RESUMO
Os defeitos de esmalte são alterações qualitativas ou quantitativas na estrutura dentária, que originam-se de fatores sistêmicos, locais ou genéticos. A hipoplasia de Turner é um defeito na espessura do esmalte localizado cuja etiologia decorre de um traumatismo ou infecção periapical presente no dente decíduo predecessor, afetando o desenvolvimento do dente permanente. O objetivo do presente estudo foi apresentar um caso clínico de paciente infantil com dente hipoplásico de Turner em pré-molar, que tornou-se não vital sem que houvesse lesão de cárie ou trauma adicional. Em razão das características clínicas e radiográficas do dente afetado, bem como do risco de cárie e do comportamento cooperador da paciente, optou-se pela reabilitação do elemento afetado por meio de tratamento endodôntico e de restauração semidireta em resina composta. Torna-se de fundamental importância o conhecimento da etiologia e a realização de um exame clínico e radiográfico minucioso visando ao diagnóstico precoce e à elaboração de um plano de tratamento adequado para todos os defeitos de desenvolvimento do esmalte, incluindo-se a hipoplasia de Turner, cujo tratamento dependerá da severidade da alteração, do comportamento do paciente e do risco de cárie. Sugere-se a realização de estudos que associem a microestrutura do esmalte hipoplásico com a ausência de vitalidade pulpar.
Enamel defects are qualitative or quantitative changes in the tooth structure originating from systemic, local, or genetic factors. Turner's hypoplasia is a defect in the thickness of the localized enamel whose etiology arises from trauma or periapical infection in the predecessor deciduous tooth, affecting the permanent tooth's development. The objective of the present study was to present a clinical case of a child patient with a hypoplastic Turner premolar tooth, which became non-vital without the occurrence of caries, or additional trauma. Due to the affected tooth's clinical and radiographic characteristics, the risk of cavities, and the patient's cooperative behavior, it was decided to rehabilitate the affected element through endodontic treatment and semidirect restoration in composite resin. It is of fundamental importance to know the etiology and carry out a thorough clinical and radiographic examination aiming at early diagnosis and the development of an adequate treatment plan for all enamel developmental defects, including Turner's hypoplasia, whose treatment will depend on the severity of the change, the patient's behavior and the risk of caries. Studies are suggested to be carried out that associate the microstructure of hypoplastic enamel with the absence of pulp vitality.

Introduction: This case report presents the intentional periodontal maintenance of two periodontal hopeless lower central incisors with a multidisciplinary approach and 20-year follow-up. Case presentation: A 36-year-old male, in 2001, was diagnosed with aggressive periodontitis, gingival swelling, bleeding, and mandibular central incisors with mobility and poor prognosis. Following periodontal therapy (phase I), root canal treatment, and occlusal adjustment, #31 and #41 were gently extracted to remove the granulation tissues, calculus, and infected cementum from the root surface. Then, tetracycline-HCl was applied for 5 minutes on the root surfaces. The teeth were repositioned into the sockets and splinted with a lingual bar. At 3 months, the bar was removed, and a free gingival autogenous graft was done to improve the local keratinized tissue width. Mobility scores, pocket depths, and clinical attachment levels were recorded, and radiographs were taken at 1, 5, and 20 years. The 5-year follow-up showed that the teeth were clinically and radiographically in function. There was a reduction in probing depth and a gain in clinical attachment and radiographic alveolar bone levels. After 20 years, #41 was stable, but #31 had external root resorption, leading to a new treatment plan (dental implants) and extraction. Conclusion: The clinical result of this case was satisfactory for 20 years. Intentional periodontal maintenance of the teeth may be an alternative treatment, even considering the high level of complexity.

INTRODUCTION: Alveolar bone coverage can be diagnosed through cone beam computed tomography (CBCT) and this information can prevent orthodontic tooth movement beyond the biological limit. OBJECTIVE: This study evaluated the impact of the bone coverage (BC) diagnosis by CBCT in the orthodontists' planning. METHODS: One hundred fifty-nine Brazilian orthodontists suggested treatment plans for six patients at two different times, using two sequential questionnaires. The first questionnaire consisted of extra and intra-oral photographs, one panoramic radiograph; one lateral cephalometric radiograph with Steiner and Tweed analysis, and the patient chief complaint. The second questionnaire included the same presentations of cases with tomographic images and the radiologist's report. The McNemar test assessed the difference between the first and the second treatment plans. RESULTS: In all six cases, most participants changed the treatment plan after evaluating the CBCT images and the radiologist's report (93.7% in case 5, 78.6% in case 4, 74.2% in case 3, 69.8% in case 6, 66% in case 2 and 61% in case 1; p≤0.01). CONCLUSION: The evaluation of bone coverage through CBCT images has a substantial impact on the orthodontic diagnosis and planning of the Brazilian orthodontists.

