BACKGROUND: Focusing on the Meta region in Colombia, we investigated the relationship between mental health, the COVID-19 pandemic, and social determinants of health influenced by over five decades of civil conflict. We studied the post-2016 peace agreement trends in mental health for the population of Meta, before and after the local onset of the pandemic. METHOD: We conducted three rounds of a longitudinal health survey in years 2018 with N = 1309 (Women = 709; Men = 600); 2019 with N = 1106 (Women = 597; Men = 509); and 2020 with N = 905 (Women = 499; Men = 406). We measured mental health through the Self-Report Questionnaire (SRQ-20), investigating population trends in the average SRQ score and SRQ-positive frequency (SRQ + , indicating positive tendency towards experiencing mental health disorders). RESULTS: Between 2018 and 2020, there were reductions in the mean SRQ-20 score by 1.74 points (95% CI -2.30 to -1.18) and in SRQ + frequency by 15 percentage points (95% CI -21.0 to -9.0) for the Meta population. Yet specific subgroups have become more vulnerable to mental illness during the pandemic, for example older age groups (e.g., increase in mean SRQ score among over 60 s by 2.49 points, 95% CI 0.51 to 4.46) and people living with children younger than five years-old (e.g., increase in mean SRQ score by 0.64 points, 95% CI 0.07 to 1.20). Increased mental health vulnerability among specific subgroups may be related to differences in the likelihood of knowing people who tested positive for COVID-19 or died from itf having been in quarantine. CONCLUSION: Our findings support the importance of public policies in Colombia (and other low- and middle-income countries) that address the social determinants of mental illness whose influence was likely exacerbated by the pandemic, including persistent job insecurity leading to work and financial pressures, and inadequate support networks for isolated individuals and vulnerable caregivers.

BACKGROUND AND OBJECTIVES: Colombia is one of the countries with the highest levels of internal displacement resulting from armed conflict. This population has greater chances of experiencing a mental health disorder, especially in territories historically affected by armed conflict. Our objective was to compare the levels of possible mental health disorder in people experiencing internal displacement in Meta, Colombia, a department historically affected by armed conflict, compared to the internally displaced population in the National Mental Health Survey of 2015. METHODS: Analysis of data collected in the National Mental Health Survey (ENSM) of 2015, study with representative data at national level and the Conflict, Peace and Health survey (CONPAS) of 2014, representative study of the degree of impact of the conflict on the municipality, conducted in the department of Meta, Colombia. To measure possible mental health disorder, the Self-Report Questionnaire-25 (SRQ-25) was used. Internal displacement is self-reported by people surveyed in both studies. An exploratory analysis is used to measure possible mental health disorders in the displaced population in the ENSM 2015 and CONPAS 2014. RESULTS: 1089 adults were surveyed in CONPAS 2014 and 10,870 adults were surveyed in the ENSM 2015. 42.9% (468) and 8.7% (943) of people reported being internally displaced in CONPAS 2014 and ENSM 2015, respectively. In both studies, internally displaced populations have greater chances of experiencing any mental health disorder compared to non-displaced populations. For CONPAS 2014, 21.8% (95%CI, 18.1-25.8) of this population had a possible mental health disorder (SRQ+) compared to 14.0% (95%CI, 11.8-16.3) in the ENSM 2015. Compared with the ENSM 2015, at the regional level (CONPAS 2014), displaced people had a greater chance of presenting depression by 12.4% (95%CI, 9.5-15.7) compared to 5.7% (95%CI, 4.3-7.4) in the ENSM 2015, anxiety in 21.4% (95%CI, 17.7-25.3) compared to 16.5% (95%CI, 14.2-19.1) in the ENSM 2015, and psychosomatic disorders in 52.4% (95%CI, 47.5-56.7) in CONPAS 2014 compared to 42.2% (95%CI, 39.0-45.4) in the ENSM 2015. At the national level (ENSM 2015), displaced people had greater possibilities of presenting, compared to the regional level, suicidal ideation in 11.9% (95%CI, 9.3-14.1) compared to 7.3% (95%CI, 5.0-10.0) in CONPAS 2014 and bipolar disorder in 56.5% (95%CI, 53.2-59.7) compared to 39.3% (95%CI, 34.8-43.9) in CONPAS 2014. CONCLUSIONS: The greater possibilities of displaced populations at the regional level of experiencing a mental health disorder, compared to this same population at the national level, may represent and indicate greater needs in mental health care services in territories affected by conflict. Therefore, and given the need to facilitate access to health services in mental health for populations especially affected by armed conflict, there is a need to design health care policies that facilitate the recovery of populations affected by war and, simultaneously, that reduce inequities and promote the fulfilment of one of the most important and, at the same time, least prioritised health objectives in international development: mental health.

RESUMEN Los trastornos mentales muestran alta prevalencia en personas que buscan atención por problemas físicos en establecimientos de salud y que no son apropiadamente identificados y tratados. El Cuestionario de Autorreporte (SRQ) ha sido diseñado para detectar posibles casos y tiene un amplio uso en el campo de la atención primaria. Objetivo: Establecer la capacidad predictiva de ítems del SRQ-27 en la identificación de trastornos depresivos y de ansiedad en usuarios de establecimientos de salud de Lima. Método: El estudio utilizó datos de la muestra de 10 885 personas del Estudio Epidemiológico de Salud Mental en establecimientos de salud de Lima Metropolitana 2015. Se analizaron las respuestas al SRQ-27 en relación a la presencia de ansiedad y depresión, establecida por la Mini Entrevista Neuropsiquiátrica, utilizando procedimientos estadísticos descriptivos, pruebas de asociación y análisis de la AUC-ROC de cada ítem. Resultados: Dos ítems referidos a "sentirse triste" y "llorar" fueron los de mayor poder predictivo para la presencia de depresión (AUC ROC 0,766 y 0,719) y el ítem referido a "sentirse nervioso o tenso" fue el de mayor poder para ansiedad (AUC ROC 0,695). Otros nueve ítems, comunes para ambos trastornos, mostraron también ser eficaces (AUC ROC >0,60) en su identificación. Conclusiones: Se identificó tres ítems del SRQ-27 con mayor poder predictivo para detectar depresión y ansiedad en general, en tanto que ítems con mayor poder predictivo son comunes para ambas condiciones.


