RESUMO

This cross-sectional study employs structural equation modelling (sEM) to explore both direct and indirect effects of parental level of education and child individual factors on the length-for-age outcomes in children aged 6-24 months assisted by the Bolsa Família Program in the State of Alagoas. A total of 1448 children were analysed by the sEM technique. A negative standardised direct effect (sDE) of the children's younger age (sDE: -0·06; P = 0·017), the use of bottle feeding (sDE: -0·11; P < 0·001) and lack of a minimum acceptable diet (sDE: -0·09; P < 0·001) on the length-for-age indicator was found. Being female (SDE: 0·08; P = 0·001), a higher birth weight (SDE: 0·33; P < 0·001), being ever breastfed (sdE: 0·07; P = 0·004) and a higher level of parental education (SDE: 0·09; P < 0·001) showed a positive SDE effect on the child's length-for-age. The model also demonstrated a negative standardised indirect effect (SIE) of the sweet beverage consumption (SIE: -0·08; P = 0·003) and a positive effect of being ever breastfed (SIE: 0·06; P = 0·017) on the child's length-for-age through parental level of education as a mediator. This research underscores the crucial role of proper feeding practices and provides valuable insights for the development of targeted interventions, policies and programmes to improve nutritional well-being and promote adequate linear growth and development among young children facing similar challenges.

RESUMO

Aim: To assess the effect of cashew nut flour on the hematological parameters of children living with HIV-AIDS. Method: A 32-week randomized, blind clinical trial conducted at a specialized outpatient clinic. Children aged 2-12 years were allocated to intervention groups (IGs) (n = 11) receiving 12 g/day of cashew nut flour and control groups (CGs) (n = 9) receiving 12 g/day of carboxymethyl cellulose. Parameters of erythrocytes, leukocytes, platelets, and lipid profiles were evaluated. Results: In the IG, the elevation and reduction of leukocyte and lipid profile biomarkers, respectively, were not statistically significant (p > 0.05). A clinically and statistically significant increase in mean corpuscular hemoglobin concentration was observed in the CG (p = 0.018), with a large effect size (Cohen's d = 0.9). There were no statistically significant changes in platelet counts among participants (p = 0.18). The effect size for white blood cell count, low-density lipoprotein cholesterol, very low-density lipoprotein, and triglycerides was moderate in the IG compared to the CG. Conclusion: Cashew nut flour supplementation may increase levels of leukocytes and lipid profile parameters in children living with HIV. Brazilian Clinical Trials Registry (REBEC): U1111.1276.6591.

RESUMO

Information on the effects of government nutrition programmes provided to socially vulnerable children to improve their nutritional status is scarce. We analysed the effectiveness of a nutritional programme, including food supplementation with infant formula, on the evolution of the weight and height of socially vulnerable children from Manaus in the Brazilian Amazon. This study included 7752 children aged 12-24 months admitted to the programme between 2017 and 2020. Weight and height measurements at admission and every three months thereafter were extracted from the programme database. Weight-for-age, weight-for-height, body mass index-for-age (BMI/A), and height-for-age z-scores were analysed using a multilevel linear regression model, which showed a statistically significant decrease in nutritional deficits toward nutritional recovery at follow-up. The programme's effectiveness was evaluated in 1617 children using a paired analysis comparing data from between 12 and 15 months of age at admission and follow-up after 6-9 months. Children admitted with wasting presented an increase in the BMI/A z-score, whereas children admitted with a risk of being overweight and obese had a statistically significant decrease in the BMI/A z-score. Children admitted with stunted growth also showed increased height-for-age z-scores. The nutrition programme was effective for children experiencing wasting and reducing excess weight.

Introduction. During childhood, children may experience some degree of difficulty eating. A tool (PediEAT) has been developed in the United States and is available to assess pediatric eating and to identify problematic symptoms. Objective. To obtain an Argentine version that is transculturally adapted, culturally adequate, and semantically equivalent to the original version. Population and methods. A self-administered version of the PediEAT was used and completed by families and/or caregivers of children aged 6 months to 7 years. In the first phase, content validity was assessed by a group of experts. This was followed by a pre-test phase with families using cognitive interviews to test word and phrase comprehension. The necessary changes were made to obtain a version adapted to the context. Results. The tool's content validity was assessed by a group of 8 experts; as a result, 36 of the 80 items were changed. During the pre-test phase, cognitive interviews were conducted with 18 caregivers; 11 items were changed to improve comprehension by the Argentine population. The Argentine version was approved by the original authors. Conclusions. The Argentine version of the PediEAT tool is linguistically equivalent to the original version, and this allows its use to screen for feeding problems in children.