The process of planning treatment for impacted maxillary canine (IMC) presents significant challenges, underscoring the need for careful consideration and expertise. OBJECTIVE: The aim of this article was to shed light on these complexities by discussing clinical case studies involving IMC, providing insights into the intricacies of their management. CONCLUSIONS: The management of IMC within orthodontics presents a multifaceted challenge that include the necessity for precise diagnostic processes, prudent use of cone beam computed tomography (CBCT), the strategic selection between open and closed exposure techniques, a in-depth understanding of the specific orthodontic biomechanics involved, and a keen awareness of potential adverse outcomes such as ankylosis, prolonged treatment times, root resorption, and additional complications.

OBJECTIVE: This cross-sectional study evaluated the bone thickness on mini-implants insertion site, the factors that influence the digital planning of MARPE appliance (miniscrew-assisted rapid palatal expansion), and its different designs. METHODS: A total of 135 plannings were assessed regarding the size of the expander screw used, the positioning and the type of the mini-implant rings, and their location in relation to the teeth. Bone thickness measurements were assessed in the region of the mini-implants' trajectory. Differences between the sexes was verified using the ANOVA test (5% significance). RESULTS: 73 cases were planned with 4 mini-implants and 62 cases, with 6 mini-implants. In 90% of cases, teeth #16 and #26 were used as supports, and the most used expander screw was 13mm (64.1% of cases). The anterior mini-implants of conventional MARPE showed more pronounced insertion in bone in males (5.9 ± 2mm; p= 0.025). The extra mini-implants (anterior region) were inserted with greater bone thickness in males (11.1 ± 2.3mm) compared to females (9.9 ± 1.8mm; p=0.041). A greater bone thickness was observed in males (10.1 ± 2.1 mm) when using mini-implants in the paramedian region. CONCLUSION: Additional rings allow more pronounced bone insertion. Male patients had greater bone thickness, which may be related to greater difficulty in opening the sutures. The alveolar process region seems to be a satisfactory site for mini-implants to those patients with reduced bone thickness in the paramedian posterior region. MARPE appliance must be customized for each patient, due to bone thickness and anatomical variations.

