OBJECTIVE: This study describes the frequency of obstetric anal sphincter injuries (OASIS) in patients after instrumental delivery according to the type of forceps used. METHODS: A retrospective comparative cohort study was conducted on patients who underwent instrumental delivery from January 2017 to April 2022. The primary outcome was the presence of OASIS following delivery. Patients were categorized into Cohort A if only rotation forceps were used, Cohort B for only traction forceps, and Cohort C if both types were used sequentially. Statistical analysis was performed with SPSS (IBM, New York, NY) with χ2, Fisher's exact, and analysis of variance testing. A P-value <0.05 was considered significant. RESULTS: OASIS occurred in 45 of 328 instrumental deliveries. OASIS after rotation forceps occurred in 12.9% (n = 8) of cases, after traction forceps in 13.2% (n = 34), and after sequential use of rotation and traction forceps in 37.5% (n = 3) of cases (p = 0.141). An odds ratio (OR) of 0.91 (95% confidence interval [CI] 0.40-2.08) for OASIS was obtained with the use of rotation forceps, 0.81 (95% CI 0.38-1.70) for traction forceps, and 3.97 (95% CI 0.91-17.2) for the sequential use of rotation and traction forceps. CONCLUSION: There were no significant differences in the presence of OASIS comparing traction and rotation forceps. A non-significant trend of higher OASIS following the sequential use of traction and rotation forceps was observed.

Background In this study, the characteristics and prognostic factors associated with the progression of myopic traction maculopathy (MTM) were evaluated in a Mexican population. Methods This is a retrospective observational study that analyzed patients with MTM who underwent optical coherence tomography (OCT). Clinical-ocular information, the MTM classification, and initial and final visual acuity (VA) were recorded. Results In total, 101 eyes of 84 patients (mean age 63.5 ± 10.7 years) were included (88.1% female and 11.9% male). The mean spherical equivalent was -16.8 ± 6.4 D, axial length was 29.6 ± 2.1 mm, and mean initial VA was 0.8 ± 0.5 logMAR. The mean follow-up time was 25.7 ± 27.6 months. The change in final VA from diagnosis to the last follow-up was +0.1 (0.2) (p = 0.001). Overall, 24.8% of patients progressed, 72.3% did not progress, and 3% showed regression. The patient-year progression rate was 0.20 ± 0.44. Factors associated with progression were initial logMAR VA (p= 0.012) and staphyloma (p= 0.001). Conclusions One in four patients with MTM progressed, and the patient-year progression rate was 0.5. The factors associated with disease progression were initial VA and the presence of staphyloma. The characteristics of Mexican patients with MTM are similar to those described in other populations.

