The hypothesis was tested that the performance of lactating ewes is affected by the supplementation level and pasture management. Two supplementation levels (0.5 and 1.0% of body weight, BW) and two pasture managements (mowed and non-mowed) were tested. Forty adult ewes (2 years old) with an average weight at lambing of 62.97 ± 7.0 kg (day 0) and an average the body condition score of 2.5 points (day 0) were evaluated. Verminosis was monitored with periodic deworming. The number of eggs per gram of feces (EPG) of ewes at lambing was used as a covariate for performance assessments. Ewes lost an average of 7.5 kg over the 90 days post-partum (12% BW). The weaning rate was 53%. The body condition of the ewes was influenced by the post-partum period. The ewes mobilized their body reserves in the first 30 days of lactation. Mowing management negatively affected the nutritive value of the forage. Supplementation with 0.5% BW was sufficient for nutritional management post-partum. Pasture management (mowed vs. not mowed) cannot prevent post-partum weight loss. Supplementation levels and pasture management altered the morphological and chemical components of the pasture.

RESUMO

Post-partum depression (PPD) with varying clinical manifestations affecting new parents remains underdiagnosed and poorly treated. This minireview revisits the pharmacotherapy, and relevant etiological basis, capable of advancing preclinical research frameworks. Maternal tasks accompanied by numerous behavioral readouts demand modeling different paradigms that reflect the complex and heterogenous nature of PPD. Hence, effective PPD-like characterization in animals towards the discovery of pharmacological intervention demands research that deepens our understanding of the roles of hormonal and non-hormonal components and mediators of this psychiatric disorder.

BACKGROUND: Women living in low- and middle-income countries are more exposed to known risk factors for depression occurrence and persistency over time. AIM: Our aim was to investigate the course of depression in the first 2 years postpartum among Brazilian women enrolled in a cash transfer program. METHOD: Longitudinal analysis of baseline (T0; mean 3.7 months postpartum) and first follow-up data (T1; mean 18.6 months postpartum) from a trial to assess the impact of a child development promotion program in 30 municipalities from six Brazilian states. The program does not include any interventions against maternal depression. The Edinburgh Postnatal Depression Scale (EPDS) at cutoff ⩾10 was applied. Women were categorized into four groups based on EPDS at T0 and T1: absence of depression, persistence, discontinuity, or emergence pattern. Adjusted Poisson regressions were run using a multilevel hierarchical model. RESULTS: Two thousand eight hundred sixty-three women were assessed. Prevalence of depression was 26.4% [24.8, 28.1] at T0 and 24.4% [22.8, 26.0] at T1. Persistence, discontinuation, and emergence were found in 14.1% [11.3, 17.6%], 12.8% [11.4, 14.3%], and 10.2% [8.0, 13.0], respectively. In adjusted analyses, the persistence pattern was directly associated with parity and inversely associated with schooling of the woman and of the child's father. Living with husband/partner and support from the child's father and family members during pregnancy were protective against persistence. The discontinuity and the emergence patterns were not associated with any of the exposure variables. CONCLUSIONS: Depressive symptoms were highly prevalent during the first 2 years postpartum. About half of the women with depression at T1 were persistent cases that could have been detected earlier. Screening for maternal depression should be an essential component in every encounter of women with health professionals in primary health care settings.

Endometritis is one of the most important causes of infertility in dairy cows, resulting in high economic losses in the dairy industry. Though the presence of a commensal uterine microbiota is now well established, the complex role of these bacteria in genital health, fertility, and susceptibility to uterine diseases remains unclear. In this study, we explore the endometrial microbiota through 16S rRNA gene profiling from cytobrush samples taken ex vivo from healthy, pregnant, and endometritis cows. There were no significant differences between healthy and pregnant cows, whose uterine microbiota were dominated by Streptococcus, Pseudomonas, Fusobacterium, Lactococcus and Bacteroides. Compared to pregnant and clinically healthy cows, the uterine bacterial community of endometritis cows was significantly decreased in species diversity (p < 0.05), reflecting uneven community composition in different patterns with either dominance of Escherichia-Shigella, Histophilus, Bacteroides and Porphyromonas or Actinobacteria.

