BACKGROUND: Chronic exposure to severe hypoxia causes an increase in hematocrit (Hct) and hemoglobin concentration ([Hb]), which can lead to excessive erythrocytosis (EE) and impact physical performance. This work aims to determine the differences in the six-minute walking test (6MWT) between EE and healthy subjects residing at more than 5000 m. METHODS: A prospective, cross-sectional study was performed on 71 men (36 healthy and 25 suffering from EE) living in La Rinconada, Peru (5100 m). Basal levels of [Hb] and Hct were obtained. All the subjects performed the 6MWT, and distance reached, vital signs, dyspnea, and fatigue (Borg scale) at the end of the test were recorded. RESULTS: The average [Hb] and Hct levels in the control group were 18.7 ± 1.2 g/dL and 60.4 ± 7.1%, respectively, contrasting with EE subjects, who showed 23.4 ± 1.6 g/dL and 73.6 ± 5.9% (p < 0.001). However, no statistically significant differences were observed in BMI or other anthropometric parameters. At the end of the 6MWT, the distance traveled and vital constants were similar between both groups, except for arterial oxygen saturation, which was consistently lower in subjects with EE throughout the test. CONCLUSION: EE does not significantly affect 6MWT performance at high altitudes, nor the hemodynamic control during moderate aerobic exercise of subjects who live permanently in a severely hypoxic environment.

El objetivo del estudio fue determinar impacto clínico, funcional respiratorio y tomográfico (TACAR) luego del alta a 12 meses del diagnóstico. Métodos : análisis según grupos NM y NG, desatura dores (PD) en prueba de caminata 6 min (PC6M) y patrón tomográfico símil fibrótico (SF). Comparamos resultados a 3 y 12 meses de seguimiento. Resultados : 194 pacientes enrolados, evaluados al año: 103 (53% ): masculinos (62.4%), edad 57.7 ± 10.9 años, comorbilidades (hipertensión arterial 38.8%, dia betes 29.6%, antecedentes respiratorios-AR- 18.4%). Com paramos variables a los 3 y 12 meses (media/DE): CVF 84%(19) a 88(19) (p = 0.01); 27% tuvo CVF<80% al año. En PC6M: 426 metros (108) a 447(92) (p = < 0.01). El 12.8% fue PD al año. NG tuvo mayor SF (40% vs. 27.9% p = 0.021). SF se relacionó con CVF <80% (p = 0.004) en toda la co horte, y NG (p < 0.001). Al año en análisis multivariado se asoció a CVF <80%, AR (OR 4.32, 1.15-16.25), diabetes (OR 2.96, 1.02-8.57) y patrón SF (OR 3.51, 1.25-9.88). PD se asoció a AR (OR 12.2, 2.41-61.85). Discusión : Se observó mejoría en todas las variables al año. Pero al año persisten alteraciones funcionales y tomográficas en <50% de los pacientes. El subgrupo de PD se relacionó a AR. Es importante el seguimiento protocolizado de los pacientes hospitalizados, especial mente los grupos NG, PD y SF.

PURPOSE: We investigated respiratory muscle strength, diaphragm mobility, lung function, functional capacity, quality of life, body composition, breathing pattern, and chest wall (VT,CW) and compartmental volumes of Mucopolysaccharidosis (MPS) patients and compared these variables with matched healthy individuals. METHODS: A cross-sectional study with data analyzed separately according to age group. A total of 68 individuals (34 MPS and 34 matched-healthy subjects) were included. Six-minute walking test assessed functional capacity and ultrasound assessed diaphragm mobility during quiet spontaneous breathing (QB). Optoelectronic plethysmography assessed VT,CW and breathing pattern during QB in two different positions: seated and supine (45° trunk inclination). RESULTS: Body composition, lung function, respiratory muscle strength, and functional capacity were reduced in MPS (all p < 0.01). Diaphragm mobility was only reduced in adolescents (p = 0.01) and correlated with body composition and breathing pattern. Upper chest wall compartmental volumes were significantly lower in MPS, while abdominal volume only differed significantly in adolescents. Percentage contribution (%) of upper ribcage compartments to tidal volume was reduced in MPS children, whereas %AB was significantly increased compared with healthy subjects. CONCLUSION: Lung function, respiratory muscle strength, functional capacity, diaphragm mobility, and quality of life are reduced in MPS compared with matched healthy subjects. VT,CW was mainly reduced due to pulmonary and abdominal ribcage impairment. Implications for RehabilitationReduction in respiratory muscle strength, functional capacity, diaphragm excursion and low lung volumes were found in individuals with Mucopolysaccharidoses (MPS).Chest wall volumes and the upper chest wall compartmental volumes during quiet spontaneous breathing are reduced in MPS.Assessment and monitoring of the respiratory system for individuals with MPS should be performed periodically through standardized assessments to enable identification of changes and early intervention by rehabilitation protocols.This study may provide the necessary basis for carrying out respiratoty rehabilitation protocols that can improving chest wall mechanics with breathing exercise in this group.

