Abstract Introduction: Blood pressure (BP) assessment affects the management of arterial hypertension (AH) in chronic kidney disease (CKD). CKD patients have specific patterns of BP behavior during ambulatory blood pressure monitoring (ABPM). Objectives: The aim of the current study was to evaluate the associations between progressive stages of CKD and changes in ABPM. Methodology: This is a cross-sectional study with 851 patients treated in outpatient clinics of a university hospital who underwent ABPM examination from January 2004 to February 2012 in order to assess the presence and control of AH. The outcomes considered were the ABPM parameters. The variable of interest was CKD staging. Confounding factors included age, sex, body mass index, smoking, cause of CKD, and use of antihypertensive drugs. Results: Systolic BP (SBP) was associated with CKD stages 3b and 5, irrespective of confounding variables. Pulse pressure was only associated with stage 5. The SBP coefficient of variation was progressively associated with stages 3a, 4 and 5, while the diastolic blood pressure (DBP) coefficient of variation showed no association. SBP reduction was associated with stages 2, 4 and 5, and the decline in DBP with stages 4 and 5. Other ABPM parameters showed no association with CKD stages after adjustments. Conclusion: Advanced stages of CKD were associated with lower nocturnal dipping and greater variability in blood pressure.


Resumo Introdução: A avaliação da pressão arterial (PA) tem impacto no manejo da hipertensão arterial (HA) na doença renal crônica (DRC). O portador de DRC apresenta padrão específico de comportamento da PA ao longo da monitorização ambulatorial da pressão arterial (MAPA). Objetivos: O objetivo do corrente estudo é avaliar as associações entre os estágios progressivos da DRC e alterações da MAPA. Metodologia: Trata-se de um estudo transversal com 851 pacientes atendidos nos ambulatórios de um hospital universitário que foram submetidos ao exame de MAPA no período de janeiro de 2004 a fevereiro de 2012 para avaliar a presença e o controle da HA. Os desfechos considerados foram os parâmetros de MAPA. A variável de interesse foi o estadiamento da DRC. Foram considerados como fatores de confusão idade, sexo, índice de massa corporal, tabagismo, causa da DRC e uso de anti-hipertensivos. Resultados: A PA sistólica (PAS) se associou aos estágios 3b e 5 da DRC, independentemente das variáveis de confusão. Pressão de pulso se associou apenas ao estágio 5. O coeficiente de variação da PAS se associou progressivamente aos estágios 3a, 4 e 5, enquanto o coeficiente de variação da pressão arterial diastólica (PAD) não demonstrou associação. O descenso da PAS obteve associação com estágios 2, 4 e 5, e o descenso da PAD, com os 4 e 5. Demais parâmetros da MAPA não obtiveram associação com os estágios da DRC após os ajustes. Conclusão: Estágios mais avançados da DRC associaram-se a menor descenso noturno e a maior variabilidade da pressão arterial.

BACKGROUND: Quality of life (QoL) is a measure to evaluate kidney transplant (KT) results. AIM: To describe the QoL profile in a larger sample of Brazilian patients who underwent KT according to age, sex, and access to KT. METHODS: We conducted a secondary data analysis of the ADHERE BRAZIL multicenter cross-sectional study including 1105 patients from 20 centers, considering KT access region and transplant activity. QoL was assessed by the WHOQOL-BREF. Data was compared using Generalized Estimating Equations. RESULTS: Overall, 58.5 % of the patients were men, mean age of 47.6 ± 12.6 years. The general QoL score was 81 ± 15.1, 58.6 ± 11.6 for physical, 65.5 ± 11.4 for psychological, 68.3 ± 17.1 for social relationships, and 64.2 ± 13.3 for environmental domain. Higher QoL scores were observed in men compared to women in three WHOQOL-BREF domains: psychological (OR:2.62; CI, 1.29 ̶ 3.95, p < 0.0001), social relationships (OR:3.21; CI, 1.2 ̶ 5.23, p = 0.002) and environmental (OR:3.79; CI:2.23 ̶ 5.35, p < 0.0001). Younger patients (18-44 years) had higher scores in the psychological (OR:-2.69; CI, -4.13 ̶ -1.25; p < 0.001; OR:-3.52; CI, -5.39 ̶ -1.66; p < 0.001) and social (OR:-3.46; CI, -5.64 ̶ -1.27; p = 0.002; OR:-7.17; CI, -10 ̶ -4.35; p < 0.0001) domains than older ones (45-59 and > 60 years, respectively). Patients from higher KT access region had higher scores in environmental domain (OR:3.53; CI, 0.28 ̶ 6.78; p = 0.033). CONCLUSIONS: Featuring the results of KT under patient view, the physical and social relationships domains were the most and least affected, respectively. Lower QoL subgroups (females and age > 45 years) should be targeted in future multi-professional interventions.

