RESUMO
Introduction and objectives: Due to the lack of structured and systematic information available, the aim of this study was to describe the epidemiology, diagnosis, healthcare processes, and treatment patterns of hereditary angioedema (HAE) in Mexico. To achieve this, different data sources were consulted regarding medical literature, structured health system databases, and angioedema-specialized physicians (AEP) opinion regarding HAE. Material and methods: A mixed methods approach was conducted in 4 phases: I) systematic literature review (SLR) and meta-analysis according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines; II) review of national health system (NHS) databases and systematic reports; III) physician survey; and IV) an epidemiologic model. ICD 10 D84.1 encoded records from the NHS were used to estimate the number of patients with HAE attended and treated during 2019. A survey was implemented to increase understanding of the clinical profile and treatments used. Results: A prevalence rate of 0.9/50 000 inhabitants was estimated for 2019. In the same year, an estimated 317 HAE type 1 patients were recorded in the NHS, aged ≥11 years old. The most frequent clinical symptoms were cutaneous edema (67.5%) and abdominal pain (47.9%). A severe episode with laryngeal edema appeared in 27.5% of cases. Acute episodes were mainly moderate to severe (77.0%), with an annual per capita frequency of emergency visits of 7.6 patient-year (range 1-12/patient-year). The main reasons for hospitalization corresponded to laryngeal facial, tongue, and abdominal edemas, representing 73.3% of annual ICD 10 D84.1 reported hospitalizations. The main treatments that patients with HAE received were fresh frozen plasma for acute attacks and danazol for short-term prophylaxis (STP). Conclusions: Despite efforts to make HAE visible, according to this study, cases recognized and treated in the NHS represent only 16.6% of the estimated prevalence.
RESUMO
Leprosy remains endemic in some regions and is a global health concern. However, the possible causes and risk factors of the disease remain unclear. Data in Wenshan, China were collected from the Wenshan Institute of Dermatology (1986-2015); data in Nepal were obtained from the Leprosy Control Division, Department of Health Services, Nepal (2011 to 2015); and data from Indonesia, India, and Brazil were collected from WHO records. We assessed the epidemiological trends of leprosy in Wenshan and compared the features of possible causes and risk factors with those of other countries. We then performed a descriptive and statistical analysis to make our study more purposeful and definitive. A total of 3,376 cases were detected in Wenshan from 1986 to 2015. The overall prevalence rate (PR) of leprosy presented a decreasing trend with a peak (4.9/10,000 population) in 1986. The detection of new leprosy cases was higher in males than in females. Visible deformity increased every year since 2005 with a disability of 34.8% in 2015 among new cases. In Nepal, 2,461 leprosy patients received multi-drug therapy (MDT) in 2015 which corresponded to the PR of 0.89/10,000 population. Geographic latitude and socio-economic situations appeared to be the main causes of leprosy, and the healthcare condition was an important factor associated with leprosy incidence. The introduction of MDT effectively reduced leprosy prevalence worldwide. Wenshan (China), Nepal, and other countries share similarities in various aspects with respect to socio-cultural features, geographical distribution, environmental factors, and economic situation, which may contribute to leprosy being endemic in these areas.

BACKGROUND: Studies on the prevalence of neuromyelitis optica spectrum disorder (NMOSD) are still scarce. The aim of the current study was to determine the prevalence rate of NMOSD in Belo Horizonte, southeast Brazil, where the prevalence rate of multiple sclerosis (MS) has already been established. METHODS: For this observational study, eligible patients had to meet the 2015 International Panel for Neuromyelitis Optica Diagnosis, be seen at the study center between January 2000 and February 2019 and live in Belo Horizonte. The prevalence rate of NMOSD was estimated based on the number of MS and NMOSD patients seen at same Center during the same period, and the previously established prevalence of MS in Belo Horizonte. RESULTS: During the study period, there were 69 patients with NMOSD, 60 (87.0%) of whom were females, and 44 (63.8%) non-whites. The median age at disease onset was 36.7 (4-72) years, the mean EDSS score 4.78±2.36, and the mean ARR 0.57±0.43. Anti-aquaporin-4 immunoglobulin testing was available for 61 (88.4%) patients, of whom 41 (67.2%) had a positive result. During the same period, 280 MS patients were seen. Considering the local known prevalence rate of MS of 18.1/100,000 inhabitants, the estimated NMOSD prevalence rate in Belo Horizonte was 4.52/100,000 (95% CI 3.72-5.43) inhabitants. CONCLUSION: The prevalence rate of NMOSD in Belo Horizonte is high as compared with those found in most of the studies reported to date.

