Objective To evaluate whether the parallelism of screws with glenoid in Latarjet surgery interferes in the positioning of the graft and to verify the reproducibility of a method of measuring screws positioning. Methods Retrospective, multicenter study, of patients with anterior shoulder instability submitted to modified Latarjet surgery and at least one year of postoperative follow-up. Two radiologists analyzed the postoperative tomographic images, acquired in a database, to evaluate the positioning of screws and radiographic complications. Results We evaluated 34 patients, aged between 21 and 60 years, one of them with bilateral shoulder involvement, totaling 35 shoulders evaluated. The tomographic evaluation of the inclination angles of the screws showed no difference between the observers. There was intra- and interobserver agreement to evaluate the following surgical parameters: graft position, presence or not of radiographic complications. Conclusion The technique described for measuring the parallelism of screws in Latarjet surgery presented a very good and excellent intra-observer agreement, respectively. Screw parallelism with glenoid is recommended; however, it is not a mandatory and unique condition to avoid radiographic complications.

Resumo Objetivo Avaliar se o paralelismo dos parafusos com a glenoide na cirurgia de Latarjet interfere no posicionamento do enxerto e verificar a reprodutibilidade de um método de mensuração da posição dos parafusos. Métodos Estudo retrospectivo, multicêntrico, de pacientes com instabilidade anterior do ombro submetidos à cirurgia de Latarjet modificada e no mínimo 1 ano de seguimento pós-operatório. Dois médicos radiologistas analisaram as imagens tomográficas pós-operatórias, adquiridas em um banco de dados, para avaliação do posicionamento dos parafusos e das complicações radiográficas. Resultados Foram avaliados 34 pacientes, com idades entre 21 e 60 anos, sendo que um deles tinha acometimento bilateral dos ombros, totalizando 35 ombros avaliados. A avaliação tomográfica dos ângulos de inclinação dos parafusos não apresentou diferença entre os observadores. Houve concordância intra e interobservador para avaliação dos seguintes parâmetros cirúrgicos: posição do enxerto, presença ou não de complicações radiográficas. Conclusão A técnica descrita para mensuração do paralelismo dos parafusos na cirurgia de Latarjet apresentou uma concordância intra e inter observador muito boa e excelente, respectivamente. O paralelismo do parafuso com a glenoide é recomendado; no entanto, não é condição obrigatória e única para se evitar as complicações radiográficas.

Objective To investigate the factors that influence the functional results of patients submitted to the Latarjet procedure. Methods Evaluation of 26 patients submitted to surgical treatment following the Latarjet technique due to traumatic recurrent anterior dislocation of the glenohumeral joint, with glenoidal bone loss greater than 20% and/or off-track injury. The minimum follow-up time was of 12 months. The Visual Analogue Scale (VAS), The Western Ontario Shoulder Instability Index (WOSI), and the Subjective Shoulder Value (SSV) scales, as well as objective data from the participants, were evaluated. Results Most patients (84.62%) did not present recurrence of the dislocation, and 92.31% were satisfied. Regarding the functional analysis, the physical component score (PCS) and the mental component score (MCS) found were within the mean quality of life of the population. The physical symptoms, according to the WOSI, presented the best percentage (8.5%), while the worst result was observed regarding lifestyle (20%). On the VAS, pain was classified as moderate (3/10) by 15.38% of the patients (4/26). In relation to sports, patients who practice sports showed improvement in SSV parameters, which had an inverse relationship with the number of relapses. It was also observed that the shorter the time between the first dislocation and the surgery, the greater the patient's satisfaction. Conclusion Early indication of surgical treatment of anterior glenohumeral instability may provide better subjective functional results to the patient.

