RESUMEN

Calcium is an element that performs many important functions in the human body. A study was conducted on the use of a pulsed electric field (PEF) to enrich cells of Lactobacillus rhamnosus B 442 in calcium ions. The highest concentration of calcium ions in bacterial cells (7.30 mg/g d.m.) was obtained at ion concentration of 200 µg/ml of medium and with the use of the following PEF parameters: field strength 3.0 kV/cm, exposure time 10 min, pulse width 75 ms and 20 h of culturing after which bacteria were treated with the field. Cell biomass varied in the range from 0.09 g/g d.m. to 0.252 g/g d.m., and the total number of bacteria ranged from 1010 CFU/ml to 1012 CFU/ml. Microscope photographs prove that calcium ions were situated within the cells of the bacteria, and electroporation contributed to an increase in the effectiveness of the ion bioaccumulation process. Samples containing calcium and subjected to electroporation displayed intensive fluorescence. The significance of this research was the possibility of using probiotic bacteria enriched with calcium ions for the production of functional food in subsequent studies.

The following strains of bacteria: Lactobacillus rhamnosus B 442, Lactobacillus rhamnosus 1937, and Lactococcus lactis JBB 500 were enriched with magnesium ions using Pulsed Electric Fields. The potentially probiotic strains were added to the mixture in the DVS process and applied for the production of ice cream which were then analyzed physicochemically and microbiologically. Results showed that addition of bacteria enriched with magnesium did not change chemical parameters of the ice cream and did not affect the freezing process, meltability, and hardness. No significant differences were noted in colour of the samples. The ice cream with addition of bacteria enriched with magnesium had higher adhesiveness. The results of viability determination showed that the total number of microorganisms in the ice cream was higher than in the starter cultures. Viability of the bacteria enriched with magnesium in the obtained ice cream was lower in comparison to the control samples.

The aim of this study was to determine the effect of pulsed electric fields (PEF) on accumulation of magnesium ions in Lactobacillus rhamnosus B 442 cells. Under optimized conditions, this is, on 15 min exposure of the 20 h grown culture to PEF of the 2.0 kV/cm and 20 µs pulse width at concentration 400 µg Mg2+/mL medium, accumulation of magnesium in the biomass reached maximum 4.28 mg/g d.m. Optimization of PEF parameters caused an increase of magnesium concentration in the cells by 220% in comparison to the control not treated with PEF. Bacterial cell biomass enriched with Mg2+ may be an alternative for pharmacological supplementation applied in deficiency of this cation.

Cardiopulmonary resuscitation (CPR) is relatively novel branch of medical science, however first descriptions of mouth-to-mouth ventilation are to be found in the Bible and literature is full of descriptions of different resuscitation methods - from flagellation and ventilation with bellows through hanging the victims upside down and compressing the chest in order to stimulate ventilation to rectal fumigation with tobacco smoke. The modern history of CPR starts with Kouwenhoven et al. who in 1960 published a paper regarding heart massage through chest compressions. Shortly after that in 1961Peter Safar presented a paradigm promoting opening the airway, performing rescue breaths and chest compressions. First CPR guidelines were published in 1966. Since that time guidelines were modified and improved numerously by two leading world expert organizations ERC (European Resuscitation Council) and AHA (American Heart Association) and published in a new version every 5 years. Currently 2010 guidelines should be obliged. In this paper authors made an attempt to present history of development of resuscitation techniques and methods and assess the influence of previous lifesaving methods on nowadays technologies, equipment and guidelines which allow to help those women and men whose life is in danger due to sudden cardiac arrest.

Usually paraneoplastic symptoms precede actual diagnosis of a neoplasm, however, may also be concurrent with it. They do not result from the localization of the primary lesion nor are associated with metastases or with other effects of presence and treatment of the neoplasm. The immunological etiology is assumed. In diagnostics estimation of level of the antionconeuronal antibodies may be helpful. Mental disorders in the course of paraneoplastic syndrome may coexist with neoplasm, but also may precede it (combined with neurological signs or without them). Such mental disorders have diverse clinical pictures and often are characterized by atypical and heavy course. Observed psychopathological symptoms do not form any specific diagnostic profile delineated in ICD 10 and encompass disturbances of consciousness, perception and thinking, as well as affective states. Although psychiatric intervention in paraneoplastic syndromes with psychopathological symptoms is merely symptomatic, one can still expect atypical response. Suspected paraneoplastic syndrome in patients not yet diagnosed as having neoplasm should persuade one to initiate a screening for potential focal change and to introduce targeted treatment should tumor growth be confirmed.

Dopa-responsive dystonia (DRD) is a rare, autosomal dominant (GTP-cyclohydroxylase gen mutation on chromosome 14q) or rarely recessive (tyrosine hydroxylase gene mutation on chromosome 11p) inherited disorder. Both enzymes take part in dopamine synthesis. Their deficiencies cause the dopamine level reduction. The first clinical symptoms occur in the childhood. The authors present the case of a 24-year-old woman in whom the lower limb dystonia occurred when she was five. Then the trunk and upper limbs dystonia appeared with skeleton deformities leading to deep disability. DRD was recognized when she was 19 and L-Dopa was administered. The clinical status improved, dystonic movements disappeared completely. The authors indicate that DRD must be taken into account in differential diagnosis of movement disorders in children, mostly in progressive cases. Good effect of L-Dopa treatment is one of the basic phenomena helpful in DRD recognition.

The authors presented the clinical and anatomopathological description of Arnold-Chiari anomaly at 25 years old man, which caused cerebellar manifestation and sudden death. At the base of anatomopathological study the first type of Arnold-Chiari anomaly with dislocation of cerebellum tonsils into foramen magnum was recognized.

RESUMEN

Cortico-basal degeneration is a rare degenerative disease connected with Tau protein pathology. Epidemiology of cortico-basal degeneration is unknown. The authors present a case of 59 years old woman with suspicion of cortico-basal degeneration. The extrapyramidal symptoms mainly on the right side with "alien limb phenomenon" and dystonia of lower limb is observed in our patient. Cortico-subcortical brain atrophy was present in MRI scans. EEG was asymmetrical. No improvement was noticed after L-Dopa. Treatment of amantidine caused the transient improvement.

RESUMEN

Oromandibular dystonic movements may present a type of focal or segmental dystonia. They often cause the malnutrition problems and social isolation. The aim of the study was to evaluate the efficacy of Botulinum Toxin type A treatment in patients with oromandibular dystonia with estimation of nutrition status. 15 patients with oromandibular dystonia, 9 women and 6 men, mean age 56 years old, were investigated. In all patients Botulinum Toxin type A was administrated intramuscularly under the EMG control. In 14 patients the improvement was significant and in 11 increase body mass was observed within 6 month after injections. Botulinum Toxin type A is useful in treatment of oromandibular dystonic movements in patients with malnutrition.

Hiccup is defined as involuntary, paroxysmal inspiratory movements of the thoracic wall, together with contraction of the diaphragm and additional respiratory muscles. The clinical problem is connected mainly with pathological chronic hiccup. The authors discuss pathophysiology of hiccup and diagnostic possibilities, as well as various therapeutical options.
