During mixed dentition, the 1st permanent molar is the most susceptible to caries, triggering a repetitive restorative cycle and loss. This study aimed to evaluate determinants of therapeutic extraction of severely damaged 1st permanent molars in mixed dentition before the eruption of the second permanent molar with favorable spontaneous closure of the residual space. The methodology involved a scoping review on PubMed using a specific search strategy. Ten articles were included addressing factors such as the ideal chronological age, stage of development of the second premolar and permanent molar, presence of the third molar, residual spontaneous closure, prognosis, and need for orthodontic treatment. In conclusion, therapeutic extraction of the 1st molar before the eruption of the second molar is associated with favorable spontaneous closure of the residual space. Greater success is evident with the presence of the third molar, the second molar in stage E, and the second premolar in stage F (Demirjian).
RESUMO
Tooth extraction is the most common procedure in dental practice. However, in the long term, it may cause alveolar ridge atrophy. This systematic review aimed to evaluate the role of platelet-rich fibrin (PRF) in post-exodontic alveolar ridge preservation in terms of its effectiveness in the regeneration of bone tissue as assessed by imaging and its efficacy compared to physiological bone healing. The study is presented in accordance with the preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidelines. This systematic review was conducted using electronic databases such as PubMed, Scopus, Web of Science, and Science Direct. The gray literature search was conducted in the New York Academy of Medicine Grey Literature Report. All the studies in this systematic review were randomized controlled trials (RCTs). The risk of bias was performed according to the Cochrane Handbook for Systematic Reviews of Interventions 6.2 (RevMan 6.2). Considering the inclusion and exclusion criteria, we included 17 randomized clinical trials published up to 2022 investigating the efficacy of PRF in post-exodontic bone regeneration. Based on the results of clinical studies, it can be stated that despite not being statistically significant, PRF promotes neoformation and prevents bone loss between three and four months post-extraction.
RESUMO
Periodontal disease is one of the main affections of the oral cavity of dogs. Its main complication is the formation of periapical abscess, which, when affecting the maxillary canine teeth, can lead to the formation of oronasal fistulae. The objective of this study was to evaluate the efficiency of self-curing glass ionomer cement, covered by photopolymerizable adhesive, as a protective element for mucoperiosteal or gingival flap sutures in oral surgery of dogs. We studied 15 dogs from the clinical routine of the dental service of a teaching veterinary hospital, which needed oral surgeries to correct oronasal fistulae, defects or oral cavity communications, regardless of the causal agent. Group one (G1) was composed of seven animals that presented oronasal fistulae after the extraction of maxillary canine teeth compromised by severe periodontal disease. These fistulae were reduced by the double-mucoperiosteal-flap technique, 15 days after the dental extraction. Group two (G2) was composed of five other dogs that presented oronasal fistulae after the extraction of maxillary canine teeth compromised by severe periodontal disease. In this group, the fistulae were reduced by the single-flap technique, immediately after the dental extraction. Group three (G3) was composed of three animals, two of which presented oronasal fistulae due to maxillary fracture and the third one after excision of oral neoplasia. In all groups, simple interrupted sutures were used with 3.0 nylon, and a thin layer of self-curing glass ionomer cement was applied immediately over the operated area. After cement's settling time, a thin layer of photopolymerizable adhesive was applied to the already polymerized cement. In G1, the protective cement was removed on average at 15 (±2) postoperative days, in G2 at 6 (±1) days and in G3 at 11 (±9) days. In the postoperative period, the animals received antibiotics and anti-inflammatory drugs, and they received their usual dry dog food diet. No Elizabethan collar or any other protective measure was used for suturing or the surgical wound. The result was healing of 100% of the oronasal fistulae, without suture dehiscence or the need for new surgical interventions. Thus, it was concluded that the use of self-curing glass ionomer cement covered by photopolymerizable adhesive was fully satisfactory, providing protection of sutures and surgical wounds and showing the potential for routine use in oral surgery in dogs.
RESUMO
Introduction: The post-exodontic alveolar atrophy requires quality bone regeneration techniques, and therefore, it is necessary to apply platelet concentrates, which are bioactive agents in the process of preserving the alveolar bone. The use of platelet concentrates proved to be an excellent osteoinducer since it leads to three important effects for maintaining bone structure. Objective: To analyze the use of platelet concentrates in post-exodontic bone regeneration. Materials and methods: A descriptive, explanatory study. A narrative review in which data was collected from 26 scientific articles published in scientific databases such as PubMed, Redalyc, ScienceDirect, and Ovid between 2012 and 2022. Conclusions: Platelet concentrates are physiological materials that speed up the healing time of post-extraction wounds. They are autologous since they are taken from the patient him/herself, reducing the risk of postoperative reactions, and the transmission of diseases by parenteral route. They relieve both inflammation and edema as well as post-surgery symptoms that appear after a dental extraction. They also help to preserve the alveolar ridge, avoiding long-term atrophy.
RESUMO
Abstract Glanzmannʼs Trombasthenia (GT) is a genetic disorder, that develops with a tendency toward bleeding and is characterized by the absence or decrease in platelet aggregation. Surgical bleeding may be difficult to control. Platelet transfusion is the main treatment, albeit refractoriness can occur. We describe the case of a patient with GT and platelet refractoriness, who was submitted to radical prostatectomy and dental extraction. The perioperative treatment with apheresis platelet concentrate and activated recombinant factor seven allowed the procedures to be performed uneventfully. We discuss the complexity of the case and the treatment option.

