RESUMO

PURPOSE: To compare tissue inflammatory response, foreign body reaction, fibroplasia, and proportion of type I/III collagen between closure of abdominal wall aponeurosis using polyglactin suture and intraperitoneal implant of polypropylene, polypropylene/polyglecaprone, and polyester/porcine collagen meshes to repair defects in the abdominal wall of rats. METHODS: Forty Wistar rats were placed in four groups, ten animals each, for the intraperitoneal implant of polypropylene, polypropylene/polyglecaprone, and polyester/porcine collagen meshes or suture with polyglactin (sham) after creation of defect in the abdominal wall. Twenty-one days later, histological analysis was performed after staining with hematoxylin-eosin and picrosirius red. RESULTS: The groups with meshes had a higher inflammation score (p < 0.05) and higher number of gigantocytes (p < 0.05) than the sham group, which had a better fibroplasia with a higher proportion of type I/III collagen than the tissue separating meshes (p < 0.05). There were no statistically significant differences between the three groups with meshes. CONCLUSIONS: The intraperitoneal implant of polypropylene/polyglecaprone and polyester/porcine collagen meshes determined a more intense tissue inflammatory response with exuberant foreign body reaction, immature fibroplasia and low tissue proportion of type I/III collagen compared to suture with polyglactin of abdominal aponeurosis. However, there were no significant differences in relation to the polypropylene mesh group.

PURPOSE: To evaluate the fibrosis induced by four different meshes: Marlex®, Parietex Composite®, Vicryl® and Ultrapro®. METHODS: Histological cutouts of abdominal wall were analyzed with polarized light 28 days after the meshes implants and colorized by picrosirius to identify the intensity of collagen types I and III, and their maturation index. RESULTS: When the four groups were compared, the total collagen area analyzed was bigger in groups A and D, with no difference between them. The collagen type I density was bigger in group A, with an average of 9.62 ± 1.0, and smaller in group C, with an average of 3.86 ± 0.59. The collagen type III density was similar in groups A, B and C, and bigger in group D. The collagen maturation index was different in each of the four groups, bigger in group A with 0.87, group B with 0.66, group D with 0.57 and group C with 0.33 (p = 0.0000). CONCLUSION: The most prominent fibrosis promotion in the given meshes was found on Marlex® (polypropylene mesh) and the Parietex Composite® (non-biodegradable polyester); the collagen maturation index was higher in the Marlex® mesh, followed by Ultrapro®, Parietex Composite® and Vicryl® meshes.

PURPOSE: To compare polyglactin 910 and simple catgut sutures for the incidence of intraperitoneal adhesions. METHODS: Twenty female Wistar rats were placed into two groups. Group 1 received ischemic sutures and Group 2 received polyglactin 910. Five sutures inductive of adhesions in each rat were made. After 14 days, the rats were euthanized with an assessment of the presence of adhesions, the number of sutures involved and classification according to the Granat et al. scale described by Ozel et al17. RESULTS: In total, 19 of the 20 rats presented adhesions, with nine from Group 1 and ten from Group 2. There was a smaller number of affected sutures in Group 1, while in Group 2 the majority of the sutures formed adhesions (p=0.0197). According to the Granat et al. scale, Group 1 predominately developed fine, filamentous adhesions or thickening in a restricted area. Group 2 mainly presented extensive, thick adhesions with the involvement of the viscera (p=0.0055). CONCLUSION: Polyglactin 910 sutures formed more adhesions that were more extensive and thicker than the simple catgut sutures.