Resumo Objetivo Investigar os fatores que influenciam os resultados funcionais dos pacientes submetidos ao procedimento de Latarjet. Métodos Avaliação de 26 pacientes submetidos ao tratamento cirúrgico, seguindo a técnica de Latarjet, devido a luxação anterior recidivante traumática da articulação glenoumeral, com perda óssea glenoidal maior do que 20% e/ou lesão off-track. O tempo mínimo de seguimento foi de doze meses. Foram avaliadas as seguintes escalas: Escala Visual Analógica (EVA), The Western Ontario Shoulder Instability Index (WOSI), e Subjective Shoulder Value (SSV), além dos dados objetivos dos participantes. Resultados A maioria dos pacientes (84,62%) não apresentou recidiva de luxação, e 92,31% ficaram satisfeitos. Em relação à análise funcional, a pontuação do componente físico (PCF) e a pontuação do compenente mental (PCM) encontradas estavam dentro da média de qualidade de vida da população. Os sintomas físicos, pela escala WOSI, apresentaram a melhor porcentagem (8,5%), ao passo que o pior resultado foi observado com relação ao estilo de vida (20%). Na EVA, a dor foi classificada como moderada (3/10) por 15,38% dos pacientes (4/26). Em relação aos esportes, os pacientes que praticavam alguma atividade esportiva apresentaram melhora nos parâmetros da escala SSV, que tiveram relação inversa com o número de recidivas. Observou-se ainda que, quanto menor foi o tempo entre a primeira luxação e a realização da cirurgia, maior foi a satisfação do paciente. Conclusão A indicação precoce do tratamento cirúrgico da instabilidade anterior glenoumeral pode proporcionar melhores resultados funcionais subjetivos ao paciente.

A review involving the six major international orthopedic journals has been published recently. It described the tools used for the evaluation of outcomes in the surgical treatment of recurrent anterior dislocation of the shoulder. There are no studies that exhibit the main outcome tools for this disease in Brazil. The authors evaluated the outcomes of clinical studies involving anterior glenohumeral instability that were published in the last decade in the two leading Brazilian orthopedic journals, Revista Brasileira de Ortopedia and Acta Ortopédica Brasileira . A review of the literature was performed, including all clinical papers published between 2007 and 2016 describing at least one outcome measure before and after surgical intervention. The outcomes were range of motion, muscle strength, physical examination testing, patient satisfaction, return to sports, imaging, complications, and functional outcomes scores. Twelve studies evaluating the clinical outcomes of surgical treatment for anterior shoulder instability were published. Ten studies (83%) were case series (level of evidence IV), 1 (8%) was a case-control study (III), and 1 was a retrospective cohort (III). On average, the number of outcomes assessed was 3.7 ± 1.7. The Rowe score was used in 9 studies (75%), and 7 (58%) papers used the University of California Los Angeles (UCLA) scale. Ten studies (83%) reported complications related to surgical treatment. The complication most frequently reported was recurrent instability, found in 9 studies (75%). The national studies have preferentially used scales considered to be of low reliability, responsiveness, and internal consistency.

Resumo Uma revisão que envolveu as seis principais revistas ortopédicas internacionais foi publicada recentemente. A publicação descreveu as ferramentas usadas para a avaliação dos desfechos no tratamento cirúrgico da luxação anterior recidivante do ombro. Não existe um levantamento que mostre as principais formas de avaliação para essa doença no Brasil. Os autores avaliaram os desfechos usados nos estudos clínicos que envolveram a instabilidade glenoumeral anterior e que foram publicados na última década nos dois principais periódicos ortopédicos brasileiros, Revista Brasileira de Ortopedia e Acta Ortopédica Brasileira. Foi feita uma revisão da literatura na qual foram incluídos todos os artigos clínicos publicados entre 2007 e 2016 que descreveram ao menos uma medida de desfecho antes e após intervenção cirúrgica. Os desfechos avaliados foram amplitude de movimento, força muscular, testes de exame físico, satisfação, retorno ao esporte, exames de imagem, complicações e escalas clínicas. Foram publicados 12 estudos que avaliaram os resultados clínicos do tratamento cirúrgico da instabilidade anterior do ombro. Dez estudos (83%) eram séries de caso (nível de evidência IV), 1 (8%) caso-controle (III), e 1 coorte retrospectiva (III). A média de desfechos avaliados por estudo foi de 3,7 ± 1,7. A escala Rowe foi usada em 9 estudos (75%), e 7 publicações (58%) usaram a escala da University of California Los Angeles (UCLA). Dez estudos (83%) relataram as complicações relacionadas ao tratamento cirúrgico. A complicação mais frequentemente relatada foi a recidiva da luxação glenoumeral, encontrada em 9 estudos (75%). Os estudos nacionais usaram preferencialmente escalas consideradas de baixa conficabilidade, responsividade, e consistência interna.

