BACKGROUND: Previous clinical and anatomic investigations have identified the clinical relevance of facial biomechanics. Based on this new understanding, principles for facial aesthetic procedures were established: Lateral Face First, Deep Layers First, and Upper Face First. OBJECTIVE: To test the upper face first principle by showing that an injection sequence, starting in the upper face is superior to an injection sequence starting in the lower face. METHODS: This study was designed as an interventional split-face study administering the same amount and type of soft tissue filler for the upper, middle, and lower face but in a different sequence: upper, middle, lower face versus lower, middle, and upper face. A total of 15 patients (5 males and 10 females) with a mean age of 39.4 years (9.6) and a mean BMI of 23.4 kg/m2 (1.7) were studied. Follow-up at D0, D30, and D90 was conducted utilizing semiquantitative scores and objective 3D imaging. RESULTS: Despite not reaching statistical significance, midfacial volume and jawline contouring were rated better at every follow-up visit (D0, D30, D90) when treated with the upper versus the lower face first injection algorithm. The global aesthetic improvement scale showed statistically significantly better values for the upper face first algorithm when compared to the lower face first algorithm at all evaluated time points with all p < 0.001. CONCLUSION: Applying the upper face first injection algorithm seems to result in better aesthetic outcomes when directly compared to the lower face first algorithm. Semiquantitative and objective outcome measurements confirm its clinical effectiveness.

RESUMO

Brazil ranked second in the world for the number of aesthetic procedures carried out in 2019. Five case reports of delayed hypersensitivity reaction to hyaluronic acid dermal filler after COVID-19 vaccination are presented in this paper. Additional vaccination for new variants, including omicron, will be necessary; therefore, aesthetic professionals should be aware of this possibility and advise patients accordingly.

Introducción: En la actualidad la litiasis renal constituye entre el 20 y el 30 por ciento de las consultas de urología en el mundo. Objetivos: Identificar las complicaciones intra- y posoperatorias de la nefrolitotomía percutánea en pacientes con litiasis coraliforme. Métodos: Se realizó un estudio descriptivo en pacientes con diagnóstico de litiasis coraliforme ingresados para nefrolitotomía percutánea electiva en el Hospital Clínico Quirúrgico Hermanos Ameijeiras entre octubre 2010 y entre octubre 2015. Resultados: Se identificaron 32 pacientes con complicaciones intra y posoperatorias. Del sexo masculino fueron 68,7 por ciento. La media de la edad fue 47,59 ± 12,2 años. La clasificación ASA más frecuente fue ASA II en 56,2 por ciento. Todos los pacientes recibieron anestesia general. El valor medio de pérdidas hemáticas intraoperatorias fue de 650,00 ± 413,09 mL. La reposición total de volumen fue de 10, 107,80 ± 2, 659,25 mL. Se administró cloro sodio al 0,9 por ciento 7743,75 ± 2007,39 mL y concentrado de hematíes a 18,8 por ciento del total. Las complicaciones intraoperatorias se presentaron en 24 pacientes 75,0 por ciento. De ellas las más frecuentes fueron las metabólicas, cardiovasculares, respiratorias y renales. La hipotermia estuvo presente en la tercera parte de los enfermos. Las complicaciones posoperatorio se presentaron en 23 pacientes 71,9 por ciento. De ellas, cardiovasculares, respiratorias, renales y sépticas fueron las más frecuentes. Conclusiones: Las complicaciones fueron frecuentes y graves; sin embargo, se logró un porcentaje importante de pacientes egresados curados(AU)


Introduction: Renal lithiasis currently accounts for 20-30 percent of urology consultations worldwide. Objectives: To identify the intra- and postoperative complications of percutaneous nephrolithotomy in patients with staghorn lithiasis. Methods: A descriptive study was conducted in patients with a diagnosis of staghorn lithiasis and admitted for elective percutaneous nephrolithotomy Hermanos Ameijeiras Clinical-Surgical Hospital between October 2010 and October 2015. Results: We identified 32 patients with intra- and postoperative complications. The male sex was represented by the 68.7 percent. The mean age was 47.59±12.2 years. The most frequent ASA classification was ASA II, accounting for 56.2 percent. All patients received general anesthesia. The mean value of intraoperative hematic losses was 650.00±413.09 mL. Total volume replacement was 10, 107.80±2, 659.25 mL. Chloride sodium 0.9 percent was administered at doses 7743.75±2007.39 mL and packed red blood cells to 18.8 percent of the total. Intraoperative complications occurred in 24 patients, accounting for 75.0 percent. Of these, the most frequent were metabolic, cardiovascular, respiratory and renal. Hypothermia was present in one third of the patients. Postoperative complications occurred in 23 patients, accounting for 71.9 percent. Of these, cardiovascular, respiratory, renal and septic were the most frequent. Conclusions: The complications were frequent and serious. However, an important percentage number was achieved for cured discharge patients(AU)

El polimetil-metacrilato es un polímero de alta resistencia al impacto, ampliamente utilizado en diferentes campos de la medicina, sin embargo, aún no está clara su indicación como relleno glúteo. A pesar de su aparente inocuidad, se han presentado complicaciones durante su uso como granulomas y nódulos palpables. Se han reportado casos de embolismo pulmonar por polimetil-metacrilato posterior a vertebroplastias. Sin embargo, no se encontró reporte bibliográfico de casos de embolismo pulmonar con su uso en procedimientos estéticos. Presentamos caso de paciente femenino de 31 años de edad, quien 4 horas luego de la inyección de 500 cm3 de polimetil-metacrilato en cada región glútea, presenta disnea en reposo de aparición súbita con tos seca y palpitaciones, disminución de agudeza visual y petequias generalizadas. Se diagnostica probable embolismo pulmonar por polimetil-metacrilato y retinopatía de Purtscher. Existe similitud clínica e imaginología del caso en estudio con embolismo por silicone, y las imágenes del fondo de ojo semejan la obstrucción de pequeñas arteriolas retinianas compatibles con este diagnóstico. Se sugieren nuevas investigaciones en el uso de polimetil-metacrilato como procedimiento estético en pro de la seguridad y en beneficio de los pacientes


Polymethyl-methacrylate is a high impact resistant polymer, widely used in different medicine fields, however its indication in buttock implants is still not clear. Even though its apparent innocuity, it has presented complications like granulomas and palpable nodules. There have been reported cases of pulmonary embolism caused by polimetil-metacrilato secondary to vertebroplasties. However, there have been no bibliographic cases of pulmonary embolism due to its use in aesthetic procedures. We present a case of a woman patient of 31 years old, who 4 hours after 500 cc injection of polymethyl- Methacrylate in each buttock, presents rest dyspnea with abrupt dry cough and palpitations, reduction in visual acuity and generalized petechiae. It is diagnosed probable pulmonary embolism by polimetil-metacrilato and Purtscher retinopathy. There is a clinical and imaginological similitude between this case and silicone embolism and the images of fondoscopy are similar to those of small retinian arteriole obstruction compatible with the diagnosis. Thereby, it is suggested new investigations in the use of polymethyl-methacrylate as esthetic procedure in favor of the patient’s security and benefit
