La medicina nuclear utiliza radiofármacos, conocidos como radiotrazadores, para evaluar la función y el metabolismo de órganos y sistemas del cuerpo. Objetivo. Examinar la relevancia de los radiotrazadores MDP, DTPA y MIBI, marcados con tecnecio-99m (Tc99m), en el diagnóstico en medicina nuclear. Metodología. Se siguió el método PRISMA para identificar estudios publicados entre 2010 y 2022. Las bases de datos consultadas incluyeron Dialnet, Elsevier, Research, Redalyc, PubMed, Google Académico y Scielo. Se utilizaron descriptores específicos como "radiotrazadores Tc99m", "MDP", "DTPA", "MIBI", y "medicina nuclear", centrando la búsqueda en diagnóstico y excluyendo propósitos terapéuticos. Resultados. De 14 estudios analizados, se observó una predominancia en investigaciones sobre radiotrazadores MDP y MIBI marcados con Tc99m, enfocándose en diagnósticos relacionados con lesiones paratiroideas, hiperparatiroidismo, enfermedades esqueléticas, enfermedad arterial coronaria y perfusión miocárdica. Conclusiones. Los radiotrazadores MDP, DTPA y MIBI marcados con Tc99m demuestran eficacia en diversas aplicaciones diagnósticas, incluyendo la localización de adenomas paratiroideos y la detección de condiciones como el mieloma múltiple. A pesar de sus beneficios, es crucial continuar investigando y desarrollando nuevos radiofármacos para expandir su utilidad clínica y mejorar aún más la atención médica en el campo de la medicina nuclear.


Nuclear medicine uses radiopharmaceuticals, known as radiotracers, to assess the function and metabolism of organs and body systems. Objective. To examine the relevance of technetium-99m (Tc99m)-labeled MDP, DTPA, and MIBI radiotracers in nuclear medicine diagnostics. Methodology. The PRISMA method was followed to identify studies published between 2010 and 2022. The databases consulted included Dialnet, Elsevier, Research, Redalyc, PubMed, Google Scholar and Scielo. Specific descriptors such as "Tc99m radiotracers", "MDP", "DTPA", "MIBI", and "nuclear medicine" were used, focusing the search on diagnosis and excluding therapeutic purposes. Results. Of 14 studies analyzed, there was a predominance of research on Tc99m-labeled MDP and MIBI radiotracers, focusing on diagnoses related to parathyroid lesions, hyperparathyroidism, skeletal diseases, coronary artery disease, and myocardial perfusion. Conclusions. Tc99m-labeled MDP, DTPA, and MIBI radiotracers demonstrate efficacy in a variety of diagnostic applications, including localization of parathyroid adenomas and detection of conditions such as multiple myeloma. Despite their benefits, it is crucial to continue researching and developing new radiopharmaceuticals to expand their clinical utility and further improve medical care in the field of nuclear medicine.


A medicina nuclear utiliza radiofármacos, conhecidos como radiotraçadores, para avaliar a função e o metabolismo de órgãos e sistemas corporais. Objetivo. Examinar a importância dos radiotraçadores MDP, DTPA e MIBI, marcados com tecnécio-99m (Tc99m), em diagnósticos de medicina nuclear. Metodologia. O método PRISMA foi usado para identificar estudos publicados entre 2010 e 2022. Os bancos de dados consultados incluíram Dialnet, Elsevier, Research, Redalyc, PubMed, Google Scholar e Scielo. Foram utilizados descritores específicos como "radiotraçadores Tc99m", "MDP", "DTPA", "MIBI" e "medicina nuclear", concentrando a busca em fins diagnósticos e excluindo fins terapêuticos. Resultados. Dos 14 estudos analisados, houve predomínio de pesquisas com os radiotraçadores MDP e MIBI marcados com Tc99m, com foco em diagnósticos relacionados a lesões da paratireoide, hiperparatireoidismo, doenças esqueléticas, doença arterial coronariana e perfusão miocárdica. Conclusões. Os radiotraçadores MDP, DTPA e MIBI marcados com Tc99m demonstram eficácia em uma variedade de aplicações diagnósticas, incluindo a localização de adenomas de paratireoide e a detecção de doenças como o mieloma múltiplo. Apesar de seus benefícios, é fundamental continuar a pesquisa e o desenvolvimento de novos radiofármacos para expandir sua utilidade clínica e melhorar ainda mais o atendimento médico no campo da medicina nuclear.

