RESUMO
Durante a ressuscitação de uma gestante em parada cardiorrespiratória (PCR), o anestesiologista tem dois pacientes em potencial a mãe e o feto. O conhecimento das alterações fisiológicas de uma mulher grávida é fundamental para que se obtenha sucesso na tentativa de reversão do colapso cardiorrespiratório. Posicionar a paciente em decúbito lateral esquerdo, fornecer oxigênio a 100%, estabelecer acesso intravenoso para administração de fluidos em bolus, considerar causas reversíveis da PCR e identificar alguma condição médica que possa estar dificultando a ressuscitação são medidas essenciais que devem ser tomadas durante o atendimento. Pequenas alterações no BLS (Basic Life Support) e no ACLS (Advanced Cardiovascular Life Support), no que se refere à gestante, auxiliam em prover manobras mais efetivas de reanimação.
ABSTRACT
During resuscitation of a pregnant woman with a cardiorespiratory arrest (CRA), the anesthetist has two potential patients, the mother and the fetus. Knowing the pregnant woman's physiological changes is fundamental to successfully try to revert the cardiorespiratory collapse. The patient should be put in a left lateral decubitus position, given oxygen at 100%, an intravenous access should be established, in order to administer fluid bolus. Reversible causes of CRA should be considered and the medical conditions which may be making it difficult to resuscitate the patient should be identified. These are all essential measures that should be taken. Small changes in Basic Life Support (BLS) and in Advanced Cardiovascular Life Support (ACLS), regarding the fact that the patient is pregnant, help provide more effective resuscitation maneuvers.
