RESUMO

A avaliação do risco é um processo científico e sistemático que incorpora quantitativamente o perigo e a exposição a diversos agentes. O processo de avaliação do risco tem evoluído nos últimos anos, indo além da exposição a únicos agentes e vias de exposição para a caracterização do risco cumulativo a múltiplos agentes. As metodologias para avaliação do risco cumulativo não são harmonizadas o que pode tornar o processo complexo. Nesta linha, a abordagem do RISK21 promovida pelo Health Environmental Science Institute (HESI) pode contribuir para desmistificar o tema. A exposição combinada da ingestão de resíduos de praguicidas através da dieta e do uso residencial de produtos a base de piretróides pela população brasileira não são conhecidas. Os piretróides são praguicidas utilizados na lavoura, bem como em ambiente doméstico no controle de pragas. O mecanismo de toxicidade destes agentes é bem conhecido e de relevância para a saúde humana, pois atuam sobre a permeabilidade iônica dos canais de sódio sensíveis a voltagem (CSSV), produzindo efeitos na excitabilidade das terminações nervosas. Como os seres humanos são potencialmente expostos a estes agentes, portanto, torna-se importante compreender os riscos cumulativos da exposição a estes praguicidas pela população brasileira. O objetivo deste trabalho foi conduzir a avaliação do risco dos piretróides registrados no Brasil com base nos princípios do RISK21. A abordagem em etapas proposta pelo RISK21 demonstrou que o risco da ingestão crônica e aguda de resíduos de piretróides foi considerado aceitável. Além disso, não foi observada qualquer preocupação toxicológica decorrente da exposição residencial a estes agentes. Quando combinados os cenários da dieta aguda e residencial, também não foram observados níveis de preocupação, portanto, o risco foi considerado aceitável. A avaliação do risco dos piretróides registrados para o uso agrícola e residencial no Brasil com base nos principios do RISK21 foi uma importante etapa neste trabalho, uma vez que foi possível avaliar o risco e preocupações para cada um dos praguicidas de maneira rápida e visual. Além disso, mesmo considerando premissas altamente conservadoras, observou-se que a população exposta de maneira combinada a estes agentes não demonstrou um nível de preocupação para o cenário brasileiro


Risk assessment is a scientific and systematic approach that quantitatively incorporates hazard and exposure to agents' evaluation. The risk assessment process has evolved in recent years, going beyond exposure to single agents and pathways to characterize multiple agents' cumulative risk. Cumulative risk assessment methodologies are not harmonized, which can make the process complex. In this line, the RISK21 approach promoted by the Health Environmental Science Institute (HESI) can demystify the subject. The combined exposure of residue intake through diet and residential use of pyrethroid-based products by the Brazilian population is unknown. Pyrethroids are pesticides used in the crop as well as in a domestic environment in pest control. The mechanism of toxicity of these agents is well known and relevant to human health, as they act on the ionic permeability of voltage-sensitive sodium channels (VSSC), producing effects on the excitability of nerve endings. As human beings are potentially exposed to these agents, it is essential to understand the cumulative risks derived from the exposure to these pesticides by the Brazilian population. The objective of this research was to conduct the risk assessment based on the principles of RISK21 of pyrethroids registered in Brazil. The stepwise approach proposed by RISK21 demonstrated that the risk of chronic and acute ingestion of pyrethroid residues was considered acceptable. Furthermore, no toxicological concern stemming from residential exposure to these agents was observed. When acute and residential diet scenarios were combined, no levels of concern were also observed, so the risk was considered acceptable. The risk assessment based on the principles of RISK21 of pyrethroids registered for agricultural and residential use in Brazil was an essential step in this research since it was possible to assess the risk and concerns for each of the pesticides in a fast and visual way. Moreover, from highly conservative premises, it was observed that the population exposed in combination with these agents did not demonstrate a level of concern for the Brazilian scena