Introduction: The reasons for tooth loss have been well defined for several centuries. However, systematic analysis of the causes of tooth loss in the first years of the present century would shed light on the current manifestation of this phenomenon. Objective: Determine the main causes of tooth mortality in the last 20 years. Methods: A systematic review was conducted using the Boolean search engines in PubMed, Web of Science, Scopus and SciELO bibliographic search platforms. The search was based on the following MeSH terms: cause, motive, mortality, extraction, dental extraction, loss, and combinations thereof. The articles included were in English or Spanish. Calibration was performed by the authors to evaluate the articles to be selected. Results: A total 593 articles were retrieved: 16 from Web of Science, 39 from Scopus, 531 from Pubmed and 1 from SciELO, of which 26 were included in the study. A predominance was found of articles published in English (80.8 percent). The main causes of tooth mortality dealt with were dental caries (92.30 percent) and periodontal disease (50.0 percent). Conclusions: The evidence obtained in the last 20 years is insufficient to determine the main reasons for dental extraction by continent. There is little availability of articles about areas from the most densely populated and developed countries in each continent. The articles included showed a predominance of dental caries and periodontal disease as the main reasons for dental extraction(AU)

Introduction: Recent simple dental extraction protocols suggest that therapeutic levels of the anticoagulant drug should be maintained because bleeding complications could be managed with local haemostasis manoeuvres. The aim of the present study was to evaluate the association between bleeding complications and the international normalized ratio (INR) values in patients after dental extractions completed with a plug of bismuth subgallate, without interrupting anticoagulant administration. Materials and Methods: Patients undergoing oral Vitamin K antagonist chronic anticoagulant therapy requiring simple dental extractions were included in the study. INRs were recorded on the day of the surgery, and dental extractions were performed applying bismuth subgallate as a haemostatic agent. Patients took their anticoagulation drug normally. Bleeding complications were recorded. Results: The study included 694 patients, of whom 11 (1.58%) presented mediate post-operative bleeding that was effectively controlled through local manoeuvres. No episode of thromboembolism or infectious endocarditis was observed. The incidence of bleeding complications was not related to the INR values (P > 0.05). Discussion: INR values were not related to bleeding complications when simple dental extractions were performed applying bismuth subgallate as a haemostatic agent.

Abstract Background Knowledge about factors associated with tooth loss contributes to the planning of preventive actions carried out by Oral Health Teams (OHT). Objective To evaluate the proportion of dental extraction indicator in the cities of the state of São Paulo, Brazil, and its relationship with socioeconomic indicators and coverage of the OHT. Method Ecological study using secondary data from 654 municipalities in the state of São Paulo conducted in 2012. The dependent variable was the percentage of dental extraction measured by the proportion of this indicator in relation to individual procedures performed by the OHT provided by the Primary Health Care (PHC) Information System. The population coverage by the OHT, the per capita Gross Domestic Product (GDP), the Human Development Index (HDI), and the São Paulo Social Responsibility Index (IPRS) of the municipalities were evaluated as independent variables. Generalized linear models, formulated by the Wald test p-value, Corrected Akaike Information Criterion (AICc), and degree of freedom, were applied. Results Higher proportions of dental extraction were found in the municipalities with lower coverage by OHT (p<0.0001), lower HDI value (p<0.0001), and higher IPRS value (p=0. 0018). Conclusion Low coverage by OHT and socioeconomic inequalities contribute to increased proportion of dental extractions in the state of São Paulo.

