RESUMO

Postoperative complications, exemplified by surgical site infections, are commonplace in the realm of daily surgical interventions. Conversely, certain infectious entities, such as cerebral myiasis (CM), are distinctly rare. This report elucidates the clinical presentation of a 74-year-old female afflicted with a CSF fistula, within the context of a preceding surgical microvascular decompression employing a suboccipital craniotomy approach. Notably, the course of evaluation and treatment unveiled an intraoperative manifestation of severe CM. This case report underscores the critical significance of prompt identification, precise diagnostic elucidation, and comprehensive multidisciplinary management to optimize patient outcomes in instances of CM. Furthermore, a systematic literature review on CM supplements this report, contributing to the understanding of this infrequent complication.

This paper reports a case of furuncular myiasis caused by Dermatobia hominis in a domestic dog from Colombia. A male domestic dog, German shepherd breed, of approximately 2 years old was brought for medical consultation. The reason was the presence of one furuncular lesion and discomfort in the patient. Clinical examination revealed a wound over the sacral area similar to an inflammatory papule, with a central punctum exuding a serosanguinous discharge. In the medical examination was applied pressure around the wound, that allowed evidencing a larva inside the tissue. The applied treatment was surgical removal of the larva in the dog. Treatment was complemented with Cephalexin (20 mg / kg, twice a day orally for 7 days), Meloxicam (0.2 mg / kg, once a day orally for 3 days) and cleaning with chlorhexidine, three times a day until removal of points 7 days later. The larva was sent to Parasitology laboratory of the UDES for morphology identification. The larva was identified as Dermatobia hominis.

We describe a case of nasal myiasis in an 89-year-old Brazilian patient affected by leprosy with severe nasal sequelae. An initial treatment comprising sinusectomy combined with nasal endoscopy removed more than 300 larvae, supplemented by systemic treatment using oral and topical ivermectin and levofloxacin. Infestation recurred after 2 months, was treated similarly, and resolved completely. The case could be attributed to severe nasal leprosy sequelae, with a lack of sneezing reflex, painless ulceration, atrophic rhinitis (ozena), and inability to clean the nose properly due to hand and nose impairment. This case illustrates the importance of long-term medical follow-up of patients with leprosy sequelae.

ABSTRACT CONTEXT: Myiasis is caused by larval infestation that usually occurs in exposed wounds. Dermatobia hominis is the most common fly species responsible for this parasitic infection. Genital piercing is an ornamental practice used in certain social circles. At placement, it transverses the skin surface and, as such, may be related to complications. CASE REPORT: We report a case of a 31-year-old man with a history of wound infection secondary to genital piercing who was exposed to an environment with flies, leading to myiasis. Mechanical removal and systemic antiparasitic drugs are possible treatments for myiasis. However, prevention that includes wound cleaning and dressing is the best way to avoid this disease. CONCLUSIONS: Genital piercing can lead to potential complications and myiasis may occur when skin lesions are not properly treated.

Dermatobia hominis is a fly endemic to and widely distributed throughout the Americas; it is found from the southern regions of Mexico to Argentina. However, because of widespread travel, myiasis has become common in countries where neither the disease nor the species that cause this infection are endemic. Central Mexico, for instance, is not a region where myiasis is endemic. We, thus, describe three cases of D. hominis myiasis: two autochthonous cases from the southern part of Mexico and one imported from Costa Rica. In addition, morphological and genetic identification was performed on the maggots extracted from the patients.

RESUMO

A 39-year-old man who was returning from the Amazon Jungle and had no medical history presented with a furuncular lesion on his right parietal scalp. Despite receiving appropriate antimicrobial treatment, his lesion did not heal. After surgical intervention, a Dermatobia hominis larva was extracted. The human botfly D. hominis is the most common causative agent of furuncular myiasis among travelers returning from Central and South America. Surgery is the main treatment option, and secondary bacterial infection should be kept in mind.

