RESUMO
Background: Establishing adequate communication is part of the therapeutic process and of the integral approach to the oncology patient. The SPIKES protocol defines a series of general recommendations aimed at facilitating this process. To date, there is no questionnaire that makes it possible to personalize the communication of bad news in a systematized way. Some studies support the hypothesis that personality influences the communicative modes; therefore, the aim of this work is to try to establish nuances in the SPIKES protocol based on personality traits. Methods: Single-center, observational, prospective, descriptive and correlational study, conducted on a sample of 51 oncology patients based on a personality questionnaire and a communication questionnaire (based on the SPIKES protocol). Results: The scores recorded in all domains of the communication questionnaire were high. There was no significant correlation with the personality questionnaire domains. Conclusions: There are certain needs tending towards universality in the communication of bad news that the SPIKES protocol adequately reflects; it can be considered the gold standard. However, it is not possible to establish nuances in it according to personality traits based on the results of this work. In the strategy phase, attention should be paid to life and family planning in the context of oncologic disease.
BACKGROUND: Communication of bad news plays a critical role in the physician-patient relationship, and a variety of consensus guidelines have been developed to this purpose, including the SPIKES protocol. However, little is known about physicians' attitudes towards breaking bad news and to be trained to deliver it. This study aimed to develop and validate a self-report questionnaire to assess physicians' attitudes towards principles of the SPIKES protocol and training on them. METHODS: The Breaking Bad News Attitudes Scale (BBNAS) was administered to 484 pediatricians and 79 medical students, recruited at two scientific conferences and two medical schools in Brazil. The questionnaire structural validity, reliability, and associations with other variables were tested. RESULTS: The BBNAS showed adequate validity and good reliability, with two factors measuring attitudes towards the SPIKES strategy for braking bad news (α = 0.81) and the possibility to be trained on it (α = 0.77), respectively. CONCLUSION: The novel questionnaire is a psychometrically sound measure that provides information on physicians' agreement with the SPIKES protocol. The BBNAS can provide useful information for planning training and continuing education programs for clinicians on communication of bad news using the SPIKES as a framework.

OBJECTIVE: Here we present cases of focal epilepsy with affective symptoms analyzing seizure characteristics, EEG pattern, treatment, and outcome. METHODS: A multicenter, descriptive, retrospective study was conducted evaluating 18 patients with self-limited epilepsy who presented with seizures with affective symptoms seen between April 2000 and April 2018 at eight Argentinian centers. RESULTS: Eighteen patients had focal seizures with affective symptoms; all of them had affective symptoms characterized by sudden fright or terror and screaming. Seizures started with manifestations of sudden fright or terror manifested by a facial expression of fear; consciousness was mildly impaired in 15/18 patients. Eleven of the patients also had autonomic manifestations, such as pallor, sweating, and abdominal pain. In addition, four of these 11 patients had ictus emeticus and one also presented with unilateral deviation of the eyes and head. Speech arrest, salivation, glottal noises, and chewing or swallowing movements were observed in 2/18 patients at the onset of the affective seizures. Two others also had mild asymmetric dystonic seizures involving both hands and arms. Three patients had tonic deviation of the mouth involving the lips and tongue as well pharyngeal and laryngeal muscles, resulting in anarthria and drooling. Two patients had brief hemifacial focal clonic seizures. CONCLUSION: Affective manifestations associated or not with motor and/or autonomic manifestations and associated with typical EEG features of the idiopathic focal epilepsies of childhood is a particular presentation of self-limited focal epilepsy in childhood.

