Conclusion Cage positioning anterior to the disc space improved lumbar and segmental lordosis on radiographs compared with a posterior placement.

Resumo Objetivo Avaliar os resultados radiográficos e comparar a lordose pós-operatória em técnica de artrodese intersomática lombar transforaminal (TLIF, na sigla em inglês), considerando como variável o posicionamento do dispositivo intersomático (cage) em relação ao espaço discal. Métodos Análise retrospectiva radiográfica de pacientes cirúrgicos, em nível único, por doença lombar degenerativa, aplicando-se TLIF e instrumentação pedicular posterior. Os pacientes foram divididos, conforme a posição do cage, em 2 grupos: 1. TLIF-A - cages na posição anterior do espaço discal; e 2. TLIF-P, cages na posição posterior do espaço discal (considerando-se o platô vertebral superior da vértebra inferior incluída na instrumentação, cages que ocuparam a superfície correspondente a 50% anterior da linha média, compuseram o grupo TLIF-A; opostamente, cages em posicionamento posterior compuseram o grupo TLIF-P). Procedeu-se à avaliação dos exames radiográficos ortostáticos em perfil no pré- e pós-operatórios, com a tomada das seguintes medidas: lordose lombar (LL) (ângulo L1-S1); lordose segmentar (LS) (L4-S1) e lordose segmentar do cage (LSC). Resultados Cem pacientes foram incluídos de 2011 a 2018, sendo 44 homens e 46 mulheres, com idade média de 50.5 anos (27-76 anos). Um total de 43 cages foram classificados como "anteriores" (TLIF-A) e 57, "posteriores" (TLIF-P); considerando o grupo TLIF- A, os resultados pós-operatórios médios foram: LL 50.7°, LS 34.9° e LSC 21.6°; para o grupo TLIF-P, comparativamente: LL 42.3° (p< 0,01), LS 30.7° (p< 0,05) e LSC 18.8° (p> 0,05). Conclusão O posicionamento anterior do cage em relação ao espaço discal correlaciona-se a melhora da lordose lombar e segmentar na radiografia em comparação com o posicionamento posterior do implante.

Abstract: Introduction: exercise programs can reduce pain and improve functionality in patients with degenerative spondylolisthesis and chronic low back pain. However, there is still no consensus surrounding the superiority of any specific routine for exercise-induced trophic changes of lumbar muscles. The aim was to compare the changes in the primary lumbar stabilizing muscle thickness after spine stabilization exercises and flexion exercises in patients with spondylolisthesis and chronic low back pain. Material and methods: prospective, longitudinal and comparative study was carried out. Twenty-one treatment-naive patients with a diagnosis of both chronic low back pain and degenerative spondylolisthesis over the age of 50 were included. A physical therapist taught participants either spine stabilization exercises or flexion exercises to execute daily at home. The thickness of the primary lumbar muscles was measured through ultrasound (at rest and contraction) at baseline and three months. A Mann-Whitney U test and Wilcoxon signed-rank test were performed for comparisons, and Spearman's rank correlation coefficients were calculated for associations. Results: we did not find statistically between the exercise programs: all patients presented significant changes in the thickness of the multifidus muscle but in none of the other evaluated muscles. Conclusion: there is no difference between spine stabilization exercises and flexion exercises after three months in terms of the changes in muscle thickness evaluated by ultrasound.