SUMMARY Mental disorders have a high prevalence among people who seek care in health facilities for physical ailments without being properly identified and treated. The Self Report Questionnaire (SRQ) has been designed to detect possible cases and is widely used in the primary care field. Objective: To establish the predictive power of items of the SRQ-27 in the identification of depressive and anxiety disorders in users of health establishments in Lima. Method: The study utilized data of the sample of 10 885 people from the Epidemiological Study of Mental Health in health establishments of Metropolitan Lima 2015. The responses to the SRQ-27 and its relationship with the presence of anxiety and depression, established by the Mini Neuropsychiatric Interview, were analyzed using descriptive statistics, association tests, and AUC-ROC analysis of each item. Results: Two items referring to "feeling sad" and "crying" had the highest predictive power for the presence of depression (AUC ROC 0.766 and 0.719), whereas the item referring to "feeling nervous or tense" had the highest power for anxiety (AUC ROC 0.695). Nine other, common to both disorders, also showed to be effective (AUC ROC >0.60) in their identification. Conclusions: Three SRQ-27 items were identified as having the highest predictive power for the detection of depression and anxiety in general, while items with the highest predictive power are common for both conditions.

Introducción y objetivos: Colombia es uno de los países del mundo con mayor volumen de desplazamiento interno a causa de un conflicto armado interno. Esta población tiene mayores posibilidades de sufrir un trastorno de salud mental, sobre todo en territorios afectados históricamente por el conflicto. El objetivo es comparar la prevalencia de posibles trastornos de la salud mental entre las personas en condición de desplazamiento en Meta, departamento de Colombia históricamente afectado por el conflicto armado, frente a población desplazada en todo el país según la Encuesta Nacional de Salud Mental (ENSM) de 2015. Métodos: Análisis de datos recolectados en la ENSM 2015, estudio a escala nacional, y la encuesta Conflicto, Salud y Paz (CONPAS) de 2014, estudio representativo del grado de afectación por el conflicto en el municipio, realizado en el departamento del Meta. Para medir un posible trastorno de la salud mental, se utiliza el Self Report Questionnaire-25 (SRQ-25). La condición de desplazamiento fue declarada por los encuestados en ambos estudios. Se hizo un análisis descriptivo sobre el posible trastorno de la salud mental en la población desplazada de la ENSM 2015 y la CONPAS 2014. Resultados: Se encuestó a 1.089 adultos en la CONPAS 2014 y 10.870 adultos en la ENSM 2015. El 42,9% (468) y el 8,7% (943) de las personas reportaron estar en condición de desplazamiento en la CONPAS 2014 y la ENSM 2015 respectivamente. En ambos estudios, la población desplazada tiene mayores posibilidades de sufrir cualquier trastorno de la salud mental que la población no desplazada. En la CONPAS 2014, el 21,8% (intervalo de confianza del 95% [IC95%], 18,1-25,8) de esta población tenía un posible trastorno de la salud mental (SRQ+) frente al 14,0% (IC95%, 11,8-16,3) en la ENSM 2015. Los encuestados en condición de desplazamiento de la CONPAS 2014 tuvieron mayor probabilidad que los de la ENSM 2015 en depresión -el 12,4% (IC95%, 9,5-15,7) frente al 5,7% (IC95%, 4,3-7,4)-, ansiedad -el 21,4% (IC95%, 17,7-25,3) frente al 16,5% (IC95%, 14,2-19,1)-y trastornos psicosomáticos -el 52,4% (IC95%, 47,5-56,7) frente al 42,2% (IC95%, 39,0-45,4)-. Los desplazados de la ENSM 2015 tenían mayor probabilidad de ideación suicida, el 11,9% (IC95%, 9,3-14,1) frente al 7,3% (IC95%, 5,0-10,0) en la CONPAS 2014, y trastorno bipolar, el 56,5% (IC95%, 53,2-59,7) frente al 39,3% (IC95%, 34,8-43,9). Conclusiones: La mayor probabilidad de trastornos de la salud mental (SRQ+) de la población regional en condición de desplazamiento frente a toda la población nacional en esa condición puede representar una mayor necesidad de servicios de atención en salud mental en los territorios afectados por el conflicto. Así pues, y dada la necesidad de facilitar el acceso y la atención médica en salud mental a poblaciones especialmente afectadas por el conflicto armado, es importante el diseño de políticas de atención en salud que faciliten la recuperación de poblaciones afectadas por la guerra y, simultáneamente, reducir inequidades y promover el cumplimiento de uno de los objetivos en salud más importantes y, a la vez, usualmente menos priorizados en el desarrollo internacional: la salud mental. © 2021 Asociación Colombiana de Psiquiatría. Resumen Los profesionales de la salud de las Unidades de Cuidados Intensivos (UCI) enfrentan situaciones de sufrimiento humano, competitividad y demanda, que podrían perjudicar su calidad de vida y su salud mental. El objetivo del estudio fue describir los trastornos mentales comunes y la calidad de vida relacionada con la salud en profesionales de la salud de la UCI en Salvador, Brasil. Este estudio transversal con 195 profesionales utilizó un cuestionario sociodemográfico y laboral, el Self-Reporting Questionnaire (SRQ-20) de trastornos mentales comunes y el 36-Item Short Form Health Survey (SF-36v2) de calidad de vida relacionada con la salud. El 29.7 % de los profesionales presentaron trastornos mentales comunes, especialmente entre profesionales de enfermería (RP = 2.28; IC 1.19-4.39; p = .007). La calidad de vida relacionada con la salud para todos los profesionales estuvo disminuida, principalmente en función social (44.25 ± 10.15) y rol emocional (45.86 ± 10.58). El SRQ-20 correlacionó fuertemente con los dominios dolor corporal (r = -.502), salud general (r = -.526), vitalidad (r = -.656), función social (r = -.608), salud mental (r = -.631) y el componente de salud mental (r = -.638) del SF-36v2 (p < .01). Los profesionales con trastornos mentales comunes mostraron una calidad de vida relacionada con la salud más deteriorada, esencialmente en dominios del componente de salud mental, y refirieron también dolor corporal. Es necesario discutir e implementar estrategias de evaluación, prevención y promoción de la salud mental entre los profesionales de las UCI para que sean consideradas dentro de las políticas de salud laboral.

