Introducción. Los pacientes conectados a ventilación mecánica invasiva pueden presentar complicaciones respiratorias, donde la retención de secreciones es una de las más frecuentes. El drenaje y eliminación de las secreciones depende entre otras variables de los flujos respiratorios generados, donde una diferencia absoluta entre el flujo espiratorio máximo (FEM) y flujo inspiratorio máximo (FIM) menor a 17 L•min-1 o una relación FIM/FEM mayor a 0.9 favorecerían la retención de secreciones. Sin embargo, falta por determinar los flujos respiratorios resultantes y la proporción de pacientes con riesgo de retención de secreciones según estos parámetros. Objetivo. Determinar los flujos respiratorios durante la ventilación mecánica invasiva y la proporción de pacientes que se encuentra en riesgo de retención de secreciones. Métodos. Estudio descriptivo transversal desarrollado en la Unidad de Paciente Crítico Médico-Quirúrgico del "Hospital Clínico de la Red de Salud UC-CHRISTUS". Se incluyeron pacientes adultos intubados y conectados a ventilación mecánica, en quienes se determinó los flujos respiratorios resultantes y se estimó la diferencia absoluta FEM-FIM, la relación FIM/FEM y la proporción de pacientes con riesgo de retención de secreciones. Resultados. Se incluyeron 100 pacientes, 45% presentaba entre sus diagnósticos patología respiratoria. La mediana de la diferencia absoluta entre FEM y FIM fue de 6 L•min-1 (-5 - 14.5) y la mediana de la tasa FIM/FEM de 0.87 (0.7 - 1.13). Un 84% presentó una diferencia absoluta entre FEM y FIM menor a 17 L•min-1, mientras que el 46% presentó una relación FIM/FEM mayor a 0.9. Conclusión. Una alta proporción de pacientes conectados a ventilación mecánica presenta riesgo de retención de secreciones independiente de la presencia o ausencia de patología respiratoria. Se requieren futuras investigaciones para evaluar el impacto de este criterio sobre complicaciones respiratorias.


Background. Patients connected to invasive mechanical ventilation may develop respiratory complications, where retention of secretions is one of the most frequent. The drainage and elimination of the secretions depend on other variables of the respiratory flows generated, where an absolute difference between the peak expiratory flow (PEF) and peak inspiratory flow (PIF) less than 17 L•min-1 or a PIF/PEF ratio greater than 0.9 would favor secretion retention. However, it is necessary to determine the respiratory flows and the proportion of patients, with and without respiratory pathology, with a risk of secretions retention according to these parameters. Objective. Determine respiratory flows during connection to invasive mechanical ventilation and the proportion of patients with and without respiratory pathology at risk of secretions retention. Methods. A descriptive cross-sectional study was conducted in the Medical-Surgical Intensive Care Unit of the "Hospital Clínico de la Red de Salud UC-CHRISTUS". Intubated adult patients connected to mechanical ventilation were included, in whom the respiratory flows were assessed, and the absolute PEF-PIF difference, PIF/PEF ratio, and the proportion of patients with a risk of secretions retention were determined. Results. 100 patients were included, of which 45% presented among their diagnoses acute or chronic respiratory pathology. For the total number of patients, the median of the absolute difference between PEF and PIF was 6 L•min-1 (-5 - 14.5), and the median of the PIF/PEF ratio of 0.87 (0.7 - 1.13). Of the total of patients, 84% presented an absolute difference between PEF and PIF less than 17 L• min-1, while 46% presented a PIF/PEF ratio greater than 0.9. Conclusion. Considering the absolute difference between PEF-PIF and the PIF/PEF ratio, many patients present a risk of secretions retention. However, whether this is associated with severe respiratory complications in patients connected to invasive mechanical ventilation should be clarified in future research.

BACKGROUND: Peak inspiratory and expiratory flows (PIF, PEF) are parameters used to evaluate the mechanics of the respiratory system. These parameters can vary based on whether they are measured using mechanical devices vs. spirometry and based on the barometric pressure at which the measurements are obtained. Our objectives were (1) to report the normal values and variability of PEF and PIF of a Latin American population living at a moderate altitude (2240 m above sea level), (2) to analyze the adjustment of reference values obtained at sea level with those obtained in healthy subjects living at a moderate altitude, and (3) to assess the correlation between PEF obtained by spirometry (PEFs) and PEF obtained by mechanical devices (PEFm). METHODS: In this prospective and transversal study, men and women with good respiratory health aged between 2.8 and 68 years old were invited to participate. Randomly, they underwent spirometry (to measure PEFs and PIFs) and mechanical flowmetry (to measure PEFm). RESULTS: A total of 314 subjects participated, with an average age of 24.3 ± 16.4 years; 59% were Women. The main determinants for the reference equations were age, weight, height and sex at birth. The agreement of the PEFm, PEFs and PIFs values was inconsistent with that reported by other authors, even at the same barometric pressure. The association between PEFm and PEFs was r = 0.91 (p < 0.001), and the correlation coefficient of concordance was 0.84. CONCLUSIONS: The PEFm, PEFs, and PIFs measurements in individuals living at moderate altitudes are different from those found by other authors in cities with different barometric pressures and ethnicities.

