RESUMEN

Altogether 300 patients suffering from bronchial asthma were examined for serotonin metabolism, the kallikrein-kinin system and histamine. A study was also made of the information relations of the indicated parameters to diverse groups of the signs using Shannon's technique. It has been revealed that serotonin and the kallikrein-kinin system are of importance in the pathogenesis of changes in the efficacy of the diffusion lung capacity in severe bronchial asthma. The authors discuss the significance of excessive accumulation of serotonin and bradykinin as a marker of the impairment of endothelial function of pulmonary vessels, specified by the damaging action of free radicals and immune complex allergic vasculitis as well. Histamine along with macrophages act as delimiters of allergic inflammation, with their action being mediated by negative feedback.

RESUMEN

Erythron membrane impairment was investigated using membrane -active drugs (obsidan, delagil, morphium, ethanol) to specify indications to acupuncture in bronchial asthma (BA). The patients were studied during the treatment and followed up for a year. It was found that both immediate and long-term results of acupuncture were related to patterns of the membrane impairment. BA patients with a sharp, greater than 1.5 fold diminution of microcyte count upon delagil test should be assigned to repeated courses of acupuncture during a year. To monitor the treatment effect, it is recommended to define osmotic resistance of the red blood cells in obsidan test in addition to clinicofunctional indices.

RESUMEN

The purpose of the work was to study some characteristic features of the membranoreceptor complex in patients suffering from atopic bronchial asthma (BA) and infection-dependent BA. That was done with the aid of the erythrocytic model (phytohemagglutinin /PHA/ reception, electrophysical analysis of the membranous properties). Electrophoretic mobility (EPM) of red blood cells and red blood cell sedimentation under the action of PHA were determined. The data obtained indicate that in patients with infection-dependent BA, the membranoreceptor complex is characterized by a decrease of EPM of red blood cells, inhibition of red blood cell sedimentation in the presence of PHA. The results of studying the membranoreceptor complex in BA patients are summarized as are the marker characteristics of atopic and infection-dependent BA, reflecting pathology of reception, glycocalyx, and lipid matrix.

The authors review potential approaches to the upgrading of the diagnosis at the out- and inpatient stages of the rendering medical assistance to patients suffering from bronchial asthma (BA). Show the necessity of the counselling specialized aid, additional studies of the signs of atopy, wise combination of non- and medicamentous treatment methods to enhance the efficacy of the treatment of BA patients. Provide evidence for the advisability of solving the organizational problems concerned with a closer interaction of the polyclinic and hospital with the leading medical institutions on the problem "Pathology of the Respiratory Organs".

RESUMEN

Analysis is made of the results of a comprehensive examination of 313 bronchial asthma patients. The examination included the revealing of the information relations of the absolute count of blood eosinophils, their changes after administration of a standard dose of adrenaline, the eosinophil count in sputum and bronchial secretion. Significant information relations with the build-up parameters, alterations in bronchial patency, a number of immunological and biochemical characteristics were discovered. Analysis of the measure of the information relation evidences the pathogenetic and prognostic role of eosinophils in bronchial asthma.

RESUMEN

A model of aggregation of erythrocytes was used to study the characteristic features of membrane disorders in different forms of bronchial asthma. It was found that erythrocytic aggregations depended on the form of bronchial asthma, primarily due to the characteristic features of the erythrocytic cellular membrane. In infectious-dependent bronchial asthma and chronic obstructive bronchitis erythrocytic aggregation was increased while in atopic bronchitis it tended to decrease.

RESUMEN

The pathogenesis of atopic bronchial asthma (ABA) is examined from a membranologic viewpoint, with a methodological approach, postulating membrane-receptor pathology as the key pathogenetic element of the disease, taken as the basis. Major results of the study of the adrenoreactive system are analysed, and pathogenetic significance of the demonstrated paradoxal cell reaction to beta-adreno-antagonist is pointed out. Prospects related to the development of studies of morphofunctional erythrone properties as a new line of research in BA pathogenesis are demonstrated, and erythrocyte qualities applicable as marker of atopia, such as microcytosis and membrane glycoprotein distribution, are identified. Specific pattern of cell membrane response to aldehyde compounds seen in ABA is described. The data obtained are a development of the earlier-postulated hypothesis of ABA-associated dedifferentiation. A notion of ABA as a membrane-receptor disease is proposed.