Abstract Introduction: Cardiovascular disease is an important cause of death among patients with chronic kidney disease (CKD). Valve calcification is a predictor of cardiovascular mortality and coronary artery disease. Objective: To assess heart valve disease frequency, associated factors, and progression in CKD patients. Methods: We conducted a retrospective study on 291 CKD patients at Hospital das Clínicas de Pernambuco. Inclusion criteria were age ≥ 18 with CKD and valve disease, while those on conservative management or with missing data were excluded. Clinical and laboratory variables were compared, and patients were categorized by dialysis duration (<5 years; 5-10 years; >10 years). Statistical tests, including chi-square, Fisher's exact, ANOVA, and Kruskal-Wallis, were employed as needed. Simple and multivariate binary regression models were used to analyze valve disease associations with dialysis duration. Significance was defined as p < 0.05. Results: Mitral valve disease was present in 82.5% (240) of patients, followed by aortic valve disease (65.6%; 86). Over time, 106 (36.4%) patients developed valve disease. No significant association was found between aortic, pulmonary, mitral, or tricuspid valve disease and dialysis duration. Secondary hyperparathyroidism was the sole statistically significant factor for mitral valve disease in the regression model (OR 2.59 [95% CI: 1.09-6.18]; p = 0.031). Conclusion: CKD patients on renal replacement therapy exhibit a high frequency of valve disease, particularly mitral and aortic valve disease. However, no link was established between dialysis duration and valve disease occurrence or progression.


Resumo Introdução: Doenças cardiovasculares são uma causa significativa de morte em pacientes com Doença Renal Crônica (DRC). A calcificação valvar é preditor de mortalidade cardiovascular e doença arterial coronariana. Objetivo: Avaliar a frequência, fatores associados e progressão de valvopatias em pacientes com DRC. Métodos: Coorte retrospectiva com 291 pacientes ambulatoriais no Hospital das Clínicas de Pernambuco. Inclusão: ≥18 anos com DRC e valvopatia; exclusão: tratamento conservador ou dados incompletos. Variáveis clínicas e laboratoriais foram comparadas e categorizadas por tempo de terapia dialítica (TTD): <5 anos, 5-10 anos, >10 anos. Foram aplicados os testes Qui-quadrado, exato de Fisher, ANOVA, Kruskal-Wallis. Associação entre valvopatia e TTD foi avaliada por regressão binária. Significância foi definida como p < 0,05. Resultados: A valvopatia mitral foi encontrada em 82,5% (240) dos casos, seguida da aórtica (65,6%; 86). Houve progressão da doença valvar em 106 (36,4%) pacientes. Não houve associação entre valvopatias aórtica, pulmonar, mitral ou tricúspide e TTD. Hiperparatireoidismo secundário foi a única variável explicativa significativa na regressão para valvopatia mitral (OR 2,59 [IC95%: 1,09-6,18]; p = 0,031). Conclusão: Encontramos alta frequência de valvopatias, especialmente mitral e aórtica, aem pacientes com DRC. Não houve associação entre TTD e valvopatia.

BACKGROUND: The Covid-19 pandemic has affected patients with end-stage kidney disease (ESKD). Whether dialysis parameters have a prognostic value in ESKD patients with Covid-19 remains unclear. MATERIALS AND METHODS: We retrospectively evaluated clinical characteristics, blood pressure (BP) and dialysis parameters in ESKD patients undergoing maintenance outpatient hemodialysis, with (Covid-ESKD) and without (No-Covid-ESKD) Covid-19, at four Brazilian hemodialysis facilities. The Covid-ESKD (n = 107; 54% females; 60.8 ± 17.7 years) and No-Covid-ESKD (n = 107; 62% females; 58.4 ± 14.6 years) groups were matched by calendar time. The average BP and dialysis parameters were calculated during the pre-infection, acute infection, and post-infection periods. The main outcomes were Covid-19 hospitalization and all-cause mortality. RESULTS: Covid-ESKD patients had greater intradialytic and postdialysis systolic BP and lower predialysis weight, postdialysis weight, ultrafiltration rate, and interdialytic weight gain during acute-illness compared to 1-week-before-illness, while these changes were not observed in No-Covid-ESKD patients. After 286 days of follow-up (range, 276-591), there were 18 Covid-19-related hospitalizations and 28 deaths among Covid-ESKD patients. Multivariable logistic regression analysis showed that increases in predialysis systolic BP from 1-week-before-illness to acute-illness (OR, 95%CI = 1.06, 1.02-1.10; p = .004) and Covid-19 vaccination (OR, 95%CI = 0.16, 0.04-0.69; p = .014) were associated with hospitalization in Covid-ESKD patients. Multivariable Cox-regression analysis showed that Covid-19-related hospitalization (HR, 95%CI = 5.17, 2.07-12.96; p < .001) and age (HR, 95%CI = 1.05, 1.01-1.08; p = .008) were independent predictors of all-cause mortality in Covid-ESKD patients. CONCLUSION: Acute Covid-19 illness is associated with variations in dialysis parameters of volume status in patients with ESKD. Furthermore, increases in predialysis BP during acute Covid-19 illness are associated with an adverse prognosis in Covid-ESKD patients.


Dialysis parameters were influenced by SARS-CoV-2 infection and may have prognostic value in patients with Covid-19.Increases in blood pressure during acute Covid-19 illness and the lack of vaccination for Covid-19 were predictors of hospitalization for Covid-19.Hospitalization for Covid-19 and age were independent risk factors for all-cause death.

