Introducción. Etimológicamente la palabra íleo proviene del griego eileos que significa rodando o girando; es descrito como el retraso temporal de la motilidad gastrointestinal. Mundialmente es reconocido que el íleo postoperatorio es una de las principales complicaciones después de cirugía y representa un problema importante por su elevado costo sanitario. Se ha investigado respecto al uso preventivo de medidas físicas y farmacológicas, como los procinéticos, para el manejo del íleo postoperatorio. Métodos. Investigación clínico-terapéutica, comparativa, de corte longitudinal prospectivo de seguimiento, con muestreo no probabilístico por conveniencia, conformado por cuatro grupos con 25 pacientes cada uno, atendidos en el Servicio de Cirugía General entre mayo y agosto de 2021. Resultados. Los pacientes presentaron una media de edad de 49,4 ± 19,6 años y el 53 % fueron hombres. El 86 % de los pacientes presentaron ruidos intestinales antes de 24 horas posteriores a la operación. La primera evacuación en los pacientes que ingirieron café tipo espresso fue a las 42,6 horas y para aquellos en el grupo de café americano fue a las 43,4 horas en comparación con 89,4 horas en el grupo control (p < 0,001). Conclusión. Se recomienda el uso del café como una medida segura y económica para el inicio de la dieta, como una alternativa al esquema tradicional, constituyéndose en una opción para el manejo del íleo postoperatorio


Introduction. Etymologically the word ileus comes from the Greek eileos, which means rolling or turning. It is described as the temporary delay of gastrointestinal motility. It is recognized worldwide that postoperative ileus is one of the main complications after surgery and represents an important problem due to its high healthcare cost. Research has been done regarding the preventive use of physical and pharmacological measures, such as prokinetics, for the management of postoperative ileus.Methods. Clinical-therapeutic, comparative, prospective longitudinal follow-up research, with non-probabilistic convenience sampling, made up of four groups with 25 patients each, treated in the General Surgery Service between May and August 2021. Results. The patients had a mean age of 49.4 ± 19.6 years and 53% were male; 86% of patients had bowel sounds within 24 hours after the operation. The first bowel movement in patients who ingested espresso coffee was 42.6 hours and for those in the American coffee group it was at 43.4 hours compared to 89.4 hours in the control group (p < 0.001). Conclusion. The use of coffee is recommended as a safe and economical measure to start the diet as an alternative to the traditional fashion, becoming an option for the management of postoperative ileus.

BACKGROUND: Digital technology and gamified apps can be useful in the health care context. Gamification uses technology to influence users' actions and motivations through experiences that resemble games. Patient adherence to the enhanced recovery after surgery (ERAS) program is crucial for achieving early recovery after surgery and continuous monitoring is essential for obtaining good results. OBJECTIVE: This study aimed to describe the development and validation of a mobile app for enhanced recovery after surgery (MobERAS), a gamified mobile health app for telemonitoring patients in the postoperative period based on the ERAS program, and to evaluate its functionality and usability and the experience of patients, health care professionals, and computer professionals with its use. METHODS: We developed MobERAS for postoperative telemonitoring, with active participation of patients in the process, and offering availability of real-time information for the health team. The app development process included idealization, interdisciplinary team formation, potential needs assessment, and product deployment. Usability tests were conducted throughout the development process with improvements, technical adjustments, and updates. After finalization, comprehensive verification tests were performed. The parameters evaluated are those that can influence the length of hospital stay, such as nausea, vomiting, pain scales, return to normal gastrointestinal function, and thromboembolic events. MobERAS was designed to be downloaded by users on their phones, tablets, or other mobile devices and to provide postoperative data. The app has a GPS that monitors the patient's walking time and distance and is connected to a virtual database that stores the collected data. RESULTS: Women undergoing medium and major gynecologic oncologic surgeries were included. We included 65 patients with an average age of 53.2 (SD 7.4, range 18-85) years. The time of use ranged from 23.4 to 70 hours (mean 45.1, SD 19.2 hours). Regarding adherence to the use of MobERAS, the mean fill rate was 56.3% (SD 12.1%, range 41.7%-100%), and ambulation data were obtained for 60 (92.3%) of the 65 patients. The researcher had access to the data filled out by the patients in real time. There was good acceptance of the use of MobERAS by the patients, with good evaluation of the app's usability. MobERAS was easy to use and considered attractive because of its gamified design. The app was rated as good or very good in all items by health care professionals (n=20) and professionals specializing in technological innovation (n=10). CONCLUSIONS: MobERAS is easy to use, safe, well accepted by patients, and well evaluated by experts. It can be of great use in clinical surgical practice and an important tool for greater engagement of patients and health care professionals with the ERAS program.

