RESUMEN

Background Congenital syndactyly is a common congenital hand anomaly that impairs daily activities and impacts both functional and aesthetic outcomes. The fusion of adjacent fingers limits functionality and often requires surgical intervention to restore web spacing, maintain function, and improve appearance. This study evaluates surgical outcomes of congenital syndactyly treatment using flap and graft techniques, focusing on older patients. Methodology This study utilized retrospective data collected from patients aged 2 to 12 years diagnosed with congenital syndactyly. These patients underwent surgical separation procedures employing various flap techniques and full-thickness skin grafts. The chosen methods aimed to minimize scarring, secure optimal blood supply, and reduce postoperative complications. Postoperative assessments included web spacing, aesthetic appearance, and functional recovery. Results Patients generally experienced improved web spacing and proper alignment, with minimal contracture post-surgery. Flap and graft techniques effectively reduced visible scarring and provided favorable cosmetic results. Functional recovery was significant, allowing patients to resume age-appropriate tasks with minimal limitations, thereby restoring confidence in daily activities. Despite not undergoing early surgery, older patients still achieved marked improvements in web spacing, aesthetics, and overall function. Conclusions Surgical treatment of congenital syndactyly using flap and graft techniques significantly enhances both functional and aesthetic outcomes, even when the intervention is delayed beyond the recommended early age. Comprehensive planning and tailored approaches are crucial to achieving optimal web spacing, minimized scarring, and restored hand function. These measures ultimately improve the quality of life for patients, regardless of age at the time of surgery.

The COVID-19 pandemic was caused by the SARS-CoV-2 virus, marking one of the most catastrophic global health crises of the 21st century. Throughout this period, widespread use and improper disposal of personal protective equipment (PPE) emerged as a pressing environmental issue, significantly impacting various life forms. During the COVID-19 pandemic, there was a high rate of PEP disposal. An alarming 1.6 × 106 tons of plastic waste each day has been generated since the onset of the outbreak, predominantly from the inadequate disposal of PPE. The mismanagement and subsequent degradation of discarded PPE significantly contribute to increased non-biodegradable micro(nano)plastic (MNP) waste. This pollution has had profound adverse effects on terrestrial, marine, and aquatic ecosystems, which have been extensively of concern recently. Accumulated MNPs within aquatic organisms could serve as a potential route for human exposure when consuming seafood. This review presents a novel aspect concerning the pollution caused by MNPs, particularly remarking on their role during the pandemic and their detrimental effects on human health. These microplastic particles, through the process of fragmentation, transform into nanoparticles, persisting in the environment and posing potential hazards. The prevalence of MNP from PPE, notably masks, raises concerns about their plausible health risks, warranting global attention and comprehensive exploration. Conducting a comprehensive evaluation of the long-term effects of these processes and implementing effective management strategies is essential.

BACKGROUND:  Supracondylar humeral fractures are the most prevalent elbow fractures in pediatric patients. Current treatment modalities typically involve closed reduction and fixation using percutaneous Kirschner wires. The lateral cross-wiring technique has demonstrated favorable functional and cosmetic outcomes, thereby reducing the incidence of ulnar nerve injury. OBJECTIVES:  This study aimed to assess the efficacy of the lateral cross-wiring technique in achieving optimal functional and cosmetic recovery while mitigating the risk of ulnar nerve injury in pediatric patients with displaced supracondylar humeral fractures. MATERIALS AND METHODS: A prospective analysis was conducted on 48 patients who underwent lateral cross-wiring for displaced supracondylar fractures (Gartland type III, including extension and flexion types) of the humerus. Follow-up assessments were performed over a minimum period of eight months post-surgery. RESULTS: Among the 48 patients, all demonstrated satisfactory restoration of the carrying angle and functional ability postoperatively. Notably, no iatrogenic cases of radial or ulnar nerve injury were observed during the follow-up period. CONCLUSION:  The lateral cross-wiring technique emerges as an effective treatment option for displaced supracondylar fractures of the humerus in pediatric patients. It facilitates both functional and cosmetic recovery while concurrently reducing the risk of ulnar nerve injury, thus warranting consideration in the management of such fractures.