RESUMO

BACKGROUND: Maternal and child malnutrition represents a public health problem in Mexico Primary care (PC) is responsible for introducing women and children under five to the health system, detecting diseases on time, and providing medical services, including pharmacological treatment if necessary. Providing these services with quality is essential to improve maternal and child health. This study evaluated the quality of nutritional care during preconception, pregnancy, postpartum, infancy, and preschool age at the PC health units across six Mexican states between 2020 and 2021. METHODS: We conducted a cross-sectional study with a mixed approach in units of the Secretary of Health to assess the quality of nutritional care during preconception, pregnancy, postpartum, childhood, and preschool age. The level of quality was calculated by the percentage of compliance with 16 indicators that integrated a Quality Index of Maternal and Child Nutritional Care (ICANMI, by its Spanish acronym). Compliance by indicator, by life stage, and overall was categorized using the following cut-off points: poor quality (≤ 70%), insufficient quality (71-89%), and good quality (≥ 90%). The perceptions of the barriers and facilitators that affect maternal and child nutrition were evaluated through semi-structured interviews with health professionals (HP) and users. All qualitative instruments were developed with a gender and intercultural perspective. RESULTS: Considering the whole sample studied, maternal and child nutritional care quality during the five life stages evaluated was bad (compliance: ≤12%), reflected in the ICANMI, which had a compliance of 8.3%. Principal barriers identified to providing high-quality nutritional care were the lack of knowledge and training of health professionals, shortages of equipment, medicine, personnel, and materials, the disappearance of the social cash transfer program Prospera, the absence of local indigenous language translators to support communication between doctor and patient, and the persistence of machismo and other practices of control over women. CONCLUSIONS: These findings underscore the need for initiatives to improve the quality of nutritional care in PC facilities across Chihuahua, State of Mexico, Veracruz, Oaxaca, Chiapas, and Yucatan. It is necessary for government and health authorities, along with various stakeholders, to collaboratively devise, implement, and assess intercultural and gender-oriented policies and programs geared towards ensuring the health infrastructure and enhancing the training of health professionals to diagnose and treat the prevalence and occurrence of diverse forms of malnutrition in both maternal and child populations.

Feeding difficulties are any problem that negatively affects the process of parents or caregivers providing food or nutrients to the child. The objective of this work was to investigate eating difficulties in preschool children at a municipal early childhood education school in Uruguaiana/RS. The study was approved by the Research Ethics Committee and carried out between October and November 2022. All parents or guardians of children (n=70) who attended school, aged 4-5 years, were invited to participate in the research. Everyone received the Free and Informed Consent Form, as well as the research questionnaire. The instrument used was the Brazilian Infant Feeding Scale (EBAI), which has 14 questions regarding children's nutrition. Descriptive statistics were performed in terms of frequency and analysis of variance (ANOVA) was performed for comparisons between sexes (p<0.05). 31 responses were obtained from parents regarding children's nutrition. The data revealed that 9.68% (n=3) had some degree of feeding difficulty, 3.33% (n=1) with a severe degree and 6.45% (n=2) with a moderate degree. The remaining children (n=28) also presented behaviors related to feeding difficulties, however, without enough scores to be classified as having feeding difficulties. The most frequently described behaviors were: the caregiver using distractions or going after the child to make them eat, feeding time around 31-60 minutes or more, child who nauseates, spits or vomits with some type of food and the negative influence of food in family relationships. Thus, a low percentage of feeding difficulties was observed in the studied population, in accordance with the literature studied.

Introduction: The influence of consuming growing-up milk on the nutrition of infants is still unknown, despite the product driving a multimillion-dollar market. Objective: To evaluate the nutritional composition of the growing-up milk and the influence of its consumption on the daily macronutrient and energy needs of young children. Methods: The research was divided into three parts, namely: a) evaluation of nutritional composition and ingredients; b) comparison of the Brazilian growing-up milk with a proposed standardization; c) calculation of nutritional inadequacy and the influence of growing-up milk consumption in young children. Results: It was possible to categorize the product into three subgroups based on ingredients, namely: group 1 (whey, sugar, and vegetable fat), group 2 (added vitamins and minerals), and group 3 (a wide range of ingredients, no added sugars, and addition of bioactives). Overall, the growing-up milk is characterized by excess protein and added sugars, a predominance of saturated fats, and the presence of chemical additives. Compared to the proposed ideal composition, the Brazilian growing-up milk exceeds all analyzed nutrients. Finally, consuming two 200mL glasses of growing-up milk per day corresponds to 100% of daily protein needs, with children aged 0 to 11 months being the most affected. Conclusion: Regardless of the subgroup and ingredient, the product has a negative impact on children's health, and the younger the age of introduction, the greater the impact on health, potentially leading to the development of overweight and obesity.