Determining what and how much each environmental factor affects pregnancy is crucial to the sustainability and management of beef cow herds. The study evaluated through logistic regression the effect of environmental variables on the increase or reduction in the pregnancy rate of beef cows. The average pregnancy rate in the cows was 73 %, with an average age for the herd of 3.7 years. An increase in age of one year represented a 30 % increase in pregnancy, while a reduction of one year reduced the odds of pregnancy in the cows by 23 %. During the lactation period, an extra seven days' lactation reduced the odds of pregnancy by 12 %. For every seven days that weaning was brought forward, the cows' pregnancy odds increased by 14 %. An increase of 0.1 kg in average daily gain represented an increase of 17 % and 15 % in the odds of pregnancy during the first 60 days post-partum and 60 days to the end of the reproductive period. On the other hand, the loss of 0.1 kg in average daily gain resulted in a 14 % and 13 % reduction in the cows' pregnancy odds, respectively. Factors such as age, the precocity of calving in the calving season, the time the calf remains with the cow and better performance in cows between calving and the end of the mating are strategies that increase the chances of pregnancy in beef cows.(AU)

Introducción: de acuerdo a los datos de la Organización Mundial de la Salud la prevalencia de anemia ferropénica en el embarazo oscila entre el 20 y el 39% de los mismos en Latinoamérica, siendo asociada con resultados adversos para la salud. Materiales y métodos: se realizó un estudio observacional, descriptivo, de corte transversal, donde fue estudiada la frecuencia de la anemia en las embarazadas usuarias del Hospital Central de las Fuerzas Armadas, y su evolución posterior luego del embarazo. Resultados: se observó que el 15,7% de las pacientes presentaba anemia en el embarazo, con un buen porcentaje de pacientes que cumplían el tratamiento con hierro indicado, y que sólo un bajo porcentaje realizaba un control de la anemia luego del embarazo. Discusión: el porcentaje de pacientes con anemia en el embarazo en este centro asistencial se encuentra por debajo de los porcentajes establecidos en la literatura internacional, se analizan las causas y se verifica que existe una dificultad en el seguimiento de estas pacientes luego del embarazo.


Introduction: according to World Health Organization data, the prevalence of iron deficiency anemia in pregnancy ranges between 20 and 39% of pregnancies in Latin America, being associated with adverse health outcomes. Materials and Methods: an observational, descriptive, cross-sectional, descriptive study was carried out to study the frequency of anemia in pregnant women users of the Central Hospital of the Armed Forces of Montevideo, and its subsequent evolution after pregnancy. Results: it was observed that 15.7% of the patients had anemia during pregnancy, with a good percentage of patients who complied with the indicated iron treatment, and only a small number of patients underwent anemia control after pregnancy. Discussion: the percentage of patients with anemia in pregnancy in this health care center is below the percentages established in the international literature, the causes are analyzed and it is verified that there is a difficulty in the follow-up of these patients after pregnancy.


Introdução: de acordo com dados da Organização Mundial da Saúde, a prevalência de anemia por deficiência de ferro na gravidez varia entre 20 e 39% das gestações na América Latina, e está associada a resultados de saúde adversos. Materiais e Métodos: foi realizado um estudo observacional, descritivo, transversal e descritivo para estudar a freqüência da anemia em gestantes usuárias do Hospital Central das Forças Armadas, e sua posterior evolução após a gravidez. Resultados: observou-se que 15,7% das pacientes tiveram anemia durante a gravidez, com uma boa porcentagem de pacientes que cumpriram o tratamento com ferro indicado, e apenas uma baixa porcentagem teve controle da anemia após a gravidez. Discussão: a porcentagem de pacientes com anemia na gravidez em nosso centro de atendimento está abaixo das porcentagens estabelecidas na literatura internacional, analisamos as causas e verificamos que existe uma dificuldade em monitorar essas pacientes após a gravidez.

OBJECTIVE: COVID-19 in post-partum women is commonly overlooked. The present study assessed whether puerperium is an independent risk factor of COVID-19 related in-hospital maternal death and whether fatality is preventable in the Brazilian context. METHODS: We retrospectively studied the clinical data of post-partum/pregnant patients hospitalized with COVID-19 gathered from a national database that registered severe acute respiratory syndromes (SIVEP-Gripe) in Brazil. Logistic regressions were used to examine the associations of in-hospital mortality with obstetric status and with the type of public healthcare provider, adjusting for socio-demographic, epidemiologic, clinical and healthcare-related measures. RESULTS: As of 30 November 2021, 1943 (21%) post-partum and 7446 (79%) pregnant patients of age between 15 and 45 years with COVID-19 that had reached the clinical endpoint (death or discharge) were eligible for inclusion. Case-fatality rates for the two groups were 19.8% and 9.2%, respectively. After the adjustment for covariates, post-partum patients had almost twice the odds of in-hospital mortality compared with pregnant patients. Patients admitted to private (not-for-profit) hospitals, those that had an obstetric centre or those located in metropolitan areas were less likely to succumb to SARS-CoV-2 infection. Those admitted to the Emergency Care Unit had similar mortality risk to those admitted to other public healthcare providers. CONCLUSION: We demonstrated that puerperium was associated with an increased odds of COVID-19-related in-hospital mortality. Only part of the risk can be reduced by quality healthcare such as non-profit private hospitals, those that have an obstetric centre or those located in urban areas.

