RESUMO

Anterior cruciate ligament (ACL) instability poses a considerable challenge in traumatology and orthopedic medicine, demanding precise diagnostics for optimal treatment. The pivot-shift test, a pivotal assessment tool, relies on subjective interpretation, emphasizing the need for supplementary imaging. This study addresses this limitation by introducing a machine learning classification algorithm integrated into a mobile application, leveraging smartphones' built-in inertial sensors for dynamic rotational stability assessment during knee examinations. Orthopedic specialists conducted knee evaluations on a cohort of 52 subjects, yielding valuable insights. Quantitative analyses, employing the Intraclass Correlation Coefficient (ICC), demonstrated robust agreement in both intraobserver and interobserver assessments. Specifically, ICC values of 0.94 reflected strong concordance in the timing between maneuvers, while signal amplitude exhibited consistency, with the ICC ranging from 0.71 to 0.66. The introduced machine learning algorithms proved effective, accurately classifying 90% of cases exhibiting joint hypermobility. These quantifiable results underscore the algorithm's reliability in assessing knee stability. This study emphasizes the practicality and effectiveness of implementing machine learning algorithms within a mobile application, showcasing its potential as a valuable tool for categorizing signals captured by smartphone inertial sensors during the pivot-shift test.

Abstract Background: Virtual consultations have increased exponentially, but a limitation is the inability to assess vital signs (VS). This is particularly useful in patients with heart failure (HF) for titrating prognosis-modifying medication. This issue could potentially be addressed by a tool capable of measuring blood pressure (BP) and heart rate (HR) accurately, remotely, and conveniently. Mobile phones equipped with transdermal optical imaging technology could meet these requirements. Objective: To evaluate the accuracy of a transdermal optical imaging-based app for estimating VS compared to clinical assessment in patients with HF. Methods: A prospective cohort study included patients evaluated in an HF outpatient unit between February and April 2022. BP and HR were simultaneously assessed using the app and clinical examination (BP with an automated sphygmomanometer and HR by brachial palpation). Three measurements were taken by both the app and clinic for each patient, by two independent blinded physicians. Results: Thirty patients were included, with 540 measurements of BP and HR. The mean age was 66 (± 13) years, 53.3% were male. The mean left ventricular ejection fraction was 37 ± 15, with 63.3% having previous hospitalizations for HF, and 63.4% in NYHA class II-III. The mean difference between the app measurement and its clinical reference measurement was 3.6 ± 0.5 mmHg for systolic BP (SBP), 0.9 ± -0.2 mmHg for diastolic BP (DBP), and 0.2 ± 0.4 bpm for HR. When averaging the paired mean differences for each patient, the mean across the 30 patients was 2 ± 6 mmHg for SBP, -0.14 ± 4.6 mmHg for DBP, and 0.23 ± 4 bpm for HR. Conclusion: The estimation of BP and HR by an app with transdermal optical imaging technology was comparable to non-invasive measurement in patients with HF and met the precision criteria for BP measurement in this preliminary study. The use of this new transdermal optical imaging technology provides promising data, which should be corroborated in larger cohorts.

RESUMO

OBJECTIVE: To assess whether using a Dietary Approaches to Stop Hypertension (DASH) diet recommendation application increases primary care physicians' knowledge and dietary counseling skills. DESIGN: A randomized controlled trial. SETTING: Brazilian public primary care service. PARTICIPANTS: Two hundred and twenty-two physicians (intervention group: n = 111; control group: n = 111). INTERVENTION: Thirty days of using the Dieta Dash application. The application provides information about nutritional recommendations for hypertension management. MAIN OUTCOME MEASURES: Nutrition knowledge score. SECONDARY OUTCOMES: self-assessment of knowledge, self-confidence, assessment of eating habits, and barriers to dietary counseling. ANALYSIS: Linear mixed-effects models for repeated measures and generalized estimating equations for comparing changes between groups. RESULTS: A total of 66.2% of participants completed the follow-up. There was no significant difference between the groups regarding the mean knowledge score (P = 0.15). The prevalence of high knowledge increased by 12% (prevalence ratio [PR] = 1.12; 95% confidence interval [CI], 1.00-1.25) in the intervention group and showed an improvement in the self-confidence assessment (PR = 1.21; 95% CI, 1.02-1.44), and increased assessment of eating habits (PR = 1.26; 95% CI, 1.10-1.55). CONCLUSIONS AND IMPLICATIONS: The Dieta Dash application helped address dietary counseling, improving knowledge and self-confidence. However, innovative strategies are needed to minimize the primary care barriers.

