INTRODUCTION: Incarcerated individuals exhibit higher suicide rates compared to the general population. Investigating risk factors aids in developing effective public policies and interventions. The goal of this study was to assess and analyze factors predicting both suicidal thoughts and suicide attempts in a population of male incarcerated individuals who engage in the use of multiple psychoactive substances. METHODS: A cross-sectional observational study was conducted. A total of 174 male individuals deprived of liberty participated in the study, all of whom were serving a closed regime sentence during the data collection steps. Participants were assessed with the following instruments: the "Addiction Severity Index" (ASI-6) and the "Barratt Impulsiveness Scale" (BIS-11). RESULTS: Amongst our sample, prevalences of 36.7% in suicidal ideation and of 16.0% in suicide attempts were found. Impulsivity (OR = 1.098, 95% CI: 1.008 - 1.197), social support (OR = 0.281, 95% CI: 0.085 - 0.925), witnessing someone being killed or beaten (OR = 5.173, 95% CI: 2.143 - 12.486), cigarette use (OR = 3.309, 95% CI: 1.063 - 10.293), and cocaine use (OR = 2.678, 95% CI: 1.040 - 6.897) were also found to be associated with suicidal ideation. No significant associations were found between drug use and suicide attempts. CONCLUSION: A high prevalence of suicidal behaviors was observed in the study's sample, with findings demonstrating that impulsivity moderately differentiates the groups 'with' and 'without' suicidal ideation. Traumatic life events and substance use were also associated with suicide ideation, while social support was established as a protective factor for it.

Objective: Understand the motivations and expectations of pregnant women using psychoactive substances during prenatal care. Methods: A qualitative study developed in the light of Alfred Schütz's Theoretical Framework of Phenomenological Sociology, in which 25 pregnant women using psychoactive substances, belonging to a Family Health Strategy, participated. Data production took place between August and November 2022. Results: Two units of meanings emerged: (i) social influences for the performance of prenatal care and (ii) expectation regarding the care to be received by the health professional. Pregnant women do pre-natal due to family influences, for fear of losing their children due to loss of guardianship and concern about the well-being and development of the baby. And, the expectations are that they receive good attention, feel safe when they are attended to by health professionals and also that they are understood and have a relationship of trust. Conclusion: Pregnant women who use psychoactive substances bring motivations for prenatal care linked to the past, such as influences from family members and previous experiences. As for expectations, they are related to the child's health and the care expected by professionals. Finally, strategies to reduce harm during pregnancy of users of psychoactive substances are fundamental for the effectiveness of care.

Objetivo. Conocer las motivaciones y expectativas de las gestantes consumidoras de sustancias psicoactivas durante la atención prenatal. Método. Se trata de un estudio cualitativo basado en el marco teórico de la sociología fenomenológica de Alfred Schütz, en el que participaron 25 gestantes consumidoras de sustancias psicoactivas pertenecientes a una Estrategia de Salud Familiar de una unidad de salud en Brasil. Los datos se recogieron entre agosto y noviembre de 2022. Resultados. Emergieron dos unidades de significado: (i) influencias sociales para la atención prenatal y (ii) expectativas sobre la atención recibida por el profesional de salud. Las gestantes acuden al control prenatal por influencias familiares, miedo a separarse de sus hijos por pérdida de la tutela y preocupación por el bienestar y desarrollo del bebé. Sus expectativas son recibir una buena atención, sentirse seguras con los profesionales sanitarios y ser comprendidas y escuchadas a través de una relación de confianza. Conclusión. Las embarazadas usuarias de sustancias psicoactivas tienen motivaciones para el cuidado prenatal relacionadas con su pasado, como influencias familiares y experiencias individuales previas. En cuanto a las expectativas, están relacionadas con la salud del niño y los cuidados esperados que les brindarán los profesionales de salud. Por último, el diseño de estrategias para reducir los daños durante el embarazo de las consumidoras de sustancias psicoactivas es fundamental para un cuidado eficaz.


Objetivo. Compreender as motivações e expectativas de gestantes usuárias de substâncias psicoativas durante a realização do pré-natal. Métodos. Estudo de abordagem qualitativa desenvolvida à luz do referencial Teórico da Sociologia fenomenológica de Alfred Schütz, em que participaram 25 gestantes usuárias de substâncias psicoativas, pertencentes a uma Estratégia Saúde da Família. A produção de dados ocorreu entre os meses de agosto a novembro de 2022.Resultados. Emergiram duas unidades de significados: (i) influências sociais para a realização do pré-natal e (ii) expectativa em relação ao cuidado a ser recebido pelo profissional de saúde. As gestantes realizam o pré-natal por influências de familiares, por receio de afastamento dos filhos decorrente de perda da tutela e pela preocupação com o bem-estar e desenvolvimento do bebê. E, as expectativas é de que recebam um bom atendimento, sintam segurança nos profissionais de saúde e que sejam compreendidas e ouvidas por meio de uma relação de confiança. Conclusão. As gestantes usuárias de substâncias psicoativas trazem motivações para a realização do pré-natal atreladas ao passado, como influências de familiares e experiências prévias. Quanto as expectativas, estão relacionadas à saúde da criança e o cuidado esperado pelos profissionais. Por fim, as estratégias para reduzir danos durante a gestação de usuárias de substâncias psicoativas é fundamental para a efetividade do cuidado.