Abstract Communicating bad news is one of the most frequent activities in hospitals, for which some recommendations have been adapted to the needs within the coronavirus-2 disease (COVID-19) context. This document presents nine steps to deliver bad news (face to face or remotely) adapted to the COVID-19 context from two international protocols (SPIKES and GRIEV_ING). The importance of promoting physical and emotional self-care skills in health personnel is also described, as well as psychological first aid strategies to address the emotional response of the family member who receives the news. Finally, the limitations and advantages of the proposal should be considered.
Differences in the intrinsic properties of intralaminar thalamo-striatal neurons such as expressing low-threshold-spikes (LTS) or after hyperpolarizing potentials (AHPs) of different duration have been attributed to different maturation stages. However, two morphological types: "diffuse" and "bushy" have been described. Therefore, we explored whether electrophysiological differences persist in adult mice using whole cell recordings. Some recorded neurons were identified by intracellular labeling with biocytin and double labeling with retrograde or anterograde tracings using Cre-mice. We classified these neurons by their AHPs during spontaneous firing. Neurons with long duration AHPs, with fast and slow components, were mostly found in the parafascicular (Pf) nucleus. Neurons with brief AHPs were mainly found in the central lateral (CL) nucleus. However, neurons with both AHPs were found in both nuclei in different proportions. Firing frequency adaptation differed between these neuron classes: those with prolonged AHPs exhibited firing frequency adaptation with fast and slow time constants whereas those with brief AHPs were slow adapters. Neurons with more prolonged AHPs had significant higher input resistances than neurons with brief AHPs. Both cell classes could fire in two modes: trains of single action potentials at depolarized potentials or high frequency bursts on top of LTS at more hyperpolarized potentials. LTS were probably generated by Cav3 calcium channels since they were blocked by the selective antagonist TTA-P2. About 11% of neurons with brief AHPs and 55% of neurons with prolonged AHPs do not show LTS and bursts, even when potassium currents are blocked.

PURPOSE: We retrospectively analyzed the seizure characteristics, EEG pattern, treatment, and outcome in a series of patients with self-limited epilepsy with centrotemporal spikes (SLECTS) who presented with unusual clinical manifestations. METHOD: A retrospective, descriptive, multicenter study was conducted evaluating 46 patients with SLECTS who had seizures with an unusual semiology. We collected data from patients with SLECTS seen at eight Argentine centers between April 1998 and April 2018. RESULTS: Thirteen patients (28.2 %) had seizures with affective symptoms characterized by sudden fright and autonomic disturbances and mild impairment of consciousness. Eleven patients (24.8 %) had frequent seizures characterized by unilateral facial sensorimotor symptoms, oropharyngolaryngeal manifestations, and speech arrest with sialorrhea only when awake. Seven patients (15.3 %) started with opercular epileptic status with unilateral or bilateral clonic seizures of the mouth with speech arrest and sialorrhea when awake and during sleep. Seven patients (15.3 %) had postictal Todd's paralysis after unilateral clonic seizures with facial and limb movements lasting between 60 min and 130 min. Six patients (13 %) had negative myoclonus, two in a unilateral upper limb, two in a unilateral lower limb, and the remaining two patients had frequent falls. One patient (2.1 %) had focal sensorimotor seizures characterized by unilateral numbness in the cheeks and one upper limb, additional to unilateral facial clonic seizures, speech arrest, and sialorrhea. The remaining patient (2.1 %) had sporadic focal tonic-dystonic seizures in the left upper limb only during sleep. CONCLUSION: In our study, we found evidence of the existence of unusual clinical cases of SLECTS with typical EEG patterns and an excellent prognosis.

During an infectious disease outbreak, mathematical models and computational simulations are essential tools to characterize the epidemic dynamics and aid in design public health policies. Using these tools, we provide an overview of the possible scenarios for the COVID-19 pandemic in the phase of easing restrictions used to reopen the economy and society. To investigate the dynamics of this outbreak, we consider a deterministic compartmental model (SEIR model) with an additional parameter to simulate the restrictions. In general, as a consequence of easing restrictions, we obtain scenarios characterized by high spikes of infections indicating significant acceleration of the spreading disease. Finally, we show how such undesirable scenarios could be avoided by a control strategy of successive partial easing restrictions, namely, we tailor a successive sequence of the additional parameter to prevent spikes in phases of low rate of transmissibility.
RESUMO
PURPOSE: Self-limited epilepsy with centrotemporal spikes, formerly called benign epilepsy with centrotemporal spikes, or rolandic epilepsy, is an age-related and well-defined epileptic syndrome. Since seizures associated with rolandic spikes are infrequent and usually occur during sleep, and repetitive or prolonged EEG recording for diagnostic purposes is not necessary for diagnosis, reports of ictal video-electroencephalographic seizures in this syndrome are rare. We aimed to show ictal video-EEG of typical rolandic seizures. METHODS: We report the ictal video-EEG recordings of two children with rolandic epilepsy who presented typical rolandic seizures during routine recording. RESULTS: Case 1: A 9-year-old boy, with normal development, had his first seizure at 8 years old, characterized by paresthesia in his left face, blocking of speech, and drooling. Carbamazepine was started with seizure control. Case 2: A 10-year-old boy, with normal development, started with focal seizures during sleep, characterized by eye and perioral deviation, and speech arrest at age of 7. He started using oxcarbazepine. Both patients underwent routine electroencephalography for electroclinical diagnosis and presented a seizure. CONCLUSION: Although self-limited epilepsy with centrotemporal spikes is a very common epileptic syndrome, seizure visualization is very difficult, and these videos may bring didactical information for recognition of this usual presentation of benign childhood focal epilepsy.