The objective of this study is to determine the clinical, respiratory, tomographic and functional impact on COVID-19 patients with moderate (MP) to severe (SP) pneumonia after six months of acute infection. METHODS: Analysis was carried out by MP and SP groups, desaturators during the 6-minute-walking test and the presence of fibrotic like pattern on HRCT. Outcomes at 3 and 6 months were compared. RESULTS: The analysis included 129 patients, between 57 ± 11 years old. Frequent comorbidities were: arterial hypertension 38.1%, diabetes 30.4%, respiratory 18.6%). Comparing 3 and 6 months, improvement in quality of life was observed in MP and SP. The DP walked less meters in the MWT, worsened life quality and more fibrotic like pattern. The fibrotic pattern was related to the fall of CVF < 80% on MP and SP (p = 0.048 and p = 0.007), and with DP (p = 0.002). On multivariated analysis, the fibrotic like pattern was associated to the reduction of CVF with OR = 4.44 (1.94-10.18, p <0.01) and desaturation OR = 5.01(1.63-15.42, p < 0.01). On this cohort it was observed more functional and tomographic compromise on the DP The fibrotic like pattern was related to worse functional evolution and oximetry. DISCUSSION: Follow-up after discharge of COVID-19 patients with SP, DP or fibrotic changes in HCRT is underlined.

Background: Congenital heart disease (CHD) entails structural defects in the morphogenesis of the heart or its main vessels. Analyzing exercise capacity of children and adolescents with CHD is important to improve their functional condition and quality of life, since it can allow timely intervention on poor prognostic factors associated with higher risk of morbidity and mortality. Objective: To describe exercise capacity in children and adolescents with CHD compared with healthy controls. Methods: A systematic review was carried out. Randomized clinical trials and observational studies were included assessing exercise capacity through direct and indirect methods in children and adolescents between 5 and 17 years-old. A sensitive analysis was performed including studies with CHD repaired participants. Additionally, it was sub-analyzed by age range (< and ≥ 12 years old). Two independent reviewers analyzed the studies, extracted the data, and assessed the quality of the evidence. Results: 5619 articles were found and 21 were considered for the review. Eighteen articles used the direct exercise capacity measurement method by cardiopulmonary exercise test (CPET). The CHD group showed significant differences in peak oxygen consumption (VO2peak) with a value of -7.9 ml/Kg/min (95% CI: -9.9, -5.9, p = 0.00001), maximum workload (Wmax) -41.5 (95% CI: -57.9, -25.1 watts, p = 0.00001), ventilatory equivalent (VE/VCO2 ) slope 2.6 (95% CI: 0.3, 4.8), oxygen pulse (O2 pulse)-2.4 ml/beat (95% CI: -3.7, -1.1, p = 0.0003), and maximum heart rate (HRmax) -15 bpm (95% CI: -18, -12 bpm, p = 0.00001), compared with healthy controls. Adolescents (≥ 12 yrs) with CHD had a greater reduction in VO2peak (-10.0 ml/Kg/min (95% CI: -12.0, -5.3), p < 0.00001), Wmax (-45.5 watts (95% CI: -54.4, -36.7), p < 0.00001) and HRmax (-21 bpm (95% CI: -28, -14), p<0.00001). Conclusion: Suffering CHD in childhood and adolescence is associated with lower exercise capacity as shown by worse VO2peak, Wmax, VE/VCO2 slope, O2 pulse, and HRmax compared with matched healthy controls. The 2-Minute Step Test (2MST) has been presented as an alternative to the 6-Minute Walk Test (6MWT) based on the association between the two tests in older adults; however, some authors propose that it should not be a substitute but rather a complement to the latter in the fitness evaluation. Specifically, in coronary disease, despite the potential and clinical utility of 2MST, the relationship of both tests in this population is unknown. This study aimed to determine the relationship between 6MWT and 2MST and to explore the relationship of biodemographic factors for both tests in subjects with treated coronary artery disease. For this, the 6MWT and the 2MST were applied to patients with coronary artery disease treated in 6 hospitals in Chile between May 2019 and February 2020. Additionally, lower limb strength was assessed by a chair-stand test, grip strength was assessed by a dynamometer, and physical measurements were applied. In total, 163 participants underwent both tests (average age = 58.7 ± 9.8 years; 73.6% men; 64.4% revascularized by angioplasty; 28.2% revascularized by surgery, and 7.4% treated by drugs or thrombolysis). Heart rate was higher at the end of the 6MWT, while the perception of effort was greater at the end of the 2MST. There was a weak positive correlation between the 6MWT and the 2MST in subjects with treated coronary disease (r = 0.28, p = 0.0003). While age (r = -0.27), weight (r = 0.25), height (r = 0.49), and strength of both lower limbs (r = 0.41) and grip strength (r = 0.53) correlated weakly or moderately to the covered distance in 6MWT, the number of steps by the 2MST correlated only weakly to height (r = 0.23), lower limb strength (r = 0.34), and grip strength (r = 0.34). Age, weight, height, lower limb strength, and grip strength would explain better the meters walked in the 6MWT than the steps achieved in the 2MST. With these findings, we can conclude that, in patients with treated coronary artery disease, it does not seem advisable to replace 6MWT with 2MST when it is possible to do so. Additionally, the 2MST may provide additional information in the fitness evaluation. However, the usefulness of 2MST in this population needs to be further studied.

