CONTEXT: Photobiomodulation therapy (PBMT) has been widely used to improve strength, fatigue resistance and increase muscle mass in healthy individuals. These effects could help critically ill patients admitted to intensive care units (ICUs) who show reduced mobility and muscle strength. ICU-acquired weakness lessens overall health and increases the patient's length of stay in the ICU. OBJECTIVE: This study evaluated the effects of PBMT using low intensity light-emitting diodes (LEDs) on the mobility and muscle strength (functional capacity) and length of stay of patients admitted to hospital ICU. METHODS: This randomized, triple-blind, sham-controlled trial was conducted in a hospital ICU. Sixty patients were randomly assigned to two equal groups: (a) PBMT and (b) Sham. PBMT was applied daily to patients until their discharge from the ICU, using a flexible neoprene array of 264 LEDs (120 at 635 nm, 1.2 mW each; 144 at 880 nm, 15 mW each) for 90s (207.36 Joules) at each site. Ten sites were located bilaterally on the thighs, legs, arms, and forearms ventrally and dorsally, 15 min totaling 2,073.6 Joules per session. Outcomes were length of stay (in h) until discharge from the ICU, muscle strength by the Medical Research Council (MRC) score and handgrip dynamometry (HGD), patient mobility by Intensive Care Unit Mobility Scale (IMS) and the Simplified Acute Physiology Score 3 (SAPS 3) for predicting mortality of patients admitted to the ICU. RESULTS: PBMT reduced the average length of stay in the ICU by ~30% (p = 0.028); increased mobility (IMS: 255% vs. 110% p = 0.007), increased muscle strength (MRC: 12% vs. -9% p = 0.001) and HGD (34% vs. -13% p < 0.001), and the SAPS3 score was similar (p > 0.05). CONCLUSION: The results suggest that daily PBMT can reduce the length of stay of ICU patients and increase muscle strength and mobility.

La investigación científica es un componente fundamental para la formación de profesionales de la salud. La Metodología de la Investigación que se imparte en el pregrado de la carrera de medicina tiene como finalidad prepararlos en la realización de investigaciones científicas, mediante las tecnologías de la información. Esta incorpora contenidos de Estadística Descriptiva para desarrollar investigaciones, lo que hace necesario que se adquieran habilidades con herramientas informáticas para procesar los datos recogidos, a través de software, preferentemente de libre distribución. El presente artículo tuvo el objetivo de divulgar el uso del software gratuito Jamovi para los contenidos de Estadística Descriptiva en el primer año de la carrera de medicina del Plan E, lo que permite concluir que esta herramienta gratuita utiliza una interfaz sencilla y amigable. Está diseñada en lenguaje R, puede ser modificada según los intereses del investigador y se ha convertido en la preferida de muchos investigadores en el procesamiento, el análisis y la representación gráfica de datos a nivel global(AU)


Scientific research is a fundamental component in the training of health professionals. The purpose of the subject Research Methodology taught s part of the undergraduate medical program is to prepare them to conduct scientific research using information technologies. This includes descriptive statistics contents to develop research, which makes it necessary to acquire skills with computer tools for processing the collected data, through softwares, preferably of free distribution. The present article had the objective of spreading the use of the free software Jamovi for descriptive statistics contents in the first academic year of the medical major belonging to the plan of studies E, which allows concluding that this free tool uses a simple and user-friendly interface. It is designed in R language, can be modified according to the researcher's interests, and has become the preferred tool of many researchers for the processing, analysis and graphical representation of data at a global level(AU)

IMPORTANCE: The scientific community debates Generative Pre-trained Transformer (GPT)-3.5's article quality, authorship merit, originality, and ethical use in scientific writing. OBJECTIVES: Assess GPT-3.5's ability to craft the background section of critical care clinical research questions compared to medical researchers with H-indices of 22 and 13. DESIGN: Observational cross-sectional study. SETTING: Researchers from 20 countries from six continents evaluated the backgrounds. PARTICIPANTS: Researchers with a Scopus index greater than 1 were included. MAIN OUTCOMES AND MEASURES: In this study, we generated a background section of a critical care clinical research question on "acute kidney injury in sepsis" using three different methods: researcher with H-index greater than 20, researcher with H-index greater than 10, and GPT-3.5. The three background sections were presented in a blinded survey to researchers with an H-index range between 1 and 96. First, the researchers evaluated the main components of the background using a 5-point Likert scale. Second, they were asked to identify which background was written by humans only or with large language model-generated tools. RESULTS: A total of 80 researchers completed the survey. The median H-index was 3 (interquartile range, 1-7.25) and most (36%) researchers were from the Critical Care specialty. When compared with researchers with an H-index of 22 and 13, GPT-3.5 was marked high on the Likert scale ranking on main background components (median 4.5 vs. 3.82 vs. 3.6 vs. 4.5, respectively; p < 0.001). The sensitivity and specificity to detect researchers writing versus GPT-3.5 writing were poor, 22.4% and 57.6%, respectively. CONCLUSIONS AND RELEVANCE: GPT-3.5 could create background research content indistinguishable from the writing of a medical researcher. It was marked higher compared with medical researchers with an H-index of 22 and 13 in writing the background section of a critical care clinical research question.