Introdução: a verificação da limitação ao fluxo aéreo ocasionado pelo tabagismo, por meio da mensuração do pico de fluxo expiratório, é uma medida viável para estudos populacionais que permite a sinalização precoce ou prevenção da instalação da doença pulmonar obstrutiva crônica. Objetivo: avaliar o grau de limitação ao fluxo expiratório em tabagistas em diferentes níveis de atividade física. Metodologia: trata-se de um estudo transversal. Foram selecionados 68 fumantes ativos por mais de um ano, acima de 18 anos, de ambos os sexos. O pico de fluxo expiratório foi obtido por meio do medidor portátil do modelo Mini-WrightTM e o nível de atividade física foi avaliado pelo International Physical Activity Questionnaire 8.0. As variáveis estudadas foram descritas pela amplitude de variação das médias, desvios padrão, máximo, mínimo e coeficiente de variação, sendo estabelecido o nível de significância de 5% através do teste Qui-quadrado. Resultados: o percentual de redução dos valores do pico de fluxo expiratório, previsto em relação ao obtido, chegou a uma redução média de 56,87% em maiores de 60 anos e 53,19% em tabagistas com um tempo de prática maior de 30 anos. Houve aumento considerável do percentual médio de redução do pico de fluxo expiratório nos grupos de tabagistas insuficientemente ativos e sedentários. Conclusão: os resultados sugerem que a limitação ao fluxo expiratório foi mais acentuada em tabagistas do sexo feminino, acima de 60 anos, com tempo superior a 30 anos de prática do tabagismo e com baixo nível de atividade física. Os tabagistas considerados muito ativos apresentaram menor nível de limitação ao fluxo expiratório.


Introduction: the verification of airflow limitation caused by smoking, through the Peak Expiratory Flow is a viable measure for population studies that allows the early signaling or prevention of the onset of Chronic Obstructive Pulmonary Disease. Objective: assess the degree of expiratory flow limitation in smokers at different levels of physical activity. Methods: this is a cross-sectional study. 68 active smokers for more than one year, over 18 years, of both sexes were selected. Peak expiratory flow was obtained using a portable Mini-WrightTM model and the level of physical activity was assessed using the International Physical Activity Questionnaire 8.0. The variables studied were described by the range of variation of the means, standard deviations, maximum, minimum and coefficient of variation, with a significance level of 5% being established using the Chi-square test. Results: the percentage of reduction in the values of peak expiratory flow, predicted in relation to that obtained, reached a mean reduction of 56.87% in people over 60 years of age and 53.19% in smokers with more than 30 years of practice. There was a considerable increase in the mean percentage of reduction in peak expiratory flow in the groups of insufficiently active and sedentary smokers. Conclusion: results suggest that expiratory flow limitation was more accentuated in female smokers, over 60 years of age, with more than 30 years of smoking practice and with a low level of physical activity. Smokers considered very active had a lower level of limitation of expiratory flow.

INTRODUÇÃO: O pico de fluxo expiratório (PFE) é comumente usado para monitorar a progressão de doenças respiratórias, pois fornece boas informações sobre o estado das vias aéreas. Uma boa quantidade de pesquisas está sendo feita em todo o mundo para estabelecer uma equação de previsão local. A força-tarefa conjunta da Sociedade Torácica Americana e da Sociedade Respiratória Europeia promoveu pesquisas a esse respeito. Na Índia, os dados derivados da população caucasiana ainda são usados para o PFE. OBJETIVO: Estudar a relação dos parâmetros do PFE e os dados antropométricos como idade, altura, peso, índice de massa corporal (IMC), área de superfície corporal (ASC) e estabelecer uma equação de regressão para jovens adultos indianos. MÉTODOS: PFE foi feito em 1000 sujeitos de 15-25 anos da região metropolitana de Mumbai. O coeficiente de correlação de Pearson foi usado para entender a relação dos parâmetros antropométricos e PFE. A análise de regressão multivariada foi feita para estabelecer uma equação de predição. (Alfa 5%) RESULTADOS: Idade e todos os parâmetros antropométricos foram correlacionados com PFE. O pico de fluxo expiratório médio da população masculina foi de 515 ml / seg, enquanto a feminina foi de 399 ml / seg. Para o PFE, a maior correlação foi observada com a ASC seguida de altura, peso e idade, enquanto o IMC apresentou o menor coeficiente de correlação. TPFE teve a melhor significância com a idade, ASC, altura e IMC. Teve menos significado com o peso. No sexo feminino, a TPFE teve a melhor significância com altura, peso, IMC e idade. CONCLUSÃO: Existem diferenças de gênero na TPFE. Portanto, equações específicas de gênero são necessárias para a estimativa da TPFE


INTRODUCTION: Peak expiratory flow rate (PEFR) is commonly used to monitor the progression of respiratory diseases as it gives good information about the status of airways. A good amount of research is going across the world to establish a local prediction equation. The joint task force of the American thoracic society and European Respiratory Society has promoted research in this regard. In India, data derived from the Caucasian population are still used for PEFR. OBJECTIVE: To verify the relationship between PEF levels and the variables age, sex, anthropometric and body surface area, and establish the regression equation for young Indian adults. METHODS: A cross-sectional observational study was conducted in 15-25 years aged 1000 subjects from the Metropolitan region of Mumbai. Pearson's correlation coefficient was used to understand the relation of anthropometric parameters and PEFR. Multivariate regression analysis was done for establishing a prediction equation (Alpha 5%). RESULTS: Age and all anthropometric parameters were correlated with PEFR. The mean PEFR of the male population was 515 ml/sec, whereas, for females, it was 399 ml/sec, for PEFR highest correlation was observed with BSA (.696) followed by weight (.667), height (.630), age (.504) whereas BMI shown lowest correlation coefficient (.445). PEFR had the best significance with age, BSA, Height, and BMI. It had less significance with weight. In females, PEFR had the best significance with Height, weight, BMI, and Age. CONCLUSION: Gender-wise differences exist in PEFR. Hence gender-specific equations are needed for the estimation of PEFR.