Abstract Introduction: premolar extraction is an alternative for the treatment of class II malocclusion. A change in biomechanics can generate alterations in the Temporomandibular Joint (TMJ), which produces greater dental wear and the appearance of joint dysfunctions. The objective was to assess the effort concentration in the TMJ by means of finite element analysis in class II malocclusions treated with premolar extraction and orthodontics. Method: two 3D simulation models each with bone structures of the 2 jaws, complete dentition and disc in the TMJ. One corresponds to the patient without recurrence (WR) treated with extraction of first premolars and orthodontics, where class I dental stability is maintained. The other model with recurrence (R) treated with extraction of first premolars and orthodontics, increased overjet and overbite and canine class II; the load was applied to the mandibular ramus. Results: loads of 900N triplicated on all structures compared to 300N in both models. However, there were considerable differences between the left and right glenoid cavities in the WR model, at 300N of 19.9 MPa and 900N at 59.3 MPa. Most tensions of the disc occur in the lateral part. Conclusions: due to the asymmetry in the TMJ structures, the stresses and stress concentration differ between the right and left sides in the two models.

OBJECTIVE: The aim of this study was to evaluate the efficacy of photobiomodulation therapy (PBMT) on the mucosal healing of patients submitted to simple dental extractions after head and neck radiation therapy (HNRT). METHODS: Forty surgical procedures were randomly assigned into two groups: G1: dental extraction + PBMT (n = 19) and G2: dental extraction + sham-PBMT (n = 21). All patients received antibiotic therapy and the surgical alveolotomy to promote primary closure of the surgical site. Group 1 was submitted to PMBT according to the following parameters: 808 nm, 40 mW, 100 J/cm2, 70 s, 2.8 J/point, 14 J/session, and area of 0.028cm2. The primary outcome was complete mucosal lining at 14 days, and the secondary outcomes were the presence of infection, postoperative pain, and analgesics intake at 7 days. The patients were evaluated every 7 days until 28 days. RESULTS: Alveolar mucosal lining was faster in G1, and at 14 postoperative days, 94.7% patients evolved with complete alveolar mucosal lining compared to no patient from G2 (p < 0.001). Patients from G1 reported postoperative pain less frequently (G1 = 4, 21.1% × G2 = 14, 66.7%, p = 0.005), and also reported lower intake of analgesic pills at D7 (21.1% × 66.7%, p = 0.005%). PBMT had a significant positive impact on both postoperative pain (NNT = 2.192, CI95% = 1.372-5.445) and mucosal healing (NNT = 1.056, CI95% = 0.954-1.181). CONCLUSIONS: This preliminary study strongly supports the use of PMBT to promote surgical alveolar mucosal lining in a shorter time and with less postoperative pain.

OBJECTIVES: The aim of this study was to evaluate glycemic levels in diabetic patients before, during, and after extractions using 2% lidocaine with 1:100,000 epinephrine (Lido/Epi) and 3% prilocaine with 0.03 IU/mL felypressin (Prilo/Fely). MATERIALS AND METHODS: A double-blind, randomized clinical trial was conducted to evaluate changes in body parameters and glycemic levels in diabetic patients undergoing two anesthetic protocols during dental extractions. During surgery, we evaluated blood pressure (BP), heart rate (HR), saturation (SpO2), and capillary glycemic levels (Gly). These parameters were measured at the following surgical moments: basal, 30 min after medication, incision, tooth removal, suture, and 30 and 60 min after anesthesia. RESULTS: Data analysis showed no differences between the groups considering age, weight, and time spent in surgery. Increased systolic BP and decreased diastolic BP were observed in the lido/epi group. No difference was observed in the prilo/fely group among the surgical moments or between the groups regarding BP. No difference was observed in HR and SpO2 between the groups at any surgical moment. However, differences were found when compared the differences in glycemic and basal levels in both groups with greater decreases in blood glucose values for the lido/epi group. In anxiety level evaluation, there was no difference between the different surgical moments. CONCLUSION: Thus, both lido/epi and prilo/fely (maximum 3.6 mL) can be safely used in controlled diabetic patients CLINICAL RELEVANCE: The use of lidocaine associated with epinephrine did not increase glycemic levels but leads to decrease over time when associated with an anxiety reduction protocol, offering some advantage over prilocaine plus felypressin for diabetic patients.