Unlike proteins, which have a wealth of validated structural data, experimentally or computationally validated DNA origami datasets are limited. Here we present a graph neural network that can predict the three-dimensional conformation of DNA origami assemblies both rapidly and accurately. We develop a hybrid data-driven and physics-informed approach for model training, designed to minimize not only the data-driven loss but also the physics-informed loss. By employing an ensemble strategy, the model can successfully infer the shape of monomeric DNA origami structures almost in real time. Further refinement of the model in an unsupervised manner enables the analysis of supramolecular assemblies consisting of tens to hundreds of DNA blocks. The proposed model enables an automated inverse design of DNA origami structures for given target shapes. Our approach facilitates the real-time virtual prototyping of DNA origami, broadening its design space.

Pharmaceutical active compound (PhAC) residues are considered an emerging micropollutant that enters the aquatic environment and causes harmful ecotoxicity. The significant sources of PhACs in the environment include the pharmaceutical industry, hospital streams, and agricultural wastes (animal husbandry). Recent investigations demonstrated that wastewater treatment plants (WWTPs) are an important source of PhACs discharging ecosystems. Several commonly reported that PhACs are detected in a range level from ng L-1 to µg L-1 concentration in WWTP effluents. These compounds can have acute and chronic adverse impacts on natural wildlife, including flora and fauna. The approaches for PhAC removals in WWTPs include bioremediation, adsorption (e.g., biochar, chitosan, and graphene), and advanced oxidation processes (AOPs). Overall, adsorption and AOPs can effectively remove PhACs from wastewater aided by oxidizing radicals. Heterogeneous photocatalysis has also proved to be a sustainable solution. Bioremediation approaches such as membrane bioreactors (MBRs), constructed wetlands (CWs), and microalgal-based systems were applied to minimize pharmaceutical pollution. Noteworthy, applying MBRs has illustrated high removal efficiencies of up to 99%, promising prospective future. However, WWTPs should be combined with advanced solutions, e.g., AOPs/photodegradation, microalgae-bacteria consortia, etc., to treat and minimize their accumulation. More effective and novel technologies (e.g., new generation bioremediation) for PhAC degradation must be investigated and specially designed for a low-cost and full-scale. Investigating green and eco-friendly PhACs with advantages, e.g., low persistence, no bioaccumulation, less or non-toxicity, and environmentally friendly, is also necessary.

The current study aimed to assess the effectiveness of biochar formed from algae in the removal of Cr(VI) through the process of impregnating brown algae Sargassum hemiphyllum with KHCO3. The synthesis of KHCO3-activated biochar (KBAB-3), demonstrating remarkable adsorption capabilities for Cr(VI), was accomplished utilizing a mixture of brown algae and KHCO3 in a mass ratio of 1:3, followed by calcination at a temperature of 700 °C. Based on the empirical evidence, it can be observed that KBAB-3 shown a significant ability to adsorb Cr(VI) within a range of 60-160 mg g-1 across different environmental conditions. In addition, the KBAB-3 material demonstrated the advantageous characteristic of easy separation, allowing for the continued maintenance of a high efficiency in removing Cr(VI) even after undergoing numerous cycles of reuse. In conclusion, the application of KBAB-3, a novel adsorbent, exhibits considerable prospects for effective removal of Cr(VI) from diverse water sources in the near future.

BACKGROUND: The Mini-Mental State Examination (MMSE) is the most widely used standardised screener for impairments across a range of cognitive domains. However, the degree to which its domains (orientation, registration, attention, recall, language, and visuospatial) capture cognitive functioning measured using standardised neuropsychological tests is unclear. METHOD: A longitudinal research design with four biannual assessments over a 6-year period was used with an initial sample of 1037 older adults (aged above 70 years). Participants completed MMSE and neuropsychological tests at each assessment. Network analysis was utilised to investigate unique associations among the MMSE and its domains and neuropsychological test performance at each time point. RESULTS: The total MMSE and two of its domains, language and recall, were associated with neuropsychological memory performance. The MMSE orientation, registration and visuospatial domains did not have any unique associations with neuropsychological performance. No stable internal interconnections between MMSE domains were found over time. The association of total MMSE as well as its recall domain with neuropsychological memory performance remained very similar over the 6-year period. CONCLUSIONS: The present study adds evidence to the validity of the MMSE and supports the clinical usage of the MMSE, whereby the total score is used for screening patients with or without cognitive impairments, with repeated administration to monitor cognitive changes over time, to inform intervention. However, the tool is not able to diagnose the cases for changes in specific cognitive domains and as such, should not replace a complete neuropsychological assessment.