RESUMO Os benzodiazepínicos, medicamentos mais prescritos no mundo, são extremamente úteis no manejo de patologias relativas ao sistema nervoso central, porém, nem sempre sua prescrição está relacionada com uma patologia. Este estudo buscou identificar os fatores influenciadores na prescrição de benzodiazepínicos na Atenção Básica do Sistema Único de Saúde e suas consequências, em uma região com alta vulnerabilidade social. Foram utilizados os princípios de um estudo qualitativo com amostra intencional de médicos prescritores construída com a ajuda dos Informantes-Chave e com tamanho (n = 10) definido pelo princípio da saturação teórica. A coleta de dados ocorreu mediante entrevistas semiestruturadas nas unidades básicas onde os profissionais atuavam. Os temas identificados relativos aos motivos da prescrição foram: relativos ao paciente (demanda do paciente; problemas sociais); relativos ao medicamento (dependência); relativos ao prescritor (uso irracional). A prescrição do benzodiazepínico deu-se devido às baixas condições sociais dos usuários nessa região e as consequências dessa conjuntura (vulnerabilidade social, problemas angustiantes de difícil resolução). Educação permanente para profissionais, ações interprofissionais e integralidade do cuidado foram demandas dos profissionais. O problema extrapola a ação do médico, e são sugeridas políticas públicas que garantam, às populações com essas características, acesso a saúde, trabalho e educação.


ABSTRACT Benzodiazepines, the most prescribed medications in the world, are extremely useful in managing conditions related to the central nervous system, however, their prescription is not always related to a pathology. This study sought to identify the factors influencing the prescription of benzodiazepines in Primary Care of the Unified Health System and their consequences, in a region with high social vulnerability. The principles of a qualitative study with intentional sampling of prescribing physicians were used, constructed with the help of Key Informants and with a size (N=10) defined by the principle of theoretical saturation. Collection took place through semi-structured interviews in the basic units where the professionals worked. The themes identified regarding the reasons for prescription were Patient-related (patient demand; social problems); Relating to medication (dependence); Relative to the prescriber (irrational use). The prescription of benzodiazepine was due to the low social conditions of users in this region and the consequences of this situation (social vulnerability, distressing problems with difficult resolution). Continuous education for professionals, interprofessional actions, and comprehensive care were demands of the professionals. The problem goes beyond the doctor's scope of action, and public policies that guarantee such populations access to health, work, and education are suggested.

PURPOSE: Substance use disorders (SUDs) increase the risk and severity of infectious diseases, including coronavirus disease 2019 (COVID-19). Adults with a co-occurring SUD and psychiatric disorder were studied to elucidate the association between SUD severity and (1) COVID-19 vaccination status, (2) receptivity to a one-session intervention with a pharmacist advocating the benefits of vaccination, and (3) acceptance of referral for vaccination following the intervention. METHODS: COVID-19 vaccination status was recorded in 460 adults with SUD (324 males and 136 females) upon entry into inpatient treatment. A 2-parameter item response theory (IRT) model quantified SUD severity. Pharmacist-delivered intervention, modeled after the screening, brief intervention, and referral to treatment (SBIRT) protocol, was offered to unvaccinated participants. RESULTS: Higher SUD severity was associated with a lower vaccination rate. Nicotine, opioid, and sedative use disorders were most frequently associated with unvaccinated status. SUD severity was not associated with receptivity to intervention advocating vaccination or subsequent acceptance of a referral for vaccination. The portion of the sample that received the intervention was over 7 times more likely to accept a referral for vaccination when compared to participants who rejected the intervention (20.8% vs 2.8%). CONCLUSION: Pharmacist-administered intervention produced motivation for vaccination in a number of recipients; however, receptivity to the intervention was not related to SUD severity.

Introducción: La relación entre las toxicomanías y el suicidio se asocian en su función como agente causal y precipitante de la conducta suicida. Objetivo: Determinar los factores pronósticos de la conducta suicida en pacientes adictos a sustancias psicoactivas. Métodos: Se desarrolló un estudio observacional transversal. Se trabajó con el universo, constituido por los pacientes adictos atendidos en el Hospital Militar Central "Dr. Carlos J. Finlay", desde 2016 a 2019. Las variables estudiadas fueron: conducta suicida, función ejecutiva, edad, sexo, características clínicas del consumo, sustancia consumida, deterioro cognitivo, ansiedad y depresión. Se utilizaron técnicas de la estadística descriptiva e inferencial. Resultados: Se estudiaron 257 pacientes con consumo, fundamentalmente de alcohol y marihuana, con edad promedio de 34 años; predominó el sexo masculino (89 %). La conducta suicida estuvo presente en el 53,7 % (n= 138). Los principales factores pronósticos en los pacientes fueron: necesidad subjetiva de consumo (odd ratio-OR= 3,4), depresión (OR= 3,3), deterioro cognitivo (OR= 3,0), tener 30 años de edad o menos al iniciar el consumo (OR= 2,6), antecedentes familiares de adicción (OR=2,3), nivel escolar de 9no grado (OR= 2,0), recaer antes de los 6 meses de tratamiento (OR= 1,1), tiempo consumiendo de 20 años o menos (OR= 3,0) y consumir otras drogas diferentes al alcohol (OR= 2,1). Conclusiones: Los principales factores pronósticos de la conducta suicida en los pacientes adictos estudiados son la necesidad subjetiva de consumo, la depresión y el deterioro cognitivo.


