RESUMO

Traumatic brain injury (TBI) is a neurotrauma with a complex pathophysiology caused by an external mechanical force. This global public health problem is a leading cause of death and disability in young adults. In this scenario, many models were developed to try to simulate human TBI. The weight drop model allows the investigation of the pathophysiological cascades of TBI without surgical interference. In this protocol, a new closed-head weight-drop rat model consisting of a 48.5g weight projectile that free falls from 1.10m high onto the skull of the animals was built. We classify the present TBI model performed as moderately severe due to its mortality rate. Animals from TBI and Control (Sham) groups underwent weight for 7 days and temperature assessments within 1 hour after TBI and for 7 days. Results demonstrated that the TBI group showed less body weight gain in the days after the injury. Temperature oscillations within the first-hour post-injury and on the 3rd day after injury were observed. As the results of this study demonstrated similarity to human TBI vital parameters, this new adaptation of the Weight-drop model injury can be a suitable candidate for translational studies.•We developed a novel closed head focal traumatic brain injury using a projectile.•This TBI model does not require surgical intervention.•The validation of this method demonstrates that the vital parameters of the injured rats exhibit similarities with those of TBI patients.

In this paper, the ballistic performance of a multilayered composite inspired by the structural characteristics of nacre is numerically investigated using finite element (FE) simulations. Nacre is a natural composite material found in the shells of some marine mollusks, which has remarkable toughness due to its hierarchical layered structure. The bioinspired nacre-like composites investigated here were made of five wavy aluminum alloy 7075-T651 (AA7075) layers composed of ~1.1-mm thick square tablets bonded together with toughened epoxy resin. Two composite configurations with continuous layers (either wavy or flat) were also studied. The ballistic performance of the composite plates was compared to that of a bulk monolithic AA7075 plate. The ballistic impact was simulated in the 300-600 m/s range using two types of spherical projectiles, i.e., rigid and elastoplastic. The results showed that the nacre plate exhibited improved ballistic performance compared to the bulk plate and the plates with continuous layers. The structural design of the nacre plate improved the ballistic performance by producing a more ductile failure and enabling localized energy absorption via the plastic deformation of the tablets and the globalized energy dissipation due to interface debonding and friction. All the plate configurations exhibited a better ballistic performance when impacted by an elastoplastic projectile compared to a rigid one, which is explained by the projectile plastic deformation absorbing some of the impact energy and the enlarged contact area between the projectile and the plates producing more energy absorption by the plates.

Introducción: las heridas por proyectil de arma de fuego en la región craneofacial provocan daños funcionales devastadoras y deformidades estéticas, que se suman al trauma psicológico al momento del regreso a la vida cotidiana de un paciente. Por esta razón, la reconstrucción adecuada es esencial para una rehabilitación integral. La fijación externa es un método de reducción cerrada de fracturas que implica el uso de tornillos para manipular segmentos sueltos de hueso, que luego se fijan mediante conexiones externas. Es importante recalcar que las fracturas mandibulares causadas por proyectil de arma de fuego son un reto para este tipo de tratamiento. Objetivo: presentar el caso de un paciente con fractura mandibular por proyectil de arma de fuego tratado con fijadores externos y revisión de la literatura sobre este tipo de tratamiento. Caso clínico: paciente masculino de 19 años que presentó fractura de rama mandibular izquierda causado por proyectil de arma de fuego; la fractura se manejó mediante la colocación de fijación intermaxilar con arcos barra tipo Erich y fijación externa durante 3 meses. Como parte del resultado, el paciente presentó una correcta oclusión dentaria y mantiene sus movimientos mandibulares sin ninguna limitación. Esto demuestra que la reducción cerrada y fijación externa debe mantenerse en el arsenal terapéutico debido a sus adecuados resultados comprobados en la literatura y en este caso. Ahora bien, aunque la reducción abierta y fijación interna con material de osteosíntesis hace parte del manejo idóneo para todo tipo de fractura, todos los casos requieren ser individualizados.


