INTRODUCTION: Bilateral acute iris transillumination (BAIT) is a clinical condition characterized by acute bilateral loss of iris pigment epithelium and iris transillumination. Because of the similar presentation, it is commonly confused with anterior uveitis, making it an important differential diagnosis for this condition. CASE DESCRIPTION: We report the case of a 60-year-old male patient who presented all the characteristics of BAIT after undergoing systemic treatment with moxifloxacin and that had the intraocular pressure (IOP) successfully controlled with selective laser trabeculoplasty (SLT). CONCLUSION: SLT might be an effective therapeutic option and should be considered for the control of IOP in BAIT.

PURPOSE: Prostaglandin analogs (PGAs) are first-line treatments for ocular hypertension (OHT) and open-angle glaucoma (OAG). However, frequent side effects and high costs hinder patient's compliance resulting in disease progression. Evidence suggests selective laser trabeculoplasty (SLT) may be considered a first-line treatment for OHT and OAG due to its safety profile, minor side effects, and reduced costs. Considering that PGAs and SLT share action mechanisms, it is hypothesized that previous PGA therapy may affect subsequent SLT efficacy. Therefore, we analyzed if PGAs reduce SLT efficacy. METHODS: An evidence-based review was performed to assess the safety and efficacy of SLT in patients previously treated with PGAs. For this purpose, we performed an extensive literature search using the National Library of Medicine's PubMed and Google Scholar database for all English language articles published until May 2021. RESULTS: There is evidence of non-superiority of PGAs therapy versus SLT for OHT and OAG. A multicenter, randomized, observer-masked clinical trial (RCT) of untreated OHT and OAG patients concluded that SLT should be offered as the first-line treatment for these patients. This study was supported by a meta-analysis of RCTs, comparing SLT efficacy versus antiglaucoma drugs only, with the advantage of an SLT lower rate of adverse effects. CONCLUSIONS: Cost-effectiveness, patient compliance, and antiglaucoma drugs' side effects, including higher surgical failure, favor consideration of SLT as first-line therapy for OAG and OHT. Furthermore, SLT efficacy does not seem to be affected by prior PGA administration; however, larger cohort, comparative, multicenter RCTs are necessary to answer this question.

PURPOSE: To report an uncommon case of hyperopic shift and corneal haze, flattening and thinning following a single session of selective laser trabeculoplasty (SLT), and provide longitudinal clinical data and serial analyses of corneal profile through Scheimpflug imaging. Furthermore, a careful literature review was undertaken to determine possible risk factors for this complication. OBSERVATIONS: A 47-year-old woman presented with blurred vision and mild corneal edema and haze three days following routine SLT. Ten days later, corneal edema resolved, but she was still symptomatic. Refraction exam revealed a significant hyperopic shift and high astigmatism (+4.00-6.00 x 80° versus -11.50 -0.75 x 170° at baseline). Scheimpflug imaging revealed a partial collapse of the corneal architecture, with an expressive central cornea flattening (>10 diopters) and thinning (>190µm). Over six months, refractometric and corneal parameters gradually (but not completely) returned to their pre-operative values. CONCLUSIONS AND IMPORTANCE: This tetrad of clinical findings - hyperopic shift, corneal haze, flattening and thinning - following SLT is referred herein by the authors as selective trabeculoplasty-induced keratopathy (STIK), which is a rare but serious complication. A critical analysis based on this case findings and on the few others reported so far suggests high-myopic patients to be more predisposed to this complication, as all reported cases had -5.0 diopters or more. Future studies are warranted to determine whether this complication can be prevented.