OBJECTIVE: We study how life course objective socioeconomic position (SEP) predicts subjective social status (SSS) and the extent to which SSS mediates the association of objective SEP with nutritional status and mental health outcomes. METHODS: We use data from participants of the INCAP Longitudinal Study 1969-2018 (n = 1258) from Guatemala. We use the MacArthur ladder for two measures of SSS - perceived community respect and perceived economic status. We estimate the association of SSS with health outcomes after adjusting for early life characteristics and life course objective SEP (wealth, schooling, employment) using linear regression. We use path analysis to study the extent of mediation by SSS on the health outcomes of body mass index (BMI; kg/m2), psychological distress (using the WHO Self-Reported Questionnaire; SRQ-20) and happiness, using the Subjective Happiness Scale (SHS). RESULTS: Median participant rating was 5 [IQR: 3-8] for the perceived community respect and 3 [IQR: 1-5] for the perceived economic status, with no differences by sex. Objective SEP in early life and adulthood were predictive of both measures of SSS in middle adulthood as well as health outcomes (BMI, SRQ-20 and SHS). Perceived community respect (z-scores; 1 z = 3.1 units) was positively associated with happiness (0.13, 95 % CI: 0.07, 0.19). Perceived economic status (z-scores; 1 z = 2.3 units) was inversely associated with psychological distress (-0.28, 95 % CI: -0.47, -0.09). Neither measure of SSS was associated with BMI. Neither perceived community respect nor perceived economic status attenuated associations of objective SEP with health outcomes on inclusion as a mediator. CONCLUSIONS: Subjective social status was independently associated with happiness and psychological distress in middle adulthood after adjusting for objective SEP. Moreover, association of objective SEP with health was not mediated by SSS, suggesting potentially independent pathways.

BACKGROUND: Wealth mobility, as both relative (positional) and absolute (material) wealth acquisition, may counteract negative consequences of early life adversities on adult health. METHODS: We use longitudinal data (1967-2018) from the INCAP birth cohort, Guatemala (n = 1386). Using wealth as a measure of socio-economic position, we assess the association of life course relative mobility using latent class analysis and absolute material gains using conditional wealth measures. We estimate associations of wealth mobility with indicators of human capital, specifically height, weight status (BMI in kg/m2), psychological distress (WHO SRQ-20 score) and fluid intelligence (Ravens Progressive Matrices score; RPM) in middle adulthood. RESULTS: We identified four latent classes of relative mobility - Stable Low (n = 498), Stable High (n = 223), Downwardly Mobile (n = 201) and Upwardly Mobile (n = 464). Attained schooling (years) was positively associated with membership in Upwardly Mobile (odds ratio; 1.50, 95%CI: 1.31, 1.71) vs Stable Low, and inversely with membership in Downwardly Mobile (0.65, 95%CI: 0.54, 0.79) vs Stable High. Being Upwardly Mobile (vs Stable Low) was positively associated with height (1.88 cm, 95%CI: 1.04, 2.72), relative weight (1.32 kg/m2, 95%CI: 0.57, 2.07), lower psychological distress (-0.82 units, 95%CI: 1.34, -0.29) and fluid intelligence (0.94 units, 95%CI: 0.28, 1.59). Being Downwardly Mobile (vs Stable High) was associated with lower fluid intelligence (-2.69 units, 95%CI: 3.69, -1.68), and higher psychological distress (1.15 units, 95%CI: 0.34, 1.95). Absolute wealth gains (z-scores) from early to middle adulthood were positively associated with relative weight (0.62 kg/m2, 95%CI: 0.28, 0.96), lower psychological distress (-0.37 units, 95%CI: 0.60, -0.14) and fluid intelligence (0.50 units, 95%CI: 0.21, 0.79). CONCLUSIONS: Higher attained schooling provided a pathway for upward relative mobility and higher absolute wealth gains as well as protection against downward relative mobility. Upward mobility was associated with lower psychological distress and higher fluid intelligence but also higher weight status.

Resumo Este estudo buscou identificar a prevalência e os fatores associados à manifestação de Transtornos Psiquiátricos Menores entre 537 cuidadores familiares de pessoas em sofrimento psíquico atendidas em 16 Centros de Atenção Psicossocial (CAPS) localizados na 21ª região de saúde do estado do Rio Grande do Sul. Para análise, munindo-se de um modelo teórico hierarquizado, utilizou-se regressão de Poisson com o cálculo das razões de prevalência ajustadas. A prevalência de Transtornos Psiquiátricos Menores encontrada na população estudada foi de 42,1% (N = 226 IC: 38% - 46,3%). Os fatores associados a esses transtornos foram: sexo feminino (RP: 1,54); vínculo próximo com o usuário, em especial pais/mães (RP: 2,00); baixa escolaridade, apresentando uma razão de prevalência de RP: 1,85 no estrato mais baixo; possuir problemas de saúde (RP: 1,24); referir problemas de nervos (RP: 3,02); baixo desempenho de avaliação da qualidade de vida nos âmbitos físico (RP:1,84) e de meio ambiente (RP:1,95); insatisfação com as relações familiares (RP: 1,56); falta de apoio familiar (RP: 1,25) e sentimento de sobrecarga, para qual foi encontrado uma razão de prevalência de RP: 2,61 entre os indivíduos com maior nível de sobrecarga.


Abstract This study aimed to identify the prevalence of the manifestation of Minor Psychiatric Disorders and its associated factors among 537 family caregivers of people living with psychological distress attended at 16 Psychosocial Care Centers (CAPS) located in the 21st health region of the state of Rio Grande do Sul. Considering a hierarchical theoretical model, the analysis adopted a Poisson regression to calculate the adjusted prevalence ratios. The prevalence of Minor Psychiatric Disorders found in the studied population was 42.1% (N = 226 CI: 38%-46,3%). Factors associated with these disorders were being female (PR: 1.54), having a close relationship with the user, especially fathers/mothers (PR: 2.00), low schooling, within a prevalence ratio of PR: 1.85 in the lowest stratum, presence of health problems (PR: 1.24); reporting neurological problems (PR: 3.02), low performance in the evaluation of the quality of life in the physical (PR: 1.84) and environmental (PR: 1.95) aspects, dissatisfaction with family relationships (PR: 1.56), lack of family support (PR: 1.25) and feeling of burden, with a prevalence ratio of PR: 2.61 among individuals with higher level of burden.