Background: wounds from a frearm projectile in the craniofacial region cause devastating functional damage and aesthetic deformities, along with psychological trauma when returning to daily life. This is why proper reconstruction is essential for comprehensive rehabilitation. External fxation is a method of closed fracture reduction that involves the use of screws to manipulate loose segments of bone that are then fxed using external connections. Objective: to present the case of a patient with a mandibular fracture caused by a frearm projectile treated with external fixators and review the literature. Clinical case: a 19-year-old male patient who presented a fracture of the left mandibular ramus caused by a frearm projectile, the fracture was managed by placing intermaxillary fxation with Erich-type bar arches and external fixation for 3 months. The patient presented a correct dental occlusion and maintains his mandibular movements without any limitation. Conclusion: mandibular fractures caused by frearm projectiles are a challenge for treatment. Open reduction and internal fixation with osteosynthesis material is the ideal management for all types of fractures, however, all cases must be individualized, and it must be considered that closed reduction and external fxation must remain in our therapeutic arsenal due to their adequate results verifed in the literature and in our case.

Resumen Las heridas por proyectil de arma de fuego se definen como los efectos que producen sobre el organismo los disparos realizados con armas cargadas de proyectiles o diversos tipos de pólvora u otros explosivos. La valoración médico legal en estos casos, en personas vivas, se realiza en la Sección Clínica Médico Forense del Departamento de Medicina Legal del Organismo de Investigación Judicial, esta incluye la historia médico legal, el examen físico, el análisis de documentos médicos aportados, pericias conexas efectuadas y revisión de bibliografía, esto para tener todos los elementos de juicio que permitan realizar un análisis objetivo y basado en la evidencia científica, y así poder responder a las interrogantes de la Autoridad Judicial. El presente artículo abarca algunos de los aspectos principales a tomar en cuenta ante hallazgos atípicos de los orificios de entrada y salida de los proyectiles, mediante la presentación y abordaje de un caso clínico.


Abstract Firearm projectile injuries are defined as the effects produced on the body by shots fired with weapons loaded with projectiles or various types of gunpowder or other explosives. The legal medical assessment in these cases, in living persons, is carried out in the Forensic Medical Clinical Section of the Department of Legal Medicine of the Organismo de Investigación Judicial, this includes the legal medical history, physical examination, analysis of medical documents provided, expert carried out and bibliography review, this to have all the elements of judgment that allow an objective analysis based on scientific evidence, and thus be able to answer the questions of the Judicial Authority. This article covers some of the main aspects to be considered when faced with atypical findings of the entry and exit holes of the projectiles, through the presentation and approach of a clinical case. Se describe el caso clínico de un paciente de 26 años de edad, atendido en la consulta de Ortopedia y Traumatología del Centro de Diagnóstico Integral Salvador Allende de Caracas, en la República Bolivariana de Venezuela, quien desde hacía 6 meses había recibido un disparo de escopeta en la cara posterior de ambos muslos. Esta vez acudió a consulta con dolor intenso en el miembro inferior izquierdo, sobre todo por debajo de la rodilla, acompañado de parestesias en la zona de inervación de ambas ramas de bifurcación del nervio ciático. Se pudo comprobar que el joven presentaba múltiples impactos de proyectiles en la cara posterior de ambos muslos y marcha claudicante. Los estudios radiográficos confirmaron la presencia de 8 proyectiles, 2 en el muslo derecho y 6 en el izquierdo. En la vista lateral se observó que uno de ellos se encontraba a una profundidad que se correspondía con la región anatómica del nervio ciático izquierdo. Para la intervención quirúrgica se utilizó anestesia local, sedación y un intensificador de imágenes. Con la cooperación del paciente se pudo extraer el proyectil alojado en el epineuro del ciático sin complicaciones. A los 3 meses habían desaparecido las molestias y pudo reincorporarse a sus actividades habituales.


The case report of a 26 years patient is described. He was assisted in the Orthopedics and Traumatology Service of Salvador Allende Integral Diagnostic Center from Caracas, in the Bolivian Republic of Venezuela who had received a shotgun shot in the posterior face of both thighs 6 months ago. This time he went to visit the doctor with intense pain in the left inferior member, mainly below the knee, accompanied by paresthesias in the innervation area of both bifurcation branches of the sciatic nerve. It could be demonstrated that the young man presented multiple projectile impacts in the posterior face of both thighs and hesitating gait. The radiographic studies confirmed the presence of 8 projectiles, two in the right thigh and 6 in the left one. In the lateral view it was observed that one of them was at a depth that matched with the anatomical region of the left sciatic nerve. Local anesthesia, sedation and an images intensifier were used for the operation. The projectile located in the epineurium of the sciatic nerve could be removed with the patient cooperation without complications. After 3 months the discomfort had disappeared and he could return to his usual activities.