BACKGROUND: Few cluster-randomized trials have been performed in rural Guatemala. Our objective was to describe the feasibility, recruitment and retention in our cluster-randomized trial. METHODS: In our cluster-randomized trial, a range of contraceptives were brought to mothers' homes in rural Guatemala. RESULTS: Of 173 women approached, 33 were excluded. Of the 140 eligible women, 127 (91%) consented to participate. Of the 87 women who should have been assessed for the primary outcome, three were lost to follow-up, which represents a retention rate of 97%. CONCLUSIONS: Nurses who are both clinical providers and study staff can feasibly conduct research, which leads to high enrollment and retention rates.

BACKGROUND: Monitoring clients' experiences with contraceptive care is vital to inform quality improvement efforts and ensure fulfillment of individuals' human rights. The Quality of Contraceptive Counseling (QCC) Scale is a previously validated scale that comprehensively measures individuals' experiences receiving counseling in three subscales: Information Exchange, Interpersonal Relationship, and Disrespect and Abuse. We sought to better understand the correlation of client, provider, and visit factors with client-reported quality of contraceptive counseling in the public sector in two Mexican states using the QCC Scale. METHODS: This cross-sectional survey study used the QCC Scale total score and subscale scores as outcome variables. Explanatory variables included clients' age, LGBTTTIQ status, relationship status, number of children, education, and occupation; providers' gender and type of provider; and the reason for visit. Linear and logistic regression models assessed bivariate associations. Multivariable, multilevel mixed-effects models with clinic as a random effect were fit. All models used complete cases (n = 470). RESULTS: In the multilevel mixed-effects analyses, patients aged 35+ years reported worse Information Exchange (coefficient - 0.29, p = 0.01). Clients receiving care post-partum reported worse Information Exchange (coefficient - 0.25, p = 0.02) and worse total scores (coefficient - 0.15, p = 0.04) compared to clients seeking contraceptive information or methods. Clients who had 1+ children reported better Information Exchange (coefficient 0.21, p = 0.01) than those with no children. Though Disrespect and Abuse subscale scores were overall high (indicating high quality of care), we found a significant association between age and report of such negative experiences: clients in increasing age categories had increasingly higher adjusted odds of reporting no disrespect and abuse (aORs compared to the youngest group were 2.50 for those aged 19-24 years, p = 0.04; 4.53 for those 25-34 years, p = 0.01; and 6.11 for those 35+ years, p = 0.01.) CONCLUSIONS: Our findings align with previous results that younger clients have lower adjusted odds of reporting high-quality services in Mexico. There is a need for continued work supporting youth-friendly services in Mexico, and efforts should aim to ensure zero tolerance for disrespectful or coercive provider behaviors, such as pressuring or scolding clients. Improvements are also needed to ensure quality in counseling for post-partum clients, those aged 35+ years, and those without children.

AIM: P-glycoprotein (P-gp) plays a critical role in the excretion of xenobiotics into bile. Previous studies have demonstrated that prolactin (PRL) regulates biotransformation and bile salt transport. Here we investigate whether the capability of the liver to transport xenobiotics into bile is altered in hyperprolactinemic states studying the modulation of hepatic P-gp by PRL. METHODS: We used lactating post-partum rats (PP), as a model of physiological hyperprolactinemia (15 and 21 days after delivery: PP15 and PP21, respectively), and ovariectomized rats treated with PRL (300 µg/day, 7 days, via osmotic minipumps, OVX + PRL). Hepatic P-gp expression and activity were evaluated by western blotting and using rhodamine 123 as substrate in vivo, respectively. Since P-gp is encoded by Mdr1a and Mdr1b in rodents, we quantified their expression by qPCR in primary hepatocyte cultures exposed to 0.1 µg/ml of PRL after 12 h. To further study the mechanism of hepatic P-gp modulation by PRL, hepatocytes were pretreated with actinomycin D and then exposed to PRL (0.1 µg/ml) for 12 h. KEY FINDINGS: We found increased hepatic P-gp protein expression and activity in PP15 and OVX + PRL. Also, a significant increase in Mdr1a and Mdr1b mRNA levels was observed in primary hepatocyte cultures exposed to PRL, pointing out the hormone direct action. Actinomycin D prevented these increases, confirming a transcriptional up-regulation of P-gp by PRL. SIGNIFICANCE: These findings suggest the possibility of an increased biliary excretion of xenobiotics substrates of P-gp, including therapeutic agents, affecting their pharmaco/toxicokinetics in hyperprolactinemic situations.

