BACKGROUND: Giardia spp. is the most common protozoan found in coproparasitoscopic tests; it is the cause of gastrointestinal discomfort, with a high prevalence in children and in low-income areas. This systematic review updates available literature on molecular identification of Giardia spp. in Latin America during 2017 to 2021. METHODOLOGY: The guidelines established in Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) were used; the study covers the period from January 1, 2017 to October 03, 2021; the search focused on the "molecular identification of Giardia spp. in Latin America" in six different scientific databases. The material found was reviewed to select only those papers that met the inclusion and exclusion criteria. RESULTS: The search yielded 1036 publications, but only 19 investigations in 6 countries (Brazil, Colombia, Cuba, Ecuador, Mexico, and Venezuela) met the selection criteria. Most were cross-sectional studies carried out in school-age children, the dominant assemblages were A and B while the most frequent subassemblages were AII, BIII and BIV, the most used target genes were tpi and gdh, the prevalence by molecular methods was higher regarding microscopy, the countries with the highest prevalence percentages for Giardiosis were Brazil and Cuba. CONCLUSIONS: More Latin America countries need to generate data of prevalence, incidence, and intensity of Giardiosis. Studies are also needed to estimate the costs of Giardiosis on economic productivity and public health. The present systematic review offers evidence based on the current literature available for the molecular identification of Giardia spp. in Latin America during 2017 to 2021.

Introducción: La Hiperplasia nodular linfoide gastrointestinal constituye una entidad infrecuente con manifestaciones clínicas diversas y con mayor frecuencia en la edad pediátrica. Objetivo: Caracterizar clínica, endoscópica e histológicamente a los pacientes pediátricos con hiperplasia linfoide de colon e íleon terminal diagnosticados en el Instituto de Gastroenterología. Material y Métodos: Se realizó un estudio descriptivo, transversal, en el periodo comprendido entre 2014 y 2016 en el Instituto de Gastroenterología. La muestra estuvo constituida por 50 pacientes, quienes cumplieron los criterios de inclusión y exclusión. Se evaluaron variables demográficas, clínicas, endoscópicas e histológicas, así como el comportamiento de la comorbilidad con enfermedades malignas, enfermedades inflamatorias intestinales, giardiosis, trastornos de la respuesta inmunohumoral y alergias alimentarias. Resultados: El sexo masculino, entre 7-10 años y el color de la piel blanca fueron los más frecuentes. El sangrado rectal fue el síntoma principal (62 por ciento) y la localización a nivel del íleon terminal en 69 por ciento, no se relacionó con enfermedad maligna, hubo tres pacientes con diagnóstico de hiperplasia linfoide de íleon terminal y colitis ulcerosa. El 74 por ciento presentó aspecto nodular por histología y 60 por ciento se le diagnosticó Giardia lamblia, en la evaluación inmunohumoral predominó los pacientes sin inmunocompromiso (78 por ciento), el Prick Test fue positivo en 60 por ciento, sobre todo, a la leche de vaca. Conclusiones: La manifestación clínica que predominó fue el sangrado rectal, endoscópicamente la localización en íleon y la forma nodular por histología. No encontramos comorbilidades con enfermedades malignas y fueron más frecuentes los trastornos alérgicos y parasitarios que las alteraciones inmunológicas(AU)


Introduction: Lymphoid nodular hyperplasia of the gastrointestinal tract is an uncommon entity with diverse clinical manifestations, which is more frequent in the pediatric age. Objective: To characterize clinically, endoscopically, and histologically, those pediatric patients with lymphoid hyperplasia of the colon and terminal ileum diagnosed in the Institute of Gastroenterology. Material and Methods: A descriptive, cross-sectional study was carried out during the period between 2014 and 2016 in the Institute of Gastroenterology. The sample consisted of 50 patients who met the inclusion and exclusion criteria. Demographic, clinical, endoscopic and histological variables were evaluated, as well as the behavior of comorbidity with malignant diseases, inflammatory bowel diseases, giardiasis, disorders of the humoral immune response and food allergies. Results: The male sex, the age group between 7-10 years, and the white skin color were the most frequent. Rectal bleeding was the main symptom (62 percent), and the location of the lesions in the terminal ileum was observed in 69 percent of the patients. There was no relationship between lymphoid hyperplasia and malignant disease, but three patients were diagnosed with lymphoid hyperplasia of the terminal ileum, and ulcerative colitis. 74 percent of the biopsies presented a nodular variety, and 60 percent of the patients were diagnosed with Giardia lamblia; the cases that were not immunocompromised prevailed in the evaluation of humoral immune response (78 percent); the Prick Test was positive in 60 percent of children, especially to cow's milk. Conclusions: The predominant clinical manifestation was rectal bleeding; the localization was in the ileum, which was seen by endoscopic procedure; and the nodular form was demonstrated by histology. We did not find comorbidities with malignant diseases, and allergic and parasitic disorders were more frequently diagnosed than immunological alterations(AU)