Las fracturas mandibulares conminutadas por impacto de proyectil balístico son un desafío terapéutico debido a la presencia de múltiples fragmentos óseos y el compromiso a nivel de periostio, plano muscular y mucoso. Su tratamiento inicial requiere de una estabilización de las urgencias médico - quirúrgicas y posteriormente tratar los segmentos óseos fracturados. La fijación externa con tutores mandibulares permite estabilizar los segmentos mandibulares de forma cerrada manteniendo así la viabilidad de los fragmentos sin interrumpir su suministro de sangre. El objetivo de este trabajo es presentar los resultados post operatorios obtenidos en tres casos clínicos de pacientes con diagnóstico de fractura mandibular conminutada por trauma por impacto de proyectil balístico, tratados por un método alternativo a los tutores de fijación externa convencional.


Comminuted mandibular fractures from ballistic missile impact are a therapeutic challenge due to the presence of multiple bone fragments and the involvement of periosteum, muscle and mucous membrane. Their initial treatment requires emergency medical and surgical stabilization and later treatment of the fractured bone segments. External fixation with mandibular stakes makes it possible to stabilize the mandibular segments in a closed manner, thus maintaining the viability of the fragments without interrupting their blood supply. The aim of this work is to present the post-surgical results obtained in three clinical cases of patients with diagnosis of comminuted mandibularfracture by ballistic missile impact, treated by an alternative method to the conventional external fixation tutors.

Resumen La migración de un proyectil de arma de fuego por la vía urinaria y expulsión espontánea del mismo por el meato urinario es un hecho infrecuente. Se presenta el caso de un masculino de 24 años de edad, quien sufrió herida por proyectil de arma de fuego con orificio de entrada en el muslo derecho, sin orifico de salida, se observó mediante una radiografía el proyectil alojado en la pelvis, dos meses y medio después el paciente se presenta al servicio de emergencias con el proyectil ubicado en la uretra, el cual se extrajo por el meato urinario sin complicaciones. Se hizo una revisión de la literatura sobre migración de proyectiles de armas de fuego.


Abstract Migration of a bullet through the urinary tract and spontaneous expulsion through the urinary meatus is an uncommon event. We present a case of a 24-year-old male, who suffered a gunshot wound with an entry hole in the right thigh, without an exit hole, and the projectile lodged in the pelvis was observed by radiography, two months and a half later the patient presented to the emergency service with the projectile located in the urethra, which was removed through the urinary meatus without complications. A review of the literature on migration of bullet projectiles was made.

BACKGROUND: Traumatic injuries represent 9.6% of the global surface of the disease and represent the third most common cause of death in adults and adolescents. Patients were admitted to the hospital presenting traumatic injuries, penetrating abdominal traumatism remains a major cause of morbidity and mortality. OBJECTIVE: To analyze the presentation, handling and development of patient's gunshot wound. METHOD: Retrospective, descriptive, observational study. It was reviewed from 2010-2015 in the statistics department of the University Hospital Dr. José E. González all patients diagnosed with firearm projectile injury in abdomen. RESULTS: Were admitted above an average of 1.22 ± 0.51 lesions, on a Glasgow coma scale of 13.59 ± 2.5, oxygen saturation of 96.2 ± 2.09, heart rate of 94.4 ± 20, respiratory rate of 22 ± 6 and shock degree of 1.5 ± 0.80. CONCLUSION: Vital signs and specific characteristics of the lesion are essential and useful for the prediction of mortality due to abdominal traumatism by firearm projectile, in addition to the quantification of wounds and associated organic lesions, these are being evaluated both as a whole and separately.


ANTECEDENTES: Las lesiones traumáticas representan el 9.6% de la carga global de la enfermedad y son la tercera causa más común de muerte en adultos y adolescentes. Entre los pacientes ingresados en el hospital con lesiones traumáticas, el traumatismo penetrante abdominal sigue siendo una importante causa de morbilidad y mortalidad. OBJETIVO: Analizar la presentación, el manejo y la evolución de los pacientes con herida por proyectil de arma de fuego en el abdomen. MÉTODO: Estudio retrospectivo, descriptivo y observacional. Se revisaron todos los pacientes con diagnóstico de herida por proyectil de arma de fuego en el abdomen en el departamento de estadística del Hospital Universitario Dr. José E. González durante los años 2010-2015. RESULTADOS: Del total, 21 (9.1%) pacientes eran del sexo femenino y 229 (90.9%) del sexo masculino. Se ingresaron con un promedio de 1.22 ± 0.51 lesiones, con una puntuación en la Escala de coma de Glasgow de 13.59 ± 2.5, saturación de oxígeno de 96.2 ± 2.09, frecuencia cardiaca de 94.4 ± 20, frecuencia respiratoria de 22 ± 6 y grado de choque de 1.5 ± 0.80. CONCLUSIÓN: Los signos vitales y las características específicas de la lesión son esenciales y útiles para la predicción de la mortalidad por traumatismo abdominal por proyectil de arma de fuego, además de la cuantificación de heridas y lesiones orgánicas asociadas, siendo evaluadas estas tanto en un todo como por separado.

