Within the complex setting of scientific publishing, the editorial board of a journal stands as the guardian of its scholarly legacy, guiding its trajectory and ensuring its enduring relevance amidst the dynamic landscape of academia1. This special responsibility often marks a symbolic "changing of the guard" as new Editors-in-Chief assume leadership, mapping new paths for the publication while drawing from the achievements of their predecessors. The tradition of acknowledging such transitions through editorial reflections is well-established within the scientific community. Just as Burt2 elucidated in his editorial for His-topathology, titled "Changing of the guard", we also find ourselves at a juncture deserving recognition of the succession within the editorial board of the Journal of Oral Diagnosis. BACKGROUND: To assess the influence of diagnosis and referral provided by specialists in oral diagnosis on disease-free survival and overall survival of patients with oral cancer. METHODS: A cohort of 282 patients with oral cancer treated at a regional cancer hospital from 1998 to 2016 was analyzed retrospectively. The referral register of the patients was analyzed and assigned to two groups: (1) those referred by oral diagnosis specialists (n = 129), or (2) those referred by nonspecialized professionals (n = 153). The cancer treatment evolution was assessed from the patients' records, and the outcome was registered concerning cancer recurrence and death. Sociodemographic and clinicopathological variables were explored as predictors of disease-free survival and overall survival. RESULTS: Group 1 exhibited lower T stages and a reduced incidence of regional and distant metastases. Surgery was performed in 75.2% of cases in Group 1, while in Group 2, the rate was 60.8%. Advanced T stages and regional metastases reduced the feasibility of surgery. Higher TNM stages and tumor recurrence were associated with decreased disease-free survival, while surgical intervention was a protective factor. Higher TNM stage had a negative impact on the overall survival. CONCLUSION: Specialized oral diagnosis did not directly impact disease-free survival and overall survival and did not influence the indication of surgery in oral cancer; however, it was associated with the diagnosis of early tumors and better prognosis.

Los desafíos del envejecimiento de la población y la acumulación de daño oral nos obligan a desarrollar herramientas diagnosticas validas y confiables que nos permitan caracterizar a la población, evaluar sus necesidades terapéuticas, planificar intervenciones significativas y realizar seguimiento de su condición. Con este propósito realizamos una revisión sistemática de la literatura relevante para establecer una metódica secuencial para la validación de la herramienta diagnostica Examen Dental Preventivo del Adulto Mayor. Seleccionamos 48 artículos relevantes, cuya heterogeneidad impidió la realización de un metaanálisis. Sin embargo, los artículos seleccionados fueron sometidos a una síntesis cuantitativa analítica, que nos permitió identificar los dominios y estrategias relevantes para la validación y proponer un protocolo de cinco fases secuenciales que presentamos en extenso en el presente artículo.


The challenges of population aging and the accumulation of oral damage force us to develop valid and reliable diagnostic tools to characterize the population, evaluate their therapeutic needs, plan significant interventions, and monitor their condition post treatment. We carried out a systematic review of the relevant literature to establish a sequential method for the validation of the Preventive Dental Examination of the Elderly diagnostic tool. We selected 48 relevant articles, whose heterogeneity prevented us from performing a meta-analysis. However, the selected articles were subjected to an analytical quantitative synthesis, which allowed us to identify the relevant domains and strategies for validation and then propose a protocol of five sequential phases that we present in detail in this article.