There are several methods to assess the function of the autonomic nervous system. Among them, heart rate recovery (HRR) is an accepted, easy, low-cost technique. Different pathological conditions have been related to the development of autonomic dysfunction. Our study aimed to evaluate the relationship between HRR and HRR-derived parameters in ambulatory patients with asthma or type 2 diabetes followed at the National Institutes of Health in Mexico City. A total of 78 participants, 50 women and, 28 men were enrolled; anthropometric, respiratory evaluations, and fasting blood samples were taken before participants performed a 6-min walking test (6MWT). Abnormal HRR was defined as a drop of ≤8 and ≤11 beats/min at 1 or 2 min and correlated negatively with basal oxygen saturation at 1 min. Heart rate at 1 min, correlated negatively with final oxygen saturation (p < 0.01). Statistically significant negative correlations were also observed between red cell count and white blood cell count and HOMA-IR with a p < 0.01. Since discrete hematological but significant changes correlated with HRR and HRR-derived parameters, we consider that these measures are helpful in clinical settings to identify subclinical autonomic dysfunction that permits us to prevent or anticipate chronic and fatal clinical outcomes.

BACKGROUND: Patients with cirrhosis are at risk of cirrhotic cardiomyopathy, with resulting cardiac dysfunction and exercise limitations. Six minute walking test (6MWT) assesses functional status and predicts morbidity and mortality in cardiopulmonary diseases. AIM: To determine if it associates with mortality by analyzing 6MWT performance in patients with liver cirrhosis. METHODS: A cohort of 106 cirrhotic patients was evaluated in the outpatient setting with echocardiogram and 6MWT and follow up for one year to document hepatic decompensation and mortality. The distance in meters was recorded at the end of 6 min (6MWD). RESULTS: This cohort had a mean age of 51 years and 56% male; patients were staged as Child A in 21.7%, B 66% and C 12.3%. Walk distance inversely correlated with Child scores, and was significantly reduced as Child stages progresses. Patients who died (10.4%) showed shorter mean 6MWD (P = 0.006). Low 6MWD was an independent predictor of mortality (P = 0.01). CONCLUSION: 6MWT is a noninvasive inexpensive test whose result is related to Child scores and mortality. It is useful to identify patients with liver cirrhosis at high risk of mortality for closer monitoring and potential early intervention.