Introduction: The relationship between drug addiction and suicide is associated in its function as a causal and precipitating agent of suicidal behavior. Objective: To determine the prognostic factors of suicidal behavior in patients addicted to psychoactive substances. Methods: An observational and cross-sectional study was developed. It worked with the universe, made up of addicted patients treated at the Central Military Hospital "Dr. Carlos J. Finlay" from 2016 to 2019. The variables studied were: suicidal behavior, executive function, age, sex, clinical characteristics of consumption, substance consumed, cognitive impairment, anxiety and depression. Descriptive and inferential statistical techniques were used. Results: 257 patients with consumption, mainly of alcohol and marijuana, with an average age of 34 years were studied; the male sex predominated (89%). Suicidal behavior was present in 53.7% (n= 138). The main prognostic factors in the patients were: subjective need for consumption (odd ratio-OR= 3.4), depression (OR= 3.3), cognitive impairment (OR= 3.0), being 30 years of age or less at the start of the consumption (OR= 2.6), family history of addiction (OR= 2.3), 9th grade school level (OR= 2.0), relapse before 6 months of treatment (OR= 1.1), time consuming 20 years or less (OR= 3.0) and consuming drugs other than alcohol (OR= 2.1). Conclusions: The main prognostic factors of suicidal behavior in the addicted patients studied are: the subjective need for consumption or craving, depression and cognitive impairment.

Objetivos: Descrever as principais manifestações bucais associadas ao uso das drogas lícitas e ilícitas mais utilizadas atualmente, bem como os cuidados que devem ser tomados durante o atendimento odontológico desses pacientes. Revisão de literatura: Na revisão foi abordada a classificação das substâncias químicas, as principais manifestações orais e os cuidados que o cirurgião-dentista deve ter durante o atendimento odontológico. As substâncias descritas foram álcool, tabaco, benzodiazepínicos, maconha, anfetaminas e cocaína/crack. As complicações de saúde bucal associadas ao abuso de drogas podem resultar da exposição direta dos tecidos orais às drogas durante o fumo ou ingestão, da interação biológica das drogas com a fisiologia normal da cavidade oral e dos efeitos das drogas na função cerebral que resultam em um espectro de comportamentos de dependência, como comportamento de risco, falta de higiene e descuido. Considerações finais: O consumo de substâncias psicoativas, seja a curto ou longo prazo, pode desencadear consequências, tanto na saúde geral como na saúde bucal do usuário. Sob essa perspectiva, o cirurgião-dentista deve estar ciente das manifestações orais e sistêmicas causadas pelas substâncias psicoativas e estar capacitado para diagnosticar, tratar ou também encaminhar o usuário, se necessário, visando um cuidado integral e multiprofissional.


Objective: to describe the main oral manifestations associated with the use of the most commonly used licit and illicit drugs today, as well as the care that should be taken during the dental care of these patients. Literature review: The review covered the classification of chemical substances, the main oral manifestations and the care that dentist should take during dental care. The substances described were alcohol, tobacco, benzodiazepines, marijuana, amphetamines and cocaine/crack. Oral health complications associated with drug abuse can result from direct exposure of oral tissues to drugs during smoking or ingestion, from the biological interaction of drugs with the normal physiology of the oral cavity, and from the effects of drugs on brain function that result in a spectrum of addictive behaviors, such as risk behavior, poor hygiene and carelessness. Final considerations: The consumption of psychoactive substances, whether in the short or long term, can have consequences for both the general health and the oral health of the user. From this perspective, the dentist must be aware of the oral and systemic manifestations caused by psychoactive substances and be able to diagnose, treat or also refer the user, if necessary, aiming for comprehensive, multi-professional care.

Women drug-users face significant challenges in accessing and adhering to outpatient treatment. However, this issue has been underexplored. The study aims to understand the experiences of a group of women undergoing drug use treatment. Between March and December 2021, a qualitative phenomenological interpretative study was carried out through semi-structured interviews with 16 women attending a drug-dependence care center in Catalonia, Spain. Data were processed through content analysis. The results indicate that, despite perceiving a positive impact of the treatment, women experience vulnerabilities related to stigmatization, lack of social support, and insufficient coverage of specific needs, impacting therapeutic follow-up. The findings emphasize the need to enhance resources for specialized care and promote a social support network where women drug-users actively participate.


Las mujeres consumidoras de drogas se enfrentan a grandes desafíos en el acceso y la adherencia al tratamiento ambulatorio. Sin embargo, esta cuestión ha sido poco estudiada. El objetivo del estudio es comprender las experiencias de un grupo de mujeres en tratamiento por consumo de drogas. Entre marzo y diciembre de 2021, se realizó un estudio cualitativo fenomenológico interpretativo mediante entrevistas semiestructuradas a 16 mujeres usuarias de un centro de atención a las drogodependencias de Cataluña (España). Los datos se procesaron mediante el análisis de contenido. Los resultados indican que las mujeres, pese a percibir un impacto positivo del tratamiento, viven realidades de vulnerabilidad relacionadas con la estigmatización, la falta de apoyo social y una baja cobertura de necesidades específicas con implicaciones en el seguimiento terapéutico. Los hallazgos enfatizan la necesidad de mejorar los recursos para una atención especializada y promover una red de apoyo social donde participen activamente las mujeres consumidoras de drogas.