OBJECTIVE: Analyze the postoperative follow-up of patients undergoing shoulder arthroscopy for treatment of anterior instability and correlate with the prevalence of recurrence. METHODS: A six-question survey was applied by phone and mail to 65 patients, seeking information on the current result of the surgical procedure. All patients were treated arthroscopically for anterior shoulder instability, with at least 12 months of postoperative time. Patients with associated posterior labial lesions and revision surgeries were not included. RESULTS: At the time of the survey the patients had a median of 56 (IQR: 34.5-110.5) postoperative months. The mean sample age was 24.6 years (maximum = 47, minimum = 12; SD = 7.3). Complaint of pain in the shoulder was observed in 20 patients (30.7%). Dislocation recurrence was observed in 10 patients (15.3%). Forty-four patients (67.6%) considered their shoulder normal, which was more frequent in non-recurrence patients (p < 0.001). Forty-three patients (66.1%) returned to their previous level of sport and there was no difference between recurrence and non-recurrence patients (p = 0.456). It was found that the prevalence of recurrence was 5.6 (95% CI: 1.30-24.46) times higher in individuals who abandoned monitoring before six months postoperatively (p = 0.012). CONCLUSION: The abandonment of postoperative monitoring in the early stages, when the patients receive orientation for muscle strengthening, proprioceptive education, and dangerous movements to avoid, can increase the rates of recurrent shoulder dislocation in patients treated for anterior instability by arthroscopy.

Todos os pacientes foram operados para corrigir uma instabilidade anterior do ombro por artroscopia e tinham pelo menos 12 meses de pós-operatório. Não foram incluídos pacientes com associação de lesão labral posterior e cirurgias de revisão. RESULTADOS: O questionário foi aplicado com uma mediana de 56 (IIQ: 34,5 a 110,5) meses. Foi verificado que os indivíduos que abandonaram o acompanhamento pós-operatório antes dos seis meses tiveram uma prevalência 5,6 (IC 95%: 1,30-24,46) vezes maior de recidiva (p = 0,012). CONCLUSÃO: O abandono do acompanhamento pós-operatório na fase inicial, na qual o paciente recebe orientações para o reforço muscular e a educação proprioceptiva, pode colaborar no aumento do índice de recidiva da luxação nos pacientes tratados por artroscopia.

OBJECTIVE: To compare the use of anchors with double and single-thread loading in the single-row Bankart arthroscopic procedure. METHODS: 252 patients (258 shoulders) underwent Bankart arthroscopic surgery with evaluation after a minimum follow-up of 2 years. They underwent repairs either using anchors with single loading of a high-resistance non-absorbable braided thread (206 shoulders; group AS) or using double loading of thread with the same characteristics (52 shoulders; group AD). The patients were evaluated using the UCLA and Carter-Rowe scales. The patients' return to sports activity and recurrences were also compared. RESULTS: There was no significant difference between the groups regarding the surgical failure rate (group AS 5.8%; group AD 7.7%; p = 0.62). Group AS presented a better mean Carter-Rowe score (group AS 94.4; group AD 88.6; p < 0.05) and greater return to the same sports level (group AS 79.1; group AD 72.1; p < 0.05). CONCLUSION: Use of anchors with double thread loading did not show any clinical advantage for arthroscopic repair of traumatic anterior shoulder instability, in relation to use of single-thread anchors, over a 2-year follow-up.


OBJETIVO: Comparar o uso de âncoras com carregamento duplo e com carregamento simples de fio no procedimento artroscópico de Bankart com fileira simples. MÉTODOS: Foram submetidos à cirurgia artroscópica de Bankart e avaliados após seguimento mínimo de dois anos 252 pacientes (258 ombros). Foram submetidos a reparo com âncoras com carregamento simples de fio trançado não absorvível de alta resistência 206 ombros (grupo AS) e com âncoras com carregamento duplo com fios de mesmas características 52 (grupo AD). Os pacientes foram avaliados segundo as escalas UCLA e Carter-Rowe. O retorno à atividade esportiva e a recidiva também foram comparados. RESULTADOS: Não houve diferença significante entre os grupos quanto à taxa de falha cirúrgica (grupo AS 5,8%; grupo AD 7,7%; p = 0,62). O grupo AS apresentou melhor Carter-Rowe médio (grupo AS 94,4; grupo AD 88,6; p < 0,05) e maior retorno ao mesmo nível esportivo (grupo AS 79,1; grupo AD 72,1; p < 0,05). CONCLUSÃO: O uso de âncoras com carregamento duplo de fios não demonstrou vantagem clínica no reparo artroscópico da instabilidade anterior traumática do ombro em relação ao uso de âncoras simples no seguimento de dois anos.