BACKGROUND: There are limitations to perform the cardiopulmonary exercise test (CPET) and the six-minute walking test (6MWT) in clinical settings. The incremental shuttle walking test (ISWT) might be an alternative assessment of exercise capacity and an estimation of cardiopulmonary fitness after stroke. OBJECTIVES: To investigate the test-retest and inter-rater reliabilities, the standard error of measurement (SEM) and the minimal detectable change (MDC) of the ISWT, its construct validity to assess exercise capacity and its concurrent validity to estimate cardiopulmonary fitness after stroke. METHODS: Prospective diagnostic accuracy study. Fifty-one individuals (54 ± 11 years) at the chronic phase (65 ± 73 months after stroke) were included. The distance walked, in meters, during the 12-stage-ISWT and the 6MWT and the peak oxygen uptake (VO2peak, ml.kg-1.min-1) in the CPET (gold standard) were assessed. RESULTS: Significant and high to very high magnitude test-retest and inter-rater reliabilities (0.88≤ICC≤0.93) of the ISWT was found. The SEM for both reliabilities was small (-23.35 m≤ SEM≤41.47 m). The MDC for test-retest and inter-rater reliabilities were 114.63 m and 64.53 m, respectively. For construct validity, a significant and high magnitude correlation between the ISWT and the 6MWT was found (ICC = 0.82). For the concurrent validity, a significant, but low magnitude correlation was found between the ISWT and the VO2peak (rho = 0.42). CONCLUSIONS: The ISWT demonstrated adequate reliabilities and construct validity for exercise capacity assessment. However, the concurrent validity of the ISWT as an estimation of cardiopulmonary fitness still requires further research.