Resumen: Introducción: el ejercicio reduce el dolor y mejora la funcionalidad en pacientes con dolor crónico lumbar y espondilolistesis degenerativa. Sin embargo, no existe a la fecha un consenso sobre la superioridad de algún programa de ejercicio para inducir cambios tróficos de los músculos estabilizadores lumbares, por lo que el objetivo fue comparar el trofismo de estos músculos mediante ultrasonido, con dos programas de ejercicio distintos: estabilización vertebral versus ejercicios flexores. Material y métodos: estudio prospectivo, longitudinal y comparativo, en veintiún pacientes mayores de 50 años, con dolor crónico lumbar y espondilolistesis degenerativa. Se entrenó a los pacientes para la ejecución diaria de ejercicio: estabilización lumbar o ejercicios flexores, los cuales fueron asignados por aleatorización como parte de un ECA en desarrollo. El trofismo muscular fue evaluado mediante ultrasonido al inicio y a tres meses. Las pruebas de U de Mann-Whitney y prueba de Wilcoxon se usaron para comparaciones entre grupos y para correlaciones se usaron los coeficientes de correlación de Spearman. Resultados: todos los pacientes presentaron ganancia en el trofismo de los músculos multífidos a tres meses, pero sin diferencias entre grupos de tratamiento. No se detectaron cambios significativos en el resto de los músculos evaluados. Conclusión: no encontramos diferencia significativa entre los ejercicios de estabilización lumbar y los ejercicios flexores, a tres meses de seguimiento, en términos de los cambios tróficos medidos por ultrasonido de los músculos estabilizadores lumbares.

BACKGROUND CONTEXT: Lumbar degenerative spondylolisthesis is a relatively common pathology, and surgical treatment is an option in selected cases. The common use of anterior column support by interbody fusion (IBF) has not been adequately studied. PURPOSE: The primary objective of this systematic review and meta-analysis was to compare the functional and surgical outcomes after posterolateral fusion (PLF) and IBF for the treatment of degenerative spondylolisthesis. STUDY DESIGN: A systematic review and meta-analysis was conducted to identify studies analyzing the effects of IBF compared to those of instrumented PLF on degenerative spondylolisthesis. PATIENT SAMPLE: A total of 1,063 patients were included in the analysis. Of them, 470 patients were treated with PLF and 593 with IBF. OUTCOME MEASURES: The outcomes analyzed were the visual analog scale and numerical rating scale scores for back pain, Oswestry Disability Index (ODI), 36-Item Short Form Health Survey (SF-36) score, 12-Item Short Form Health Survey (physical and mental components) score, blood loss amount, surgical time, hospital stay duration, fusion rate, lumbar lordosis, sacral slope, and postoperative complications. METHODS: A search was conducted using the MEDLINE, Cochrane, and Google Scholar databases for studies published between January 1985 and February 2021. Both retrospective and prospective studies that compared between IBF and PLF were included. RESULTS: A total of 2,819 articles were screened, and 38 full-text articles were selected for a detailed investigation, of which 12 articles were found to match all the inclusion criteria and were included in the meta-analysis. There was an improvement in the sacral slope (mean difference [MD] = 3 [95% confidence interval {CI} = 0.83-5.17]) and fusion rate (odds ratio [OR] = 0.47 [95% CI = 0.26-0.86]) in the IBF group. The incidence of neural injury was higher in the IBF group (OR = 0.28 [95% CI = 0.13-0.60]) than in the PLF group. There were no differences in the back pain scores, ODI, SF-36 score, blood loss amount, surgical time, hospital stay duration, lumbar lordosis, infection, and durotomy between the groups. CONCLUSIONS: IBF for the treatment of lumbar degenerative spondylolisthesis showed better results in terms of the fusion rate and sacral slope, but which did not translate into better clinical outcomes. Further randomized and prospective studies are necessary to elucidate the optimal therapeutic options.

Twenty-one treatment-naive patients with a diagnosis of both chronic low back pain and degenerative spondylolisthesis over the age of 50 were included. A physical therapist taught participants either spine stabilization exercises or flexion exercises to execute daily at home. The thickness of the primary lumbar muscles was measured through ultrasound (at rest and contraction) at baseline and three months. A Mann-Whitney U test and Wilcoxon signed-rank test were performed for comparisons, and Spearman's rank correlation coefficients were calculated for associations. RESULTS: we did not find statistically between the exercise programs: all patients presented significant changes in the thickness of the multifidus muscle but in none of the other evaluated muscles. CONCLUSION: there is no difference between spine stabilization exercises and flexion exercises after three months in terms of the changes in muscle thickness evaluated by ultrasound.