Abstract Objective To investigate whether patients undergoing anatomic total shoulder arthroplasty (ATSA) between January and March 2020 experienced different postoperative outcomes than patients in 2019. We hypothesized that patients in 2020 would have less access to physical therapy (PT) and experience different postoperative outcomes. Methods Records from patients who received ATSA between January 1st, 2019, and March 17th, 2019, and January 1st, 2020, to March 17th, 2020, were analyzed. Patient data, including demographic information, range of motion (ROM), strength, and PT was collected and compared between the two groups. The 2020 patients were contacted by phone during October 2022 and patient-reported metrics were gathered. Results The present study identified 24 patients in 2019 and 27 patients in 2020 who underwent ATSA during the specified time frame and had a minimum 1-year follow-up. Patients in 2019 experienced improvements in forward elevation (FE) ROM (125.4° to 146.7°; p = 0.008), external rotation (ER; 33.0° to 47.7°; p < 0.001), and internal rotation (IR; S1 to L4; p = 0.019). Patients in 2020 also experienced significant improvements in FE (120.2° to 141.1°; p = 0.009), ER (32.9° to 42.0°; p = 0.037), and IR (S1 to L3; p = 0.002). Patients in 2020 terminated PT earlier (2019: 125.8 days; 2020: 91.1 days; p = 0.046) and completed fewer sessions (2019: 21.4 sessions; 2020: 13.1 sessions; p = 0.003). At the final follow-up, patients in 2020 reported an average Visual Analogue Scale (VAS) pain score of 1.67 ± 1.1. Conclusion Despite decreased PT, patients who underwent ATSA in 2020 had significant improvements in ROM and strength and were comparable to patients in 2019.


Resumo Objetivo Investigar se os pacientes submetidos à artroplastia total anatômica do ombro (ATAO) entre janeiro e março de 2020 tiveram resultados pós-operatórios diferentes dos pacientes operados em 2019. Nossa hipótese é a de que os pacientes de 2020 teriam menos acesso à fisioterapia (FT) e apresentariam desfechos pós-opera-tórios diferentes. Métodos Foram analisados os prontuários de pacientes submetidos à ATAO de 1° de janeiro de 2019 a 17 de março de 2019 e de 1° de janeiro de 2020 a 17 de março de 2020. Os dados dos pacientes, incluindo informações demográficas, amplitude de movimento (ADM), força e FT foram coletados e comparados entre os dois grupos. Os pacientes de 2020 foram contatados por telefone durante outubro de 2022, e as métricas relatadas por eles pacientes foram coletadas. Resultados Este estudo identificou 24 pacientes em 2019 e 27 pacientes em 2020 que foram submetidos à ATAO durante o período especificado e tiveram acompanhamento mínimo de 1 ano. Os pacientes em 2019 apresentaram melhoras na ADM de elevação anterior (EA; 125,4° a 146,7°; p = 0,008), rotação externa (RE; 33,0° a 47,7°; p < 0,001) e rotação interna (RI; S1 a L4; p = 0,019). Os pacientes em 2020 também apresentaram melhoras significativas de EA (120,2° a 141,1°; p = 0,009), RE (32,9° a 42,0°; p = 0,037) e RI (S1 a L3; p = 0,002). Os pacientes de 2020 terminaram a FT mais cedo (2019: 125,8 dias; 2020: 91,1 dias; p = 0,046) e fizeram menos sessões (2019: 21,4 sessões; 2020: 13,1 sessões; p = 0,003). No acompanhamento final, os pacientes de 2020 relataram uma pontuação média de dor na Escala Visual Analógica (EVA) de 1,67 ± 1,1. Conclusão Apesar da menor realização de FT, os pacientes submetidos à ATAO em 2020 apresentaram melhoras significativas na ADM e na força e foram comparáveis aos pacientes de 2019.

INTRODUÇÃO: Cardiac surgery (CS) may be associated with several organic repercussions responsible for the appearance of cardiac risk factors during the postoperative period. These, associated with prolonged hospital length of stay (LoS), may trigger critical manifestations in individuals undergoing this surgical procedure. OBJECTIVE: To investigate the relationships between postoperative cardiac risk factors, LoS, and changes in functioning state. METHODS: Patients undergoing reconstructive, substitutive, or corrective cardiac surgeries were evaluated. The presence of postoperative cardiovascular risks was assessed using the InsCor score, while LoS and functionality were collected from medical records. RESULTS: One-hundred patients with a mean age of 59.2±12.3 years were included. Significant correlations between functionality and both the hospital and Intensive Care Unit (ICU) LoS (p<0.0001, ρ=0.56; p=0.002, ρ=0.29, respectively), as well as between hospital LoS and the number of comorbidities (p=0.003, ρ=0.28) were found. No significant relationships were observed between the number of postoperative risk factors and LoS. CONCLUSION: Functionality and comorbidities are associated with increased hospital and ICU LoS in patients undergoing cardiac surgery.