ABSTRACT Objective: to evaluate a mobile application developed to prevent the use of alcohol and/or other drugs among adolescent students. Method: methodological study with a quantitative approach carried out from July 2018 to July 2021 based on the stages of Contextualized Learning Design for technology development: analysis, design, development, implementation and evaluation. The content evaluation was carried out by 22 experts. The application's graphic design and usability were tested by 13 teenagers representing the target audience. Results: the mobile application "Educação Para Prevenção" [Education For Prevention] addressed the following psychoactive substances: alcohol, marijuana, cigarettes, ecstasy, crack, cocaine, inhalants (glue and poppers) and amphetamines, dealing with topics such as concepts, physiological and behavioral effects, use during adolescence, prevention and harm reduction strategies. The average Content Validity Index score was 0.87. There was a high level of agreement among adolescents regarding the graphic design and the usability (96.6%) of the application. Conclusion: the results show that the experts and the adolescents considered the application valid, and nurses will be able to use it as a facilitating instrument in health education actions in schools, aiming to prevent the use of alcohol and/or other drugs among adolescents. It is recommended that an intervention study be carried out to evaluate the effect of the technology developed.


RESUMEN Objetivo: evaluar una aplicación móvil desarrollada para prevenir el consumo de alcohol y/u otras drogas por parte de estudiantes adolescentes. Método: estudio metodológico, con enfoque cuantitativo, realizado de julio de 2018 a julio de 2021, basado en las etapas del Diseño Instruccional Contextualizado para la construcción de tecnología: análisis, diseño, desarrollo, implementación y evaluación. La evaluación del contenido se llevó a cabo por 22 expertos. El aspecto gráfico y la usabilidad de la aplicación fueron testeados por 13 adolescentes que representaban al público objetivo. Resultados: la aplicación móvil "Educação Para Prevenção" (Educación para la Prevención) abordó sustancias psicoactivas: alcohol, marihuana, cigarrillos, éxtasis, crack, cocaína, inhalantes (pegamento/lanzaperfume) y anfetaminas, abordando temas como conceptos, efectos fisiológicos y conductuales, uso durante la adolescencia, estrategias de prevención y reducción de daños. El Índice de Validación de Contenido promedio fue de 0,87. Hubo un alto nivel de acuerdo entre los adolescentes sobre el aspecto gráfico y la usabilidad (96,6%) de la aplicación. Conclusión: los resultados demuestran que expertos y adolescentes consideraron válida la aplicación, por lo que los enfermeros podrán utilizarla como instrumento facilitador en acciones de educación en salud en la escuela, con el objetivo de prevenir el uso de alcohol y/u otras drogas entre los adolescentes. Se recomienda realizar un estudio de intervención para evaluar el efecto de la tecnología desarrollada.


RESUMO Objetivo: avaliar um aplicativo móvel desenvolvido para prevenir o uso de álcool e/ou outras drogas por estudantes adolescentes. Método: estudo metodológico, de abordagem quantitativa, realizado no período de julho de 2018 a julho de 2021, baseado nas etapas do Design Instrucional Contextualizado para construção de tecnologia: análise, desenho, desenvolvimento, implementação e avaliação. A avaliação de conteúdo foi realizada por 22 especialistas. O aspecto gráfico e a usabilidade do aplicativo foram testados por 13 adolescentes representantes do público-alvo. Resultados: o aplicativo móvel "EPP: Educação Para Prevenção" abordou as substâncias psicoativas: álcool, maconha, cigarro, ecstasy, crack, cocaína, inalantes (cola e lança-perfume) e anfetaminas, tratando de temas como conceitos, efeitos fisiológicos e comportamentais, uso durante a adolescência, estratégias de prevenção e redução de danos. A média do Índice de Validação de Conteúdo foi de 0,87. Verificou-se elevado nível de concordância entre os adolescentes quanto ao aspecto gráfico e à usabilidade (96,6%) do aplicativo. Conclusão: os resultados demonstram que os especialistas e os adolescentes consideraram o aplicativo válido, e os enfermeiros poderão utilizá-lo como um instrumento facilitador nas ações de educação em saúde na escola, visando à prevenção do uso de álcool e/ou outras drogas entre adolescentes. Recomenda-se a realização de um estudo de intervenção para avaliar o efeito da tecnologia desenvolvida.

RESUMEN Las mujeres consumidoras de drogas se enfrentan a grandes desafíos en el acceso y la adherencia al tratamiento ambulatorio. Sin embargo, esta cuestión ha sido poco estudiada. El objetivo del estudio es comprender las experiencias de un grupo de mujeres en tratamiento por consumo de drogas. Entre marzo y diciembre de 2021, se realizó un estudio cualitativo fenomenológico interpretativo mediante entrevistas semiestructuradas a 16 mujeres usuarias de un centro de atención a las drogodependencias de Cataluña (España). Los datos se procesaron mediante el análisis de contenido. Los resultados indican que las mujeres, pese a percibir un impacto positivo del tratamiento, viven realidades de vulnerabilidad relacionadas con la estigmatización, la falta de apoyo social y una baja cobertura de necesidades específicas con implicaciones en el seguimiento terapéutico. Los hallazgos enfatizan la necesidad de mejorar los recursos para una atención especializada y promover una red de apoyo social donde participen activamente las mujeres consumidoras de drogas.