INTRODUCCIÓN: el ejercicio reduce el dolor y mejora la funcionalidad en pacientes con dolor crónico lumbar y espondilolistesis degenerativa. Sin embargo, no existe a la fecha un consenso sobre la superioridad de algún programa de ejercicio para inducir cambios tróficos de los músculos estabilizadores lumbares, por lo que el objetivo fue comparar el trofismo de estos músculos mediante ultrasonido, con dos programas de ejercicio distintos: estabilización vertebral versus ejercicios flexores. MATERIAL Y MÉTODOS: estudio prospectivo, longitudinal y comparativo, en veintiún pacientes mayores de 50 años, con dolor crónico lumbar y espondilolistesis degenerativa. Se entrenó a los pacientes para la ejecución diaria de ejercicio: estabilización lumbar o ejercicios flexores, los cuales fueron asignados por aleatorización como parte de un ECA en desarrollo. El trofismo muscular fue evaluado mediante ultrasonido al inicio y a tres meses. Las pruebas de U de Mann-Whitney y prueba de Wilcoxon se usaron para comparaciones entre grupos y para correlaciones se usaron los coeficientes de correlación de Spearman. RESULTADOS: todos los pacientes presentaron ganancia en el trofismo de los músculos multífidos a tres meses, pero sin diferencias entre grupos de tratamiento. No se detectaron cambios significativos en el resto de los músculos evaluados. CONCLUSIÓN: no encontramos diferencia significativa entre los ejercicios de estabilización lumbar y los ejercicios flexores, a tres meses de seguimiento, en términos de los cambios tróficos medidos por ultrasonido de los músculos estabilizadores lumbares.

ABSTRACT Objective: To evaluate the temporal trends of thoracolumbar fusion procedures performed by the United Health System in Brazil from 2009 to 2019. Methods: This was an observational ecological study based on data collected from the information systems under the aegis of DATASUS, especially the Hospital Information System, which manages the Hospital Admission Authorizations (Autorizações de Internação Hospitalar - AIHs). All patients who had undergone the procedures of interest, that is, elective and emergency short and long thoracolumbar arthrodeses, were included. The temporal trends of the procedures performed in Brazil and in its five regions were calculated using polynomial regression. Results: The temporal trend of elective thoracolumbar arthrodesis decreased, while that of emergency arthrodesis increased, with the peak in 2015, followed by a marked decline. Short fusions were more frequent in both elective and emergency modalities, and the South and Central-West Regions had the highest indices of procedures per million inhabitants during the entire 2009 to 2019 period. Conclusions: The temporal trends of thoracolumbar fusions performed by SUS have decreased over the last decade, a phenomenon which may be explained by the growing criticism of indications of the procedure in the current literature. RESUMO: Objetivo: Analisar a tendência temporal dos procedimentos de artrodese toracolombar realizados pelo Sistema Único de Saúde no Brasil entre os anos de 2009 e 2019. Métodos: Trata-se de estudo observacional de delineamento ecológico, baseado em dados secundários coletados a partir dos sistemas de informação sob guarda do DATASUS, em especial do Sistema de Informações Hospitalares, que gerencia as Autorizações de Internação Hospitalar (AIHs). Foram incluídos todos os pacientes submetidos aos procedimentos de interesse, de caráter eletivo e de urgência, correspondentes às artrodeses toracolombares curtas e longas. A tendência temporal foi avaliada através da regressão polinomial dos procedimentos realizados no Brasil e em suas cinco regiões federativas. Resultados: A tendência temporal das artrodeses eletivas foi decrescente e a das artrodeses de urgência foi crescente, com pico em 2015, seguida de franco declínio. As artrodeses curtas foram mais frequentes em ambas as modalidades de urgência e eletiva, e as regiões Sul e Centro-Oeste obtiveram os maiores índices de procedimentos por milhão de habitantes em todo período de 2009 a 2019. Conclusão: A tendência temporal das artrodeses toracolombares realizadas pelo SUS tem caráter decrescente na última