Lupeol exhibits anti-inflammatory effects; unfortunately it shows low water solubility. An alternative to overcome this is the development of nanomaterials. Several methods for nanomaterial production are available. One of them is emulsification/solvent-evaporation. The objective of the present work was to evaluate physical properties, transport and in vitro modulator effects on NF-κB of poly (lactide-co-glycolide) (PLGA) nanoparticles loaded with lupeol. Nanonutraceuticals were prepared with 16% (w/v) of lupeol. Size distribution and morphology were measured by particle size analyzer and TEM. In vitro release of lupeol was studied by three different models: Higuchi, Siepmann & Peppas, and Power law. Transport of nanonutraceutical was studied in a Caco-2 cell model and by GC-MS. Modulator effect on NK-κB was studied by western blot analysis. Nanonutraceuticals were 10% larger than the nanoparticles without lupeol (372 vs 337 nm) and presented a broader size distribution (0.28 vs 0.22). TEM results displayed spherical structures with a broader size distribution. Entrapment efficiency of lupeol was 64.54% and it in vitro release data fitted well to the Power law and Higuchi equation (R > 0.84-0.84). Strong regulation of NF-κB of nanonutraceutical was observed. It was not observed any transport across the Caco-2 cell model at the different experimental conditions.

The aim of the study was to compare the efficacy and safety of barbed unidirectional vs. polyglactin 910 suture in vaginal cuff closure on patients submitted to total laparoscopic hysterectomy. From November 2011 until March 2012, a prolective, comparative, longitudinal, non-randomized study was performed on patients submitted to total laparoscopic hysterectomy (TLH). On entry, patients were assigned to two different groups, Group 1: Vaginal cuff closure with unidirectional #00 (n = 25) barbed suture, and Group 2: Vaginal cuff closure with polyglactin 910 #1 suture. The length of closure time and the frequency of dehiscence in vaginal vault were measured on the 14th postoperative day. We did not find significant differences between the groups on the general characteristic analysis. However, significant differences were found (P = .029) in the average vaginal cuff closure time (12.7 ± 3.1 min. for barbed suture group vs. 20.4 ± 7.1 min. for polyglactin 910 group). No dehiscence case was found in any group. Based on the results of this study, we can conclude that unidirectional barbed suture reduces the surgical time for vaginal cuff closure during total laparoscopic hysterectomy and doesn't increase the vaginal cuff dehiscence risk.

PURPOSE: To assess the tensile strength of polypropylene and polypropylene associated with polyglactin meshes (Vypro II--Ethicon, Somerville, NJ, USA) in a situation of partial separation of abdominal muscle aponeurosis on rats. METHODS: Thirty rats were used of the Wistar strain, which were randomized into two groups of 15 specimens each. In both groups an aponeurotic-muscle deformity was created on the abdominal wall measuring 3.0 x 1.0 cm, which was closed with polypropylene mesh (polypropylene group) or Vypro mesh (vypro group). After 28 days the rats underwent euthanasia and an area was removed from the abdominal wall with which a strip was made measuring 2.0 cm in length and 6.0 cm in width comprising the abdominal muscles with the implanted mesh. This sample was placed in a mechanical test machine in which a constant force was applied contrary to the tissue strips. Maximum force expressed in Newton was considered until full rupture of the sample occurred. The non-parametric Kruskal-Wallis test was used for statistical analysis admitting p< 0.05. RESULTS: Out of the thirty animals, there were two deaths in the vypro group and one unit in the polypropylene group was lost. One animal in the polypropylene group developed hernia during the study and another one developed granuloma of the abdominal wall. All animals in both groups developed epiplon adherence to the mesh. The average force was 48.08 N for the polypropylene group and 45.32 for the vypro group. CONCLUSION: In these experimental conditions it could be observed that there is no statistically significant difference in the rupture force of the polypropylene and Vypro meshes (p=0.54).