Introducción: describir el caso de un paciente con pancreatitis aguda secundaria a hipercalcemia por hiperparatiroidismo prImario. Esta es una causa poco frecuente de pancreatitis, asociada a morbimortalidad significativa en caso de no ser diagnosticada oportunamente Caso clínico: un hombre de 44 años, con antecedente de pancreatitis de presunto origen biliar que había requerido previamente colecistectomía, consultó por dolor abdominal y náuseas. Los estudios complementarios fueron compatibles con un nuevo episodio de pancreatitis aguda. Presentaba hipercalcemia y hormona paratiroidea (PTH) elevada, configurando hiperparatiroidismo primario. La gammagrafía informó hallazgos compatibles con adenoma paratiroideo. Se inició tratamiento con reanimación hídrica y analgesia con adecuada disminución de calcio sérico y resolución de dolor abdominal. Después de la paratiroidectomía se logró normalizar los niveles de calcio y PTH. Discusión: la pancreatitis aguda es una condición potencialmente fatal, por lo que la sospecha de causas poco frecuentes como la hipercalcemia debe tenerse en cuenta. El tratamiento de la hipercalcemia por adenoma paratiroideo se basa en reanimación hídrica adecuada y manejo quirúrgico del adenoma, con el fin de evitar recurrencia de pancreatitis y mortalidad. Introduction: we describe the case of a patient with acute pancreatitis secondary to hypercalcemia due to primary hyperparathyroidism. This is a rare cause of pancreatitis associated with significant morbidity and mortality if not diagnosed in time. Clinical case: a 44-year-old man with a history of pancreatitis of presumed biliary origin, which had previously required cholecystectomy, consulted for abdominal pain and nausea. The laboratory findings were compatible with a new episode of acute pancreatitis. He presented hypercalcemia and an elevated parathyroid hormone (PTH), configuring primary hyperparathyroidism. Scintigraphy was performed, yielding findings compatible with parathyroid adenoma. Treatment with fluid resuscitation and analgesia was started, resulting in an adequate decrease in serum calcium and resolution of abdominal pain. After parathyroidectomy, calcium and PTH levels were normalized. Discussion: acute pancreatitis is a potentially fatal condition; therefore the suspicion of rare causes, such as hypercalcemia, should be considered. The treatment of hypercalcemia due to parathyroid adenoma is based on adequate fluid resuscitation and surgical management of the adenoma, to avoid recurrence of pancreatitis and death. (AU)

RESUMO

The anomalous origin of the coronary arteries (AOCA) has several patterns. Most are functional and asymptomatic. However, some are associated with persistent chest pain and sudden cardiac death. Multiple imaging techniques are available for the assessment of AOCA. We present a report of 4 cases with AOCA, including the anomalous aortic origin of a coronary artery (AAOCA) of the right coronary artery, AAOCA of the circumflex artery, AAOCA of the left anterior descending artery, and AAOCA of the circumflex artery with retroaortic trajectory, in which the clinical manifestations throughout the cases are discussed, highlighting the similarity among patients despite having different patterns. Multiple imaging techniques are indispensable for assessing AOCA, where transthoracic echocardiogram is the first-line study, and cardiac computed tomography provides detailed cardiac and coronary anatomy.

Abstract Introduction Surgical treatment of hyperparathyroidism related to chronic kidney disease is a real challenge for Brazilian public health care. High cost medications and long waiting lines to perform preoperative exams, especially technetium Tc 99m Sestamibi (MIBI) are some of the reasons. Despite the reality that the aid of localization exams are questionable in this scenario, doctors are too apprehensive in performing surgery without it. Objective The study aimed at evaluating the efficacy of surgery for renal hyperparathyroidism without preoperative MIBI. Methods A total of 114 patients were surgically treated. Total parathyroidectomy with autotransplantation and subtotal parathyroidectomy were carried out without preoperative MIBI. Results and conclusion Among the 114 patients undergoing surgery, 37 had secondary hyperparathyroidism in dialysis replacement, and 77 patients had post-renal transplant persistent disease. We were successful in 107 cases with only 7 failures (93.8% of success rate). Among these failures, only one parathyroid gland was not found in 4 cases, 2 parathyroid glands were not found in 2 cases and in 1 patient the 4 glands were found but this patient remained hypercalcemic and a postoperative diagnosis of supernumerary parathyroid gland was made. Surgery for treatment of renal hyperparathyroidism proved to be an effective (93.8%) and reproductible procedure, even without MIBI.