BACKGROUND: The modified Telephone Interview for Cognitive Status (TICS-M) is a widely used tool for assessing global cognitive functions and screening for cognitive impairments. The tool was conceptualised to capture various cognitive domains, but the validity of such domains has not been investigated against comprehensive neuropsychological assessments tools. Therefore, this study aimed to explore the associations between the TICS-M domains and neuropsychological domains to evaluate the validity of the TICS-M domains using network analysis. MATERIALS AND METHODS: A longitudinal research design was used with a large sample of older adults (aged above 70 years; n = 1037 at the baseline assessment) who completed the TICS-M and comprehensive neuropsychological assessments biennially. We applied network analysis to identify unique links between the TICS-M domains and neuropsychological test scores. RESULTS: At baseline, there were weak internal links between the TICS-M domains. The TICS-M memory and language domains were significantly related to their corresponding neuropsychological domains. The TICS-M attention domain had significant associations with executive function and visuospatial abilities. The TICS-M orientation domain was not significantly associated with any of the five neuropsychological domains. Despite an attrition of almost 50% at wave four, weak internal links between the TICS-M domains and most associations between TICS-M and neuropsychological domains that were found initially, remained stable at least over two waves within the 6-year period. CONCLUSIONS: This study supports the overall structural validity of the TICS-M screener in assessing enduring global cognitive function. However, separate TICS-M cognitive domains should not be considered equivalent to the analogous neuropsychological domains.

The Telephone Interview for Cognitive Status-modified (TICS-M) is a well-established and widely used screening instrument for dementia and assessment of global cognitive function in older people. This study aimed to evaluate the psychometric properties of the TICS-M and to enhance the accuracy of the instrument using Rasch methodology. Partial Credit Rasch model was applied to the TICS-M scores. The sample selected for Rasch analysis consisted of 432 participants aged 70-90 years (M = 78.85, SD = 4.73) including 195 males (237 females), and 132 (30.56%) of whom were diagnosed with dementia after the baseline assessment. Initial analysis indicated good reliability of the TICS-M assessment scores, but there were three misfitting items and local dependency issues. Combining locally dependent and misfitting items into super-items achieved the best Rasch model fit for the TICS-M. This modification improved reliability of the assessment scores and resulted in no misfitting items, no local dependency, strict unidimensionality, and invariance across individual factors such as participants age, sex, diagnosis, and in-person neuropsychological assessment scores. Satisfying Rasch model expectations allowed for creation of a transformation table to convert raw TICS-M scores into interval-level data, which improves precision of the instrument. In summary, the TICS-M assessment scores demonstrated excellent reliability as reflected by Person Separation Index (PSI = 0.86) and met expectations of the unidimensional Rasch model after minor adjustments. The ordinal-to-interval transformation table can be used to increase accuracy of the TICS-M without altering its current format. These findings contribute to more accurate assessments of cognitive decline in older people and screening for conditions such as dementia. INTRODUCTION AND IMPORTANCE: Madelung disease is a rare condition of unknown etiology, characterized by large masses of subcutaneous fat in the upper body bilaterally. It rarely affects the lower extremities and genital region. CASE PRESENTATION: Here, we report a patient with Donhouser's type III Madelung's disease. A 47-year-old male patient presented with a giant fatty scrotal tumor that caused deformation of the scrotum and penis, made it difficult to perform daily activities, and hindered sexual activity. The adipose tumor was completely removed using a midline scrotal incision. The scrotum was reconstructed with bilateral anterior and posterior scrotal skin flaps. The excess skin was cut into a wedge shape between the anterior and posterior scrotal regions. CLINICAL DISCUSSION: At 3 months postoperatively, the scrotum was normal in shape and size, and the patient was able to perform personal activities and normal sexual activity. The surgical options, lipectomy results, and experiences drawn from the clinical cases have been discussed. CONCLUSION: Giant scrotal lipomas are very rare in Madelung's disease. Lipectomy and scrotal reconstruction are required. Wedge-shaped scrotal skin excision in the midsection on each side of the scrotum removes excess skin, which could restore the shape and function of the penis and scrotum.