BACKGROUND: Myopic traction maculopathy (MTM) is a complication of pathological myopia and encompasses various pathological conditions caused by tractional changes in the eye. These changes include retinoschisis, foveal retinal detachment, and lamellar or full-thickness macular holes (FTMHs). This meta-analysis evaluated the safety and efficacy of novel surgical for treating MTM. METHODS: To compare the outcomes of different surgical approaches for MTM, multiple databases, including Web of Science, PubMed, Scopus, ClinicalTrials.gov, the Cochrane Central Register of Controlled Trials, Ovid MEDLINE, Embase, and the Meta-Register of Controlled Trials, were comprehensively searched. The meta-analysis was performed using RevMan 5.1. RESULTS: Nine comparative studies involving 350 eyes were included in this meta-analysis. There were significant differences between fovea-sparing internal limiting membrane peeling (FSIP) and standard internal limiting membrane peeling (ILMP). Preoperative best-corrected visual acuity BCVA (standard mean difference (SMD): -0.10, 95% CI: -0.32 to 0.12) and central foveal thickness CFT (SMD: 0.05, 95% CI: -0.22 to 0.33) were not significantly different (p = 0.39 and p = 0.71, respectively). However, the postoperative BCVA improved significantly (SMD = - 0.47, 95% CI: - 0.80, - 0.14, p = 0.006) in the FSIP group compared to the standard ILMP group. Postoperative CFT did not differ significantly between the two groups (p = 0.62). The FSIP group had a greater anatomical success rate than the other groups, although the difference was not statistically significant (p = 0.26). The incidence of postoperative macular hole formation was significantly lower (OR = 0.19, 95% CI = 0.07-0.54; p = 0.05) in the FSIP group than in the standard ILMP group. The unique characteristics of highly myopic eyes, such as increased axial length and structural changes, may have contributed to the greater incidence of FTMH in the ILMP group. CONCLUSION: Based on the findings of this meta-analysis, FSIP is the initial surgical approach for early-stage MTM and has shown promising outcomes. However, to establish the safest and most efficient surgical technique for treating different MTM stages, further comparative studies, specifically those focusing on ILMP and FSIP, are necessary. BACKGROUND: The choroidal vasculature supplies the outer retina and is altered in many retinal diseases, including myopic traction maculopathy (MTM). Choroid health is typically assessed by measuring the choroidal thickness; however, this method has substantial limitations. The choroidal vascularity index (CVI) was recently introduced to provide quantitative information on the vascular flow in the choroid. This index has been evaluated in a wide range of diseases but has not been extensively used to characterize MTM. AIM: This study aimed to investigate the CVI across different stages of MTM and the influence of macular surgery on choroidal perfusion markers in different surgically resolved MTM stages. METHODS: Eighteen healthy myopic eyes in the control group and forty-six MTM eyes in the surgical group were evaluated using enhanced optical coherence tomography (OCT) imaging. Binarized OCT images were processed to obtain the luminal choroidal area (LCA) and stromal choroidal area (SCA), which were used to calculate CVI in the form of a percentage ratio. CVI data were collected at baseline, one and four months postoperatively, and at the final clinical visit. MTM eyes were divided into four stages based on disease severity. The choriocapillaris flow area (CFA) and central subfield thickness (CSFT) were measured along side the CVI. RESULTS: No significant differences were observed between the two groups at baseline, except for visual acuity (p < 0.0001). Surgery significantly improved vision at all postoperative time points (p < 0.0001). At baseline, there were no significant differences in CVI, CFA, or CSFT scores between the control and surgical groups. However, all three measurements were lower at the final visit in the surgical group (p ≤0.0001). No significant differences were found in any of the parameters among the four stages of MTM (p > 0.05). Ultimately, correlation and multivariate linear regression analyses did not reveal any significant association between CVI and visual acuity. CONCLUSIONS: This study did not find significant preoperative differences in CVI between healthy myopic eyes and eyes with MTM. However, the postoperative CVI and CFA values were significantly lower than those of the control eyes. Thus, CVI may not be a good biomarker for surgical outcomes, as the correlation between CVI and visual acuity was not statistically significant.The CVI and CFA decreased after surgery, providing evidence of choroidal changes after surgical management.

Background: The orthodontic traction of impacted canines represents a great challenge for Orthodontics. Surgical exposure of the impacted canine and the complex orthodontic mechanics applied to align the tooth back to the arch can lead to complications involving supporting tissues inducing gingival recession when the teeth are moved out of the alveolar bone. Aim: The aim of this study is to present an updated bibliographic review of the main periodontal results found in the literature after the clinical management of impacted canines and the prevalence of gingival recession. Materials and methods: Research in electronic databases PubMed, PMC, and MedLine until June 2020 and reference lists of relevant publications were used to identify studies that assessed the periodontal status of impacted and orthodontically tractioned canines. Controlled and randomized clinical trials, literature reviews, systematic reviews, studies in humans, meta- analyzes and text that had at least one occurrence relating to gingival recession, periodontal outcomes and impacted canines tractioned orthodontically, whether by buccal or palatal, superior and / or lower as the eligibility criteria. Results: 691 articles were found in a free search. After applying the eligibility criteria, 7 relevant articles were subtracted, and these results were more frequent for upper canines. Conclusion: Currently, there is no clear evidence to determine which surgical technique procedure is better to discover canines in terms of periodontal outcomes. The results found stated that clinically the evidence were insignificant when compared to teeth normally erupted.