RESUMO Objetivo: Relatar a independência funcional e o grau de comprometimento pulmonar em pacientes adultos 3 meses após a alta da unidade de terapia intensiva. Métodos: Este foi um estudo de coorte retrospectiva conduzido em uma unidade de terapia intensiva multiprofissional para pacientes adultos em um único centro. Incluíram-se pacientes admitidos à unidade de terapia intensiva entre janeiro de 2012 e dezembro de 2013 que, 3 meses mais tarde, foram submetidos à espirometria e responderam ao questionário Medida de Independência Funcional. Resultados: Os pacientes foram divididos em grupos segundo sua classificação de independência funcional e espirometria. O estudo incluiu 197 pacientes, que foram divididos entre os grupos maior dependência (n = 4), menor dependência (n = 12) e independente (n = 181). Na comparação dos três grupos com relação à classificação pela Medida de Independência Funcional, pacientes com maior dependência tinham escores Acute Physiology and Chronic Health Evaluation II e Sequential Organ Failure Assessment mais altos quando da admissão à unidade de terapia intensiva, idade mais avançada, mais dias sob ventilação mecânica e tempo mais longo de permanência na unidade de terapia intensiva e no hospital. A maioria dos pacientes apresentava comprometimento pulmonar, sendo o padrão obstrutivo o mais frequentemente observado. Na comparação da independência funcional com a função pulmonar, observou-se que, quanto pior a condição funcional, pior a função pulmonar, observando-se diferenças significantes em relação ao pico de fluxo expiratório (p = 0,030). Conclusão: Em sua maioria, os pacientes que retornaram ao ambulatório 3 meses após a alta tinham boa condição funcional, porém apresentavam comprometimento pulmonar relacionado com o grau de dependência funcional.


ABSTRACT Objective: To relate functional independence to the degree of pulmonary impairment in adult patients 3 months after discharge from the intensive care unit. Methods: This was a retrospective cohort study conducted in one adult intensive care unit and a multi-professional post-intensive care unit outpatient clinic of a single center. Patients admitted to the intensive care unit from January 2012 to December 2013 who underwent (3 months later) spirometry and answered the Functional Independence Measure Questionnaire were included. Results: Patients were divided into groups according to the classification of functional independence and spirometry. The study included 197 patients who were divided into greater dependence (n = 4), lower dependence (n = 12) and independent (n = 181) groups. Comparing the three groups, regarding the classification of the Functional Independence Measure, patients with greater dependence had higher Acute Physiology and Chronic Health Evaluation II and Sequential Organ Failure Assessment values at intensive care unit admission with more advanced age, more days on mechanical ventilation, and longer stay in the intensive care unit and hospital. The majority of patients presented with pulmonary impairment, which was the obstructive pattern observed most frequently. When comparing functional independence with pulmonary function, it was observed that the lower the functional status, the worse the pulmonary function, with a significant difference being observed in peak expiratory flow (p = 0.030). Conclusion: The majority of patients who returned to the outpatient clinic 3 months after discharge had good functional status but did present with pulmonary impairment, which is related to the degree of functional dependence.

Abstract Introduction: Obesity, characterized by the accumulation of excess body fat, can alter respiratory mechanics and compromise functional capacity. Given its increasing prevalence and the significant morbidity associated with the condition, it is important to investigate techniques that enable rapid, easy measurement of lung function and the possible correlation between obesity and functional capacity. Objective: To assess lung function and functional capacity in obese adults and determine whether there is a correlation between reduced peak expiratory flow and impaired functional capacity. Methods: 30 participants, distributed into two groups: obese (36 ± 13 years) and normal weight (31 ± 9 years) were evaluated based on anthropometric measurements, the six-minute walk test (6MWT) and peak expiratory flow (PEF). Results: The obese individuals showed reduced PEF (382 ± 99 vs. 497 ± 104 L/min, p < 0.01), walked shorter distances in the 6MWT (453 ± 37 vs. 617 ± 50 m, p < 0.01), had higher blood pressure and perceived exertion (p < 0.05) when compared to normal weight participants. Additionally, there was a positive significant association between expiratory flow and distance walked in the 6MWT (r = 0.635 and p < 0.001). Conclusion: Obese individuals exhibited lower PEF and walked shorter distances in the 6MWT, indicating a positive correlation between these two variables. In light of this outcome, the assessment methods used could contribute to improving analysis of respiratory and functional status in this population and aid in exercise prescription.


Resumo Introdução: A obesidade, caracterizada por acúmulo excessivo de tecido adiposo no organismo, pode alterar a mecânica ventilatória, trazendo prejuízos funcionais. Considerando a prevalência crescente da obesidade, bem como a grande morbidade associada a esta condição, torna-se relevante investigar técnicas de mensuração rápidas e fáceis da função ventilatória e a possível correlação com a capacidade funcional. Objetivo: Avaliar a função ventilatória e a capacidade funcional em adultos obesos e determinar se pico de fluxo expiratório reduzido está correlacionado com prejuízo da capacidade funcional. Métodos: 30 participantes, distribuídos em dois grupos: obeso (36 ± 13 anos) e eutrófico (31 ± 9 anos) foram avaliados por meio de medidas antropométricas, do teste de caminhada de seis minutos (TC6´) e da avaliação do pico de fluxo expiratório (PFE). Resultados: Os indivíduos obesos apresentaram reduzido PFE (382 ± 99 vs. 497 ± 104 L/min, p < 0,01) e percorreram menor distância no TC6´ (453 ± 37 vs. 617 ± 50 m, p < 0,01) com maior pressão arterial e percepção de esforço (p < 0,05) quando comparados aos eutróficos. Além disso, foi encontrada correlação positiva e significativa entre fluxo expiratório e distância percorrida (r = 0,635 e p < 0,001). Conclusão: Os indivíduos obesos tiveram menor PFE e menor distância percorrida no TC6´, constatada correlação positiva entre essas duas variáveis. Diante de tal desfecho, a inclusão dos métodos avaliativos utilizados podem contribuir para melhor análise da condição respiratória e funcional dessa população e auxiliar na prescrição de exercícios.

