The aim of this article is to share our experience at the Aiglé Foundation in fostering research that can conducted by (or in collaboration with) clinicians within a specific type of naturalistic setting-one that not only provides psychological services but also trains psychotherapists. After presenting the structure of Aiglé and the implementation of its scientific-practitioner philosophy, we describe some of the research that has been conducted with our network of clinicians and the benefits of connecting clinical practice and academic work. We then discuss some of the obstacles that we have encountered in conducting such studies, as well as a number of strategies that we adopted in attempting to address these challenges. We end this article by briefly describing the current state of our practice-research network, and by offering some recommendations to facilitate the conduct of research by and for clinicians.

RESUMEN: El presente trabajo de investigación se realizó en la Fundación de Ortodoncia, alhaberse detectado una reducción progresiva de la cantidad de odontólogos queefectúan allí su formación de posgrado.Se desarrolló un análisis de la Institución considerando los factores externos einternos y se plantearon Lineamientos Estratégicos para la Fundación de Ortodonciaorientados a incrementar su nivel de competitividad.El trabajo se llevó a cabo en etapas o fases:- La primera consta de la revisión documental, revisión de páginas Web, seestudiaron distintos modelos de planificación estratégica tomando de ellos loselementos particulares y de importancia para este trabajo.- En la segunda fase se efectuó un estudio de campo, basado en entrevistassemiestructuradas con miembros Directivos de la Fundación, también se realizaronreuniones grupales con los docentes y a través de encuestas mediante uncuestionario estructurado, realizadas a Docentes de la Fundación, a odontólogosactuales Alumnos (clientes) y a grupo de odontólogos No Alumnos de la Fundación(es decir odontólogos no clientes actuales y que constituyen el mercado potencialpara la Institución).- En la tercera fase, se efectuó el análisis de datos recabados y con los resultados delas encuestas, de las entrevistas y de las reuniones se desarrolló el AnálisisEstratégico considerando los factores externos e internos, determinándose lasAmenazas, Oportunidades, Fortalezas y Debilidades, que se trasladaron al diseño dela matriz FODA. This work focuses on a discussion about the extent to which the level of organizational structure interferes in the internal control practices of non-governmental organizations (NGOs), especially those related to health. The objective of this work was to observe the efficiency of the internal control tests applied within the organizational structure of the Foundation for Cancer Research, Prevention and Care, checking the reliability of the accounting records and operational controls. A case study in a third sector health organization was the chosen methodology. The case study involved company interviews and the analysis of confidential reports. After an evaluation of the organizational structure (of the relations between officials and volunteers) and the application of evaluation proceedings on the quality of the internal controls, the extent to which the organizational structure interferes with the internal control practices of the hospital was assessed. It was revealed that there are structured mechanisms of control in the institution, however the implementation of these controls is inadequately performed. It was further detected that the level of the organizational structure does indeed interfere in internal control practices at the entity.

O estudo centra-se na discussão de que o nível da estrutura organizacional interfere nas práticas de controle interno das organizações não governamentais (ONGs), sobretudo ligadas à saúde. O objetivo deste trabalho foi o de observar a eficiência dos testes de controle interno aplicados dentro de uma estrutura organizacional da Fundação para Pesquisa, Prevenção e Assistência do Câncer, verificando a confiabilidade nos relatórios contábeis e controles operacionais. Como metodologia, realizou-se um estudo de caso em uma organização de saúde do terceiro setor. O estudo de caso ocorreu por meio de entrevistas e análises de relatórios confidenciais. Diante de uma avaliação da estrutura organizacional (das relações entre funcionários e voluntários) e da aplicação de procedimentos de avaliação da qualidade dos controles internos, avaliou-se o quanto o nível da estrutura organizacional interfere nas práticas de controle interno do hospital. Observou-se que existem mecanismos de controle estruturados na instituição, porém a execução desses controles deixa a desejar. Pode-se constatar também que o nível da estrutura organizacional interfere, sim, nas práticas de controle interno da entidade.

The rapidly growing burden of chronic kidney diseases (CKD) is a major public health problem that will stretch the healthcare system of all countries, especially those not yet industrialized. Guatemala has a population of approximately 14 million, of which 60% are younger than 20 years. It is estimated that only 35% of Guatemalan patients with CKD Stage 5 (ESRD) are diagnosed and treated. Therefore, the cost of death and disability due to CKD in this young population is particularly profound. CKD programs have to compete with many demands (often viewed in the short term as more urgent or widespread) for finite healthcare resources. FUNDANIER (Foundation for Children with Kidney Diseases) has been the instrument that facilitated changes in the Guatemalan health system, in order to establish a comprehensive Pediatric Nephrology program. FUNDANIER is the first local initiative to provide comprehensive prevention and management of kidney disease in children and adolescents in Guatemala. Through an agreement with the National Health Authorities, FUNDANIER is involved in the primary, secondary and tertiary prevention of CKD in the pediatric population. This might be a model applicable in other developing countries.

