RESUMO

Immune reconstitution inflammatory syndrome (IRIS) is an atypical and unexpected reaction related to highly active antiretroviral therapy (HAART) in human immunodeficiency virus (HIV) infected patients. IRIS includes an atypical response to an opportunistic pathogen (generally Mycobacterium tuberculosis, Mycobacterium avium complex, cytomegalovirus and herpes varicella-zoster), in patients responding to HAART with a reduction of plasma viral load and evidence of immune restoration based on increase of CD4+ T-cell count. We reported a case of a patient with AIDS which, after a first failure of HAART, developed a subcutaneous abscess and supraclavicular lymphadenitis as an expression of IRIS due to Mycobacterium avium complex after starting a second scheme of HAART.

Different types of samples from a 27-year-old female patient who was diagnosed AIDS after presenting an acute respiratory picture caused by Pneumocistis carinii, in 1997, were studied. From that moment on, the patient continued having respiratory pictures repeatedly, until she was admitted at "Pedro Kouri" Institute of Tropical Medicine with a chronic diarrheical picture, significant loss of weight and extreme anorexia, together with a constant wet cough, fever, etc. The necessary complementary tests were made and a hyperergic Mantoux and very low levels of CD4 cells (under 200 cells/m3) outstood. Microbiological studies were also conducted with samples from sputum, hemocultures and cerebrospinal fluid to discard the presence of acid-alcohol resistant bacilli (AARB). As a result of these studies, the repeated isolation was obtained in all types of worked samples from a slow-growth, creamy and non pigmented strain corresponding to Runyon's group III, which was classified as from the Mycobacterium avium-intracellulare complex. The patient was treated with antibacillary drugs without results, since she dies 4 weeks later due to the total dissemination of this infection. It should be stressed that it is the first case reported and microbiologically diagnosed of a generalized mycobacteriosis in a HIV positive patient due to Mycobacterium avium-intracellulare.

RESUMO

This study was done to determine the occurrence of mycobacteria in the bloodstreams of patients with fever and advanced AIDS in a Brazilian hospital. We also verified the capability of an automated method for recovering these bacteria. During a period of 19 months, 254 patients with AIDS were evaluated. Blood cultures were generally submitted in pairs and drawn separately. Blood cultures were processed by the BACTEC 460TB System (Becton Dickinson Microbiology Systems, Sparks, MD), using the Bactec 13A media (Becton Dickinson Microbiology Systems, Sparks, MD). Of the 530 vials submitted, 77 (14.5%) from 41 (16%) patients were positive. Mycobacterium avium complex was recovered from 45 (58.4%) of the 77 positive vials, corresponding to 22 (53.6%) patients with positive blood cultures. The average time to detect Mycobacterium avium complex was 15 days. Mycobacterium tuberculosis was recovered from 26 (33.8%) of the 77 positive vials, corresponding to 15 (36.6%) patients with positive blood cultures, with an average detection time of 24 days. Other species of mycobacteria were recovered from 6 (7.8%) of the 77 vials, corresponding to 4 (9.8%) patients. M.avium complex was fairly prevalent (8.7%) in severely ill patients with AIDS in our hospital. M. tuberculosis was also an important (6.0%) agent of systemic bacterial infections in these patients. The rapid diagnosis of mycobacteremia was possible with the implementation of this automated technology.

RESUMO

To describe the characteristics of mycobacterial infection in Mexico, we reviewed records from patients who were seen at the AIDS Clinic of the National Institute of Nutrition in Mexico City from 1983 to 1992. Of 460 AIDS patients, 118 (25.6%) were found to have mycobacterial infections by positive Ziehl-Neelsen stain, culture, or both. Cultures were completed for 66 of the 118 stain-positive specimens. Mycobacterium tuberculosis was the most common species found (n = 13), followed by M. avium complex (n = 12); 21 infections were identified a nonspecific mycobacteria other than tuberculosis (MOTT) and 20 infections were from species other than tuberculosis. Susceptibility testing was performed in only two tuberculosis cases, with one strain showing multidrug resistance. We conclude that mycobacterial infection is common among our AIDS population, and MOTT may be at least as common as M. tuberculosis. Previous reports of the rarity of MOTT could be related to the lack of adequate diagnostic methods in developing countries.