Resumo O presente estudo é parte de uma pesquisa mais ampla sobre maternidade e cuidados na primeira infância, e tem como objetivo investigar como as mulheres vivenciam o puerpério atualmente. Para tanto, foi realizada uma pesquisa qualitativa, na qual foram entrevistadas dez mulheres das camadas médias da população carioca, atuantes profissionalmente, com idades entre 29 e 37 anos, casadas, legalmente ou não, heterossexuais primíparas e com o filho(a) entre 8 meses e 2 anos de idade. Os resultados foram analisados segundo o método de análise de conteúdo na sua vertente categorial. Para atingir os objetivos deste estudo, serão discutidas as seguintes categorias de análise: baby blues; rede de apoio; e a história não foi bem assim… Os resultados apontaram o puerpério como um período idealizado pelas mulheres, mas vivido com sofrimento e instabilidade emocional. Constatamos a importância da rede de apoio para a mãe e o bebê.


Résumé La présente étude s'inscrit dans le cadre d'une recherche plus large sur la maternité et les soins de la petite enfance, et vise à examiner comment les femmes vivent actuellement le post-partum. Une recherche qualitative a été menée, au cours de laquelle dix femmes âgées de 29 à 37 ans, de la classe moyenne de Rio de Janeiro ont été interviewées. Elles étaient mariées (officiellement ou non), hétérosexuelles, primipares et avaient un enfant âgé de 8 mois à 2 ans. Les résultats ont été analysés selon la méthode d'analyse du contenu catégorique. Pour atteindre les objectifs de cette étude, les catégories d'analyse suivantes ont été discutées : baby blues ; réseau de soutien ; et « Cela ne s'est pas tout à fait passée comme-ça... ¼. Les résultats montrent que la période post-partum était idéalisée par les femmes, mais vécue avec souffrance et instabilité émotionnelle. Nous soulignons l'importance du réseau de soutien pour les mères et les enfants.


Resumen El presente estudio forma parte de una investigación más amplia acerca de la maternidad y los cuidados en la primera infancia y tiene como objetivo investigar la forma en la que las mujeres viven el postparto en la actualidad. Por lo anterior, se llevó a cabo una investigación cualitativa, en la que fueron entrevistadas diez mujeres de clase media de la población de Río de Janeiro, activas profesionalmente, con edades entre los 29 y los 37 años, casadas legalmente o no, heterosexuales, primíparas y con hijo/a de entre 8 meses y 2 años de edad. Se analizaron los resultados según la técnica de análisis de contenido en su vertiente categorial. Para alcanzar los objetivos de este estudio, se discutirán las siguientes categorías de análisis: baby blues; red de apoyo; y la historia no era como la contaban.... Los resultados mostraron que el puerperio es un periodo idealizado por las mujeres, pero es vivido con sufrimiento e inestabilidad emocional. Se corroboró la importancia de la red de apoyo tanto para la madre como para el bebé.


Abstract Our study is part of a broader research on maternity and early childhood care. It aims to investigate how women experience their postpartum period today. Qualitative research was conducted for such purpose, and ten middle-class women from Rio de Janeiro were interviewed. The women were professionally active, aged between 29 and 37 years, married (formally or otherwise), primiparous, heterosexual, and had a child aged between eight months and two years old. The results were analyzed according to the content analysis method and its categorical aspect. To achieve the goals of this study, we discussed the following analysis categories: baby blues; support network; and The story was not quite like that... The results showed that women idealize the postpartum period, actually experiencing distress and emotional instability. We emphasize the importance of the support network for both mothers and their children.