La Fundacion Andina del Corazón(Fundacor) se encarga del diagnóstico, tratamiento, prevención y rehabilitación en pacientes con enfermedades cardiovasculares. Conocer la casuística de la consulta cardiológica infantil de Fundacor entre enero 2003 y diciembre 2006, determinando: distribución por edad, sexo, procedencia, estado nutricional, presión arterial, tipo y motivo de referencia, estudios realizados, diagnóstico principal, diagnóstico asociado, tratamiento recibido y evolución. Estudio retrospectivo, descriptivo de datos registrados de 1292 pacientes en consulta de primera vez, entre 0 días y 16 años, de ambos sexos y con historia clínica completa. De acuerdo al grupo etario, la consulta fue mas frecuente en los pre-escolares (36,3 por ciento) y escolares (27,6 por ciento). Relación varones/hembras de 1,3:1. Procedieron principalmente del municipio San Cristobal (42,1 por ciento). Predominó la normalidad en los indicadores peso/edad, talla/edad, índice de masa corporal y en la presión arterial. Principales motivos de consulta: soplo (38,4 por ciento) y dolor toraxico (12,2 por ciento). Exámenes: 1179 electrocardiogramas, 353 ecocardiogramas. Pacientes referidos por médicos: 64,6 por ciento y sin referencia 35,4 por ciento. Evaluaciones normales: 79,8 por ciento y patológicas: 20,2 por ciento. Se evidenciaron defectos septales, tetralogía de Fallot, coartación aórtica, estenosis valvular principalmente. Pacientes con Síndrome de Down 33, 81, 9 por ciento con defectos cardiacos. Cardiopatías corregidas 18,7 de ellas en la institucion. No ameritaron control 87,7 por ciento. Ocurrieron cuatro muertes. Primera publicación sobre casuística relacionada con atención cardiológica infantil en el estado Táchira. Resultados similares a estadísticas nacionales e internacionales. Son necesarios la referencia oportuna por pediatras, obstetras y médicos generales y mejores recursos para aumentar el número de casos corregidos.
Fundacor takes charge of the diagnosis, treatment, prevention and rehabilitation in patients with cardiovascular diseases. Get knowledge of casuistic about pediatric cardiologys consultation since january 2003 to december 2006, determining: distribution for age, sex, procedence, nutritional status, blood pressure, type and motive of reference, executed studies, principal diagnosis, diagnosis associated, received treatment and evolution. Retrospective and descriptive study of information registered of 1292 patients in first consultation, between 0 days and 16 years, of both sexes and with clinical complete history. According to age group: pre-school (36.3%) and school-age children (27.6%). Relation boys/girls 1.3/1. They came principally from the San Cristobals county (42.1%). Predominance of normal weight/age, height/age, body mass index and blood pressure. Principal motive of consultation: heart murmur (38.4%) and chest pain (12.2%). Examinations: 1179 electrocardiogram, 353 ecocardiogram. The 64.6% of the patients were refered by physicians; 35.4%t came without reference. There were 79.8% of normal evaluations and 20.2% of pathological ones. Main congenital heart defects: septal defects, tetralogia of Fallot, aortic coarctation, valvular stenosis. In 33 patients with Downs syndrome, 81.9% cardiopathy. Cardiopathies were corrected in 18 cases, 7 of them in the institution. Patients who no need control: 87.7%. Four deaths happened. This is the first publication on casuistic related to cardiological infantile attention in Táchira state of Venezuela. Results were similar to national and international reports. the opportune reference for pediatricians, obstetricians and physician and to increase the number of corrected cases by provided better resources are necessary.

La cardiología es una de las especializaciones de la medicina que se encuentra más organizada en relación con su estructura científica y su alcance poblacional. El movimiento societario cardiológico tiene sus orígenes en 1994 con la creación de la Sociedad Interamericana de Cardiología, en México mientras que las fundaciones tienen su origen en 1915, con la creación de la primera asociación para la prevención y alivio de las enfermedades cardiovasculares en Nueva York. Con el correr de los años estas estructuras se han organizado a nivel mundial en la Federación Mundial del Corazón, que es la que dicta las políticas en esta área. La presencia de estas organizaciones de cardiología, sociedades y fundaciones, ha llevado a revisar sus modelos de relacionamiento y líneas de acción, como están estructuralmente organizadas y cuales son sus ámbitos de acción.
Cardiology is one of the most organized medical specialties in terms of scientific structure and population coverage. The movement of establishing cardiology societies originated in 1994 when the Inter-American Society of Cardiology was created in Mexico. On the other hand, foundations date from 1915 when the first association for the preventión and relief of cardiovascular diseases was created in New York. Since then, these strutures have become organized worldwide as the World Heart Federation, which is responsible for dictating policies in this area. The existence of said cardiology organizations, societies and foundations, has lead to review the ways they relate to each other and their action lines, as well as their organizational structures and their fields of action.
