BACKGROUND: It is well known that patients with ST-elevation myocardial infarction (STEMI) show both systolic and diastolic left ventricular dysfunction. The aim of this study was to assess post-myocardial infarction diastolic dysfunction using left atrial ejection force (LAEF) in patients treated with primary percutaneous coronary intervention (PCI). METHODS AND RESULTS: We enrolled 58 patients presenting with STEMI who were treated with primary PCI and 23 healthy subjects as a control group. A detailed transthoracic echocardiogram, including mitral flow velocities, tissue Doppler mitral annular velocities, and left atrial (LA) phasic volumes, was performed in both groups. We also measured the level of B-type natriuretic peptide (BNP). LAEF was calculated using the formula: 0.5 × P × Mitral orifice area × (Peak A velocity)(2) . Correlations between variables were studied using "Pearson and Spearman's rho" test. In the test group, we found that the level of BNP in the plasma, E/E' ratio, and the LA volume measurements were higher than that of the control group, and those differences were statistically significant. LAEF was increased in patients with myocardial infarction (MI); moderately correlated to BNP (r = 0.383 and P = 0.001) and E/E' (r = 0.473 and P = 0.001), and strongly correlated to A-wave velocity (r = 0.731 and P = 0.001). LAEF was also negatively correlated to E/A ratio (r = -0.419 and P = 0.001) and LVEF (r = -0.339 and P = 0.003). CONCLUSION: Impaired diastolic function in STEMI affects LA and increased LAEF is one of its manifestations. LAEF may also have diagnostic importance in diastolic dysfunction, but these findings should be confirmed by further studies.

An adequate supply of dissolved oxygen is important in natural rivers and in some water treatment processes. The dissolved oxygen concentration can be enhanced by entraining air bubbles in a receiving pool. When a water jet impinges a receiving pool at rest, air bubbles may be entrained and carried away below the pool free surface. This process is called plunging water jet entrainment and aeration. This paper describes an experimental study of the air entrainment rate and oxygen transfer efficiency of circular nozzles with and without air holes. In particular, the effect of varying the number, positions, and open/close status of the air holes is investigated. A negative pressure occurred depending on the air holes opened on the circular nozzles. This phenomenon affected the water jet expansion, water jet shape, air entrainment, and bubble penetration depth and, hence, the oxygen transfer efficiency. It was demonstrated that the air entrainment rate and the oxygen transfer efficiency of the circular nozzles with air holes were better than those of the circular nozzles without air holes. Therefore, adding air holes to a simple, circular nozzle could lead to a significantly increased air entrainment rate and oxygen transfer efficiency.

Mutations in genes encoding gap- and tight-junction proteins have been shown to cause distinct forms of hearing loss. We have now determined the GJB2[connexin 26 (Cx26)] mutation spectrum in 60 index patients from mostly large Turkish families with autosomal-recessive inherited non-syndromic sensorineural hearing loss (NSSHL). GJB2 mutations were found in 31.7% of the families, and the GJB2-35delG mutation accounted for 73.6% of all GJB2 mutations. The carrier frequency of GJB2-35delG in the normal Turkish population was found to be 1.17% (five in 429). In addition to the described W24X, 233delC, 120delE and R127H mutations, we also identified a novel mutation, Q80R, in the GJB2 gene. Interestingly, the Q80R allele was inherited on the same haplotype as V27I and E114G polymorphisms. As little is known about the mutation frequencies of most other recently identified gap- and tight-junction genes as a cause for hearing loss, we further screened our patients for mutations in GJB3 (Cx31), GJA1 (Cx43), DeltaGJB6-D13S1830 (Cx30) and the gene encoding the tight-junction protein, claudin 14 (CLDN14). Several novel polymorphisms, but no disease-associated mutations, were identified in the CLND14 and GJA1 genes, and we were unable to detect the DeltaGJB6-D13S1830 deletion. A novel putative mutation, P223T, was found in the GJB3 gene in heterozygous form in a family with two affected children. Our data shows that the frequency of GJB2 mutations in Turkish patients with autosomal-recessive NSSHL and the carrier rate of the GJB2-35delG mutation in the Turkish population, is much lower than described for other Mediterranean countries. Furthermore, mutations in other gap- and tight-junction proteins are not a frequent cause of hearing loss in Turkey.