Sialolitíase é caracterizada pela formação de estruturas calcificadas (sialolítos ou cálculos) nos parênquimas ou ductos das glândulas salivares maiores (GSM). Exames radiográficos ou de tomografia computadorizada (TC) podem ser utilizados na detecção dessas estruturas, embora apresentem limitações para a detecção de sialolítos pouco mineralizados, assim como para a diferenciação de tecidos moles. A ressonância magnética (RM) apresenta boa acurácia para o diagnóstico de alterações em GSM ou de cálculos salivares mineralizados ou não, porém, seu uso na prática clínica é limitado devido ao seu alto custo financeiro e a baixa disponibilidade. A ultrassonografia (US) tem sido descrita como uma ótima alternativa de exame por imagem para a avaliação das GSM, devido ao seu custo reduzido, obtenção de imagem em tempo real, boa resolução da imagem e pela não utilização da radiação ionizante. O objetivo deste trabalho é analisar a aplicabilidade da ultrassonografia na detecção de sialolitíase em GSM. Foi realizada uma revisão de literatura sobre o uso da US na avaliação de sialolitíase. Nesta revisão foram coletados dados sobre os autores, ano de publicação, tipo de estudo realizado, qual GSM foi avaliada, o valor diagnóstico da US (acurácia, sensibilidade, especificidade, valor preditivo positivo, valor preditivo negativo), principais características ultrassonográficas das sialolitíases, dados clínicos relevantes e outras informações adicionais. Cálculos salivares, únicos ou múltiplos, com tamanho maior que 2 mm, foram bem detectados pela US, assim como tampões mucosos não mineralizados. Cálculos menores que 1 mm foram mais difíceis de serem visualizados na US sendo a técnica de sonopalpação, associada com o uso de suco de limão ou ácido ascórbico intraoral importante nesses casos. A acurácia, sensibilidade, especificidade, valor preditivo positivo e valor preditivo e negativo foram satisfatórias. Eventuais alterações parenquimais foram bem visualizadas através da US. Dados clínicos e ultrassonográficos foram importantes no estabelecimento dos diagnósticos diferenciais. A ultrassonografia, portanto, pode ser considerada como um método de imagem adequado para a avaliação inicial das sialolitíases nas GSM, tanto em indivíduos adultos como em crianças.


Sialolithiasis is characterized by the formation of calcified structures (sialoliths or stones) in the parenchyma or ducts of the major salivary glands (GSM). Radiographic or computed tomography (CT) examinations can be used to detect these structures, although they present limitations for the detection of poorly mineralized sialoliths, as well as for the differentiation of soft tissues. Magnetic resonance imaging (MRI) has good accuracy for diagnosing alterations in GSM or mineralized or non-mineralized salivary stones, however, its use in clinical practice is limited due to its high financial cost and low availability. Ultrasonography (US) has been described as an excellent imaging alternative for the evaluation of GSM, due to its reduced cost, real-time image acquisition, good image resolution and the non-use of ionizing radiation. The objective of this work is to analyze the applicability of ultrasound in detecting sialolithiasis in GSM. A literature review was carried out on the use of US in the evaluation of sialolithiasis. In this review, data were collected about the authors, year of publication, type of study carried out, which GSM was evaluated, the diagnostic value of US (accuracy, sensitivity, specificity, positive predictive value, negative predictive value), main ultrasound characteristics of sialolithiasis, relevant clinical data and other additional information. Salivary stones, single or multiple, larger than 2 mm in size, were well detected by US, as well as non-mineralized mucous plugs. Stones smaller than 1 mm were more difficult to visualize in the US, with the sonopalpation technique, associated with the use of lemon juice or intraoral ascorbic acid, being important in these cases. The accuracy, sensitivity, specificity, positive predictive value and negative predictive value were satisfactory. Any parenchymal alterations were clearly visualized using US. Clinical and ultrasound data were important in establishing differential diagnoses. Ultrasonography, therefore, can be considered as a suitable imaging method for the initial assessment of sialolithiasis in GSM, both in adults and children.

Abstract Objective: To identify the most prevalent oral lesions based on reports from a pathology institute's reports and associations between malignant and oral potentially malignant disorders with patient's demographic variables and the anatomical location. Material and Methods: All 1,298 histopathological reports of oral lesions recorded in the database were reviewed. Demographic variables, anatomical location of the lesion, histopathological diagnosis of the lesions, and their biological behavior were analyzed. Results: Regarding the biological behavior of the identified lesions, benign lesions were predominant (70%), followed by lesions of undetermined behavior (14.3%), malignant lesions (14.2%), absence of histological alteration (1.2%), and finally, oral potentially malignant disorders (0.5%). The anatomical locations of the most prevalent oral lesions potentially malignant disorders and malignant were in the following structures of the oral cavity: gums, buccal mucosa, floor of the mouth and hard palate (p=49.2%), and tongue (p=48.7%). Conclusion: The probability of malignant and premalignant lesions was higher among males (PR= 4.21; 95% CI 2.08-6.22), the increase in age (PR = 1.06; 95% CI 1.05-1.08), and in the tongue region (PR = 5.48; 95% CI 1.67; 17.92). Identification of malignant and potentially malignant oral conditions is higher in older men and in tongue specimens.