RESUMO

Strategies to stir and mix reagents in microfluid devices have evolved concomitantly with advancements in manufacturing techniques and sensing. While there is a large array of reported designs to combine and homogenize liquids, most of the characterization has been focused on setups with two inlets and one outlet. While this configuration is helpful to directly evaluate the effects of features and parameters on the mixing degree, it does not portray the conditions for experiments that involve more than two substances required to be subsequently combined. In this work, we present a mixing characterization methodology based on particle tracking as an alternative to the most common approach to measure homogeneity using the standard deviation of pixel intensities from a grayscale image. The proposed algorithm is implemented on a free and open-source mobile application (MIQUOD) for Android devices, numerically tested on COMSOL Multiphysics, and experimentally tested on a bidimensional split and recombine micromixer and a three-dimensional micromixer with sinusoidal grooves for different Reynolds numbers and geometrical features for samples with fluids seeded with red, blue, and green microparticles. The application uses concentration field data and particle track data to evaluate up to eleven performance metrics. Furthermore, with the insights from the experimental and numerical data, a mixing index for particles (mp) is proposed to characterize mixing performance for scenarios with multiple input reagents.

RESUMO

INTRODUCTION: Medical students are a population at increased risk for anxiety due to their demanding schedule and concerns about potential stigmatization, which often leads to discouragement when seeking help. COVID-19 pandemic has been reported to worsen this issue by restricting social interaction and mobility. To address this problem, an innovative method known as Asynchronous Digital Cognitive Education GAMA-AIMS (DCE GAMA-AIMS) has been introduced. Compared to traditional therapy, this modality can be accessed independently without the guidance of a therapist. OBJECTIVES: To compare the effectiveness of DCE GAMA-AIMS in reducing anxiety scores compared to guided brief Cognitive Behavioral Therapy (guided bCBT). METHODS: A non-blinding RCT was conducted on 66 medical students. The participants were equally divided into two groups, namely intervention and control. The intervention group was given DCE GAMA-AIMS, while the control was administered with guided bCBT. The data obtained were analyzed using independent t-test and ANOVAs. RESULTS: The application had a significant effect on reducing anxiety scores from the 2nd week (M TMAS = 18) to the 8th week (M TMAS = 13). A faster and more significant improvement was observed in the intervention group from the 1st to the 2nd week compared to the control, which began to improve in the 4th week. Furthermore, the intervention group had larger effect sizes (1.32) compared to the control (0.79) from the 1st to 8th week. CONCLUSION: Asynchronous DCE GAMA-AIMS and guided bCBT could reduce TMAS scores in medical students with anxiety, but DCE GAMA-AIMS yielded a greater effect size.

RESUMO

CONTEXT: Arterial Hypertension (AH) and Diabetes Mellitus (DM) are diseases that are getting worse all over the world. Linked to this advance, is the growing digital health market with numerous mobile health applications, which aim to help patients and professionals in the proper management of chronic diseases. The aim of this study was to analyze, through a systematic review and meta-analysis, the effectiveness of using mobile health applications in monitoring AH and/or DM in the adult and elderly population. METHODS: The systematic review and meta-analysis was carried out in accordance with the Preferred Reporting Items for Systematic Reviews and Metanalyses guidelines and involved searching five databases - Medline/PubMed, Embase, CINAHL, Virtual Library in Health and Cochrane Library. The review included randomized and cohort clinical trials testing the effects of the intervention on changing biochemical parameters and clinical efficacy in people treated for AH and/or DM. The quality of the selected studies was assessed based on the evaluation criteria of the Joanna Briggs Institute tool. The random effects meta-analysis method was used to explain effect distribution between studies, by Stata® software (version 11.0) and publication bias was examined by visual inspection of graphs and Egger test. RESULTS: We included 26 studies in the systematic review and 17 in the meta-analysis. These studies were published between 2014 to 2022 in 14 countries. Were reported improvement in knowledge and self-management of AH and DM, social motivation with treatment and behavioral change, reduction in glycated hemoglobin values, fasting glucose and blood pressure, improvement in adherence to drug treatment, among others. The result of the meta-analysis showed that there is evidence that the use of mobile applications can help reduce glycated hemoglobin by 0.39% compared to the usual care group. CONCLUSIONS: Monitoring and self-monitoring of behaviors and health care related to AH and DM in adults and the elderly through mobile applications, has clinically significant effectiveness in reducing glycated hemoglobin levels. Future studies should provide more evidence and recommendations for best practices and development of digital health interventions. TRIAL REGISTRATION: PROSPERO. RESUMO