Introducción: hemos observado en nuestra práctica ortodóncica una gran cantidad de pacientes con plano oclusal inclinado, con un ángulo goníaco alto y postero-rotación mandibular. Todo parece indicar que el plano oclusal juega un papel muy importante en el desarrollo de las maloclusiones. Objetivo: mostrar en el presente caso clínico de un niño, si existe una correlación entre la inclinación del plano oclusal, la altura del ángulo goníaco, la longitud de la rama mandibular y la proyección del mentón antes y después, al corregirlo y hacerlo más horizontal con tratamiento ortodóncico. Caso clínico: paciente masculino de 11 años de edad, hiperdivergente, clase II esquelética severa, perfil convexo, apiñamiento dental superior e inferior, clase II molar, incompetencia labial severa, mordida profunda, overjet aumentado, mentón retrusivo, cervicales rectificadas y anterorotación de cabeza. El tratamiento de ortodoncia se realizó con brackets Roth slot 22 y extracciones de primeros premolares superiores e inferiores y primeros molares superiores (siendo reemplazados por los terceros molares superiores), curvas inversas, cierre de espacios con pérdida de anclaje y ajuste oclusal. Resultados: al finalizar el tratamiento, se logró mejorar la estética facial, un ángulo goníaco más desarrollado con mayor crecimiento vertical, mayor proyección del mentón, un plano oclusal más horizontal, un adecuado overbite y overjet, clase I molar y canina, antero-rotación mandibular, buena intercuspidación, oclusión funcional y excelente estabilidad oclusal sin retención, mejoría en lordosis cervical y posición de la cabeza. Conclusiones: la corrección de un plano oclusal inclinado en niños y adolescentes hiperdivergentes, puede mejorar de manera importante el vector de crecimiento, ayudar a un mayor desarrollo en la altura del ángulo goníaco y longitud de la rama mandibular, proyección del mentón al corregirlo y hacer el plano oclusal más horizontal con la mecánica ortodóntica de curvas inversas; por lo tanto, en el presente caso clínico: sí existe una correlación muy importante entre la inclinación del plano oclusal, la altura del ángulo goníaco y la proyección del mentón antes y después del tratamiento ortodóncico (AU)
Introduction: we have observed in our orthodontic practice a large number of patients with inclined occlusal plane, with a high goniac angle and posterior mandibular rotation. Everything seems to indicate that the occlusal plane plays a very important role in the development of malocclusions. Objective: to show in the present clinical case of a child, if there is a correlation between the inclination of the occlusal plane, the height of the goniac angle, the length of the mandibular branch and the projection of the chin before and after, when correcting it and making it more horizontal with orthodontic treatment. Case report: an 11 year old male patient, hyperdivergent, severe skeletal class II, convex profile, upper and lower dental crowding, molar class II, severe labial incompetence, deep bite, increased overjet, retrusive chin, rectified cervicals, and anterorotation of the head. Orthodontic treatment was performed with Roth slot 22 brackets and extractions of upper and lower first premolars and upper first molars (being replaced by upper third molars), inverse curve, space closure with loss of anchorage and occlusal adjustment. Results: at the end of the treatment, it was possible to improve facial aesthetics, a more developed gonial angle with greater vertical growth, greater chin projection, a more horizontal occlusal plane, an adequate overbite and overjet, molar and canine class I, mandibular anterorotation, good intercuspidation, functional occlusion and excellent occlusal stability without retention, improvement in cervical lordosis and head position. Conclusions: the correction of an inclined occlusal plane in hyperdivergent children and adolescents can significantly improve the growth vector, help further development in the height of the gonial angle and length of the mandibular ramus, chin protection when correcting it and making the most horizontal occlusal plane with the orthodontic mechanics of inverse curves; therefore, in the present clinical case: there is a very important correlations between the inclination of the occlusal plane, the height of the gonial angle and chin projection before and after orthodontic treatment (AU)

In this regard, concepts of the desirable evolution from the origin of the dental follicles to their inclusion in the arch and contact with the antagonist will be synthesized, and examples of alterations ordered according to the biological progress of the dentition will be presented (AU)

El desarrollo de nuevos abordajes terapéuticos ha generado un aumento en la esperanza de vida de los pacientes con enfermedades neuromusculares (ENM). Se trata de un grupo de enfermedades heterogéneas desde la clínica y los posibles tratamientos. La transición en los pacientes con ENM, implica un gran desafío por presentar niveles intelectuales dentro de rangos promedio, compromisos motor, respiratorio y cardiológico progresivos que resulta en aumento de la dependencia física conforme aumenta la necesidad de autonomía emocional del adolescente. La descripción de transiciones exitosas en ENM incluye intervenciones psicosociales individuales o grupales con un enfoque multidimensional e interdisciplinario, que contemple la participación de la familia para reducir la ansiedad y la preocupación sobre sus hijos. En el Hospital Garrahan los pacientes con ENM son atendidos dentro del Programa de Atención, Docencia e Investigación de Pacientes con Enfermedad Neuromuscular desde 2008. En este trabajo nos proponemos describir la experiencia en transición pre y post pandemia, de los adolescentes con ENM en seguimiento en el Hospital de Pediatría Garrahan (AU)
The development of new care and therapeutic approaches has generated an increase in the life expectancy of patients with neuromuscular diseases (NMD), a group of heterogeneous diseases from a clinical point of view. The transition in patients with MND involves a great challenge due to progressive motor, respiratory and cardiological compromises that result in an increase in physical dependence as the adolescent needs emotional autonomy. The description of successful transitions in patients with MND includes individual and psychosocial interventions with a multidimensional and interdisciplinary approach with family participation. Since 2008, we developed a Care, Teaching and Research Program for Patients with NMD Disease at the Garrahan Hospital. The objective of this work is to describe the pre- and post-pandemic transition experience of adolescents with NMD follow-up in our hospital (AU)