Avoidant/Restrictive Food Intake Disorders have been widely described due to the similarity in their characteristics and the consequences they have on the development and growth of children who experience them. Picky eating is a multifactorial disorder where children exhibit negative behaviors towards food or show preference for a specific group of foods. This narrative literature review aims to present the latest research related to picky eating, offering guidance on signs and symptoms, assessment, and intervention in children with picky eating conducted by a multidisciplinary team. The literature review was conducted using the PubMed and Scopus databases, with the main inclusion criterion being articles in English, Spanish, and Portuguese, on picky eating in children from birth to 5 years old. The results of this review suggest that picky eating is determined by three important stages in a child's life; gestation, the early phase of feeding, and experiences during their second year of life. Therefore, it is necessary to recognize warning signs (refusal to eat certain foods, lack of feeding skills, excessive duration during feeding times, etc.), which allows for an early assessment that guides therapeutic intervention, thus promoting safe and effective feeding for the nutrition and quality of life of the child.

Pediatric feeding disorders (PFD) have a significant prevalence in typical development children and increase drastically when there are comorbilities health conditions. PFD are one of the most frequent concerns in pediatric consultant. The lack of accured detection can negatively impact the comprehensive health of children. The objective of this article is to describe the approach that was carried out in children and adolescents who presented pediatric feeding disorders with severe nutritional impact in the context of the "Interdisciplinary Swallowing and feeding team" that operates in a public hospital in the City from Buenos Aires, Argentina. A descriptive and retrospectivestudy was carried out. 16 families were included in the final sample. The children included an age range of 2 to 13 years. The most prevalentreasons for hospitalization were functional impotence of the lower limbs and visual alterations. 93.75% of the population presented challenges in their development. The average age of onset of feeding difficulties was between 12 and 18 months of age. 56.25% had previously consulted with a health professional. 56.25% did not have adequate therapeutic support. 100% had difficulties in forming routines. Pediatric Feeding Disorders (PFD) constitute a strong challenge for the health team, so awareness and training on this topic is essential. Early detection, timely interventions, and a family-centered approach are essential to avoid severe complications and promote a pleasant mealtime for all

RESUMO

Introduction: Studies in Latin America have focused either on analyzing factors associated with exclusive breastfeeding (EBF) or infant formula (IF). Purpose: Analyze the association between economic, sociodemographic, and health factors with EBF, mixed milk feeding (MixMF), and exclusive use of IF in three Latin American and Caribbean countries in the 1990s, 2000s, and 2010s. Methods: Cross-sectional time-series study using data from Demographic and Health Surveys between the 1990s and 2010s in Colombia (1995-2010), Haiti (1994-2017), and Peru (1996-2012) accounting for a sample of 12,775 infants under 6 months. Hierarchical logistic multilevel regression models were used to estimate the adjusted association between infant feeding outcomes (EBF, MixMF, exclusive use of IF) and contextual level DHS survey decade (1990s, 2000s and 2010s) and economic factors (Gross Domestic Product by purchasing power parity, female wage and salaried workers, labor force participation rate female) as well as individual level sociodemographic (maternal age, maternal education, number of children in the household, wealth index, mother living with a partner, area of residence, mother working outside of home), and health factors (breastfed in the first hour, C-section). Results: Factors associated with EBF cessation were c-section (OR: 0.76; 95%CI: 0.64, 0.92), mothers working outside of the home (OR: 0.79; 95%CI: 0.69, 0.90), families in the highest income quintile (OR: 0.64; 95%CI: 0.49, 0.84), and female wage and salaried workers (OR: 0.92; 95%CI: 0.91, 0.94). MixMF was associated with women with higher education (OR: 1.54; 95%CI: 1.21, 1.97), mother working outside of the home (OR: 1.26; 95%CI: 1.10, 1.43), c-section (OR: 1.37; 95%CI: 1.15, 1.62), families in the highest income quintiles (OR: 2.77; 2.10, 3.65). and female wage and salaried workers (OR: 1.08;95% CI: 1.05, 1.09). Exclusive use of IF was associated with a mother working outside of the home (OR: 2.09; 95%CI: 1.41, 3.08), c-section (OR: 1.65; 95%CI: 1.09, 2.51), families in the highest income quintiles (OR: 12.08; 95% CI: 4.26, 34.28), the 2010s (OR: 3.81; 95%CI: 1.86, 7.79), and female wage and salaried workers (OR: 1.12; 95%CI: 1.07, 1.16). Discussion/Conclusion: Factors related to women empowerment and gender equality jeopardized EBF and favored the exclusive use of IF in Latin America. Therefore, workplace interventions to promote, protect, and support breastfeeding practices are key to reducing exclusive use of IF.

