RESUMO

The risk of distant metastasis of salivary gland cancers has usually been associated with histological type, tumour size, and site. The aim of this study was to evaluate a series of patients with major salivary gland carcinomas in order to identify potential risk factors associated with distant metastasis. 255 patients treated for major salivary gland carcinoma in Brazil from 1953 to 2004 were reviewed. Clinical and treatment data were obtained from the medical records and histological features reviewed. 57 (22%) of 255 patients had distant metastasis. The lungs were the most common metastatic site (40 cases, 65%) and adenoid cystic carcinoma the most frequent histological type involved (27 cases, 47%). The percentage of tumours in the submandibular, parotid, and sublingual glands that presented distant metastasis was 42%, 20%, and 17%, respectively. These results provide evidences that clinicopathological factors (tumour site and histology) are significant predictors of distant metastasis in patients with major salivary gland carcinomas.

BACKGROUND: Sublingual gland tumors are rare, although frequently malignant. This study describes the clinicopathologic features and treatment results and reviews the literature. METHODS: Thirteen cases treated between 1996 and 2007 were reviewed with interest on clinical, pathologic, and therapeutic information. Survival data were calculated by the Kaplan-Meier method. RESULTS: Malignancies represented 92.3% of cases. Adenoid cystic carcinoma was the most common malignant type (66.7%). Most patients (83.3%) presented in advanced pathologic TNM stages (III or IV). All cases underwent surgical treatment. Neck dissection was performed in 69.2% with no metastases detected. Ten patients (83.3%) had adjuvant radiotherapy. Distant metastases occurred in 3 patients (25%). The 5-year overall and disease-free survival rates were 78.7% and 87.5%, respectively. CONCLUSIONS: Tumors of the sublingual gland are rare and are usually malignant. Radical surgery and adjuvant radiotherapy seems to offer adequate local and regional control. Unlike distant failure, local recurrence and regional metastases are not common.

OBJECTIVE: The aim of this study was to assess the clinical and histologic features of 6 cases of sublingual salivary gland tumors treated in a single institution. STUDY DESIGN: The clinical data were obtained from the medical records, and the histopathologic slides were reviewed. RESULTS: Four cases were adenoid cystic carcinoma (ACC) and 2 were mucoepidermoid carcinoma (MEC). Most cases were clinically staged as T3N0M0. Histologically, 3 cases of ACC were of the cribriform subtype and 1 was tubular. One MEC was classified as intermediate and 1 as high grade of malignancy. After surgical treatment, 3 patients (2 MEC and 1 ACC) did not show tumoral recurrence after a mean time of 162 months of follow-up (ranging from 120 to 216 months). CONCLUSIONS: Sublingual salivary gland tumors are very rare and most are malignant; in our series, 3 cases of ACC had poor prognosis but the patients with MEC were considered cured of the disease.

RESUMO

Os autores apresentam o relato de seis pacientes com neoplasias malignas da glândula sublingual, sendo dois com tumores mucoepidemóides, três com carcinomas adenocísticos (cilindromas) e um com carcinoma indiferenciado. Comentam as características principais destas neoplasias, referindo suas formas clínicas e as dificuldades na confirmaçäo do diagnóstico no mucoepidermóide e no de células acinares. No que diz respeito à terapêutica, referem a grande agressividade do carcinoma adenocístico, assim como sua capacidade de recidiva local, para o qual é indicada a ressecçäo da lesäo primária, aliada aos esvaziamento cervical de princípio, sendo reservadas aos demais as indicaçöes do esvaziamento cervical de necessidade
