RESUMO

BACKGROUND: Preoperative anxiety in children causes negative postoperative outcomes. Parental presence at induction is a non-pharmacological strategy for relieving anxiety; nevertheless, it is not always possible or effective, namely when parents are overly anxious. Parental presence via video has been demonstrated to be useful in other contexts (divorce, criminal court). This study reports the feasibility of a randomized controlled trial to investigate the effect of video parental presence and parental coaching at induction on preoperative anxiety. METHODS: The study was a randomized, 2 × 2 factorial design trial examining parental presence (virtual vs. physical) and coaching (provided vs. not provided). Feasibility was assessed by enrollment rate, attrition rate, compliance, and staff satisfaction with virtual method with the NASA-Task Load Index (NASA-TLX) and System Usability Scale (SUS). For the children's anxiety and postoperative outcomes, the modified Yale Preoperative Anxiety Scale (mYPAS) and Post-Hospitalization Behavioral Questionnaire (PHBQ) were used. Parental anxiety was evaluated with the State-Trait Anxiety Inventory (STAI) questionnaire. RESULTS: A total of 41 parent/patient dyads were recruited. The enrollment rate was 32.2%, the attrition rate 25.5%. Compliance was 87.8% for parents and 85% for staff. The SUS was 67.5/100 and 63.5/100 and NASA-TLX was 29.2 (21.5-36.8) and 27.6 (8.2-3.7) for the anesthesiologists and induction nurses, respectively. No statistically significant difference was found in mYPAS, PHBQ and STAI. CONCLUSION: A randomized controlled trial to explore virtual parental presence effect on preoperative anxiety is feasible. Further studies are needed to investigate its role and the role of parent coaching in reducing preoperative anxiety.

Telemedicine, through the use of Internet videoconferencing platforms, is increasingly widespread in Chile, as a consequence of the COVID-19 pandemic and the restrictions to control the virus. The nutritionist is no stranger to this situation and has had to modify consultations to respond to this new reality. However, there is insufficient information regarding the level of satisfaction of telemedicine users employed by this professional. The objective of the research was to describe the perception of user satisfaction related to nutritional consultations through telemedicine, carried out during the pandemic. An observational, descriptive and cross-sectional study was developed. Sixty-two patients were recruited from Santiago, Chile, who were attended in a nutritional consultation by videoconference. An online questionnaire consisting of six items was designed to determine the acceptability of the nutritional consultation by telemedicine. The role of the nutritionist in the nutritional consultation by videoconference had a degree of satisfaction higher than 70percen. 62.9percentof the respondents prefer the nutritional consultation to be combined. Only 50percent indicated that the zoom videoconferencing platform favors the nutritionist-patient relationship. The telemedicine nutritional consultation makes it possible to monitor the patient's nutritional status without the need to attend in person, which improves access to care. There is a high level of acceptance by patients who have received nutritional care via telemedicine(AU)

RESUMO

Objective: To describe the usefulness of teledentistry during the COVID-19 pandemic and how virtualization can benefit dentists and patients. Methods: The bibliographic search will be carried out using the main data sources of international literature on health sciences (MEDLINE) databases up to September 30, 2020 with no language restriction. The information sought will include publications on the benefits in tele-dentistry during the COVID-19 pandemic using the search terms "Teledentistry, COVID-19, Telehealth, and video consultation". Results: Triage will be carried out virtually. Patient data, signs and symptoms of discomfort, medical history, among other variables. will be recorded. Cases of emergency or urgency will be treated in person, while a COVID-19 questionnaire including symptoms, travel history and contact with COVID-19 patients will be given by telephone. Conclusions: Teledentistry is being used for giving advice and to triage patients with symptoms and emergency treatments.

RESUMO

Ideas about psychoanalysis via videoconference-videoconference teleanalysis (VT)-are presented with the general understanding that these settings produce a twofold split between various degrees of recognition/negativisation of the absence of the other, on one hand, and the expectation of physical co-presence, on the other. This split has been put forward as dismantling the here, now, with me pre-reflexive unity of the analytic experience. This article suggests that both members of the analytic dyad will seek to reappropriate the experience through a forced ego integration that interferes with accessing states of unintegration in the analytic treatment and produces subtle alterations to symbolisation work. The effort to overcome this condition is illustrated with clinical vignettes and therapists' comments about feelings of inauthenticity and discontent when trying to sustain evenly-suspended attention, as well as in the perception of a form of flattening of the alive nature of speech. However, this is not a constant for all VT, and mutual understanding can be an important mitigating feature. The focus of the discussion should be on the capacity of the analytic dyad to overcome such a split and not on a direct extrapolation of the perceptual limitations of VT to possible effects on transference / countertransference.

RESUMO

BACKGROUND: training of Community Health Agents on hearing health through videoconference. AIM: to verify the effectiveness of a training program for community health agents, through videoconference, on child hearing health. METHOD: participants of the study were 50 professionals who were randomly divided in two groups: 31 community health agents were physically present in the training program and 19 participated via videoconference. Training was provided in eight hours, through classes and using specific material adapted from the Primary ear and hearing care training resource - basic, intermediate and advanced levels for both groups, simultaneously. Training was organized and evaluated according to a previous study. RESULTS: both groups presented a statistically significant difference when comparing pre and post training performances. However, for the group of community health agents who were physically present during the training program, this difference was higher not only when considering the final total scores but also when looking at the individual topics. CONCLUSION: videoconference was effective as a learning tool for the training of community health agents on child hearing health. However, this instrument should be used as a complementary material to the traditional form of training.