This study aimed to evaluate how different study designs influences the presence and characteristics of diagnosis of oral manifestations in patients with COVID-19. Methods: This study is a meta-search conducted with studies evaluating the relationship between patients with oral manifestations and COVID-19. A search was conducted in three databases (PubMed, Web of Science, and Scopus), using terms specific to COVID-19 and oral lesions, including only primary studies. Study selection was undertaken by two reviewers independently. A descriptive analysis was performed. Results: The results included 55 studies in the data synthesis analysis. Most of the studies were case reports and case series. Some of the studies did not relate a biological test to confirm COVID-19. The ulcer lesion and that clinical examination by specialists were the most reported lesion in case reports and case series studies, while taste alterations and patient self-report were more reported in observational studies. Conclusions: This review concluded that the oral lesions in patients with COVID-19 differ according to the study design. The lack of high-quality scientific evidence in the dental literature on COVID-19 makes the findings inconsistent by the lack of sample selection criteria, diagnostic criteria and classification of oral lesions

Objetivo: Avaliar se a hospitalização na gestação pode influenciar na condição bucal do filho no terceiro ano de vida. Métodos: Estudo longitudinal com bebês de gestantes internadas e acompanhadas no setor da Obstetrícia de um Hospital Escola em Pelotas, RS, Brasil. Os dados referentes a hospitalização e ao parto foram coletados do prontuário hospitalar e no terceiro de vida do filho (a) de um questionário aplicado a mãe e do exame bucal da criança. Cada agravo bucal foi avaliado com critérios específicos, por uma examinadora calibrada e analisado no programa IBM SPSS Statistics com 5% de nível de significância. Resultados: Participaram 20 díades mãe-filho (a). Alterações da oclusão acometeram 95% das crianças, sendo a mordida aberta anterior (MAA) a principal. Ainda, 25% das crianças apresentaram opacidades demarcas e/ou hipoplasia do esmalte, sendo significativamente maior em filhos de mães mais jovens e 20% tinham cárie da primeira infância (CPI), estando relacionada à ausência de creme dental fluoretado e à qualidade da higiene bucal. Conclusão: O reflexo mais evidente da hospitalização na gestação na saúde bucal no terceiro ano de vida do filho (a) foi a oclusão alterada, especialmente a MAA.(AU)


Objective: To assess whether hospitalization during pregnancy can influence the child's oral condition in the third year of life. Methods: Longitudinal study with babies of pregnant women hospitalized and followed up in the Obstetrics sector of a Teaching Hospital in Pelotas, RS, Brazil. Data referring to hospitalization and childbirth were collected from the hospital records and in the child's third of life through a questionnaire applied to the mother and the child's oral examination. Each oral condition was evaluated with specific criteria, by a calibrated examiner and analyzed in the IBM SPSS Statistics program with a 5% minimum significance level. Results: 20 mother-child participated. Occlusion alterations affected 95% of the children, with anterior open bite (AOB) being the main. Still, 25% of the children had opacities and/or enamel hypoplasia, which was significantly higher in children of younger mothers, and 20% had early childhood caries, which is related to the absence of fluoride toothpaste and the quality of oral hygiene. Conclusion: The clearest reflection of hospitalization during pregnancy on oral health in the third year of the child's life was altered occlusion, especially the AOB.(AU)

