INTRODUCTION: Traumatic spinal root injury caused by shoulder dislocation may involve the brachial plexus or, in some cases, a single nerve. The degree of severity of the injury depends on many patient-specific factors as well as the mechanism of injury. It is essential to suspect this type of lesion by means of a thorough physical examination in order to have better patient outcomes. CASE PRESENTATION: We presented the subtle magnetic resonance imaging (MRI) findings in a 35-yearold male with left shoulder trauma and dislocation after falling off a bicycle. He complained of decreased muscle strength and sensitivity in the C8 dermatome. Atrophy of the hypothenar region and flexion deformity of the 4th and 5th digits were noted. Magnetic resonance imaging findings were consistent with a partial preganglionic C8 motor root lesion. We found T2 increased signal intensity and thinning of the intradural segment of the C8 motor nerve root and low signal in the sequence of a multi- echo gradient recalled echo (GRE). CONCLUSION: MRI is a noninvasive tool that allows a detailed anatomical characterization of the nerves. In brachial plexus injuries, the use of the GRE sequence is useful to identify the lesions, even if they are subtle; however, some lesions may go unnoticed. It is important to note that these patients require an interdisciplinary group to reach a correct diagnosis, which is vital to establish the appropriate treatment and follow-up.

Objective The incidence of traumatic brachial plexus injuries has been increasing considerably in Brazil, mainly due to the increase in the number of motorcycle accidents. The aim of the present study is to evaluate the sensitivity and specificity of magnetic resonance imaging (MRI) in the diagnosis of brachial plexus avulsion lesions, comparing it with the findings of physical and intraoperative examination. Methods A total of 16 patients with brachial plexus injury were prospectively evaluated and treated at the hand surgery outpatient clinic from our service. All patients underwent MRI of the brachial plexus, and the findings were inserted on a table, as well as the physical examination data, and part of the patients had the plexus evaluated intraoperatively. Resumo Objetivo A incidência de lesões traumáticas do plexo braquial vem aumentando consideravelmente no Brasil, principalmente devido ao aumento do número de acidentes de motocicleta. O objetivo do presente estudo é avaliar a sensibilidade e a especificidade da ressonância magnética (RM) no diagnóstico das lesões por avulsão do plexo braquial, comparando com os achados do exame físico e do intraoperatório. Métodos Foram avaliados prospectivamente 16 pacientes com lesão do plexo braquial atendidos no ambulatório de cirurgia da mão de nosso serviço. Todos os pacientes foram submetidos ao exame de RM do plexo braquial e os achados foram inseridos em uma tabela, assim como os dados do exame físico, e parte dos pacientes teve o plexo avaliado intraoperatoriamente. Resultados No presente estudo, a acurácia da RM na identificação de avulsão de raízes foi de 100%, com 100% de sensibilidade e especificidade comparando-se achados da imagem e cirúrgicos. Conclusão A RM mostrou alta sensibilidade e especificidade, confirmadas por achados intraoperatórios, o que permite considerar este exame como padrão outro no diagnóstico de avulsão nas lesões traumáticas do plexo braquial.

RESUMEN 15. La rotura de la porción larga del tendón del bíceps produce dificultades para la función del miembro superior y la estética del brazo. Se presenta un paciente con rotura de tendón largo del bíceps, que fue tratado hace 15 años con tratamiento quirúrgico; se muestra desde su lesión y tratamiento hasta la recuperación total y sus condiciones actuales. El siguiente trabajo tiene como objetivo la presentación de un paciente, el cual fue atendido en el Hospital General Provincial Universitario «Mártires del 9 de Abril», en Sagua la Grande, por una rotura de la porción larga de bíceps en el año 2005. Se utilizó una forma de anclaje al hueso diferente a la clásica descrita en la literatura, con excelente resultado. Tras 15 años, se expone el caso y se detalla la evolución posterior, se valora la función actual y se aporta conocimiento interesante para los profesionales que tratan esta afección.


ABSTRACT 19. Rupture of the long head of the biceps tendon causes difficulties for upper limb function and arm aesthetics. We present a male patient with a rupture of the long biceps tendon, who was treated 15 years ago with surgical treatment; he is shown from his injury and treatment to his full recovery and current condition. The aim of the following work is to present a patient who was treated at "Mártires del 9 de Abril" Provincial General University Hospital, in Sagua la Grande, due to a rupture of the long head of the biceps in 2005. A form of bone anchorage, different from the classic one described in the literature, was used with excellent results. After 15 years, the case is presented and the subsequent evolution is detailed, the current function is assessed and interesting knowledge is provided for professionals who treat this condition.