Introdução: O tracionamento ortodôntico de caninos inclusos representa um grande desafio para a Ortodontia. A exposição cirúrgica do canino impactado e a complexa mecânica ortodôntica aplicada para alinhar o dente de volta ao arco podem levar a complicações envolvendo os tecidos de suporte, induzindo recessão gengival quando os dentes são movimentados para fora do osso alveolar. Objetivo: O objetivo deste estudo foi apresentar uma revisão bibliográfica atualizada dos principais resultados periodontais encontrados na literatura após o manejo clínico de caninos impactados e a prevalência de recessão gengival. Materiais e métodos: Foram feitas pesquisas eletrônicas no PubMed, PMC e MedLine até junho de 2020 e uma listas de referência de publicações relevantes foram usadas para identificar estudos que avaliaram o estado periodontal de caninos impactados e tracionados ortodonticamente. Ensaios clínicos controlados e randomizados, revisões de literatura, revisões sistemáticas, estudos em humanos, metanálises e textos que tiveram pelo menos uma ocorrência relacionada a recessão gengival, desfechos periodontais e caninos impactados tracionados ortodonticamente, seja por vestibular ou palatino, foram os critérios de elegibilidade. Resultados: Foram encontrados 691 artigos em busca livre. Após a aplicação dos critérios de elegibilidade, 7 artigos relevantes foram subtraídos, sendo esses resultados mais frequentes para caninos superiores. Conclusão: Atualmente, não há evidências claras para determinar qual técnica cirúrgica é melhor para descobrir caninos em termos de resultados periodontais. Os resultados encontrados afirmaram que clinicamente as evidências foram insignificantes quando comparadas a dentes normalmente

Introdução: Odontoma é o tipo mais comum de tumor odontogênico, contudo são lesões assintomáticas, de crescimento lento e indolor, descobertos geralmente em consultas de rotina, e que muito comumente são causadores de impactação dentária. Esse trabalho tem por objetivo relatar o caso clínico de uma criança que apresentava um odontoma composto na pré-maxila, ocasionando a impactação de seu incisivo central e comprometimento estético. Relato de caso: Paciente de 12 anos, em preparo para tratamento ortodôntico, no qual na etapa de exames de imagem, foi percebido um odontoma composto na região anterior da maxila o qual impedia a irrupção do dente 21, onde para que se conseguisse o melhor aproveitamento estético e funcional, foi indicado a remoção do tumor com 43 estruturas semelhantes a dentes e o tracionamento do dente associado. Conclusão: O tratamento do odontoma não costuma ter recidivas, permitindo o aproveitamento de possíveis dentes associados e manutenção de estruturas nobres adjacentes. Introduction: Odontoma is the most common type of odontogenic tumor. They are asymptomatic, slow-growing lesions of unknown etiopathogenesis. They have a density similar to teeth, surrounded by a thin radiolucent halo. Objective: To report a clinical case of traction surgery of an impacted tooth associated with compound odontoma, and a brief literature review. Case report: Patient presented composite odontoma in the anterior region of the maxilla, preventing the eruption of tooth 21. The lesion was noticed during the preparation of the orthodontic treatment and for the success of such procedure, the treatment consisted of the excision of the lesion composed of 43 similar structures to teeth and the preparation of the traction of the maxillary central incisor. Conclusion: The removal of the odontoma was extremely relevant, with this, the impacted tooth was exposed, allowing the placement of the orthodontic button for its traction. Introducción: El odontoma es el tipo más común de tumor odontogénico. Son lesiones asintomáticas, de crecimiento lento y de etiopatogenia desconocida. Tienen una densidad similar a los dientes, rodeados de un fino halo radiotransparente. Objetivo: Reportar un caso clínico de cirugía de tracción de un diente retenido asociado a odontoma compuesto, y una breve revisión de la literatura. Reporte de caso: Paciente presentó odontoma compuesto en la región anterior del maxilar, impidiendo la erupción del diente 21. La lesión fue notada durante la preparación del tratamiento de ortodoncia y para el éxito de dicho procedimiento, el tratamiento consistió en la escisión de la lesión. compuesto por 43 estructuras similares a los dientes y la preparación de la tracción del incisivo central maxilar. Conclusión: La remoción del odontoma fue de suma relevancia, con esto se logró exponer el diente impactado, permitiendo la colocación del botón de ortodoncia para su tracción. BACKGROUND: We conducted a systematic review to compare  the effects of pneumatic vitreolysis (PV), enzymatic vitreolysis (EVL) with ocriplasmin, and pars plana vitrectomy (PPV) on vitreomacular traction (VMT) syndrome and macular holes (MHs) to assess their efficacy as treatment options. METHODS: Databases, including PubMed, ClinicalTrials.gov ( www. CLINICALTRIALS: gov ), the Cochrane Central Register of Controlled Trials (CENTRAL)-including the Cochrane Eyes and Vision Group Trials Register (The Cochrane Library 2013, Issue 2)-, Ovid MEDLINE, and EMBASE (January 2000-October 2022), were searched to identify studies comparing the outcomes of PV versus PPV, PPV versus ocriplasmin and ocriplasmin versus PV. RevMan 5.1 was used for the meta-analysis of the studies. RESULTS: Among the 89 studies, 79 were considered eligible for qualitative analysis, and 10 quantitative studies were subjected to meta-analysis. PPV resulted in better postoperative visual acuity improvement than ocriplasmin (standardized mean deviation (SMD) = 0.38, 95% CI 0.03-0.73, p = 0.0003). PV resulted in no significant difference in visual improvement compared  with  PPV (SMD = - 0.15, 95% CI - 0.47 to 0.16, p = 0.35). PPV was significantly more effective in terms of the VMT release rate (risk ratio = 0.48, 95% CI 0.38-0.62, p = 0.00001) and MH closure rate (risk ratio = 0.49, 95% CI 0.30-0.81, p = 0.006) than ocriplasmin. PV was more effective than ocriplasmin in terms of the VMT release rate (risk ratio = 0.49, 95% CI 0.35-0.70, p = 0.0001). Qualitative analysis showed MH closure rates of 46%, 47.8%, and 95% and VMT releases rates of 46%, 68% and 100% after ocriplasmin, PV, and PPV treatments, respectively.  Adverse events and postoperative complications occurring after treatment have also been documented in these studies. CONCLUSION: PPV appears to be the most promising option for MH closure and VMT release, with fewer serious complications than EVL  or PV. However, given the limited number of studies comparing these treatments, further research is needed to establish the superiority of PPV over the other options.