OBJECTIVES: to compare the obtained and predicted peak expiratory flow (PEF) values between frail, pre-frail, and non-frail older adults; verify the association between PEF and FS; and establish cut-off points for PEF as determinants of frailty syndrome (FS). DESIGN: Cross-sectional study. LOCATION: Macapá, Amapá, Brazil. PARTICIPANTS: Community-based study community-dwelling older people. METHODS: PEF was evaluated using a peak-flow meter and FS was evaluated using Fried's frailty phenotype. The statistical analyses performed included a multinomial logistic regression model and Receiver Operating Characteristic (ROC) curves to establish cut-off points for discriminating FS. RESULTS: 409 older adults with a mean age of 70.09±7.22 years were evaluated; of these, 58.7% were pre-frail, and 12.7% were frail. PEF was lower in frail than in pre-frail participants, and lower in pre-frail than in non-frail. Frail and pre-frail older adults obtained lower PEF values than predicted. PEF (% Predicted) was inversely associated with frailty and pre-frailty even after adjustment. Cut-off points were established to discriminate the presence of frailty in older men (PEF≤350L/min; AUC=0.669; sensitivity=76.92%; specificity=52%) and women (PEF≤220L/min; AUC=0.597; sensitivity=71.79%; specificity=46.98%). CONCLUSIONS: Frail and pre-frail older adults presented significantly lower PEF than non-frail participants, and these values were lower than predicted. PEF was inversely associated with frailty and pre-frailty. PEF cut-off points can be used as a complementary method to indicate frailty in community-dwelling older adults.

OBJECTIVES: The purpose of this study was to compare the predictive value of three cough peak flow (CPF) maneuvers in predicting the extubation outcome in a cohort of mechanically ventilated subjects. METHODS: Eighty-one mechanically ventilated subjects who succeeded in the spontaneous breathing trial were included. In a randomized order, CPF was stimulated and measured using three methods: voluntary command (V_CPF), tracheal saline instillation (S_CPF), and mechanical stimulation with a catheter (C_CPF). Additionally, CPF was measured 20 min after the extubation (PE_CPF). The diagnostic accuracy of the CPF methods in relation to the extubation outcome was measured using the receiver operating characteristic (ROC) curve. ROC curve results were compared using the Hanley and McNeil method. RESULTS: The three methods presented high accuracy in predicting the extubation outcome (V_CPF = 0.89, S_CPF = 0.93, and C_CPF = 0.90), without statistically significant differences between them (V_CPF vs. S_CPF, p = 0.14; V_CPF vs. C_CPF, p = 0.84; S_CPF vs. C_CPF, p = 0.13). The optimum cutoff values were V_CPF = 45 L/min, S_CPF = 60 L/min, and C_CPF = 55 L/min. PE_CPF also showed high accuracy in predicting the extubation outcome (AUC = 0.95; cutoff = 75 L/min). CONCLUSIONS: In mechanically ventilated and cooperative subjects, there is no difference in the accuracy of CPF measured voluntarily, with stimulation using saline or by catheter stimulation in predicting the reintubation. CPF recording after endotracheal tube removal has high accuracy to predict the extubation outcome.

Introdução: a disfunção pulmonar no pós-operatório de cirurgia cardíaca continua sendo uma das mais importantes causas demorbidade, sendo que o comprometimento da função pulmonar, nessa circunstância, é frequente e contribui, significativamente, para o aumento do tempo de permanência hospitalar. O objetivo deste estudo foi avaliar o comportamento da função pulmonar em pacientes adultos hospitalizados, submetidos à cirurgia cardíaca por esternotomia mediana, no momento a alta da unidade tratamento intensivo, comparado ao quarto dia após a alta dessa unidade. Metodologia: o estudo foi realizado em uma unidade de pós-operatório de cirurgia cardiovascular, após aprovação do Comitê de Ética em Pesquisa. O Termo de Consentimento Livre Esclarecido foi obtido em todos os casos. Incluíram-se pacientes maiores de 18 anos, submetidos à cirurgia cardíaca eletiva. A função pulmonar foi realizada na alta da unidade de tratamento intensivo e, posteriormente, repetida no quarto dia. A função pulmonar foi mensurada pela capacidade vital forçada, pico de fluxo expiratório, além de variáveis do exame físico, como frequência respiratória e ausculta pulmonar. Resultados: 94 pacientes foram submetidos à cirurgia cardíaca eletiva via esternotomia mediana. A média (desvio padrão) de idade foi 50,64 (16,53) anos, com predomínio do sexo masculino (52,1%). A mediana (Q1-Q3) do tempo de permanência na unidade de tratamento intensivo foi de 2,00 dias (2,00-3,00), ao passo que a mediana (Q1-Q3) do tempo de permanência hospitalar foi de 6,00 dias (5,00-8,00). A mediana (Q1-Q3) da capacidade vital forçada e do fluxo respiratório, obtida pela espirometria no pós-operatório, no momento da alta, foi significativamente menor quando comparada ao quarto dia após alta da unidade de tratamento intensivo (p< 0,01). O tempo de permanência na unidade, assim como o tempo de permanência hospitalar não impactou na evolução das variáveis de função pulmonar. Conclusão: a função pulmonar é potencialmente reduzida no período inicial após a cirurgia cardíaca, com evolução gradual, e de recuperação lenta, no curso da recuperação cirúrgica. A manutenção desses valores reduzidos a níveis não críticos por um período prolongado no pós-operatório parece não afetar o desempenho dos pacientes, no que tange a capacidade de respirar profundamente e na efetividade de tosse.