Nesta tese, objetivou-se caracterizar o perfil, os fatores associados à suspeição de Transtornos Mentais Comuns (TMC) e a autopercepção da qualidade de vida (QV) nos estudantes do curso de Medicina da Universidade Federal da Integração Latino-Americana (UNILA). Foi realizado um estudo analítico transversal com 202 alunos brasileiros e estrangeiros latinoamericanos. O questionário Perfil do Participante foi validado através do índice de validade de conteúdo (IVC) e aplicado nos alunos brasileiros e estrangeiros. Foram aplicados nos estudantes brasileiros o Self-Reporting Questionnaire-20 (SRQ-20) e o World Health Organization Questionnaire for Quality of Life - bref (WHOQoL-BREF) para avaliação da suspeição de TMC e da QV, respectivamente. Foram observadas diferenças estatisticamente significativas entre os grupos de alunos brasileiros e estrangeiros. As médias de idade (p = 0,006), da renda familiar (p = 0,002) e de horas por semana de atividade física (p < 0,0001) eram maiores nos brasileiros, assim como, foi encontrado menor número de pessoas que viviam com a renda familiar média (p = 0,002) e maior porcentagem de alunos que consideraram o autodesempenho acadêmico bom/excelente (p = 0,032) neste grupo. A condição de estar solteiro (p < 0,0001), ter religião (p = 0,007), nunca ter feito tratamento psiquiátrico/psicológico (p < 0,0001), sexo feminino (p = 0,041) e menor média da carga horária de sono diária (p < 0,0001) foram observadas em maior porcentagem nos estrangeiros. A prevalência de suspeição de TMC no grupo de estudantes brasileiros foi de 40% com IC95%: [0,31 ; 0,49]. Através da análise pela regressão logística, os domínios psicológico (p < 0,0001) e físico (p = 0,028) do WHOQoL-BREF, o período do curso (p = 0,039), a alimentação (p = 0,009) e achar o curso estressante (p = 0,034) foram associados à suspeição de presença de TMC no grupo de brasileiros. A partir dos resultados encontrados nesse trabalho, verifica-se a necessidade de pesquisas de causalidade de suspeição de TMC e de intensificar os conhecimentos, ampliar a prática de condutas no dia a dia e planejar ações a favor da saúde mental e saúde física do futuro médico


This thesis aimed to characterize the profile, the associated factors with suspicion of Common Mental Disorders (CMD) and the self-perception of quality of life (QoL) in Medicine students at the Federal University of Latin American Integration (UNILA). A cross-sectional analytical study was conducted with 202 Brazilian and foreign (latin american) students. The Participant Profile questionnaire was validated through the content validity index (CVI) and applied to Brazilian and foreign students. Self-Reporting Questionnaire-20 (SRQ-20) and World Health Organization Questionnaire for Quality of Life - bref (WHOQoL-BREF) were applied to Brazilian students to evaluate suspicion of CMD and QoL, respectively. Statistically significant differences were observed between groups of Brazilian and foreign students. The average age (p = 0.006), family income (p = 0.002) and hours per week of physical activity (p < 0.0001) were higher in Brazilians, and a lower number of people living with average family income (p = 0.002) and higher percentage of students who considered good / excellent academic self-performance (p = 0.032) in this group. The condition of being single (p < 0.0001), having religion (p = 0.007), never having received psychiatric / psychological treatment (p < 0.0001), female gender (p = 0.041) and lower average daily load of sleep (p < 0.0001) were observed in higher percentage in foreign students. The prevalence of suspected CMD in Brazilian students was 40% with 95% CI: [0.31 ; 0.49]. Through logistic regression analysis, the psychological (p < 0.0001) and physical (p = 0.028) domains of the WHOQoLBREF, the course time (p = 0.039), the diet (p = 0.009) and finding the course stressful (p = 0.034) were associated with suspected presence of CMD in the group of Brazilians students. From the results found in this study, it is necessary to investigate causality of suspected CMD and to intensify knowledge, to expand the practice of daily conduct and to plan actions and to improve of mental health and physical health of the future Medicine doctor

Abstract This study aims to analyze the occurrence of minor psychiatric disorder and their associations in relatives of people with mental disorders. This is a cross-sectional study of 1164 relatives. For the tracking of minor psychiatric disorders the Self-Reporting Questionnaire Scale (SRQ20) was used, adopting 6/8 as cut-off point. Bivariate analyzes were conducted using Chi-squared test. Trends among strata of independent variables were investigated in relation to the outcome using nonparametric linear trend test. Statistic significance was defined as p-value < 0.05. Crude and adjusted binary logistic regressions were conducted using as a basis the hierarchical model developed through a systematic literature review. It was observed in the population a prevalence of 46.9% for minor psychiatric disorders. Higher prevalence of minor psychiatric disorders were strongly associated with the female gender, older age, first degree family ties, not having a paid work, lower education level, lower income, health problems, lower quality of life and feeling of burden. Many factors are related to the emotional and mental illness of family caregivers, demanding health services to be prepared to recognize and intervene in these situations.


Resumo Este estudo tem por objetivo analisar a ocorrência de transtornos psiquiátricos menores e suas associações em familiares de pessoas com transtornos mentais. Estudo transversal realizado com 1.164 familiares. Para o rastreamento utilizou-se o Self-Reporting Questionnaire Scale (SRQ20). Análises bivariadas foram realizadas com uso do teste Qui-quadrado. Tendências entre os estratos foram investigados em relação ao desfecho por meio do teste não paramétrico de tendência linear. Significância estatística foi definida como p < 0,05. Regressões logísticas binárias brutas e ajustadas foram realizadas utilizando como base um modelo hierárquico desenvolvido através de uma revisão sistemática da literatura. Observou-se prevalência de 46,9% de transtornos psiquiátricos menores. Maior prevalência desses transtornos esteve fortemente associada ao sexo feminino, idade avançada, laços familiares de primeiro grau, não possuir trabalho remunerado, nível de educação mais baixo, menor renda, problemas de saúde, baixa qualidade de vida e sentimento de sobrecarga. Muitos fatores estão associados à doença emocional e mental dos cuidadores familiares, exigindo que serviços de saúde estejam preparados para reconhecer e intervir nessas situações.