OBJETIVO: Avaliar a resistência tênsil das telas de polipropileno e polipropileno associado à poliglactina (Vypro II® - Ethicon®, Somerville, NJ, USA) em situação de afastamento parcial da aponeurose dos músculos abdominais em ratos. MÉTODOS: Foram utilizados trinta ratos da linhagem Wistar randomizados em dois grupos de quinze exemplares cada um. Em ambos os grupos criou-se um defeito músculo-aponeurótico na parede abdominal medindo 3,0 x 1,0 cm que foi fechado com tela de polipropileno (grupo polipropileno) ou Vypro® (grupo vypro). Após 28 dias, foi feita a eutanásia e retirou-se uma área da parede abdominal com a qual fez-se uma tira medindo 2,0 cm de comprimento por 6,0 cm de largura englobando os músculos abdominais com a tela implantada. Essa amostra foi presa em máquina de ensaios mecânicos na qual se aplicou força constante contrária às tiras de tecido. Foi considerada a força máxima expressa em Newton até ocorrer a ruptura total da amostra. Para a análise estatística, utilizou-se teste não paramétrico de Kruskal - Wallis admitindo-se p<0,05. RESULTADOS: Ocorreram dois óbitos do grupo vypro e uma unidade do grupo polipropileno foi perdida no teste mecânico. Um animal do grupo polipropileno desenvolveu hérnia durante o período do estudo e outro desenvolveu um granuloma de parede abdominal. Todos os animais de ambos os grupos desenvolveram aderência de epíplon à tela. A média das forças foi de 48.08 N para o grupo polipropileno e 45.32 N para o grupo vypro®. CONCLUSÃO: A tela de polipropileno apresentou uma força média de resistência à tração maior quando comparada com a de Vypro® nessas condições experimentais.(AU)

Foi realizado estudo morfométrico do cólon descendente de 15 eqüinos hígidos, nove fêmeas e seis machos, submetidos a enterorrafias aposicionais com fio de poliglactina 910 ou com cianoacrilato. Os animais foram distribuídos aleatoriamente em cinco grupos de três animais cada e sacrificados aos três, sete, 14, 35 e 70 dias de pós-operatório para colheita dos segmentos intestinais operados e posterior processamento histológico de rotina. A análise morfométrica foi feita em microscópio óptico adaptado a um sistema de análise de imagens, onde se quantificou o infiltrado inflamatório, mediante contagens de neutrófilos, eosinófilos, macrófagos, linfócitos e plasmócitos, não sendo observadas diferenças (P>0,05) entre as enterorrafias. Houve diferenças significativas entre os dias de pós-operatório para as contagens de neutrófilos, eosinófilos, macrófagos e linfócitos. Também se mensuraram as espessuras das regiões operadas (cicatriz) e das áreas contíguas não operadas (controle), registrando-se diferenças entre os dias de pós-operatório e as regiões (P<0,05), mas não entre as enterorrafias. O fio de poliglactina 910 e o cianoacrilato apresentam a mesma eficácia para serem usados em enterorrafias aposicionais do cólon descendente de eqüinos.(AU)


A morphometric study was carried out in descending colon of 15 healthy horses, nine females and six males, submitted to appositional suture techniques with polyglactin 910 or with cyanoacrilate. The animals were randomly distributed in five groups of three animals each and sacrificed on postoperative days 3, 7, 14, 35 or 70 in order to collect the operated intestinal segments. The morphometric evaluation was done with an optic microscope adapted to an image analysis system, where was counted the inflammatory infiltrated, through neutrophils, eosinophils, macrophages, lymphocytes and plasmacytes counting. No difference (P>0.05) was observed between the two suture techniques. There were significant differences between postoperative days related to the counting of neutrophils, eosinophils, macrophages and lymphocytes. It was also measured the thickness of operated regions (scar) and non operated nearby areas (control); it registered differences between the postoperative days and the regions (P<0.05), but not between the sutures. RESUMO

