RESUMO

One of the initial steps in the preprocessing of digital fundoscopy images is the identification of pixels containing relevant information. This can be achieved through different approaches, one of them is implementing background extraction, reducing the set of pixels to be analyzed later in the process. In this work, we present a background extraction method for digital fundoscopy images based on computational topology. By interpreting binarized images as cubical complexes and extracting their homological groups in 1 and 2 dimensions we identify a subset of luminescence values that can be used to binarize the original grayscale image, obtaining a mask to achieve background extraction. This method is robust to noise and suboptimal image quality, facilitating the analytical pipeline in the context of computer aided diagnosis approaches. This method facilitates the segmentation of the background of a digital fundoscopy image, which allows further methods to focus on pixels with relevant information (eye fundus). This tool is best suited to be implemented in the preprocessing stages of the analytical pipeline by computational ophthalmology specialists.•It is robust to noise and low-quality images.•Output provides an ideal scenario for down-the-line analysis by facilitating only relevant pixels in a digital fundoscopy.

BACKGROUND AND HYPOTHESIS: Despite accounting for significant disease morbidity in schizophrenia, the neuropathological basis of negative symptoms remains poorly understood and options for treatment limited. Our recent study identified robust associations between diminished auditory cortex (AC) dynamic range and social functioning impairments and negative symptoms in first episode psychosis (FESz). The current investigation examined the progression of these relationships 4-8 months from baseline testing. STUDY DESIGN: Twenty-six FESz and 38 healthy controls (HC) were tested at baseline and follow-up. Magnetoencephalography (MEG) was recorded during binaural presentation of tones (75, 80, and 85 dB). Assessments included the MATRICS cognitive consensus battery (MCCB) and Global Functioning: Role and Social scales (GFR/GFS) and the Positive and Negative Syndrome Scale. STUDY RESULTS: Overall, FESz exhibited a blunted response to increasing tone intensity relative to HC. While this deficit did not change over time at the group level, recovery of right hemisphere AC dynamic range (85-75 dB response) among FESz individuals was associated with reductions in negative symptoms (ρ = -0.50). Diminished dynamic range was also associated with impaired GFS (ρ = 0.65), GFR (ρ = 0.51), and MCCB (ρ = 0.49) at baseline and increased negative symptoms at baseline (ρ = -0.53) and follow-up (ρ = -0.51). CONCLUSION: Despite persistent dynamic range impairment in FESz as a group, individual recovery of this AC response property was associated with significant reduction in negative symptoms. Identification of a functional neural deficit that tracts progression of negative symptoms during a critical period for disease modification is essential to the management of these devastating and historically treatment refractory symptoms.

BACKGROUND: An electrical potential not previously reported-electrical cochlear response (ECR)-observed only in implanted patients is described. Its amplitude and growth slope are a measurement of the stimulation achieved by a tone pip on the auditory nerve. The stimulation and recording system constructed for this purpose, the features of this potential obtained in a group of 43 children, and its possible clinical use are described. The ECR is obtained by averaging the EEG epochs acquired each time the cochlear implant (CI) processes a tone pip of known frequency and intensity when the patient is sleeping and using the CI in everyday mode. The ECR is sensitive to tone pip intensity level, microphone sensitivity, sound processor gain, dynamic range of electrical current, and responsiveness to electrical current of the auditory nerve portion involved with the electrode under test. It allows individual evaluation of intracochlear electrodes by choosing, one at the time, the central frequency of the electrode as the test tone pip frequency, so the ECR measurement due to a variable intensity tone pip allows to establish the suitability of the dynamic range of the electrode current. RESULTS: There is a difference in ECR measurements when patients are grouped based on their auditory behavior. The ECR slope and amplitude for the Sensitive group is 0.2 µV/dBHL and 10 µV at 50 dBHL compared with 0.04 µV/dBHL and 3 µV at 50dBHL for the Inconsistent group. The clinical cases show that adjusting the dynamic range of current based on the ECR improved the patient's auditory behavior. CONCLUSIONS: ECR can be recorded regardless of the artifact due to the electromyographic activity of the patient and the functioning of the CI. Its amplitude and growth slope versus the intensity of the stimulus differs between electrodes. The relationship between minimum ECR detection intensity level and auditory threshold suggests the possibility of estimating patient auditory thresholds this way. ECR does not depend on the subject's age, cooperation, or health status. It can be obtained at any time after implant surgery and the test procedure is the same regardless of device manufacturer.

Introduction. The traditional way of facilitating a good singing voice has been achieved through rigorous voice training. In the modern days, however, there are some aspects of the singing voice that can be enhanced through digital processing. Although in the past, the frequency or intensity manipulations had to be achieved through the various singing techniques of the singer, technology today allows the singing voice to be enhanced from the instruments within recording studios. In es-sence, the traditional voice pedagogy and the evolution of digital audio processing both strive to achieve a better quality of the singing voice, but with different methods. Nevertheless, the major aspects of how the singing voice can be manipulated are not communicated among the professionals in each field.Objective. This paper offers insights as to how the quality of the singing voice can be changed physiologically through the traditional ways of voice training, and also digitally through various instruments that are now available in recording studios.Reflection. The ways in which singers train their voice must be mediated with the audio technology that is available today. Although there are aspects in which the digi-tal technology can aid the singer's voice, there remain areas in which the singers must train their singing system in a physiological level to produce a better singing voice