African hair shaft and pigmented scalp have unique features that challenge diagnosis in scarring alopecia. In addition, Black patients may associate 2 or more types of hair disorders. Therefore, it is imperative to understand their findings thoroughly to establish a good diagnosis. Differential diagnosis on the frontal scalp includes traction alopecia and frontal fibrosing alopecia. Disorders such as central centrifugal cicatricial alopecia, fibrosing alopecia in a pattern distribution, discoid lupus erythematosus, and lichen planopilaris usually affect the middle scalp. Folliculitis decalvans, dissecting cellulitis, and acne keloidalis nuchae are the main differential diagnosis of the posterior scalp.

BACKGROUND: the study determined the validity and reliability of measurements obtained using the portable traction dynamometer (PTD) (E-Lastic, E-Sports Solutions, Brazil) and the reproducibility between evaluators (precision) in the evaluation of the isometric muscle strength of the knee extensors of healthy male adults, compared to measurements obtained with the "gold standard" computerized dynamometer (CD) (Biodex System 3, Nova York, NY, USA). METHODS: we evaluated sixteen recreationally active men (29.50 ± 7.26 years). The test-retest reliability of both equipment to determine quadriceps strength, agreement analysis, and the minimal important difference were verified. RESULTS: excellent test-retest interrater reliability was observed for absolute and relative measurements, with a low absolute error for both sets of equipment and excellent validity of the PTD against the CD, as verified by linear regression and Pearson's correlation coefficient. CONCLUSIONS: PTD is a valid and reliable instrument for assessing the isometric strength of knee extensors, with results similar to the isometric CD "gold standard".

To describe a case of transient bilateral vitreomacular traction syndrome associated with topical 1% pilocarpine ophthalmic solution in both eyes as a treatment for advanced glaucoma. Observations: Spectral-domain OCT demonstrated bilateral vitreomacular traction syndrome after initiation of topical 1% pilocarpine solution in both eyes for advanced glaucoma. Follow-up imaging revealed the resolution of vitreomacular traction after cessation of the drug without a complete posterior vitreous detachment. Conclusions and Importance: In the advent of new pilocarpine formulations, this case raises the concern of vitreomacular traction syndrome as a serious potential sequela of long-term topical pilocarpine use.

High-energy trauma has increased significantly in the last decade, mostly in the lower limbs, in many cases requiring fasciotomy due to the subsequent compartment syndrome. In this context, its closure often leads to a delay in the patient's comprehensive treatment and the return to their activities and may lead to local infection, in addition to generating high costs. There are many options for the plastic surgeon to try to bring the edges together and reconstruct the extremities, such as flaps, grafts, vacuum dressings, and elastic sutures, in addition to expansion devices, sometimes with a combination of the above.