INTRODUÇÃO: A cirurgia cardíaca (CC) pode estar associada a diversas repercussões orgânicas responsáveis pelo surgimento de fatores de risco cardíaco no pós-operatório. Estas, associadas ao tempo de internação hospitalar (TIH) prolongado, podem desencadear manifestações críticas em indivíduos submetidos a esse procedimento cirúrgico. OBJETIVO: Investigar a correlação entre os fatores de risco cardíaco pós-operatórios em CC, funcionalidade e o (TIH). MÉTODOS: Foram avaliados pacientes submetidos a CC reconstrutivas, substitutivas ou corretivas. A presença de riscos cardiovasculares pós-operatórios foi avaliada pelo escore da InsCor, enquanto o TIH e a funcionalidade foram coletados em prontuários médicos. RESULTADOS: Cem pacientes com idade média de 59,2±12,3 anos foram incluídos. Correlações significativas entre funcionalidade e TIH total e na Unidade de Terapia Intensiva (UTI) (p<0.0001, ρ=0.56; p=0.002, ρ=0.29, respectivamente), bem como entre o TIH e o número de comorbidades (p=0.003, ρ=0.28) foram encontradas. Não foram observadas correlações significativas entre o número de fatores de risco pós-operatórios e o TIH. CONCLUSÃO: O número de alterações funcionais e o número de comorbidades estão associados ao aumento do TIH total e na UTI em pacientes submetidos à CC.

Objetivo: identificar os principais diagnósticos e intervenções de enfermagem em crianças em pós-operatório imediato de queiloplastia. Métodos: estudo documental e retrospectivo, desenvolvido entre março e agosto de 2020, em um hospital público, de nível terciário, referência no tratamento de pacientes com anomalias craniofaciais e síndromes relacionadas. Utilizou-se o Referencial Teórico das Necessidades Humanas Básicas. Referente ao Processo de Enfermagem, foram considerados dados do histórico de enfermagem e, a partir deles, os diagnósticos e as intervenções, por meio das taxonomias da NANDA Internacional e da Nursing Intervention Classification, respectivamente. Utilizou-se a análise estatística descritiva. Resultados: a amostra constou de 78 prontuários. Predominaram as necessidades psicobiológicas: oxigenação, hidratação, nutrição, eliminação, integridade cutâneo-mucosa e física, percepção dolorosa e ambiental. Foram identificados 11 diagnósticos de enfermagem, sendo seis com foco no problema e cinco de risco. Elencaram-se ainda, 14 intervenções de enfermagem. Conclusão: a utilização dos Sistemas de Linguagens Padronizadas norteou a identificação dos diagnósticos e intervenções de enfermagem, subsidiando o raciocínio clínico para a construção e organização do cuidado. Objective: to identify the main nursing diagnoses and interventions in children in the immediate postoperative period of cheiloplasty. Methods: documentary and retrospective study, carried out between March and August 2020, in a public, tertiary-level hospital, a reference in the treatment of patients with craniofacial anomalies and related syndromes. The Theoretical Framework of Basic Human Needs was used. Regarding the Nursing Process, data from the nursing history were considered and, based on them, diagnoses and interventions, through the taxonomies of NANDA International and the Nursing Intervention Classification, respectively. Descriptive statistical analysis was used. Results: the sample consisted of 78 medical records. Psychobiological needs predominated: oxygenation, hydration, nutrition, elimination, cutaneous-mucous and physical integrity, pain and environmental perception. Eleven nursing diagnoses were identified, six focused on the problem and five on risk. 14 nursing interventions were also listed. Conclusion: the use of Standard Language Systems guided the identification of nursing diagnoses and interventions, supporting clinical reasoning for the construction and organization of care. (AU)

Métodos: estudio documental y retrospectivo, realizado entre marzo y agosto de 2020, en un hospital público de tercer nivel, referencia en el tratamiento de pacientes con anomalías craneofaciales y síndromes relacionados. Se utilizó el Marco Teórico de las Necesidades Humanas Básicas. En cuanto al Proceso de Enfermería, se consideraron datos de la historia de enfermería y, a partir de ellos, diagnósticos e intervenciones, a través de las taxonomías de la NANDA Internacional y la Nursing Intervention Classification, respectivamente. Se utilizó análisis estadístico descriptivo. Resultados: la muestra estuvo constituida por 78 historias clínicas. Predominaron las necesidades psicobiológicas: oxigenación, hidratación, nutrición, eliminación, cutáneo-mucosa e integridad física, dolor y percepción ambiental. Fueron identificados once diagnósticos de enfermería, seis enfocados al problema y cinco al riesgo. También se listaron 14 intervenciones de enfermería. Conclusión: el uso de los Sistemas de Lenguaje Estándar orientó la identificación de diagnósticos e intervenciones de enfermería, apoyando el razonamiento clínico para la construcción y organización del cuidado. Objective: The purpose of this study was to compare postoperative pain between SF flap and serratus anterior muscle (SM) in direct-to-implant breast reconstruction. Methods: This is a prospective cohort study that included 53 women diagnosed with breast cancer who underwent mastectomy and one-stage implant-based breast reconstruction from January 2020 to March 2021. Twenty-nine patients (54.7%) had SF elevation, and 24 patients (45.3%) underwent SM elevation. We evaluated patient-reported early postoperative pain on the first day after surgery. Also, it was reported that all surgical complications in the first month and patient reported outcomes (PROs) were measured with the BRECON 23 questionnaire. Results: The serratus fascia group used implants with larger volumes, 407.6 ± 98.9 cc (p < 0.01). There was no significant difference between the fascial and muscular groups regarding the postoperative pain score reported by the patients (2 versus 3; p = 0.30). Also, there was no difference between the groups regarding early surgical complications and PROs after breast reconstruction. Conclusion: The use of SF seems to cause less morbidity, which makes the technique an alternative to be considered in breast reconstruction. Although there was no statistical difference in postoperative pain scores between the fascia and serratus muscle groups.

