PURPOSE OF REVIEW: The impact of nephrolithiasis on skeletal growth and bone health across the life span of kidney stone formers is reviewed. MAIN FINDINGS: Bone disease is an early event among kidney stone formers (SF), with distinct phenotypes according to each age, sex, menopausal status, dietary, hormonal and genetic factors. Nephrolithiasis-associated bone disorder is characterized by reduced bone mineral density (BMD) and histologically discloses low bone formation, high bone resorption and abnormal mineralization. Although hypercalciuria has been presumed to be pathogenic for bone loss in SF, the association of BMD with urinary calcium is not uniform in all studies. Hypocitraturia, metabolic disturbances, cytokines and receptors, growth factors and acid-base status may all influence skeletal outcomes. The potential link of bone disease with vascular calcification and cardiovascular disease among SF is discussed. The unique vulnerability of the younger skeleton to the effects of nephrolithiasis on attainment of peak bone mass and strength is highlighted and the association of bone loss with kidney stone formation early in life indicate the opportunity for intervention to reduce the risk of future bone fractures.

SUMMARY: Binge drinking in adolescents has a negative effect on the developing skeleton and the attainment of peak bone mass. Our study aimed to examine the effect of binge drinking on the growth and functional integrity of the adolescent Sprague Dawley rat mandible and to determine if a dosage of 1.5 g/kg is sufficient to produce a binge-model of consumption. A total of eight 7-week-old adolescent (male) Sprague Dawley rats were randomly placed into 4 groups with two rats each: 1-week alcohol-exposed rats, 1-week pair- fed control rats, 4-week alcohol-exposed rats and 4-week pair-fed control rats. The alcohol exposed groups were administered a single daily dose via oral gavage of 1.5 g/kg of 20 % alcohol 3 days a week (alternate days) for 7 or 28 days. The pair-fed control groups were administered a caloric equivalent dose of maltose dextrin via oral gavage on the same days as the alcohol-exposed rats. The one-week alcohol exposed, and control rats were terminated on day 7 and the four-week alcohol exposed and control rats on day 28. The mandibles were dissected out and osteometric measurements determined using a digital vernier caliper. Bones were scanned using a 3D-microCT scanner (Nikon XTH 255L). Biomechanical tests were done using a Shimadzu universal testing machine. Differences observed were regarding mandibular osteometry, which showed a reduced height in the central portion of the alveolar bone (Al'-Me), and an increase in the height of the condylar head (Cd-Ag) in the 1-week alcohol-exposed rats when compared to the 1-week pair-fed control rats. No other differences were noted. Lack of significant changes seen between the alcohol and pair-fed control groups in both acute binge and chronic binge exposed rats is likely due to the low dose of alcohol administered to the rats in the study thus a higher dose is proposed.


El consumo excesivo de alcohol en adolescentes tiene un efecto negativo en el desarrollo del esqueleto y en la consecución de la masa ósea máxima. Nuestro estudio tuvo como objetivo examinar el efecto del consumo excesivo de alcohol sobre el crecimiento y la integridad funcional de la mandíbula de la rata adolescente Sprague Dawley y determinar si una dosis de 1,5 g/kg es suficiente para producir un modelo de consumo compulsivo. Un total de ocho ratas Sprague Dawley adolescentes (machos) de 7 semanas de edad se colocaron aleatoriamente en 4 grupos con dos ratas cada uno: ratas expuestas al alcohol durante 1 semana, ratas de control alimentadas en parejas durante 1 semana, ratas expuestas al alcohol durante 4 semanas, y ratas de control alimentadas en parejas durante 4 semanas. A los grupos expuestos al alcohol se les administró una dosis única diaria mediante sonda oral de 1,5 g/kg de alcohol al 20 % 3 días a la semana (días alternos) durante 7 o 28 días. A los grupos de control alimentados por parejas se les administró una dosis calórica equivalente de maltosa dextrina mediante sonda oral los mismos días que a las ratas expuestas al alcohol. Las ratas expuestas al alcohol durante una semana, las ratas de control al día 7, las ratas expuestas al alcohol durante cuatro semanas y las ratas de control al día 28. Se diseccionaron las mandíbulas y se determinaron las mediciones osteométricas utilizando un calibre vernier digital. Los huesos se escanearon utilizando un escáner 3D-microCT (Nikon XTH 255L). Las pruebas biomecánicas se realizaron utilizando una máquina de pruebas universal Shimadzu. Las diferencias observadas se relacionaron con la osteometría mandibular, que mostró una altura reducida en la porción central del hueso alveolar (Al'-Me) y un aumento en la altura de la cabeza condilar (Cd-Ag) en las ratas expuestas al alcohol durante una semana, en comparación con las ratas control alimentadas en parejas durante una semana. No se observaron otras diferencias. La falta de diferencias significativas entre los grupos de alcohol y de control alimentados en parejas expuestas a ebriedad aguda y ebriedad crónica, probablemente se deba a la baja dosis de alcohol administrada a las ratas en el estudio, por lo que se propone una dosis más alta.