PURPOSE: The aim was to apply the Advanced Nursing Process and demonstrating procedures of surgical positioning, as well as to show the participating the Nursing Outcomes Classification findings in order to translate the knowledge on specific preventive perioperative positioning into practice, the review of clinical protocols and nursing care plan. METHODS: The methods used include case report about knowledge translation by applying training modalities, review and adaptation of clinical protocols and examination of nursing care plans. FINDINGS: One hundred and nine healthcare providers attended the training, including nurses and nursing technicians working in the surgical center and the outpatient surgical center of the hospital chosen for this study. The surgical positioning protocols were revised based on the evidence described in the literature and the main surgical guidelines. The review of care registered in the institution's electronic system for the nursing prescription stage of the diagnosis Risk of perioperative positioning injury was based on the review on evidence on risk factors, the main guidelines in the area and the interventions suggested by Nursing Intervention Classification. The electronic system had registered 14 cares for this diagnosis, and after the review, one care was excluded and eight new cares were included, totaling 21 cares. CONCLUSION: Nursing teams play a prominent role in positioning patients for surgery, protecting them in a moment of extreme vulnerability, thus making knowledge about the fundamental elements of surgical positioning essential. This emphasizes the importance of training, and of reviewing protocols and records of procedures that promote safety to patients and care teams. IMPLICATION FOR NURSING PRACTICE: The translation of knowledge about the Advanced Nursing Process in the perioperative area contributes to the refinement of classifications and standardization of language in this scenario, subsidizing an evidence-based clinical practice.


OBJETIVO: O objetivo foi aplicar o Processo de Enfermagem Avançado e demonstrar procedimentos de posicionamento cirúrgico, bem como mostrar aos participantes os achados da Nursing Outcomes Classification para traduzir na prática o conhecimento específico sobre prevenção no posicionamento perioperatóriontivo, bem como a revisão de protocolos clínicos e do plano de cuidados de enfermagem. MÉTODOS: Relato de caso sobre tradução do conhecimento por meio da aplicação de modalidades de treinamento, revisão e adaptação de protocolos clínicos e análise de planos de cuidados de enfermagem. RESULTADOS: 109 profissionais de enfermagem participaram do treinamento, incluindo enfermeiros e técnicos de enfermagem que atuam no centro cirúrgico e no centro cirúrgico ambulatorial do hospital escolhido para este estudo. Os protocolos de posicionamento cirúrgico foram revisados com base nas evidências descritas na literatura e nas principais diretrizes cirúrgicas. A revisão dos cuidados registrados no sistema eletrônico da instituição para a etapa de prescrição de enfermagem do diagnóstico Risco de lesão por posicionamento perioperatório foi baseada na revisão das evidências sobre os fatores de risco, as principais diretrizes da área e as intervenções sugeridas pela Nursing Intervention Classification. O sistema eletrônico tinha 14 cuidados cadastrados para este diagnóstico, e após a revisão, um cuidado foi excluído e oito novos cuidados foram incluídos, totalizando 21 cuidados. CONCLUSÃO: A equipe de enfermagem tem papel de destaque no posicionamento do paciente para a cirurgia, protegendo-o em um momento de extrema vulnerabilidade, tornando imprescindível o conhecimento sobre os elementos fundamentais do posicionamento cirúrgico. Ressalta-se a importância do treinamento e da revisão de protocolos e registros de procedimentos que promovem a segurança do paciente e da equipe assistencial. IMPLICAÇÃO PARA A PRÁTICA DE ENFERMAGEM: A tradução do conhecimento sobre o Processo de Enfermagem Avançado na área perioperatória contribui para o refinamento das classificações e padronização da linguagem neste cenário, subsidiando uma prática clínica baseada em evidências.

ABSTRACT Objective: to report the path taken to implement the Thirst Management Model using the Knowledge Translation Evidence-based Practice for Improving Quality intervention in a Burn unit. Method: an experience report on the implementation, which took place in two stages: Preparation; and Implementation/Change, both requiring a sequence of steps. Results: the implementation was performed in four cycles of the PDSA improvement tool. All had the same indicator collected, with increasing goals to be attained. Considering the barriers identified, multiple combined Knowledge Translation strategies were used, namely: posters; theoretical and practical training sessions, individual or in group; videos; dynamics; music; logo development for implementation visibility; audit and feedback; and didactic and illustrated clinical protocols. Conclusion: the report of the entire implementation process using the Evidence-based Practice for Improving Quality intervention, pointing out its weaknesses and strengths, proves to be useful, necessary and innovative. This study may assist in future evidence-based implementations that choose to use multifaceted interventions.