BACKGROUND: The 16-item Informant Questionnaire on Cognitive Decline in the Elderly (IQCODE-16) is a well-validated and widely-used measure of cognitive changes (CCs) among older adults. This study aimed to use Rasch methodology to establish psychometric properties of the IQCODE-16 and validate the existing ordinal-to-interval transformation algorithms across multiple large samples. METHODS: A Partial Credit Rasch model was employed to examine psychometric properties of the IQCODE-16 using data (n = 918) from two longitudinal studies of participants aged 57-99 years: the Older Australian Twins Study (n = 450) and the Canberra Longitudinal Study (n = 468), and reusing the Sydney Memory and Ageing Study (MAS) sample (n = 400). RESULTS: Initial analyses indicated good reliability for the IQCODE-16 (Person Separation Index range: 0.82-0.90). However, local dependency was identified between items, with several items showing misfit to the model. Replicating the existing Rasch solution could not reproduce the best Rasch model fit for all samples. Combining locally dependent items into three testlets resolved all misfit and local dependency issues and resulted in the best Rasch model fit for all samples with evidence of unidimensionality, strong reliability, and invariance across person factors. Accordingly, new ordinal-to-interval transformation algorithms were produced to convert the IQCODE-16 ordinal scores into interval data to improve the accuracy of its scores. CONCLUSIONS: The findings of this study support the reliability and validity of the IQCODE-16 in measuring CCs among older adults. New ordinal-to-interval conversion tables generated using samples from multiple independent datasets are more generalizable and can be used to enhance the precision of the IQCODE-16 without changing its original format. An easy-to-use converter has been made available for clinical and research use.

BACKGROUND Amyloid light-chain (AL) amyloidosis is usually due to deposition of immunoglobulin lambda light chains from plasma cells in patients with multiple myeloma. AL amyloid may involve the salivary glands, gastrointestinal tract, peripheral nerves, and skin. However, musculoskeletal amyloid and amyloid arthropathy are rare. This report is of a woman with bilateral upper limb musculoskeletal amyloid and amyloid arthropathy associated with multiple myeloma, initially diagnosed and managed as a case of rheumatoid arthritis. CASE REPORT A 59-year-old woman who was initially diagnosed with rheumatoid arthritis presented with bilateral polyarthritis in the upper limbs. Despite treatment with corticosteroids, methotrexate, and hydroxychloroquine, her symptoms did not improve. After 4 months, she revisited our hospital with the appearance of swollen soft tissue in the upper right arm and numbness of the right hand. She had an arthroscopic synovectomy of the right shoulder joint, and the mass in the right elbow area was removed. These specimens were positive by Congo red stain and confirmed the deposition of light chain protein as amyloid. She was diagnosed with multiple myeloma according to International Myeloma Working Group criteria, including bone marrow plasma cells more the 10%, lytic lesions in bone, and anemia. CONCLUSIONS This report highlights the importance of imaging, biopsy, and laboratory investigations in patients with arthropathy and musculoskeletal disease. In this case, the patient was seronegative for rheumatoid arthritis, and the presentation with very thick and nodular synovium supported an alternative diagnosis. The identification of musculoskeletal amyloid and amyloid arthropathy confirmed underlying multiple myeloma.

BACKGROUND: RNA-based genomic risk assessment estimates chemotherapy benefit in patients with hormone-receptor positive (HR+)/Human Epidermal Growth Factor 2-negative (ERBB2-) breast cancer (BC). It is virtually used in all patients with early HR+/ERBB2- BC regardless of clinical recurrence risk. PATIENTS AND METHODS: We conducted a retrospective chart review of adult patients with early-stage (T1-3; N0; M0) HR+/ERBB2- BC who underwent genomic testing using the Oncotype DX (Exact Sciences) 21-genes assay. Clinicopathologic features were collected to assess the clinical recurrence risk, in terms of clinical risk score (CRS) and using a composite risk score of distant recurrence Regan Risk Score (RRS). CRS and RRS were compared to the genomic risk of recurrence (GRS). RESULTS: Between January 2015 and December 2020, 517 patients with early-stage disease underwent genomic testing, and clinical data was available for 501 of them. There was statistically significant concordance between the 3 prognostication methods (P < 0.01). Within patients with low CRS (n = 349), 9.17% had a high GRS, compared to 8.93% in patients with low RRS (n = 280). In patients with grade 1 histology (n = 130), 3.85% had a high GRS and 68.46% had tumors > 1 cm, of whom only 4.49% had a high GRS. Tumor size > 1cm did not associate with a high GRS. CONCLUSION: Genomic testing for patients with grade 1 tumors may be safely omitted, irrespective of size. Our finds call for a better understanding of the need for routine genomic testing in patients with low grade/low clinical risk of recurrence.