Introduction: pulmonary dysfunction in the postoperative period of cardiac surgery continues to be one of the most important causes of morbidity, and the involvement of pulmonary function in this circumstance is frequent and contributes significantly to an increase in the length of hospital stay. Objective: the objective of this study was to evaluate the pulmonary function behavior in hospitalized adult patients submitted to cardiac surgery by median sternotomy at the time of discharge from the intensive care unit, compared to the fourth day after discharge from this unit. Metodology: the study was performed in a postoperative unit of cardiovascular surgery, after approval of the Research Ethics Committee. The Informed Consent Form was obtained in all cases. Patients older than 18 years who underwent elective cardiac surgery were included. Pulmonary function was performed on discharge from the intensive care unit and then repeated on the fourth day. Pulmonary function was measured by forced vital capacity, peak expiratory flow, besides variables of physical examination, such as respiratory rate and pulmonary auscultation. Results: a number of 94 patients underwent elective cardiac surgery via median sternotomy. Mean (standard deviation) of age was 50.64 (16.53) years, with a predominance of males (52.1%). Median (Q1-Q3) of the time spent in the intensive care unit was 2.00 days (2.00-3.00), while the median (Q1-Q3) of the length of hospital stay was 6.00 days (5.00-8.00). Median (Q1-Q3) of forced vital capacity and respiratory flow, obtained by spirometry in the postoperative period at discharge, was significantly lower when compared to the fourth day after discharge from the intensive care unit (p <0.01 ). The length of stay in the unit, as well as the length of hospital stay, did not affect the evolution of pulmonary function variables. Conclusion: pulmonary function is potentially reduced in the initial period after cardiac surgery, with gradual evolution, and slowness in the course of surgical recovery. Maintaining these reduced values at uncritical levels for a prolonged postoperative period does not appear to affect patients' performance with regard to their ability to breathe deeply and in cough effectiveness.

BACKGROUND: Sensitization to Blomia tropicalis (Bt) is very frequent in the tropics, and particularly in Cuba, being a significant cause of allergic asthma. Allergen immunotherapy (AIT) with Bt can be a therapeutic option, however, placebo-controlled clinical trials have not been reported. OBJECTIVE: To assess the therapeutic effect and safety of AIT for asthma using a standardized allergen vaccine of B. tropicalis by subcutaneous route, in allergic asthmatic patients exposed and sensitized to this mite species. METHODS: A double-blind, placebo-controlled Phase II trial was conducted in 35 adults (18 with treatment and 17 with placebo), with mild to moderate asthma, predominantly sensitized to Bt. AIT was administered subcutaneously in increasing doses from 4 to 6000 Biological Units using a locally manufactured standardized extract (BIOCEN, Cuba). Patient assessment was performed using symptom-medication score (SMS), peak expiratory flow and skin reactivity relative to Histamine as measured by skin prick test (SPT). RESULTS: The 12-month treatment achieved a significant (p < 0.001) decrease of SMS. Symptom score showed only 41% (CI: 26-61) of placebo values, whereas medication was 34.5% (22.4%-63.3%). Treatment was regarded clinically effective in 67% of patients (OR 32; 95%CI: 17 to 102). The effect size on symptoms and medication was higher than has been reported with equivalent allergen dosages of D. pteronyssinus and D. siboney in Cuban asthmatic patients. Skin reactivity to Bt was also significantly reduced (p = 0.0001), increasing 148-fold the allergen threshold to elicit a positive skin test. This desensitization effect was specific to Bt and did not modify the reactivity to Dermatophagoides. The change of specific skin reactivity was significantly (p < 0.05) correlated to clinical improvement. All adverse events were local with a frequency of 2.4% of injections. CONCLUSIONS: Subcutaneous AIT with Blomia tropicalis was effective and safe in asthmatic adults exposed and sensitized to this mite species in a tropical environment. Abstract Background: Atrial fibrillation (AF) is associated with increased mortality in heart failure (HF) patients. Objective: To evaluate whether the risk of AF patients can be precisely stratified by relation with cardiopulmonary exercise test (CPET) cut-offs for heart transplantation (HT) selection. Methods: Prospective evaluation of 274 consecutive HF patients with left ventricular ejection fraction ≤ 40%. The primary endpoint was a composite of cardiac death or urgent HT in 1-year follow-up. The primary endpoint was analysed by several CPET parameters for the highest area under the curve and for positive (PPV) and negative predictive value (NPV) in AF and sinus rhythm (SR) patients to detect if the current cut-offs for HT selection can precisely stratify the AF group. Statistical differences with a p-value <0.05 were considered significant. Results: There were 51 patients in the AF group and 223 in the SR group. The primary outcome was higher in the AF group (17.6% vs 8.1%, p = 0.038). The cut-off value of pVO2 for HT selection showed a PPV of 100% and an NPV of 95.5% for the primary outcome in the AF group, with a PPV of 38.5% and an NPV of 94.3% in the SR group. The cut-off value of VE/VCO2 slope showed lower values of PPV (33.3%) and similar NPV (92.3%) to pVO2 results in the AF group. Conclusion: Despite the fact that AF carries a worse prognosis for HF patients, the current cut-off of pVO2 for HT selection can precisely stratify this high-risk group.