Spontaneous rupture of the urinary bladder (SRUB) is a rare urological emergency and even more uncommon in the puerperium. Post-partum bladder rupture might be associated with fetal pressure on the not emptied bladder wall during labor. We report a case of SRUB three days after normal vaginal delivery with high values of creatinine and absence of hematuria, surgically managed in an emergency hospital in the Amazon.

Esta revisão sistemática visa identificar os fatores de risco psicossociais para depressão pós-parto (DPP). Foram selecionados 11 artigos por meio das bases: Embase, Medline e SciELO. Os critérios de inclusão foram: data de publicação nos últimos cinco anos, língua inglesa ou portuguesa, delineamento observacional longitudinal e ser referente a fatores de risco psicossociais para DPP. Foram encontrados 25 fatores de risco psicossociais. Entre eles, os fatores mais citados foram: falta de apoio social/familiar, gravidez não planejada, história pessoal de doença psiquiátrica, relacionamento ruim com o parceiro, idade menor que 20 anos e baixa escolaridade. A prevalência de DPP alcançou valores de 4,5% até 50,3% nos estudos avaliados. Isso demonstra a importância da detecção precoce desses fatores de risco e de mais investimentos na área de prevenção dessa doença. Para uma visão integral da doença, uma análise envolvendo uma amostra maior de artigos de bases de dados alternativas se faz necessária.(AU)


This systematic review has the purpose of identifying psychosocial risk factors on post-partum depression (PPD). Eleven articles were selected utilizing three databases: Embase, Medline and SciELO. The inclusion criteria were: publication data in the last five years, English or Portuguese, longitudinal observational design and reference to psychosocial risk factors for Postpartum Depression.Twenty-five factors were identified. Some of them were identified more than others, such as social and/or family support, unplanned pregnancy, personal history of psychiatric diseases, bad relationship with the partner, age under 20 years old, and low levels of schooling. PPD's prevalence was between 4,5% and 50,3%. These numbers confirm the importance of both early detection of risk factors as well as more investments in the prevention of this disease. For an integral view of the disease, an analysis involving a larger sample of alternative database articles is required.(AU)

OBJECTIVE: To characterise the occurrence of fever (≥38.0°C) after treatment for post-partum haemorrhage (PPH) with sublingual misoprostol 800 mcg in Latin America, where elevated rates of misoprostol's thermoregulatory effects and recipients' increased susceptibility to high fever have been documented. METHODS: A prospective observational study in hospitals in Argentina enrolled consenting women with atonic PPH after vaginal delivery, eligible to receive misoprostol. Corporal temperature was assessed at 30, 60, 90 and 120 min post-treatment; other effects were recorded. The incidence of high fever ≥ 40.0°C (primary outcome) was compared to the rate observed previously in Ecuador. Logistic regressions were performed to identify clinical and population-based predictors of misoprostol-induced fever. RESULTS: Transient shivering and fever were experienced by 75.5% (37/49) of treated participants and described as acceptable by three-quarters of women interviewed (35/47). The high fever rate was 12.2% (6/49), [95% Confidence Interval (CI) 4.6, 24.8], compared to Ecuador's rate following misoprostol treatment (35.6% (58/163) [95% CI 28.3, 43.5], P = 0.002). Significant predictors of misoprostol-induced fever (model dependent) were as follows: pre-delivery haemoglobin < 11.0g/dl, rapid placental expulsion, and higher age of the woman. No serious outcomes were reported prior to discharge. CONCLUSIONS: Misoprostol to treat PPH in Argentina resulted in a significantly lower rate of high fever than in Ecuador, although both are notably higher than rates seen elsewhere. A greater understanding of misoprostol's side effects and factors involved in their occurrence, including genetics, will help alleviate concerns. The onset of shivering may be the simplest way to know if fever can also be expected.