Las fisuras labio-máxilo-palatinas (FLMP) son malformaciones congénitas que requieren de un manejo quirúrgico complejo para devolver un correcto funcionamiento del sistema estomatognático. Sin embargo, aún no existe un consenso sobre qué modalidad quirúrgica representa los mejores resultados y sus efectos a largo plazo en el desarrollo esqueletal y funcional de los individuos. El objetivo de este trabajo fue describir mediante la información científica disponible; el efecto de los diferentes protocolos quirúrgicos para el cierre de la FLMP sobre el crecimiento y desarrollo esqueletal de los pacientes. Se realizó una búsqueda de la literatura en bases de datos Pubmed, Cochrane y Epistemonikos, contemplando estudios desde el año 2013 hasta la actualidad. Se obtuvieron un total de 999 estudios encontrados. Una vez aplicados los criterios de inclusión y exclusión, se seleccionaron 13 estudios. El tratamiento quirúrgico de las FLMP consiste en procedimientos complejos que implican alteraciones en el desarrollo esqueletal de los individuos intervenidos a largo plazo. Las discrepancias en el desarrollo esqueletal de los pacientes empiezan a detectarse durante la dentición mixta. Es recomendable extender el seguimiento hasta la etapa de la pubertad y adultez, ya que podría demostrar diferencias significativas entre las diferentes cronologías quirúrgicas empleadas.


Cleft Lip and Palate (CLP) belong to a group of congenital anomalies that require complex surgical management, in order to restore the normal function of the stomatognathic system. However, there is no consensus that determines which surgical technique is better according to its outcomes and long- term effects on the skeletal and functional development of treated individuals. The objectives of this study are To describe, using the available scientific information, the effect of the different surgical protocols regarding CLP closure, on the skeletal and functional development patients undergoing surgery. A research of the available information was made in three different databases: PubMed, Cochrane and Epistemonikos. This research included articles released since the year 2013 onwards. A total of 999 studies were found. Once exclusion and inclusion criteria were applied, 13 studies were selected. Surgical treatment of CLP consists of complex procedures which imply long-term skeletal alterations in undergoing patients. These skeletal discrepancies are first detected during the mixed dentition phase. It is recommended to extend patient follow-up up until puberty and adulthood, as it might demonstrate significant differences among the different surgical approaches.

OBJECTIVE: Apply elliptic Fourier analysis to find shape differences among the hypodivergent, normodivergent, and hyperdivergent growth patterns in skeletal classes I, II and, III in mandibular and maxillary curves and evaluate the discriminatory capacity of these differences. MATERIALS AND METHODS: A total of 626 adult patients were included: 354 Brazilian patients (52 with tomographic information and 302 with radiographic information) and 272 Colombian patients with radiographic information. Lateral views were selected. The maxillary and mandibular curves were digitized. Elliptic Fourier analysis was employed considering with 20 harmonics as well as filtering size, rotation, and translation properties. One-way non-parametric MANOVA was employed to determine differences. A confusion matrix tool was employed to analyze the discriminatory capacity of the model. RESULTS: Significant shape differences in the mandibular and maxillary contours were found among the hypodivergent, normodivergent, and hyperdivergent growth patterns in classes I, II, and III (p < 0.05). The accuracies obtained from the confusion matrix were respectively 74.1, 79.5, and 90.1% in classes I, II, and III in the mandibular curves and respectively 71.9, 73.9, and 75% in classes I, II, and III in the maxillary curves. CONCLUSIONS: Elliptic Fourier analysis can be used to find shape differences with an acceptable discriminatory capacity, especially in the mandible contour. Maxillary and mandibular bone curves each significantly defined facial biotypes regardless of the size and position properties. CLINICAL RELEVANCE: This exploration offers a way to quantify mandibular morphology for the construction of an economic mandibular prediction system applicable to the Latin American population.

BACKGROUND: Pregnancy and lactation in adolescents with low calcium intake may impair fetal growth and infant bone mass. OBJECTIVE: We investigated the effects of calcium plus vitamin D supplementation during pregnancy in Brazilian adolescent mothers consuming low calcium diets (∼600 mg/d) on fetal biometry and infant bone mass, and the relation between infant and maternal bone mass during early lactation. METHODS: Infants of mothers who received calcium (600 mg/d) plus cholecalciferol (200 IU/d) supplementation (n = 30) or placebo (n = 26) from 26 wk of gestation until parturition were studied. Fetal biometric measurements at 23 and 36 wk of gestation were obtained from medical records. Infant anthropometric and total body bone measurements [bone mineral content (BMC), bone area (BA), and bone mineral density (BMD)] at 5 wk postpartum were assessed by dual-energy X-ray absorptiometry. Maternal BMD z scores for total body, lumbar spine, total hip, and femoral neck at 5 wk postpartum were obtained. Group comparisons were adjusted for significant covariates. RESULTS: Maternal mean serum 25-hydroxyvitamin D was 59 nmol/L at baseline in both groups. No differences in fetal measurements at 36 wk of gestation were observed between the groups, except for body weight and its increment from 23 to 36 wk, which were higher in the supplemented group (6.8%, P = 0.014 and 10.5%, P = 0.07, respectively). Infant BMC (61.1 ± 21.7 g), BA (167 ± 79 cm(2)), and BMD (0.385 ± 0.069 g/cm(2)) did not significantly differ between the groups. In the placebo group, infant BMC and BA were negatively correlated with maternal BMD z scores for total body (r = -0.40 and r = -0.47; P < 0.05) and hip (r = -0.41 and r = -0.46; P < 0.05). In contrast, no correlations were observed in the supplemented group. CONCLUSIONS: Calcium and vitamin D supplementation of the adolescents studied resulted in higher fetal body weight at 36 wk of gestation and had no effect on infant bone mass at 5 wk postpartum. Because correlations between maternal and infant bone mass were evident only in the placebo group, infant bone mass appeared to be more dependent on maternal skeletal mass when calcium intake was low.