Se analiza el comportamiento balístico de dos tipos de perdigones "menos letales" usados por las fuerzas policiales en Chile para entender mejor los factores fundamentales que conllevan a la inusual cifra de lesiones oculares en el marco de las manifestaciones masivas ocurridas en Chile desde octubre de 2019. Para evaluar los riesgos de penetración y trauma ocular severo, se construyeron curvas de energía normalizada (E/a) en base a información publicada por las fuerzas policiales y fabricantes de municiones. Aun cuando el riesgo asociado al uso de las municiones según protocolos es leve, este artículo presenta que de todas formas existe riesgo de trauma ocular severo en todas las distancias de uso. La balística de los perdigones, combinada con la imprecisión de municiones multiproyec-tiles, son factores importantes para explicar la alta incidencia de trauma ocular severo. El caso de Chile es parte de un aumento global en la incidencia de lesiones oculares causadas por municiones consideradas menos letales, lo cual demanda una reevaluación de las políticas que regulan su uso.


The ballistic characteristics of two types of "less-lethal" multi-projectile shotgun rounds used by law enforcement in Chile are analyzed to better understand their contribution to the unusually high occurrence of ocular injuries at mass demonstrations since October 2019. Normalized energy (E/a) curves are constructed using publicly-available information from law enforcement and the manufacturers of the munitions to evaluate the risks of skin penetration and severe ocular trauma. Although the risk of penetration is small when these munitions are used according to local protocols, the risk of severe ocular trauma exists at all distances of approved use, and the ballistics of these pellets­along with the imprecision of multi-projectile shotgun rounds­help to explain the high inci-dence of severe ocular trauma. The example of Chile is part of a worldwide acceleration in the incidence of ocular injuries by "less-lethal" munitions and demands a reevaluation of their suitability for crowd control.

BACKGROUND: The wounds caused by the firearm projectile are published to date in a public health problem in the world. As an example, we mentioned the injuries caused by firearms are the first cause of death in the age group between 1 to 19 years in the United States, as in Mexico. OBJECTIVE: Analysis of the prognostic factors of mortality and evaluation of the evolution in patients with TPD due to abdominal HPPAF. METHOD: Retrospective, observational, descriptive study. Helped by 49 clinical files of patients who were admitted to the department of surgery of the University Hospital José Eleuterio González, between 2011 to 2015 and whose diagnosis was due to pancreatic trauma (TP), duodenal trauma (TD) or pancreatoduodenal trauma (TPD) by wounds caused by the firearm projectile. RESULTS: During a collection period of 5 years, a total of 49 clinical records applicable to the study were obtained according to the inclusion criteria, of which 36 (73%) suffered only from TD, 37 (75%) from TP and 24 (49%) about TPD. Significant differences were obtained for mortality associated with TD and TPD, but not for TP. The most affected organ as a lesion associated with a TPD was the liver, followed by thoracic structures and the stomach. The most significant risk factor for mortality was a prolonged stay in the intensive care unit. CONCLUSIONS: Data obtained are consistent with those consulted, providing new reproducible statistics for future studies regarding the increasing violence in our country and around the world.