Pigment dispersion syndrome (PDS) represents a clinical spectrum of a relatively common and usually underdiagnosed phenomenon produced by spontaneous pigment dispersion from the iris into the anterior segment. PDS is often bilateral, has no gender predisposition, and presents at a young age, particularly in myopes. Although most patients experiencing an episode of pigment dispersion are asymptomatic, extreme photophobia, ocular pain, redness, and blurred vision may occur. Other characteristic signs are iridolenticular contact, concave iris configuration, 360° peripheral iris transillumination, and pigment deposition on the anterior chamber angle or the corneal endothelium (Krukenberg spindle). Early PDS diagnosis is crucial to detect patients with pigment-related ocular hypertension (POHT) that can eventually lead to pigmentary glaucoma (PG). The latter represents a sight-threatening condition in which mechanical, environmental, and genetic factors contribute to optic nerve damage. In this review, we update the pathogenic mechanisms involved in the clinical spectrum of the disease. We describe its clinical presentation, ophthalmologic manifestations, and complications, including the factors influencing the development of POHT and PG. Because PDS has variable clinical presentations that lead to misdiagnoses, we emphasize the differential diagnosis and the actual therapeutic strategies according to disease status.

Selective laser trabeculoplasty (SLT) has been used for treatment of primary open-angle glaucoma, ocular hypertension, pigmenter and pseudoexfoliative glaucoma being considered a low-risk procedure. Therefore, transitory and permanent adverse effects have been reported, including corneal changes, subclinical edema, and reduction in endothelial cells and in central corneal thickness. Despite rarer, serious corneal complications after SLT can be permanent and lead to visual impairment, central corneal haze, opacity and narrowing. The mechanism involves increase of vasoactive and chemotactic cytokines causing inflammatory infiltrate, destruction of stromal collagen by fibroblasts and increase of matrix metalloproteinases type 2, which impair reepithelization. SLT also increases free radical production and reduces antioxidant enzymes, resulting in endothelium damages. Low-power laser therapy (LPLT) has been used in regenerative medicine based on its biostimulatory and anti-inflammatory effects. Biostimulation occurs through the interaction of laser photons with cytochrome C oxidase enzyme, which activates intracellular biochemical cascades causing synthesis of a number of molecules related to anti-inflammatory, regenerative effects, pain relief and reduction in edema. It has been showed that LPLT reduces gene expression related to pro-inflammatory cytokines and matrix metalloproteinases, and it increases expression of growth factors related to its proliferative and healing actions. Although radiations emitted by low-power lasers are considered safe and able to induce therapeutic effects, researches based on experimental models for glaucoma could bring important data if LPLT could be an alternative approach to improve acceptation for patients undergoing SLT.

BACKGROUND: To identify success predictors and to study the role of the fellow untreated eye as a co-variable for adjustment of intraocular pressure (IOP) outcomes following selective laser trabeculoplasty (SLT) in early open-angle glaucoma (OAG) patients. METHODS: A case series was carried out. Patients with uncontrolled early OAG or ocular hypertension (inadequate IOP control requiring additional treatment) underwent SLT (one single laser session) performed by the same surgeon in a standardized fashion. The same preoperative medical regimen was maintained during follow-up for all patients. Post-treatment assessments were scheduled at week 1 and months 1, 2, and 3. In order to account for possible influence of IOP fluctuation on laser outcomes, post-laser IOP values of the treated eye of each patient were also analyzed adjusting for IOP changes (between visits variation) of the untreated fellow eye (adjusted analysis). Pre and post-laser IOP values were compared using paired t-test. Factors associated with the magnitude of IOP reduction were investigated using multiple regression analysis. RESULTS: A total of 45 eyes of 45 patients were enrolled. Mean IOP was reduced from 20.8 ± 5.1 to 14.9 ± 2.9 mmHg at month 3 (p < 0.001). Adjusted success rate (defined as IOP reduction ≥ 20%) was 64% and mean percentage of IOP reduction was 23.1 ± 14.3% at last follow-up visit. Considering unadjusted post-laser IOP values, it was found a 20% greater absolute IOP reduction (median [interquartile range] 6 mmHg [4-7] vs 5 mmHg [3-7]; p = 0.04), with a success rate of 76%. Although baseline IOP was significantly associated with both adjusted and unadjusted post-laser IOP reduction, a stronger association was found when unadjusted IOP values were considered (p < 0.001 and R 2 = 0.35; p < 0.001 and R 2 = 0.67, respectively). Age, mean deviation (MD) index, central corneal thickness and type of glaucoma were not significant predictors (p ≥ 0.150). CONCLUSIONS: In this group of patients with early OAG or ocular hypertension, our short-term results confirmed SLT as a safe and effective alternative for IOP reduction. Although better outcomes were found in eyes with higher preoperative IOP, this effect was mitigated when results were adjusted to the fellow untreated eye (to the influence of between visits-IOP fluctuations).