RESUMEN Objetivo: informar el camino recorrido para implementar el Modelo de Manejo de la Sed recurriendo a la intervención Knowledge Translation llamada Evidence-based Practice for Improving Quality (Práctica Basada en Evidencia para Mejorar la Calidad) en una unidad especializada en Quemaduras. Método: informe de experiencia sobre la implementación, que tuvo lugar en dos etapas: Preparación e Implantación/cambio, ambas obedeciendo una secuencia de pasos para su realización. Resultados: la implementación se realizó en cuatro ciclos de la herramienta de mejoras PDSA. En todos los ciclos se recolectó el mismo indicador, con metas crecientes por alcanzar. Considerando las barreras identificadas, se utilizaron múltiples estrategias combinadas de Knowledge Translation, a saber: posters; sesiones de capacitación teóricas y prácticas, individuales o en grupo, videos, dinámicas, música, desarrollo de un logotipo para conferir visibilidad a la implementación; auditoría y feedback; y protocolos clínicos didácticos e ilustrados. Conclusión: el informe de la totalidad del proceso de implementación recurriendo a la intervención Evidence-based Practice for Improving Quality, incluso señalando sus debilidades y puntos fuertes, demuestra que es útil, necesaria e innovadora. Este estudio pode auxiliar futuras implementaciones de evidencias que decidan utilizar intervenciones multifacéticas.


RESUMO Objetivo: Relatar o caminho percorrido para a implantação do Modelo de Manejo da Sede com o uso da intervenção de Knowledge Translation Evidence-based Practice for Improving Quality (Prática Baseada em Evidência para a Melhoria do Processo de Qualidade) em uma unidade de queimados. Método: Relato de experiência sobre a implantação que ocorreu em duas etapas: Preparação e Implantação/ mudança, ambas obedecendo uma sequência de passos para sua realização. Resultados: A implementação foi realizada em quatro ciclos da ferramenta de melhoria PDSA. Todos tiveram o mesmo indicador coletado, com metas crescentes a serem alcançadas. Considerando as barreiras identificadas, utilizaram-se múltiplas estratégias combinadas de Knowledge Translation: cartazes, capacitações teóricas e práticas, individuais ou em grupo, vídeos, dinâmicas, músicas, desenvolvimento de logo para visibilidade da implantação, auditoria e feedback, protocolos clínicos didáticos e ilustrados. Conclusão: O relato de todo o processo de implantação com o uso da intervenção Evidence-based Practice for Improving Quality, apontando suas fragilidades e fortalezas, mostra-se útil, necessária e inovador. Este estudo pode auxiliar futuras implantações de evidências que escolham utilizar intervenções multifacetadas.

Esta es la segunda parte de un artículo sobre la búsqueda de financiamiento para un proyecto de investigación. Todo proyecto de investigación requiere una fuente de financiamiento para poder ser llevado adelante. La búsqueda de fondos es una tarea que lleva tiempo y esfuerzo con una baja tasa de éxito. Compartimos algunos consejos que podrían ayudar a aumentar esa tasa de éxito en relación con: 1) cómo reconocer la necesidad de búsqueda de una fuente de financiamiento externo, 2) de dónde provienen los fondos, 3) qué gastos se pueden financiar habitualmente con los fondos y 4) cómo mejorar la escritura y la presentación a una convocatoria. This is the second part of our series on searching funds for a research plan. Every research proposal requires a source of funding to be carried out. Looking for funds is a time and effort consuming task with a low success rate. We share some tips that may help to improve that success rate related to (1) how to recognize the need of an external funding source, (2) where the funds are coming from, (3) what costs can be funded and (4) how to improve a proposal writing and submission. Background: The literature reports long time lags between the several processes involved in the translation of drug research and development into clinical application. To expedite these processes, translational research has emerged as a process that can be applied to reduce the lag between scientific discoveries and their practical application. Thus, the objective of this study was to estimate the time lag in translational research of biological drugs for the treatment of rheumatoid arthritis included in the Brazilian Unified Health System [Sistema Único de Saúde (SUS)]. Methods: A descriptive retrospective study was conducted based on secondary data loaded by SUS users in public sources and systems to estimate the time lag between the publication of phase I clinical trial results to drug use in clinical settings. The dates of translational research activities were identified from markers and steps. Structured searches were conducted in the literature and reports from the National Commission for the Incorporation of Technologies in the SUS (Conitec) as well as from health authorities, and analyzed. Results: Between 2012 and 2019, SUS included five biological agents for the treatment of rheumatoid arthritis. The mean time lag from clinical development to use of these agents was 11.13 years (range, 8.57 to 12.90 years). The mean time lag for the stages of translational research were 5.30 (T1-basic research to clinical research), 5.08 (T2-clinical research to research synthesis), and 0.75 (T3-research synthesis to evidence-based practice) years. A shorter time lag was observed in the Brazilian case when it was possible to compare with other studies. Conclusions: The estimated time lag of biological drugs used in the treatment of rheumatoid arthritis was determined based on the translational research steps model adapted to the Brazilian context. Brazil has instituted legal frameworks that set deadlines for sanitary registration, health technology assessment (HTA), and the availability of drugs in the SUS, thus, allowing for a reduced stage T2 time lag. Nevertheless, improvements are still required in stages T1 and T2, especially in publishing the results of clinical trials.