OBJECTIVE: The aim of this double-blind, randomized trial was to evaluate the influence of the occlusogingival distance (OGD) in noncarious cervical lesions (NCCLs) on the clinical performance of a regular bulk-fill resin composite and a regular nanofilled resin composite. MATERIALS AND METHODS: One hundred and forty NCCLs in 77 participants were randomly divided into four groups (n = 35), according to OGD (1.5 mm ± 10% or 3 mm ± 10%) and resin composites (Filtek Bulk Fill Posterior [B] or Filtek Z350 XT [C]), namely: 1.5 mm-B, 1.5 mm-C, 3 mm-B, and 3 mm-C. The restorations were bonded using a two-step self-etch adhesive (Clearfil SE Bond), applied following the manufacturer's instructions. Restorations were polished 1 week after placement. Two experienced and calibrated examiners evaluated the restorations using modified US Public Health Service criteria at baseline (7 days) and after 6, 12, 18, 24, and 30 months. Statistical analyses were carried out using Kruskal-Wallis, Friedman's repeated measures analysis of variance, and the Wilcoxon signed rank test (α = 0.05). RESULTS: After 30 months, the recall rate was 94.2%. Eight restorations were lost (3 for 1.5 mm-C, 2 for 1.5 mm-B, 1 for 3 mm-C, and 2 for 3 mm-B). All groups resulted in a significantly worse marginal discoloration and surface texture at 30 months in comparison with the baseline (1 week). No significant difference was found for the other parameters. CONCLUSION: The restorations performed with both resin composites produced clinically acceptable restorations. The OGD of NCCLs did not influence the clinical performance of restorations. CLINICAL SIGNIFICANCE: The OGD of NCCLs did not have a significant effect on the clinical performance of a regular bulk-fill and nanofilled restorations. Both materials showed a very good performance on 30-month evaluation.

Aim: To determine the prevalence of dental development anomalies and type of influence on the smile of adolescent students. Method: This was a cross-sectional and analytical study carried out in two public (A1) and two private (A2) schools chosen by lot in the city of Parnaíba-Piauí. The sample calculation was based on the target population: number of people enrolled in public and private schools between 15 and 19 years, in the city of Parnaíba-PI, which totaled 6209 students in 2020, according to a survey carried out by the Brazilian Institute of Geography and Statistics ­ IBGE. A questionnaire on epidemiological data and aesthetic self-perception of the smile was applied to 160 adolescents between 15 and 19 years old, from August 2020 to July 2021. The clinical examination was carried out under natural light, to check for the presence of anomaly(s) in the dental development. Students who presented only one pathology would be called group 1 (G1), those who presented two would be called group 2 (G2) and those who presented 3 or more would be called group 3 (G3). On the other hand, adolescents in whom no anomaly was evidenced would participate in the control group (CG), both in A1 and A2. Results: It was observed that 37.5% of the sample had only a type of dental anomaly, corresponding to 60 individuals. The most prevalent were enamel hypoplasia, fusion, transposition, agenesis, ectopic eruption, microdent and dens-in-dent. It was possible to verify a higher prevalence in the maxilla, private schools (76.6%) and females (86.6%). In 45% of adolescents with dental anomalies, embarrassment was observed when smiling. Conclusion: The prevalence was relatively high, highlighting the enamel hypoplasia, influencing the smile esthetics of a reasonable number of adolescents, whether for acquaintances, strangers or even for photographs

Resumo Objetivo investigar a autopercepção das condições bucais e fatores associados em idosos quilombolas rurais do norte de Minas Gerais, Brasil. Método Trata-se de um estudo analítico e transversal de base populacional, no qual utilizou-se uma amostragem por conglomerados com probabilidade proporcional ao tamanho (n=406). A coleta de dados envolveu a realização de entrevistas e exames clínicos odontológicos. A autopercepção das condições bucais foi avaliada por meio do GOHAI (Índice de Determinação da Saúde Bucal Geriátrica). Resultados A maioria dos idosos autopercebeu a saúde bucal como ótima (46,3%) ou regular (30,2%). Os pesquisados revelaram ainda precária saúde bucal e acesso restrito aos serviços odontológicos. Verificaram-se, na análise múltipla, associações significantes (p˂0,05) entre GOHAI regular e variáveis relativas ao local da última consulta e uso de prótese, bem como entre GOHAI ruim e variáveis atinentes ao estado conjugal, religião, motivo da última consulta, índice CPO-D e uso de prótese. Conclusão Parcela expressiva dos idosos quilombolas manifestou uma autoavaliação mais positiva da saúde bucal, divergente do quadro odontológico constatado profissionalmente. Constatou-se ainda que o relato de percepção ruim das condições bucais esteve fortemente associado a uma saúde bucal mais precária entre os investigados.