Resumo Fundamento: A fibrilação atrial (FA) está associada ao aumento da mortalidade em pacientes com insuficiência cardíaca (IC). Objetivo: Avaliar se o risco de pacientes com FA pode ser estratificado com precisão em relação aos pontos de corte do teste de esforço cardiopulmonar (TECP) para seleção do transplante cardíaco (TC). Métodos: Avaliação prospectiva de 274 pacientes consecutivos com IC com fração de ejeção do ventrículo esquerdo ≤ 40%. O endpoint primário foi um composto de morte cardíaca ou TC urgente no seguimento de 1 ano. O endpoint primário foi analisado através de vários parâmetros do TECP para a maior área sob a curva e para o valor preditivo positivo (VPP) e negativo (VPN) em pacientes com FA e ritmo sinusal (RS) para detectar se os atuais pontos de corte para a seleção de TC podem estratificar com precisão o grupo com FA. Diferenças estatísticas com valor de p < 0,05 foram consideradas significativas. Resultados: Havia 51 pacientes no grupo de FA e 223 no grupo RS. O endpoint primário foi maior no grupo FA (17,6% vs. 8,1%, p = 0,038). O valor de corte de pVO2 para a seleção do TC mostrou um VPP de 100% e um VPN de 95,5% para o endpoint primário no grupo FA, com um VPP de 38,5% e um VPN de 94,3% no grupo RS. O valor de corte da inclinação VE/VCO2 apresentou valores mais baixos de VPP (33,3%) e valor semelhante de VPN (92,3%) aos resultados de pVO2 no grupo FA. Conclusões: Apesar do fato de a FA apresentar um pior prognóstico para os pacientes com IC, o atual ponto de corte de pVO2 para a seleção de TC pode estratificar com precisão esse grupo de alto risco.

Abstract Subclinical ventilatory dysfunction is observed in individuals with spinocerebellar ataxias (SCA). No studies have correlated ventilatory dysfunction to clinical and functional decline in SCA2. Objective: To evaluate the relationship between the values of peak expiratory flow (PEF), maximum inspiratory pressure (MIP), and presence of respiratory complaints with age, disease duration, age at onset of symptoms, balance scores, independence in basic (ADL) and instrumental (IADL) Activities of Daily Living (ADLs), and severity of ataxia (SARA) in individuals with SCA2. Methods: Cross-sectional study evaluating age, disease duration, age at onset of symptoms, scores in the Berg Balance Scale and in the SARA, Functional Independence Measure and Lawton's scale, values of PEF and MIP, and the presence of respiratory complaints. Results: The study included 36 individuals with SCA2, with a mean age of 42.5±2.4 years, disease duration of 7.6±8.2 years, age 33.7±11.5 years at onset of symptoms, and 9.9±10.3 points in the SARA scale. The lowest PEF values correlated with the longer disease duration (p=0.021). The lowest values of PEF and MIP correlated with greater balance impairment (p=0.019 and p=0.045, respectively), increased degree of dependence in the ADL (p=0.006 and p=0.050, respectively) and IADL (p=0.003 and p=0.001, respectively) scales, and highest severity of ataxia (p=0.00 and p=0.017, respectively). Respiratory complaints were observed in 12 (33.3%) individuals and were not related to age, disease duration, age at onset of symptoms, balance, independence, ataxia severity, or PEF and MIP values. Conclusion: Ventilatory dysfunction, even when asymptomatic, is related to balance impairment, independence, and ataxia severity in individuals with SCA2.


Resumo Disfunção ventilatória subclínica tem sido observada em indivíduos com ataxias espinocerebelares (SCA). Não existem estudos relacionando disfunção ventilatória ao declínio clínico e funcional na SCA2. Objetivo: Avaliar a relação dos valores de Pico de Fluxo Expiratório (PFE), Pressão Inspiratória Máxima (PIMAX) e presença de queixas respiratórias com idade, tempo de doença, idade de início dos sintomas, escore de equilíbrio, independência para atividades básicas (AVD) e instrumentais (AIVD) de vida diária e gravidade da ataxia (SARA) em indivíduos com SCA2. Métodos: Estudo transversal, considerando: idade, tempo de doença, idade de início dos sintomas, escores nas Escalas SARA, Equilíbrio de Berg, Medida da Independência Funcional e de Lawton, valores de PFE, PIMAX e queixas respiratórias. Resultados: Foram avaliados 36 indivíduos com SCA2 com média de 42,5±2,4) anos de idade, 7,6±8,2 anos de tempo de doença, 33,7±11,5 anos de idade de início dos sintomas e 9,9±10,3 pontos na escala SARA. Os menores valores de PFE estiveram relacionados ao maior tempo de doença (p=0,021). Os menores valores de PFE e PIMAX estiveram relacionados ao maior comprometimento do equilíbrio (p=0,019; p=0,045, respectivamente), maior dependência para ADV (p=0,006; p=0,050, respectivamente) e AIVD (p=0,003; p=0,001, respectivamente) e maior gravidade da ataxia (p=0,006; p=0,017, respectivamente). Foram observadas queixas respiratórias em 12 (33,3%) indivíduos que não estiveram relacionadas à idade, idade de início dos sintomas, tempo de doença, equilíbrio, independência, gravidade da ataxia, ou valores de PFE e PIMAX. Conclusão: A disfunção ventilatória, mesmo quando assintomática, está relacionada ao comprometimento do equilíbrio, à independência e à gravidade da ataxia em indivíduos com SCA2.