O trauma de grande energia vem aumentando de maneira expressiva na última década, em boa parte de membros inferiores, necessitando, em muitos casos, de fasciotomia devido à síndrome compartimental subsequente. Neste contexto muitas vezes seu fechamento acaba por levar a um retardo no tratamento integral do paciente, do retorno a suas atividades e podendo levar a infecção local, além de gerar altos custos. Há muitas opções ao cirurgião plástico para a tentativa de aproximação de bordos e reconstrução das extremidades, como retalhos, enxertos, curativo a vácuo e sutura elástica, além de dispositivos de expansão, sendo, às vezes, com combinação das anteriores. O método de fechamento apresentado através do alongamento progressivo da pele com fio de Kirschnner representa uma forma de baixo custo e facilmente reproduzível para lidar com este tipo de ferida.

INTRODUCTION: This study aimed to highlight that atlantoaxial rotatory fixation (AARF) can be related to neurosurgery procedures in children, with an afterward demonstration of good results after halo-gravity traction and C1-C2 stabilization using the Harms technique. CASE DESCRIPTION: To the best of our knowledge, this is the first case to report a 4-year-old boy who presented with AARF after a posterior fossa craniotomy to treat a cerebellar astrocytoma. At our medical facility, AARF was diagnosed using plain radiography and computed tomography imaging. The patient was treated with continuous cranial traction for 14 days. Initially, we detected that the patient had no C1 posterior arch or C2 spinous process; therefore, the best option was to perform the Harms technique. Postoperatively, the patient was placed in a cervical collar for 4 weeks. At the 4-year postoperative follow-up, the patient was doing well and had not developed any complications. CONCLUSION: Herein, we report a case in which AARF can be developed after neurosurgical procedure. Surgical techniques used for atlantoaxial subluxation should be carefully selected. In our case, the Harms technique after cranial traction was an excellent option for correcting and stabilizing the abnormal neck position. However, further studies are required to determine the best technique to use in the pediatric population.

INTRODUCTION: Patients with severe spinal deformities represent a major clinical and surgical challenge. Halo-gravity traction (HGT) is a traditional method to correct the deformity prior to surgery. Typically, children undergoing HGT remain in the hospital until surgery. Therefore, it has been suggested to treat these children at lower level healthcare centers or even at home. The aim of this study was to develop a tool to assess patient adherence to HGT together with a program to analyze traction results. MATERIALS AND METHODS: An original recording system was designed with an Arduino Nano®. The data extracted from the memory card were compiled into a text file and then analyzed with the MatLab R2018a MathWorks®. RESULTS: Five patients receiving HGT for severe scoliosis were asked to use the device both in the wheelchair and in bed to evaluate its usefulness. CONCLUSIONS: A device was developed to monitor the use of HGT at home. The device provides information on the time of HGT use and the traction weight placed throughout the day, as well as on the correct functioning of the system in bed and in the wheelchair.

Soil decompaction is an alternative for soybean cultivation in rice areas, but it can affect the growth of irrigated rice in rotation. The objective of the study was to evaluate the effects of soil deep tillage performed before soybean cultivation on rice irrigation water, grain yield, and operational parameters of rice sowing carried out in the following crop season. Soil scarification was implemented in September 2017, prior to soybean crop season in the 2017/18 crop, and the rice experiment was conducted in the 2018/19 season. Treatments were: soil with scarification and soil without scarification. Deep tillage decreased soil density and increased macroporosity, microporosity, and total porosity. Rice cultivation decreased macroporosity and increased microporosity. Soil scarification management had no influence on operational parameters in rice sowing. Soil with scarification required 5.3% more water for rice irrigation than soil without deep tillage. In conclusion, soildeep tillage before the cultivation of soybean crop has effects on rice in thefollowing crop season, maintaining greater soil porosity in relation to the non-deep tillage area and increasing the amount of water needed for irrigation of rice cultivated in the sequence. Deep tillage did not affect sowing operational parameters and rice grain yield.