década, fenômeno que pode ser justificado pela crescente criticidade perante as indicações do procedimento na literatura contemporânea. Resultados: La tendencia temporal de la artrodesis electiva fue decreciente y la de la artrodesis de urgencia fue creciente, alcanzando el pico en 2015, seguido de un fuerte descenso. Los casos de artrodesis cortas fueron más frecuentes tanto en las modalidades de urgencia como en las electivas, y las regiones Sur y Medio Oeste tuvieron las tasas más altas de procedimientos por millón de habitantes en todo el período de 2009 a 2019. Conclusión: La tendencia temporal de las artrodesis toracolumbares realizadas por el SUS tiene un carácter decreciente en la última década, fenómeno que puede justificarse por la creciente criticidad de las indicaciones del procedimiento en la literatura contemporánea. Introduction The management of spondylolisthesis and spondylolysis is primarily conservative or surgical. There are various surgical procedures available for spondylolisthesis. Objective To evaluate the functional outcome and efficacy in patients undergoing transforaminal lumbar interbody fusion with transpedicular screws and rods in symptomatic lumbar spondylolisthesis and spondylolysis. Methods From 2017 to 2018, a prospective observational study was performed in a tertiary care hospital. The preoperative evaluation was performed both clinically and radiologically. Based on indications, transforaminal interbody fusion was performed. A total sample of 20 patients was included. The primary outcome variables were the visual analogue scale (VAS), the Oswestry Disability Index (ODI), slip percentage, and disc height at follow-up. For the statistical analysis, coGuide (BDSS CORP, Bangalore, Karnataka, India) was used. Results In 20 participants studied, the mean age was 48.25 5.35 years old. Degenerative spondylolisthesis was seen in 60% of the participants. The majority (70%) of the patients had grade 2 slips. The mean difference of the VAS, the ODI, slip percentage, and disc height between the preoperative and postoperative periods was statistically significant (p < 0.001). The majority (70%) of the patients had no complications after the procedure. Conclusions Transforaminal interbody fusion with pedicle screws and rods is a safe, simple procedure and has less morbidity. This procedure also provides better functional outcomes and reduction in symptoms by maintaining the disc height and providing sagittal balance.
Introdução O tratamento da espondilolistese e espondilólise é principalmente conservador ou cirúrgico. Existem vários procedimentos cirúrgicos disponíveis para espondilolistese. Objetivo Avaliar o resultado funcional e a eficácia em pacientes submetidos a fusão intersomática lombar transforaminal com parafusos e hastes transpediculares em espondilolistese e espondilólise lombar sintomática. Métodos De 2017 a 2018, foi realizado um estudo observacional prospectivo em um hospital terciário. A avaliação pré-operatória foi feita clínica e radiologicamente. Com base nas indicações, a fusão intersomática transforaminal foi feita. Uma amostra total de 20 pacientes foi incluída. As variáveis de desfecho primárias foram a escala visual analógica (EVA), o Oswestry Disability Index (ODI, na sigla em inglês), porcentagem de escorregamento e altura do disco no acompanhamento. Para análise estatística, foi utilizado o coGuide (BDSS CORP, Bangalore, Karnataka, Índia). Resultados Nos 20 participantes estudados, a média de idade foi de 48,25 5,35 anos. Espondilolistese degenerativa foi observada em 60% dos participantes. A maioria (70%) dos pacientes apresentou deslizamentos de grau 2. A diferença média da EVA, do ODI, da porcentagem de escorregamento e da altura do disco entre os períodos pré- e pós-operatório foi estatisticamente significativa (p < 0,001). A maioria (70%) dos pacientes não apresentou complicações após o procedimento. Conclusões A fusão intersomática transforaminal com parafusos pediculares e hastes é um procedimento seguro, simples e de menor morbidade. Este procedimento também fornece melhores resultados funcionais e redução dos sintomas, mantendo a altura do disco e proporcionando equilíbrio sagital.