El avance de la tecnología permite realizar cambios en la rehabilitación y estética, reduce los tiempos de trabajo y disminuye los riesgos que se presentan al momento de utilizar materiales convencionales para la toma de impresiones. Existen dos técnicas que se pueden emplear, la analógica y la digital; la primera presenta mayores riesgos que el operador no puede controlar, así como tiempos más prolongados. Este reporte describe la rehabilitación bucal en dos pacientes con desgaste dental, se optó por usar ambas técnicas, la analógica y la digital, como tratamiento; en la primera se empleó el encerado convencional y en la segunda el software Exocad para el encerado digital. Finalmente en ambos casos se aplicó (AU)
The advance of technology has allowed changes in rehabilitation and esthetics, providing shorter working times and reducing the risks that occur when using conventional materials for taking impressions. There are two techniques that can be used, analog and digital, the first one presents greater risks that the operator cannot control, as well as more time. This report describes the oral rehabilitation in patients with dental wear. The treatment was carried out using the analogical and digital techniques, the former using conventional waxing and the latter using Exocad software for the digital waxing; finally, in both cases the injection technique with high-load Flow resin was used (AU)

La estética ha llegado a formar una parte muy importante en la odontología actual. Sin embargo, no debemos sobreponer las necesidades estéticas sobre las necesidades funcionales. La pérdida de estructura dental asociada a bruxismo puede ser considerada patológica cuando compromete la guía anterior, la cual es la influencia en los movimientos mandibulares que proveen las superficies contactantes de los dientes maxilares anteriores con los mandibulares anteriores y evita contactos excéntricos dañinos en los dientes posteriores. El objetivo de este reporte de caso es presentar una alternativa conservadora para restablecer tanto la estética como la funcionalidad de una paciente de 34 años de edad que acudió a la Clínica de Prostodoncia de la Universidad Autónoma de Guadalajara en busca de rehabilitación estética de dientes anteriores. Una vez realizada su evaluación inicial se llegó al diagnóstico de pérdida de guía anterior por desgaste patológico asociado a bruxismo. El tratamiento incluyó tratamientos de conductos, remoción de caries y restauraciones mal ajustadas, coronas y carillas para restablecer la guía anterior. Se cumplieron con las expectativas estéticas que tenía la paciente al igual que con las necesidades funcionales que fueron objetivo desde el inicio, logrando un restablecimiento de la guía anterior de manera conservadora (AU)
Esthetics has become a very important part of dentistry today. However, we should not superimpose esthetic needs over functional needs. The loss of tooth structure associated with bruxism can be considered pathologic when it compromises the anterior guidance, which is the influence on mandibular movements that provides the contacting surfaces of the maxillary anterior teeth with the mandibular anterior teeth and avoids damaging eccentric contacts in the posterior teeth. The aim of this case report is to present a conservative way to restore both esthetics and function in a 34-year-old patient who came to the Prosthodontics Clinic of the Universidad Autónoma de Guadalajara in search of an esthetic appearance of her anterior teeth. After her initial evaluation, a diagnosis of anterior guide loss due to pathological wear associated with bruxism was made. Treatment included root canal treatment, removal of caries, and ill-fitting restorations, crowns, and veneers to reestablish the anterior guidance. The aesthetic expectations of the patient have met as well as the functional needs that were aimed from the beginning, achieving a conservative reestablishment of the anterior guidance system (AU)