Introducción. La forma tradicional de facilitar una buena voz para cantar se ha lo-grado mediante un riguroso entrenamiento de la voz. Sin embargo, en la actualidad, existen aspectos de la voz cantada que pueden mejorarse mediante el procesamiento digital. Aunque en el pasado las manipulaciones de frecuencia o intensidad tenían que lograrse a través de las diversas técnicas de canto del cantante, la tecnología actual permite ahora mejorar la voz del canto desde los instrumentos dentro de los estudios de grabación. En esencia, la pedagogía de la voz tradicional y la evolución del procesamiento de audio digital se esfuerzan por lograr una mejor calidad de la voz cantada, pero con métodos diferentes. No obstante, los principales aspectos de cómo se puede manipular la voz cantada no se comunican entre los profesionales de cada campo respectivo. Objetivo. Este artículo ofrece información sobre cómo la calidad de la voz cantada se puede cambiar fisiológicamente a través de las formas tradicionales del entrena-miento de la voz, y también digitalmente a través de varios instrumentos que ahora están disponibles en los estudios de grabación. Reflexión. Las formas en que los cantantes entrenan su voz deben estar mediadas por la tecnología de audio que está disponible en la actualidad. Aunque hay aspectos en los que la tecnología digital puede ayudar a la voz del cantante, quedan áreas en las que los cantantes deben entrenar su sistema de canto a nivel fisiológico para pro-ducir una mejor voz al cantar.

The rostral agranular insular cortex (RAIC) is a relevant structure in nociception. Indeed, recruitment of GABAergic activity in RAIC promotes the disinhibition of the locus ceruleus, which in turn inhibits (by noradrenergic action) the peripheral nociceptive input at the spinal cord level. In this regard, at the cortical level, oxytocin can modulate the GABAergic transmission; consequently, an interaction modulating nociception could exist between oxytocin and GABA at RAIC. Here, we tested in male Wistar rats the effect of oxytocin microinjection into RAIC during an inflammatory (by subcutaneous peripheral injection of formalin) nociceptive input. Oxytocin microinjection produces a diminution of (1) flinches induced by formalin and (2) spontaneous firing of spinal wide dynamic range cells. The above antinociceptive effect was abolished by microinjection (at RAIC) of the following: (1) L-368899 (an oxytocin receptor [OTR] antagonist) or by (2) bicuculline (a preferent GABAA receptor blocker), suggesting a GABAergic activation induced by OTR. Since intrathecal injection of an α2A-adrenoceptor antagonist (BRL 44408) partially reversed the oxytocin effect, a descending noradrenergic antinociception is suggested. Further, injection of L-368899 per se induces a pronociceptive behavioral effect, suggesting a tonic endogenous oxytocin release during inflammatory nociceptive input. Accordingly, we found bilateral projections from the paraventricular nucleus of the hypothalamus (PVN) to RAIC. Some of the PVN-projecting cells are oxytocinergic and destinate GABAergic and OTR-expressing cells inside RAIC. Aside from the direct anatomic link between PVN and RAIC, our findings provide evidence about the role of oxytocinergic mechanisms modulating the pain process at the RAIC level.SIGNIFICANCE STATEMENT Oxytocin is a neuropeptide involved in several functions ranging from lactation to social attachment. Over the years, the role of this molecule in pain processing has emerged, showing that, at the spinal level, oxytocin blocks pain transmission. The present work suggests that oxytocin also modulates pain at the cortical insular level by favoring cortical GABAergic transmission and activating descending spinal noradrenergic mechanisms. Indeed, we show that the paraventricular hypothalamicnucleus sends direct oxytocinergic projections to the rostral agranular insular cortex on GABAergic and oxytocin receptor-expressing neurons. Together, our data support the notion that the oxytocinergic system could act as an orchestrator of pain modulation.

Sigma Delta analogue-to-digital converters allow acquiring the full dynamic range of biomedical signals at the electrodes, resulting in less complex hardware and increased measurement robustness. However, the increased data size per sample (typically 24 bits) demands the transmission of extremely large volumes of data across the isolation barrier, thus increasing power consumption on the patient side. This problem is accentuated when a large number of channels is used as in current 128-256 electrodes biopotential acquisition systems, that usually opt for an optic fibre link to the computer. An analogous problem occurs for simpler low-power acquisition platforms that transmit data through a wireless link to a computing platform. In this paper, a low-complexity encoding method is presented to decrease sample data size without losses, while preserving the full DC-coupled signal. The method achieved a 2.3 average compression ratio evaluated over an ECG and EMG signal bank acquired with equipment based on Sigma-Delta converters. It demands a very low processing load: a C language implementation is presented that resulted in an 110 clock cycles average execution on an 8-bit microcontroller.

RESUMO

O controle de qualidade da imagem em equipamentos ultra-sônicos de diagnósticos é baseado em parâmetros determinados por norma (IEC, 1996). Todos os parâmetros citados nesta norma, com exceção da Largura de Banda, podem ser avaliados utilizando-se de phantoms de tecidos biológicos. Baseado em normas e procedimentos internacionais, confeccionou-se um phantom para medição do parâmetro Faixa Dinâmica Exibida. Como material mimetizados de tecido, utilizou-se a solução matriz (agar, glicerina e água) adicionada de pó de grafite, apresentando coeficiente de atenuação (alfa 0) e velocidade da onda ultrasônica (c) compatíveis com a norma (alfa 0 = 0,7 dB.cm elevado a menos 1.MHz elevado a -0,83 e v = 1540 m.s elevado a menos 1). Adicionaram-se como conservantes, os antibióticos: clorafenicol, tetraciclina, penicilina G e os antifúngicos nistativa e anfotericina B. Foi também desenvolvido um protocolo detalhado para aplicação do phantom. Este protocolo foi testado em um aparelho comercial novo e os resultados iniciais apresentaram-se satisfatórios sugerindo sua adequação (tanto do phantom como do protocolo) numa aplicação rotineira em aparelhos comerciais