ANTECEDENTES: Las heridas por proyectil de arma de fuego constituyen un problema de salud pública en el mundo. Como ejemplo mencionamos que este tipo de lesiones son la primera causa de muerte en el grupo de edad de 1 a 19 años en los EE.UU., al igual que en México. OBJETIVO: Análisis de los factores pronósticos de mortalidad y evaluación de la evolución en pacientes con TPD por HPPAF abdominal. MÉTODO: Estudio retrospectivo, observacional y descriptivo, realizado con 49 expedientes clínicos de pacientes que fueron admitidos en el departamento de cirugía del Hospital Universitario José Eleuterio González entre los años 2011 y 2015, cuyo diagnostico fue herida por proyectil de arma de fuego con trauma pancreático (TP), duodenal (TD) o pancreatoduodenal (TPD). RESULTADOS: Durante un periodo de recolección de 5 años se obtuvieron 49 expedientes clínicos aplicables al estudio según los criterios de inclusión, de los cuales 36 pacientes (73%) sufrieron únicamente TD, 37 (75%) TP y 24 (49%) TPD. Se obtuvieron diferencias significativas para la mortalidad asociada a TD y TPD, pero no para TP. El órgano más afectado como lesión asociada a un TPD fue el hígado, seguido de las estructuras torácicas y el estómago. El factor de riesgo para mortalidad más significativo fue una estancia prolongada en la unidad de cuidados intensivos. CONCLUSIONES: Los datos conseguidos concuerdan con los consultados, otorgando nueva estadística reproducible para futuros estudios respecto a la violencia creciente en nuestro país y alrededor del mundo.

Analysis of crime scenes involving single-fire-gun projectiles requires the determination of the direction of arrival of a projectile at the target and other factors to reconstruct events. The movement of a projectile can be analyzed by applying Euler's equations to a solid symmetrical rigid body. The present work starts from a Newtonian reformulation of these equations to show that, in the presence of a gravitational field, the system can be expressed with a complex variable nonlinear equation, where the inclusion of small nutation variables allows us to find possible solutions. As a particular case, we analyzed the movement of a 9-mm projectile fired from distances greater than 1 m to demonstrate that the direction of arrival of the projectile at the target cannot be traced by a stick placed in the target hole, as is usually performed in crime investigations. A series of shots were fired from distances varying between 1 m and 7 m. Impact data were recorded on Riemann planes of projection for the description of nutation and precession motions, allowing the observation of the motion dynamics of the projectile. We show that the direction of arrival at the target can be determined approximately from the analysis of the nutation and precession curves through Riemann planes of projection. The results presented in this work will allow more accurate judgements to be made in judicial investigations.

BACKGROUND: The introduction of firearms has increased deaths, assassinations and suicides. Firearm projectile injuries (PAF) increasingly affect civilian populations (500,000 injuries annually), it is the second cause of death in young people. Half required fracture treatment (45%), accompanied by reports of 6% who developed a deep infection. MATERIAL AND METHODS: Retrospective, observational, cross-sectional study in four hospitals of the Secretary of Health of CDMX of the frequency of infection in fractures exposed by PAF and if there is any other factor that is associated with the infection. RESULTS: In the review of the files of the four hospitals, 67 patients were found, men, with an incidence of infections of 4.5%, when analyzing the risk factors these did not present a significant association. DISCUSSION: No risk factor has statistical significance for any infectious process to occur, probably due to a good initial treatment.


ANTECEDENTES: La introducción de armas de fuego ha incrementado las muertes, asesinatos y suicidios. Las lesiones por proyectil de arma de fuego (PAF) afectan cada vez más a poblaciones civiles (500,000 lesiones anualmente) y es la segunda causa de muerte en jóvenes. La mitad requiere tratamiento de fracturas (45%), acompañado de reportes de 6% que desarrollaron una infección profunda. MATERIAL Y MÉTODOS: Estudio retrospectivo, observacional y transversal en cuatro hospitales de la Secretaría de Salud de la CDMX de la frecuencia de infección en fracturas expuestas por PAF y si existe algún otro factor que acompañe a la infección. RESULTADOS: Se encontraron en la revisión de los expedientes de los cuatro hospitales a 67 pacientes, hombres, con una incidencia de infecciones de 4.5%, al analizar los factores de riesgo éstos no presentaron asociación significativa. DISCUSIÓN: Ningún factor de riesgo posee significancia estadística de que se presente algún proceso infeccioso, probablemente por un buen tratamiento inicial.