BACKGROUND: Surgery is the main treatment for patients with high-grade L5-S1 isthmic spondylolisthesis, which can result in neurologic complications, but little is known about its clinical course. The present study evaluated the presence of L5 radiculopathy in high-grade L5-S1 spondylolisthesis in adults in pre- and postoperative periods and after a 2-year follow-up. METHODS: A series of 16 patients who underwent reduction and instrumented fusion for high-grade 5 and 6 spondylolisthesis between 2018 and 2019 were retrospectively evaluated in the pre- and postoperative periods as well as after 6 weeks, 3 months, 6 months, and 1 and 2 years of follow-up. Clinical and surgical data on possible neurological complications of L5 radiculopathy were prospectively collected. RESULTS: The age was 20.1 ± 12.0 years, and preoperative L5-S1 slip was 89.0%. Five patients presented motor deficit in the preoperative period. In the immediate postoperative period, 9 patients (56%) experienced motor deficits or worsening of the preoperative condition. At the 6-week follow-up, only 1 patient showed resolution of the motor deficit. Three patients presented healed motor deficits after 3 months, and 1 patient demonstrated a healed L5 motor radiculopathy after 6 months. At the 1-year follow-up, only 1 patient exhibited an L5 radiculopathy motor deficit, and at the 2-year follow-up, none of the patients exhibited an L5 radiculopathy motor deficit. CONCLUSION: L5 radiculopathy was frequent in the preoperative period and increased after reduction and instrumented fusion of high-grade L5-S1 spondylolisthesis in the postoperative period and in the 6-week follow-up. Three and 6 months after the surgery, there were consecutive motor improvements. After 2 years of follow-up, no patients showed neurological deficit of L5 radiculopathy. LEVEL OF EVIDENCE: 2. CLINICAL RELEVANCE: This is the first study reporting a reduction in complications of L5 neurological motor deficit over a 2-year follow-up in high-grade L5-S1 spondylolisthesis in young adults.

OBJECTIVE: Exercise is the mainstay of treatment in individuals with low back pain and the first-line option in degenerative spondylolisthesis (DS); however, there is still no consensus surrounding the superiority of any specific exercise program. Thus, the primary aim of this study was to compare the effectiveness of lumbar stabilization exercises and flexion exercises for pain control and improvements of disability in individuals with chronic low back pain (CLBP) and DS. METHODS: A randomized controlled trial was conducted in a tertiary public hospital and included 92 individuals over the age of 50 years who were randomly allocated to lumbar stabilization exercises or flexion exercises. Participants received 6 sessions of physical therapy (monthly appointments) and were instructed to execute exercises daily at home during the 6 months of the study. The primary outcome (measured at baseline, 1 month, 3 months, and 6 months) was pain intensity (visual analog scale, 0-100 mm) and disability (Oswestry Disability Index, from 0% to 100%). Secondary outcomes were disability (Roland-Morris Disability Questionnaire, from 0 to 24 points), changes in body mass index, and flexibility (fingertip to floor, in centimeters) at baseline and 6 months, and also the total of days of analgesic use at 6-month follow-up. RESULTS: Mean differences between groups were not significant (for lumbar pain: 0.56 [95% CI = -11.48 to 12.61]; for radicular pain: -1.23 [95% CI = -14.11 to 11.64]; for Oswestry Disability Index: -0.61 [95% CI = -6.92 to 5.69]; for Roland-Morris Disability Questionnaire: 0.53 [95% CI = -1.69 to 2.76]). CONCLUSION: The findings from the present study reveal that flexion exercises are not inferior to and offer a similar response to stabilization exercises for the control of pain and improvements of disability in individuals with CLBP and DS. ABSTRACT Objective: Evaluate the relationship between the incidence of different types of degenerative diseases of the spine and lumbopelvic biomechanics, according to the types of lordosis of Roussouly's classification. Methods: Retrospective