Worldwide the assistance on renal replacement therapy (RRT) is carried out mainly by private for-profit services and in a market with increase in mergers and acquisitions. The aim of this study was to conduct an integrative systematic review on privatization and oligopolies in the RRT sector in the context of contemporary capitalism. The inclusion criteria were scientific articles without language restrictions and that addressed the themes of oligopoly or privatization of RRT market. Studies published before 1990 were excluded. The exploratory search for publications was carried out on February 13, 2024 on the Virtual Health Library Regional Portal (VHL). Using the step-by-step of PRISMA flowchart, 34 articles were retrieved, of which 31 addressed the RRT sector in the United States and 26 compared for-profit dialysis units or those belonging to large organizations with non-profit or public ones. The main effects of privatization and oligopolies, evaluated by the studies, were: mortality, hospitalization, use of peritoneal dialysis and registration for kidney transplantation. When considering these outcomes, 19 (73%) articles showed worse results in private units or those belonging to large organizations, six (23%) studies were in favor of privatization or oligopolies and one study was neutral (4%). In summary, most of the articles included in this systematic review showed deleterious effects of oligopolization and privatization of the RRT sector on the patients served. Possible explanations for this result could be the presence of conflicts of interest in the RRT sector and the lack of incentive to implement the chronic kidney disease care line. The predominance of articles from a single nation may suggest that few countries have transparent mechanisms to monitor the quality of care and outcomes of patients on chronic dialysis.

INTRODUCTION: Hemodiafiltration (HDF) and high-flux hemodialysis (hf-HD) are different methods of kidney replacement therapy (KRT) used for the treatment of kidney failure patients. A debate has raged over the last decade about the survival benefit of patients with the use of HDF compared with hf-HD, but with divergent results from randomized controlled trials. Therefore, this study aimed to perform a meta-analysis to compare HDF and hf-HD regarding all-cause and cardiovascular mortality. METHODS: PubMed and Cochrane databases were searched until July 19, 2023, for randomized clinical trials comparing HDF and hf-HD in patients on maintenance dialysis. A meta-analysis was performed using Stata 16.1, applying fixed or random effect models according to the heterogeneity between studies. FINDINGS: Of the 496 studies found, five met the inclusion criteria. Compared with the hf-HD group, the risk ratio (RR) for all-cause mortality with HDF use was 0.76 (95% CI: 0.67-0.88, I2 = 0%). HDF was associated with lower cardiovascular mortality, although the sensitivity analysis showed that the result differed between scenarios. Subgroup analysis showed lower all-cause mortality among patients without diabetes in the HDF group compared with hf-HD (RR 0.66, 95% CI: 0.51-0.81, I2 = 0%), but not in diabetic patients (RR = 0.89, 95% CI: 0.65-1.12, I2 = 0.0%). A subgroup analysis considering convection volumes was not performed, but the studies with the highest weight in the meta-analysis described convection volume as more than 20 L/session. DISCUSSION: More clinical studies considering critical risk factors, such as advanced age and preexisting cardiovascular disease, are needed to confirm the supremacy of HDF over hf-HD on the survival of patients treated by these two forms of kidney replacement therapy.

Diet therapy in conservative treatment of chronic kidney disease involves protein restriction, but there is not enough evidence to recommend a particular type of protein, whether animal or plant based. However, studies suggest that plant-based diets help reduce the consumption of total and animal protein, reduce the need for nephroprotective drugs, improve complications and bring advantages in terms of disease progression and patient survival. The article considers up-to-date data on the effects of this diet and observed that when low in protein, primarily vegetable and in some cases supplemented with ketoanalogues, it can result in positive clinical outcomes, such as: delay in the decrease in the glomerular filtration rate, lower concentrations of urea, reduction of serum creatinine and phosphorus concentrations, lower metabolic acidosis, higher insulin sensitivity and lower systemic inflammation. As a whole, this dietary pattern may be able to postpone the start of dialysis with less progression of renal insufficiency. Additional research is needed to better characterize this dietary pattern.


La dietoterapia en el tratamiento conservador de la enfermedad renal crónica implica la restricción de proteínas, pero aún no hay pruebas suficientes para recomendar un tipo concreto de proteínas, ya sean animales o vegetales. Sin embargo, los estudios sugieren que las dietas basadas en plantas ayudan a reducir la ingesta de proteínas totales y animales, disminuyen la necesidad de fármacos nefroprotectores, mejoran las complicaciones y presentan ventajas con respecto a la progresión de la enfermedad y la supervivencia de los pacientes. En este artículo se consideran datos actualizados sobre los efectos de esta dieta y se observa que, cuando es hipoproteica, principalmente vegetal y en algunos casos se complementa con cetoanálogos, puede dar lugar a resultados clínicos positivos, como una disminución retardada de la tasa de filtración glomerular, concentraciones más bajas de urea, concentraciones reducidas de creatinina y fósforo séricos, menor acidosis metabólica, mayor sensibilidad a la insulina y menor inflamación sistémica. En conjunto, este patrón dietético tiene el potencial de retrasar el inicio de la diálisis con una menor progresión de la insuficiencia renal. Es necesario seguir investigando para caracterizar mejor este patrón dietético.