Resumo Introdução O tratamento cirúrgico do hiperparatireoidismo relacionado à doença renal crônica é um verdadeiro desafio para a saúde pública brasileira. Medicamentos de alto custo e longas filas de espera para exames pré‐operatórios, principalmente a cintilografia com tecnécio Tc‐99m Sestamibi, MIBI, são alguns dos motivos. Apesar da contribuição de exames de localização ser questionável nesse cenário, os médicos ficam muito apreensivos por fazer uma cirurgia sem ele. Objetivo Avaliar a eficácia da cirurgia para hiperparatireoidismo renal sem o MIBI pré‐operatório. Método Foram tratados cirurgicamente 114 pacientes. A paratireoidectomia total com autotransplante e a paratireoidectomia subtotal foram feitas sem MIBI pré‐operatório. Resultados e conclusão Entre os 114 pacientes submetidos à cirurgia, 37 apresentavam hiperparatireoidismo secundário em reposição dialítica e 77 doença persistente pós‐transplante renal. Tivemos sucesso em 107 casos, com apenas 7 falhas (93,8% de taxa de sucesso). Entre essas falhas, uma glândula paratireoide não foi encontrada em 4 casos, 2 glândulas paratireoides não foram encontradas em 2 casos e em um paciente as 4 glândulas foram encontradas, mas ele permaneceu hipercalcêmico com diagnóstico pós‐operatório de glândula paratireoide supranumerária. A cirurgia para tratamento do hiperparatireoidismo renal mostrou‐se um procedimento eficaz (93,8%) e reprodutível mesmo sem MIBI.

We report a patient with multiple myeloma (MM) and polyarthritis of large joints. During the staging of the disease, bone marrow diffusely involved by MM was clearly demonstrated by 99mTc-2-methoxy-isobutyl-isonitrile (MIBI) single-photon emission computed tomography/computed tomography (SPECT/CT) but not by 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography/CT images. On the other hand, a very intense uptake of 18F-FDG was detected in periarticular tissues of multiple joints, with nonabnormal 99mTc-MIBI accumulation. Rheumatology tests were negative. A subsequent bone scintigraphy demonstrated radiolabeled bisphosphonate accumulation in periarticular tissues, suggesting amyloid arthropathy.

RESUMEN El hiperparatiroidismo terciario es una complicación presentada en pacientes con enfermedad renal crónica llevados a trasplante renal cuyo manejo quirúrgico requiere de la ubicación mediante imágenes diagnosticas para garantizar el éxito de esta intervención. Se presenta caso de mujer en hemodiálisis crónica que presentó hiperparatiroidismo terciario por hiperplasia paratiroidea confirmada con Tc-99m MIBI SPECT.


ABSTRACT Tertiary hyperparathyroidism is a complication presented in patients with chronic kidney disease subjected to renal transplantation whose surgical management requires location through diagnostic images to guarantee the success of this intervention. We present the case of a woman on chronic hemodialysis who presented tertiary hyperparathyroidism due to parathyroid hyperplasia confirmed by Tc-99m MIBI SPECT.

OBJECTIVE: The purpose of this study was to evaluate the effects of electrical muscle stimulation (EMS) on muscles, using 99mTc-sestamibi SPECT/CT. MATERIALS AND METHODS: We prospectively enrolled 20 consecutive male professional water polo players. The mean age was 25 years (range, 18-36 years). All athletes underwent 99mTc-sestamibi SPECT/CT of the thigh (rectus femoris and vastus medialis muscle groups) before and after EMS. Images were quantified to identify increases in perfusion after EMS. RESULTS: Before EMS, there were no significant differences between the right and left thigh (rectus femoris and vastus medialis muscles) in terms of perfusion (p = 0.4). However, the comparison between the pre- and post-EMS analyses of the same muscle groups showed significant differences in radiotracer uptake (p < 0.001), with a mean increase in perfusion of 128% for the rectus femoris muscle group (95% CI: 0.86-1.61) and 118% for the vastus medialis muscle group (95% CI: 0.96-1.79). CONCLUSION: 99mTc-sestamibi SPECT/CT is an objective means of evaluating blood flow in muscles submitted to EMS, which appears to promote significant increases in such blood flow.