A doença renal crônica (DRC) é uma condição clínica de alto risco cardiovascular e os pacientes nos estágios mais avançados da doença que dependem de terapia renal substitutiva frequentemente tem prejuízo cardiorespiratório, níveis elevados de pressão arterial (uso de múltiplas medicações para controle), modulação autonômica prejudicada e graus variados de inflamação. Deste modo este estudo tem como objetivo verificar se o exercício físico aeróbio intradialítico tem impacto em modificar estas alterações. Os pacientes foram selecionados em duas unidades de hemodiálise em São Luís do Maranhão, Brasil, entre junho de 2016 e outubro de 2019, e foram alocados conforme aceitação em grupo controle (GC) e grupo exercício (GE). O GE foi submetido a treinamento aeróbio com bicicleta por um período de 12 semanas. Avaliação física antropométrica, teste de caminhada de 6 minutos (TC6m), ecocardiograma, eletrocardiograma com análise da variabilidade da frequência cardíaca e medidas laboratoriais foram realizadas incluindo interleucina 6 (IL6) antes e após 12 semanas em ambos os grupos. Trinta e um pacientes foram avaliados 15 pacientes no grupo controle (GC) e 16 pacientes no grupo exercício (GE). Após 12 semanas de treinamento houve diminuição da pressão arterial sistólica do grupo exercício em relação ao basal (129,8 ± 9,41mmHg vs 112,00 ± 12,0 mmHg p = 0,03). Não houve alterações na composição corporal e na maioria dos exames laboratoriais, exceto pelo aumento do KTV (índice de adequação de diálise) e diminuição do LDL colesterol no grupo exercício em relação ao grupo controle. No entanto, os níveis de HDL colesterol aumentaram (39,92 ± 6,1 mg/dL vs 48,00 ± 7,85 mg/dL p = 0,02) e IL6 diminuíram (4,56 ± 1,2 pg / mL vs 2,14 ± 1,0 pg / mL p = 0,02). Houve aumento da distância percorrida no teste de caminhada no grupo exercício (473,80 ± 98,6 metros vs 573,50 ± 74,22 metros p = 0,01). Na avaliação ecocardiográfica, verificou-se que no GE houve diminuição da pressão da artéria pulmonar estimada (31,38 ± 2,9 mmhg vs 24,2 ± 1,7 mmhg p = 0,001). Houve melhora na modulação autonômica no GE (RMSSD 11,7 ± 4,2 vs 18,4 ± 5,7 p=0,02), LFnu (52,9 ± 17,2 vs 32,0 ± 18,2 p=0,02) e HFnu (48,1 ± 17,2 vs 68,0 ± 18,2 p=0,01). Não foram evidenciados efeitos adversos e não houve abandono do treinamento. Baseados nestes resultados, é possível concluir que o exercício aeróbio intradialítico por 12 semanas pode melhorar parâmetros cardiorrespiratórios, hemodinâmicos e autonômicos, com boa aderência e sem eventos adversos, podendo ser usado como medida coadjuvante para melhora clínica destes pacientes.
Chronic kidney disease (CKD) is a clinical condition of high cardiovascular risk and patients in the more advanced stages of the disease who depend on renal replacement therapy often experience cardiorespiratory impairment, high blood pressure levels (use of multiple medications for control), modulation impaired autonomy and varying degrees of inflammation. Thus, this study aims to verify whether intradialytic aerobic exercise has an impact on modifying these variables. The patients were selected in two hemodialysis units in São Luís do Maranhão, Brazil, between May 2016 and October 2019, and were allocated according to acceptance in the control group (CG) and exercise group (EG). The group exercise was submitted to aerobic exercise with bicycle for a period of 12 weeks. Anthropometric physical evaluation, 6-minute walk test (6MWT), echocardiogram, electrocardiogram with analysis of heart rate variability (VFC) and laboratory measurements were performed including interleukin 6 (IL6) before and after 12 weeks in both groups. Thirty-one patients were evaluated 15 patients in the control group (CG) and 16 patients in the exercise group (EG). After 12 weeks of training, there was a decrease in systolic blood pressure in the exercise group compared to baseline (129.8 ± 9.41 mmHg vs 112.00 ± 12.0 mmhg p = 0.03). There were no changes in body composition and in most laboratory tests, except for an increase in KTV (dialysis adequacy index) and a decrease in LDL cholesterol in the exercise group compared to the control group. However, HDL cholesterol levels increased (39.92 ± 6.1 mg / dL vs 48.00 ± 7.85 mg / dL p = 0.02) and IL6 decreased (4.56 ± 1.2 pg / mL vs 2.14 ± 1.0 pg / mL p = 0.02). There was an increase in the distance covered in the walking test in the exercise group (473.80 ± 98.6 m vs 573.50 ± 74.22 m p = 0,01). In the echocardiographic evaluation, it was found that in the EG there was a decrease in the estimated pulmonary artery pressure (31.38 ± 2.9 mmhg vs 24.2 ± 1.7 mmhg p = 0.001). There was an improvement in autonomic modulation in the EG (RMSSD 11.7 ± 4.2 vs 18.4 ± 5.7 p = 0.02), LFnu (52.9 ± 17.2 vs 32.0 ± 18.2 p = 0.02) and HFnu (48.1 ± 17.2 vs 68.0 ± 18.2 p = 0.01). There were no adverse effects and training was not abandoned. Based on these results, it is possible to conclude that intradialytic aerobic exercise for 12 weeks can improve cardiorespiratory, hemodynamic, and autonomic parameters, with good adherence and without adverse events, and can be used as a supporting measure for the clinical improvement of these patients.

OBJECTIVE: To explore measures of peripheral muscular oxygenation, coupled to gait characteristics, between preterm and full-term children during a 6-minute walking test (6MWT). STUDY DESIGN: Prepubescent children performed a 6MWT. During the test, changes in muscular oxyhemoglobin, deoxyhemoglobin, and total hemoglobin were measured with Near-infrared spectroscopy technology, positioned on subjects' calves. Gait variables were monitored with the OptoGait system. RESULTS: Forty-five children (33 full-term children and 12 preterm children, mean age, 4.9 ± 0.7 and 4.6 ± 0.9 years, respectively) participated in this study. Statistical analysis highlighted a decreased walking performance for preterm children, with significantly lower walking distance (P < .05) than children born full-term (405.1 ± 91.8 m vs 461.0 ± 73.3 m respectively; -9%). A concomitant increase of oxygen extraction (over the time course of Variation of desoxyhemoglobin) was observed from the third minute of the test (P < .05). No statistically significant difference was found for other near-infrared spectroscopy measures. Finally, the analysis of gait variables highlighted a group effect for walking speed (P < .05) and stride length (P < .01). CONCLUSIONS: Premature children showed decreased walking performance and greater change in peripheral muscular oxygen extraction, associated with slower walking speed and stride length. This may point to a muscular maladjustment and reduced functional capacities for children born preterm. These phenomena could be responsible for greater muscular fatigue.