OBJECTIVE: This study aimed to evaluate the impact of an intervention using the Treatment Summary and Survivorship Care Plan (TSSCP-P) on self-efficacy and quality of life (QoL) in breast cancer survivors. METHOD: A clinical study, randomized and controlled, conducted to assess self-efficacy and QOL in breast cancer survivors. The participants were randomly assigned to either an intervention group or a control group. The intervention group received individualized nursing consultations guided by the TSSCP-P, while the control group received standard care. Self-efficacy and QoL were assessed at three evaluation moments using validated scales: Functional Assessment of Cancer Therapy-Breast Plus Anm Morbidity (FACT B + 4) and Perceived General Self-Efficacy Scale. Statistical analyses, including regression analysis and hypothesis tests, were conducted to examine the effects of the intervention on self-efficacy and QoL. RESULTS: Female breast cancer survivors (N = 101) were recruited. The intervention group demonstrated a significant improvement in self-efficacy over time compared to the control group (p = 0.01). However, no significant differences were observed in the overall QoL scores between the two groups. Subscale analysis revealed a significant improvement in physical well-being for the intervention group (p = 0.04), while emotional well-being showed a non-significant improvement (p = 0.07). The study suggests that individualized care plans and support strategies, such as the TSSCP-P, can positively influence breast cancer survivors' self-efficacy and certain aspects of their QoL. CONCLUSION: These findings highlight the potential benefits of the TSSCP-P intervention in enhancing self-efficacy among breast cancer survivors. However, further research is needed to explore its impact on overall QoL and its effectiveness across different stages of breast cancer, as well as with longer follow-up periods. OBJECTIVES: The aim of this systematic review was to verify the accuracy of linear measurements performed on low-dose CBCT protocols for implant planning, in comparison with those performed on standard and high-resolution CBCT protocols. METHODS: The literature search included four databases (Pubmed, Web of Science, Embase, and Scopus). Two reviewers independently screened titles/abstracts and full texts according to eligibility criteria, extracted the data, and examined the methodological quality. Risk of bias assessment was performed using the Quality Assessment Tool For In Vitro Studies. Random-effects meta-analysis was used for pooling measurement error data. RESULTS: The initial search yielded 4684 titles. In total, 13 studies were included in the systematic review, representing a total of 81 samples, while 9 studies were included in the meta-analysis. The risk of bias ranged from medium to low. The main results across the studies indicate a strong consistency in linear measurements performed on low-dose images in relation to the reference methods. The overall pooled planning measurement error from low-dose CBCT protocols was -0.24 mm (95% CI, -0.52 to 0.04) with a high level of heterogeneity, showing a tendency for underestimation of real values. Various studies found no significant differences in measurements across different protocols (eg, voxel sizes, mA settings, or dose levels), regions (incisor, premolar, molar) and types (height vs. width). Some studies, however, noted exceptions in measurements performed on the posterior mandible. CONCLUSION: Low-dose CBCT protocols offer adequate precision and accuracy of linear measurements for implant planning. Nevertheless, diagnostic image quality needs must be taken into consideration when choosing a low-dose CBCT protocol.

O objetivo no presente estudo foi avaliar o desempenho de discentes, que participaram do Programa de Monitoria no Curso de Odontologia da Universidade Federal de Juiz de Fora em disciplinas com práticas clínicas, em relação à construção do diagnóstico e plano de tratamento, comparando essas habilidades com as apresentadas por alunos não monitores. Para isso foi aplicado um questionário a 12 alunos que haviam participado do programa de monitoria em disciplinas com práticas clínicas e a 12 alunos não monitores. O questionário foi composto por seis questões sobre o perfil dos discentes monitores e a experiência desses alunos com o programa; e por três casos clínicos fictícios, que permitiram a avaliação da construção do diagnóstico e do plano de tratamento. As respostas das questões sobre o perfil dos discentes foram apresentadas em frequências absolutas e relativas. Testes t para amostras independentes foram utilizados para comparar as notas dos discentes monitores e não monitores para cada caso clínico. Os resultados obtidos mostraram que não houve diferença significativa entre os dois grupos avaliados nas pontuações obtidas pelas respostas aos casos clínicos. Dentre os motivos que levaram os discentes a participar dos programas de monitoria, destacam-se: "oportunidade de reforçar o conhecimento", "aumentar a pontuação do currículo" e "por causa da remuneração". O interesse pela docência foi a justificativa menos prevalente. Pode-se concluir que, mesmo queo resultado não tenha mostrado diferença entre os discentes, a prática da monitoria tem sido relevante, colaborando para o desenvolvimento de competências e habilidades que são indispensáveis à formação do cirurgião-dentista (AU).
El objetivo del presente estudio fue evaluar el desempeño de los estudiantes que participaron del Programa de Tutoría de la Carrera de Odontología de la Universidade Federal de Juiz de Foraen materias con prácticas clínicas, en relación a la construcción del plan dediagnóstico y tratamiento, comparando estas habilidades con las presentadas por estudiantes no tutores. Para ello se aplicó un cuestionario a 12 estudiantes que habían participado en el programa de tutoría en asignaturas con prácticas clínicas y a 12 estudiantes no tutores. El cuestionario constaba de seis preguntas sobre el perfil de los estudiantes tutores y su experiencia con el programa; y tres casos clínicos ficticios, que permitieron evaluar la construcción del diagnóstico y plan de tratamiento. Las respuestas a las preguntas sobre el perfil de los estudiantes se presentaron en frecuencias absolutas y relativas. Se utilizaron pruebas T para muestras independientes para comparar las puntuaciones de los estudiantes tutores y no tutores para cada caso clínico. Los resultados obtenidos mostraron que no hubo diferencia significativa entre los dos grupos evaluados en las puntuaciones obtenidas de las respuestas a los casos clínicos. Entre los motivos que llevaron a los estudiantes a participar en programasde tutoría destacan: "oportunidad de reforzar conocimientos", "aumentar la puntuación del currículo" y "por la remuneración". El interés por la enseñanza fue la justificación menos frecuente. Se puede concluir que, si bien el resultado no mostró diferencia entre los estudiantes, la práctica de tutoría ha sido relevante, contribuyendo al desarrollo de habilidades y habilidades esenciales para la formación de los cirujanos dentistas (AU).
The aim of the present study was to evaluate the performance of students, who participated in the Monitoring Program of the Dentistry Course at the Federal University of Juiz de Fora, in subjects with clinical practices, relative to construction of the diagnosis and treatment plan, to compare these skills with those presented by non-monitor students. For this purpose, a questionnaire was applied to 12 students who had participated in the monitoring program in subjects with clinical practices, and to 12 students who werenot monitors. The questionnaire consisted of six questions about the profile of student monitors and their experience with the program; and three fictitious clinical cases that allowed evaluation of the construction of the diagnosis and treatment plan. The answers to the questions about the students' profile were presented in absolute and relative frequencies. For each clinical case, T-tests for independent samples were used to compare the scores of student monitors and students who were not monitors. The results obtained showed that there was no significant difference between the two groups evaluated, relative to the scores attributed to the responses to the clinical cases. Among the reasons that led students to participating in monitoring programs, the following were outstanding: "opportunity to reinforce knowledge", "increase the CV score" and "because of the remuneration". Interest in teaching was the least prevalent justification. It can be concluded that although the result did not show a difference between students, the practice of monitoring has been relevant, by contributing to the development of skills and abilities that are essential to the training of dental surgeons (AU).