RESUMO

Postoperative pain after cardiac surgery plays an important role in the patient's recovery process. In particular, pain at the chest tube site can negatively affect the comfort and recovery of these patients. Effective pain control minimizes the risk of many complications. Oral and intravenous analgesics, epidural anesthesia, paravertebral block, and intercostal nerve blockade are used in chest tube pain control. We routinely use the surgical cryoablation method in the presence of atrial fibrillation in the preoperative period of cardiac surgery in our clinic. Here we aimed to describe our method of using the cryoablation catheter for intercostal nerve blockade.

Objetivo: Identificar os eventos adversos no pós-operatório imediato de queiloplastia e/ou palatoplastia em crianças e comparar os eventos identificados aos notificados ao Núcleo de Segurança do Paciente. Métodos: Estudo descritivo, retrospectivo e quantitativo, realizado em um hospital público e terciário brasileiro. Os dados foram coletados por meio da descrição nos registros de enfermagem e comparados aos notificados ao Núcleo de Segurança do Paciente, referente a junho e dezembro de 2019. Os resultados foram submetidos a análise estatística descritiva. Resultados: A amostra constou de 203 crianças, das quais 51% (n=103) apresentaram evento adverso. Foram identificados 176 eventos adversos, de 8 tipos, com prevalência da laringite pós-extubação (n=50; 28%), edema de língua (n=34; 19%) e lesão de comissura labial (n=25; 14%). Destes, apenas 5% (n=9) foram notificados ao Núcleo de Segurança do Paciente. Conclusão: Os eventos adversos prevalentes se relacionaram a cavidade oral e tecidos adjacentes, e a subnotificação foi expressiva. (AU)

The results were subjected to descriptive statistical analysis. Results: The sample consisted of 203 children, of which 51% (n=103) had an adverse event. A total of 176 adverse events of 8 types were identified, with prevalence of post-extubation laryngitis (n=50; 28%), tongue edema (n=34; 19%) and labral commissure lesion (n=25; 14%). Of these, only 5% (n=9) were notified to the Patient Safety Center. Conclusion: The prevalent adverse events were related to the oral cavity and adjacent tissues, and underreporting was significant. Los datos se recolectaron mediante descripciones en registros de enfermería y se compararon con los notificados al Centro de Seguridad del Paciente, referidos a junio y diciembre de 2019. Los resultados fueron sometidos a análisis estadístico descriptivo. Resultados: La muestra estuvo conformada por 203 niños, de los cuales el 51% (n = 103) tuvo un evento adverso. Se identificaron un total de 176 eventos adversos de 8 tipos, con prevalencia de laringitis posextubación (n=50; 28%), edema de lengua (n=34; 19%) y lesión de la comisura del labrum (n=25; 14%). De estos, solo el 5% (n=9) fueron notificados al Centro de Seguridad del Paciente. Conclusion: Los eventos adversos prevalentes se relacionaron con la cavidad bucal y los tejidos adyacentes y el subregistro fue significativo. (AU)