INTRODUCTION: Colombia has a large population exposed to violence. Our data suggest a significant number displaced by the conflict. As there is an increased risk of vulnerability, their problems and mental disorders need to be assessed in order to determine specific treatments. OBJECTIVES: To determine the prevalence of problems and mental disorders in those internally displaced by the conflict. METHODS: Data was obtained from the National Mental Health Survey 2015. The diagnostic tools used were the composite international diagnosis interview (CIDI-CAPI), Self-reporting questionnaire (SQR). Alcohol consumption was assessed with the Alcohol Use Disorders Identification test (AUDIT). A survey based on the Alcohol, Smoking and Substance Involvement Screening Test (ASSIST) was developed. The modified Post-traumatic Stress Disorder (PTSD) Checklist-Civilian version (PCL-C) was used to determine possible post-traumatic stress Disorder. Multidimensional poverty index (MPI) and Family-Apgar questionnaire were applied to general individual and household data. RESULTS: A total of 943 persons displaced by the conflict were reported, with self-report of symptoms in 16.4% (95% CI, 13.2-20.1). The prevalence of any of the measured mental disorders (CIDI-CAPI) ever in life was 15.9% (95% CI, 11.9-21.1), with a suicidal ideation of 12.5% (95%CI, 9.0-17.1), and excessive alcohol consumption in 10.1% (95% CI, 7.2-13.9). More than one-third (35.6%, (95% CI, 30.7-40.8) of people report having experienced, witnessed, or been told that someone close had had a traumatic event related to the armed conflict. An increased risk of PTSD is reported by 3.6% (95% CI, 2.2-5.9) displaced people that had reported at least one traumatic event. Family dysfunction in the displaced population is absent (74.8% (95%.CI, 70.4-78.8). CONCLUSIONS: The displaced population has a high prevalence of problems and mental disorders, which confirms their disadvantaged situation.

Resumen Introducción: Colombia tiene una gran población expuesta a la violencia, con una cifra importante de desplazados por esta, quienes se encuentran en mayor riesgo de vulnerabilidad, por lo que es necesario evaluar en ellos problemas y trastornos mentales para definir intervenciones específicas necesarias. Objetivos: Determinar la prevalencia de problemas y trastornos mentales de la población colombiana desplazada por el conflicto armado interno. Métodos: Datos emanados de la Encuesta Nacional de Salud Mental 2015. Se realizó la entrevista diagnóstica internacional compuesta (CIDI-CAPI) y el Cuestionario de Síntomas (SRQ) como indicador de posible enfermedad psiquiátrica; el consumo de alcohol se evaluó con el test de identificación de trastornos por consumo de alcohol (AUDIT); se diseñó una encuesta basada en la Prueba de Detección de Consumo de Alcohol, Tabaco y Sustancias (ASSIST); se modificó la Lista de Chequeo de Estrés Postraumático (PCL-C) para determinar posible estrés postraumático y se aplicó el Índice Multidimensional de Pobreza (IPM) y el APGAR familiar a los datos generales del individuo y del hogar. Resultados: Se detectó a 943 personas desplazadas por la violencia. Se presenta SQR positivo en el 16,4% (intervalo de confianza del 95% [IC95%], 13,2%-20,1%); la prevalencia de cualquiera de los trastornos mentales medidos (CIDI-CAPI) alguna vez en la vida es del 15,9% (IC95%, 11,9%-21,1%); la de ideación suicida es del 12,5% (IC95%, 9,0%-17,1%); la de consumo de alcohol excesivo, del 10,1% (IC95%, 7,2%-13,9%); el 35,6% (IC95%, 30,7%-40,8%) de las personas reportan que han vivido o presenciado o les han contado que alguien cercano ha tenido un evento traumático relacionado con el conflicto armado; se reporta un mayor riesgo de estrés postraumático en personas desplazadas que han reportado por lo menos un evento traumático (3,6%; IC95%, 2,2%-5,9%); la disfunción familiar en desplazados es ausente (74,8%; IC95%, 70,4%-78,8%). Conclusiones: La población desplazada tiene una alta prevalencia de problemas y trastornos mentales, lo que ratifica la situación de desventaja en que se encuentran

Resumo Introdução: à medida que se reconhece o papel dos agentes comunitários de saúde (ACS) para os objetivos do sistema de saúde, é evidente a preocupação com a prevalência de sintomas psíquicos entre esses trabalhadores, conclamando a identificação de indicadores passíveis de serem modificados em favor da saúde mental dos ACS. Objetivo: examinar associações entre a prevalência de transtornos mentais comuns (TMC) e condições de trabalho entre ACS inseridos nos serviços de atenção básica de Belo Horizonte, MG. Método: a amostra aleatória e representativa incluiu 196 sujeitos. Utilizou-se o Self-Reporting Questionnaire (SRQ-20) e questionário autopreenchível que continha blocos relacionados às questões sociodemográficas, ocupacionais e hábitos de vida. A análise multivariável adotou entrada hierárquica das variáveis e regressão de Poisson com estimativa da variância robusta. Resultados: a prevalência de TMC de 26,5% foi associada à alta demanda psicológica, relato de agressões contra o trabalhador e insatisfação com as relações pessoais. Conclusão: a significância da associação no tocante à demanda psicológica no trabalho é preocupante, pois seria possível projetar o trabalho dos ACS sem ultrapassar os seus próprios limites. Adequações do modelo organizacional e redesenho das tarefas podem contribuir favoravelmente para a saúde mental dos ACS.