BACKGROUND: There is no consensus about which type of imaging study, computed tomography myelography (CTM) or magnetic resonance imaging (MRI), provides better information concerning root avulsion in adult brachial plexus injuries. METHODS: Patients with upper brachial plexus traumatic injuries underwent both CTM and MRI and surgical exploration. The imaging studies were analyzed by two independent radiologists and the data were compared with the intraoperative findings. The statistical analysis was based on dichotomous classification of the nerve roots (normal or altered). The interobserver agreement was assessed using Cohen's Kappa. The accuracy of CTM and MRI in comparison with the intraoperative findings was evaluated using the same methodology. RESULTS: Fifty-two adult patients were included. CTM tended to yield slightly higher percentages of alterations than MRI The interobserver agreement was better on CTM than on MRI for all nerve roots: C5, 0.9960 (strong) vs. 0.145 (poor); C6, 0.970 (strong) vs. 0.788 (substantial); C7, 0.969 (strong) vs. 0.848 (strong). The accuracy regarding the intraoperative findings was also higher on CTM (moderate, kappa 0.40-0.59) than on MRI (minimal, kappa 0.20-0.39) for all nerve roots. Accordingly, the overall percentage concordance (both normal or both altered) was superior in the CTM evaluation (approx. 70-75% vs. 60-65%). CTM was superior for both sensitivity and specificity at all nerve roots. CONCLUSION: CTM had greater interobserver agreement and higher diagnostic accuracy than MRI in adult patients with root avulsions due to brachial plexus injury.

Background Traumatic brachial plexus injuries (BPIs) represent a major cause of disability in young patients. The purpose of this study was to compare two populations (from Argentina and Germany) who suffered a traumatic BPI after a motorcycle accident to identify predictors of BPI and brain injury severity. Methods Univariate and multivariable intergroup comparisons were conducted, and odds ratios were calculated to assess the associations between the different demographic, morphometric, and trauma-related variables, and the type and severity of patients' injuries. Pearson correlation coefficients were generated to identify statistically significant correlations. Results A total of 187 patients were analyzed, 139 from Argentina and 48 from Germany. The two countries differed significantly in age and several morphometric and trauma-related variables. The clinical presentation was also convincingly different in the two countries. The following three variables remained as statistically significant predictors of a complete (vs. partial) BPI: living in Argentina ( p < 0.001), presenting prior to 2015 ( p = 0.004), and greater estimated speed at the time of impact ( p = 0.074). As for BPIs, a disproportionate percentage (85.6%) of more severe brain injuries occurred in Argentinian patients ( p < 0.001) and among those whose accident involved striking a stationary vertical object. Conclusions This study identified several factors that might be considered when planning governmental policies and education initiatives to reduce BPI and brain injuries related to motorcycle use. OBJECTIVE: To compare magnetic resonance imaging and intraoperative findings in patients diagnosed with traumatic injury to the brachial plexus. METHODS: Patients with a diagnosis of traumatic injury to the brachial plexus admitted to the hand and microsurgery outpatient consult of the Hospital das Clínicas at the University of São Paulo were selected during December 2016. A total of three adult patients with up to six months of injury who underwent surgical treatment were included in the study. A diffusion-weighted sequence magnetic resonance protocol and fluid-sensitive volumetric reformatting sequence were applied. The magnetic resonance results were compared with the diagnoses obtained from the injuries observed during the surgery. The study was double-blind (surgeon and radiologist). RESULTS: A descriptive correlation was found between the magnetic resonance imaging results and the diagnostic findings from the surgeries, for both pre- and post-ganglionic injuries. CONCLUSION: Magnetic resonance imaging has shown to be a promising diagnostic method in preoperative assessment of brachial plexus lesions; it is less invasive than other common methods, showing not only avulsion lesions but also localized postganglionic lesions in the supra- and infraclavicular region. OBJETIVO: Comparar resultados de ressonância magnética e achados intraoperatórios de pacientes com diagnóstico de lesão traumática do plexo braquial. MÉTODOS: Foram selecionados pacientes com diagnóstico de lesão traumática do plexo braquial admitidos no ambulatório de mão e microcirurgia do Hospital das Clínicas de São Paulo no mês de dezembro de 2016. Foram incluídos pacientes adultos com até seis meses de lesão que seriam submetidos a tratamento cirúrgico, totalizando três para o estudo. RESULTADOS: Houve correlação em termos descritivos entre os laudos das ressonâncias magnéticas e os achados diagnósticos das cirurgias, tanto para as lesões pré como pós-ganglionares. CONCLUSÃO: A ressonância magnética mostrou ser um método diagnóstico promissor na avaliação pré-operatória das lesões do plexo braquial, sendo menos invasiva que os outros métodos mais utilizados, permitindo o estudo não somente das lesões por avulsão, mas também das lesões pós-ganglionares localizadas na região supra e infraclavicular. Lesions of the axillary artery and consequent compression of the brachial plexus are extremely rare in patients with glenohumeral dislocation and may have greatly varying clinical manifestations. This joint is one of the most affected by dislocation in the human body, accounting for approximately 45% of cases. Less than 1% of patients with shoulder dislocation have vascular complications; however, when there is damage in the axillary artery, the incidence of associated brachial plexus injury is 27% to 44%. The authors report on a case of brachial plexus compression by an axillary artery pseudoaneurysm after a glenohumeral dislocation, aiming to highlight the existence of this association, in order to make an early diagnosis and avoid serious complications, such as neurologic injury.