Abstract Objective to investigate self-perception of oral conditions and associated factors in rural quilombola older people in northern Minas Gerais, Brazil. Method This is an analytical and cross-sectional population-based study, in which cluster sampling with probability proportional to size (n=406) was used. Data collection involved conducting interviews and clinical dental examinations. Self-perception of oral conditions was assessed using the GOHAI (Geriatric Oral Health Determination Index). Results Most older people self-perceived oral health as excellent (46.3%) or regular (30.2%). Those surveyed also revealed precarious oral health and restricted access to dental services. In the multiple analysis, significant associations (p˂0.05) were found between regular GOHAI and variables related to the location of the last consultation and use of prosthesis, as well as between poor GOHAI and variables related to marital status, religion, reason for last consultation, CPO-D index and use of prosthesis. Conclusion A significant portion of the quilombola older people showed a more positive self-assessment of oral health, which differs from the professionally verified dental condition. It was also found that the report of poor perception of oral conditions was strongly associated with poorer oral health among those investigated.

Abstract The aim of this study was to evaluate the observers diagnostic performance in panoramic radiography using monitor, tablet, X-ray image view box, and against window daylight as a visualization method in different diagnostic tasks. Thirty panoramic radiography were assessed by three calibrated observers for each visualization method, in standardized light conditions, concerning dental caries, widened periodontal ligament space, and periapical bone defects from the four first molars; mucosal thickening and retention cysts in maxillary sinus; and stylo-hyoid ligament calcification and atheroma. A five-point confidence scale was used. The standard-reference was performed by two experienced observers. Diagnostic values using window light were significantly lower for caries and periapical bone defect and retention cyst, stylo-hyoid ligament calcification detection (p<0.05). For atheroma detection, X-ray image view box, tablet, and widow light had lower accuracy than the evaluation on the monitor (p<0.05). Observers diagnostic performances are worsened using window light as an evaluation method for panoramic radiography for dental, sinus, and calcification disorders, while the monitor was the most reliable method.


Resumen El objetivo de este estudio fue evaluar el desempeño diagnóstico de los observadores en la radiografía panorámica utilizando monitor, tablet, caja de visualización de imágenes de rayos X y contra la luz del día de la ventana como método de visualización en diferentes tareas de diagnóstico. Treinta radiografías panorámicas fueron evaluadas por tres observadores calibrados para cada método de visualización, en condiciones de luz estandarizadas, con respecto a caries dental, espacio del ligamento periodontal ensanchado y defectos óseos periapicales de los cuatro primeros molares; engrosamiento de la mucosa y quistes de retención en el seno maxilar; y calcificación y ateroma del ligamento estilohioideo. Se utilizó una escala de confianza de cinco puntos. La referencia estándar fue realizada por dos observadores experimentados. Los valores diagnósticos con luz de ventana fueron significativamente menores para caries y defecto óseo periapical y quiste de retención, detección de calcificación del ligamento estilohioideo (p <0.05). Para la detección de ateroma, la caja de visualización de imágenes de rayos X, el tablet y la luz de viuda tuvieron una precisión menor que la evaluación en el monitor (p <0.05). El rendimiento diagnóstico del observador empeora al utilizar la luz de la ventana como método de evaluación de la radiografía panorámica para los trastornos dentales, de los senos nasales y de la calcificación, mientras que el monitor fue el método más fiable.