Background: Stereotactic and functional neurosurgery (SFN) is a rapidly evolving field and some emerging countries, especially Mexico, have made significant contributions to this discipline. A bibliometric analysis has never been performed in Latin America, and this would be particularly important to show the areas that remain poorly studied, and design research strategies for the future. Methods: Scopus was queried using keywords pertaining to functional neurosurgery, restricting the affiliation country to Mexico, and considering documents published after 1949. Added to the initial search, a complementary literature exploration by author, considering the publications of the most productive neurosurgeons, was performed. A descriptive statistical analysis was carried out. Results: From 5,109 articles, only 371 were eligible. Scientific production has gradually increased with time. Epilepsy (31%) and movement disorders (27.4%) were the most studied neurological conditions, whereas the other 41.6% corresponded to pain, behavior disorders, spinal cord injuries, neuromodulation, stereotactic biopsies, and SFN history. Level of evidence was predominantly level V (59.1%). Publication output is highly skewed to Mexico City, which represents 78.4% of national production. Relative to factors associated with impact of research, publications in English had more citations (28.5 mean citations per paper), and journals with an impact factor greater than one had more than 10 mean citations per paper. Conclusions: Mexico has experienced an increase in the productivity of SFN literature, addressing the most prevalent issues in the country (epilepsy and motor disorders). However, it is necessary to report studies with a higher level of evidence, as well as to decentralize the research collaborating with national institutions outside Mexico City. On the other hand, it is imperative to promote scientific production in English and in high-impact indexed journals to increase the visibility of our production. We would like to call upon our colleagues in other countries to reproduce our methodology, in order to determine the factors associated with the impact and productivity on SFN research.

Introdução: O emprego de questionários de qualidade de vida tem se mostrado muito útil no sentido de dar maior objetividade à avaliação de resultados de tratamentos. A internacionalização desses instrumentos, por sua vez, permite a comparação interpopulacional, mas requer uma metodologia específica, a fim de não causar distorções devido a falhas na tradução ou a diferenças culturais. O questionário Blepharoplasty Outcomes Evaluation, de língua inglesa, é uma ferramenta de simples aplicação, com perguntas objetivas com boa aplicação para esse fim. O questionário já foi testado em relação à sua confiabilidade, validade e capacidade de resposta. Métodos: Realizada tradução e adaptação cultural para a língua portuguesa, segundo a metodologia proposta por Beaton et al., na qual existem 5 estágios. Estágio 1 - tradução por meio de dois tradutores nativos de língua portuguesa. Estágio 2 - confecção de versão de síntese. Estágio 3 - tradução reversa por dois tradutores nativos de língua inglesa. Estágio 4 - revisão por um comitê avaliador. Estágio 5 - aplicação a uma população de 20 pessoas. Resultados: A partir do comitê avaliador, não houve problemas de compreensão para a população final. Conclusão: O questionário foi traduzido e adaptado com sucesso.


Introduction: The use of quality of life questionnaires has proved to be very useful in giving greater objectivity to evaluating treatment results. The internationalization of these instruments, in turn, allows for interpopulation comparison but requires a specific methodology in order not to cause distortions due to failures in translation or cultural differences. The Blepharoplasty Outcomes Evaluation questionnaire, in English, is a simple application tool with objective questions with a good application for this purpose. The questionnaire has already been tested for reliability, validity and responsiveness. Methods: According to the methodology proposed by Beaton et al., translation and cultural adaptation into Portuguese was performed with 5 stages. Stage 1 - translation by two native Portuguese-speaking translators. Stage 2 - preparation of the synthesis version. Stage 3 - reverse translation by two native English-speaking translators. Stage 4 - review by an evaluation committee. Stage 5 - application to a population of 20 people. Results: There were no comprehension problems for the final population from the evaluation committee. Conclusion: The questionnaire was successfully translated and adapted.