BACKGROUND: Type 2 diabetes mellitus is one of the most common causes of chronic kidney disease (CKD) worldwide and prevalence of 1.75 per 100 inhabitants in Colombia. The aim of this study was to describe the treatment patterns of a group of patients with type 2 diabetes mellitus and CKD in an outpatient setting from Colombia. METHODS: A cross-sectional study in adult patients with type 2 diabetes mellitus and CKD identified in the Audifarma S.A. administrative healthcare database between April 2019 and March 2020 was performed. Sociodemographic, clinical and pharmacological variables were considered and analyzed. RESULTS: A total of 14,722 patients with type 2 diabetes mellitus and CKD were identified, predominantly male (51%), with a mean age of 74.7 years. The most common treatment patterns of type 2 diabetes mellitus included the use of metformin monotherapy (20.5%), followed by the combination of metformin + dipeptidyl peptidase-4 inhibitor (13.4%). Regarding the use of drugs with nephroprotective properties, the most prescribed treatments were angiotensin receptor blockers (67.2%), angiotensin converting enzyme inhibitors (15.8%), sodium glucose cotransporter 2 inhibitors (SGLT2i) (17.0%) and glucagon-like peptide-1 analogs (GLP1a) (5.2%). CONCLUSION: In Colombia, the majority of patients with type 2 diabetes mellitus and CKD identified in this study were treated with antidiabetic and protective medications to ensure adequate metabolic, cardiovascular, and renal control. The management of type 2 diabetes mellitus and CKD may be improved if the beneficial properties of new groups of antidiabetics (SGLT2i, GLP1a), as well as novel mineralocorticoid receptor antagonists, are considered.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is the most common genetic cause of chronic kidney disease (CKD) that requires dialysis. Knowing geographical clusters can be critical for early diagnosis, progression control, and genetic counseling. The objective was to establish the prevalence, geographic location, and ethnic groups of patients with ADPKD who underwent dialysis or kidney transplant in Colombia between 2015 and 2019. METHODS: We did a cross-sectional study with data from the National Registry of Chronic Kidney Disease (NRCKD) managed by the High-Cost Diseases Fund (Cuenta de Alto Costo [CAC] in Spanish) between July 1, 2015, and June 30, 2019. We included Colombian population with CKD with or without renal replacement therapy (RRT) due to ADPKD. Crude and adjusted prevalence rates were estimated by state and city. RESULTS: 3,339 patients with ADPKD were included, period prevalence was 9.81 per 100,000 population; there were 4.35 cases of RRT per 100,000 population, mean age of 52.58 years (± 13.21), and 52.78% women. Seventy-six patients were Afro-Colombians, six were indigenous, and one Roma people. A total of 46.07% began scheduled dialysis. The highest adjusted prevalence rate was in Valle del Cauca (6.55 cases per 100,000 population), followed by Risaralda, and La Guajira. Regarding cities, Cali had the highest prevalence rate (9.38 cases per 100,000 population), followed by Pasto, Medellin, and Bucaramanga. CONCLUSIONS: ADPKD prevalence is lower compared to Europe and US; some states with higher prevalence could be objective to genetic prevalence study.

Chronic kidney disease (CKD) has become a public health concern over the last several years. Nowadays developed countries spend around 3% of their annual health-care budget on patients with CKD. According to the scientific community the most remarkable risk factors for CKD are diabetes and hypertension. Unknown CKD etiology has been reported as a global phenomenon including uncommon risk factors such as: dehydration, leptospirosis, heat stress, water quality, and others. This study aims to report non-traditional risk factors for ESRD based on a scoping review methodology. The scoping review methodology described by Arksey and O'Malley was used by performing an extensive review of the information. A total of 46 manuscripts were reviewed. The non-traditional ESRD risk factors are depicted based on six categories. Gender and ethnicity have been considered as risk factors for ESRD. Erythematous systemic lupus (ESL) is reported as an important risk factor for ESRD. Pesticide use has been an significant risk factor due to its effects on human and environmental health. Some compounds commonly used in homes against insects and plants are related to ESRD. Congenital and hereditary diseases in the urinary tract have been studied as a cause of ESRD in children and young adults. End-stage renal disease is a major concern for public health on a global level. As it can be seen, non-traditional risk factors are several and have different etiologies. It is necessary to put the issue on the table and add it to the public agenda in order to find multidisciplinary solutions.

BACKGROUND: Patients with end-stage renal disease on hemodialysis have postural balance impairments due to uremic syndrome and hemodialysis complications. This study evaluated the factors associated with postural balance in patients on hemodialysis. METHODS: This cross-sectional study included patients on hemodialysis [n = 93, 62.0 (16.0) years]. Postural balance was evaluated using a force plate in a static position with eyes opened and eyes closed on a firm surface and with eyes opened on a foam surface to register the center of pressure path length. Physical function was assessed by isometric handgrip force, the 5-repetition sit-to-stand test, and gait speed. The level of physical activity and quality of life were evaluated by accelerometry and the 36-Item Short Form Health Survey, respectively. FINDINGS: After adjustment for potential confounders, the multiple linear regression analysis showed that the presence of diabetes mellitus and neurological disease and gait speed were significantly associated with the center of pressure path length in the eyes opened test (R2 = 0.263; p < 0.001). The center of pressure path length in the eyes closed test was significantly associated with the presence of neurological disease (R2 = 0.177; p = 0.002). The center of pressure path length in the eyes opened on a foam surface test was significantly associated with the presence of diabetes mellitus (R2 = 0.223; p < 0.001). INTERPRETATION: Poor postural balance was associated with the presence of diabetes mellitus and neurological disease and a slower gait speed in patients on hemodialysis.