Objective: To develop and validate an educational-care video produced for patients with self-care guidelines in the postoperative period of cardiac surgery. Method: Methodological study, developed in the stages of pre-production, production, and post-production. Content validation was carried out with 13 expert judges, with technical and scientific expertise in the field. Data were tabulated, processed, and analyzed through descriptive analysis. Content validity index (CVI) and Cronbach's alpha tests were performed for reliability. Results: The educational-care video contains information on surgical wound, wound care, alcoho-lic beverages, sexual activity, return to work, physical activity, and diet. Conclusion: The video validated by experts obtained maximum CVI and an attributed reliability of 0.728, indicating it is an educational technology that can be applied in nursing practice to guide patients in the postoperative period of cardiac surgery. Objetivo: Construir y validar un video educativo sobre cuidados elaborado para pacientes con instrucciones de autocuidado en el postopera-torio de cirugía cardíaca. Método: Estudio metodológico, desarrollado en las etapas de preproducción, producción y postproducción. La validación del contenido se realizó con 13 jueces expertos, con experiencia técnica y científica en el área. Los datos fueron tabulados, procesados y analizados mediante análisis descriptivo. Se realizaron pruebas de confiabilidad del Índice de Validez de Contenido (IVC) y Alfa de Cronbach. Resultados: El video educativo de cuidado contiene información sobre la herida quirúrgica; cuidados de la herida quirúrgica; consumo de alcohol; actividad sexual; retorno al trabajo; actividad física y nutrición. Conclusión: El video validado por expertos obtuvo un IVC máximo y una confiabilidad asignada de 0,728, lo que indica que es una tecnología educativa que puede ser aplicada en la práctica de enfermería para guiar a los pacientes en el postoperatorio de cirugía cardíaca. Objetivo: Construir e validar um vídeo cuidativo-educacional produzido para pacientes com orientações de autocuidado no pós-operatório de cirurgia cardíaca. Método: Estudo metodológico, desenvolvido nas etapas de pré-produção, produção e pós-produção. A validação de conteúdo ocor-reu com 13 juízes especialistas, com expertise técnica e científica na área. Os dados foram tabulados, processados e analisados perante análise descritiva. Realizaram-se os testes de índice de validade de conteúdo (IVC) e de alfa de Cronbach, para confiabilidade. Resultados: O vídeo cuidativo-educacional contém informações quanto a ferida cirúrgica, cuidados com a ferida cirúrgica, bebidas alcoólicas, atividade sexual, retorno ao trabalho, atividade física e alimentação. Conclusão: O vídeo validado pelos especialistas obteve IVC máximo e a confiabilidade atribuída de 0,728, indicando ser uma tecnologia educacional que pode ser aplicada na prática da enfermagem para orientar pacientes no pós-operatório de cirurgia cardíaca. OBJECTIVE: Translate and cross-culturally adapt into Brazilian Portuguese the Glasgow Children's Benefit Inventory instrument used for the quality-of-life assessment after pediatric ENT interventions. METHOD: This is a methodological study of translation and cross-cultural adaptation of the GCBI instrument following seven stages: 1) Translation of two versions by two independent translators, 2) Elaboration of a consensual synthetized version, 3) Assessment of the synthetized version by experts, 4) Assessment by the target audience, 5) Back-translation, 6) Pilot study and 7) Use of the instrument. The final version of the instrument was answered by a sample of 28 people responsible for children aged from 2 to 7 years, submitted to tonsillectomy between January 2019 and December 2021, in a public hospital in Porto Alegre. The collection considered patients with a minimum of 6-months and a maximum of 3-years of postoperative follow-up. RESULT: The instrument final version was compared to the original version showing semantic equivalence, absence of consistent translation difficulties and appropriate cross-cultural adaptation, and well understood by the target audience. The application of the questionnaire in the sample showed a Cronbach alpha coefficient of 0.944 corresponding to a high degree of reliability of the instrument. CONCLUSION: The translation and cross-cultural adaptation showed semantic appropriateness and its use when assessing ENT postoperative results in a pediatric population showed high reliability of the instrument.

ABSTRACT Postoperative pain after cardiac surgery plays an important role in the patient's recovery process. In particular, pain at the chest tube site can negatively affect the comfort and recovery of these patients. Effective pain control minimizes the risk of many complications. Oral and intravenous analgesics, epidural anesthesia, paravertebral block, and intercostal nerve blockade are used in chest tube pain control. We routinely use the surgical cryoablation method in the presence of atrial fibrillation in the preoperative period of cardiac surgery in our clinic. Here we aimed to describe our method of using the cryoablation catheter for intercostal nerve blockade.

ABSTRACT BACKGROUND: There is still a debate regarding the most appropriate pleural collector model to ensure a short hospital stay and minimum complications. OBJECTIVES: To study aimed to compare the time of air leak, time to drain removal, and length of hospital stay between a standard water-seal drainage system and a pleural collector system with a unidirectional flutter valve and rigid chamber. DESIGN AND SETTING: A randomized prospective clinical trial was conducted at a high-complexity hospital in São Paulo, Brazil. METHODS: Sixty-three patients who underwent open or video-assisted thoracoscopic lung wedge resection or lobectomy were randomized into two groups, according to the drainage system used: the control group (WS), which used a conventional water-seal pleural collector, and the study group (V), which used a flutter valve device (Sinapi® Model XL1000®). Variables related to the drainage system, time of air leak, time to drain removal, and time spent in hospital were compared between the groups. RESULTS: Most patients (63%) had lung cancer. No differences were observed between the groups in the time of air leak or time spent hospitalized. The time to drain removal was slightly shorter in the V group; however, the difference was not statistically significant. Seven patients presented with surgery-related complications: five and two in the WS and V groups, respectively. CONCLUSIONS: Air leak, time to drain removal, and time spent in the hospital were similar between the groups. The system used in the V group resulted in no adverse events and was safe. Objective: the objective of this study is to examine the relationships between sleep, quality of life and anxiety in patients undergoing cardiac surgeries during the preoperative period, at discharge, two weeks after discharge and three months after discharge. Method: this study had a prospective, descriptive and correlational design and was conducted in a single center. The sample consisted of 68 patients who had undergone cardiac surgeries. The data were collected using an Information Form, the State-Trait Anxiety Inventory, the Richard-Campbell Sleep Questionnaire and the Nottingham Health Profile. Results: the patients' sleep quality increased from moderate to good at each measurement moment after the surgeries, when compared to sleep quality measured at their first hospitalization. While the state anxiety scores decreased at discharge and 2 weeks after the initial hospitalization, they increased to a moderate level 3 months after discharge. There was no significant relationship between anxiety levels and sleep quality at any measurement moment. Additionally, the patients' quality of life was significantly improved 2 weeks and 3 months after discharge. Conclusion: The results of this study showed that the sleep quality of patients who had undergone cardiac surgeries was improved during the postoperative period, and that this improvement exerted a positive effect on their quality of life.