BACKGROUND: The execution of undergraduate thesis is a period in which students have an opportunity to develop their scientific knowledge. However, many barriers could prevent the learning process. This cross-sectional study aimed to analyze the scientific dissemination of results from undergraduate theses in physical therapy programs and verify the existence of barriers and challenges in the preparation of undergraduate thesis. Second, to investigate whether project characteristics and thesis development barriers were associated with the dissemination of undergraduate thesis results. METHODS: Physical therapists who graduated as of 2015, from 50 different educational institutions, answered an online questionnaire about barriers faced during the execution of undergraduate thesis and about scientific dissemination of their results. RESULTS: Of 324 participants, 43% (n = 138) of participants disseminated their results, and the main form of dissemination was publishing in national journals (18%, n = 58). Regarding the barriers, 76% (n = 246) of participants reported facing some difficulties, and the main challenge highlighted was the lack of scientific knowledge (28%, n = 91). Chances of dissemination were associated with barriers related to scientific understanding and operational factors, such as the type of institution, institutional facilities, and involvement with other projects. CONCLUSION: Scientific knowledge seems to be a determining factor for the good development of undergraduate theses. In addition, it is clear the need to stimulate more qualified dissemination that reaches a larger audience. Changes in operational and teaching factors may improve the undergraduate thesis quality. However, the importance of rethinking scientific education within physical therapy programs draws attention.

Translational research, in its regulatory function as a current policy for the research activities being performed at the Mexican Institute for Social Security (Instituto Mexicano del Seguro Social: IMSS), demands a collaborative work among people involved in the generation of knowledge and in those who consume such knowledge. Being an organization who has among its major objectives for almost 80 years the health care of Mexican population, the Institute has an important human capital represented by its physician leaders, researchers and directors, who, by working closely together, will be able to find a better respond to the health care needs of the Mexican population. Through collaborative groups, transversal research networks oriented to the priority health problems of Mexican people are being organized as a strategy whose purpose is making research more efficient and ensuring rapidly applicable results, in order to improve the quality of health care services offered by the Institute, whose commitment is primarily to Mexican society, although the results of this strategy could also be shown to the world, considering that the Institute is one of the largest public health service organizations, at least in Latin America, and its results could be a benchmark for the region. Collaborative work in research networks began more than 15 years ago at IMSS, but today it is being consolidated and its objectives are being reoriented, aligning them with both national policies and those of the Institute itself.


La investigación traslacional, como la política vigente de las actividades de investigación que se realizan en el Instituto Mexicano del Seguro Social (IMSS), requiere del trabajo colaborativo de los actores involucrados en la generación del conocimiento y de los que consumen el conocimiento. Al ser una organización que tiene como uno de sus principales objetivos la atención a la salud de la población mexicana desde hace casi 80 años, el Instituto cuenta en sus filas con un importante capital humano, representado por sus líderes médicos, investigadores y directivos, que, al trabajar en conjunto, podrán encontrar mejores respuestas a las necesidades de salud de la población mexicana. La organización por medio de grupos colaborativos en redes transversales de investigación orientadas a los problemas prioritarios de salud de los mexicanos es una estrategia que busca que la investigación sea más eficiente y que sus resultados sean prontamente aplicables, a fin de mejorar la calidad de la atención de los servicios de salud que oferta el Instituto, compromiso que se tiene primero con la sociedad mexicana, pero además se puede mostrar al mundo, ya que el Instituto es una de las más grandes organizaciones de servicios de salud pública, al menos en Latinoamérica, por lo que sus resultados pueden ser un referente para la región. El trabajo colaborativo en redes de investigación inició hace más de 15 años en el IMSS, pero hasta el día de hoy se fortalece y se reorientan sus objetivos, los cuales se alinean a las políticas nacionales y a las propias del Instituto.