Introdução: este estudo comparou características dentárias e craniofaciais de crianças e adolescentes com fissura labiopalatal unilateral (UFLP) e bilateral (BFLP) não sindrômica reparada. Métodos: foram extraídos dados secundários dos prontuários de 42 pacientes com FLPU (n = 31) e FLPB (n = 11), 6 e 17 anos de ambos os sexos. Foram avaliadas distâncias intercaninas e intermolares, relações caninas e molares, overjet, overbite e mordida cruzada posterior em modelos de gesso; anomalias dentárias e outras ausências dentárias em radiografias panorâmicas digitais; e medidas cefalométricas em telerradiografias laterais digitais. Os dados foram expressos em frequência absoluta e percentual (qui-quadrado ou teste exato de Fisher) e média ± DP (teste de Kolmogorov-Smirnov e teste t de Student) (SPSS, p < 0,05). Resultados: a agenesia foi a anomalia dentária mais prevalente (n = 29; 69,0%) na amostra. Houve diferença estatisticamente significativa entre os grupos para overbite (UFLP 2,04 ± 2,82 mm e BFLP 4,26 ± 2,63 mm; p = 0,028) e as medidas cefalométricas (p < 0,05) 1/./1, 1/NS, 1/Orbita, 1/NB, 1-NB, 1/NPog, Pog-NB, 1-Linha I, (Go-Me).(V-T), F.(V-T), A-(V-T), discrepância cefalométrica, Prn.(Sn-Ls), Sfl/-A perpendicular e Iii-A(A-Pog). Conclusões: há diferenças dentárias e craniofaciais entre UFLP e BFLP, e isso deve ser considerado no plano de tratamento ortodôntico-cirúrgico desses pacientes, a fim de proporcionar uma abordagem mais adequada.
Introduction: this study compared dental and craniofacial characteristics of children and adolescents with unilateral cleft lip and palate (UFLP) and bilateral cleft lip and palate (BFLP) non-syndromic repairs. Methods: secondary data were extracted from the medical records of 42 patients with UFLP (n = 31) and BFLP (n = 11), aged 6 to 17 years and both genders. Intercanine and intermolar distances, canine and molar relationships, overjet, overbite, and posterior crossbite were evaluated on plaster models; dental anomalies and other dental absences on digital panoramic radiographs; and cephalometric measurements on digital lateral cephalometric radiographs. Data were expressed as absolute frequency and percentage (chi-square or Fisher's exact test) and mean ± SD (Kolmogorov-Smirnov test and Student's t-test) (SPSS, p < 0.05). Results: agenesis was the most prevalent dental anomaly (n = 29; 69.0%) in the sample. There was a statistically significant difference between the groups for overbite (UFLP 2.04 ± 2.82 mm and BFLP 4.26 ± 2.63 mm; p = 0.028) and cephalometric measurements (p < 0.05) 1/./1, 1/NS, 1/Orbita, 1/NB, 1-NB, 1/NPog, Pog-NB, 1-Line I, (Go-Me).(V-T), F.(V-T), A-(V-T), cephalometric discrepancy, Prn.(Sn-Ls), Sfl/-A perpendicular, and Iii-A(A-Pog). The median lingual foramen (MLF), which contains neurovascular bundles, is located in an area commonly considered safe for surgical procedures. However, published reports of severe complications after interventions in the mandibular symphysis area indicate the need for caution when approaching this region surgically. The aim of this study was to evaluate the vertical location of the MLF and the median lingual canal (MLC) by measuring the distances of these landmarks to the root apex of the lower central incisors (LCI) and to the menton cephalometric point (Me) on pre-orthognathic surgery cone beam computed tomography scans (N = 100). The results were analyzed in relation to the patients' type of deformity, age, sex, and number of foramina (single vs multiple). The median MLF-LCI and MLF-Me distances were 5.9 mm and 15.0 mm, respectively, while the mean MLC-LCI and MLC-Me distances were 9.7 mm and 11.6 mm, respectively. The mean LCI-Me distance was 21.3 mm, while the mean MLC length was 3.4 mm. Apart from the length of the MLC, the distances were all significantly greater in the male patients than in the female patients. The MLC-Me distance and MLC length differed significantly according to the number of foramina. In preoperative planning, the vertical locations of the MLF and respective MLC appear to be relevant for avoiding neurovascular complications.

