RESUMEN

The quantitative measurement of urinary biomarkers in wastewater has emerged as a robust tool for estimating alcohol and tobacco consumption in populations. In this study, we applied the wastewater-based epidemiology (WBE) approach to compare alcohol and tobacco use between university students and urban inhabitants in Ho Chi Minh City, Vietnam. Ethyl sulfate and cotinine serve as markers for alcohol and tobacco use, respectively. Our findings reveal that urban inhabitants aged 15 and above consume 1.56 ± 0.23 mL of pure ethanol and 2.8 ± 0.33 mg of nicotine per day, while university students consume 0.69 ± 0.13 mL of pure alcohol and 1.2 ± 0.2 mg of nicotine per day. This indicates that, on average, students consume less alcohol and tobacco compared with urban adults. A Monte Carlo simulation indicated that, on average, university students in our study smoke 1.5 cigarettes per day, while urban residents aged 15 and above smoke 4.3 cigarettes per day. Considering the smoking prevalence, a student smoker in this study consumes 6.5 cigarettes per day, a level high enough to establish addiction. On the other hand, alcohol use estimation is significantly lower than previous survey-based reports, likely due to degradation within on-site septic tanks. Future research should aim to extend the sampling period to capture seasonal variations and improve the understanding of tobacco and alcohol consumption patterns. The results from this study are crucial for decision-makers in Ho Chi Minh City to develop effective public health strategies and interventions. PRACTITIONER POINTS: Wastewater-based approach is applicable to estimate the tobacco consumption in Ho Chi Minh City. Each current smoker in the urban area of Ho Chi Minh City smokes nearly a package a day. The estimated consumption for student smokers in U-town is 6.5 cigarettes per day, a level high enough to establish addiction. The existence of septic tanks within Vietnam's drainage systems prevents reliable estimation of alcohol consumption for the entire population.

INTRODUCTION: Left ventricular dysfunction is quite common in septic shock. Speckle-tracking echocardiography (STE) is a novel, highly sensitive method for assessing left ventricular function, capable of detecting subclinical myocardial dysfunction, which is not identified with conventional echocardiography. We sought to evaluate subclinical left ventricular systolic function in patients with septic shock using speckle-tracking echocardiography. METHODS: From May 2017 to December 2018, patients aged ≥18 years admitted to the intensive care unit with the diagnosis of sepsis and septic shock based on the sepsis-3 definition were included. Patients with other causes of cardiac dysfunction were excluded. Transthoracic echocardiography was performed for all the patients within 24 hours of diagnosis. Left ventricular systolic function was assessed using conventional echocardiography and speckle-tracking echocardiography. RESULTS: Patients with septic shock (n = 90) (study group) and 37 matched patients with sepsis but no septic shock (control group) were included. Left ventricular ejection fraction (LVEF) by conventional echocardiography showed no significant difference between two groups (58.2 ± 9.9 vs. 58.6 ± 8.3, p=0.804). The global longitudinal strain (GLS) by STE was significantly reduced in patients with septic shock compared with that in the control (-14.6 ± 3.3 vs. -17.1 ± 3.3, p < 0.001). Based on the cutoff value of GLS ≥ -15% for the definition of subclinical left ventricular systolic dysfunction, this dysfunction was detected in 50 patients with septic shock (55.6%) and in 6 patients in the control group (16.2%) (p < 0.05). CONCLUSIONS: Speckle-tracking echocardiography can detect early subclinical left ventricular systolic dysfunction via the left ventricular global longitudinal strain compared with conventional echocardiographic parameters in patients with septic shock.

RESUMEN

A new alkenylphenol, bavinol A (1), together with six known compounds (2-7) were isolated from the aerial parts of Piper bavinum (Piperaceae). The chemical structures of these compounds were determined by spectroscopic analyses including 2D NMR spectroscopy. The anti-Alzheimer effects of compounds 1-7 were evaluated from acetylcholinesterase (AChE) and butyrylcholinesterase (BChE) inhibitory activity assays. Bavinol A (1), ampelopsin (3), and violanthin (4) exhibited AChE inhibitory activities with IC50 values of 29.80, 59.47 and 79.80 µM. Compound 1 also showed the most potent BChE inhibitory activity with an IC50 value of 19.25 µM.

Accumulated clinical studies have demonstrated that histone deacetylase (HDAC) inhibitors show great potential for the treatment of cancer. SAHA (Vorinostat, trade name Zolinza) was approved by the FDA in 2006 for the treatment of the cutaneous manifestations of cutaneous T-cell lymphoma. As a continuity of our ongoing effort to identify novel small molecules targeting these important enzymes, we designed and synthesized two series of isatin-3'-oxime- and isatin-3'-methoxime-based hydroxamic acids (3a-g and 6a-g) as analogues of SAHA. Generally in both series it was found that, compounds bearing no substituent or with 5'-F, 5'-Cl, 7'-Cl substitutents on the isatin moiety exhibited good inhibition against histone-H3 and histone-H4 deacetylation at the concentrations of 1 µM, as evaluated by Western Blot assay. The compounds also displayed potent cytotoxicity against five cancer cell lines with IC50 values of as low as 0.08 µM, more than 45-fold lower than that of SAHA. Docking study performed with selected compounds 3a and 6a revealed that these compounds bound to HDAC8 with higher affinities compared to SAHA. Compounds 3a and 6a also bound to HDAC2 at the binding site with high binding affinity. These findings should encourage further elaboration with the isatin moiety to produce more potent HDAC inhibitors with potential anticancer activity.

RESUMEN

Falls in older adults are a serious problem for individuals, their families, and the health care system. This article describes research regarding fall risk assessment, risk reduction interventions, and public policy aimed at reducing the risk of falls for older adults in home settings. Assessments for frail older adults should include observations of not only the physical environment, but also the interactions among the environment, behavior, and physical functioning so that interventions are tailored to the specific situation of the individual. Home modification and technology can prove useful when designing interventions aimed at reducing fall risks. Problems such as cost, reluctance to adopt or implement suggestions, and a lack of knowledge may present barriers to effective home modification. Program and policy options for the future include improved training for service personnel who visit the homes of older adults, increased awareness of and coordination between programs or interventions aimed at reducing the risk of falls in older adults, new sources of funding, and building more housing that follows the principles of universal design.

Background: \r\n', u'Japanese encephalitis virus (JE) is the most prevalent in Asia, Pacific and in mainland northern Australia; and considered to be the leading cause to the acute encephalitis. The case- fatality and sequela rates in children stay rather high. There were some medical technologies for the JE diagnosis, of which is the application of the expensive MAC- ELISA and PEN TAX kits.\r\n', u'Objectives: \r\n', u'To evaluate and compare the light sensitivity of MAC- ELISA and PENTAX Kits in JE diagnosis.\r\n', u'Subjects and method: \r\n', u'48 pairs of sera samples obtained from the patients with clinically manifested Japanese encephalitis (JE) in 2003- 2005, in some northern provinces, they were applied and tested into MAC- ELISA Kits.\r\n', u'Results:\r\n', u'The sensitivity of MAC- ELISA and PENTAX kits detecting IgM against JE virus were 95.71%, and 98.57%, respectively.