Capacitive deionization (CDI) has been considered as an efficient, energy-saving and environmental friendly technology for water treatment. For the practical application of CDI, high-performance electrode materials beyond standard activated carbon should be developed. In this study, biochar derived from brown algae Sargassum hemiphyllum prepared by pyrolysis at 300-700 °C and then used as the CDI electrode to remove Cu(II) from aqueous solutions. According to the findings, the optimal pyrolysis temperature was 700 °C, and the electrosorption capacity of BAB700 was 75-120 mg·g-1 at an applied voltage of 1.2 V across wide range of initial pH, temperatures and ion types. Moreover, BAB700 also exhibited outstanding ability to electrosorb other heavy metals (Zn(II), Ni(II), and Cd(II)). In addition, the BAB700 retained the Cu(II) removal efficiency of 70 % in 10 cycles. Cu(II) in actual water is completely eliminated with great reproducibility, resulting in a high degree of applicability for water treatment.

From the CHCl3-soluble extract of Annona muricata L. (Annonaceae) leaves, one new 3-benzazepine-type alkaloid, anonazepine (1), and four known aporphine-type alkaloids, (+)-laurotetanine (2), (+)-norglaucine (3), (-)-xylopine (4), and lanuginosine (5), were isolated. Except for (-)-xylopine (4), these remaining known alkaloids were first reported in A. muricata. The structures of the isolated alkaloids were established by 1D and 2D NMR spectroscopy and MS, as well as comparison with literature data. The new 3-benzazepine-type alkaloid existed in an inseparable mixture of two equilibrium conformers. Its absolute configuration was determined based on comparing their experimental and calculated ECD data. The anti-inflammatory activity of the isolated alkaloids was investigated, but none of the alkaloids showed a significant result.

In this study, the brown algae Sargassum Hemiphyllum was used as a carbon source for synthesis of magnetic porous biochar via pyrolyzing at high temperature and and doping iron oxide particles (Fe-BAB). Cu (II) species were removed from aqueous solutions using Fe-BAB under various conditions. Fe-BAB demonstrated superior Cu (II) adsorption (105.3 mg g-1) compared to other biochars. On the surface of Fe-BAB, there are several oxygen-containing functional groups, such as -COOH and -OH, which are likely responsible for the excellent heavy metal removal performance. By utilizing magnet, the Fe-BAB can be conveniently separated from the solution and ready for further usage. Multi-adsorption mechanisms were responsible for Cu adsorption on Fe-BAB. Using the magnetic algal biochar for heavy metal removal is feasible due to its high adsorption efficiency and simplicity of separation.