BACKGROUND: The diagnosis of asthma is confirmed with a spirometry: FEV1 ratio (forced expiratory volume in one second)/FVC (forced vital capacity) <80% with reversibility (FEV1 >12% or 200 mL) after using salbutamol. The peak expiratory flow is cheap and easy to use; it measures the forced expiratory flow, of which reversibility > 20% suggests asthma. OBJECTIVE: To know the sensitivity, specificity, and the positive and negative predictive values of the flowmeter. METHODS: A cross-sectional, observational, comparative study. Individuals aged >18 years without contraindications for spirometry were included. They underwent spirometry and peak expiratory flow, and the ACT (Asthma Control Test) questionnaire was applied to them. Sensitivity, specificity, positive predictive value and negative predictive value of the flowmetry were calculated. ROC curve was carried out in order to know the cut-off point of greater sensitivity and specificity. RESULTS: Of 150 patients, 66% were male; the median age was 38 years. According to the guidelines of GINA 2018 (Global Initiative for Asthma); 58.7% were controlled. The sensitivity of the peak expiratory flow was 47%, and the specificity was 87%, with a positive predictive value of 54.8% and a negative predictive value of 84%. The peak expiratory flow showed higher specificity with FEV1 <59%. The cut-off point of greater sensitivity and specificity was a reversibility of 8%, with an area under the curve of 0.70. CONCLUSIONS: The flowmeter has got greater sensitivity in airway obstructions; it is useful when a spirometer is not available.


Antecedentes: El diagnóstico de asma se confirma con espirometría: VEF1 (volumen espiratorio forzado del primer segundo)/CVF (capacidad vital forzada) < 80 %, con reversibilidad (VEF1 >12 % o 200 mL) tras utilizar salbutamol. El flujómetro es barato y fácil de utilizar, mide el flujo espiratorio forzado, cuya reversibilidad > 20 % sugiere asma. Objetivo: Conocer sensibilidad, especificidad y valores predictivos positivos y negativo del flujómetro. Métodos: Estudio transversal, observacional, comparativo. Se incluyó a individuos > 18 años sin contraindicaciones para espirometría, quienes fueron sometidos a espirometría y flujometría y se les aplicó el Asthma Control Test. Se calculó sensibilidad, especificidad y valores predictivos positivo y negativo de la flujometría. Se realizó curva ROC para conocer el punto de corte de mayor sensibilidad y especificidad. Resultados: De 150 pacientes, 66 % fue del sexo masculino; la mediana de edad fue de 38 años. Conforme los criterios de Global Initiative for Asthma 2018, 58.7 % estaba controlado. La sensibilidad de la flujometría fue de 47 %, la especificidad de 87 %, valor predictivo positivo de 54.8 % y negativo de 84 %. La flujometría mostró mayor especificidad con VEF1 < 59 %. El punto de corte de mayor sensibilidad y especificidad fue una reversibilidad de 8 %, con área bajo la curva de 0.70. Conclusiones: El flujómetro tiene mayor sensibilidad en obstrucciones de vía aérea; es de utilidad cuando no se cuenta con un espirómetro.

Objective: We compare physical performance from three U.S. national surveys and nationally representative surveys in England, Taiwan, and Costa Rica. Method: For each performance test, we use local mean smoothing to plot the age profiles by sex and survey wave and then fit a linear regression model to the pooled data, separately by sex, to test for significant differences across surveys controlling for age and height. Results: Age profiles of performance vary across U.S. surveys, but levels of lung function (peak expiratory flow) and handgrip strength in the United States are as high as they are in the other three countries. Americans also perform as well on the chair stand test as the English and Costa Ricans, if not better, but exhibit slower gait speed than the English at most ages. Discussion: With the exception of walking speed, we find little evidence that older Americans have worse physical performance than their peers.

Objective: To analyze the peak expiratory flow (PEF) in obese subjects in standing (ST), seated (SE), supine (SU), prone (PR), and right and left lateral decubitus (RL and LL) positions. Methods: 36 obese volunteers, aged between 18 and 54 years, and nonsmokers, were evaluated by anthropometric data; spirometry; international physical activity questionnaire (IPAQ); and PEF in ST, SE, SU, PR, RL, and LL positions, randomly. Results: The mean age was 27.3 ± 7.4 years and BMI of 38.7 ± 3.2 kg/m2; IPAQ results: active (50%), irregularly active (25%), very active (16.7%), and sedentary (08.3%); spirometry results: FVC = 100.33%, FEV1 = 92.83%, FEV1/FVC ratio = 0.95, FEF25-75% = 85.55%. The PEF (L/min) were within the normal range and there was no difference between the ST and SE (428.6 ± 100.9 and 430.0 ± 100.1). Comparing ST and SE to the other positions, in absolute and percent of predicted values (L/min and %), there was a decrease in the SU (85.4 ± 14.9), PR (401.1 ± 92.4 and 84.1 ± 14.4), RL (398.6 ± 94.7 and 83.3 ± 14.9) (p < 0.05), and there was no difference to SU (409.4 ± 102.8) and LL (410.3 ± 93.9 and 85.8 ± 14.5). Conclusion: The PEF of healthy obese are similar in the standing and seated positions. The PEF decreases in the lying positions, except for the LL, that could be used as an alternative for measurements.