study of medical records and results of imaging exams of patients seen at a private hospital in São Paulo. The sagittal alignment of these patients was evaluated by classifying them according to Roussouly into 4 types, based on panoramic radiographs of the spine. These results were correlated with the patient's degenerative diagnosis (Herniated disc, Canal stenosis, Spondylolisthesis, degenerative discopathy and Facet arthrosis). Statistical tests were performed comparing the types of curvature and diagnoses identified. Results: 418 patients were evaluated, 51.4% male and 49.6% female. The vast majority of patients, about 54%, had a diagnosis of herniated lumbar disc. There was a statistically significant difference that showed a predilection for surgical treatment in cases classified as Type I and Type II in the Roussouly classification. There was no statistically significant difference that correlated the types of lumbar lordosis with the diagnosis presented by the patients. Conclusion: There is no statistically significant difference that correlates the type of lumbar lordosis according to Roussouly with lumbar degenerative diseases. In contrast, patients classified as Type 1 and Type 2 by Roussouly underwent a greater number of surgical treatments compared to patients type 3 and 4, with statistical relevance. RESUMO Objetivos: Avaliar a relação da incidência dos diferentes tipos de doenças degenerativas da coluna com a biomecânica lombopélvica, de acordo com os tipos de lordose segundo Roussouly. Métodos: Estudo retrospectivo de prontuários médicos e de resultados de exames de imagens de pacientes atendidos em um hospital privado de São Paulo. Foi avaliado o alinhamento sagital desses pacientes classificando-os de acordo com Roussouly em 4 tipos, com base nas radiografias panorâmicas da coluna vertebral. Esses resultados foram correlacionados com o diagnóstico degenerativo do paciente (Hérnia de disco, Estenose do canal, Espondilolistese, Discopatia degenerativa e Artrose facetaria). Testes estatísticos foram realizados comparando os tipos de curvatura e diagnósticos identificados. Resultados: Foram avaliados 418 pacientes, sendo que 51,4% do sexo masculino e 49,6% do sexo feminino. A grande maioria dos pacientes, cerca de 54%, apresentavam como diagnóstico hérnia de disco lombar. Houve uma diferença estatisticamente significativa que evidenciou uma predileção do tratamento cirúrgico nos casos classificados como Tipo I e Tipo II na classificação de Roussouly. Não houve diferença estatisticamente significativa que correlacionasse os tipos de lordose lombar com o diagnóstico apresentado pelos pacientes. Conclusões: Não houve diferença estatisticamente significativa que correlacione o tipo de lordose lombar de acordo com Roussouly com as doenças degenerativas lombares. Em contrapartida, os pacientes classificados como Tipo 1 e Tipo 2 de Roussouly foram submetidos em maior número ao tratamento cirúrgico em comparação com os pacientes tipo 3 e 4, com relevância estatística. Estos resultados se correlacionaron con el diagnóstico degenerativo del paciente (disco herniado, estenosis del canal, espondilolistesis, discopatía degenerativa y artrosis facetaria). Se realizaron pruebas estadísticas comparando los tipos de curvatura y los diagnósticos identificados. Resultados: Se evaluaron 418 pacientes, 51.4% hombres y 49.6% mujeres. La gran mayoría de los pacientes, alrededor del 54%, tenían un diagnóstico de hernia de disco lumbar. Hubo una diferencia estadísticamente significativa que mostró una predilección por el tratamiento quirúrgico en los casos clasificados como Tipo I y Tipo II en la clasificación de Roussouly. No hubo diferencias estadísticamente significativas que correlacionaran los tipos de lordosis lumbar con el diagnóstico presentado por los pacientes. RESUMEN Los estudios han determinado la importancia del balance sagital en el desarrollo de entidades degenerativas de la columna vertebral, fundamentalmente de la región lumbar; así como en los resultados de las intervenciones quirúrgicas de estas afecciones. Es propósito de este trabajo revisar y discutir los conceptos actuales sobre la estabilidad sagital vertebral y lumbar, así como su influencia sobre el proceso degenerativo espinal, de manera fundamental sobre estenosis, espondilolistesis y escoliosis degenerativa. Existen parámetros pélvicos y parámetros espinales que tienen que tomarse en cuenta para la determinación preoperatoria y posoperatoria del equilibrio sagital. El equilibrio sagital se clasifica en: balance normal, balance compensado y desbalance. Esto tiene gran importancia para llegar al diagnóstico correcto y aplicar el tratamiento quirúrgico necesario(AU)