OBJETIVO: O objetivo deste estudo foi avaliar o efeito da estimulação elétrica transcutânea na musculatura esquelética utilizando-se SPECT/CT com 99mTc- sestamibi. MATERIAIS E MÉTODOS: Foram prospectivamente avaliados 20 atletas masculinos de polo aquático (18-36 anos de idade; média de 25 anos). Todos os atletas foram submetidos a SPECT/CT com 99mTc- sestamibi das coxas antes e após estimulação elétrica transcutânea dos grupos musculares reto femoral e vasto medial. As imagens foram quantificadas para identificar aumento da perfusão. RESULTADOS: A comparação da perfusão dos grupos musculares (reto femoral e vasto medial) direito versus esquerdo durante o repouso não demonstrou diferença significativa (p = 0,4). Entretanto, quando comparados esses mesmos grupos musculares antes e após a estimulação elétrica, observou-se diferença significativa na captação do traçador (p < 0,001), com aumento perfusional médio de 128% do grupo reto femoral (IC 95%: 0,86-1,61) e 118% do grupo vasto medial (IC 95%: 0,96-1,79) no grupo estimulado. CONCLUSÃO: A SPECT/CT com 99mTc- sestamibi é capaz de demonstrar de forma objetiva que a estimulação elétrica transcutânea aumenta significativamente o fluxo sanguíneo muscular.

Radiopharmaceuticals are radioactive compounds that can be used for diagnostic and therapeutic purposes. Technetium (99mTc) Sestamibi is the most commercialized radiopharmaceutical in the world. It includes a coordination complex consisting of the radioisotope 99 m technetium bound to six copper tetramibi tetrafluorborate ligands, and is mainly used to image the myocardium via scintigraphy. As radiopharmaceuticals are regarded as drugs, they are subject to the same regulations; therefore, the objective of this study was to develop a quantification method for the active pharmaceutical ingredient before their complexation with the radioisotope by employing high-performance liquid chromatography (HPLC) methodology. A simple and efficient method (retention time = 2.5 min) was developed and validated for copper tetramibi tetrafluorborate in the final product using a buffer and organic solvent mixtures (ACN-methanol-ammonium sulfate buffer) and a C18 column. The analytical protocol was fast, taking around 30 min until evaluation of results. The validation parameters were evaluated with satisfactory results: in terms of linearity r > 0.99 (160-240 µg/mL) and no deviation was observed. The RSD of precision was <5%, and an average recovery of 99% was observed for accuracy. The proposed method was thus considered adequate for routine analysis in the pharmaceutical industries.

Background: 99mTc-sestamibi parathyroid SPECT scintigraphy is a useful tool in the pre-operative study of hyperparathyroidism. False negatives (FN) have been reported in 5.7-14% of the examinations. Aim: To characterize 99mTc-sestamibi FN in cases referred for primary hyperparathyroidism (PHP) to a university hospital. Material and Methods: Descriptive retrospective analysis. We included patients with PHP, studied with SPECT scintigraphy, operated at our center between 2008 and 2015. Clinical and surgical data were recorded; biopsies of the FN were blindly reviewed by one pathologist. Results: One hundred twenty one scintigraphies fulfilled the inclusion criteria. Seven (5.8%) were negative and 114 positive. There was no difference in age, sex and PTH levels between FN and true positive scintigraphies. At surgery, one FN case had two hyperplasic glands and two cases had ectopic glands. Pathology reported adenoma in three cases, hyperplasia in three and carcinoma in one. The largest diameter of the lesion was lower in FN (1.3 and 2.1 cm respectively, p = 0.02) and the proportion of adenomas was higher in true positive cases (29% and 75% respectively; p < 0.01). The interval between scintigraphy and parathyroidectomy was greater in FN with a median of 92 days (range 20 days-3.2 years, p < 0.01). The percentage of oxyphilic cells observed was similar in both groups. Conclusions: FN parathyroid SPECT scintigraphies in PHP are uncommon. They corresponded to lesions under the equipment's resolution limit and resulted in longer time lags between scintigraphy and surgery.