Abstract Introduction: as the role of community health workers (CHW) is recognized to health system objectives it is clear the concern about prevalence of psychological symptoms among these workers, urging the identification of indicators that can be modified in favor of CHWs' mental health. Objective: to examine associations between the prevalence of common mental disorders (CMD) and work conditions among CHWs inserted into the primary health care of the city of Belo Horizonte, state of Minas Gerais, Brazil. Method: random and representative sample that included 196 individuals. Data was collected through a Self-Reporting Questionnaire (SRQ-20) and a self-administered questionnaire containing blocks related to sociodemographic questions, occupational and lifestyle habits. Multivariate analysis adopted hierarchical input of variables and Poisson regression with robust variance estimate. Results: the prevalence of CMD was 26.5% and it was associated with high job demands, reports of aggression against workers and dissatisfaction with personal relationships. Conclusion: the association of significance regarding job demands at work is worrisome because it would be possible to design CHWs' work without having them exceeding their limits. Adequacy of the organizational model and redesign of tasks can contribute positively to CHWs' mental health.

A demanda extra de cuidados é definida como sobrecarga e pode impactar a vida da família, considerando que a natureza crônica da doença mental submete o familiar ao efeito prolongado de eventos estressores envolvidos na experiência cotidiana de cuidar, o que pode afetar a sua própria saúde mental. Este estudo teve por objetivo avaliar a ocorrência de sobrecarga entre cuidadores de pessoas com transtorno mental em início de internação psiquiátrica e sua relação com a ocorrência de estresse e de sofrimento mental. Foi realizado um estudo epidemiológico, transversal, com duração de dois anos. Participaram do estudo 112 cuidadores, de ambos os sexos, maiores de 18 anos. Para coleta de dados foram utilizados: um questionário para coleta de dados sociodemográficos, de condições ocupacionais, de saúde e de conhecimento pelo cuidador a respeito da doença do paciente com transtorno mental; a Escala de Sobrecarga Zarit Burden Interview (ZBI); o Inventário de Sintomas de Stress para Adultos da LIPP (ISSL); e o Self-Reporting Questionnaire (SRQ 20). Utilizou-se estatísticas descritiva e analítica, com medidas de tendência central, testes Qui- Quadrado de Pearson, o Exato de Fisher e teste de Concordância Kappa, com coeficiente de correlação de Pearson e regressão logística. Foi considerado nível de significância de 0,05. Todos os aspectos éticos foram respeitados. Entre os cuidadores, houve predomínio do sexo feminino (82%), a maioria era casada ou possuía companheiro fixo (59%), e idade média de 49 anos. Os escores de sobrecarga entre os cuidadores variaram de 13 a 81 pontos, com média de 50,2. Evidenciou-se que apenas 3,6% destes cuidadores não apresentaram sobrecarga e que a maioria apresentava sobrecarga de moderada a severa; 79,5% apresentaram sintomas de estresse, sendo que a maioria estava na fase de estresse chamada de Resistência (52,7%) e com predomínio de sintomas psicológicos de estresse (66,1%); 67,9% dos cuidadores estavam em sofrimento mental. Houve associação com elevada significância estatística entre a sobrecarga destes cuidadores e a ocorrência de estresse e de sofrimento mental. Os sintomas psicológicos do estresse, como: preocupação, ansiedade e irritabilidade, foram fatores de risco para sobrecarga severa. Assim, os resultados deste estudo deflagram a alarmante situação dos cuidadores de pessoas com transtornos mentais em início de internação psiquiátrica, evidenciando sua situação de sobrecarga e vulnerabilidade ao adoecimento e ao sofrimento mental. Portanto, é evidente a necessidade de intervenções e estudos que visem ampliar e qualificar o cuidado a saúde destes familiares cuidadores


The extra demand for care is defined as burden and it can affect family life whereas mental illness chronic nature shall expose the family to the prolonged effect of stressful events involved in everyday experience of care, which can affect their own mental health. This study aimed to evaluate the burden occurrence among caregivers of patients in early psychiatric hospitalization and the relation with their stress and mental suffering. An epidemiological, cross-sectional study was conducted, lasting two years. A number of 112 family carers, older than 18 years old, participated in the study. For data collection, it was used: a questionnaire to collect sociodemographic data, occupational condition, health and knowledge by the caregiver about the mental disorder patient illness; Zarit Burden Interview (ZBI); Inventory of Stress Symptoms for Adults of Lipp (ISSL); and the Self-Reporting Questionnaire (SRQ 20). It was used descriptive and analytical statistics, performing chi-square tests of Pearson, Fisher's exact test and Kappa Agreement with Pearson's correlation coefficient and logistic regression, considering ,0.05 significance level. All ethical aspects were respected. There was a female predominance (82%), most of them were married (59%) and had an average age of 49 years old. The burden scores among carers ranged from 13 to 81 points, averaging 50.2. Only 3.6% of these caregivers did not show burden, and most of them had moderate to severe burden; 79.5% showed stress symptoms and most of them was in the phase of stress called resistance (52.7%), with a predominance of psychological symptoms of stress (66.1%); 67.9% of caregivers were in mental suffering. There was an association with high statistical significance between the burden of these caregivers and the occurrence of stress and mental suffering. Psychological symptoms of stress, such as worry, anxiety and irritability, were risk factors for severe burden. Thus, the results of this study trigger the alarming condition of the carers of people with mental disorders in early psychiatric hospitalization, indicating their vulnerability to illness and mental suffering. Therefore, it is clear the need for interventions and studies that aim to widen and improve the health care of these family caregivers

Este estudo objetivou verificar a prevalência de transtornos mentais comuns (TMC) entre os profissionais de saúde de um hospital de universitário. Trata-se de um estudo observacional, transversal com abordagem quantitativa, realizado com 359 profissionais de saúde, no Hospital de Clínicas da Universidade Federal do Triângulo Mineiro, Minas Gerais. A coleta de dados ocorreu entre os meses de junho a agosto de 2013. Os participantes responderam um instrumento contendo variáveis sociodemográficas e profissionais e outro para verificar a prevalência de TMC o Self Reporting Questionnaire. Entre os profissionais de saúde detectou-se uma prevalência geral de 27,9% para TMC. Os resultados obtidos neste estudo evidenciaram a presença de TMC, sendo necessário propor medidas para promover à saúde dos profissionais de saúde.