ABSTRACT Women drug-users face significant challenges in accessing and adhering to outpatient treatment. However, this issue has been underexplored. The study aims to understand the experiences of a group of women undergoing drug use treatment. Between March and December 2021, a qualitative phenomenological interpretative study was carried out through semi-structured interviews with 16 women attending a drug-dependence care center in Catalonia, Spain. Data were processed through content analysis. The results indicate that, despite perceiving a positive impact of the treatment, women experience vulnerabilities related to stigmatization, lack of social support, and insufficient coverage of specific needs, impacting therapeutic follow-up. The findings emphasize the need to enhance resources for specialized care and promote a social support network where women drug-users actively participate.

ABSTRACT BACKGROUND: Adolescence is characterized by complex and dynamic changes, often involving experimentation, including the use of psychotropic substances. Although it is well-established that recreational psychotropic drugs are associated with suicide ideation in adults, evidence of this association in adolescents remains limited. OBJECTIVE: To investigate the relationship between suicide ideation and psychotropic recreational drug use among adolescents. DESIGN AND SETTING: Systematic review with meta-analysis developed at Universidade Federal de Uberlândia (UFU) and Universidade Estadual de Campinas (UNICAMP), Brazil. METHODS: A search across eight electronic databases for observational studies, without language or publication year restrictions, was conducted. The Joanna Briggs Institute tool was used to assess the risk of bias. Random-effects meta-analyses and odds ratios were used to measure the effects. RESULTS: The search yielded 19,732 studies, of which 78 were included in the qualitative synthesis and 32 in the meta-analysis. The findings indicated that suicidal ideation was 1.96 times more likely (95% confidence interval, CI = 1.47; 2.61) for adolescents who used some drug recurrently and 3.32 times more likely (95%CI = 1.86; 5.93) among those who abused drugs. Additionally, adolescents who used cannabis were 1.57 times more likely (95%CI = 1.34; 1.84) to experience suicide ideation compared with non-users, while cocaine users had 2.57 times higher odds (95%CI = 1.47; 4.50). CONCLUSIONS: Psychotropic recreational drug use is associated with suicidal ideation among adolescents regardless of current or previous use, abuse, or type of substance used. RESUMO: Objetivo: compreender as ações de cuidado prestadas pelos trabalhadores de saúde aos usuários de substâncias psicoativas no território. Materiais e Método: pesquisa qualitativa, descritiva e exploratória. Desenvolvida em duas Estratégias Saúde da Família de um município do Rio Grande do Sul-Brasil. Realizou-se entrevistas semiestruturadas com trabalhadores de saúde e utilizou-se análise de conteúdo temática. Resultados: fos dados foram organizados em duas categorias: ações no território: cuidado em liberdade? E, fragilidades do território. Percebeu-se que os trabalhadores enfrentam desafios na promoção do cuidado em liberdade aos usuários, tais como a fragilidade e o retrocesso das políticas públicas. Considerações finais: é relevante a resistência dos trabalhadores frente aos retrocessos e o fortalecimento de outras estratégias de cuidado no território, promovendo cuidado em liberdade e integral aos usuários de substâncias psicoativas.


ABSTRACT Objective: To understand the care actions provided by health workers to users of psychoactive substances in the territory. Materials and Method: Qualitative, descriptive, and exploratory research. It was carried out in two Family Health Strategies in a municipality in Rio Grande do Sul, Brazil. Semi-structured interviews were conducted with health workers, and thematic content analysis was used. Results: The data was organized into two categories: actions in the territory and care in freedom. And the frailties of the territory. It was noted that workers face challenges in promoting care and freedom for users, such as the fragility and setbacks of public policies. Final considerations: It is important for workers to resist setbacks and to strengthen other care strategies in the territory, promoting free and comprehensive care for users of psychoactive substances.


RESUMEN: Objetivo: Conocer las acciones de atención prestadas por el personal de salud a los usuarios de sustancias psicoactivas en el territorio. Materiales y Métodos: Investigación cualitativa, descriptiva y exploratoria. Fue realizada en dos Estrategias de Salud de la Familia en un municipio de Rio Grande do Sul, Brasil. Se realizaron entrevistas semiestructuradas a trabajadores de salud y se utilizó el análisis temático de contenido. Resultados: Los datos se organizaron en dos categorías: las acciones en el territorio: ¿el cuidado en libertad? y las fragilidades del territorio. Se observó que los trabajadores enfrentan desafíos para promover el cuidado en libertad de los usuarios, como la fragilidad y el retroceso de las políticas públicas. Consideraciones finales: es importante que los trabajadores resistan los retrocesos y fortalezcan otras estrategias de atención en el territorio, promoviendo la atención en libertad e integral a los usuarios de sustancias psicoactivas.

Resumo Objetivo: Discutir a possível função de insumo das práticas artísticas e culturais no contexto da Redução de Danos (RD). Métodos: Pesquisa etnográfica desenvolvida no Ponto de Cultura do Centro de Convivência "É de Lei", localizado em São Paulo-SP, em dois momentos: no primeiro, em outubro de 2016, realizou-se observação participante das atividades artísticas e culturais. No segundo, em novembro de 2016, realizaram-se entrevistas semiestruturadas com profissionais e frequentadores. O estudo identificou e descreveu práticas artísticas e culturais, bem como investigou suas possíveis relações com os insumos em RD na produção do laço social. A análise e a discussão dos dados foram embasadas por conceitos na interface entre a Saúde Coletiva e a Psicanálise. Resultados e Discussão: A pesquisa caracterizou o insumo em RD como promotor de vínculo entre o toxicômano e quem o oferta, com potencial para deslocar o sujeito da relação solitária estabelecida com a substância na direção dos laços sociais que possibilitam exercer sua condição de sujeito singular na cultura. Conclusão: O estudo considerou que as práticas artísticas e culturais podem exercer a função de insumos em RD no que concerne ao estabelecimento de laços sociais.