In this case report, we present the evolution of a heart failure with reduced ejection fraction (HFrEF) patient who was set to receive end-of-life care but demonstrated improvement following treatment with vericiguat in combination with foundational therapy. Vericiguat is a novel soluble guanylate cyclase stimulant that has been proven helpful for treating decompensated heart failure with HFrEF, decreasing hospitalization rates and mortality of cardiovascular causes. This medication is currently indicated in patients who require IV diuretics administration or hospitalization due to decompensated heart failure. This is a case study of a 62-year-old woman with dilated heart failure and reduced left ventricular ejection fraction (LVEF), who was a wheelchair user due to severe cardiovascular symptoms and various comorbidities, who was referred to our heart failure program for treatment. Despite previous treatment, the patient experienced persistent cardiovascular symptoms and required palliative care. After optimizing the foundational therapy, the patient's condition improved but continued to require hospitalization. Vericiguat was initiated as an add-on. After six months, the patient's LVEF improved by 9%, and she is now asymptomatic with a considerable decrease in pro-B-type natriuretic peptide levels and is wheelchair independent due to enhance exercise resistance. However, the echocardiogram revealed a progression in the dysfunction of both the mitral and aortic valves. The patient's renal function and quality of life scores also changed over time. Vericiguat therapy, as an adjunct to foundational therapy, improved exercise tolerance and symptom relief. However, further investigation is necessary to assess the effects of vericiguat on renal function and disease progression in individuals with HFrEF.

BACKGROUND: Strongyloidiasis is an infectious disease that can be fatal in immunocompromised patients. Patients with end-stage renal failure who are on dialysis have a considerably weakened immune system, and organ transplantation is a major risk factor for severe strongyloidiasis. Knowledge of the local epidemiology in tropical and subtropical areas is an essential prerequisite for designing an appropriate strategy to prevent this potentially lethal complication. In this study, we aimed to estimate the prevalence and associated risk factors of S. stercoralis infection in patients on dialysis in Cochabamba, Bolivia. METHODS: A cross-sectional study was carried out among patients undergoing haemodialysis in Cochabamba (elevation 2,500 m, temperate climate), collecting information on socio-demographic, lifestyle, and clinical variables, and using one coproparasitological technique (the modified Baermann technique) and one serological (ELISA) test for S.stercoralis diagnosis. RESULTS: In total, 149 patients participated in the study (mean age = 51.4 years, 48.3% male). End-stage renal disease was predominantly (59%) of hypertensive and/or diabetic origin. The positive serological prevalence was 18.8% (95% CI: 13.3%-25.9%). Based on the sensitivity and specificity of the ELISA test, the estimate of the actual prevalence was 15.1% (95% CI: 9.4%-20.7%). Stool samples of 105 patients (70.5%) showed a coproparasitological prevalence of 1.9% (95% CI: 0.52%-6.68%). No potential risk factors were significantly associated with S. stercoralis infection. CONCLUSIONS: We found a high seroprevalence of S. stercoralis in Bolivian patients undergoing haemodialysis in Cochabamba. We recommend presumptive antiparasitic treatment at regular intervals to avoid the potentially fatal complications of severe strongyloidiasis.

Hemodialysis, along with chronic kidney disease (CKD), intensifies the inflammatory process and oxidative stress in patients undergoing treatment. In this context, Vitamin E supplementation can mitigate the deleterious effects resulting from these processes. This is a systematic review whose objective was to evaluate the effect of Vitamin E supplementation on inflammatory biomarkers and oxidative stress in patients with CKD on hemodialysis. This review was prepared according to the methodology for systematic reviews and meta-analyses (PRISMA) and the search was performed in Pubmed, Cochrane Library, Scopus and Web of Science databases. The risk of bias of the included studies was assessed with the Cochrane Risk of Bias Tool. Twelve studies developed between 2006 and 2020 were included in this review. Most of them (n= 11) used vitamin E doses ranging from 400 IU to 888 IU and the supplementation time ranged from 2 weeks to 12 months. Of all the 12 articles included, 25% (n= 3) analyzed supplementation on biomarkers of oxidative stress and 25% (n= 3) addressed these parameters simultaneously. A positive effect was observed in 58.4% of the studies (n= 7). Thus, Vitamin E supplementation can be effective in mitigating the inflammatory process and oxidative stress, however, it is worth noting that the effect depends on the dose and time of supplementation.


La hemodiálisis, junto con la enfermedad renal crónica (ERC), intensifica el proceso inflamatorio y el estrés oxidativo en los pacientes en tratamiento. En este contexto, la suplementación con vitamina E puede mitigar los efectos nocivos resultantes de estos procesos. Esta es una revisión sistemática cuyo objetivo fue evaluar el efecto de la suplementación con vitamina E sobre biomarcadores inflamatorios y estrés oxidativo en pacientes con ERC en hemodiálisis. La revisión se elaboró según la metodología para revisiones sistemáticas y metanálisis (PRISMA) y la búsqueda se realizó en las bases de datos Pubmed, Cochrane Library, Scopus y Web of Science. El riesgo de sesgo de los estudios incluidos se evaluó con la Herramienta Cochrane de Riesgo de Sesgo (Cochrane Risk of Bias Tool). En esta revisión se incluyeron doce estudios desarrollados entre 2006 y 2020. La mayoría (n= 11) utilizó dosis de vitamina E que oscilaban entre 400 UI y 888 UI y el tiempo de suplementación osciló entre 2 semanas y 12 meses. De los 12 artículos incluidos, 25% (n= 3) analizaba la suplementación en biomarcadores inflamatorios, 50% (n= 6) en biomarcadores de estrés oxidativo y 25% (n= 3) en estos parámetros simultáneamente. Se observó un efecto positivo en 58,4% de los estudios (n= 7). Por lo tanto, la suplementación con vitamina E puede ser efectiva para mitigar el proceso inflamatorio y el estrés oxidativo, sin embargo, vale la pena señalar que el efecto depende de la dosis y el tiempo de suplementación.