INTRODUCTION: Long-term persistent Helicobacter pylori infection has been associated with ulceropeptic disease and gastric cancer. Although H. pylori is predominantly acquired early in life, a clear understanding of infection dynamics during childhood has been obfuscated by the diversity of populations evaluated, study designs, and methods used. AIM: Update understanding of true prevalence of H. pylori infection during childhood, based on a critical analysis of the literature published in the past 5 years. METHODS: Comprehensive review and meta-analysis of original studies published from 2011 to 2016. RESULTS: A MEDLINE® /PubMed® search on May 1, 2016, using the terms pylori and children, and subsequent exclusion, based on abstract review using predefined criteria, resulted in 261 citations. An Embase® search with the same criteria added an additional 8 citations. In healthy children, meta-analysis estimated an overall seroprevalence rate of 33% (95% CI: 27%-38%). Seven healthy cohort studies using noninvasive direct detection methods showed infection prevalence estimates ranging from 20% to 50% in children ≤5 and 38% to 79% in children >5 years. The probability of infection persistence after a first positive sample ranged from 49% to 95%. Model estimates of cross-sectional direct detection studies in asymptomatic children indicated a prevalence of 37% (95% CI: 30%-44%). Seroprevalence, but not direct detection rates increased with age; both decreased with increasing income. The model estimate based on cross-sectional studies in symptomatic children was 39% (95% CI: 35%-43%). CONCLUSIONS: The prevalence of H. pylori infection varied widely in the studies included here; nevertheless, model estimates by detection type were similar, suggesting that overall, one-third of children worldwide are or have been infected. The few cohort and longitudinal studies available show variability, but most studies, show infection rates over 30%. Rather surprisingly, overall infection prevalence in symptomatic children was only slightly higher, around 40%. Studies including only one positive stool sample should be interpreted with caution as spontaneous clearance can occur.

The aim of the present study was identifying the prevalence of Benzodiazepine (BZD) by women in a Family health care program (FHPs). It was also analyzed the possible associations of the use of BZD with sociodemographic factors, affection of chronic diseases and use of other psychotropic drugs. The choice of gender was justified by scientific literature pointing women to be the major BZD users. The choice of basic health care was done because of the lack in studies in those settings in Brazil. It is a quantitative study of cross section and of correlational descriptive type. Data were collected on the charts and registration sheets and recipes from responsible pharmacy for the geographical region. Analyses were performed using descriptive statistics and logistic regression, considering as the outcome variable the use of BZDs. The prevalence found in this study was 7.4% (n = 1094) and was below than others the found in similar studies. It is understood that the most preventive focus of the FHPs may contribute to these findings or the quality of the records have interfered in these results. Users of BZDs studied were in majority in the age range from 56 to 74 years (48.4%), studied only up to the first degree (65.6%), self- mentioned at least one disease (82.2%) and almost half used at least one other psychotropic drug (48.4%) at the same time. There was a significant association between using BZDs and have low educational level (p = 0.005), present self- mentioned diseases (p = 0.000) and the use of other psychotropic drugs (p = 0.000). The association between higher ages and use of BZDs (p = 0.005) was also significant. Analyzes also showed that women who used other psychotropic drugs were five times more likely to use BZDs than those who did not. Women with self- reported disease were almost five times more likely to use BZDs than women who did not report diseases. We concluded that the groups that should receive more attention in relation to the consumption of these substances are those of middle-aged women or older with low schooling, and with chronic disease. It is recommended that the health teams pay attention to the psychosocial needs of persons with mental disorder, discuss and take efforts to implement actions for insertion and social reintegration as well as the expansion of networks of support that can be a strategic aid to the containment of symptoms and improving of the quality of life of these people, in an attempt to avoid the use of more psychotropic drugs and/or of the services of greater complexity

BACKGROUND: The birth prevalence rate (BPR) of congenital anomalies (CAs) is heterogeneous and exhibits geographical and sociocultural variations throughout the world. In South America (SA), high birth prevalence regions of congenital anomalies have been observed. The aim of this study was to identify, describe, and characterize geographical clusters of congenital anomalies in SA. METHODS: This observational descriptive study is based on clinical epidemiological data registered by the Latin-American Collaborative Study of Congenital Malformations network. Between 1995 and 2012, a total of 25,082 malformed newborns were ascertained from 2,557,424 births at 129 hospitals in SA. The spatial scan statistic was used to determine geographical regions with high BPR of CAs. The BPR was obtained with a Poisson regression model. Odds ratios were estimated for several risk factors inside the geographical clusters. RESULTS: We confirmed the existence of high BPR regions of CAs in SA. Indicators of low socioeconomic conditions, such as a low maternal education, extreme age childbearing, infectious diseases, and medicine use during pregnancy were detected as risk factors inside these regions. Native and African ancestries with high frequency of consanguineous marriages could explain partially these high BPR clusters. CONCLUSION: The recognition of clusters could be a starting point in the identification of susceptibility genes associated with the occurrence of CA in high BPR regions.

Objective: estimating HIV and syphilis prevalence rates among parturient women and coverage of HIV and syphilis testing during pregnancy bymeans of registered information in prenatal and hospital records. Methods: the sampling was probabilistic in two selection stages. In the first one,150 hospitals were selected, stratified by population size of the municipality. In each health establishment, 100-120 parturient were randomly selected.Coverage of syphilis testing was estimated among parturient women who took two syphilis tests during prenatal care and one test at delivery. Syphilis prevalence rate was estimated among parturient women tested at the delivery. Results: only 14.1% of the women followed the recommendations of two syphilis tests during pregnancy and one at the delivery. The percentage of pregnant women with at least one syphilis test registered in the prenatal record was 75.1% but only 16.9% showed two test results. Among pregnant women that were tested at delivery, 1.1% had positive results.Among women that were tested during prenatal care and were negative, 0.4% got positive in the second test. Among women who did not have prenatalcare and were tested at delivery, 1.8% had positive results. Conclusion: the low coverage of the second syphilis test indicates that the Ministry of Health recommendations are not being obeyed in the routine of health services. The findings depicted indicate the need of the second syphilis test during pregnancy for the adequate vertical transmission control.