Resumen: Antecedentes: La introducción de armas de fuego ha incrementado las muertes, asesinatos y suicidios. Las lesiones por proyectil de arma de fuego (PAF) afectan cada vez más a poblaciones civiles (500,000 lesiones anualmente) y es la segunda causa de muerte en jóvenes. La mitad requiere tratamiento de fracturas (45%), acompañado de reportes de 6% que desarrollaron una infección profunda. Material y métodos: Estudio retrospectivo, observacional y transversal en cuatro hospitales de la Secretaría de Salud de la CDMX de la frecuencia de infección en fracturas expuestas por PAF y si existe algún otro factor que acompañe a la infección. Resultados: Se encontraron en la revisión de los expedientes de los cuatro hospitales a 67 pacientes, hombres, con una incidencia de infecciones de 4.5%, al analizar los factores de riesgo éstos no presentaron asociación significativa. Discusión: Ningún factor de riesgo posee significancia estadística de que se presente algún proceso infeccioso, probablemente por un buen tratamiento inicial.


Abstract: Background: The introduction of firearms has increased deaths, assassinations and suicides. Firearm projectile injuries (PAF) increasingly affect civilian populations (500,000 injuries annually), it is the second cause of death in young people. Half required fracture treatment (45%), accompanied by reports of 6% who developed a deep infection. Material and methods: Retrospective, observational, cross-sectional study in four hospitals of the Secretary of Health of CDMX of the frequency of infection in fractures exposed by PAF and if there is any other factor that is associated with the infection. Results: In the review of the files of the four hospitals, 67 patients were found, men, with an incidence of infections of 4.5%, when analyzing the risk factors these did not present a significant association. Discussion: No risk factor has statistical significance for any infectious process to occur, probably due to a good initial treatment.

RESUMEN: El trauma maxilofacial por arma de fuego representa un desafío terapéutico principalmente debido a la gran conminución de tejidos que genera. Específicamente, en casos de fracturas mandibulares conminutadas, las modalidades de tratamientos incluye la reducción cerrada, fijación con tutor externo, fijación interna con alambres y reducción abierta y fijación interna utilizando placas y tornillos. El objetivo del presente trabajo es describir un tratamiento inicial con un método de fijación para fracturas mandibulares conminutadas como dispositivo alternativo al tutor externo clásicamente utilizado. Se desarrolla una revisión de la literatura actual acerca del tratamiento inicial o de urgencia de fracturas mandibulares conminutadas por impacto de proyectil balístico, haciendo énfasis en las indicaciones de cada modalidad de tratamiento y sus respectivas ventajas y desventajas. Luego se describe detalladamente el proceso de confección de un método de fijación alternativo junto con la exposición de casos clínicos donde fue utilizado como alternativa de tratamiento. La reducción y fijación obtenida por el dispositivo presentado logra resultados satisfactorios, sin encontrarse diferencias significativas a los elementos de fijación clásicos. Tanto el uso de placas de reconstrucción como el de tutor externo constituyen alternativas válidas para el tratamiento de fracturas mandibulares conminutadas. El dispositivo descrito y utilizado en los pacientes es una alternativa eficiente, de fácil confección y bajo costo económico demostrando buenos resultados en relación a la consolidación de fracturas conminutadas. Se debe conocer las indicaciones para la aplicación de cada modalidad de tratamiento.


ABSTRACT: Ballistic maxillofacial trauma represents a challenge for surgeons because of the important comminution process it presents. Specifically, in cases of mandibular comminuted fractures, the different treatment modalities include closed reduction, external fixation and internal fixation. The purpose of this case series is to describe an initial treatment modality, with a fixation method for comminuted mandibular fractures as an alternative for the classic external fixator. We conducted a literature review about the initial treatment for ballistic comminuted mandibular fractures, emphasizing indications of each treatment modality and their respective advantages and disadvantages. Next, we described the preparation process of an alternative fixation method along with a case series, where it was used as a therapeutical alternative. The reduction as well the fixation achieved by the external fixator, showed satisfactory results without significant differences noted with traditional fixation methods. Both, reconstruction plates and external fixators, are valid alternatives for treating comminuted mandibular fractures. The method described and used in these patients is and efficient alternative, easy to make and at a low cost, with good results in relation to fracture consolidation. We propose that every treatment modality and its indications should be evaluated in order to treat each case adequately.

Rubber projectiles are used as an alternative to metal bullets owing to their lower morbidity and mortality rate. There are few reports of vascular lesions of extremities caused by rubber projectiles in the literature. The authors report the case of a 37-year-old man who was the victim of a penetrating injury to the left thigh with a rubber projectile. He reported only pain at the site of the injury; pulses were decreased in the affected limb. After arteriography confirmed an injury to the superficial femoral artery, he underwent an arterial and venous femorofemoral bypass using a reversed contralateral saphenous vein.