Abstract Objective: To discuss the possible input function of artistic and cultural practices in the context of Harm Reduction (HR). Methods: Ethnographic research developed at the Ponto de Cultura of the Centro de Convivência "É de Lei", located in São Paulo-SP, which took place in two moments: in the first, in October 2016, participative observation of artistic and cultural activities was carried out. In the second, in November 2016, semi-structured interviews were carried out with professionals and regulars. The study identified and described artistic and cultural practices, as well as investigated their possible relationships with HR inputs in the production of the social bond. The discussion of the data was based on concepts at the interface between public health and psychoanalysis. Results and Discussion: The research characterized the input in HR as a promoter of a bond between the drug addict and whoever offers it with the potential to divert the subject from the established solitary relationship with the substance towards the creation of social ties that stimulates the exercise of their condition as a singular subject in culture. Conclusion: The study concluded that artistic and cultural practices can play the role of inputs in HR with regard to the establishment of social bonds.

BACKGROUND: Psychoactive substance use among pregnant women has reached alarmingly high rates. Our aim was to characterize the psychiatric and clinical profiles of pregnant crack users in Brazil. METHODS: This was a cross-sectional study of 24 pregnant crack users admitted to a referral hospital for psychiatric disorders in pregnant women, in Porto Alegre, Brazil, over three years. The following instruments were applied: a clinical-obstetric questionnaire; the condensed version of the Addiction Severity Index; a diagnostic interview for psychoactive substance use based on DSM-5; the Mini International Neuropsychiatric Interview for DSM-IV; and the Semi-Structured Clinical Interview for DSM-IV Axis II Personality Disorders (SCID-II). RESULTS: Most patients had severe crack dependence and used other substances, such as tobacco, cannabis, and alcohol. The median duration of crack use was three years, ranging between three and 12 years. Most women subsisted from illegal or informal activities; a fifth had previously been arrested and often had relationship problems. Twenty percent had HIV (n = 5), and 37.5% (n = 9) had syphilis. Borderline personality disorder was the most prevalent mental condition (62.5%), followed by suicidal tendencies (45.8%), hypomanic episodes due to substance use (37.5%), and past major depressive episodes (33.3%). CONCLUSIONS: An alarmingly high prevalence of consumption of other drugs, psychiatric disorders, and difficult-to-treat personality disorders was observed in our study. Investigating the psychiatric profile of women who use substances is essential to minimize the impacts on the mother and child, optimize therapeutic approaches to comorbidities, and enable more effective relapse prevention.

OBJECTIVE: Sublingual (SL) buprenorphine is a cornerstone of care in the treatment of adult opioid use disorder. Recent studies have demonstrated its advantages in the management of neonatal opioid withdrawal syndrome (NOWS). Commercially available SL tablets and transdermal patches are not amenable to neonatal use, and published compounding formulas of SL solutions contained undesirable excipients, including ethanol, sugars, and preservatives. The objective of this research is to explore the stability of a novel SL buprenorphine formulation free of alcohol, sugars, and preservatives. METHODS: A 0.075 mg/mL buprenorphine solution was prepared by diluting the commercial injectable solution with normal saline and packaged into polyethylene terephthalate amber prescription bottles and polypropylene amber oral syringes and stored in refrigeration. Quality assessments were conducted by visual, pH, and high-performance liquid chromatography (HPLC) analysis immediately after preparation, and at 7 and 14 days of storage. RESULTS: There were neither visual nor pH changes detected through 14 days. HPLC analysis indicated that all samples retained >99% initial buprenorphine concentration. Drug concentration increased slightly in the oral syringe after day 7, probably due to moisture loss. No degradation peaks were observed in chromatograms. CONCLUSIONS: This novel buprenorphine is free of alcohol, sugar, and preservatives, and it may offer a significant safety advantage for NOWS patients. Additional clinical studies are recommended to verify the bioavailability and efficacy of this formulation.

Fundamento el tabaquismo en la adolescencia constituye un problema de salud pública mundial, al punto de ser considerado la principal causa prevenible de enfermedad y muerte prematura. Objetivo describir el comportamiento del tabaquismo en adolescentes. Métodos estudio de corte transversal, realizado en el periodo septiembre-diciembre de 2022. De un universo de 1561 adolescente de 15-19 años de edad, se seleccionó una muestra de 1444 (92,50 %) mediante muestreo deliberado. Las variables del estudio: edad, sexo, fumador, exfumador, edad de inicio, años fumando, número de cigarrillos consumidos por día, carga tabáquica, dependencia y motivación. Resultados la prevalencia de tabaquismo resultó de 17,94 %. El 0,83 % de los adolescentes expresó haber abandonado el hábito (exfumadores). El 55,98 % tenía entre 17-18 años de edad, y el 77,61 % perteneció al sexo masculino. El 64,86 % comenzó a fumar entre los 12 y 14 años, el 52,51 % fumaba desde hacía 1-2 años; mientras un 59,85 % consumía entre 6-10 cigarrillos diarios. La carga tabáquica recibida por el 44,34 % resultó 0,25-0,50. El 57,08 % declaró dependencia física baja, y el 81,13 % motivación baja. Conclusiones la prevalencia del tabaquismo en los adolescentes de Moa fue similar a la reportada por otras investigaciones. Resulta preocupante que más de la mitad de la población estudiada consuma cigarros desde los 12-14 años, y que, a pesar de mostrar bajos niveles de dependencia, no sientan motivación por abandonar este mal hábito.