Introdução: A hipotermia terapêutica é o tratamento indicado para encefalopatia moderada a grave em recém-nascidos. A terapia requer uma equipe de enfermagem capacitada e integrada, visando um cuidado qualificado, efetivo e seguro. Modelos teóricos têm sido desenvolvidos para auxiliar a incorporação de evidências científicas à prática dos enfermeiros, representando um desafio na área da saúde. A implementação de uma intervenção educativa, guiada pela estrutura i-PARIHS (Estrutura Integrada de Promoção da Ação na Implementação de Pesquisa em Serviços de Saúde), poderá preencher a lacuna entre a teoria e a prática, beneficiando a assistência e tornando os sujeitos ativos no manejo do recém-nascido em hipotermia terapêutica. Objetivo geral: avaliar o impacto de uma intervenção educativa, guiada pelo referencial teórico i-PARIHS, sobre o manejo do recém-nascido com asfixia perinatal em hipotermia terapêutica na unidade intensiva neonatal no conhecimento, atitudes e práticas de enfermeiros. Objetivos específicos: analisar o conhecimento, atitude e prática dos enfermeiros sobre o manejo do recém-nascido com asfixia perinatal em hipotermia terapêutica pré e pós-intervenção educativa; identificar as barreiras e facilitadores percebidos pelos enfermeiros sobre o manejo do recém-nascido com asfixia perinatal em hipotermia terapêutica na unidade intensiva neonatal; implementar uma intervenção educativa, guiada pelo referencial i-PARIHS, para melhorar o conhecimento, a atitude e a prática dos enfermeiros sobre o manejo do recém-nascido com asfixia perinatal em hipotermia terapêutica na unidade intensiva neonatal; comparar o conhecimento, atitude e prática dos enfermeiros após a intervenção educativa e os indicadores quanto ao manejo do recém-nascido com asfixia perinatal em hipotermia terapêutica. Método: trata-se de um estudo de intervenção, do tipo quase-experimental, realizado com 29 enfermeiros de uma unidade intensiva neonatal, referência no Rio de Janeiro. O desfecho principal: conhecimento, atitudes e práticas dos enfermeiros no manejo do recém-nascido com asfixia perinatal em hipotermia terapêutica na unidade intensiva neonatal A intervenção compreendeu três fases: pré-intervenção - intervenção educativa- pós-intervenção. A intervenção educativa contou com cinco encontros: "Asfixia Perinatal x Hipotermia Terapêutica", "Controle da temperatura", "Cuidados de enfermagem na HT: avaliação de dor", "Monitoramento neurológico" e "Cuidado Centrado na Família". Para a análise estatística utilizou-se de análise descritiva e aplicação dos testes Wilcoxon-Mann-Whitney e Mc Nemar, sendo o nível de significância adotado de 0,05. Resultados: a análise dos resultados do pré e pós-teste demonstrou um incremento no escore de acertos das questões sobre conhecimento, atitude e prática dos enfermeiros no manejo do recém-nascido submetido à hipotermia terapêutica na unidade intensiva neonatal, apresentando significância estatística para a maioria dos itens. Para a inovação foram construídos lembretes, fluxo de admissão para recém-nascido da instituição e uma cartilha para os pais como produto da intervenção com os enfermeiros. Conclusão: O resultado das auditorias realizadas, após a implementação das evidências, constatou uma transformação positiva da prática dos enfermeiros. A utilização da estrutura i-PARIHS evidenciou a necessidade e o valor de investir no engajamento das partes interessadas, na avaliação colaborativa do contexto e na cocriação de inovação usando facilitação qualificada. A intervenção educativa, guiada pela estrutura i-PARIHS, mostrou ter impacto no manejo do recém-nascido submetido à hipotermia terapêutica por enfermeiros.


Introduction: Therapeutic hypothermia is the currently indicated treatment for moderate to severe encephalopathy in newborns. Therapy requires a trained and integrated nursing team, aiming at qualified, effective and safe care. Theoretical models have been developed to help the incorporation of scientific evidence into nurses' practice, representing a challenge in the health area. The implementation of an educational intervention, guided by the i-PARIHS (Integrated Promoting Action on Research Implementation in Health Services Framework) framework, can fill the gap between theory and professional practice, benefiting care and making subjects active in the management of newborns with therapeutic hypothermia. General objective: to evaluate the impact of an educational intervention guided by the theoretical framework i-PARIHS, on the management of newborns with perinatal asphyxia in therapeutic hypothermia in the neonatal intensive care unit on the knowledge, attitudes and practices of nurses. Specific objectives: to analyze the knowledge, attitude and practice of nurses on the management of newborns with perinatal asphyxia in pre- and post-educational therapeutic hypothermia; to identify barriers and facilitators perceived by nurses on the management of newborns with perinatal asphyxia in therapeutic hypothermia in the neonatal intensive care unit; implement an educational intervention, guided by the i-PARIHS framework, to improve nurses' knowledge, attitude and practice on the management of newborns with perinatal asphyxia in therapeutic hypothermia in the neonatal intensive care unit and compare the knowledge, attitude and practice of nurses after the participatory educational intervention program and indicators regarding the management of newborns with perinatal asphyxia in therapeutic hypothermia. Method: this is a quasi-experimental intervention study carried out with 29 nurses from a neonatal intensive care unit, a reference in Rio de Janeiro. The main outcome: knowledge, attitudes and practices of nurses in the management of newborns with perinatal asphyxia in therapeutic hypothermia in the neonatal intensive unit The intervention comprised three phases: pre-intervention - educational intervention - post-intervention. The educational intervention had five meetings: "Perinatal Asphyxia x Therapeutic Hypothermia", "Temperature control", "Nursing care in HT: pain assessment", "Neurological monitoring" and "Family-Centered Care". For the statistical analysis, descriptive analysis and application of the Wilcoxon-Mann-Whitney and Mc Nemar tests were used, with the adopted significance level of 0.05. Results: the analysis of pre- and post-test results showed an increase in the correct score of questions about nurses' knowledge and practices in the management of newborns submitted to therapeutic hypothermia in the neonatal intensive care unit, showing statistical significance for most items. For innovation, reminders, admission flow for newborns at the institution and a booklet for parents were created as a product of the intervention with nurses. Conclusion: The result of the audits carried out, after the implementation of the evidence, found a positive transformation of the nurses' practice. Using the i-PARIHS framework highlighted the need and value of investing in stakeholder engagement, collaborative context assessment, and co-creation of innovation using qualified facilitation. The educational intervention guided by the i-PARIHS framework was shown to have an impact on the management of newborns with perinatal asphyxia in therapeutic hypothermia by nurses.