OBJECTIVE: The purpose of this interdisciplinary study was to determine Dominican adolescents' preferences for the content, functionality (i.e. food's glycemic index list, and amount of insulin based on foods' carbohydrate count), and design of a culturally and linguistically relevant mobile application (app) for type 1 diabetes mellitus (T1DM) self-management. The app would facilitate T1DM self-management education and support by addressing providers' challenges in monitoring patients' disease progression and promoting patient adherence to recommended lifestyle changes. Findings inform an app development process that considers the linguistic and cultural values, norms, and structures of people with T1DM and their providers in the Dominican Republic. METHODS: Phone interviews were conducted with 23 adolescents (14 to 18 years of age), using a semistructured questionnaire. The research team conducted the data analysis using NVivo through a deductive and inductive approach. RESULTS: The findings suggest that, regardless of the context, adolescents with T1DM desire similar features and functionalities in a self-management app. Overall, participants preferred an app with a graphic format that has accessible information, a straightforward design, and instructional videos. Participants also desired that an app provide information and reminders about proper eating and insulin administration timing and be a vehicle to access a social network to foster mutual support and encouragement. CONCLUSIONS: Our study highlights adolescents' perspectives on components for inclusion in an app for T1DM self-management. Participants' recommendations for the app's potential usability, contents, and design features will be used to guide the development of a new app to promote engagement and foster better health outcomes.

RESUMO

Health guidelines inform recommendations for different clinical practices or public health policies. They are a simple way to organize and retrieve relevant information that can impact patient care. Although these documents are easy to use, most are not user-friendly because they are difficult to access. Our work aims to present the developing approach for a decision-making tool based on health guidelines to assist health professionals in caring for patients with tuberculosis. This tool is being developed for use on mobile devices and as a web-based system, which will transform a passive and declarative health guideline document into an interactive tool that will provide data, information, and knowledge. User tests with functional prototypes developed for the Android platform show that this application has the potential to be applied in TB healthcare facilities in the future.

RESUMO

Traditional learning techniques have evolved slowly and have yet to adapt the course content delivery to today's students' approaches to acquiring new knowledge. However, micro-learning has become popular in e-Learning environments as a course design technique due to short attention spans, demand for small chunks of information, and time constraints. Hence, it has been selected for creating reading mobile applications provided to the nature of its learning approach. In order to describe the multiple iterations of design, development, and evaluation of this general framework, a methodology named Design-Based Research (DBR) is implemented. First, the article presents the abstract framework components and a cloud-based software architecture that allows a modular approach to creating such applications. The pathway developed through adapting the iPAC framework, which involves personalization, authenticity, and collaboration, is part of the methodology used to design the app under pedagogical and technological considerations. The process demanded the following phases: analysis and exploration, design and construction, evaluation and reflection, redesign and reconstruction, and final critical reflections. Four applied instruments also validate the framework implementation: The iPAC Rubric, an aphorisms checklist, a pre and post-test, a focus group, and a usability test taken by 28 students in a private university in Colombia. Findings indicated that Design-Based Research (DBR) methodology emerged as an appropriate tool to encounter the needs behind reading applications design due to its sequence of operations yields results successively closer to adequate usability standards and smooth implementation. They also reveal the positive impact of new types of texts on students' motivation and awareness toward other reading strategies and micro-learning. This impact indeed proved the proposed framework's effectiveness for designing micro-learning applications.