Particularly Doxorubicin has been proven to be effective in the treatment of many soft and solid tumors for locally advanced and metastatic cancer. It is not easy to clinically evaluate the chemotoxic or chemoprotective effect of some drugs, even more when there is a subclinical toxicity. OBJECTIVE: To determine the usefulness of the hepatobiliary, colloid and cardiac scintigraphies, employing99mTcdisida, 99mTc-phytate and 99mTc-sestamibi respectively, in the evaluation of the hepato and cardiotoxicity of two chemotherapeutic treatments assessed in rats. METHOD: Two groups were submitted to doxorubicin (DOX) treatment and one was co-administered with histamine (DOX+HIS). Static 99mTc-phytate and 99mTc-sestamibi scintigraphies as well as a dynamic 99mTc-disida study were performed in a small field of view gamma camera at: 0 weeks (control), 1 week and 2 weeks of treatment. Imagenological parameters were calculated: Liver/Bone Marrow ratio (L/BM), Heart/Background ratio (H/B) and time to the maximum (Tmax) for 99mTc-phytate, 99mTc-sestamibi and 99mTc-disida extraction, respectively. RESULTS: Control (L/BM= 98±3; H/B=2.3±0.4; Tmax=8±3), DOX (L/BM: 85±3, 80±3; H/B, 3.5±0.5, 3.3±0.5 and Tmax 6±1, 4±1) for 1 and 2 weeks respectively and DOX+HIS (L/BM: 99±0.3, 98±1; H/B 2.9±0.5, 2.9±0.5 and Tmax, 8±2, 9±2) for 1 and 2 weeks, respectively. Histological analysis showed cardio and hepatotoxicity induced by doxorubicin. CONCLUSION: Imagenological parameters showed differences among treated and control groups and between both chemotherapy treatments. Thus, these radiopharmaceutical functional approaches were able to reflect heart and liver toxicity produced by doxorubicin.

Background: The evaluation of coronary artery disease (CAD) can be performed with stress test and myocardial SPECT tomography. Aim: To assess the predictive value of myocardial SPECT using stress test for cardiovascular events in patients with good exercise capacity. Material and Methods: We included 102 males aged 56 ± 10 years and 19 females aged 52 ± 10 years, all able to achieve 10 METs and ≥ 85% of the theoretical maximum heart rate and at least 8 min in their stress test with gated 99mTc-sestamibi SPECT. Eighty two percent of patients were followed clinically for 33 ± 17 months. Results: Sixty seven percent of patients were studied for CAD screening and the rest for known disease assessment. Treadmill stress test was negative in 75.4%; 37% of patients with moderate to severe Duke Score presented ischemia. Normal myocardial perfusion SPECT was observed in 70.2%. Reversible defects appeared in 24.8% of cases, which were of moderate or severe degree (> 10% left ventricular extension) in 56.6%. Only seven cases had coronary events after the SPECT. Two major (myocardial infarction and emergency coronary revascularization) and 5 minor events (elective revascularization) ere observed in the follow-up. In a multivariate analysis, SPECT ischemia was the only statistically significant parameter that increased the probability of having a major or minor event. Conclusions: Nearly a quarter of our patients with good exercise capacity demonstrated reversible defects in their myocardial perfusion SPECT. In the intermediate-term follow-up, a low rate of cardiac events was observed, being the isotopic ischemia the only significant predictive parameter.