INTRODUCTION: Hemodialysis patients have complications that increase fear of falling (FOF). This study evaluated the association between FOF and a retrospective history of falls in hemodialysis patients and investigated the ability of the Fall Efficacy Scale-International (FES-I) to discriminate fallers from nonfallers. METHODS: A retrospective study was conducted with 183 patients to investigate the history of falls in the last 12 months and to evaluate FOF by the FES-I. RESULTS: The univariate linear regression model showed that the FES-I score was significantly associated with a history of falls (p = 0.01). After adjustment for potential confounders, this association remained significant (R2  = 0.19, p < 0.001). The FES-I score showed an area under the curve of 0.660 with a cutoff point of 25 (sensitivity-61.8%; specificity-62.2%). CONCLUSION: FOF was associated with a history of falls in the 12-month period in hemodialysis patients, and FOF assessed by the FES-I was able to discriminate fallers from nonfallers.

Objetivo:Identificar as intervenções de enfermagem prestada aos pacientes que realizam hemodiálise. Método:Trata-se de um artigo de revisão integrativa realizado por meio da análise de 10 artigos científicos Portal da Biblioteca Virtual em Saúde (BVS), nas bases de dados Literatura Latino-Americano do Caribe em Ciências da Saúde (LILACS) e Biblioteca Eletrônica Base de Dados de Enfermagem (BDENF) e Scientific Electronic Library Online (SCIELO). Resultados: O enfermeiro é o responsável pela avaliação e manutenção dos acessos em hemodiálise, à interpretação de exames laboratoriais e também, pela tomada de decisões juntamente com a equipe médica, além de ser capaz de prevenir, identificar e tratar complicações apresentadas pelos pacientes antes, durante e após o procedimento. Conclusão:Em suma, essa pesquisa é importante porque fornece subsídios, conscientizar a equipe responsável pelo tratamento hemodialítico, avalia as necessidades e a importância da qualidade de vida do paciente, em fazer hemodiálise e fornecer a eles métodos otimizados desta forma de tratamento.

Resultados: El enfermero es responsable de la evaluación y mantenimiento de los accesos de hemodiálisis, la interpretación de las pruebas de laboratorio y también de la toma de decisiones junto con el equipomédico, además de poder prevenir, identificar y tratar las complicaciones presentadas por los pacientes antes, durante y después del procedimiento. Conclusión: En definitiva, esta investigación es importante porque proporciona subvenciones, sensibiliza alequipo responsable de la hemodiálisis, evalúa las necesidades y la importancia de la calidad de vida del paciente en la hemodiálisis y le proporciona métodos optimizados de esta forma de tratamiento.

Abstract The objective was to systematically review the relationship between Chronic Kidney Failure and Hemodialysis with cognitive function. This systematic review followed the parameters proposed by PRISMA. The search for articles was carried out in the PubMed database. A total of 113 articles were found, of which 31 were selected for analysis. Analyzing the studies' samples, it was found that 19.3% of them had a control group, that the most used instrument was the Mini-Mental State Examination (41.9%), and that patients of older age and patients with longer hemodialysis treatment had worse cognitive scores. It is concluded that HD can reduce the cognitive function of people with CKF.


Resumo O objetivo foi revisar sistematicamente a relação da Insuficiência Renal Crônica e da Hemodiálise com a função cognitiva. Esta pesquisa de revisão sistemática seguiu os parâmetros propostos pelo PRISMA. A busca dos artigos foi realizada na base de dados PubMed. Foram encontrados 113 artigos, sendo que destes 31 foram selecionados para análise. Analisando a amostra dos estudos verificou-se que 19,3% dos estudos tiveram grupo controle, que o instrumento mais utilizado foi Mini Exame do Estado Mental (41,9%) e que pacientes com idade mais avançada e os pacientes com mais tempo de tratamento hemodialítico tinham piores escores cognitivos. Conclui-se que a HD pode reduzir a função cognitiva de pessoas com IRC.

Introdução: o objetivo foi avaliar a prevalência do distúrbio mineral e ósseo em pacientes com doença renal crônica e a associação entre Taxa de Filtração Glomerular estimada (TFGe) e os indicadores do distúrbio mineral e ósseo (DMO) (cálcio, fósforo e PTH) em pacientes renais crônicos não dialíticos. Materiais e Métodos: estudo seccional da linha de base de uma coorte de dois anos, com adultos e idosos renais crônicos em tratamento conservador. Para identificação do DMO utilizamos os seguintes valores séricos: PTH (> 150 pg/mL) e/ou hipocalcemia (Ca < 8,8mg/dl) e/ou hiperfosfatemia (P > 4,6 mg/dl). Na análise estatística utilizou-se: regressão de Poisson; T de Student, Mann Whitney e correlações de Pearson e Spearman. Nível de significância foi de 5%. Resultados: prevalência de DMO de 54,6% (n=41) (IC 95%: 43,45 - 65,43). A maior prevalência de DMO foi em pessoas do sexo feminino, alfabetizadas, idosas, não etilistas, não tabagistas, sedentárias e de cor de pele branca, porém, sem diferença estatística entre os grupos com e sem DMO. As correlações entre P e PTH com TFGe foram significativas, inversas, de força moderada (p= <0,005 e p = 0,003; coeficientes de correlação = - 0,312 e - 0,379 respectivamente). Discussão:os achados desse estudo mostraram que existe uma lacuna no acompanhamento do DMO-DRC pela atenção primária e a prática clínica deve ser revista. Conclusão:identificou-se prevalência robusta de DMO nos estágios precoces da DRC, além de correlações significativas entre o aumento dos níveis de fósforo e PTH e piora das funções renais.