RESUMO

INTRODUCTION: Stroke is highly prevalent in Latin America and one of the leading causes of morbidity and mortality in the world. Educating children about stroke has been established as an effective method to detect symptoms early, reduce hospital visits, and raise awareness among adults. OBJECTIVE: To analyze the effectiveness of a mobile application to improve knowledge and understanding of stroke among children. METHOD: We conducted a focus group session including 12 children in order to analyze the behavior of 6 questions previously validated by expert neurologists. Subsequently, 105 primary school students between the ages of 7 and 12 completed a questionnaire on stroke symptoms and how to contact the emergency services before and after using an application on stroke symptoms. Qualitative analyses and the Student t test were used to verify the existence of differences between pre- and post-intervention test results. RESULTS: After a single 40-min working session with the application, between 50% and 67% of the children were able to identify the signs of stroke, and 96.2% knew the national emergency services telephone number. Statistical analysis revealed statistically significant differences before and after the intervention with the digital application (t=19.54; p<0.001) and intragroup differences in the post-intervention test results (t=40.71; p<0.001). CONCLUSION: Primary school children who used our digital application increased their knowledge, understanding, and learning of stroke symptoms.

This article presents original findings from a longitudinal qualitative study on changes in individual and family life associated with safety and health measures implemented during the COVID-19 pandemic in three regions of Chile. We developed a methodological approach based on multimodal diaries in a mobile application, in which participants submitted photographs and texts to express changes in their daily lives under residential confinement. Content and semiotic visual analyses show a significant loss in instances of collective recreation, partially compensated through new personal and productive activities performed at home. Our results suggest that modal diaries serve as potential tools to capture people's perceptions and meanings as their lives go through exceptional and traumatic times. We assert that using digital and mobile technologies in qualitative studies could allow subjects to actively participate in the co-construction of fieldwork and produce quality knowledge from their situated perspectives.

Dentistry is an area of knowledge strongly characterized by its technical-scientific aspect in addition to the artisanal way in which it is performed. In recent years, some phenomena such as the massive application of technology in its processes in addition to the expansion of the professionals' field of activity have modernized the sector without, however, changing the fact that the majority of dentists continue to carry out their activities in clinical environments. , which generates demands for conducting business between peers and support professionals. There is, however, a significant barrier to the effectiveness of transactions of this nature: the decentralized way in which the dentist's clinical work takes place, which keeps these professionals resorting to the traditional method of being recommended by close colleagues to buy, rent and hire, reducing, thus, the possibilities of achieving good business. In a society where "connection" is a keyword for success, it is necessary and urgent to modernize the way in which commercial relationships occur between dental professionals so that this niche of opportunities becomes a relevant market in this sector. Thus, the objective of this work was to develop a mobile application with the purpose of connecting dental professionals who wish to sell, rent or provide services to those who wish to buy, rent or hire in an agile, effective and reliable way throughout the national territory. Based on research into the fundamentals, dynamics and trends of e-commerce, sharing economy, bilateral platforms and the dental market, this tool was created using a lowcode development methodology with customization carried out using HTML as a programming language, resulting in an application mobile device whose content is presented in the form of advertisements, divided into 8 categories and 16 subcategories. The tool can be accessed without the need for download and has search and sorting features for personalized viewing, which adds speed to the user experience. As a form of monetization, taxation will be carried out per published advertisement, which may be highlighted and displayed in areas designated for this purpose in the product layout. Having received the name Odontolinks, the application is now available for access in its trial version and can be accessed at: https://app.vc/odontolinks. It is expected that the Odontolinks platform will become the biggest Brazilian reference in bilateral business in the dental sector, constituting a pioneering, effective, modern and safe tool, promoting increased profitability of dentistry businesses and, consequently, of the entire sector in addition to promotingjob creation and the consolidation of the dental community across the country (AU)

RESUMO

The high prevalence of mental health problems among university students poses a challenge when developing effective interventions, with digital technologies emerging as a potential resource to address this problem. The inclusion of student input in the design and development of such interventions is critical to improving their impact. This study contributed to the initial phase of a research project that aims to adapt and evaluate the feasibility and acceptability of an early intervention for anxiety and depression based on digital technologies for university students. Three participatory workshops were conducted with 13 university students in Chile to inquire about the features and content that a mental health mobile app should include to meet their needs and preferences. The workshop transcripts were analysed using inductive thematic analysis. The results of this study highlight the value of modifications such as the personalisation of some features of the app. The students recommended incorporating topics related to university life and the possibility of contacting a mental health professional, as well as the inclusion of peer interaction or other forms of support.