Resumen El perito en balística de la Policía Nacional de Colombia, dentro de sus funciones, realiza el procedimiento de cotejo microscópico de vainillas y proyectiles disparados con armas de fuego, dentro del cual se encuentra el análisis de las características de clase, subclase e identidad a proyectiles cuya constitución no es la misma, como es el caso que se ha evidenciado, donde los cartuchos calibre 7.65 mm (para pistola) son utilizados en los revólveres calibre .32 largo; el objetivo del estudio es efectuar un análisis comparativo del micro-rayado de las estrías entre dichos proyectiles; se utilizó el método de observación científica, con enfoque cuantitativo de tipo comparativo, el cual permitió evidenciar que el nivel de uniprocedencias, al realizar cotejos entre estrías de proyectiles calibre 7.65 mm encamisado y .32 largo en plomo, es muy bajo; este resultado se logró documentar y consolidar en una tabla de datos, que al someterla al sistema estadístico obtuvo la cuantificación, disposición y análisis de las observaciones. Se llegó a la conclusión de que no es viable realizar cotejos microscópicos de proyectiles cuyas constituciones sean diferentes, como es el caso de las balas calibre 7.65 mm (encamisado) y .32 largo (plomo); de esta manera se orientan los procedimientos de los profesionales en balística.


Abstract The expert in ballistics of the National Police of Colombia, among its functions is in charge of performing performs the procedure of microscopic comparison of vanillas and projectiles shot with firearms, within which is the analysis of the class, subclass and identity features of projectiles with a constitution differing from each other like in the case having been demonstrated, where 7.65 mm caliber cartridges (for pistols) are used in .32 long caliber revolvers. The objective of the study is to carry out a comparative analysis between micro-scratching present in these projectiles; the scientific observation method was used with a quantitative approach allowing for demonstrating that the level of uni procedences or uni provenances at the time of making comparisons between out grooves of out comparing among to 7.65 mm jacketed he grooves said projectiles; the scientific observation was used, with a quantitative approach of a comparative type, which showed that the level of uni procedences or uni provenances at the time of carrying out en projectiles caliber 7.65 mm jacketed and .32 long lead, is very low; this result was documented and consolidated in a data table, which when subjected to the statistical system obtained the quantification, disposition and analysis of the observations. It was concluded that it is not feasible to perform microscopic comparisons of projectiles whose constitutions are different, as is the case with bullets caliber 7.65 mm (jacketed) and .32 long (lead); in this way the procedures of ballistics professionals are oriented.


Resumo O perito na balística da Policia Nacional da Colômbia, dentro das suas funções, faz o procedimento de comparação microscópica dos cartuchos e os projéteis disparados com armas de fogo, dentro da qual está a análise das características da classe, subclasse e da identidade aos projéteis cuja constituição não é a mesma, como é o caso que foi demonstrado, onde os cartuchos calibre 7.65 milímetros (para a pistola) são usados nos revólveres calibre .32 longo; o objetivo do estudo é realizar uma análise comparativa dos micro-aranhões das estrias entre estes projéteis; o método usado foi a observação científica, com aproximação quantitativa do tipo comparativo, que permitiu demonstrar que o nível das uni-procedências, quando fazer comparações entre estrias dos projéteis calibre 7.65 milímetros encamisado e .32 longo em chumbo, é muito baixo; este resultado logrou-se documentar e consolidar em uma tabela dos dados, que quando é submetida ao sistema estatístico obteve-se a quantificação, a disposição e a análise das observações. A conclusão foi que não é viável fazer comparações microscópicas dos projéteis cujas constituições são diferentes, como é o caso das balas calibre 7.65 milímetros (encamisado) e .32 longo (chumbo); assim os procedimentos dos profissionais na balística são orientados.

OBJECTIVE: Demonstrate the use of wide-field optical fluorescence as an adjunctive examination in a clinical routine to oral diagnosis. BACKGROUND DATA: Use of wide-field optical fluorescence in the oral cavity has been restricted to topics related to the detection and diagnosis of oral cancer. MATERIALS AND METHODS: In a regular medical appointment, a 58-year-old female patient, without any complaint or oral symptom, underwent the complementary examination by wide-field optical fluorescence. A device with high-power light-emitting diode emitting light centered at a wavelength of (400 ± 10) nm and maximum irradiance of (0.040 ± 0.008) W/cm2 was used for fluorescence visualization. RESULTS: We report the location of a firearm projectile, intraosseous, in the maxillary tuberosity using wide-field optical fluorescence. CONCLUSIONS: It is evidenced that wide-field optical fluorescence, within a clinical routine, can provide relevant images and data, with an immediate result, without the use of ionizing radiation, enabling an efficient oral diagnosis.