OBJECTIF: Caractériser la survenue de fièvre (≥ 38,0°C) après traitement d'une hémorragie post-partum (HPP) avec du misoprostol sublingual à 800 mcg en Amérique latine, où des taux élevés d'effets thermorégulateurs du misoprostol et une sensibilité accrue des receveurs à une forte fièvre ont été documentés. MÉTHODES: Une étude observationnelle prospective dans des hôpitaux en Argentine a recruté des femmes consentantes atteintes d'HPP atonique après un accouchement vaginal éligibles pour recevoir du misoprostol. La température corporelle a été évaluée 30, 60, 90 et 120 minutes après le traitement; d'autres effets ont été enregistrés. L'incidence d'une fièvre élevée ≥40,0°C (critère principal) a été comparée au taux observé précédemment en Equateur. Des régressions logistiques ont été effectuées pour identifier les prédicteurs cliniques et ceux basés sur la population de la fièvre induite par le misoprostol . RÉSULTATS: Des frissons transitoires et de la fièvre ont été ressentis par 75% (37/49) des participantes traitées et décrits comme acceptables par les trois quarts des femmes interrogées (35/47). Le taux de fièvre élevé était de 12% (6/49), [intervalle de confiance (IC) à 95%: 4,6, 24,8] contre 35,6% en Equateur après traitement au misoprostol (58/163) [IC95%: 28,3, 43,5], p = 0,002). Les prédicteurs significatifs de la fièvre induite par le misoprostol (selon le modèle) étaient: hémoglobine avant l'accouchement <11,0 g/dL, expulsion placentaire rapide et âge plus élevé de la femme. Aucun résultat sévère n'a été signalé avant le sortie d'hôpital. CONCLUSIONS: Le misoprostol pour traiter l'HPP en Argentine a entraîné un taux de fièvre élevée significativement plus bas qu'en Equateur, bien que les taux dans les deux pays soient notablement plus élevés que les taux observés ailleurs. Une meilleure compréhension des effets secondaires du misoprostol et des facteurs impliqués dans leur apparition, y compris la génétique, aidera à atténuer les inquiétudes. L'apparition de frissons peut être le moyen le plus simple de savoir si l'on peut également s'attendre à de la fièvre.

BACKGROUND: Obstetric guidelines are useful to improve the quality of care. Availability of international guidelines has rapidly increased, however the contextualization to enhance feasibility of implementation in health facilities in low and middle-income settings has only been described in literature in a few instances. This study describes the approach and lessons learned from the 'bottom-up' development process of context-tailored national obstetric guidelines in middle-income country Suriname. METHODS: Local obstetric health care providers initiated the guideline development process in Suriname in August 2016 for two common obstetric conditions: hypertensive disorders of pregnancy (HDP) and post partum haemorrhage (PPH). RESULTS: The process consisted of six steps: (1) determination of how and why women died, (2) interviews and observations of local clinical practice, (3) review of international guidelines, (4) development of a primary set of guidelines, (5) initiation of a national discussion on the guidelines content and (6) establishment of the final guidelines based on consensus. Maternal enquiry of HDP- and PPH-related maternal deaths revealed substandard care in 90 and 95% of cases, respectively. An assessment of the management through interviews and labour observations identified gaps in quality of the provided care and large discrepancies in the management of HDP and PPH between the hospitals. International recommendations were considered unfeasible and were inconsistent when compared to each other. Local health care providers and stakeholders convened to create national context-tailored guidelines based on adapted international recommendations. The guidelines were developed within four months and locally implemented. CONCLUSION: Development of national context-tailored guidelines is achievable in a middle-income country when using a 'bottom-up' approach that involves all obstetric health care providers and stakeholders in the earliest phase. We hope the descriptive process of guideline development is helpful for other countries in need of nationwide guidelines.

O estudo objetivou analisar a experiência de mulheres no pós-parto, sobre o planejamento familiar. Neste período, a mulher tende a receber menor atenção dos profissionais de saúde, uma vez que os olhares estão mais voltados ao recém-nascido, e as questões relacionadas à sexualidade não recebem atenção. Pesquisa qualitativa, descritivo-exploratória, realizada com 17 mulheres no período de até um ano pós-parto. Foi utilizada a técnica da entrevista semiestruturada para a coleta de dados e a análise temática de Bardin para a sua organização. Após a análise do material verbal coletado, três categorias emergiram: Gravidez não planejada pela 'falha' do método; Consulta puerperal; e Planejamento familiar após o parto. O planejamento familiar preconizado pelo Ministério da Saúde não é compatível com as ações realizadas pela Estratégia de Saúde da Família (ESF). Isso porque as mulheres entrevistadas experienciaram, em sua maioria, gravidez indesejada e continuam vulneráveis a adquirir outras futuras gestações.