McArdle disease results from a lack of muscle glycogen phosphorylase in skeletal muscle tissue. Regenerating skeletal muscle fibres can express the brain glycogen phosphorylase isoenzyme. Stimulating expression of this enzyme could be a therapeutic strategy. Animal model studies indicate that sodium valproate (VPA) can increase expression of phosphorylase in skeletal muscle affected with McArdle disease. This study was designed to assess whether VPA can modify expression of brain phosphorylase isoenzyme in people with McArdle disease. This phase II, open label, feasibility pilot study to assess efficacy of six months treatment with VPA (20â¯mg/kg/day) included 16 people with McArdle disease. Primary outcome assessed changes in VO2peak during an incremental cycle test. Secondary outcomes included: phosphorylase enzyme expression in post-treatment muscle biopsy, total distance walked in 12 min, plasma lactate change (forearm exercise test) and quality of life (SF36). Safety parameters. 14 participants completed the trial, VPA treatment was well tolerated; weight gain was the most frequently reported drug-related adverse event. There was no clinically meaningful change in any of the primary or secondary outcome measures including: VO2peak, 12 min walk test and muscle biopsy to look for a change in the number of phosphorylase positive fibres between baseline and 6 months of treatment. Although this was a small open label feasibility study, it suggests that a larger randomised controlled study of VPA, may not be worthwhile.

Introduction: The efficacy of long-acting bronchodilators for COPD associated with biomass (BE-COPD) has not been properly evaluated. Objective: To determine the acute effect of indacaterol (IND) 150 µg q.d and tiotropium (TIO) 18 µg q.d. on lung hyperinflation, walking distance (WD) and dyspnea during the six-minute walking test (6MWT) in moderate BE-COPD at 30, 60 and 240 mins post-drug administration. Design: Randomized, controlled, open-level, crossover noninferiority clinical trial. Forty-two women with BE-COPD were randomly assigned to a bronchodilator sequence: IND-TIO or vice versa. Results: There were statistically significant changes over time in inspiratory capacity (IC) (p<0.0001), FEV1 (p<0.0001) and FVC (p<0.0001) when IND was used. When TIO was administered, an increase over all time periods was observed only for FEV1 (p<0.0001) and FVC (p<0.0001), whereas for IC an increase was observed only at 30 mins and 24 hrs after TIO administration. We did not find clinically significant increases in WD and dyspnea after the administration of both bronchodilators. Conclusion: Both IND and TIO showed significant and fast onset improvement in hyperinflation. Therefore, either of them may be recommended as a first line of treatment for COPD associated with BE-COPD.

Introdução: O incremental shuttle walk test (ISWT) pode estar mais bem correlacionado com o nível de atividade física diária (NAFVD) comparado ao teste de caminhada de 6 minutos (TC6). Objetivo: Avaliar as correlações entre o NAFVD e as distâncias percorridas no TC6 e ISWT. Métodos: Selecionamos 29 adultos e idosos assintomáticos por conveniência. Estatura e peso foram coletados para cálculo do IMC. Todos foram submetidos aos testes de caminhada duas vezes em dias alternados. Mensuramos a pressão arterial, frequência cardíaca, dispneia e fadiga de membros inferiores antes e após os testes. O NAFVD foi avaliado por meio de um acelerômetro uniaxial e pelo questionário de atividade física (IPAQ). Resultados: Após análise de regressão múltipla, apenas a distância do ISWT foi determinante para número de passos diários obtidos pela acelerometria, explicando 26% da variabilidade total dessa variável. Conclusão: O ISWT se mostrou válido para estimar o NAFVD de maneira mais adequada quando comparado ao TC6.
Introduction: The incremental shuttle walk test (ISWT) may be better correlated with the level of physical activity in daily life (NAFVD) compared to the 6-minute walk test (6MWT). Objective: To evaluate the correlations between the NAFVD and the distances covered in the 6MWT and ISWT. Methods: We selected 29 asymptomatic middle-aged and older adults for convenience. Height and weight were collected to calculate BMI. All were submitted to walking tests twice in alternate days. We measured blood pressure, heart rate, dyspnea, and leg fatigue before and at the end of the tests. NAFVD was evaluated using an accelerometer with a pedometer and the International Physical Activity Questionnaire (IPAQ). Results: After multiple regression analysis, only the ISWT distance was determinant for the number of daily steps obtained by the accelerometry, explaining 26% of the total variability of this variable. Conclusion: The ISWT proved valid to estimate NAFVD more adequately when compared to the 6MWT.