RESUMEN

Forty-six patients with bronchial asthma and preasthma and 6 normal persons were examined before and after graded exercise. Measurements were made of changes in glycogenolysis in peripheral lymphocytes, in blood plasma osmolarity, external respiration function ad diffusion capacity of the lungs, and of blood and sputum eosinophilia. Some of the patients were also examined for blood content of catecholamines. The group of patients with asthma of effort showed, in contrast to those without that illness, the signs of allergic inflammation of the distal parts of the bronchi and a direct correlation between changes in plasma osmolarity and glycogen content in lymphocytes, a decrease in the grade of the glycogenolytic response to adrenaline after exercise. It seems likely that in the pathogenesis of asthma of effort, of certain importance is inadequate reaction of the adrenoreceptor apparatus of the cells to the changes in the microenvironment provoked by exercise, which is manifested by the lack of the rise of beta-adrenoreactivity.

In 50 patients with asthma we have determined skin tests on dust and pollen allergens, forced expiratory volume for one second (FEV1) before and after subcutaneous injections of epinephrine standard dose, adrenoreactivity by adrenodependent cyclic AMP levels and glycogenolysis, cyclic AMP phosphodiesterase activity, levels of eosinophils in blood (in all 28 characteristic features). Factor and correlation analysis were carried out by electronic computer. In addition, in 121 patients we have determined adrenodependent glycogenolysis in leucocyte suspension incubated with various allergens the content of diene conjugates and total antioxidative activity of plasma. It is concluded that atopic and nonatopic forms of asthma are diseases of different nature in their constitutional origin, pathogenic agents of environment and cellular and subcellular mechanisms of their development. Moreover, the pathogenetic role of allergen-induced lipid peroxidation that correlates with lymphocyte adrenoreactivity is shown.

Peripheral blood erythrocytes of patients with bronchial asthma (BA) and preasthma (PA) were investigated using the following methods: 1) the radiochemical method--the level of specific binding of beta-adrenergic ligands; 2) direct erythrocytometry--erythrocyte diameter; 3) the cytochemical method (paraldehyde fuchsine/PAF/-reaction)--membranous glycoproteins. The level of erythrocytic specific binding of beta-adrenergic ligands was decreased in BA and PA patients. The level of small size erythrocytes (SSE) was on an increase in patients with atopic BA, extrapulmonary allergoses as well as in healthy persons with aggravated atopic heredity. The content of PAF-negative erythrocytes turned out to be considerably raised in atopic BA. A conclusion was made of general disorders of the membranoreceptor system as a fundamental cause of allergically changed reactivity.

Leukosuspensions from 217 patients with bronchial asthma and 73 normal subjects were examined for alterations in the levels of cAMP and cGMP under the effect of incubation with adrenaline, propranolol, acetylcholine. A study was also made of the activity of cAMP PDE and glycogen metabolism. Simultaneous measurement of the basal and stimulated levels of cyclic nucleotides and their ratios revealed a decrease in cell adrenoreactivity and a rise in cholinoreactivity. Variation of the beta-adrenoblocking action of propranolol typical for the atopic mechanism of bronchial asthma was demonstrated. Comparison of the adrenodependent glycogenolysis and alterations in the cAMP level, analysis of the action of analgin on these characteristics allowed the conclusion about the secondary nature of alterations in cAMP metabolism dependent on the features of prostaglandin metabolism. A significant decrease in the adrenodependent glycogenolysis was revealed in lymphocytes, with that decrease induced by drug, dust and pollen allergens. It was demonstrated that in verified viral infection, cAMP PDE was significantly reduced.