OBJECTIVE: To evaluate the functional results from arthroscopic repair of SLAP lesions through the portal described by O'Brien. METHODS: A retrospective evaluation was conducted on 19 shoulders in 18 patients who underwent arthroscopic repair of SLAP lesions through the O'Brien portal between November 2007 and January 2012. RESULTS: Nineteen shoulders in 18 patients were evaluated: 16 male patients (84.2%) and three female patients (15.7%). The patients' ages ranged from 27 to 40 years (mean of 34.3 years). There were 12 patients (63.1%) with injuries on the right shoulder, six (31.5%) with injuries on the left shoulder and one (5.2%) with bilateral injury. In relation to dominance, 13 patients (68.4%) presented the injury on the dominant limb and five (26.3%) were affected on the non-dominant limb. We observed that nine cases (47.3%) had SLAP lesions alone and 10 cases (52.6%) were related to glenohumeral instability. There was one case (5.2%) of recurrence of glenohumeral dislocation, but this patient chose not to undergo a new surgical intervention. According to the UCLA and ASES scales translated and adapted to the Portuguese language, 96% of the results were good or excellent. CONCLUSION: The approach for treating SLAP lesions through the portal described by O'Brien et al. is easy to reproduce, with a high rate of good and excellent results and a low complication rate.


OBJETIVO: Avaliar os resultados funcionais do reparo artroscópico da lesão SLAP pelo portal descrito por O'Brien. MÉTODOS: Foi feita avaliação retrospectiva de 19 ombros de 18 pacientes submetidos ao reparo artroscópico da lesão SLAP pelo portal de O'Brien, de novembro de 2007 a janeiro de 2012. RESULTADOS: Foram avaliados 19 ombros de 18 pacientes, 16 (84,2%) do sexo masculino e três (15,7%) do feminino. A idade variou de 27 a 40 anos (média de 34,3). No estudo, 12 (63,1%) pacientes tiveram lesão no ombro direito, seis (31,5%) no ombro esquerdo e houve um (5,2%) caso de lesão bilateral. Em relação à dominância, 13 (68,4%) pacientes apresentaram a lesão no membro dominante e cinco (26,3%) tiveram o membro não dominante acometido. Observamos que nove (47,3%) casos tiveram lesão SLAP isolada, 10 (52,6%) casos foram relacionados a instabilidade glenoumeral e apenas um (5,2%) caso teve recidiva da luxação glenoumeral. Esse paciente optou por não fazer nova intervenção cirúrgica. De acordo com as escalas ULCA e ASES traduzida e adaptada para a língua portuguesa, obteve-se 96% de excelentes e bons resultados. CONCLUSÃO: A abordagem da lesão SLAP pelo portal descrito por O'Brien et al. é de fácil reprodutibilidade, com alto índice de excelentes e bons resultados e baixo índice de complicações.

To compare the use of anchors with double and single-thread loading in the single-row Bankart arthroscopic procedure. METHODS: 252 patients (258 shoulders) underwent Bankart arthroscopic surgery with evaluation after a minimum follow-up of 2 years. They underwent repairs either using anchors with single loading of a high-resistance non-absorbable braided thread (206 shoulders; group AS) or using double loading of thread with the same characteristics (52 shoulders; group AD). The patients were evaluated using the UCLA and Carter-Rowe scales. The patients' return to sports activity and recurrences were also compared. RESULTS: There was no significant difference between the groups regarding the surgical failure rate (group AS 5.8%; group AD 7.7%; p = 0.62). Group AS presented a better mean Carter-Rowe score (group AS 94.4; group AD 88.6; p < 0.05) and greater return to the same sports level (group AS 79.1; group AD 72.1; p < 0.05). CONCLUSION: Use of anchors with double thread loading did not show any clinical advantage for arthroscopic repair of traumatic anterior shoulder instability, in relation to use of single-thread anchors, over a 2-year follow-up...