Abstract Objective: to evaluate the relationship between sarcopenia, functional capacity and nutritional status among elderly women living in the community. Method: an observational, cross-sectional study was performed with 100 elderly women aged over 60 years. A questionnaire containing identification and socioeconomic data and information relating to the practice of physical activity was applied, while anthropometric and body composition data were measured through bioimpedance and functionality data was assessed using the six-minute walk test (6MWT). Independent t-tests were performed for the quantitative variables and analysis of variance (ANOVA) was used to compare the means of the variables. Multiple linear regression analyzes were performed to estimate the mean 6MWTfor each of the variables studied. Results: The average age of the elderly women was 67 (± 8.0) years, 41% practiced physical activity, 38% had at least an elementary school education, 48% received up to two minimum wages and 91% were obese according to waist circumference (WC). The prevalence rates for the presence of sarcopenia were: 5% for sarcopenic obesity (SO), 63% for obesity, 14% for sarcopenia and 18% had adequate weight. Elderly women who practiced physical activity, had at least an elementary education and who were non-obese according to WC, performed better in the 6MWT. There were no significant differences in the 6MWTbased on income or Body Mass Index (p>0.05). Non-obese and non-sarcopenic women walked further in the functional test than the other women (p = 0.021). Conclusion: SO was present in 5% of the elderly women and is related to poor physical performance, which was also present in elderly women with sarcopenia and obesity.
Resumo Objetivo: Avaliar a relação entre a sarcopenia, a capacidade funcional e o estado nutricional de idosas da comunidade. Método: Estudo observacional analítico, de caráter transversal, realizado com 100 idosas acima de 60 anos de idade. Para a coleta de dados, aplicou-se um questionário contendo informações de identificação, dados socioeconômicos, prática de atividade física, dados antropométricos e de composição corporal por meio da aplicação da bioimpedância e dados de funcionalidade por meio do teste de caminhada de seis minutos (TC6min). Realizaram-se testes t independente para variáveis quantitativas e análises de variância (ANOVA) para comparar as médias das variáveis. Foram realizadas análises de regressão linear múltipla para estimar as médias do TC6min para cada uma das variáveis estudadas. Resultados: A média de idade das idosas foi de 67 (±8,0) anos, 41% praticavam atividade física, 38% possuíam pelo menos o ensino fundamental, 48% recebiam até dois salários mínimos e 91% eram obesas de acordo com a circunferência da cintura (CC). As taxas de prevalência quanto à presença de sarcopenia foram: 5% para obesidade sarcopênica (OS), 63% obesidade, 14% sarcopenia e 18% adequado. Idosas que praticavam atividade física, que tinham pelo menos o ensino fundamental e não obesas, de acordo com a CC, tiveram melhor desempenho no TC6min. Não houve diferenças significativas no TC6min de acordo com a renda ou Índice de Massa Corporal (p>0,05). As idosas não obesas e não sarcopênicas caminharam mais no teste de funcionalidade que as demais (p=0,021). Conclusão: A OS foi presente em 5% das idosas e tem relação com pior desempenho físico, também presente em idosas com sarcopenia e obesidade.

