As technology evolves, the role of medical equipment in the healthcare system, as well as technology management, becomes more important. Although the existence of large databases containing management information is currently common, extracting useful information from them is still difficult. A useful tool for identification of frequently failing equipment, which increases maintenance cost and downtime, would be the classification according to the corrective maintenance data. Nevertheless, establishment of classes may create inconsistencies, since an item may be close to two classes by the same extent. Paraconsistent logic might help solve this problem, as it allows the existence of inconsistent (contradictory) information without trivialization. In this paper, a methodology for medical equipment classification based on the ABC analysis of corrective maintenance data is presented, and complemented with a paraconsistent annotated logic analysis, which may enable the decision maker to take into consideration alerts created by the identification of inconsistencies and indeterminacies in the classification.

OBJECTIVES: The role of biomedical engineers (BMEs) has changed widely over the years, from managing a group of technicians to the planning of large installations and the management of medical technology countrywide. As the technology has advanced, the competence of BMEs has been challenged because it is no longer possible to be an expert in every component of the technology involved in running a hospital. Our approach has been to form a network of professionals that are experts in different fields related to medical technology, where work is coordinated to provide high quality services at the planning and execution stages of projects related to medical technology. METHODS: A study of the procedures involved in the procurement of medical technology has been carried out over the years. These experiences have been compared with several case studies where the approach to problem solving in this area has been multidisciplinary. Planning and execution phases of projects involving medical technology management have been identified. RESULTS: After several instances of collaboration among experts from different fields, a network for management of healthcare technology has been formed at our institution that incorporates the experience from different departments that were dealing separately with projects involving medical technology. CONCLUSIONS: This network has led us to propose this approach to solve medical technology management projects, where the strengths of each subgroup complement each other. This structure will lead to a more integrated approach to healthcare technology management and will ensure higher quality solutions.

Este artículo presenta los resultados de un estudio realizado a 49 instituciones prestadoras de servicios de salud (IPS) del área metropolitana del Valle de Aburrá que cuenta con un considerado número de servicios de media y alta complejidad, avaladas por la Dirección Seccional de Antioquia. Los resultados obtenidos de enfocan a percibir el nivel de conocimiento sobre el área de la Ingeniería Clínica, identificar el desarrollo de sus tareas típicas, el grado de conformidad de la labor realizada, la oportunidad de empleo, el nivel de interés para el desarrollo de estudios en el área, el programa académico preferido, la modalidad de enseñanza, el tiempo de dedicación presencial y actitudes que mejoren el perfil profesional de un ingeniero clínico. Este documento es un extracto del trabajo de grado “Estudio descriptivo de la Ingeniería Clínica para proponer un plan de estudios para la especialización en Ingeniería Clínica”. En particular se evalúa la práctica, percepción y oportunidad de desempeño de la Ingeniería Clínica entre los administradores, técnicos e ingenieros hospitalarios de la IPS. Se concluye principalmente que, la percepción sobre la aplicación y desarrollo académico de la Ingeniería Clínica es viable y seria aceptada como un programa educativo de postgrado, el cual será una base apropiada para el desarrollo adecuado de la gestión tecnológica. Over the past ten years the American College of Clinical Engineering has conducted numerous Advanced Clinical Engineering Workshops in developing countries around the world. Most of the recent workshops have been conducted in Latin America and the Caribbean in cooperation with the Pan American Health Organization, an affiliate of the World Health Organization. This presentation describes workshop objectives and topics. It also describes the author's participation in workshops conducted in Costa Rica (2002) and El Salvador (2003).

RESUMO

Since 1976, Clinical Engineering (CE) has been studied at the Simón Bolívar University (USB) as part of the Bioengineering Studies program developed at that University. However, it was not until 1996 that Clinical Engineering activities were established in a Venezuelan hospital. This paper describes how the USB, using its own human resources, has achieved the establishment of a Clinical Engineering Department in a national reference hospital for the first time.

Technology management services at Thomas Jefferson University Hospital are provided by two distinct cost centers: The Department of Biomedical Instrumentation and Jefferson Biomedical Shared Services. The in-house division of the Department of Biomedical Instrumentation (BMI) provides clinical engineering services to the hospital, a 717-bed, tertiary care facility. BMI supports traditional patient care instrumentation, as well as dialysis machines, anesthesia machines, lasers, and the neonatal extracorporeal membrane oxygenation (ECMO) systems. In addition, the department supports over 3,000 personal computers and associated peripherals, and provides research, design, database support, device evaluation, incident investigation, and product problem investigation services. Jefferson Biomedical Shared Services, an integral component of the Department of Biomedical Instrumentation, offers a shared services program to local area hospitals. It has a current client list of 11 major healthcare institutions with annual revenues approaching $3,000,000 per year.

Recent restructuring of the quality assurance standards of the JCAHO and the benefits to be gained from assurance indicators are requiring clinical engineering support groups to manage their equipment inventories with greater precision. Technological enhancements in the database industry have provided these groups with a mechanism with which they can manage. Assessments of equipment reliability, departmental management, and quality assurance give supervisory staff an important tool for feedback.