INTRODUCTION: Acquired Immunodeficiency Syndrome (AIDS) is the clinical presentation of immunological deficiency caused by the Human Immunodeficiency Virus (HIV). The disclosure of this diagnosis is a communication that deeply affects the lives of the people involved, both professionals and patients. OBJECTIVE: This study aims to analyze the disclosure of the HIV / AIDS diagnosis, considering its psychosocial, affective and neurocognitive impacts. METHODS: To understand the communicational process, collective health studies were used and psychological impacts were analyzed in the light of Donald Winnicott's psychoanalytic concepts. The methodology is exploratory and descriptive, with a mixed approach and a qualitative predominance. The instruments for data collection were: patient record; semistructured interview, which compared stages of the SPIKES Protocol; the Mini Mental State Examination and the Hospital Anxiety and Depression Scale. Content analysis was carried out in ten interviews, referenced in Laurence Bardin. RESULTS AND FINAL CONSIDERATIONS: The results showed that the communication of the diagnosis had several psychosocial impacts on the subjects and the reception based on the principles of the SPIKES protocol proved to be an excellent foundation for diagnostic communication, especially when associated with sufficiently good environmental arrangements, such as family support and access to information. thus promoting greater adherence to treatment.

A major frontier in neuroscience is to find neural correlates of perception, learning, decision making, and a variety of other types of behavior. In the last decades, modern devices allow simultaneous recordings of different operant responses and the electrical activity of large neuronal populations. However, the commercially available instruments for studying operant conditioning are expensive, and the design of low-cost chambers has emerged as an appealing alternative to resource-limited laboratories engaged in animal behavior. In this article, we provide a full description of a platform that records the operant behavior and synchronizes it with the electrophysiological activity. The programming of this platform is open source, flexible, and adaptable to a wide range of operant conditioning tasks. We also show results of operant conditioning experiments with freely moving rats with simultaneous electrophysiological recordings.

PURPOSE: The purpose of this study was to demonstrate the association between benign rolandic epilepsy of childhood (BREC) and central auditory processing disorders (CAPDs) and to test the hypothesis that an early onset of BREC could be associated with more cases of CAPD. METHOD: This study has a retrospective cross-sectional design conducted from January 2006 to January 2016 including 93 patients with BREC and without intellectual disability, dyslexia, and attention-deficit hyperactivity disorders. All patients were evaluated for central auditory processing (CAP), and its presence or absence was compared with age of seizure onset. RESULTS: In all patients, audiometric test results were normal, and in 43 cases (46.2%), CAPD was detected. There was no significant statistical difference in the mean age of seizure onset. CONCLUSION: A significant proportion of children with BREC will have a diagnosis of CAPD at school age.

In this study, we aimed to evaluate the attentional and executive functions in patients with benign childhood epilepsy with centrotemporal spikes (BCECTS) with and without attention-deficit hyperactivity disorder (ADHD) compared with controls and compared with patients with ADHD without epilepsy. We evaluated 12 patients with BCECTS and ADHD (66.7% boys; mean age of 9.67years); 11 children with non-ADHD BCECTS (63.6% boys; mean age of 11.91years); 20 healthy children (75% boys; mean age of 10.15years); and 20 subjects with ADHD without epilepsy (60% boys; mean age of 10.9years). We used a comprehensive battery of neuropsychological tests to evaluate attentional and executive functions in their broad domains. Patients with BCECTS and ADHD had worse performance in Conners' Continuous Performance Test II (reaction time standard error [p=0.008], variability [p=0.033], perseverations [p=0.044] and in reaction time interstimuli interval [p=0.016]). Patients with ADHD showed worse performance in Trail Making Test B errors [p=0.012]. In conclusion, patients with BCECTS and ADHD had worse executive and attentional performance compared with controls than non-ADHD patients with BCECTS. Regardless of the presence of epilepsy, ADHD also negatively impacted executive and attentional functions but in different executive subdomains compared with patients with epilepsy.