Foundation smoking in adolescence constitutes a global public health problem, is being considered the main preventable cause of illness and premature death. Objective to describe smoking behavior in adolescents. Methods cross-sectional study, carried out from September to December 2022. From a universe of 1,561 adolescents aged 15-19, a sample of 1,444 (92.50%) was selected through deliberate sampling. The study variables: age, sex, smoker, ex-smoker, age of initiation, years of smoking, number of cigarettes consumed per day, smoking load, dependence and motivation. Results the prevalence of smoking was 17.94%. 0.83% of adolescents expressed having quit the habit (ex-smokers). 55.98% were between 17-18 years of age, and 77.61% were male. 64.86% started smoking between 12 and 14 years old, 52.51% had been smoking for 1-2 years; while 59.85% consumed between 6-10 cigarettes a day. The smoking burden received by 44.34% was 0.25-0.50. 57.08% declared low physical dependence, and 81.13% reported low motivation. Conclusions the prevalence of smoking in Moa adolescents was similar to that reported by other investigations. It is worrying that more than half of the population studied consumes cigarettes from the age of 12-14, and that, despite showing low levels of dependence; they do not feel motivated to abandon this bad habit.

Objetivo: evaluar la capacidad de los Centros de Atención Psicosocial de Alcohol y Otras Drogas 24 horas para manejar situaciones de crisis de las personas que consumen AOD en la atención integral. Método: estudio cuantitativo, evaluativo y longitudinal, realizado de febrero a noviembre de 2019. La muestra inicial estuvo compuesta por 121 personas que consumen AOD, que recibieron atención integral en situaciones de crisis en dos Centros de Atención Psicosocial para Alcohol y Otras Drogas 24 horas en el centro de São Paulo. Los mismos fueron reevaluados después de 14 días de atención. La capacidad para manejar la crisis se evaluó mediante un indicador validado. Los datos se analizaron utilizando estadísticas descriptivas y modelos de regresión de efectos mixtos. Resultados: sesenta y siete personas que consumen AOD completaron el follow-up (54,9%). Durante la atención de las situaciones de crisis, nueve personas que consumen AOD (13,4%; p=0,470) fueron derivadas a otros servicios de la red de salud: siete por complicaciones clínicas, una por intento de suicidio y una por hospitalización psiquiátrica. La capacidad de los servicios para manejar situaciones de crisis fue del 86,6%, fue considerada positiva. Conclusión: los dos servicios evaluados fueron capaces de manejar situaciones de crisis en su área de influencia, evitando internaciones y contando con el apoyo de la red cuando fue necesario, logrando así los objetivos de desinstitucionalización.


Objective: to assess the ability of 24-hour Psychosocial Care Centers specialized in Alcohol and Other Drugs to handle the users' crises in comprehensive care. Method: a quantitative, evaluative, and longitudinal study was conducted from February to November 2019. The initial sample consisted of 121 users, who were part of the comprehensibly care in crises by two 24-hour Psychosocial Care Centers specialized in Alcohol and other Drugs in downtown São Paulo. These users were re-evaluated 14 days after admission. The ability to handle the crisis was assessed using a validated indicator. The data were analyzed using descriptive statistics and regression of mixed-effects models. Results: 67 users (54.9%) finished the follow-up period. During crises, nine users (13.4%; p=0.470) were referred to other services from the health network: seven due to clinical complications, one due to a suicide attempt, and another for psychiatric hospitalization. The ability to handle the crisis in the services was 86.6%, which was evaluated as positive. Conclusion: both of the services analyzed were able to handle crises in their territory, avoiding hospitalizations and enjoying network support when necessary, thus achieving the de-institutionalization objectives.


Objetivo: avaliar a capacidade dos Centros de Atenção Psicossocial Álcool e outras Drogas 24 horas em manejar situações de crise dos usuários no acolhimento integral. Método: estudo quantitativo, avaliativo e longitudinal, realizado de fevereiro a novembro de 2019. A amostra inicial foi composta por 121 usuários, acolhidos integralmente em situações de crise por dois Centros de Atenção Psicossocial Álcool e outras Drogas 24 horas do centro de São Paulo. Estes foram reavaliados após 14 dias de acolhimento. A capacidade de manejar a crise foi avaliada por um indicador validado. Os dados foram analisados por estatística descritiva e por regressão de modelos de efeitos mistos. Resultados: sessenta e sete usuários concluíram o follow-up (54,9%). Durante o acolhimento às situações de crise, nove usuários (13,4%; p=0,470) foram encaminhados para outros serviços da rede de saúde: sete por complicações clínicas, um por tentativa de suicídio e um para internação psiquiátrica. A capacidade de manejo das situações de crise pelos serviços foi de 86,6%, avaliada como positiva. Conclusão: os dois serviços avaliados foram capazes de manejar situações de crise no próprio território, evitando internações e tendo apoio da rede quando necessário, atingindo assim, os objetivos da desinstitucionalização.