RESUMEN Fundamento: En escolares de 4.to grado están presentes enfermedades bucales y se realizan acciones curativas, mientras que las educativas precedidas de un diagnóstico educativo, son deficientes e insuficientes. Objetivo: Identificar los conocimientos, actitudes, prácticas sobre salud bucal y la eficiencia del cepillado bucodental en los escolares de 4.to grado. Metodología: Se realizó un estudio observacional, descriptivo y transversal durante el curso 2020-2021 de la Escuela Primaria Pepito Tey del municipio Trinidad, provincia Sancti Spíritus, con la totalidad (71) de los escolares de 4.to grado. Las variables estudiadas fueron: conocimientos, prácticas, actitudes sobre salud bucal y eficiencia del cepillado bucodental. Se utilizaron métodos del nivel teórico, empírico y estadístico-matemático con la aplicación de un cuestionario y una guía de entrevista, para la eficiencia del cepillado se determinó el Índice de Love. Resultados: Se constató el 71.8 % de los escolares con conocimientos insuficientes, el 74.6 % con actitud desfavorable y el 81.7 % con prácticas insuficientes sobre salud bucal, así como el 93 % con cepillado bucodental no eficiente. Conclusiones: Más de la tercera parte de los escolares tuvo conocimientos insuficientes, actitudes no favorables y prácticas insuficientes de salud bucal y la mayoría un cepillado no eficiente.


ABSTRACT Background: Oral diseases and curative actions are present in 4th grade schoolchildren while educational actions preceded by an educational diagnosis are deficient and insufficient. Objective: To identify the knowledge, attitudes, practices on oral health and the efficiency of Oral brushing in 4th grade schoolchildren. Methodology: An observational, descriptive and cross-sectional study was carried out during the 2020-2021 academic course at Pepito Tey primary school in Trinidad city, in Sancti Spíritus province, with all (71) of the 4th grade students. The variables studied were: knowledge, practices, attitudes about oral health and efficiency of oral brushing. Some methods from the theoretical, empirical and statistical-mathematical level were used with the application of a questionnaire and an interview guide, for brushing efficiency. Results: 71.8 % of schoolchildren with insufficient knowledge were found, 74.6 % with an unfavorable attitude and 81.7 % with insufficient practices on oral health, as well as the 93 % with inefficient oral brushing. Conclusions: More than a third of the schoolchildren had insufficient knowledge, unfavorable attitudes and insufficient practices of oral health and most brushing is not efficient.

RESUMEN Introducción: la epidemiología en salud bucal es fundamental para comprender el impacto de la enfermedad y contribuir con la organización de los servicios de salud. Objetivo: determinar el grado de cumplimiento de las metas de salud bucal propuestas para 2015, en el Consultorio del Médico de la Familia no. 3. Materiales y métodos: se realizó un estudio observacional descriptivo transversal en el Consultorio del Médico de la Familia no. 3, del área Centro del municipio Sancti Spíritus, entre septiembre y noviembre de 2019. El universo estuvo constituido por toda la población del consultorio, y la muestra -150 pacientes- por un muestreo aleatorio simple. Se estudiaron las variables: edad, prevalencia de caries dental, enfermedad periodontal y maloclusiones. Se emplearon métodos empíricos y estadísticos. Resultados: el grupo más afectado resultó el de 60 a 74 años, con un índice CPO-D de 14,12 a expensas de los dientes perdidos. El 71,3 % de la muestra no estuvo enfermo periodontalmente; de los afectados, el grupo de 60 a 74 años mostró una prevalencia del 68 %. El 27 % de la población menor de 19 años presentó algún tipo de anomalía oclusiva, predominando el grupo de 18 años. Solo se incumplió con el porcentaje de población sin afección periodontal (63 % enfermos) respecto a las proyecciones propuestas para el año 2015. Conclusiones: se incumplió con el propósito relacionado con el porcentaje de población sin afección periodontal; el resto de los propósitos fueron cumplidos, incluso superando lo obtenido en la provincia y el país en el año 2015.


