RESUMO

INTRODUCTION: Shared paranoid disorder is characterised by the development of psychotic symptoms in people who have a close affective bond with a subject suffering from a mental disorder. This case is the first case of burn injuries reported in the context of this disorder. CASE: We describe a young couple, with a similar pattern of burns caused by contact with a griddle. The injuries are the result of the aggression caused by a relative of one of them, who presented psychotic symptoms, related to the previously undiagnosed spectrum of schizophrenia. CONCLUSIONS: The impact of this condition encompasses social, physical and psychological components, requiring multidisciplinary management and a high index of diagnostic suspicion.

BACKGROUND: Folie à deux is a very rare psychiatric syndrome in which a psychotic symptom is transmitted from one individual to another. We present a case of folie à deux occurring during childhood, which is not an usual presentation of this syndrome. In this case, the disorder is correlated with child abuse and neglect, which possibly had a role in the development of the symptoms in our case. CASE PRESENTATION: We present a case of folie à deux between an "induced" 9-year-old black Brazilian boy and the "inducer", his grandmother. They were found to be sharing similar auditory and visual hallucinations and delusional beliefs. The boy was neglected by his parents and was being cared for by his grandmother, who had a history of mental disorder. CONCLUSIONS: The close relationship between the boy and his grandmother, the family history of first-degree psychosis, and the child abuse and neglect suffered by the boy may have altered his vulnerability to early-onset psychosis and, in this case, folie à deux.

RESUMO

Shared psychiatric disorder (folie deux) is a rare condition. But its prevalence can be 5-25% in patients with delusional parasitic infestation. We report the a case of a 62 years-old female with psychotic symptoms. For 15 years, she has lived with her younger sister. Since the patient was well-controled, her sister interrupted her antipsychotic drug administration. So, the patient initiated delusional parasitic infestation accompanied by visual hallucinations. Her sister, who did not have psychiatric history, initiated to believe that the patient was really infested. Moreover, she started to believe that was infested by the patient. This case report aims to discuss the relation between folie deux and delusional parasitic infestation.

A case of folie à deux dissociative (dissociative hysteria) disorder in an 8 and 12 year-old sister and brother is presented. Illnesses of this type are very rare and there is little medical literature on this subject. Our patients, almost simultaneously, abruptly had complete loss of memory, disorientation, loss of awareness about who they were, and much anxiety, which lasted about 15 hours. Both patients were physically well and no abnormalities were found on physical examination, routines laboratory tests and EEG studies. Speculations about the emotional and interpersonal causes of this illness in these two patients are given.

Santeria, a religious system that blends African and Catholic beliefs, is practiced by many Cuban Americans. One aspect of this system is the belief in spirit possession. Basic santeria beliefs and rituals, including the fiesta santera (a gathering at which some participants may become possessed), are briefly described, and four cases in which the patients' belief in possession played a role in their mental illness are presented. The belief in possession can complicate the diagnosis and treatment of mental illness, but it should not be considered a culture-bound syndrome. Rather, it may be a nonspecific symptom of a variety of mental illnesses and should be evaluated in the context of the patient's overall belief system and ability to carry out usual activities.

On the basis of the pathology described by Lasègue & Falret, this papers intends to further a psychoanalytic distinction between both types of psychosis which exemplify 'folie à deux'. Hypothesis are proposed which refer to the difference in the psychic structure, in view of the 'body into pieces' experience in Type I delirium cases and a mainly neurotic organization in Type II delirium cases. A case in brought up where part of the interview reveals the rigid as well as sealed bond organization between significant and meaning. Reference is also made to the qualities of symbiotic attachment between both partners in the delirious intercourse, and to the importance the consideration of this diagnostic difference has with regards to the therapeutic strategy.

Este trabajo toma como base la patología descripta por Lasègue y Falret para hacer desde aquí una discriminación psicoanalítica entre los dos tipos de psicosis que ejemplifica la "folie à deux". Se proponen hipótesis referidas a la distinta estructuración del psiquismo en estos dos tipos de delirios a partir de la existencia de una vivencia de cuerpo despedazado en los casos de tipo I y una estructura prevalentemente neurótica en los del tipo II. Se ejemplifica con un caso actual en el que un fragmento de entrevista muestra la estructuración fija e inamovible de relación sellada entre significante y significado.