This study aimed at assessing the prevalence of common mental disorders (CMD) among health professionals of a teaching hospital. It is a quantitative cross-sectional observational study conducted with 359 health professionals, at Hospital de Clínicas da Universidade Federal do Triângulo Mineiro, Minas Gerais, Brazil. Data collection occurred from June to August, 2013. Participants answered both an instrument with professional and social and demographic variables and another one for assessing the prevalence of CMD - the Self Reporting Questionnaire. Among those health professionals, an overall 27.9% prevalence for CMDwas observed. Results identified CMD. It is therefore necessary to propose health promotion measures for health professionals.


Este estudio pretende verificar la prevalencia de trastornos mentales comunes (TMC) entre los profesionales de sanidad de un hospital universitario. Se trata de un estudio observacional, transversal con abordaje cuantitativo, realizado con 359 profesionales de sanidad, en el Hospital de Clínicas de la Universidade Federal de Triangulo Mineiro, Minas Gerais. La recolecta de datos se produjo entre los meses de julio y agosto de 2013. Los partícipes respondieron un instrumento que describe variables sociodemográficas y profesionales y otro para verificar la prevalencia del TMC o Self Reporting Questionnaire. Entre los profesionales de salud se detectó una prevalencia general de 27,9% para TMC. Los resultados obtenidos en este estudio destacaron la presencia de TMC, haciendo necesaria la propuesta de medidas de fomento de la salud de los profesionales sanitarios.

As características do trabalho na Estratégia Saúde da Família (ESF) são complexas e ainda há poucos estudos que abordam a temática. OBJETIVO: descrever características sociodemográficas e do trabalho e a prevalência de Distúrbios Psíquicos Menores (DPM) e de positivos ao teste CAGE (abuso no consumo de álcool) entre médicos, enfermeiros e cirurgiões-dentistas da ESF de Feira de Santana, Bahia. METODOLOGIA: estudo epidemiológico de corte transversal que utilizou questionário padronizado, não identificado, para descrever: o perfil sociodemográfico; as características do trabalho na ESF; a saúde mental e o uso abusivo de bebidas alcoólicas utilizando o Self Reporting Questionnaire (SRQ-20) e o Teste CAGE,. Os dados foram analisados no Statistical Package for Social Scince (SPSS). RESULTADOS: Entre os 152 respondentes, que correspondem a uma taxa de resposta de 86,4%, a prevalência de DPM foi de 16,0%. Entre os médicos, foi de 17,4%; entre os enfermeiros, de 15,5%; e 15,2% entre os cirurgiões-dentistas. Quanto ao uso abusivo de bebida alcoólica, os médicos apresentaram prevalência de 5,0% e os cirurgiões-dentistas de 7,7%. Os resultados apontaram o predomínio do sexo feminino, de adultos jovens, casados, com vínculo cooperativado, carga horária semanal elevada e mais de um vínculo empregatício. CONCLUSÃO: A prevalência de DPM e de positivos ao teste CAGE observada na população estudada foi menor do que a encontrada em outros estudos.


To date, few studies have looked into the complex aspects of work at the Family Health Strategy (FHS). OBJECTIVE: To describe sociodemographics, work aspects, Minor Psychiatric Disorders (MPD), and CAGE positivity (alcoholism) among physicians, nurses and dentists working with FHS in Feira de Santana (Bahia, Brazil). METHODS: Cross-sectional study using standard questionnaires (SRQ-20 and CAGE) to collect anonymous information on the sociodemographic profile, work aspects, mental health, and alcohol consumption. Data were analyzed with SPSS. RESULTS: Based on 152 respondees (response rate: 86.4%), the overall prevalence of MPD was 16.0% (physicians=17.4%, nurses=15.5%, dentists=15.2%). The prevalence of alcoholism was 5.0% among physicians and 7.7% among dentists. The characteristics "female", "young adult", "married", "affiliated to a cooperative", "long work week" and "more than one job" predominated in this population. CONCLUSION: the prevalence of MPD and CAGE positivity was smaller in our sample than in other similar studies.

INTRODUÇÃO: Os Transtornos Mentais Comuns (TMC) acometem mais frequentemente as mulheres. A baixa gratificação e a falta de visibilidade na realização do trabalho doméstico constituem quadro determinante para esse adoecimento psíquico. OBJETIVOS: Avaliar a associação entre sobrecarga doméstica e a ocorrência de transtornos mentais comuns em mulheres da zona urbana do município de Feira de Santana - BA. METODOLOGIA: Estudo epidemiológico de corte transversal incluindo 2.057 mulheres com 15 anos ou mais de idade, selecionadas através de amostragem aleatória por conglomerado. O indicador de sobrecarga doméstica foi criado a partir das atividades domésticas: lavar, passar, limpar e cozinhar, ponderadas pelo número de moradores do domicílio. Os transtornos mentais comuns foram avaliados através do SRQ-20. RESULTADOS: Mulheres com alta sobrecarga doméstica apresentaram prevalência de TMC mais elevada do que as mulheres com baixa sobrecarga: 45,6% contra 36,2%. A análise de regressão logística múltipla confirmou associação entre sobrecarga doméstica e TMC (RP: 1,23; IC95%: 1,05 - 1,44), ajustada pelas variáveis renda, escolaridade e atividades de lazer. CONCLUSÃO: Os achados sustentam a hipótese de que o trabalho doméstico, em elevada sobrecarga, está associado a transtornos mentais.


INTRODUCTION: Common mental disorders (CMD) are more frequent in women. Low reward and lack of visibility in performing housework are the determining factors for these psychological illnesses. OBJECTIVES: To evaluate the association between housework overload and the occurrence of common mental disorders in women living in the urban zone of the municipality of Feira de Santana, Bahia. METHODS: cross-sectional epidemiological study that included 2,057 women aged 15 years or over selected by random sampling in clusters. The housework overload indicator was created from the domestic activities of washing, ironing, cleaning and cooking, weighted according to the number of individuals living in the home. The common mental disorders were assessed using SRQ-20. RESULTS: Women with a high housework overload had a higher prevalence of CMD than women with a low overload (45.6% versus 36.2%). Multiple logistic regression analysis confirmed the association between housework overload and CMD (PR: 1.23; 95% CI: 1.05 - 1.44), adjusted for income, level of schooling and leisure activities. CONCLUSION: The findings support the hypothesis that high housework overload is associated with mental disorders.