ABSTRACT Studies have determined the importance of sagittal balance in the development of degenerative entities of the spine, mainly in the lumbar region; as well as in the results of the surgical interventions of these affections. The purpose of this paper is to review and discuss current concepts on vertebral and lumbar sagittal stability, as well as its influence on the degenerative spinal process, basically on stenosis, spondylolisthesis and degenerative scoliosis. There are pelvic parameters and spinal parameters that have to be taken into account for the preoperative and postoperative determination of sagittal balance. Sagittal balance is classified into normal balance, compensated balance and imbalance. This is very important to reach the correct diagnosis and apply the necessary surgical treatment(AU)

Abstract: There are various approaches and surgical techniques with the objective of nerve root decompression, restrict mobility, and fusion of the listhesis. Among the techniques, posterior interbody fusion combines direct and indirect root decompression with the fusion between vertebral bodies, placing an autologous bone graft between transverse apophysis and vertebral bodies. Transforaminal lumbar and posterior interbody fusion, on the same way, look to decompress and fuse but with a different approach to the spine. The anterior approach for interbody fusion provides a better fusion rate. Lateral lumbar interbody fusion is considered less invasive, with an anterolateral transpsoas approach. The lumbar fusion technique in degenerative spondylolisthesis must be individualized. Non-fusion decompression is considered a less invasive procedure. Various studies suggest that decompression has better results when fusion is added. Surgery had several potential benefits and greater improvement in those patients who fail conservative management. An optimal technique is not conclusively identified.
Resumen: El tratamiento de la espondilolistesis degenerativa lumbar es específico para cada etapa de la enfermedad y el manejo quirúrgico no debe de ser la primera elección en la mayoría de los casos. El manejo conservador está basado en el uso de antiinflamatorios no esteroideos, control de peso y rehabilitación. En caso de falla después de cuatro a seis semanas, el siguiente paso es la infiltración facetaria. En caso de dolor persistente, alteraciones neurológicas o claudicación neurogénica el siguiente paso es la cirugía. Existen varios abordajes y técnicas quirúrgicas con el objetivo de descomprimir las raíces nerviosas, restringir la movilidad y fusionar la listesis. Entre las técnicas quirúrgicas, la fusión posterior combina la descompresión directa e indirecta con artrodesis entre los cuerpos vertebrales, colocando injerto entre las apófisis transversas y los cuerpos vertebrales. La artrodesis intersomática transforaminal y posterior buscan de la misma manera la descompresión y fusión, pero con un abordaje distinto. El abordaje anterior para artrodesis intersomática provee la mejor tasa de fusión. La artrodesis intersomática lateral se considera un procedimiento menos invasivo, con un abordaje anterolateral transpsoas. La técnica de artrodesis lumbar en la espondilolistesis debe ser individualizada. La descompresión sin artrodesis se considera un procedimiento menos invasivo; varios estudios sugieren que la descompresión tiene mejores resultados cuando se agrega una artrodesis. La cirugía tiene múltiples beneficios posibles en pacientes en quienes el tratamiento conservador ha fallado. No se ha identificado una técnica óptima de tratamiento.