Introduction: mineral and bone disorder (BMD) is a serious complication of chronic kidney disease (CKD) that increases risks for death from cardiovascular causes and impairs quality of life of affected patients. Objetive: to evaluate the prevalence of BMD in patients with CKD and the association between estimated Glomerular Filtration Rate (eGFR) and BMD indicators (calcium, phosphorus and PTH) in non-dialysis patients. Materials and Methods:sectional study of a two-year cohort of chronic renal adults and elderly patients on conservative treatment. BMD was identified by serum values of: PTH (> 150 pg/mL) and/or hypocalcemia (Ca < 8.8mg/dl) and/or hyperphosphatemia (P > 4.6 mg/dl). The statistical analysis used: Poisson regression; Student's T, Mann Whitney and Pearson and Spearman correlations with 5% significance level. Results:BMD prevalence was 54.6% (n=41) (95% CI: 43.45 - 65.43), more frequent in women, literate, elderly, non-drinkers, non-smokers, sedentary and white skin color. Correlations between P and PTH with GFRe were significant, inverse, moderate strength (p= <0.005 and p = 0.003; correlation coefficients = - 0.312 and - 0.379 respectively). Discussion: the findings of this study highlighted gaps in the monitoring of BMD-DRC by primary care, requiring a review of clinical practices. Conclusion: robust prevalence of BMD in the early stages of CKD was identified, in addition to correlations between increased phosphorus and PTH levels and worsening kidney function.

Objetivo: evaluar la prevalencia de la DMO en pacientes con RDC y la asociación entre la tasa de filtración glomerular estimada (TFGe) y los indicadores de DMO (calcio, fósforo y PTH) en pacientes no dialíticos. Materiales y Métodos: estudio seccional de una cohorte de dos años de pacientes renales crónicos adultos y ancianos en tratamiento conservador. La DMO se identificó por los valores séricos de: PTH (> 150 pg/mL) y/o hipocalcemia (Ca < 8,8mg/dl) y/o hiperfosfatemia (P > 4,6 mg/dl). El análisis estadístico utilizado: regresión de Poisson; T de Student, Mann Whitney y correlaciones de Pearson y Spearman con un nivel de significación del 5%. Resultados: la prevalencia de DMO fue del 54,6% (n=41) (IC 95%: 43,45 - 65,43), más frecuente en mujeres, alfabetizadas, de edad avanzada, no bebedoras, no fumadoras, sedentarias y de color de piel blanca. Las correlaciones entre el P y la PTH con el GFRe fueron significativas, inversas, de fuerza moderada (p= <0,005 y p = 0,003; coeficientes de correlación = - 0,312 y - 0,379 respectivamente). Discusión: los resultados de este estudio evidencian lagunas en el seguimiento de la DMO-DRC por parte de la atención primaria, lo que requiere una revisión de las prácticas clínicas. Conclusión: se identificó una fuerte prevalencia de la DMO en las primeras fases de la ERC, además de correlaciones entre el aumento de los niveles de fósforo y PTH y el empeoramiento de la función renal.

Resumen: Introducción: La enfermedad renal crónica (ERC) pediátrica es un problema de salud grave, que repercute en la vida de adulto y genera impacto psicosocial en la red familiar. Objetivo: Develar las representaciones y perspectivas de los cuidadores principales de hijos con enfermedad renal crónica. Metodología: Estudio con diseño fenomenológico administrado en 9 informantes, recolección de la información a través de entrevistas en profundidad, análisis de categorías, triangulación por investigador, resguardo de criterios de rigor de Guba y Lincoln, y respeto de los principios éticos de Ezekiel Emanuel. Resultados: Emergieron las metacategorías "viviendo la enfermedad", "enfermedad altamente demandante" y "redes de apoyo presentes". Discusión: El afrontamiento hacia la enfermedad se ve amenazado por el cansancio físico, incertidumbre y alteración del apoyo dentro y fuera de la estructura familiar; similar resultado se encuentra en el estudio base. El manejo del niño lo realiza la madre en casi la totalidad de las actividades asociadas al tratamiento. Conclusiones: La mujer proporciona cuidados en forma invisible y continua. Es necesario que se visibilice como problemática social, se establezcan políticas con enfoque de género que determinen correcciones de inequidades que proporcionan los estereotipos culturales, al igual que se visibilice la necesidad de una mayor intervención de enfermería como apoyo al cuidado informal.


Resumo: Introdução: A doença renal crônica infantil (DRC) é um grave problema de saúde que afeta a vida adulta e gera impacto psicossocial na rede familiar. Objetivo: Desvelar as representações e perspectivas de cuidadores primários de filhos com doença renal crônica. Metodologia: Estudo com desenho fenomenológico aplicado em 9 informantes, coleta de informações por meio de entrevistas em profundidade, análise de categorias, triangulação por pesquisador, resguardando os rigorosos critérios de Guba e Lincoln, e respeito aos princípios éticos de Ezekiel Emanuel. Resultados: Emergiram as metacategorias "vivendo a doença", "doença muito exigente" e "redes de apoio presentes". Discussão: O enfrentamento da doença é ameaçado pelo cansaço físico, incerteza e alteração do suporte dentro e fora da estrutura familiar, resultado semelhante é encontrado no estudo de base. O manejo da criança é realizado pela mãe em quase as atividades associadas ao tratamento desta Conclusões: As mulheres prestam cuidados de forma invisível e contínua. É necessário torná-la visível como problema social, estabelecer políticas com perspectiva de gênero que determinem correções das iniquidades que os estereótipos culturais proporcionam, bem como tornar visível a necessidade de maior intervenção da enfermagem como suporte para o cuidado informal.