Dominant mutations in the GJB2 gene encoding connexin 26 (Cx26) can cause non-syndromic hearing impairment alone or in association with palmoplantar keratoderma (PPK). We have identified the novel G224A (R75Q) mutation in the GJB2 gene in a four-generation family from Turkey with autosomal dominant inherited hearing impairment and PPK. The age of onset and progression of hearing loss were found to be variable among affected family members, but all of them had more severe impairment at higher hearing frequencies. Interestingly, the novel R75Q mutation affects the same amino acid residue as described recently in a small family (R75W) with profound prelingual hearing loss and PPK. However, the R75W mutation was also observed in a control individual without PPK and unknown hearing status. Therefore, the nature of the R75W mutation remains ambiguous. Our molecular findings provide further evidence for the importance of the conserved R75 in Cx26 for the physiological function of the inner ear and the epidermal cells of the skin.

RESUMEN

Earlobe reconstruction following traumatic damage is a challenge for most of the plastic and reconstructive surgeons. Several methods developed for reconstruction of this deformity have shown to bear various advantages and disadvantages. Our modified Gavello's procedure, without using skin grafting in resurfacing the flap donor site, has shown to be satisfactory. This paper describes our modified operative procedure as well as a case resulting in a good looking earlobe and fine scars in flap donor site.

RESUMEN

Malignant Triton tumor (or malignant schwannoma with rhabdomyoblastic differentiation) is a rare entity with a very poor prognosis. This is the second report of this type of rare and aggressive sarcoma arising in the maxilla. The clinical course, therapeutic approach, and histopathological aspects of the case are presented.

The distal fasciocutaneous portion of the lateral thigh is supplied by the direct cutaneous branch of the lateral descending branch of the lateral circumflex femoral artery and the third perforating artery of the deep femoral artery. This consistent vascular anatomy allows raising a distal skin island based on both arteries on the lateral thigh, and this flap can be advanced into a trochanteric defect according to the V-Y technique. Based on anatomic and clinical study, a new design has been developed of the tensor fasciae latae fasciocutaneous flap, which is supplied by two pedicles. Seven island V-Y tensor fasciae latae fasciocutaneous flaps have been used for the coverage of trochanteric pressure sores in six patients. It is believed that the island V-Y tensor fasciae latae fasciocutaneous flap could overcome the traditional drawbacks of the conventional tensor fasciae latae flap and its modifications, and this is an ideal flap for covering trochanteric pressure sores without any donor site deformity and morbidity, which greatly improves the aesthetic result.

The effects of an intravenous infusion of L-arginine as a physiological precursor of endothelium-derived relaxing factor/nitric oxide (EDRF/NO), iloprost (a stable prostacyclin (PGI2) analogue), and L-arginine combined with iloprost on skin viability were studied in 9 x 3 cm random pattern skin flaps in rats. Intravenous infusion of all drugs was started at the beginning of the operation and continued for 60 minutes. At the end of infusion period the depth of fluorescein dye penetration in the skin flap was assessed visually from photographic records, and the flap survival area was measured by the grid method at the seventh postoperative day. There was a significant reduction in distal necrosis of random skin flaps after intravenous infusion of L-arginine, iloprost, and L-arginine combined with iloprost (p < 0.01). Possible mechanisms that may be responsible for impairment of endothelium-dependent vasodilation and vasospasm in the microvasculature of random skin flap and their prevention with L-arginine and iloprost include restoration of the depleted stores of NO which in turn causes vasodilatation and has an antithrombotic effect.

RESUMEN

The additional use of the palmaris longus tendon is presented with the composite radial forearm-palmaris longus free flap, as useful for improving the final cosmetic appearance. This flap has the additional advantage of avoiding gravitational descent, which is a common problem encountered with many free flaps in head and reconstructions. A clinical case is also presented as an example of this approach.

Congenital absence of a vagina is a rare but outstanding anomaly of plastic surgery. Although many methods are described for reconstruction of vaginal agenesis, there is not a method yet to be approved as a perfect solution to this problem. With the aim of solving the problems faced with conventional methods, free jejunal transplantation was planned to construct a neovagina. For this method, we isolated a 15-cm jejunal segment, with its pedicle, transferred it to the preprepared vaginal pouch, and anastomosed the donor vessels of the mesentery to the inferior epigastric vessels. In the postoperative sixth month, problems of intercourse or the need to use any lubricator or stent were still not experienced and reported. In this paper, we judge and compare our approach by reviewing others.

RESUMEN

A new surgical approach has been used in a case of genital lymphedema. After resection of the lymphedematous mass, U-shaped flaps were made from the suprapubic region anteriorly and the posterior scrotal skin posteriorly. The denuded penis was transposed to its original place by passing it through a buttonhole incision made on the anterior flap. The testicles were placed and fixed in pouches prepared between the anterior and posterior flaps. The patient had an acceptable postoperative outcome both in testicular function and habitual sexual activities.