ABSTRACT Introduction: epidemiology in oral health is essential to understand the impact of the disease and contribute to the organization of health services. Objective: to determine the degree of fulfilment of the proposed health goals for 2015, in the Family Physician's Office Nr. 3. Materials and methods: a cross-sectional descriptive observational study was conducted in the Family Physician's Office Nr. 3, ot the area Centro of the municipality of Sancti Spiritus, between September and November 2019. The universe was formed by the entire population of the physician's office, and the sample -150 patients- by a simple random sampling. The variables studied were: age, prevalence of dental caries, periodontal disease and malocclusions. Empiric and statistical methods were used. Results: the most affected group was the 60 to 74-year-old age group, with a COP-D index of 14.12 at the expense of the lost teeth. 71.3 % of the sample did not have periodontal disease. Of those affected, the age group aged 60 to 74 years showed a prevalence of 68 %. 27 % of the population under 19 years had some kind of occlusive anomaly, predominating the 18-year-old age group. Only the percentage of population with without periodontal disease (63 % sick) was not accomplished with respect to the projections for 2015. Conclusions: it was no accomplished the purpose related to the percentage of population without periodontal disease; the rest of the purposes were achieved, even surpassing the achievements in the province and the country in 2015.

Chronic kidney disease (CKD) has become a global public health challenge. The objective of this study was to analyze the relationship between self-perception of oral health and clinical condition among patients with CKD. This isa quanti-qualitative survey conducted in a CKD specialized service. The sample consisted of 60 patients who underwent oral examinations to have their severity of caries (DMFT) and need for dental prosthesis checked. Age, sex, time on dialysis, marital status, skin color, education and pre-existing diseases were also analyzed. Among the kidney patients who agreed to undergo the clinical examinations and showed communication skills, some were selected, and three focus groups were created, with the participation of a moderator and six to 10 kidney patients in each group. Their speeches were processed in the IRAMUTEQ software and analyzed through the similarity analysis and word cloud techniques. As for profile, the patients were aged 60.23 ± 10.87 years old; were male (73.33%); were on dialysis for 41.90 ± 56.57 months; were married (61.67%); were white (76.67%); had incomplete primary education (41.66%); had arterial hypertension (76.67%); had a DMFT index of 22.55 ± 8.39; 43.33% needed an upper complete denture; and 30.00% needed a lower complete denture. The similarity analysis revealed many doubts and uncertainties about current health services, which can be proven by the words 'no' and 'treatment'. The quanti-qualitative analysis showed a high rate of dental loss and the need for complete dentures and suggests inequities in oral health care for chronic kidney disease patients, especially in tertiary care. There was a positive representation regarding oral health, but the lexicographical analyses of the textual corpusconfirmed the self-perception of lack of dental care.

BACKGROUND: The role of mastication on gastroesophageal reflux disease (GERD) is unknown. AIMS: To assess whether reduced masticatory function predicts GERD and esophageal dysphagia in patients investigated with upper endoscopy. METHODS: In this cross-sectional study, 179 adult patients referred for elective upper gastrointestinal endoscopy agreed to participate. Before endoscopy, an expert dentist performed an oral examination and scored chewing function in three levels (normal, regular, and reduced). Patients replied questionnaires for assessment of GERD (heartburn, regurgitation, and dysphagia), xerostomia, and mastication (normal, regular, and reduced). Poor chewing was defined when either oral examination or mastication questionnaire rated the chewing function as reduced. Associations of mastication with GERD and dysphagia were estimated using Poisson regression. RESULTS: Eleven patients were excluded. Among 168 analyzed (aging 49.8 ± 15.5 years; 58.9% women), 46 had reduced masticatory function (27.4%), and 122 had regular/normal mastication (72.6%). Reduced mastication was associated with GERD [PR = 1.38 (95%CI 1.12 - 1.70)], adjusting for age, and with esophageal dysphagia [PR = 2.03 (95%CI 1.02 - 4.04)], adjusting for age and xerostomia. CONCLUSIONS: In outpatients referred for upper gastrointestinal endoscopy, reduced masticatory function defined by an expert dentist may be a risk factor for GERD and esophageal dysphagia.