Comparar o uso de âncoras com carregamento duplo e com carregamento simples de fio no procedimento artroscópico de Bankart com fileira simples. MÉTODOS: foram submetidos à cirurgia artroscópica de Bankart e avaliados após seguimento mínimo de dois anos 252 pacientes (258 ombros). Foram submetidos a reparo com âncoras com carregamento simples de fio trançado não absorvível de alta resistência 206 ombros (grupo AS) e com âncoras com carregamento duplo com fios de mesmas características 52 (grupo AD). Os pacientes foram avaliados segundo as escalas UCLA e Carter-Rowe. O retorno à atividade esportiva e a recidiva também foram comparados. RESULTADOS: não houve diferença significante entre os grupos quanto à taxa de falha cirúrgica (grupo AS 5,8%; grupo AD 7,7%; p = 0,62). O grupo AS apresentou melhor Carter-Rowe médio (grupo AS 94,4; grupo AD 88,6; p < 0,05) e maior retorno ao mesmo nível esportivo (grupo AS 79,1; grupo AD 72,1; p < 0,05). CONCLUSÃO: o uso de âncoras com carregamento duplo de fios não demonstrou vantagem clínica no reparo artroscópico da instabilidade anterior traumática do ombro em relação ao uso de âncoras simples no seguimento de dois anos...

This paper presents the main surgical techniques applied in the treatment of anterior recurrent shoulder dislocation, aiming the achievement of the normality of articulate movements. This was obtained by combining distinct surgical procedures, which allowed the recovery of a complete functional capacity of the shoulder, without jeopardizing the normality of movement, something that has not been recorded in the case of the tense sutures of the surgical procedures of Putti-Platt, Bankart, Latarjet, Dickson-O'Dell and others. The careful review of the methods applied supports the conclusion that recurrent shoulder dislocation can be cured, since cure has been obtained in 97% of the treated cases. However, some degree of limitation in the shoulder movement has been observed in most of the treated cases. Our main goal was to achieve a complete shoulder functional recovery, by treating simultaneously all of the anatomical–pathological lesions, without considering the so-called essential lesions. The period of post-operatory immobilization only last for the healing of soft parts; this takes place in a position of neutral shoulder rotation, since the use of vascular bone graft eliminates the need for long time immobilization, due to the shoulder stabilization provided by rigid fixation of the coracoid at the glenoid edge, as in the Latarjet's technique. Our procedure, used since 1959, comprises the association of several techniques, which has permitted shoulder healing without movement limitation. That was because of the tension reduction in the sutures of the subescapularis, capsule, and coracobraquialis muscles...


O presente trabalho analisa as principais técnicas cirúrgicas empregadas no tratamento da luxação recidivante do ombro (LRO), com o objetivo de obter a normalidade da amplitude dos movimentos articulares e associar diferentes tempos cirúrgicos num único procedimento para obter uma capacidade funcional completa, sem comprometer a normalidade dos movimentos, por causa das suturas tensas usadas nas cirurgias de Putti-Platt, Bankart, Latarjet, Dickson-O'Dell e outras. Após cuidadosa revisão desses métodos em uso, chegamos à conclusão de que a LRO pode ser considerada resolvida quanto à porcentagem de cura (97%). Permanecem, no entanto, limitações dos movimentos na grande maioria dos casos, aceitas até como necessárias para evitar recidivas. O nosso objetivo cirúrgico visa à obtenção de uma recuperação funcional completa, atuar simultaneamente sobre as várias lesões anatomopatológicas e abandonar a ideia das chamadas "lesões essenciais". A imobilização do ombro operado será feita somente durante a cicatrização das partes moles em rotação neutra. Com o uso de um enxerto ósseo pediculado dispensa-se qualquer tipo de imobilização prolongada, por causa da estabilidade obtida pela osteossíntese da coracoide no rebordo da glenoide, como na técnica de Latarjet. Essa nossa conduta, empregada desde 1959, consiste, portanto, na associação das várias técnicas com as quais se obtêm a cura sem limitação dos movimentos, por causa da redução da tensão nas suturas da cápsula e dos músculos subescapular e coracobraquial empregadas nas técnicas acima...