INTRODUCTION: Oral anticoagulants are widely used worldwide for many systemic diseases. Recent oral surgical protocols suggest that therapeutic levels of the anticoagulant drug should be maintained for simple dental extractions because bleeding complications could be managed with proper local hemostasis. The aim of the present study was to compare bleeding complication of three different local hemostatic agents for dental extractions without interrupting drug administration in patients undergoing oral Vitamin K antagonist chronic anticoagulant therapy. MATERIAL AND METHODS: Randomized control trial of three hemostatic agents for dental extractions, in patients under oral anticoagulant therapy without drug interruption. The present study included 240 patients with international normalized ratio between 1.5 and 3.5. Patients took their anticoagulation drug normally. A single surgeon performed calibrated simple dental extractions and applied a plug of bismuth subgallate (BS), fibrin tissue adhesive (FTA) or microfibrillar collagen (MC), assigned randomly. Statistical analysis of bleeding between the groups was performed using the Chi-square test. RESULTS: There was no hemorrhagic complication in the BS group, and only one in the FTA group. However, in the MC group, 10 patients (12.5%) suffered postoperative bleeding. Data analysis showed statistical differences between the MC group and the other two groups (P < 0.05). DISCUSSION: BS and FTA showed similar clinical effectiveness and were more effective than MC for the control of postoperative bleeding in oral anticoagulated patients.
Aim:To investigate the behavior of children during dental treatments of prophylaxis and dental extraction. Methods: Fifty-eight children, 6 to 9 years of age, and their caregivers participated of this cross-sectional study. Of these, 29 children underwent prophylaxis and 29 dental extraction. Child behavior was assessed through the Brazilian version of the Venham's Behavior Rating Scale (BvVBRS), and dental anxiety was measured by the Venham Picture Test modified (VPTm). Clinical examination was performed by a calibrated examiner (Kappa > 0.7), using the decay, missing, filled index for primary and permanent dentition dmft/DMFT index. The caregivers answered a socioeconomic questionnaire and questions related to the previous dental experience of the children. Descriptive data analysis and Chi-square test were performed, with a significance level of 5%. Results:Regarding behavior, the prophylaxis group presented 100% of children with a positive behavior, while the extraction group contained 75.9% of children with positive behavior, and this difference was statistically significant (p = 0.010). Age, gender, and presence of anxiety prior to the procedures had no statistically significant relationship to the presented child behavior. The presence of caries in the deciduous dentition was associated with child behavior during dental procedures (p = 0.013). Conclusion: It was observed that the type of procedure interfered directly in the negative behavior of the children, with a difference in cooperation with invasive (dental extraction) and non-invasive (prophylaxis) procedures.

PURPOSE: To evaluate the clinical outcomes of platelet-rich fibrin (PRF) application for hemostasis after dental extraction in patients receiving anticoagulant therapy with factor Xa inhibitors. METHODS: In total, 25 patients receiving anticoagulant therapy with rivaroxaban or apixaban who required routine dental extraction were evaluated. In all patients, PRF was used for hemostasis in addition to adapting sutures. Bleeding was subjectively assessed using a sterilize gauze pad at 24, 48, and 72 h after the procedure. RESULTS: All invited participants (n = 25) consented to participate. The PRF clots successfully arrested bleeding after extraction in all patients, with no complications at any time point after the procedure. Favorable soft tissue healing was observed during suture removal at 10 days after the procedure in all patients, with no signs of infection or late healing. CONCLUSIONS: The results of this case series indicate that PRF is a promising natural hemostatic agent for the management of bleeding after dental extraction in patients receiving factor Xa inhibitor therapy. Further controlled clinical studies with larger patient samples are necessary to clarify the findings of this case series.