O objetivo desta pesquisa foi avaliar possíveis indicadores clínicos e laboratoriais que podiam determinar um padrão na evolução das infecções e orientar os profissionais a tomarem uma melhor conduta para o caso. Foram colhidos os dados de 440 prontuários que apresentaram diagnóstico compatível e confirmado de infecção oral grave atendidos durante 3 anos no Hospital Municipal Alípio Correa Netto na cidade de São Paulo ­ SP. O período de coleta de dados foi compreendido entre 01 julho de 2018 até 01 julho de 2021. Foram analisados a presença de doenças de base e o trismo, o número de espaços bucais acometidos e se as alterações de exames laboratoriais podiam ou não determinar um padrão no curso do tratamento das infecções orais graves. A idade média dos pacientes foi 26,5 anos, e a origem da infecção foi a pulpar em 80%. Os principais sinais e sintomas observados foram: Dor, Prostração, Trismo, Febre, Odinofagia e a dispneia. Em 65% dos casos, múltiplos espaços estavam envolvidos na infecção, sendo os espaços submandibular (68,4%) e o bucal (53,7%) os mais frequentemente envolvidos. A variante doença de base foi uma das que mais apresentaram significância estatística em nosso trabalho. Observamos que a maioria dos pacientes com doença de base precisaram ser tratados com anestesia geral devido ao maior grau de gravidade dos casos. A média de dias de internação foi de 8,1 dias para pacientes sistemicamente comprometidos, contra 4,2 dos pacientes hígidos. O valor da PCR também foi fortemente influenciado pela doença de base, apresentando uma média de 45 mg/L para pacientes comprometidos contra menos de 20 mg/L dos pacientes sem alteração. Podemos concluir, portanto, que os pacientes sistêmicos devem ser tratados com uma atenção maior, porque tendem a apresentar maior trismo, maiores valores da PCR, maior necessidade de tratamento em centro cirúrgico, maior tempo de internação clínica e principalmente nas unidades de terapias intensivas, o que demonstra uma maior gravidade dos casos (AU).


The objective of this research was to evaluate possible clinical and laboratory indicators that could determine a pattern in the evolution of infections and guide professionals to take a better approach to the case. Data were collected from 440 patients with a compatible and confirmed diagnosis of severe oral infection treated for 3 years at the Hospital Municipal Alípio Corrêa Netto in the city of São Paulo ­ SP. The data collection period was from July 1, 2018 to July 1, 2021. The presence of underlying diseases and trismus, the number of affected mouth spaces and whether or not changes in laboratory tests could determine a pattern were analyzed. in the course of treating serious oral infections. The mean age of the patients was 26.5 years, and the origin of the infection was the pulp in 80%. The main signs and symptoms observed were: Pain, Prostration, Trismus, Fever, Odynophagia and dyspnea. In 65% of the cases, multiple spaces were involved in the infection, with the submandibular (68.4%) and buccal (53.7%) spaces being the most frequently involved. The underlying disease variant was one of the most statistically significant in our study. We observed that most patients with underlying disease needed to be treated with general anesthesia due to the greater degree of severity of the cases. The mean length of hospital stay was 8,1 days for systemically compromised patients versus 4,2 for healthy patients. The CRP value was also strongly influenced by the underlying disease, with an average of 45 mg/L for compromised patients versus less than 20 mg/L for patients without change. We can conclude, therefore, that systemic patients should be treated with greater attention, because they tend to have greater trismus, higher CRP values, greater need for treatment in the operating room, longer clinical stays, especially in intensive care units, which demonstrates a greater severity of the cases. Abstract Objective: To compare the Oncostatin M (OSM) concentrations in tissues of patients with chronic periodontitis with and without diabetes. Material and Methods: Sixty-four subjects visiting the dental outpatient department were categorized as "healthy" (Group 1), "periodontitis" (Group 2), and "diabetes with periodontitis" (Group 3) groups. The clinical oral examination included assessment of plaque, gingivitis, probing depth, clinical attachment level. Blood glucose was assessed for group 3 patients. OSM concentration in the tissues was assessed using ELISA in all groups. Results: The mean OSM was 0.02 ± 0.04 pg/mg in the healthy group, 0.12 ± 0.09 pg/mg in the chronic periodontitis group and 0.13 ± 0.10 pg/mg in the diabetes-periodontitis group. A significantly higher mean OSM was seen in Group 2 and Group 3 than Group 1. The amount of OSM positively correlated with probing depth and clinical attachment level. Conclusion: Periodontal disease causes a rise in Oncostatin M, independent of the diabetic status. Expression of OSM in the gingival tissues can serve as an inflammatory marker.
