RESUMO
We present a case of an elderly patient with uncontrolled diabetes mellitus, who presented with recurrent fever and abdominal pain, after which he was diagnosed with an infected abdominal aortic aneurysm, which represents only 1% of all aneurysms. The patient underwent surgical resection of the aneurysm, rifampicine-impregnated Dacron graft placement and intravenous antibiotic treatment. Microbiology reported Salmonella infection in the aneurysm. Currently, the patient is asymptomatic and without laboratory evidence of inflammatory process.
Presentamos el caso de un paciente de edad avanzada, con diabetes mellitus descompensada, quien presentó un cuadro clínico de fiebre y dolor abdominal recurrente, tras lo cual fue diagnosticado con un aneurisma infeccioso de la aorta abdominal, los cuales representan solamente un 1% de todos los aneurismas. El paciente fue sometido a resección quirúrgica del aneurisma, injerto con dacrón impregnando con rifampicina y tratamiento antibiótico intravenoso. La microbiología del aneurisma confirmó infección por Salmonella. Actualmente, el paciente se encuentra asintomático y sin evidencia laboratorial de proceso inflamatorio.

An infant had a subdural empyema caused by the rare Salmonella species enterica subspecies houtenae (IV) serotype 44:z4,z23:- after only indirect exposure to exotic reptiles in her foster home. Infants recovering from preexisting subdural hematoma are at risk for development of empyema.

The aim of this study was to correlate the findings obtained by transesophageal echocardiography (TEE) with those obtained during surgery and histopathologic studies in patients with mitral and/or aortic infective endocarditis. Twenty eight patients were studied. In all of them clinical history, bacteriological study and TEE. In 15 cases a positive blood culture was found and in 13 the blood culture was negative. The TEE demonstrate infective aortic vegetations in 12 cases, in mitral valve in 11 and in five in both valves. The endocarditis-associated complications were found in 53% of cases and the incidence of embolic events was of 25%. TEE findings related with the presence of vegetations were confirmed in the studies of pathologic anatomy in 92.8% and at surgery in 89.2%. These data suggest that the TEE leads to a significant improvement in the diagnosis and prognosis of mitral and/or aortic infective endocarditis, because it has a high correlation with the studies of pathologic anatomy and surgery.

El objetivo de este estudio fue correlacionar los hallazgos obtenidos por ecocardiografía transesofágica con los obtenidos durante la cirugía y el estudio de anatomía patológica en pacientes con endocarditis infecciosa (EI) en válvula mitral y/o aórtica. Se estudiaron 28 pacientes a quienes se les realizó historia clínica, estudio bacteriológico y ecocardiograma transesofágico (ETE). En el estudio bacteriológico se encontraron hemocultivos positivos en 15 casos, en 13 los hemocultivos fueron negativos. El ETE demostró vegetaciones infecciosas en válvula mitral en 11 casos y en 5 enfermos en ambas válvulas. Complicaciones relacionadas con el proceso infecciosos se encontraron en el 53 por ciento de los casos. La incidencia de eventos embólicos fue del 25 por ciento. La concordancia entre el ETE y la anatomía patológica, en relación a la presencia de vegetaciones, fue del 92.8 por ciento y entre el ETE y la cirugía, en relación a la presencia de vegetaciones y complicaciones, fue del 89.2 por ciento. El ETE es un método de gran utilidad no sólo para el diagnóstico, sino también para el pronóstico de la EI, por su alta concordancia con los estudios de anatomía patológica y cirugía

A 59 year old female patient was admitted to the hospital complaining of lower back pain and fever for four months, with worsening of symptoms during the last two weeks. A painful pulsatile abdominal mass was the only positive sign at her physical examination. Her CT-Scan showed a periaortic hematoma and a rupture of the aortic wall while the Aortography disclosed a false aneurysm below the renal arteries. All blood cultures were negative. The patient was operated on and had her aneurysm resected and her aorta reconstructed with an in situ bifurcated aorto-femoral Dacron graft. The bacteriological examination of the aortic wall revealed a Salmonella type B, which confirmed the clinical hypothesis of abdominal aortic mycotic aneurysm. It was not possible to maintain a prolonged antibiotic therapy and the patient had to be submitted to a second operation three months later because of an infected graft; when the aorto-femoral graft was removed and an axillobifemoral bypass prosthesis was done. After a three years follow-up period the patient is doing well and has no signs of recurrences. We conclude, based on the literature review and our personal experience, that: 1) the symptomatologic triad presented by the patient is highly suggestive of those diagnosis; 2) blood cultures may not be positive; 3) the CT-Scan and aortography are the best examinations for the diagnosis of abdominal aortic mycotic aneurysm; 4) either the in situ or the extra-anatomical arterial reconstruction may be employed with good results. Higher infection rates are reported with the in situ grafts; 5) life-long antibiotic therapy is recommended whatever reconstruction procedure is selected.

Se realizó un estudio transversal retrospectivo para determinar las complicaciones quirúrgicas de un grupo de 35 pacientes ingresados con diagnóstico de salmonelosis, durante el período comprendido mayo 1988-abril 1989 en el Hospital Padre Billini, Santo Domingo. Se determinó que el 42.8 por ciento presentaron complicaciones quirúrgicas tales como hemorragia y perforación; el 75 por ciento de estos pacientes fueron jovenes menores de 30 años con predominio del sexo masculino. El 57.1 por ciento (8) se perforó antes de los 10 dias del inicio del cuadro clínico. Todas las lesiones se localizaron en Ileon Terminal, ocurriendo 3 defunciones