OBJECTIVE: To evaluate the clinical results from patients with traumatic anterior shoulder instability that was treated surgically through arthroscopic viewing, using bioabsorbable anchors and a technique for remove the cartilage of the anterior glenoid rim for repairing a Bankart lesion. METHOD: Between March 2006 and October 2008, 27 shoulders in 27 patients with a diagnosis of traumatic anterior shoulder instability were operated. The patients' mean age was 28 years and they had had between two and 25 previous episodes of dislocation. The patients were predominantly male (24; 89%). The minimum length of follow-up was 24 months and the mean was 36 months. None of the patients had previously undergone surgery on the affected shoulder or had any significant bone lesion at the glenoid margin. The postoperative clinical assessment was done using the Rowe scale. To measure the preoperative and postoperative joint range of motion, we used the method described by the American Academy of Orthopaedic Surgeons (AAOS). RESULTS: According to the Rowe criteria, 25 patients (93%) achieved excellent results and two (7%) had poor results. None of the patients presented good or fair results. Twenty-three patients were satisfied with the results obtained (85%), and returned to their activities without limitations, while four patients (15%) had some degree of limitation. There was recurrence of instability in two patients (7%). CONCLUSION: Treatment of traumatic anterior shoulder instability through arthroscopic viewing using a technique for remove the cartilage of the anterior glenoid rim for repairing a Bankart lesion provided excellent results for 93% of the patients operated.

OBJECTIVE: Evaluate the results from the Latarjet procedure in patients with anterior recurrent dislocation of the shoulder who present bone loss of the glenoid cavity greater than 25%. METHODS: Twenty six male patients underwent the Latarjet procedure, The bone loss was evaluated by means of radiography using the Bernageau view and by means of CAT scan. The patients were evaluated with regard to range of motion, using the Rowe and UCLA scales, before and after the operation, and by radiographs to assess the presence of arthrosis, position and consolidation of the graft and positioning of the screws. Statistical analysis was used to assess whether there was any relationship between the number of episodes of dislocation and the presence of arthrosis, , and any relationship between arthrosis and limitations on lateral rotation. Differences in range of motion between the operated and unaffected sides and in the UCLA and Rowe scale. RESULTS: The means for elevation and lateral rotation were statistically poorer on the operated side. The UCLA and Rowe scale showed that there was a statistically significant improvement in the clinical-functional results (P < 0.001 for both). There was a relationship between the number of episodes of dislocation and the presence of arthrosis, We also did not observe any correlation between limitations on lateral rotation and arthrosis. CONCLUSION: The Latarjet procedure is an efficient method for cases of severe erosion of the glenoid margin.

OBJECTIVE: To evaluate the clinical result from the filling ("remplissage") technique in association with Bankart lesion repair for treating recurrent anterior shoulder dislocation. METHODS: Nine patients (10 shoulders), with a mean follow-up of 13.7 months, presented traumatic recurrent anterior shoulder dislocation. All of them had a Bankart lesion, associated with a Hill-Sachs lesion showing the "engaging" sign. The Hill-Sachs lesion defect was measured and showed an average bone loss of 17.3% (7.7% to 26.7%) in relation to the diameter of the humeral head. All the cases underwent arthroscopic repair of the Bankart lesion, together with filling of the Hill-Sachs lesion by means of tenodesis of the infraspinatus. RESULTS: The Rowe score ranged from 22.5 (10 to 45) before the operation to 80.5 (5 to 100) after the operation (p > 0.001). The UCLA score ranged from 18.0 (8 to 29) to 31.1 (21 to 31) (p > 0.001). The measurements of external and internal rotation at abduction of 90° after the operation were 63.5° (45° to 90°) and 73° (50° to 92°) respectively. Two patients presented recurrence (one with dislocation and the other with subluxation). None of the patients presented pain in the region of the infraspinatus tendon after the operation. CONCLUSION: Over the short term, the filling ("remplissage") arthroscopic technique produced improvements in functional scores and a low complication rate when used for treating glenohumeral instability associated with Hill-Sachs lesions.