Introduction. Negative health consequences due to social isolation during the COVID-19 pandemic were observed worldwide; overweight and obesity showed increasing trends. Objective. To analyze the changes in overweight, obesity and diet of schoolchildren in northwest Mexico before and after lockdown due to COVID-19. Materials and methods. Weight, height, and waist circumference of schoolchildren (n=479 pre-pandemic and n=820 post-pandemic) were collected in public schools located in medium to high marginalization neighborhoods. In the same periods dietary data was collected from a subsample of 203 and 179 schoolchildren, respectively. Results. The average age of schoolchildren in 2019 was 8,9 ±1,75 and 9,1 ± 1,54 in 2022. An increase in percentage of 6,2 was observed in the overweight plus obesity prevalence and a significant difference in the distribution of nutritional status (p=0,049) between the two periods. In addition, changes in central adiposity were observed, with an increase of 3 centimeters in waist circumference (p=0,001; 62,6 and 65,6 cm). The healthy eating index (HAI) classified the diet of schoolchildren as unhealthy during both periods. Conclusions. The increase in the prevalence of overweight and obesity, as well as central adiposity is worrying given that they were already a health problem before the COVID 19 confinement(AU)

RESUMO

To determine the association between women's autonomy and the presence of childhood anemia in children under five years of age in Peru, a cross-sectional study utilizing data from the 2019 Demographic and Family Health Survey was carried out. The study employed generalized linear models with a Poisson distribution and log link function. Crude and adjusted prevalence ratios (aPR) were calculated, along with their corresponding 95% confidence intervals (CI), to assess the association of interest. A total of 15,815 women and their children under five years of age were analyzed. The prevalence of childhood anemia was 30.4% (95%CI: 29.5-31.3%), while the proportions of low, moderate and high autonomy of the mothers were 44.5%, 38.4% and 17.1%, respectively. Children under five years of age of women with a low level of autonomy were more likely to have anemia (aPR: 1.10; 95%CI: 1.00-1.21). Three out of ten children under five years of age suffer from anemia, and four out of ten mothers have a low level of autonomy. A low level of women's autonomy was associated with a higher probability of anemia in children under 5 years of age.

RESUMO

Ultraprocessed foods (UPF) consumption plays a critical role in the development of chronic diseases, but evidence of their effect on children's health is limited. We hypothesized that the consumption of UPF can influence blood pressure levels in 6-year-old children. This study is a secondary analysis of a randomized field trial in Brazil that is registered at clinicaltrial.gov (NCT00635453). Dietary intake was obtained using 2 multiple-pass 24-hour recalls when 305 children were 3 and 6 years old. We classified foods according to the NOVA system and determined the percentage of total energy intake derived from ultraprocessed foods. We collected anthropometric measures from and evaluated systolic and diastolic blood pressures of 6-year-old children. Linear regression analysis was used to assess the association between UPF consumption and blood pressure levels. UPF represented 40.3% (interquartile range, 34.1-48.5) of the total energy intake at 3 years and 45.2% (interquartile range, 41.5-53.2) at 6 years. The adjusted linear regression analyses showed that systolic blood pressure was associated with UPF consumption at 6 years (P = .05), birth weight (P = .02), waist circumference (P < .01), and physical activity (P = .04), whereas diastolic blood pressure was associated with UPF consumption at 3 and 6 years (P = .01 and P < .01, respectively), birth weight (P = .05), and waist circumference (P < .01). Our data suggest that UPF consumption played a role in increasing 6-year-old children's blood pressure. These results reinforce the importance of effective strategies to prevent the excessive consumption of UPF in childhood.

Introduction: World hunger is a colossal problem that is increasing every day, in Mexico more than half of households present some degree of food insecurity, existing a higher prevalence in rural localities. This situation has an impact on the nutritional status of Mexican children who suffer from obesity, among other types of malnutrition. Objective: To analyze the relationship between food insecurity and the nutritional status of children in the community of Tziritzícuaro Michoacán, Mexico. Methodology: This is a quantitative and correlational study. The sample consisted of 159 children between 6 and 12 years of age from the rural community of Tziritzícuaro. The instrument used was the Latin American and Caribbean Food Security Scale (ELCSA). Anthropometric measurements we are taken, including weight and height. Once the BMI of the participants had been determined, the data were analyzed with the WHO Anthro Plus V.3.22 program, to obtain Z-scores for two indicators of growth achieved, i.e. height for age and body mass index for age. Results: We were able to assess the food security of 100 households, finding some level of food insecurity in 65% of them. The average age of the head of household is 35 years old and most of them (42%) have a high school education, the average food expenditure is 1030 pesos per week and the average number of household members is four members. Only 18% of households have government support. Conclusions: Although more than half of the population studied is food insecure, these levels are lower compared to what has been reported in other studies. It would be necessary to analyze the quality of food consumption in order to evaluate food security from another perspective besides availability, which would allow a better understanding of the nutritional health conditions of the population.