A descompactação do solo é uma alternativa para o cultivo da soja em áreas de arroz, mas pode afetar o desenvolvimento do arroz irrigado em rotação. O objetivo do estudo foi avaliar o efeito remanescente da escarificação do solo realizada antes do cultivo da soja, sobre o uso da água de irrigação, o rendimento de grãos e parâmetros operacionais da semeadura do arroz, realizada na safra seguinte. A escarificação do solo foi feita no mês de setembro de 2017, anteriormente à soja na safra 2017/18 e o experimento de arroz foi conduzido na safra 2018/19. Os tratamentos foram:escarificação e não escarificação do solo. A escarificação aumentou a macroporosidade, a microporosidade e a porosidade total e diminuiu a densidade do solo. O cultivo do arroz reduziu a macroporosidade e aumentou a microporosidade. Não se observou influência do manejo de escarificação do solo sobre os parâmetros operacionais na semeadura do arroz. O solo escarificado necessitou 5,3 % a mais de água para irrigação do arroz, em relação ao solo não escarificado. Conclui-se que a escarificação do solo antes do cultivo da cultura da soja tem efeito remanescente em arroz na safra seguinte, mantendo maior porosidade do solo em relação à área não escarificada e elevando a quantidade de água necessária para irrigação do arroz cultivado na sequência. Não se verificou interferência da escarificação em parâmetros operacionais da semeadura e sobre o rendimento de grãos do arroz.

ABSTRACT: Soil decompaction is an alternative for soybean cultivation in rice areas, but it can affect the growth of irrigated rice in rotation. The objective of the study was to evaluate the effects of soil deep tillage performed before soybean cultivation on rice irrigation water, grain yield, and operational parameters of rice sowing carried out in the following crop season. Soil scarification was implemented in September 2017, prior to soybean crop season in the 2017/18 crop, and the rice experiment was conducted in the 2018/19 season. Treatments were: soil with scarification and soil without scarification. Deep tillage decreased soil density and increased macroporosity, microporosity, and total porosity. Rice cultivation decreased macroporosity and increased microporosity. Soil scarification management had no influence on operational parameters in rice sowing. Soil with scarification required 5.3% more water for rice irrigation than soil without deep tillage. Resumo O objetivo deste artigo é descrever o caso clínico com uma má oclusão Classe I de Angle com ambos os caninos superiores impactados e a biomecânica ortodôntico-cirúrgica para o posicionamento na linha de oclusão. Paciente do sexo masculino, com 16,7 anos de idade. No exame facial apresentou perfil reto tendendo ao côncavo, nariz e pogônio proeminentes. No exame dentário, má oclusão Classe I, overjet normal, overbite exagerado e a retenção prolongada dos caninos superiores decíduos que, segundo radiografia panorâmica e tomografia, estavam altos e retidos por palatino. Tal quadro estava associado à dilaceração do ápice radicular de ambos os caninos. Durante o progresso do tratamento, observou-se a anquilose do canino esquerdo e preconizou-se a apicotomia, que respondeu à movimentação até a linha de oclusão. O tratamento foi finalizado com overbite adequado com estética e manutenção da harmonia facial. A apicotomia foi eficaz como a última alternativa para o nivelamento. Os resultados de estética, função, saúde e estabilidade após a apicotomia foram obtidos e estão mantidos no follow-up de 10 anos. (AU)


Abstract The aim of this study was to describe an Angle Class I malocclusion with both impacted maxillary canines and orthodontic-surgical biomechanics for positioning in the line of occlusion. Male patient, 16.7 years old. The facial examination showed a straight profile tending to concave, prominent nose, and pogonion. On dental examination, Class I malocclusion, normal overjet, exaggerated overbite, prolonged presence of deciduous canines. Panoramic radiography and tomography revealed a high and palatal position, associated with laceration of the root apex of both canines. During the treatment progress, left canine ankylosis was observed. After the apicotomy, the canine responded to the movement up to the occlusion line and the treatment ended with an appropriate overbite with aesthetics and the maintenance of facial harmony. Apicotomy was effective as an alternative. The results of aesthetics, function, health, and stability after failure of conventional ortho-surgical traction were obtained and are maintained in the 10-year follow-up.(AU)