Benign epilepsy with centrotemporal spikes, aka benign rolandic epilepsy, is characterized by partial motor crisis in childhood and electroencephalography showing point discharges in medial centrotemporal regions. The condition has been recognized as benign due to the absence of evident neurological deficits. However, in recent years several publications have appeared in international medical literature in which its favorable evolution is questioned. The objective of the present study is to update some criteria differing from the good prognosis initially stated. It constitutes a fundamental stage in the history of epilepsy, since for the first time a case of focal or partial epilepsy was being described in which presumably there did not exist an underlying cortical lesion. The evolution of this form of epilepsy may display features denying its benignity. It is recommended that in the future a study be conducted in our service confirming the criteria expounded in international medical literature(AU)

PURPOSE: To present a retrospective study of 13 children with benign epilepsy with centrotemporal spikes (BECTS), also known as benign rolandic epilepsy (BRE), associated with generalized spikes and waves as the only EEG manifestation at onset. METHOD: Charts of children with typical clinical criteria of BRE electroclinically followed-up between February 2000 and February 2015 were reviewed. RESULTS: Among 309 patients who met the electroclinical criteria of BRE, we identified 13 children who presented with the typical clinical manifestations but who, on the EEG, only had generalized paroxysms at onset that continued along the course of the syndrome. Generalized spike-and-wave discharges were observed in all patients when awake and during sleep (100%). During the evolution no particular electroclinical pattern was observed. The patients responded well to antiepileptic drugs, such as valproic acid and levetiracetam. Outcome was good in all patients. CONCLUSIONS: We found evidence that patients with BRE may have generalized EEG discharges at onset as the sole manifestation lasting throughout the course of the syndrome. In some, focal paroxysms developed later. The course was benign. In our group of patients, clinical features and evolution were similar to those of typical cases of BRE. Response to valproic acid and levetiracetam was found to be particularly good.

Abstract: Rolandic Epilepsy (ER) or epilepsy with centrotemporal spikes, is a focal and selflimited epilepsy and it is the most frequent and well-recognized idiopathic epilepsy. It is characterized by an age of presentation between 3 and 13 years, with stereotyped seizures during sleep, at the beginning or on awakening, which can be focal, motor and sensory. The electroencephalogram shows a normal base with interictal epileptiform activity, centrotemporal spikes with poor slow wave development, which can be unilateral o bilateral. In recent years, an atypical presentation of Rolandic Epilepsy (ERA) has been described, with presence of other types of seizures, with poor response to treatment, continued seizures and associated neuropsychological complications and deterioration of the electroencephalogram, in some cases with continuous or almost continuous activity during sleep. Our work aims to characterize the patients that are controlled in the Red Salud UC-Christus and to describe the presence of possible risk factors associated with the atypical evolution of Rolandic Epilepsy. We studied 16 patients with typical ER and other 6 with ERA, both groups with an age average of 6 years at time of debut presentation, more frequent in males, most with good school performance and normal neurological examination. In ER there were focal prevalence seizures and 62% had an EEG with a normal base. The EEG showed occasional to very frequent epileptiform discharges. In ERA, only 33% of the EEG had a normal base, and it most frequently showed generalized seizure. Most of the patients followed up with continuous or nearly continuous discharges in the EEG during REM sleep. There was worse response to FAEs in ERA. We conclude that this study is consistent with that described in the current literature for patients with ER and ERA, we recommend physicians to consider the presence of possible initial of belated indicators of atypical forms of ER.

The nitrogen fertilization of grassland was held during two consecutive years, using four dosages of nitrogen (0, 100, 200 and 300 kg/ha/year). These areas were dry to no-till corn. Was used a basal fertilization of 70 kg of N/ha for maize cultivation, applied in coverage. Agronomic evaluations were carried out (stand, height of plants and insertion of cob, number of spikes, number of grains on the cob and stay-green) and corn production (production of silage dry matter production, production of spikes) collected from point of silage. Dosages of N applied in pastures only affected the percentage of ears of corn and the cob weight. There was interaction between dose and cultivate for the percentage of dry matter of the bracts and the amount of rows of spikegrains. The residual effect of nitrogen after growing Marandu pastures and Xaraés does not affect subsequent crops, making it necessary to total fertilization of culture.(AU)