Abstract: Introduction: Pediatric chronic kidney disease (CKD) is a serious health problem that affects the life of the adults and generates psychosocial impact on the family network. Objective: To reveal the representations and perspectives of primary caregivers of children with chronic kidney disease. Methodology: Study with phenomenological design administered in 9 informants, collection of information through in-depth interviews, category analysis, triangulation by researcher, safeguarding the rigorous criteria of Guba and Lincoln, and respect for the ethical principles of Ezekiel Emanuel. Results: The meta categories "living the disease", "highly demanding disease" and "support networks present" emerged. Discussion: Coping with the disease is threatened by physical fatigue, uncertainty, and alteration of support within and outside the family structure. A similar result is found in the base study. The management of the child is carried out by the mother in almost all the activities associated with its treatment. Conclusions: Women provide care invisibly and continuously. It is necessary to make it visible as a social problem, to establish policies with a gender perspective that determine corrections of inequities that cultural stereotypes provide, as well as to make visible the need for greater nursing intervention as support for informal care.

Resumen Objetivo: Describir las tendencias del diagnóstico primario de defunción, el diagnóstico de egreso hospitalario así como los años de vida potencialmente perdidos de pacientes con enfermedad renal crónica compatible con enfermedad renal crónica de causas no tradicionales según datos registrados en bases de acceso público en el país entre 2014 y 2019. Métodos: Se efectuó un estudio longitudinal descriptivo de la información sobre defunciones y egresos hospitalarios registrada en bases de datos de acceso público sobre el diagnóstico primario de enfermedad renal crónica compatible con enfermedad renal crónica de causas no tradicionales. Con los resultados obtenidos se calcularon porcentajes y frecuencias absolutas y relativas de la mortalidad y los egresos hospitalarios. Las tasas de mortalidad general, específica y ajustada fueron calculadas por el método directo. Además, se estimaron, por provincia y sexo, los años de vida potencialmente perdidos en el país. Resultados: En Costa Rica, entre 2014 y 2019, se registraron 2548 muertes y 1893 egresos hospitalarios relacionados con la Enfermedad Renal Crónica de Causas no tradicionales. Se encontró que las tasas de mortalidad ajustadas fueron elevadas para las provincias de Guanacaste y Limón en comparación con las tasas nacionales y con respecto a las de otras provincias. En la provincia de Guanacaste, además, la tendencia de egresos resultó elevada. Adicionalmente, se detectó que los años de vida potencialmente perdidos evidenciaron un incremento progresivo y constante en el país, predominantemente en el sexo masculino y en la provincia de Guanacaste. Se reportaron trece defunciones de personas menores de veinte años de edad. Conclusión: La actualización de los datos en las tendencias de mortalidad ha permitido identificar que las provincias de Guanacaste y Limón presentan tasas ajustadas superiores a la media nacional, así como menores edades promedio de defunción según el resto de las provincias. Las defunciones y egresos hospitalarios en personas menores de veinte años de edad sugieren daño renal temprano y no relacionado con enfermedad ocupacional. La mortalidad y los años de vida potencialmente perdidos proporcionan información sobre la carga de enfermedad en la población del país. Se debe agregar que la codificación específica para vigilar la patología estaba escasamente registrada en el periodo de estudio.


Abstract Aim: To describe the trends of the primary diagnosis of death, the diagnosis of hospital discharge, as well as years of potential life lost by patients with chronic kidney disease compatible with chronic kidney disease of non-traditional causes according to data registered in public access databases in the country between 2014 and 2019. Methods: A longitudinal descriptive study of the information on deaths and hospital discharges registered in public access databases on the primary diagnosis of chronic kidney disease compatible with chronic kidney disease of non-traditional causes was carried out. With the results obtained, absolute and relative percentages and frequencies were calculated for mortality and hospital discharges. The general, specific, and adjusted mortality rates were calculated by the direct method. In addition, years of potential life lost in the country were estimated by province and sex. Results: In Costa Rica, between 2014 and 2019, 2,548 deaths and 1,893 hospital discharges related to chronic kidney disease of non-traditional causes were recorded. Adjusted mortality rates were found to be elevated for the provinces of Guanacaste and Limón compared to national rates and relative to other provinces. In the province of Guanacaste, in addition, the trend of discharges was high. Additionally, it was detected that years of potential life lost showed a progressive and constant increase in the country, predominantly in the male sex and in the province of Guanacaste. Thirteen deaths of persons under twenty years of age were reported. Conclusion: Updating the data on mortality trends has made it possible to identify that the provinces of Guanacaste and Limón have adjusted rates higher than the national average, as well as lower average ages of death according to the rest of the provinces. Deaths and hospital discharges in people under 20 years of age suggest early kidney damage unrelated to occupational disease. Mortality and lost years of potential life lost provide information on the burden of disease in the country's population. It should be added that the specific coding to monitor the pathology was scarcely recorded in the study period.