ABSTRACT Objective: Spinal traction by a cranial halo is a method with more than half a century of application, presenting the possibility of gradual correction of deformities while the patient is awake, also improving respiratory and nutritional patterns. This study aimed to evaluate the complications of pins and traction during their use in treating severe spinal deformities. Methods: We evaluated 27 patients undergoing surgical treatment using halo-gravitational traction pre or interoperatively between 2014 and 2020. Inclusion criteria were the presence of severe deformity (>100º) in the coronal and/or sagittal plane and traction for at least seven days. Two subgroups were identified: patients presenting only severe coronal deformity (Group 1) and patients with severe deformity in the sagittal plane accompanied by coronal deformity (Group 2). Clinical and radiological data were analyzed retrospectively, evaluating the variables: age, sex, weight, height, etiological diagnosis, number of pins, traction time, sagittal and coronal Cobb angle before and after traction, and complications related to pins and traction. Results: Age and weight showed a significant correlation with the occurrence of complications related to the pins (p=0.007; p<0.001), as well as the congenital etiology of deformity (p=0.001), and those patients in group 2 (p=0.006). There was no significant correlation between the variables studied and the occurrence of neurological complications. Conclusion: Halo-gravitational traction is an important adjunctive method in treating severe spinal deformities. Despite having a considerable complication rate, there were no serious events. Resumo: Objetivo: A tração espinhal por halo craniano é um método com mais de meio século de aplicação, apresentando a possibilidade de correção gradual de deformidades com o paciente desperto, além da melhoria do padrão respiratório e nutricional. Este estudo teve como objetivo avaliar as complicações relacionadas aos pinos e à tração durante o seu uso no tratamento de deformidades graves. Métodos: Foram avaliados 27 pacientes submetidos a tratamento cirúrgico com uso de tração halo-gravitacional pré ou inter-operatória, entre 2014 e 2020. Os critérios de inclusão foram a presença de deformidade grave (>100º) coronal e/ou sagital, e duração mínima de 7 dias de tração. Dois subgrupos foram identificados: pacientes com deformidade grave coronal (Grupo 1) e pacientes com deformidade grave sagital acompanhada de deformidade coronal (Grupo 2). Os dados clínicos e radiológicos foram analisados retrospectivamente, incorporando as variáveis: idade, sexo, peso, altura, diagnóstico etiológico, número de pinos, tempo de tração, ângulo de Cobb sagital e coronal pré e pós tração, complicações relacionadas aos pinos e à tração. Resultados: Idade e peso demonstraram correlação significativa com a ocorrência de complicações relacionadas aos pinos (p=0,007; p<0,001), assim como etiologia congênita (p=0,001), e os pacientes incluídos no grupo 2 (p=0,006). Não houve correlação significativa com a ocorrência de complicações neurológicas. Conclusão: A tração halo-gravitacional é um importante método adjuvante no tratamento de deformidades graves da coluna vertebral. Apesar de ter apresentado taxa de complicações considerável, não ocorreram eventos graves. Nível de evidência IV; Série de casos.

Este estudio evaluó las complicaciones relacionadas con los tornillos y la tracción durante el tratamiento de deformidades espinales graves. Métodos: Se evaluó a 27 pacientes sometidos a cirugía con uso de tracción halo-gravitatoria pre o inter quirúrgica, entre 2014 y 2020. Los criterios de inclusión fueron la presencia de deformidad severa (>100º) en el plano coronal y/o sagital y tiempo mínimo de tracción de 7 días. Dos subgrupos fueron identificados: pacientes con deformidad severa en el plano coronal (Grupo 1), y pacientes con deformidad sagital severa acompañada de deformidad coronal (Grupo 2). Los datos clínicos y radiológicos se analizaron retrospectivamente, evaluando edad, sexo, peso, talla, diagnóstico etiológico, número de tornillos, tiempo de tracción, ángulo de Cobb sagital y coronal pre y post tracción, complicaciones relacionadas con los tornillos y tracción. Resultados: Se demostró que la edad y el peso eran factores significativamente correlacionados con las complicaciones de los tornillos (p=0,007; p<0,001), así como la etiología congénita (p=0,001), y los pacientes incluidos en el grupo 2 (p=0,006). No hubo correlación significativa entre las variables estudiadas y complicaciones neurológicas. Conclusión: La tracción halo-gravitacional es un método adyuvante importante en el tratamiento de deformidades espinales severas. Objective: This systematic review aimed to evaluate periodontal parameters in orthodontically tractioned teeth compared with the respective non-tractioned contralateral teeth. Methods: Search strategies were developed for six electronic databases and gray literature. Random-effects meta-analyses were performed for the outcomes of interest. Furthermore, the certainty of the evidence was assessed using the GRADE (Grading of Recommendations Assessment, Development, and Evaluation) tool. Results: Overall, 2,082 articles were identified, of which 24 were selected for the qualitative synthesis. A significant difference was observed between the impacted and contralateral teeth (mean difference [MD] = 0.25; 95% confidence interval [CI] = 0.10-0.40; I2 = 0%) when the gingival index was evaluated. Additionally, impacted teeth showed a greater probing depth, with a significant mean difference between the groups (MD = 0.14; 95% CI = 0.07-0.20; I2 = 6%). Most studies had a low risk of bias; however, the certainty of the evidence was very low owing to the design of existing studies. Conclusions: The evidence in the literature indicated that tractioned teeth might show worsening of periodontal parameters related to the gingival index and probing depth; however, the evidence remains uncertain about this outcome. Furthermore, probing depth should be considered regarding its clinical significance because of the small effect size observed.