Objetivo: el objetivo de este estudio es examinar las relaciones entre el sueño, la calidad de vida y la ansiedad en pacientes sometidos a cirugías cardíacas durante el período preoperatorio, al momento del alta hospitalaria, y dos semanas y tres meses después del alta. Método: este estudio tuvo un diseño prospectivo, descriptivo y correlacional y se realizó en un único centro. La muestra estuvo compuesta por 68 pacientes que habían sido sometidos a cirugías cardíacas. Los datos se recolectaron por medio de un Formulario de Información y a través de los siguientes instrumentos: State-Trait Anxiety Inventory , Richard-Campbell Sleep Questionnaire y Nottingham Health Profile . Resultados: la calidad del sueño de los pacientes mejoró de moderada a buena en cada medición después de las cirugías, en comparación con la evaluada al momento de la primera internación. Si bien las puntuaciones de ansiedad rasgo disminuyeron al momento del alta hospitalaria y 2 semanas después de la internación inicial, aumentaron al nivel moderado 3 meses después del alta. No se registró ninguna relación significativa entre niveles de ansiedad y calidad del sueño en ninguna de las mediciones. Además, la calidad de vida de los pacientes mejoró significativamente 2 semanas y 3 meses después del alta hospitalaria. Conclusión: los resultados de este estudio demostraron que la calidad de sueño de los pacientes sometidos a cirugías cardíacas mejoró durante el período postoperatorio, además de que esta mejora ejerció un efecto positivo sobre su calidad de vida.

Embora a pontuação de ansiedade-estado tenha diminuído na alta e duas semanas após a internação inicial, ela aumentou para um nível moderado três meses após a alta. Não houve relação significativa entre os níveis de ansiedade e a qualidade do sono em nenhum momento de medição. Além disso, a qualidade de vida dos pacientes melhorou significativamente duas semanas e três meses após a alta. Conclusão: os resultados desse estudo mostraram que a qualidade de sono dos pacientes sujeitos a cirurgias cardíacas melhorou durante o período pós-operatório, e que essa melhora exerceu um efeito positivo em sua qualidade de vida.

Abstract Objective: The purpose of this study was to compare postoperative pain between SF flap and serratus anterior muscle (SM) in direct-to-implant breast reconstruction. Methods: This is a prospective cohort study that included 53 women diagnosed with breast cancer who underwent mastectomy and one-stage implant-based breast reconstruction from January 2020 to March 2021. Twenty-nine patients (54.7%) had SF elevation, and 24 patients (45.3%) underwent SM elevation. We evaluated patient-reported early postoperative pain on the first day after surgery. Also, it was reported that all surgical complications in the first month and patient reported outcomes (PROs) were measured with the BRECON 23 questionnaire. Results: The serratus fascia group used implants with larger volumes, 407.6 ± 98.9 cc (p < 0.01). There was no significant difference between the fascial and muscular groups regarding the postoperative pain score reported by the patients (2 versus 3; p = 0.30). Also, there was no difference between the groups regarding early surgical complications and PROs after breast reconstruction. Conclusion: The use of SF seems to cause less morbidity, which makes the technique an alternative to be considered in breast reconstruction. Although there was no statistical difference in postoperative pain scores between the fascia and serratus muscle groups.

Introducción: La intervención quirúrgica cardiaca es compleja, pero con resultados satisfactorios para el paciente, pues incrementa su supervivencia y calidad de vida. El síndrome de bajo gasto cardiaco en el periodo posoperatorio es una de las entidades que ensombrece el pronóstico y eleva los índices de morbilidad y mortalidad en dichos pacientes. Objetivo: Estimar la supervivencia de los pacientes con síndrome de bajo gasto cardiaco en el periodo posoperatorio según variables clínicas y ecocardiográficas. Métodos: Se realizó una investigación de cohorte de supervivencia en 56 pacientes operados del corazón, diagnosticados con síndrome de bajo gasto cardiaco posoperatorio, quienes fueron atendidos en el Centro de Cardiología y Cirugía Cardiovascular de Santiago de Cuba, desde enero del 2019 hasta noviembre del 2021. Resultados: Predominaron el grupo etario de 65 o más años (60,7 %), la diabetes mellitus (44,6 %) como antecedente patológico personal y los pacientes con fracción de eyección de 45 % o más, quienes presentaron mayor cantidad de decesos (29,8 %). La función del ventrículo derecho afectada se halló en 52,6 % de los fallecidos. El tiempo de circulación extracorpórea de 90 minutos o más primó en 67,9 % de los afectados, de los cuales 42,1 % murieron. Conclusiones: Se observó que la diabetes mellitus, el tiempo de circulación extracorpórea y el sangrado perioperatorio prolongados, así como la función sistólica biventricular se relacionaron con el pronóstico del síndrome de bajo gasto cardiaco en el periodo posoperatorio.