RESUMO Existe ampla evidência que a contenção da pandemia de Covid-19 requer vigilância sindrômica e isolamento de casos suspeitos/confirmados. É essencial a disponibilidade de testes diagnósticos no Sistema Único de Saúde, que poderia ser facilitada pela soberania nacional no desenvolvimento e produção, considerando-se a alta demanda/escassez no mercado internacional. Este estudo identificou as etapas da pesquisa translacional de testes diagnósticos para Covid-19 no Brasil, verificando sua distribuição geográfica, entre outros indicadores. Estudo transversal, exploratório, partindo de banco público com 789 projetos de Pesquisa, Desenvolvimento e Inovação (PD&I) em Covid-19, complementado com outras buscas, inclusive no CVLattes dos pesquisadores. No banco, havia 89 projetos de testes diagnósticos. Em 45 casos, foi possível obter informações complementares para classificá-los conforme as etapas da pesquisa translacional. Identificaram-se 15 inovações que atingiram o estágio T3, ou seja, tiveram seus produtos incorporados em protocolos clínicos na atenção à saúde, mesmo considerando-se as profundas restrições orçamentárias em PD&I. O Brasil possui potencial de desenvolvimento e implementação de produtos tecnológicos na área de testes de diagnóstico para Sars-CoV-2. Políticas públicas de PD&I em saúde necessitam ser priorizadas para ampliação de cooperações nacionais e internacionais, a fim de promover efetiva autonomia nacional na vigilância sindrômica e à saúde da população.


ABSTRACT There is much evidence suggesting that mitigating the COVID-19 pandemic requires syndromic surveillance and isolation of suspected/confirmed cases. The availability of diagnostic tests in the Brazilian Unified Health System (SUS) is essential, which could be facilitated by national sovereignty in development and production, considering the high demand/lack of supply in the international market. This study identified the stages of translational research into diagnostic tests for COVID-19 in Brazil, verifying their geographic distribution, among other indicators. A cross-sectional, exploratory study based on a public database with 789 Research, Development, and Innovation (RD&I) projects regarding COVID-19, complemented by other searches, including the researchers' curricula (CVLattes). There were 89 diagnostic test projects in the database. In 45 cases, it was possible to obtain additional information to classify them according to the translational research stages. Fifteen innovations that reached the T3 stage were identified, with their products incorporated into clinical protocols in healthcare, even considering the deep budget restrictions in RD&I. Brazil has the potential to develop and implement technological products in the field of diagnostic tests for SARS-CoV-2. Public health RD&I policies need to be prioritized to expand national and international cooperation to promote effective national autonomy in syndromic surveillance and population health.

Abstract: This study aimed to screen the public procurement for dental adhesives in a public health system and evaluate the variables associated with the type and cost of purchased materials. A time series was conducted with secondary data collated from the Brazilian Databank of Healthcare Prices (BDHP). All public procurements for adhesive systems registered from 2010 to 2019 were collated. The frequency of purchase was described, and a multiple linear regression model was used to assess the influence of unit price on predictor variables. Multinomial logistic regression was conducted to describe the influence of selected variables on the probability of purchase for different classes of adhesive systems. A total of 2,752 purchases were analyzed, and the total cost of these materials reached US$ 7,022,659.00 in the 10 analyzed years. The 3-step Etch & Rinse adhesives corresponded to 45.63% of the processes, with increased probability of being purchased over time. The process conducted by public universities in recent years, using the auction modality, presented lower unit prices. The year of purchase, the type of institutions, the institution's location, and the procurement modality influenced the procurement process. The performed analyses contribute to the understanding of the variables associated with the procurements for adhesive systems, providing valuable information for rationalizing public expenditure and for implementing evidence-based practices to guarantee cost-effective and clinically effective procedures for users of the Brazilian public healthcare system.