PURPOSE: To evaluate the heart and the Tc-99m-sestamibi biodistribution after statin pretreatment in a rat model of abdominal sepsis. METHODS: Twenty-four Wistar rats were randomly distributed into four groups (n=6 per group): 1) sepsis with simvastatin treatment, 2) sepsis with vehicle, 3) sham control with simvastatin and 4) sham control with vehicle. 24 hours after cecal ligation and puncture rats received 1.0MBq of Tc-99m-sestamibi i.v. 30min after, animals were euthanized for ex-vivo tissue counting and myocardium histological analysis. RESULTS: Myocardial histologic alterations were not detected 24 hours post-sepsis. There was significantly increased cardiac Tc-99m-sestamibi activity in the sepsis group with simvastatin treatment (1.9±0.3%ID/g, p<0.001) in comparison to the sepsis group+vehicle (1.0±0.2%ID/g), control sham group+ simvastatin (1.2±0.3%ID/g) and control sham group (1.3±0.2%ID/g). Significant Tc-99m-sestamibi activity in liver, kidney and lungs was also detected in the sepsis group treated with simvastatinin comparison to the other groups. CONCLUSIONS: Statin treatment altered the biodistribution of Tc-99m-sestamibi with increased cardiac and solid organ activity in rats with abdominal sepsis, while no impact on controls. Increased myocardial tracer activity may be a result of a possible protection effect due to increased tissue perfusion mediated by statins. .

Objective: despite the technologic advances in myocardial perfusion imaging, we keep using an uncomfortable and sometimes impracticable patient position - supine with arms raised above the head (U). The purpose of this study was to investigate whether perfusion and functional cardiac gated SPECT scan results of acquisition U are equivalent to another position modality: supine with arms down at the sides of the trunk (D). Methods and Results: we performed U acquisition and in sequence D acquisition in 120 patients (pts) using a one-day MPI (rest-gated/stress), with 99mTc-sestamibi (370 MBq and 1110 MBq). Images were processed by the iterative reconstruction method (OSEM). Rest (R) and stress (S) studies were scored using 17-segments model. Functional parameters (left ventricular ejection fraction, and volumes) were automatically obtained by the quantitative gated SPECT (QGS) program. According to the degree of stress defects observed in U study, the patients were categorized in two subgroups: normal (SSS ≤ 3 or < 5%) and abnormal (SSS>3 or ≥ 5%). Shoulder/back pain occurred in 23.3% of U patients and in 5% of D. No significant differences between U and D were found for SSS (p = 0.82) and SRS (p = 0.74) in normal group. In abnormal group, good correlation was found between U and D modes for SSS (Rho = 0.95, p = 0.0001) and SRS (Rho = 0.96 p = 0.0001), but the mean SSS (12.53 ± 7.54) and SRS (10.60 ± 7.08) values of D were significantly lower (p < 0.05) than SSS (13.43 ± 6.81) and SRS (11.33 ± 6.97) of U mode. Function measurements presented good correlations, except for end-diastolic volume (p = 0.0001). Conclusion: although D mode appears to be more comfortable and presented a good correlation with U values of SSS and SRS, in abnormal pts, the extent and severity of defects can be underestimated. Objetivo: no estudo de perfusão miocárdica, apesar dos avanços tecnológicos, continuamos utilizando o desconfortável e às vezes impraticável posicionamento em decúbito dorsal com braços acima da cabeça (U). O objetivo do estudo foi investigar se os resultados da perfusão e da função cardíacas obtidos com a aquisição U são equivalentes ao de outra modalidade de posição: com braços para baixo nas laterais do tronco (D). Métodos e resultados: adquirida a aquisição U, na sequência foi realizada a aquisição D em 120 pacientes (pts) em um único dia (repouso e estresse sincronizado ao ECG), com 99mTc-sestamibi (370 MBq e 1110 MBq). As imagens foram processadas usando reconstrução iterativa (OSEM). Cada estudo foi quantificado usando o modelo de 17-segmentos ao repouso (R) e ao estresse (S). Os parâmetros funcionais (fração de ejeção do ventrículo esquerdo e volumes) foram obtidas automaticamente pelo programa gated SPECT quantitativa (QGS). De acordo com a dimensão da alteração perfusional no estresse na aquisição U, os pacientes foram classificados em dois subgrupos: 80 pts normais (SSS ≤ 3 ou < 5%) e 40 pts anormais (SSS > 3 ou ≥ 5%). Dor no ombro e/ou nas costas ocorreu em 23,3% dos pacientes de U e em 5% de D. Não houve diferenças significativas entre U e D quanto ao SSS (p = 0,82) e SRS (p = 0,74) no grupo normal. No grupo anormal, boa correlação foi encontrada entre U e D para SSS (Rho = 0,95, p = 0,0001) e SRS (Rho = 0,96 p = 0,0001), entretanto a média dos valores de SSS (12,53 ± 7,54) e SRS (10,60 ± 7,08 ) de D foram significativamente menores (p < 0,05) que o SSS (13,43 ± 6,81) e SRS (11,33 ± 6,97) de U. PURPOSE: The present a novel adenocarcinoma model in athymic mice. METHODS: Seven athymic mice were used. Colon diversion and distal fistula were made. Adenocarcinoma cells were inoculated in the submucosa of fistula. Tumor growth was monitored daily. Scintigraphy with 99mTc-MIBI was performed to identify the tumor. RESULTS: The model of distal colon cancer is feasible. Tumor detection was possible by both, macroscopically and molecular imaging. All resections demonstrated poorly differentiated tumors. Colon obstruction occurred in one case, similarly to evolution in human tumors of distal colon. CONCLUSION: The proposed model of distal colon cancer is feasible, allows for easy monitoring of tumoral growth by both, macroscopically and molecular imaging, and is suitable for studying the evolution of tumor with implementation of cytotoxic therapy in vivo.