Background: The use of benzodiazepines as a treatment for insomnia can have side effects such as impaired coordination causing falls in adults and even dependence. Objective: To assess the factors associated with dependence on benzodiazepines in patients with insomnia. Methods: Observational, cross-sectional, prospective, and analytical study, at the first level of care. Patients older than 18 years with a diagnosis of insomnia and a benzodiazepine prescription were selected. The dependency was measured with the International Neuropsychiatric Interview. Results: 107 patients were included. Median age 67 years, predominantly female (72%), 74% attended secondary school or more, 71% had more than 3 years of diagnosis, 84% used clonazepam. The 54% presented dependency. In the bivariate analysis, schooling RM 0.392 (95%CI: 0.15-0.96) p = 0.038, moderate and severe clinical insomnia RM 3.618 (95%CI: 1.44-9.08) p = 0.005 and more than 3 years of diagnosis RM 2.428 (95%CI: 1.03-5.71) p = 0.040. In the multivariate model, schooling (p = 0.084), years of diagnosis (p = 0.062) and frequency of consumption (p = 0.065) obtained an R2 of 0.13. Conclusions: Primary schooling showed a lower risk of presenting dependence on benzodiazepines. The risk was increased in those with more than 3 years of diagnosis, and in those with moderate and severe insomnia.


Introducción: el uso de benzodiacepinas como tratamiento para el insomnio puede tener efectos secundarios, como el deterioro de la coordinación que puede provocar caídas en adultos e, incluso, dependencia. Objetivo: evaluar los factores asociados a la dependencia a benzodiacepinas en pacientes con insomnio. Material y métodos: estudio observacional, transversal, prospectivo y analítico, llevado a cabo en el primer nivel de atención. Se seleccionaron pacientes mayores de 18 años con diagnóstico de insomnio y prescripción de benzodiacepina. La dependencia se midió con la Entrevista Neuropsiquiátrica Internacional. Resultados: se incluyeron 107 pacientes, la mediana de edad fue de 67 años, con predominio del sexo femenino (72%), el 74% cursó educación secundaria o más, el 71% tenía más de tres años con diagnóstico de insomnio, el 84% usaba clonazepan. El 54% presentó dependencia. En el análisis bivariado, la escolaridad primaria mostró una razón de momios (RM) de 0.392 (IC95%: 0.15-0.96), p = 0.038; el insomnio clínico moderado y grave RM de 3.618 (IC95%: 1.44-9.08) p = 0.005, y más de tres años de diagnóstico con una RM de 2.428 (IC95%: 1.03-5.71) p = 0.040. En el modelo multivariado, la escolaridad (p = 0.084), los años de diagnóstico (p = 0.062) y la frecuencia de consumo (p = 0.065) obtuvieron una R2 de 0.13. Conclusiones: los pacientes con escolaridad primaria mostraron un menor riesgo de presentar dependencia a benzodiacepinas. El riesgo se incrementó en los pacientes con más de tres años de diagnóstico y en aquellos con insomnio moderado y grave.

Zolpidem is a non-benzodiazepine hypnotic drug that works as a positive modulator of Gamma-Amino Butyric Acid-A (GABA-A) receptors, with high selectivity for α1 subunits. Given this selective binding, the drug has a strong hypnotic activity. Social isolation during the SARS-CoV-2 pandemic has contributed to increased rates of anxiety, depression, and insomnia. As a result, studies have pointed to a possible increase in the indiscriminate use of drugs with sedative effects, such as Zolpidem, during the pandemic. The aim of this work was to present prospective evidence that warns of the possibility of the abusive use of Zolpidem even after the pandemic. High rates of addiction to this drug have been reported around the world after the emergence of the coronavirus. Data from the National Survey on Drug Use and Health and from Medicaid support the continuing growth in prescription and indiscriminate use of Zolpidem during the pandemic and afterward. Therefore, there is enough evidence to support the indiscriminate use of this drug since the beginning of the pandemic. Rates of indiscriminate use of sedatives may continue to increase in the post-pandemic period, especially if strict control measures are not taken by health authorities.

Objectives: To examine drug overdose records in Brazil from 2000 to 2020, analyzing trends over time in overdoses and overall sociodemographic characteristics of the deceased. Methods: Using data from the Brazilian Mortality Information System (Sistema de Informações sobre Mortalidade), we identified records from 2000-2020 in which the underlying cause-of-death was one of the following codes: X40-X45 (accidental poisoning), X60-X65 (intentional poisoning), or Y10-Y15 (undetermined intentionality poisoning). The Brazilian dataset included 21,410 deaths. We used joinpoint regression analysis to assess changes in trends over time. Results: People who died of drug overdoses in Brazil between 2000 and 2020 had a mean age of 38.91 years; 38.45% were women, and 44.01% were identified as White. Of the overdose deaths, 44.70% were classified as intentional and 32.12% were classified as unintentional. Among the identified drugs, stimulants were the most common class. However, most records did not report which drug was responsible for death. Conclusion: Sociodemographic trends in overdose deaths in Brazil must guide country-specific policies. Nevertheless, data collection protocols must be improved, particularly regarding the drug used in overdoses.