ABSTRACT BACKGROUND AND OBJECTIVES: Gunshot wounds may have fatal results. Even when not causing major injuries to soft and hard tissues, there may be other severe problems. This study aimed at reporting the case of a projectile located in the left infratemporal fossa and at discussing treatment, risks and complications. CASE REPORT: Male patient, 18 years old, suffered a gunshot wound which has penetrated the face by the left zygomatic region, and was lodged in the homolateral infratemporal fossa. This has caused jaw function impairment and pain. Foreign body was surgically removed by preauricular access and patient was then submitted to physiotherapy. After treatment, temporomandibular joint function was reestablished and esthetic results were considered excellent without sequelae. CONCLUSION: Management of patients hit by projectiles is more complex when these are located in an area of difficult access and close to noble structures. Radiographic techniques obtained by means of different planes allow the accurate location of the object. There may be major deformity and functional incapacity, especially if the facial nerve is affected during bullet trauma or during surgery. Although there were no nervous injuries, functional impairment of orofacial structures was decisive to indicate the surgical procedure. Surgical removal of the projectile from the infratemporal fossa, combined with postoperative physiotherapy, has shown to be an effective treatment.


RESUMO JUSTIFICATIVA E OBJETIVOS: Ferimentos causados por arma de fogo podem ter resultados fatais. Mesmo que a bala não cause grandes lesões para os tecidos moles e duros, outros problemas graves podem ocorrer. O objetivo deste estudo foi relatar o caso de um projétil localizado na fossa infratemporal esquerda, discutir o tratamento cirúrgico, seus riscos e complicações. RELATO DO CASO: Paciente do gênero masculino, 18 anos, sofreu um ferimento por arma de fogo que penetrou na face pela região zigomática esquerda, alojando-se na fossa infratemporal homolateral. Esse ocasionou comprometimento da função mandibular e dor. O corpo estranho foi removido cirurgicamente por meio do acesso pré-auricular e o paciente foi posteriormente submetido a sessões de fisioterapia. Após o tratamento, foi reestabelecida a função da articulação temporomandibular, a dor desapareceu e os resultados estéticos foram considerados excelentes, sem sequelas. CONCLUSÃO: O manuseio dos pacientes acometidos por projétil torna-se mais complexo quando esse está localizado em uma área de difícil acesso e ao lado de estruturas nobres. Técnicas radiográficas, obtidas por meio de diferentes planos, permitem uma localização precisa do objeto. Grande deformidade e incapacidade funcional podem ocorrer, especialmente, se o nervo facial é afetado durante o trauma balístico ou durante o ato cirúrgico. Apesar de não haver lesões nervosas, o comprometimento funcional das estruturas orofaciais foi decisivo para indicar o procedimento cirúrgico. A remoção cirúrgica do projétil da fossa infratemporal combinado com a fisioterapia, pós-operatória, mostraram ser um tratamento eficaz.

This study investigates evidence of changes and continuities in ancient Maya violence and warfare in inland northwest Yucatan, Mexico from the Middle Preclassic (600-300 BC) to the Postclassic (AD 1050-1542) through bioarchaeological analysis of cranial and projectile trauma. It is hypothesized that the frequency of violence increases before the Classic Maya collapse and remains high during the Postclassic period. It is also hypothesized that the flat, open terrain was conducive to warfare and resulted in higher trauma frequencies than in other parts of the Maya area. Results show that the frequency of cranial trauma decreases before the Classic collapse and increases in the Postclassic, partially matching the expected chronological trends. The frequency of cranial trauma does not differ significantly from other Maya regions but the pattern does: for all periods, males have more healed injuries than females and they are concentrated on the left side of the anterior of the skull. Some injuries appear to be from small points hafted in wooden clubs. In addition, projectile trauma is evident in a scapula with an embedded arrowhead tip, the first such case reported in a Maya skeleton. Overall, these results suggest greater reliance on open combat and less on raids in this region compared with other parts of the Maya area, possibly due to the flat, open terrain, though the identification of perimortem trauma in both women and men indicates surprise raids on settlements were also practiced.