Introdução: Para o tratamento da doença renal crônica os pacientes necessitam de hemodiálise ou diálise peritoneal enquanto aguardam em lista de espera pelo transplante renal. Apesar dos avanços nos procedimentos de diálise, os pacientes que fazem esse tratamento apresentam baixa capacidade física e funcional e comorbidades que influenciam no tempo de espera. Objetivo: Descrever as características físicas, antropométricas e cardiorrespiratórias de pacientes em lista de espera para o transplante renal, assim como, analisar as variáveis de acordo com o tempo em lista de espera e tempo de diagnóstico da doença. Materiais e métodos: Estudo transversal, descritivo e analítico, submetido e aprovado pelo Comitê de Ética em Pesquisa da UNIJUÍ parecer n° 1.992.013. Foram avaliados 11 pacientes cadastrados no Hospital de Caridade de Ijuí, quanto aos dados sobre a doença renal e seu tratamento, tempo em lista de espera, fatores de risco cardiovasculares, resistência muscular de membros inferiores, força muscular respiratória e capacidade funcional submáxima. Resultados: Foram avaliados 11 pacientes, sendo 54,5% do sexo feminino, média de idade de 51,9±11,4 anos, 45,5% tinham doença renal de origem hipertensiva, tempo de diagnóstico da doença de 84,0 (48,0-180,0) meses, 90,9% realizavam hemodiálise há uma mediana de 15,0 (8,0-41,0) meses e estavam em lista de espera há 12,0 (6,0-32,0) meses. Os fatores de risco cardiovasculares mais prevalentes foram hipertensão e sedentarismo. No teste de sentar e levantar em um minuto realizaram 17,0 (16,0-22,0) repetições, pressão inspiratória máxima de 62,0 (50,0-80,0) cmH2O, pressão expiratória máxima de 78,0 (64,0-99,0) cmH2O e percorreram 452,5 (416,1-503,2) metros no teste de caminhada em seis minutos. Conclusão: Os pacientes cadastrados em lista de espera para o transplante renal do nosso estudo apresentaram uma boa condição físico funcional e cardiorrespiratória. O tempo de diagnóstico da doença renal se correlacionou moderada e negativamente com a pressão expiratória máxima. O tempo de permanência em lista de espera não se correlacionou com a resistência de membros inferiores, força muscular respiratória e capacidade funcional submáxima. [AU]
Introduction: For the treatment of chronic kidney disease patients require hemodialysis or peritoneal dialysis while they wait for a kidney transplant waiting list. Despite advances in dialysis procedures, the patients who undergo this treatment have low physical condition and comorbidities that influence waiting time. Objective: To describe the physical characteristics, anthropometric and cardiorespiratory of patients on the waiting list for renal transplantation, as well as to analyze the variables according to waiting list time and disease diagnosis time. Materials and methods: A cross-sectional, descriptive and analytical study submitted and approved by the Research Ethics Committee of UNIJUÍ, report No. 1.992.013. Eleven patients registered at Ijuí Charity Hospital were evaluated for data on kidney disease and its treatment, waiting list time, cardiovascular risk factors, lower limb muscle resistance, muscle strength respiratory and submaximal functional capacity. Results: Eleven patients were evaluated: 54.5% female, average age 51.9 ± 11.4 years, 45.5% had hypertensive renal disease, diagnosis time of 84.0 (48.0-180.0) months, 90.9% underwent hemodialysis an average of 15.0 (8.0-41.0) months and were on the waiting list for 12.0 (6.0-32.0) months. The most prevalent cardiovascular risk factors were hypertension and sedentary lifestyle. In sit and stand up test in a minute performed 17.0 (16.0-22.0) repetitions, maximum inspiratory pressure 62.0 (50.0-80.0) cmH2O, maximum expiratory pressure 78.0 (64.0-99.0) cmH2O and walked 452.5 (416.1-503.2) meters in the walk test in six minutes. Conclusion: The patients registered on the waiting list in our study had a good physical functional and cardiorespiratory fitness. The time to diagnosis of renal disease was moderately and negatively correlated with maximal expiratory pressure. Waiting time did not correlate with lower limb muscle resistance, muscle strength respiratory and submaximal functional capacity. [AU]

Propentofylline (PPF) is a xanthine derivative with pharmacological effects that are distinct from those of classic methylxanthines. It depresses the activation of microglial cells and astrocytes, which is associated with neuronal damage during neural inflammation and hypoxia. Our previous studies showed that PPF improved remyelination following gliotoxic lesions that were induced by ethidium bromide (EB). In the present study, the long-term effects of PPF on open field behavior in rats with EB-induced focal demyelination were examined. The effects of PPF were first evaluated in naive rats that were not subjected to EB lesions. Behavior in the beam walking test was also evaluated during chronic PPF treatment because impairments in motor coordination can interfere with behavior in the open field. The results showed that PPF treatment in unlesioned rats decreased general activity and caused motor impairment in the beam walking test. Gliotoxic EB injections increased general activity in rats that were treated with PPF compared with rats that received saline solution. Motor incoordination was also attenuated in PPF-treated rats. These results indicate that PPF reversed the effects of EB lesions on behavior in the open field and beam walking test.