Introducción. Frecuentemente los estudios de perfusión miocárdica con dipiridamol (DIP) son analizados cuantitativamente usando bases de datos de ejercicio (EXE). Objetivo. Comparar un grupo de pacientes con enfermedad coronaria conocida, procesados con bases de datos de DIP y EXE. Métodos. Se analizó los SPECT de perfusión miocárdica con Tc99m-Sestamibi y DIP de 20 hombres y 19 mujeres. Luego de su procesamiento, las imágenes de DIP SPECT fueron comparadas con bases de datos de mapas polares de EXE y DIP. Se analizó la extensión (en pixeles) y la severidad (DS) de los defectos y de las zonas reversibles. Resultados. Hubo concordancia de 92 por ciento en las interpretaciones de los estudios que tenían necrosis, isquemia o ambos (kappa: 0,859). El tamaño de los defectos de estrés fue el mismo con ambas bases de datos tanto en hombres como en mujeres. Sin embargo, en hombres la reversibilidad fue mayor (156,4 +/- 107,7 versus 128,6 +/-113,6 píxeles; p=0,0251) y más severa (699,6 +/- 665,8 versus 486,6 +/- 551,9 SD; p=0,0003) con EXE. En mujeres la reversibilidad fue menor (92,8 +/- 110,3 versus 113 +/- 121,9 pixeles; p=0,0024) y menos severa (278,5 +/- 356,7 versus 363,8 +/-432,7 SD; p=0,0009) con EXE. Conclusiones. Los pacientes que se sometieron a DIP fueron clasificados correctamente como isquemia, necrosis o ambos usando bases de datos de mapas polares de EXE. No obstante, la base de datos EXE subestimó la extensión de los defectos en mujeres y la sobreestimó en hombres.

To compare a group of patients with known coronary artery disease, processed with DIP and EXE polar maps. Method. We studied 20 males and 19 females with DIP Tc99m-Sestamibi myocardial SPECT. After similar processing, the DIP SPECT images were read using both EXE and DIP polar map databases. We analyzed defect and reversibility extension (in pixels) and severity in standard deviations. Results. Agreement to classify patients as having necrosis, ischemia or both was 92 percent (kappa: 0.859). Stress defect size was similar with both databases in males and females. However, in males reversibility resulted larger (156.4 +/- 107.7 versus 128.6 +/- 113.6 pixels; p=0.0251) and more severe (699.6 +/- 665.8 versus 486.6 +/- 551.9 SD; p=0.0003) with EXE. In females, reversibility resulted smaller (92.8 +/- 110.3 versus 113.0 +/- 121.9 pixels; p=0.0024) and less severe (278.5 +/- 356.7 versus 363.8 +/- 432.7 SD; p=0,0009) with EXE. Conclusion. Patients undergoing DIP stress were correctly classified as presenting ischemia, necrosis or both by EXE polar map database. However, EXE database underestimated ischemia extension in females and overestimated it in males.