Current research analyzed the experience of post-partum females on family planning. During this period, females tend to receive less attention by health professionals since the newly born baby comes foremost and sexual issues are not focused. Current qualitative, descriptive and exploratory research was undertaken with 17 females during a period of up to one year after birth. The half-structured interview was prepared for data collection and Bardin´s thematic analysis for organization. After analyzing the verbal material, three categories emerged: non planned pregnancy due to the method´s failure; puerperal attendance, family planning after child birth. Family planning as conceived by the Ministry of Health is not compatible with activities by the Family Health Strategy due to the fact that most interviewed females had undesired pregnancy and remained vulnerable to other future ones.

Objective ­ To identify the risk factors for postpartum depression in the reports of postpartum women who had this disorder, and to compare differences and similarities with the literature. When addressing the postpartum depression is essential to identify women with risk factors, by monitoring during prenatal care, and child care, the current health policy of women adopted by the Ministry of Health includes the nurse as professional able to develop actions in all phases of the female life cycle, the puerperal phase stands out, because in this situation the greatest organic and social changes that a woman can face, altering her state of health and well-being. Methods ­ A qualitative approach to the field of study in which ten women with a diagnosis of postpartum depression were selected by snowball sampling technique following a semi-structured interview form whose responses were analyzed by the Bardin content analysis technique. Results ­ Ten women were interviewed and, based on their responses, the topic emerged: understanding the symptoms and factors that favor postpartum depression. and two categories: Feelings and relationships of women interviewed with their mothers, fathers and their current socio-family context and their connections to postpartum depression. Conclusion ­ Through this study, it was possible to verify the symptoms and risk factors presented by the women, although they are in agreement with the literature, it was verified the necessity of qualitative studies in this area as well as evaluation on the assistance practices given to these patients


Objetivo ­ Identificar os fatores de risco frente à depressão pós-parto por relatos de puérperas que tiveram esse transtorno, e confrontar diferenças e semelhanças com a literatura. Ao se tratar a depressão puerperal é imprescindível identificar as mulheres com fatores de risco, por meio do acompanhamento durante o pré-natal e na puericultura. A atual política de saúde da mulher adotada pelo Ministério da Saúde inclui o enfermeiro como profissional apto para desenvolver ações em todas as fases do ciclo de vida feminino, com foco na fase puerperal, pois nesta estão as maiores alterações orgânicas e sociais que uma mulher pode enfrentar alterando seu estado de saúde e bem-estar. Métodos ­ Estudo de campo de abordagem qualitativa, no qual dez mulheres com diagnóstico médico de depressão pósparto foram selecionadas pela técnica snowball sampling, seguido de uma entrevista com formulário semiestruturado, cujas respostas foram analisadas por meio da técnica de análise de conteúdo de Bardin. Resultados ­ Foram entrevistadas dez mulheres e a partir de suas respostas emergiu o tema: "compreendendo os sintomas e fatores que favorecem a depressão pós-parto", e duas categorias: "Sentimentos" e "Relações das mulheres entrevistadas com suas mães, pais e seu contexto sociofamiliar atual e suas conexões com a depressão pós-parto". Conclusão ­ Por meio deste estudo, foi possível verificar os sintomas e fatores de risco apresentados pelas mulheres, muito embora estejam de acordo com a literatura. Verificou-se também a necessidade de estudos qualitativos nessa área, assim como avaliação sobre práticas assistenciais prestadas a essas pacientes

Inflammation is not only the first line of defense of the organism but is also required in many reproductive processes such as ovulation, corpus luteum development, luteolysis, uterine clearance after insemination and post partum. Nevertheless, if excessive or persistent, inflammation can switch from a positive mechanism to a deleterious process, impairing oocyte quality and embryo development. Not only uterine but also non genital inflammatory sites can depreciate reproductive performances, with a carry over effect of 2 to 4 months. Since the metabolic challenges of the peripartum transition period make difficult for the cow to control inflammation, dairy cows are frequently in a pro-inflammatory stage, suggesting that inflammation, rather than infection, is a limiting factor of fertility in modern dairy cows. Within the first week after calving, cows have to mount an intense inflammatory response to the bacterial invasion of the uterine cavity with the challenge of being able to switch it off in no more than 5-6 weeks. The absence of neutrophils on endometrial smear is associated with the highest success rate at insemination. Since a fine tuning - rather than an absence - of inflammation is required along the reproductive cycle, anti-inflammatory drugs do not allow any improvement of pregnancy rate, except in the specific case of embryo transfer. Appropriate management of the transition period (especially nutritional) and in a long term perspective, genetic selection contribute to improve the aptitude of cows to controls the intensity of inflammatory process.