Introduction: Heart surgical intervention is complex, but with satisfactory results for the patient, because it increases his survival and life quality. The low cardiac output syndrome in the postoperative period is one of the entities that darkens the prognosis of this operation and elevates the morbidity and mortality indexes in these patients. Objective: To estimate the survival of patients with low cardiac output syndrome in the postoperative period according to clinical and echocardiographyc variables. Methods: A cohort investigation of survival in 56 operated patients was carried out, who were diagnosed with postoperative low cardiac output syndrome and were assisted in the Cardiology and Cardiovascular Surgery Center of Santiago de Cuba, from January, 2019 to November, 2021. Results: There was prevalence of the 65 years and over age group (60.7%), the diabetes mellitus (44.6%) as personal pathological history and patients with ejection fraction of 45% or more who presented higher quantity of deaths (29.8%). The function of the affected right ventricle was found in 52.6% of deaths. The time of extracorporeal circulation of 90 minutes or more prevailed in 67.9% of those affected, of whom 42.1% died. Conclusions: It was observed that diabetes mellitus, the time of extracorporeal circulation and the long perioperative bleeding, as well as the biventricular systolic function was related to the prognosis of low cardiac output syndrome in the postoperative period.

ABSTRACT Objective: To evaluate the occurrence of adverse events in the postoperative period of cardiac surgery in a pediatric intensive care unit and to find any patient characteristics that can predict such events. Methods: This was a historical cohort study of patients recovering in the pediatric intensive care unit for the first 7 days after cardiac surgery between April and December 2019, by reviewing the medical records. The following were reviewed: demographic, clinical, and laboratory characteristics; patient severity scores; and selected adverse events, grouped into device-related, surgical, and nonsurgical. Results: A total of 238 medical records were included. At least one adverse event occurred in 110 postoperative patients (46.2%). The total number of adverse events was 193 (81%). Vascular catheters were the most common cause, followed by cardiac arrest, bleeding, and surgical reexploration. In the univariate analysis, the vasoactive-inotropic score (VIS), Risk Adjustment in Congenital Heart Surgery (RACHS-1) score, age, Pediatric Index of Mortality (PIM-2), cardiopulmonary bypass and aortic clamping duration were significantly associated with adverse events. In the multivariate analysis, VIS ≥ 20 (OR 2.90; p = 0.004) and RACHS-1 ≥ 3 (OR 2.11; p = 0.019) were significant predictors, while age and delayed sternal closure showed only trends toward significance. To predict the occurrence of adverse events from VIS and RACHS-1, the area under the curve was 0.73 (95%CI 0.66 - 0.79). Conclusion: Adverse events were quite frequent in children after cardiac surgery, especially those related to devices. The VIS and RACHS-1, used together, predicted the occurrence of adverse events well in this pediatric sample.


RESUMO Objetivo: Avaliar a ocorrência de eventos adversos em pós-operatório cardíaco em uma unidade de terapia intensiva pediátrica e estabelecer eventuais associações das características dos pacientes e a possibilidade de predizer tais eventos. Métodos: Coorte histórica de 7 dias de pós-operatório cardíaco, de abril a dezembro de 2019, por revisão de prontuários de pacientes com recuperação em unidade de terapia intensiva pediátrica. Foram revisados: características demográficas e clínico-laboratoriais, escores de gravidade dos pacientes e eventos adversos selecionados agrupados em: relacionados a dispositivos, a aspectos cirúrgicos e a aspectos não cirúrgicos. Resultados: Foram incluídos 238 prontuários. Ocorreu pelo menos um evento adverso em 110 pós-operatórios (46,2 %). O número total de eventos adversos foi 193 (81%), sendo mais frequente a complicação com cateteres vasculares, seguida de parada cardíaca, sangramento e reexploração cirúrgica. Na análise univariada, escore vasoativo-inotrópico (VIS- vasoactive-inotropic score), Risk Adjustment in Congenital Heart Surgery (RACHS-1) score, idade, Pediatric Index of Mortality (PIM-2), tempo de circulação extracorpórea e de clampeamento aórtico foram estatisticamente significantes com eventos adversos. Na análise multivariável, VIS ≥ 20 (OR 2,90; p = 0,004) e RACHS-1 ≥ 3 (OR 2,11; p = 0,019) mostraram-se relevantes e com significância estatística, enquanto idade e fechamento tardio do esterno possuíam apenas tendência a essa associação. Considerando a previsão de ocorrência de eventos adversos a partir dos valores de escore vasoativo-inotrópico e de RACHS-1, a área sob a curva mostrou valor de 0,73 (IC95% 0,66 - 0,79). Conclusão: A frequência de eventos adversos foi expressiva e aqueles relacionados a dispositivos foram os mais frequentes. O VIS e o RACHS-1, utilizados em conjunto, foram capazes de predizer a ocorrência de eventos adversos nesta amostra pediátrica.

