CASE REPORT: This case report describes the multidisciplinary approach performed in a 9-year-old male patient with dental agenesia affecting teeth 21 and 22. Autotransplantation of the right upper second premolar with incomplete rhizogenesis to the missing area was combined with coronary reshaping with resin composite and orthodontic therapy. The treatment began with the extraction of the deciduous upper left central incisor, bone preparation for the recipient site of the donor tooth, atraumatic extraction of the right upper second premolar and immediate autotransplantation in the surgically prepared recipient site. Subsequently, sutures to reposition the flap and a rigid splint were performed. After 12 months, coronary reshaping of the autotransplanted tooth with resin composite was carried out. Orthodontic treatment involving the use of a fixed appliance was used to correct the interdental spaces and achieve adequate occlusion. Clinical and radiographic follow-up 10 years after tooth autotransplantation and 9 years after reshaping revealed partial obliteration of the pulp chamber, root resorption, ankylosis and the presence of endodontic treatment. CONCLUSIONS: The long-term outcomes highlighted that tooth autotransplantation represents a biologically and cost-effective procedure for replacing missing teeth in young patients, particularly in cases of incomplete rhizogenesis of the autotransplanted tooth. CLINICAL SIGNIFICANCE: This case report discusses tooth autotransplantation and resin composite reshaping as viable and long-term clinical options for treating young patients with dental agenesis.

This case report demonstrates the possibility of functional and aesthetic rehabilitation using an interdisciplinary approach in a child following avulsion of a tooth. Autotransplantation after the development of calcific metamorphosis of the pulp and acute apical periodontitis provided satisfactory repair after root canal treatment. A 7-year-old white boy avulsed the right maxillary central incisor (tooth 11). The extra-alveolar time exceeded 60 minutes, and this triggered external replacement resorption of the avulsed tooth. At 11 years of age, the child underwent tooth autotransplantation. After 4 years of follow-up, intraoral clinical and radiographic examination led to the diagnosis of calcific metamorphosis of the pulp and acute apical periodontitis. The patient had minimally invasive root canal treatment, orthodontic treatment and esthetic rehabilitation with a porcelain crown. Control periapical radiography and tomography of the autotransplanted tooth after 7 years revealed bone repair in the periapical region. This case report indicates that tooth autotransplantation can be used to replace anterior teeth after avulsion followed by replacement resorption. Interdisciplinary management helped to maintain the esthetics and function of the rehabilitated area.

In young patients, premature tooth loss in the anterior maxilla after trauma is challenging for the patient and the dental professional, with serious implications from aesthetic and functional points of view, as well as from a craniofacial growth aspect perspective. Premolars autotransplanted into the maxillary anterior region have been shown to be a biological alternative in this situation. This report describes the clinical management of a case of premature loss of a maxillary central incisor after traumatic injury. A mandibular premolar at the stage of initial root development was transplanted into the alveolar socket of the lost incisor. After 18 years, the transplanted tooth remained responsive to pulp sensibility tests and the periradicular bone and soft tissues were within normal limits. Autotransplantation of premolar teeth into the maxilla could be considered an excellent treatment choice with many biological advantages over implants or fixed dentures as long as proper case selection is followed.

Antecedentes: los autotrasplantes dentales son una técnica que se ha utilizado durantemucho tiempo; sin embargo, son poco utilizados por las múltiples variables que determinansu éxito y la falta de unificación de criterios en la técnica. Objetivo: identificar, por medio deuna revisión sistemática de la literatura, las tasas de éxito de los autotrasplantes dentalesy algunos de los factores que modifican dichas tasas. Método: se realizó una búsqueda deliteratura sobre autotrasplantes dentales en revistas incluidas en las bases de datos biomédicasMedline, EMBASE, CINAHL, Scisearch y PsycLIT, así como en bases de datos electrónicascon un foco específico como Cochrane Controlled Trials Register de la Cochrane Library,Health Technology Assessment y NHS-EED. Se elaboraron medidas de resumen y evaluaciónde la calidad metodológica mediante el programa Revman 5. Resultados: se seleccionaronveintidós artículos en los que las tasas de éxito de los autotrasplantes alcanzaron un 84 %general: 83 % en dientes con ápices cerrados, 95 % cuando se utilizó sutura con férula acrílica y95,75 % en dientes a los que se les realizó terapia endodóntica un mes después del autotransplante.Conclusión: la literatura revisada sugiere que los autotrasplantes dentales pueden seruna alternativa para la restitución a corto, mediano y largo plazo de dientes perdidos, aunqueel nivel de evidencia es III, dado que los estudios analizados son solamente de tipo descriptivo...


Background: Dental autotransplantation is a technique that has been used for a long time;however, it is not frequently used due to the many variables that determine its effectivenessand the lack of unified technical criteria. Aim: Identify, through a systematic review of literature,success rates of tooth autotransplantation and some factors that modify those rates.Method: A literature search on autologous dental transplantation was carried out in journalsincluded in the Medline, EMBASE, CINAHL, SCISEARCH and PsycLIT biomedical databases,as well as in the Cochrane Controlled Trials Register (Cochrane Library), Health TechnologyAssessment and NHS-EED specialized databases. Summary measures were developed andmethodological quality was assessed through the RevMan 5 software. Results: A total of 22articles were selected. They showed 84 % general success rates of autotransplantations, 83 %in closed apex teeth, 95 % when an acrylic splint suture was used, and 95.75 % when rootcanal therapy was carried out one month after the transplantation. Conclusion: The literaturestudied suggests that dental autotransplantation may be an alternative for the restitution oftooth loss in the short-, mid-, and long-term, even though the level of evidence is III becausethe studies analyzed are only descriptive...