PURPOSE: Compare, in a rabbit model, the inflammatory response and adhesions formation following surgical fixation of polypropilene and Vypro mesh in the inguinal preperitoneal space. METHODS: Fourteen male New Zealand rabbits, weighing between 2.000 to 2.500 g were used. A midline incision was made and the peritoneal cavity was exposed. The 2.0 x 1.0 cm polypropylene mesh was fixed in the left flank and secured to the margins with 3-0 prolene in a separate pattern. In the right flank, a 2.0 x 1.0 cm Vypro II mesh was sewn in the same way. After the post surgical period, the animals were again anesthetized and underwent laparoscopic approach, in order to identify and evaluate adhesions degree. Both fixed prosthesis were excised bilaterally with the abdominal wall segment, including peritoneum, aponeurosis and muscle and sent to a pathologist. RESULTS: Operative time ranged from 15 to 25 minutes and no difficulties in applying the mesh were found. From the 14 polypropylene meshes fixed to the intact peritoneum, 11 had adhesions to the abdominal cavity (78.6%). Concerning Vypro mesh, 12 animals developed adhesions from the 14 with mesh fixation (85.7%). Histological examination of tissues harvested revealed fibroblasts, collagen, macrophages and lymphocytes between the threads of the mesh. CONCLUSION: Polypropylene and Vypro mesh, when implanted in the peritoneal cavity of rabbits provoke similar amount of adhesions. Vypro mesh tissues had higher fibrosis resulting in better mesh incorporation to the abdominal wall.

O presente estudo tem por objetivo relacionar o fio cirúrgico e sua natureza, na gênese das infecções da ferida operatória, bem como verificar se o uso do cautério propicia maiores índices de infecção. Foram utilizados 78 ratos Wistar, com peso corporal entre 180 e 250 gramas. Após anestesia geral, quatro incisões foram realizadas no dorso do animal. Para a realização da hemostasia e ao mesmo tempo síntese do plano músculo-aponeeurótico, foram utilizados três tipos de fio: Poligglactina, Algodão, Categute simples. Na Quarta incisão foi utilizado cautério. Os animais foram divididos em grupos de acordo com os procedimentos no plano músculo-aponeurótico: GI - utilizou-se fios e cautério GII - inoculou-se bactérias. GIII - utilizou-se fio, cautério e inoculou-se bactérias. Uma semana após, material foi colhido das feridas cirúrgicas para realização de culturas. O grupo GI (n=10), nenhuma das culturas destas feridas desenvolveu crescimento de microorganismos. O grupo II (n=20), 35 por cento infectaram. No grupo III (n=48), verificou-se a presença de 69,8 por cento feridas infectadas. Concluimos que o fio é importante fator na gênese de infecção de feridas independente de suas características e que o uso abusivo do cautério propiciou os maiores índices de infecção.

RESUMO

Avaliou-se a resposta tecidual na regiäo subcutânea de ratos frente aos fios de sutura: catgut, poliglactina 910 e poliglecaprone 25. Para tanto foram utilizados 20 ratos albinos, nos quais praticaram-se três pontos de sutura em planos profundos na regiäo dorsal com os três diferentes materiais. Após períodos de 1, 2, 7 e 10 dias, grupos de cinco animais foram sacrificados para obtençäo de espécimes que foram processados e corados pela hematoxilina e eosina. Após a análise dos resultados histológicos pode-se verificar uma resposta tecidual mais favorável aos fios sintéticos que ao catgut, com menor fase exsudativa e mais precoce proliferaçäo celular. Entre estes, o fio de poliglecaprone permitiu uma melhor organizaçäo do tecido conjuntivo circunvizinho, com menor infiltrado inflamatório

A 20 ratas se les realizó una plástica de la pared abdominal mediante prótesis colocadas intraperitonealmente. En 10 se empleó una malla de polipropileno y en las restantes prótesis de poliglactina 370. Luego de 45 días todos los animales habían sido sacrificados. El examen histológico demostró escasa reacción inflamatoria en los preparados con mallas absorbibles, mientras que aquellos con materiales no absorbibles desarrollaron una gran respuesta inflamatoria con la formación de bridas y granulomas. Estos hallazgos inducen, a través de un modelo experimental, a continuar evaluando y preferir en la práctica, la utilización de mallas sintéticas absorbibles, en aquellos procedimientos de reemplazo de la pared abdominal, que necesitan una prótesis en posición intraperitoneal.
