RESUMO

Seborrhoeic dermatitis is a disease of unknown etiopathogenesis that affects 5% of the population. In this study, we investigated expression of mRNA for IL-1 alpha, IL-6, IL-4, IFN-gamma, and the stress-inducible MICA molecule in skin biopsies from 12 patients with moderate to severe seborrhoeic dermatitis and 2 healthy volunteers by RT-PCR and hybridization with specific probes. Eight patients expressed INF-gamma, 2 expressed IL-6, 8 expressed IL-1 alpha, and 2 expressed IL-4 (1 with moderate disease). Eight patients expressed inflammatory cytokines (IL-1 alpha, IL-6, and/or IFN-gamma) in healthy skin. Higher cytokine mRNA in damaged vs healthy skin was also observed, suggesting the existence of an inflammation that predisposes healthy skin to develop overt disease. Up-regulated expression of MICA mRNA was observed in 8 patients. Although the pathogenesis of seborrhoeic dermatitis remains to be elucidated, expression of cytotoxicity-activating ligands (MICA), recruitment of NK cells, and a local pro-inflammatory microenvironment may facilitate the development of tissue injury.

Humoral immune responses against exoantigen components of oval, elliptic and round yeast forms of Malassezia furfur were analysed by ELISA and Western blotting assays, using sera from patients with pityriasis versicolor (PV), seborrheic dermatitis (SD) and healthy adults (HA), as control. Sera from patients with SD showed IgG anti-oval M. furfur titers ranging from 1/400 to 1/6400 showing geometric mean (GM) of 1/1472, higher than those obtained with sera from patients with PV (1/200 to 1/6400, GM = 1/1239). Both patient groups showed mean titres statistically superior (P < 0.05) than those obtained form HA (GM = 1/229). Similar data were also obtained with the elliptic and round antigens. However, the anti-oval IgG mean titers from patients' sera were much higher than those obtained with elliptic or round antigenic components (p < 0.05) Anti-M furfur IgM titers obtained from patient's sera with PV against all three exoantigens were statistically superior (p < 0.05) than HA group. Patients with SD showed IgM titers statistically superior (p < 0.05) only to oval yeasts of M. furfur. The IgA mean titers from patients' groups against the different morphological antigens were shown be slightly higher than those HA group. By Western blot, using rabbit anti-sera, the different antigenic components of M.furfur showed a close relationship mainly between oval and elliptic yeast cells antigens. The 70 kDa component of the M. furfur exoantigen of oval morphology was recognized by 84% of the PV patients' sera. On the other hand, SD patients' sera recognized 3 principal components of 70 kDa (100%), 65 kDa (67%) and 84 kDa (53%). These components may be considered immunological markers for PV and SD. Twenty-five percent of HA sera recognized the components of 65, 70 and 94 kDa. This investigation shows that M. furfur antigens can sensitize the host, mainly the oval yeast form of M. furfur with a very important specific IgG response in patients with SD and PV.

La dermatitis seborreica (DS) ha sido informada como una entidad frecuente en pacientes con SIDA. En este estudio evaluamos 26 pacientes con SIDA para conocer la prevalencia de DS y su relación con la cuenta de Pytirosporum ovale mediante tinción de Gram y cinta adhesiva transparente, en comparación con controles hospitalizados por enfermedad no relacionadas con el SIDA ni neurológicas. Los resultados fueron semejantes en ambos grupos: la cuenta de P. ovale correlacionó con la forma clínica de la DS; no hubo diferencia entre pacientes con SIDA y controles, ni en los que tomaban ketoconazol. La cuenta de P. ovale fue semejante en las áreas corporales en que se tomaron las muestras. Las técnicas de Gram y cinta adhesiva transparente fueron comparables para cuenta de P. ovale. Conclusiones. En nuestro medio, la DS no es un hallazgo frecuente en población general ni en pacientes con SIDA, y no hay diferencias en el número de P. ovale encontrado en comparación con controles; estos resultados pueden sugerir que la DS, para expresarse, requiere de factores raciales y que se trata de una entidad diferente en la que el P. ovale no es un factor importante en la fisiopatogenia de la enfermedad

In order to analyse the humoral immune response to the commensal yeast Pityrosporum ovale, we developed a western immunoblot technique with a salt soluble extract of P. ovale cytoplasm. In the present study, we tested sera from patients with psoriasis (n = 15), seborrhoeic dermatitis (n = 10), pityriasis versicolor (n = 8), and normal controls (n = 10). Seventy-three per cent (11/15) of the patients with psoriasis showed specific reactivity with a protein derived from P. ovale of estimated molecular mass 120 kDa, and 46% (7/15) of the cases recognized a 100-kDa protein. Sera from pityriasis versicolor and normal donors showed nonspecific reactivity with several bands of lower molecular weight. To characterize the location of the 100 and 120-kDa proteins, we performed a lyticase digestion of the cell wall, and analysed the soluble digested products by western blotting. The sera from psoriasis patients detected several bands in the range 100-120 kDa. The finding of the immunoreactive 120-kDa protein in this fraction suggests its location at the space between cell wall and membrane (periplasmic space). As a control, we performed an extraction of the cytoplasmic proteins of the dimorphic yeast Candida albicans. C. albicans showed a different pattern of banding in SDS-PAGE. Immunoblots with C. albicans did not allow the detection of any related band. A smear was observed in the high molecular weight range consistent with the presence of lipopolysaccharides. The role of the immune response in infection by P. ovale has not yet been fully explored.(ABSTRACT TRUNCATED AT 250 WORDS)

RESUMO

Two children with acute lymphoblastic leukemia undergoing chemotherapy had a special form of scabies characterized by fine scaling of the scalp simulating seborrheic dermatitis. Pruritus was mild or absent. Immunocompromised children and adults should receive whole-body and scalp antiscabietic treatment.

RESUMO

Elderly patients are more likely than the general population to have significant cutaneous diseases due to structural changes, alteration of immunologic response, and different environmental influences. This review focuses on the pathogenesis, diagnosis, and management of the most common inflammatory dermatoses likely to be encountered in the geriatric population.

Reportamos datos recogidos estudiando la presencia y la numerosidad de Pityrosporum ovale (Pov) y P. orviculare (Por) en muestras epicutáneas de 155 personas residentes en Venezuela: 128 muestras de cuero cabelludo (con dermatitis seborreica, 90; asintomático, 38), 77 muestras de conducto auditivo externo (43 signos de dermatitis y 34 asintomáticos) y 27 muestras de lesiones dermatológicas varias. Usamos cinta adhesiva transparente para tomar impronta de regiones planas; usamos cureta para raspar el oído. Teñimos con azul de metileno. Entre otras cosas, encontramos que: 1)Por y Pov se hallan casi con la misma frecuencia y abundancia en los cueros cabelludos de las personas con dermatitis seborreica que en los de las personas asintomáticas; 2) Por parece preferir oídos sintomáticos y Pov oídos asintomáticos. Los cocos no parecen tener preferencia; los bacilos prefieren los oídos asintomáticos. en conjunto, los datos recogidos no permiten atribuir la patogenia de la dermatitis seborreica directamente a la presencia y/o abundancia de Pityrosporum