As relações entre as condições de trabalho e de saúde dos trabalhadores têm-se destacado como elemento estratégico da luta dos trabalhadores em defesa de padrões mais qualificados e saudáveis de vida. O objetivo deste trabalho foi descrever as condições de trabalho e estimar a prevalência de suspeitos de Transtornos Mentais Comuns (TMC) e de casos positivos ao Teste CAGE (bebedores-problema) em profissionais de enfermagem de um Hospital Especializado de Feira de Santana, Bahia. Realizou-se um estudo epidemiológico de Corte Transversal, utilizando na coleta de dados um questionário autoaplicável, não identificado, com destaque para itens relativos a aspectos sociodemográficos, características do trabalho, hábitos de vida e suspeitos de TMC e de bebedores-problema (Self Reporting Questionnaire, SRQ-20; Teste CAGE). As informações foram processadas e analisadas utilizando-se o programa SPSS 9.0 da Sala de Situação e Análise Epidemiológica e Estatística (SSAEE) do Departamento de Saúde (DSAU) da Universidade Estadual de Feira de Santana (UEFS). Os profissionais de enfermagem referiram sobrecarga de trabalho e baixa remuneração no hospital estudado. As queixas de saúde mais frequentes estavam relacionadas à postura corporal e à saúde mental. A prevalência de suspeitos de TMC foi de 14,6 porcento. O teste CAGE triou 3,6 porcento de indivíduos como bebedores-problema. As condições de trabalho e saúde observadas apontam para a necessidade de mudanças na organização do trabalho de enfermagem.


The survey investigated the conditions work and descriptive was prevalence of CMD and positive from CAGE test among nursing of professionals from the Hospital Specialized in Feira de Santana City, Bahia, Brazil. The cross-sectional and descriptive study explores. The data collection was carried out using an unidentified self-administered questionnaire, with emphasis on items relating to demand-control-support situation and occurrence of CMD and positive from CAGE Test (Self Reporting Questionnaire, SRQ-20 and CAGE Test). The data and statistical analyses were stored using the software SPSS for Windows, 9.0 of Situation and Analyses Epidemiologic andStatistics Room from the Health Department of Feira de Santana State University. The nursing of professionals referred excessive work overload, multiple jobs, low income. The most frequent health complaints were related to body posture and to mental health. The overall prevalence of CMD in the professionals surveyed was 14.6 percent. The prevalence of positive from CAGE Test was 3,6percent.The observed work and health conditions point out the need of deep changes in the nursing work organization.


Estudio epidemiológico de la sección transversal, que trató de estimar la prevalência de sospecha de trastornos mentales comunes (TMC) y de cazos pozitivos al teste CAGE (bebedores-problema) y a posible asociación entre las características sociales y de trabajo em personal de enfermería en un hospital especializado, Feira de Santana, Bahia. Los datos fueron recolectados através de cuestionario auto-administrado, no identificado, con énfasis en lãs características sociales y de trabajo y la sospecha de TMC e bebedores-problema (Self Reporting Questionnaire/SRQ-20 y Teste CAGE). Los datos fueron procesados y analizados utilizando el paquete estadístico SPSS for Windows 9.0, de La Sala de Situación e Análize Epidemiologica e Estadistica de lo Departamento de Salud da Universidade Estadual de Feira de Santana (SSAEE/DSAU/UEFS). El personal de enfermería referiu elevada carga de trabajo e baja remuneración. Lãs queixas de salud mais frequentes fueram relacionadas la postura corporal y la salud mental. La prevalencia de sospecha de TMC foi de 14,6 por ciento. Lo teste CAGE triou 3,6 por ciento en los trabajadores encuestados como bebedores-problema.

Objetivo: Estimar a prevalência de transtornos mentais comuns (TMCs) em trabalhadores de Feira de Santana/BA, avaliando a ocorrência desses transtornos segundo diferentes aspectos da vida: características sociodemográficas, do trabalho profissional (incluindo aspectos psicossociais) e atividades de lazer. Métodos: Estudo de corte transversal, de caráter exploratório, com amostra aleatória de trabalhadores de Feira de Santana. Um questionário estruturado com questões sobre características sociodemográficas, trabalho profissional (incluindo aspectos psicossociais) e atividades de lazer foi utilizado. Os TMCs foram avaliados pelo SRQ-20 em 2002. Para análise de associação entre TMC e as variáveis de interesse, foram conduzidas análise bivariada e análise de regressão logística. Resultados: Foram estudados 1.311 trabalhadores. A prevalência global de TMC foi 25,2%. As variáveis estatisticamente associadas à TMC, no modelo final de análise, foram: sexo feminino (RP=3,04; 2,34-3,95), baixa escolaridade (RP=2,02; 1,57-2,60), renda mensal baixa (RP=1,78 1,12-2,84), ausência de atividades de lazer (RP=1,35; 1,10- 1,66) e trabalho de alta demanda psicológica (RP=1,45; 1,14-1,84). Conclusão: Evidenciou-se associação entre TMC e características sociodemográficas e do trabalho. Atenção especial deve ser dada a essas características para a promoção da saúde mental entre trabalhadores.


Objective: To estimate the prevalence of common mental disorders (CMD) among workers in Feira de Santana city, Bahia, Brazil, according to sociodemographic characteristics, occupation, work psychosocial aspects, and leisure activities. Methods: Exploratory cross sectional study, with random sampling of workers ofFeira de Santana. A structured questionnaire on sociodemographic characteristics, professional work, psychosocial aspects at work, and leisure activities have been used in 2002. CMD was evaluated by using the SRQ-20. Logistic regression techniques were used to analyze the association between CMD and the variablesof interest. Results: 1.311 workers have been studied and the prevalence of CMD was 25.2%. The characteristics statistically associated to CMD, in the final model of analysis, were: female (PR=3.04; 2.34-3.95), low education level (PR=2.02; 1.57-2.60), low monthly income (PR=1.78; 1.12-2.84), absence of leisure activities (PR=1.35; 1.10-1.66), and work with high psychological demand (PR=1.45; 1.14-1.84). Conclusion: Association between CMD and several sociodemographiccharacteristics have been evidenced. Special attention must be given to thesecharacteristics in order to promote worker’s mental health.