Background: The quality of recovery is an important component in the evaluation of perioperative care. To measure this, the Quality of Recovery-15 (QoR-15) scale has been validated previously. The aim of this study was to culturally, linguistically, and psychometrically adapt this scale to the Colombian Spanish language. Methods: After linguistic adaptation, a validation study of the psychometric properties was carried out. These included validity, reliability, and responsiveness. The scale was administered after the administration of general anaesthesia. Validity was determined by correlating with the general recovery VAS, surgical risk, surgical duration, and length of hospital stay. Structural validity was assessed using factor analysis. Test-retest and internal consistency were used to measure reliability. Results: Interviews were conducted with 161 adults. A positive correlation was found between the Spanish version of the QoR-15 scale (QoR-15C) and VAS scores (r=0.51), and a negative correlation between the QoR-15C score and the duration of surgery (r=-0.47) and hospital stay (r=-0.62). The reliability of the scale was adequate. Cronbach's alpha was 0.74, and Lin's correlation concordance coefficient was 0.99. Confirmatory factor analysis indicated that the scale in the Colombian Spanish version does not have a single domain, whilst exploratory factor analysis indicated that the scale may measure an additional factor. Conclusions: The QoR-15C scale for assessing the quality of recovery after general anaesthesia showed psychometric properties comparable with those of the English scale. This allows the scale to be considered for use in research and clinical practice.

El ejercicio sigue siendo una intervención necesaria para la pérdida y mantenimiento del peso en pacientes postquirúrgicos de cirugía bariátrica. Sin embargo, los principios básicos de su prescripción luego de este tipo de intervenciones son aún controversiales en la literatura. En este contexto, el objetivo de este estudio fue describir los principales componentes de la prescripción del ejercicio en pacientes postquirúrgicos de cirugía bariátrica y sus beneficios asociados. Se realizó una revisión exploratoria usando las bases de datos Pubmed, PEDro, Springer, Science Direct, y Scielo. Se utilizó un algoritmo de búsqueda con los términos clave "cirugía bariátrica", "obesidad", "ejercicio", "post-quirúrgico", "prescripción del ejercicio" y "actividad física", en inglés y español. Fueron incluidos ensayos clínicos y estudios observacionales publicados entre 2010-2020, donde hubo prescripción del ejercicio posterior a cirugía bariátrica. Un total de 13 estudios fueron analizados. Los parámetros recomendados incluyeron la frecuencia de 3 a 5 veces por semana, intensidad aeróbica entre el 60-85% de la FCmáx o 60-70% de VO2máx, intensidad anaeróbica entre el 60-85% de 1 repetición máxima (1RM), y duración entre 12 y 18 semanas. El ejercicio es una herramienta importante para mantener y reducir el peso, atenuar el sedentarismo y mejorar la calidad de vida en paciente postquirúrgicos de cirugía bariátrica con resultados seguros y eficaces siempre y cuando se garanticen los niveles de intensidad, frecuencia, duración, tipo y progresión sugeridos.


Exercise remains a necessary intervention for weight loss and weight maintenance in post-bariatric surgery patients. However, the basic principles of its prescription after this type of interventions are still controversial in the literature. In this context, our objective was to describe the main components of exercise prescription in post-bariatric surgery patients and its associated benefits. A scoping review was carried out using the Pubmed, PEDro, Springer, Science Direct, and Scielo databases. A search algorithm was used with the terms "bariatric surgery", "obesity", "exercise", "post-surgical", "exercise prescription" and "physical activity", in English and Spanish. Clinical trials and observational studies published between 2010-2020, where there was exercise prescription after bariatric surgery, were included. A total of13 studies were analyzed. The recommended parameters included frequency 3 to 5 times per week, aerobic intensity between 60-85% of HRmax or 60-70% of VO2max, anaerobic intensity between 60-85% of 1 repetition maximum (1RM), and duration period between 12 and 18 weeks. Exercise is an important tool to maintain and reduce weight, attenuate sedentary lifestyle and improve quality of life in post-bariatric surgery patients with safe and effective results as long as the suggested levels of intensity, frequency, duration, type and progression are guaranteed.

OBJECTIVE: To explain the rationale and protocol of the methods and analyses to be used in the LIVER-PAM randomized clinical trial, which seeks to understand whether a higher mean arterial pressure is capable of reducing the incidence of renal dysfunction postoperatively after liver transplantation. METHODS: LIVER-PAM is an open-label, randomized, controlled, singlecenter clinical trial. Patients randomized to the intervention group will have a mean arterial pressure of 85 - 90mmHg in the initial 24 hours of postoperative management, while patients in the control group will have a mean arterial pressure of 65 - 70mmHg in the same period. A sample of 174 patients will be required to demonstrate a 20% reduction in the absolute incidence of renal dysfunction, with a power of 80% and an alpha of 0.05. CONCLUSION: If a 20% reduction in the absolute incidence of renal dysfunction in the postoperative period of liver transplantation is achieved with higher target mean arterial pressure in the first 24 hours, this would represent an inexpensive and simple therapy for improving current outcomes in the management of liver transplant patients.ClinicalTrials.gov Registry: NCT05068713.