The air pollution emitted by petrochemical industrial complexes (PICs) may affect the respiratory health of surrounding residents. Previous meta-analyses have indicated a higher risk of lung cancer mortality and incidence among residents near a PIC. Therefore, in this study, a meta-analysis was conducted to estimate the degree to which PIC exposure increases the risk of the development of nonmalignant respiratory symptoms among residents. We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines to systematically identify, select, and critically appraise relevant research. Finally, we identified 16 study groups reporting 5 types of respiratory symptoms: asthma, bronchitis, cough, rhinitis, and wheezing. We estimated pooled odds ratios (ORs) using random-effect models and investigated the robustness of pooled estimates in subgroup analyses by location, observation period, and age group. We determined that residential exposure to a PIC was significantly associated with a higher incidence of cough (OR = 1.35), wheezing (OR = 1.28), bronchitis (OR = 1.26), rhinitis (OR = 1.17), and asthma (OR = 1.15), although the latter two associations did not reach statistical significance. Subgroup analyses suggested that the association remained robust across different groups for cough and bronchitis. We identified high heterogeneity for asthma, rhinitis, and wheezing, which could be due to higher ORs in South America. Our meta-analysis indicates that residential exposure to a PIC is associated with an increased risk of nonmalignant respiratory symptoms.

In rural populations, the proximity to areas with intensive pesticide application represents a risk factor of xenobiotic exposure. Here, we investigated whether newborns born to mothers residing in an area with intensive pesticide application show alterations in placental and neonatal morphometric standards, umbilical cord blood (UCB) biochemical parameters, and/or biomarkers related to oxidative stress and oxidative damage. Samples were collected from 151 healthy pregnant women residing in a rural area (rural group; RG) during the pesticide spraying (SS) and nonspraying (NSS) seasons, as well as from women from an urban population (control group; CG), and grouped according to the delivery type (vaginal or cesarean). In the vaginal delivery group, the placental weight and placental index were higher in the RG groups than in the CG (p = 0.01), whereas in the cesarean delivery group, newborn weight was lower in the RG-SS group than in the CG. In the RG-SS group, UCB erythrocyte osmotic fragility and the DNA damage index (DI) were higher, and superoxide dismutase (SOD) activity was lower than in the RG-NSS group. Acetylcholinesterase and SOD activities were found to be inversely correlated with the DI.

La zona frutihortícola del alto Valle de Río Negro, en Argentina, constituye una región de exposición residencial a plaguicidas en razón de la dinámica ambiental de estos compuestos y la situación geográfica de sus comunidades. Considerando que los comportamientos preventivos de la población dependen del nivel educativo, este trabajo se planificó con el objetivo de capacitar al personal de salud del hospital de Cinco Saltos, localidad situada en esta región, como agentes multiplicadores en el fortalecimiento de dichas conductas. Previo a la capacitación, se realizaron entrevistas domiciliarias en un asentamiento rural en el que, si bien el 92% de las mujeres estaba alfabetizada, el 81% desconocía el período anual de fumigaciones y sólo el 12% conocía las vías de incorporación. Se registraron conductas de riesgo para el uso doméstico de plaguicidas y para la exposición residencial. Aunque el 95% identificaba a los niños como el grupo social más vulnerable y el 58% estimaba que la exposición materna afecta al feto, solamente el 56 % tomaba precauciones durante el período de fumigación y el 92% utilizaba plaguicidas en el hogar. La encuesta diagnóstica aplicada al personal de salud reveló un importante grado de desconocimiento sobre la exposición indirecta. En base a la información recabada en ambas actividades se dictó un Taller sobre toxicología de plaguicidas cuya acreditación incluyó el diseño de un plan de trabajo para la transferencia hacia las comunidades de riesgo. También se llevaron a cabo acciones para la difusión de esta problemática en medios locales.


In Argentina, the agricultural area of High Valley of Río Negro and Neuquén, is a region of pesticide residential exposition in view of the environmental dynamics of these compounds and the geographic situation of their communities. Considering that people prevention behavior depends on their educational level, this work was designed with the purpose of preparing health workers of Cinco Saltos Hospital, location settled in this area, as multiplying agents to strengthen those conducts. Previously to this preparation, interviews were performed in a rural community where, though 92% of the women were alphabetized, 81% ignored the time covered by the annual fumigation period and only the 12% knew about pesticides routes of absorption. Risk behaviors were registered for domestic use of pesticides and for residential exposure. Although children were identified (95%) as the most vulnerable social group and 58% estimated that maternal exposition affects the fetus, only 56 % took cares during fumigation period and 92% used pesticides at home. A diagnostic poll performed with the health workers revealed an important lack of knowledge about indirect exposition. According to the information obtained in both activities a Workshop about pesticides toxicology was organized, whose approval included the design of a work plan to be carried out with the communities under risk. Additionally, actions were performed to diffuse this theme in local media.