ABSTRACT Objectives The aim of this study is to report an unusual form of penile cancer presentation associated with myiasis infestation, treatment options and outcomes. Materials and Methods We studied 10 patients with suspected malignant neoplasm of the penis associated with genital myiasis infestation. Diagnostic assessment was conducted through clinical history, physical examination, penile biopsy, larvae identification and computerized tomography scan of the chest, abdomen and pelvis. Clinical and pathological staging was done according to 2002 TNM classification system. Radical inguinal lymphadenectomy was conducted according to the primary penile tumor pathology and clinical lymph nodes status. Results Patients age ranged from 41 to 77 years (mean=62.4). All patients presented squamous cell carcinoma of the penis in association with myiasis infestation caused by Psychoda albipennis. Tumor size ranged from 4cm to 12cm (mean=5.3). Circumcision was conducted in 1 (10%) patient, while penile partial penectomy was performed in 5 (50%). Total penectomy was conducted in 2 (20%) patients, while emasculation was the treatment option for 2 (20%). All patients underwent radical inguinal lymphadenectomy. Prophylactic lymphadenectomy was performed on 3 (30%) patients, therapeutic on 5 (50%), and palliative lymphadenectomy on 2 (20%) patients. Time elapsed from primary tumor treatment to radical inguinal lymphadenectomy was 2 to 6 weeks. The mean follow-up was 34.3 months. Conclusion The occurrence of myiasis in the genitalia is more common in patients with precarious hygienic practices and low socio-economic level. The treatment option varied according to the primary tumor presentation and clinical lymph node status.

Myiasis refers to Diptera larvae infesting vertebrate animals. There are two forms of the disease: primary and secondary. In primary myiasis, fly larvae invade and develop in healthy tissue; in secondary myiasis, flies lay their eggs in skin ulcerations, and the larvae develop in tissue necrosis products. Furuncular myiasis is a type of primary myiasis. Treatment for it consists of techniques such as the production of localized hypoxia to force the emergence of the larvae, and mechanical or surgical removal of the maggots. These techniques, however, are painful and often unsuccessful. We propose a new technique for extraction of myiasis larvae, which might facilitate the surgical procedure and constitute a virtually painless and aesthetic option for the patient.

Las miasis son infestaciones en vertebrados vivos causados por las larvas de diversas especies de moscas (dípteros). El objetivo del trabajo es presentar la evolución de este caso por ser una enfermedad muy rara en Cuba. Se presenta una paciente femenina, piel blanca, de 70 años de edad y antecedentes de hipertensión arterial crónica. Hace nueve meses le fue diagnosticado un proceso neoformativo de vulva, motivo por el cual fue intervenida quirúrgicamente hace 39 días en otro centro hospitalario. Ahora acude al cuerpo de guardia por constatarse aumento de volumen en la zona quirúrgica, prurito intenso y secreciones fétidas. Por este motivo fue ingresada para estudio y tratamiento en sala. Se le diagnosticó miasis vulvovaginal. Tuvo una evolución satisfactoria en sala gracias a las curas locales y el tratamiento con antibióticos(AU)


The myiasis are infestations in living vertebrates caused by the larvae of several species of flies (Diptera). The aim of this paper is to present the evolution of this case as a very rare disease in Cuba. A case is presented of a female patient, white skin, 70 years old having a history of chronic hypertension. Nine months earlier, she had been diagnosed with vulvar neoformative process, for which she underwent surgery -39 days before this consultation in another hospital. She went to the emergency room due to an increased volume in the surgical area, severe itching, and fetid secretions. She was admitted in hospital for study and treatment. She was diagnosed vulvovaginal miasis. She had a satisfactory evolution thanks to local cures and treatment with antibiotics(AU9

As miíases são relativamente frequentes na medicina veterinária causando grandes defeitos cutâneos com margens irregulares e perdas teciduais, necessitando, muitas vezes, técnicas de cirurgias reconstrutivas, como o retalho de avanço unipediculado para resolução do problema. O objetivo deste trabalho é relatar o caso de um canino, macho, sem raça definida, com dez anos de idade, com a presença de miíase cutânea na região frontal e orbitária no lado esquerdo da face. No primeiro momento a ferida foi tratada como ferida aberta, com debridamento e bandagem adequada até a formação do tecido de granulação para assim ocluir o defeito com um retalho de avanço.(AU)


Myiasis are relatively common in veterinary medicine, causing large cutaneous injuries with irregular margins and tissue 1055, which often requires reconstructive surgery techniques, such as advancement flap, a cosmetic and functional option. The objective of this study is to report a case of a 10 years old half-blood male dog who was affected by a cutaneous myiasis in the left-sided orbital and frontal region. Firstly the wound was treated with debridement and appropriate bandage until the formation of the granulation tissue, occluding it with an advancement flap later.(AU)

El objetivo de este artículo es reportar el caso de un perro macho mestizo de 10 años, que tenía una miasis cutánea en el lado izquierdo de la región orbitaria y frontal. RESUMO

Dermatobia hominis (human Bot fly) causes furuncular myiasis (larval infection) in Central and South America. This report describes a case in a member of the UK Armed Forces who had recently taken part in an overseas training exercise in Belize. The importance of clinical history (including travel history) is highlighted. We also describe the outcomes of conservative treatment and surgical intervention for separate lesions in the same patient.