BACKGROUND: Gastric cancer (GC) is a highly heterogeneous disease, and the identification of molecular subtyping of gastric adenocarcinoma emerged as a promising option to define therapeutic strategies and prognostic subgroups. However, the costs and technical complexity of molecular methodologies remains an obstacle to its adoption, and their clinical significance by other approaches needs further evidence. AIM: To evaluate the clinicopathological characteristics and long-term survival of GC based on the subgroups of molecular classification by immunohistochemistry (IHC) and in situ hybridization (ISH). METHODS: We retrospectively evaluated all patients who underwent D2-gastrectomy between 2009 and 2016 in a Western cohort of GC patients treated with curative intent. Microsatellite instability (MSI) status, E-cadherin, and p53 expression were analyzed by IHC, and Epstein-Barr virus (EBV) by ISH. Tissue microarrays were constructed for analysis. Clinicopathological characteristics and survival of GC were evaluated according to subtypes defined by The Cancer Genome Atlas (TCGA) Research Network Group and Asian Cancer Research Group (ACRG) classification systems. RESULTS: A total of 287 GC patients were included. Based on IHC and ISH analysis, five profiles were defined as follows: E-cadherin aberrant (9.1%), MSI (20.9%), p53 aberrant (36.6%), EBV positivity (10.5%), and p53 normal (31%), which corresponded to tumors that showed no alteration in another profile. A flowchart according to the TCGA and ACRG classifications were used to define the subtypes, where clinical and pathological characteristics associated with GC subtypes were evidenced. Proximal location (P < 0.001), total gastrectomy (P = 0.001), and intense inflammatory infiltrate (P < 0.001) were characteristics related to EBV subtype. MSI subtype was predominantly associated with advanced age (P = 0.017) and the presence of comorbidities (P = 0.011). While Laurén diffuse type (P < 0.001) and advanced stage (P = 0.029) were related to genomically stable (GS) subtype. GS tumors and microsatellite stable/epithelial to mesenchymal transition phenotype subtype had worse disease-free survival (DFS) and overall survival (OS) than other subtypes. Conversely, MSI subtype of GC had better survival in both classifications. Type of gastrectomy, pT and the TCGA subtypes were independent factors associated to DFS and OS. CONCLUSION: The IHC/ISH analysis was able to distinguish immunophenotypic groups of GC with distinct characteristics and prognosis, resembling the subtypes of the molecular classifications. Accordingly, this method of classification may represent a viable option for use in a clinical setting.

Medical research is essential to develop new and better therapies, increase social standards and a better life for all of us. Scientific curiosity has helped to achieve many successful innovations, but history also demonstrates that research can lead to abuses of individuals neglecting autonomy and integrity of the human being. Since the 1960ies we have witnessed a continuous development of international regulations and ethics guidelines (soft law) in medical research, leading to a higher quality of scientific results. An important focus lies on recognizing human vulnerability and a therefore adapted informed consent procedure. Our modern clinical trials structure requires the inclusion of healthy volunteers in the first phases of the development of a new medicinal product, leading to new ethical questions and challenges. The Corona-Pandemic has accelerated vaccine development in a successful way also leading to a new importance of healthy volunteers in the medical research landscape.


A pesquisa médica é essencial para desenvolver novas e melhores terapias, aumentar os padrões sociais e uma vida melhor para todos nós. A curiosidade científica ajudou a alcançar muitas inovações bem-sucedidas, mas a história também demonstra que a pesquisa pode conduzira abusos de indivíduos, negligenciando a autonomia e a integridade do ser humano. Desde a década de 1960, temos testemunhado um desenvolvimento contínuo de regulamentos internacionais e de diretrizes éticas (soft law) em pesquisa médica, levando a resultados científicos de maior qualidade. Um foco importante está no reconhecimento da vulnerabilidade humana e, consequentemente, num procedimento de consentimento informado adaptado. A nossa estrutura moderna de ensaios clínicos requer a inclusão de voluntários saudáveis nas primeiras fases do desenvolvimento de um novo medicamento, suscitando novas questões e desafios éticos. A pandemia de coronavírus acelerou o desenvolvimento de vacinas de uma forma bem-sucedida, contribuindo também para uma maior importância de voluntários saudáveis no cenário da pesquisa médica.

La curiosidad científica ha ayudado a lograr muchas innovaciones exitosas, pero la historia también demuestra que la investigación puede conducir a abusos deindividuos que descuidan la autonomía y la integridad del ser humano.Desde la década de 1960 hemos sido testigos de un desarrollo continuo de las regulaciones internacionales y de las directriceséticas(soft law) en la investigación médica, lo que ha llevado a una mayor calidad de los resultados científicos. Un enfoque importante radica en el reconocimiento de la vulnerabilidad humana y, por lo tanto, en un procedimiento de consentimiento informado adaptado.Nuestra moderna estructura de ensayos clínicos requiere la inclusión de voluntarios sanos en las primeras fases del desarrollo de un nuevo medicamento, lo que genera nuevas cuestiones y desafíos éticos. La pandemia de coronavirus ha acelerado el desarrollo de vacunas de una manera exitosa, lo que también ha dado lugar a una nueva importancia de los voluntarios sanos en el panorama de la investigación médica.