As lesões de artéria axilar e consequente compressão de plexo braquial são extremamente raras em pacientes com luxação de glenoumeral e podem ter manifestações clínicas bastante variadas. Essa articulação é uma das mais acometidas por luxação do corpo humano, representando cerca de 45% dos casos. Menos de 1% dos pacientes com luxação de ombro apresentam complicações vasculares; no entanto, quando há lesão da artéria axilar, a incidência de lesão de plexo braquial associada é de 27% a 44%. Relatamos um caso de compressão do plexo braquial por um pseudoaneurisma de artéria axilar após uma luxação glenoumeral. O objetivo é lembrar a existência dessa associação, a fim de diagnosticá-la precocemente e evitar complicações graves, como a lesão neurológica.

OBJECTIVE: Gain in elbow flexion in patients with brachial plexus injury is extremely important. The transfer of a fascicle from the ulnar nerve to the motor branch of the musculocutaneous nerve (Oberlin surgery) is a treatment option. However, in some patients, gain in elbow flexion is associated with wrist and finger flexion. This study aimed to assess the frequency of this association and the functional behavior of the limb. METHODS: Case-control study of 18 patients who underwent the Oberlin surgery. Group 1 included patients without disassociation of range of elbow flexion and that of the fingers and wrist; Group 2 included patients in whom this disassociation was present. In the functional evaluation, the Sollerman and DASH tests were used. RESULTS: It was observed that 38.89% of the patients did not present disassociation of elbow flexion with flexion of the wrist and fingers. Despite the existence of a favorable difference in the group with disassociation of the movement, when the Sollerman protocol was applied to the comparison between both groups, this difference was not statistically significant. With the DASH test, however, there was a statistically significant difference in favor of the group of patients who managed to disassociate the movement. CONCLUSION: The association of elbow flexion with flexion of the wrist and fingers, in the group studied, was shown to be a frequent event, which influenced the functional result of the affected limb.


OBJETIVO: O ganho da flexão do cotovelo em pacientes com lesão no plexo braquial é de suma importância. A cirurgia de transferência de fascículo do nervo ulnar para ramo motor do nervo musculocutâneo (cirurgia de Oberlin) é uma opção de tratamento. Contudo, o ganho da flexão do cotovelo, em alguns pacientes, vem associado à flexão do punho e dos dedos. O objetivo deste trabalho é avaliar a frequência dessa associação e o comprometimento funcional do membro. MÉTODOS: Estudo tipo caso-controle de 18 pacientes submetidos à cirurgia de Oberlin. No Grupo 1 foram incluídos os pacientes que não apresentavam dissociação do ganho da flexão do cotovelo com a dos dedos e do punho; no Grupo 2, os pacientes em que havia dissociação. Os testes de Sollerman e Disabilities of the Arm, Shoulder and Hand (Dash) foram usados na avaliação funcional. RESULTADOS: Observou-se que 38,89% dos pacientes não dissociavam flexão de cotovelo de flexão de punho e dos dedos. Apesar de existir uma diferença favorável ao grupo que dissociava o movimento quando aplicado o protocolo de Sollerman na comparação entre os pacientes dos dois grupos, essa não se mostrou estatisticamente significante. Já no teste Dash, observou-se diferença estatisticamente significante, favorável ao grupo de pacientes que consegue dissociar o movimento. CONCLUSÃO: A associação da flexão do cotovelo com a flexão de punho e dos dedos no grupo estudado mostrou ser um evento frequente, teve influência no resultado funcional do membro acometido.

CONCLUSÃO: A associação da flexão do cotovelo com a flexão de punho e dos dedos no grupo estudado mostrou ser um evento frequente, teve influência no resultado funcional do membro acometido.

OBJECTIVE: To evaluate the factors influencing the results of ulnar nerve neurotization at the motor branch of the brachii biceps muscle, aiming at the restoration of elbow flexion in patients with brachial plexus injury. METHODS: 19 patients, with 18 men and 1 woman, mean age 28.7 years. Eight patients had injury to roots C5-C6 and 11, to roots C5-C6-C7. The average time interval between injury and surgery was 7.5 months. Four patients had cervical fractures associated with brachial plexus injury. The postoperative follow-up was 15.7 months. RESULTS: Eight patients recovered elbow flexion strength MRC grade 4; two, MRC grade 3 and nine, MRC <3. There was no impairment of the previous ulnar nerve function. CONCLUSION: The surgical results of ulnar nerve neurotization at the motor branch of brachii biceps muscle are dependent on the interval between brachial plexus injury and surgical treatment, the presence of associated fractures of the cervical spine and occipital condyle, residual function of the C8-T1 roots after the injury and the involvement of the C7 root. Signs of reinnervation manifested up to 3 months after surgery showed better results in the long term. LEVEL OF EVIDENCE: IV, Case Series.

First-rib fractures are uncommon and are generally related to major thoracic trauma: so much so that they serve to indicate the severity of the trauma. Isolated bilateral first-rib fractures without major thoracic trauma are rarely described in the literature. The symptoms may go unnoticed and be minimized, thus making this condition difficult to diagnose. The present report presents a case of direct trauma on the supraclavicular region with symptoms of contusion of the brachial plexus, caused by a fall from a jet ski while a life vest was being used. The literature was reviewed to show the various facets of the problem and the treatment for this condition was discussed.
