INTRODUCCION:
La OMS estimó en el 2008 9.4 millones de casos nuevos de tuberculosis en el mundo, el Perú posee una tasa tres veces mayor al promedio mundial y los estudiantes de medicina tienen de 2 a 50 veces más probabilidades de adquirir la infección los hospitales.OBJETIVOS:
Determinar la incidencia y factores relacionados con la conversión de la prueba de tuberculina (PPD) en internos de medicina.DISEÑO:
Estudio de tipo analítico, observacional, cohorte retrospectiva. LUGAR Hospital Nacional Arzobispo Loayza (HNAL), hospital docente para la UNMSM. PARTICIPANTES Se estudió a 34 internos de medicina del HNAL de la UNMSM. Intervenciones Se administró pruebas de PPD al inicio y final de internado médico y se buscó asociación con variables sociodemográficas y de práctica hospitalarias.RESULTADOS:
Se obtuvo una prevalencia de 26.5 por ciento y una incidencia de 23.8 por 100 personas - año. Se determinó que hay un menor riesgo por no estar expuesto a pacientes con TBC [RR=0.24 (0.13 - 0.44)] y menos riesgo respecto a uso de mascarillas en pacientes con TBC [RR=0.75 (0.5 - 0.96)], pero sin asociación significativa. El resto de variables no fueron estadísticamente significativas.CONCLUSIONES:
Se obtuvo una alta prevalencia e incidencia de conversión del PPD en internos de medicina. El encontrar un menor riesgo respecto a medidas protectoras indica la importancia de fomentar la bioseguridad en el ámbito hospitalario.ABSTRACT

WHO estimated 9.4 million new cases of tuberculosis in the world in 2008. Peru has a rate of three times the world average and medical students are 2 to 50 times more likely to acquire the infection in hospitals.OBJECTIVES:
To determine the incidence and factors related to the conversion of the tuberculin test (PPD) in internal students of the medicine service.DESIGN:
analytical, observational, retrospective cohort study. PLACE National Hospital Arzobispo Loayza (HNAL), teaching hospital for UNMSM.PARTICIPANTS:
We studied 34 in internal students of the medicine service of HNAL and San Marcos.Interventions:
PPD test was administered at the beginning and end of inpatient medical and sought partnership with sociodemographic variables and hospital practice.RESULTS:
The prevalence of 26.5 per cent and an incidence of 23.8 per 100 person-years was obtained. Lower risk was determined with exposure to TB patients [RR = 0.24 (0.13 to 0.44)] and less risk than use of masks in TB patients [RR = 0.75 (from 0.5 to 0.96)], but no significant association. All other variables were not statistically significant.CONCLUSIONS:
A high prevalence and incidence of PPD conversion in internal students of the medicine service was obtained. Finding a lower risk regarding protective measures indicates the importance of promoting biosecurity.