Giardiosis is a parasitic disease caused by the protozoan Giardia intestinalis, which is distributed worldwide. Most of the data on the prevalence of giardiosis in Mexico comes from research, but it is also necessary to study the data provided by the Mexican Health Ministry and issued by the General Directorate of Epidemiology. The aim of this work was analyse the national surveillance data for human giardiosis in order to update the epidemiological data of this disease in Mexico. A retrospective observational analysis of giardiosis (from January 2011 to December 2015) was performed in the annual reports emitted by the GDE in Mexico. The cases were classified by year, state, age group, gender and seasons of the year. During the period of 2011-2015, a reduction of 38.51% was observed in the total number of new cases of giardiosis reported in the whole country The states of Sinaloa, Yucatan, and Chiapas presented the highest number of new cases reported during the analysed period. Giardiosis rates were always higher among women in all age groups, but the maximum incidence was observed in both sexes in the age group of 1-4 years old (the most susceptible group). On the other hand, the number of cases increased dramatically in southern states during warmer months. Giardiosis is influenced by ambient temperature changes along the year, although this study suggests that tends to decrease in all the analysed states and could be related to the overall improvement of hygienic practices within the Mexican population.

Limited data are available on the epidemiology of G. duodenalis in dairy cattle from Turkey. Determining the zoonotic potential of the Giardia infection requires molecular characterization. The aim of the present study was to investigate the prevalence and to molecularly characterize G. duodenalis in calves less than three months of age in Aydin, Aegean region of Turkey. Materials, Methods & Results: The study was conducted on different dairy farms in the south-western part of the Turkey, Aegean Region, Aydin. A total of 198 Holstein Friesian calves less than three months of age, of both sexes were enrolled into the study. Faecal samples from each calf were collected manually from the rectum using a disposable latex glove. The consistency of collected samples was recorded as diarrhoeic or non-diarrhoeic. Diagnosis of G. duodenalis infection was made microscopically by detection of cysts in the faecal samples. One hundred and sixteen (58.5%) of the 198 faecal samples were diarrheic. Giardia cysts were found in 27 (23.28%) of the diarrheic samples and in 8 (9.76%) of nondiarrheic samples (P < 0.05). The overall prevalence of giardiosis in calves was determined as 17.67%. The prevalence of Giardia genotypes was identified by DNA sequence analysis of the beta-giardin gene for every PCR positive sample. The beta-giardin nested PCR assay was revealed assemblage A and sub-genotype A3 was detected in all of 35 samples (100%). Discussion: The highest prevalence of Giardia infection in calves is reported at the age between 1 and 6 months, and the prevalence shows decreased rate from the age of 6 months. It has been suggested that the clinical features of giardiasis might vary in patients of different age groups. Objective: To compare clinical presentation of giardiasis in children less than 5 years of age and older children. Methods: A retrospective study of patients hospitalized with giardiasis during 2007 was performed. The clinical features of the two age groups were compared. Additionally, length of stay was analyzed. Results: In total, 170 patients were included. In children ≥ 5 years of age, abdominal pain and urticaria were significantly more frequent (OR=9.46; 95%CI: 4.35-20.52,P5=0.000+ and OR=11.0; 95%CI: 1.205-101.11, P=0.023, respectively). Diarrhoea was more frequently found in children younger than 5 years (OR=6.45; IC 95%: 3. 12-13.37, P=0.000+). The frequencies of other symptoms were similar. Length of stay was significantly higher in children under 5 years of age. Discussion and conclusions: Clinical presentations differed in some aspects among the examined age group of our paediatric patients. Our finding might be of importance for an early recognition of giardiasis, which is essential for an accurate diagnosis and a prompt treatment.


La giardiosis es una enfermedad frecuente a escala internacional. Se ha sugerido por algunos autores que su presentación clínica puede variar con la edad. Objetivo: Comparar las manifestaciones clínicas de la giardiosis en los niños hospitalizados bajo 5 años de edad con las de niños de 5 años o más. Métodos: Estudio retrospectivo realizado en el Hospital Pediátrico de Centro Habana durante el año 2007. Las manifestaciones clínicas fueron extraídas utilizando un formulario prediseñado y separadas de acuerdo a los grupos de edades estudiados. Adicionalmente, se extrajo la estadía hospitalaria. Resultados: Se incluyeron 170 pacientes. Los niños de 5 y más años de edad manifestaron con mayor frecuencia dolor abdominal (OR = 9,46; IC 95%: 4,35-20,52, p = 0,000+) y urticaria (OR = 11,0; IC 95%: 1,205-101,11, p = 0,023), mientras que la diarrea se encontró más frecuentemente bajo 5 años de edad (OR = 6,45; IC 95%: 3,12-13,37, p = 0,000+). En el resto de las manifestaciones clínicas no se encontraron diferencias significativas en sus frecuencias entre ambos grupos de edades (p > 0,05). La estadía hospitalaria fue significativamente más prolongada entre los pacientes bajo 5 años de edad. Discusión y conclusiones: Las manifestaciones clínicas pueden variar en algunos aspectos entre los diferentes grupos de edades pediátricas. Su rápido reconocimiento es esencial con el fin de arribar a un diagnóstico preciso y al tratamiento oportuno.