OBJETIVOS: Determinar eficacia y seguridad de la trabeculoplastia selectiva láser en pacientes portadores de glaucoma primario de ángulo cerrado, operados de cataratas y/o glaucoma, e identificar la frecuencia de los principales efectos adversos. MÉTODOS: Investigación observacional descriptiva prospectiva en 35 pacientes (50 ojos) portadores de glaucoma primario de ángulo cerrado sometidos a cirugía de cataratas y/o glaucoma, en el Instituto Cubano Oftalmológico Ramón Pando Ferrer entre mayo y noviembre de 2007. Se utilizó equipo Tango LT 5106-T, de Laserex. RESULTADOS: Predominaron pacientes femeninas, mayores de 60 años y piel no blanca, hipermetropes, elevada frecuencia de enfermedades crónicas no transmisibles. Se logró un 41,2 por ciento de reducción de la presión intraocular a los 6 meses. La técnica en pacientes con antecedentes de cirugía filtrante previa mostró resultados muy similares a los no intervenidos. La terapia mostró efectividad en 88 por ciento de los ojos, se logró disminuir la terapia médica hipotensora. Eventos adversos: picos tensionales, epiteliopatía corneal y dispersión de pigmentos. CONCLUSIONES: La técnica láser resultó ser eficaz para lograr control tensional y/o reducción de terapia médica antihipertensiva ocular en este tipo de pacientes


OBJECTIVES: To determine the efficiency and safety of selective laser trabeculoplasty in patients who suffered primary close angle glaucoma and were operated from cataract and/or glaucoma, and to identify the frequency of the main adverse effects. METHODS: Prospective descriptive and observational research study of 35 patients (50 eyes) carrying primary close angle glaucoma, who were operated on from cataract and/or glaucoma at Ramón Pando Ferrer Cuban Institute of Ophthalmology from May to November 2007. The equipment called Tango LT 5106-T from Laserex was used. RESULTS: Female patients, people over 60 years-old, Caucausians, hypermetropics, high frequency of non-communicable chronic diseases prevailed. After six months, the reduction of intraocular pressure amounted to 41,2 percent. In those patients with a history of previous filtering surgery, the use of this technique showed results very similar to those of the non-operated patients. The therapy proved to be effective in 88 percent of eyes and the medical hypotensive treatment was reduced. Some adverse events were blood pressure peaks, corneal epitheliopathy and pigment dispersion. CONCLUSIONS: Laser technique was effective to control blood pressure and/or reduction of ocular anti-hypertensive medical therapy in this type of patients

Se realiza una breve revisión bibliográfica sobre la trabeculoplastia selectiva con Nd YAG Láser de frecuencia doblada en la enfermedad glaucomatosa. El equipo Laserex Tango LT 5106-T Nd: YAG Láser (Ellex Medical Pty Ltd) para trabeculoplastia selectiva se encuentra en nuestro instituto desde marzo de 2006, única institución del país que dispone de este equipo hasta este momento. La alta esperanza de vida de nuestra población, que conlleva a un incremento en la incidencia de glaucoma y cataratas, induce a ser consecuentes con todo aquello que contribuya a mejorar la calidad de vida de los pacientes: esta circunstancia motivó esta revisión. Se presentan sus indicaciones, parámetros más utilizados, efectividad y complicaciones.


A brief literature review on selective trabeculoplasty with doubled frequency Nd:YAG laser in the treatment of glaucoma was made. Laserex Tango LT 5106-T Nd: YAG Laser (Ellex Medical Pty Ltd) equipment for selective trabeculoplasty is available in our institute since March 2006 (the only center having this equipment in our country up to the present). The high life expectancy of our population, with an increase of glaucoma and cataract incidence, prompts us to be consistent with everything that supports the improved quality of life of patients; this was the reason for this review. Indications, most used parameters, effectiveness and complications were presented.