Objectives: Evaluating comprehensive reliability of the Vietnamese Self-Compassion Scale (VSCS) and its ability to distinguish between trait (stable) vs state (dynamic) aspects of self-compassion using Generalisability Theory (G-Theory) is necessary. This investigation contributes to both reliability and validity of research that uses the VSCS to measure self-compassion in Vietnamese adults. Methods: In a sample of 155 Vietnamese adults who completed the VSCS at three occasions that were each 2 weeks apart, a G-study was conducted to measure reliability and trait vs state aspects of each VSCS subscale and the short-form VSCS, and a D-study was conducted to examine the effects of removing subscales on overall scale reliability as well as evaluate trait vs state aspects of each item. Results: With G-coefficients of 0.93-0.98, both the complete and short-form VSCS (VSCS-SF) demonstrated excellent reliability in measuring trait self-compassion. Three of the six subscales-self-judgement, mindfulness, and kindness-also demonstrated excellent reliability, with G-coefficients of 0.82-0.85. Eighteen of the 26 items measured trait more than state. The remaining eight items reflected a mixture of trait and state, but this did not affect overall reliability. Conclusions: This study indicated that the VSCS, VSCS-SF, and three VSCS subscales reliably measured trait self-compassion, with scores generalisable across the Vietnamese population and occasions. Thus, overall self-compassion levels remained stable over time, which is useful for evaluating the effectiveness of an intervention because significant changes of self-compassion are likely to be long-lasting. BACKGROUND: Liquid biopsy testing offers a significant potential in selecting signal-matched therapies for advanced solid malignancies. The feasibility of liquid biopsy testing in a community-based oncology practice, and its actual impact on selecting signal-matched therapies, and subsequent survival effects have not previously been reported. PATIENTS AND METHODS: A retrospective chart review was conducted on adult patients with advanced solid cancer tested with a liquid-biopsy assay between December 2018 and 2019, in a community oncology practice. The impact of testing on treatment assignment and survival was assessed at 1-year follow-up. RESULTS: A total of 178 patients underwent testing. A positive test was reported in 140/178 patients (78.7%), of whom 75% had an actionable mutation. The actual overall signal-based matching rate was 17.8%. While 85.7% of patients with no actionable mutation had a signal-based clinical trial opportunity, only 10% were referred to a trial. Survival analysis of lung, breast, and colorectal cancer patients with actionable mutations who received any therapy (n = 66) revealed a survival advantage for target-matched (n = 22) compared to unmatched therapy (n = 44): patients who received matched therapy had significantly longer progression-free survival (PFS) (mPFS: 12 months; 95%CI, 10.6-13.4 vs. 5.0 months; 95%CI, 3.4-6.6; P = .029), with a tendency towards longer overall survival (OS) (mOS: 15 months; 95%CI, 13.5-16.5 vs. 13 months; 95%CI: 11.3-14.7; P = .087). CONCLUSIONS: Implementation of liquid biopsy testing is feasible in a US community practice and impacts therapeutic choices in patients with advanced malignancies. Receipt of liquid biopsy-generated signal-matched therapies conferred added survival benefits.

Biochars derived from three species of algae was synthesized by impregnating the green algae Ulva Ohnoi, red algae Agardhiella subulata, and brown algae Sargassum hemiphyllum with ZnCl2 chemical activator and employed as a long-term adsorbent for ciprofloxacin (CIP) removal from water. The results revealed that combination of brown algae and ZnCl2 chemical activator (ZBAB) successfully produced mesoporous biochar with excellent physicochemical characteristics and gave the best CIP adsorption capacity. The ZBAB yielded a high CIP adsorption capacity (190-300 mg g-1) under various parameter effects (initial pH, temperature and major ions). Throughought the surface characterization techniques, the proposed adsorption mechanisms were electrostatic interaction, π-π EDA interaction, pore filling and hydrogen bonding. Moreover, not only algal biochars exhibited innovative and potential adsorbent for rapid and effective remediate pollution from water, but combination of algal biomass and ZnCl2 activator also created renewable source of energy from biomass pyrolysis.

BACKGROUND: A major issue in evaluating the cognitive status of ageing populations is a clear distinction between enduring and dynamic aspects of global cognition necessary for evaluating risks of dementia and effectiveness of preventive interventions. MATERIALS AND METHODS: Generalizability Theory was applied to investigate dynamic and enduring aspects of global cognition using longitudinal data over 10 years of follow-up. Measures included the Mini-Mental Status Examination (MMSE) and the Telephone Interview for Cognitive Status-modified (TICS-M). The sample (N = 238) included 154 females, mean age = 76.54 years, SD = 3.94 from the Sydney Memory and Ageing Study. RESULTS: The MMSE measured dynamic and enduring aspects of cognition to a comparable degree with 56% of variance explained by enduring aspects and 44% by dynamic aspects and showed low sensitivity/high specificity in detecting dementia. A shortened version of the MMSE (MMSE-D8) better captured dynamic aspects of cognition after removing three items less sensitive to change. The TICS-M predominantly measured enduring aspects of cognition (72%) with the remaining 28% due to dynamic aspects and displayed high sensitivity/high specificity for dementia screening. CONCLUSIONS: The MMSE measures both dynamic and enduring cognitive aspects and is suitable for general clinical assessments, while the MMSE-D8 can be used to monitor transitory changes of global cognition over time. The TICS-M is more useful for measuring enduring features of cognition and screening for dementia. Our findings highlight the value of generalizability theory to distinguish dynamic and enduring features of cognition, which may contribute to preventive interventions and monitoring cognitive ability over time.