INTRODUCTION: The management of eruption disturbances in orthodontics may be challenging and requires a careful diagnosis and treatment planning. This case report discusses the challenges of a two-phase orthodontic treatment of a patient presenting with a dental eruption pattern anomaly. PATIENT CONCERNS: A 10-year-old boy presented with no complaints for a routine orthodontic evaluation during the mixed dentition. PRIMARY DIAGNOSES: The patient was diagnosed with a skeletal Class I malocclusion with unilateral posterior crossbite, incomplete mandibular lateral incisor-canine transposition and a unilateral maxillary ectopic canine. INTERVENTIONS: Phase 1 started with rapid maxillary expansion to correct the maxillary constriction and the ectopic eruption of the right maxillary canine. In the mandibular arch, phase 1 included the extraction of the left primary lateral incisor and canine, alignment of the left permanent lateral incisor and orthodontic traction of the left permanent canine. The duration of phase 1 was 14 months. Phase 2 involved a comprehensive course of orthodontic treatment and started when the patient was aged 13 years. This phase lasted 18 months. RESULTS: An adequate dental occlusion was obtained, and the treatment results were stable after an 18-month follow-up. CONCLUSION: In this case, the early diagnosis of the dental anomalies was valuable as it allowed an early intervention to be undertaken, which resulted in overall treatment simplification and potentially minimised the adverse effects. This case report reinforces the importance of a careful follow-up during mixed dentition.

ABSTRACT Objective: To assess children's self-reported distress during dental procedures and investigate risk factors. Material and Methods: A total of 163 children (3-10 years old) were included from a clinical trial on diagnostic strategies for evaluating restorations in primary teeth. Treatment plans were elaborated based on the clinical examination performed at the baseline of the study. Dentists performed 742 dental procedures, and an external evaluator collected children's self-reported distress through the Wong Backer Facial Scale (WBFS) and dentists' opinions about children's behavior during the treatment. Kruskal-Wallis Test was performed to compare the distress and the dentists' perception of the different dental procedures, and multilevel ordered logistic regression analysis was conducted to the evaluate association between explanatory variables and the outcomes. Results: More complex procedures caused more distress in children (p=0.017), with a 5.5 times higher risk than simple operative treatments. Similarly, dentists reported children's worse behavior (p<0.001). Older children (older than 7 years) reported less distress than younger children (OR 0.52; CI 0.30-0.87; p=0.014). Patients reported greater distress in the first consultations, reducing the chance of higher scores by 16% in the next interventions. Conclusion: Children experience higher levels of distress during their first treatment appointments. More complex operative procedures and the child's age below 7 years were risk factors associated with greater distress during dental treatment.