BACKGROUND: Cataract surgery with multifocal IOLs could give patients good vision and great satisfaction, at the same time generating high expectations; therefore, its precise indication is essential if we are to reach our goal. The use of optical coherence tomography may be a valuable tool in the screening of macular diseases, which often cannot be detected in routine clinical examinations. This study evaluates the benefit of including spectral domain optical coherence tomography (SD-OCT) in routine preoperative cataract surgery protocols for better case selection in multifocal IOLs. METHODS: Observational and retrospective clinical study that includes patients with an indication for multifocal IOL implantation who underwent retinal fundus exam and SD-OCT examination between 2018 and 2019. The clinical examination with ophthalmoscopy and SD-OCT imaging results were evaluated to observe their influence on the final choice of the lens implanted lens in cataract surgery. RESULTS: 405 eyes from 207 patients with multifocal IOL indication were included. It was found that 220 (54.2%) of all indicated multifocal or trifocal IOLs were in fact implanted. The most important reason for not implanting the indicated IOL was financial, in 116 (59.46%) eyes. The second cause were retinal abnormalities detected by SD-OCT, 63 eyes (15.6%). Those abnormalities included dry age-related macular degeneration (AMD) (50.7%), neovascular AMD (3.1%), vitreomacular adhesion (11.1%), diabetic macular edema (3.1%), epiretinal membrane (ERM) (25.3%) and other macular abnormalities (6.3%). Of the 63 eyes with an abnormal SD-OCT result, 44 (69.8%) were also identified by fundus examination. Nineteen (30.2%) eyes had abnormalities detected only by SD-OCT imaging with a normal clinical exam. CONCLUSIONS: Routine use of SD-OCT imaging may help diagnose pre-existing macular pathologies not identified by clinical exam, helping both physicians and patients choose the ideal IOL individually and has the potential to prevent unsatisfactory functional results.

PURPOSE: To report the results of prolonged post-operative halo-gravity traction in a patient in whom the surgery had to be interrupted unexpectedly and for whom subsequently specific clinical circumstances contraindicated completion of the surgical procedure. METHODS: The patient was a 15-year-old male with severe cervico-dorsolumbar lordoscoliosis who was being studied for associated diffuse axonal injury. He performed halo-gravity traction for 12 weeks. Subsequent surgical management consisted of occipito-lumbar posterior instrumented fusion. During the surgical approach, electrocardiographic changes with hemodynamic decompensation were detected that did not improve with anesthetic reanimation. The intervention was stopped, the surgical wound was closed, and the patient was transferred to the intensive care unit (ICU). It was decided that a revision surgery with the aim to continue with the previous strategy would imply a high risk of perioperative morbidity and mortality. RESULTS: Orthopedic management was decided upon consisting of continued halo-gravity traction with wheelchair modification at home, which was extended to a period of 12 months because of the good results obtained in terms of cervicothoracic realignment. Two years after halo-gravity discontinuation, clinical and radiographic occipito-cervical alignment was good and the patient conserved certain occipito-cervical range of motion and had the capacity of maintaining a horizontal gaze. CONCLUSION: We considered the outcome extraordinary and relevant in this complex and unusual patient. A longer follow-up will provide more data regarding the final outcome of this treatment.