BACKGROUND: Several authorities recommend measuring peak expiratory flow (PEF) standing. Limited evidence suggests that PEF obtained sitting is similar in magnitude but there are no studies in African populations. AIM: To determine in adults aged 18-60 years if PEF measured sitting differs from that measured standing. DESIGN & SETTING: Crossover design with alternating position of initial measurement in people attending primary care clinics in Barbados. METHOD: Quota sampling by age, sex, and clinic of adults aged 18-60 years was done and an interviewer-administered questionnaire was completed. PEF sitting and standing was measured with an European Union (EU) scale Mini-Wright® meter. The highest of three readings in each position was used and the difference in means tested for significance using the paired sample t-test. RESULTS: Characteristics of the 199 participants were 44% male; 96.5% of African descent; mean age 37 years (standard deviation [SD] 12.8); 22% with an asthma diagnosis; 23% tobacco users; and 22% marijuana users. Mean PEF standing was 438.4 versus 429.7 lmin-1 sitting, mean difference 8.7 (95% confidence interval [CI] = 3.6 to 13.8). For men, mean PEF standing was 518.7 versus 506.3 lmin-1 sitting, mean difference 12.4 (95% CI = 3.3 to 21.5). For women, mean PEF was 374.7 standing versus 368.9 lmin-1 sitting, mean difference 5.8 (95% CI = 0.11 to 11.5). A Bland-Altman plot accounting for trend and a Lin's correlation coefficient of 0.935 demonstrated good agreement between standing and sitting PEF. CONCLUSION: PEF measurements are reduced when performed sitting compared to standing. The difference is small and unlikely to alter clinical management in most cases.

INTRODUÇÃO: A tosse, voluntária ou reflexa, é um dos principais mecanismos de depuração para proteção das vias aéreas. O pico de fluxo de tosse (PFT) é um método de avaliação específica, mas que vem demonstrando utilidade para avaliar riscos de complicação pulmonares bem como sua gravidade. Variável muito semelhante ao pico de fluxo expiratório cuja principal diferença consiste no fechamento da glote durante a manobra de tosse. Então avaliar a tosse é importante para identificar pacientes com alterações no pico de fluxo de tosse, e consequente risco de complicações pulmonares. OBJETIVO: Analisar o PFT voluntário e parâmetros clínicos e epidemiológicos em uma população internada nas enfermarias de um hospital de urgências. MÉTODOS: Estudo transversal com pacientes internados nas enfermarias de um Hospital Público de Goiânia. Foram coletados dados epidemiológicos e clínicos. O PFT foi mensurado pelo peak flow meter e a avaliação de dor através da Escala de Dor Visual Numérica associada à Escala de Dor de Faces. RESULTADOS: A amostra foi composta em sua maioria por homens 288 (81,36). A média de idade da população estudada foi de 45,91 anos (±20,14). Sessenta e um por cento dos pacientes (219) apresentaram tosse eficaz, e 43 (12,15%) tosse ineficaz, revelou associação com idade, sexo e diagnóstico. CONCLUSÃO: Os pacientes que mais apresentaram alterações na força de tosse foram os idosos, sexo feminino e vítimas acometidas por desordens neurológicas e toracoabdominais. [AU]


INTRODUCTION. Cough, whether voluntary or reflex, is one of the major clearance mechanisms for airway protection. Peak cough flow is a recent and specific evaluation method, but it has been useful for evaluating pulmonary complications as well as their severity. Variable very similar to the peak of expiratory flow whose main difference consists in the closure of the glottis during the maneuver of cough. Therefore, assessing cough is important to identify patients with changes in peak cough flow, and the consequent risk of pulmonary complications. OBJECTIVE: To analyze the peak flow of voluntary cough and its association with clinical and epidemiological parameters in a population hospitalized in the wards of an emergency hospital. METHODS: A cross-sectional study with 354 patients admitted to the wards of a Public Hospital of Goiânia. Epidemiological and clinical data were collected. Optic cough flow was measured by the peak flow meter and the pain assessment using the Numerical Visual Pain Scale associated with the Face Pain Scale. RESULTS: The sample consisted mostly of 288 (81,36) men. The mean age of the study population was 45,91 years (± 20,14). 61, 86% (219) of the patients had effective cough, and 43 (12,15%) had an ineffective cough. CONCLUSION: Patients who presented the greatest changes in coughing strength were the elderly, females and victims of neurological and thoracoabdominal disorders. [AU]

Resumen Introducción: Las enfermedades respiratorias crónicas tienen alta prevalencia en países en desarrollo, en poblaciones rurales y deprivadas. El flujo espiratorio máximo (FEM) obtenido mediante espiración forzada tiene uso clínico y de investigación. Describimos valores de medición del FEM en la medición basal de un estudio de cohorte en curso (Cohorte del Maule-MAUCO). Material y Método: Diseño transversal en 3.465 adultos (40-74 años) con registros de FEM (ATS). (Flujómetro Mini-Wright), usando valores de Gregg y Nunn. Valores < 80% del predicho se consideraron disminuidos. Se obtuvo sexo, edad, nivel educacional, actividad física y tabaquismo; se calculó índice de masa corporal (IMC) usando mediciones antropométricas. Resultados: La muestra tuvo 63,9% de mujeres; edad media de 55 (± 9) años, escolaridad de 9 (± 4) años; sobrepeso y obesidad fueron 43,1% y 41,5%: 81,5% fueron inactivos y 29,4% fumadores actuales. El valor medio de FEM fue 330 (± 80) L/min (mujeres) y 460 (± 119) L/min (hombres): el FEM disminuido alcanzó el 50,6% ([48,9-52,3]) con diferencias según edad, educación, IMC y actividad física. Conclusiones: Se observó alta prevalencia de FEM disminuido con variaciones según sexo, edad, escolaridad, IMC e inactividad física. Como otros estudios latinoamericanos, los valores bajos también fueron altamente prevalentes, sugiriendo sobreestimación de valores predichos al usar valores de Gregg & Nunn. Estos resultados sugieren la conveniencia de estudiar factores ambientales locales.


Introduction: Chronic respiratory (CRD) diseases show high prevalence in developing countries, rural and deprived populations. Peak expiratory flow rate (PEFR) is a functional measurement obtained through forced expiratory used for clinical and research purposes. We described PEFR in a rural setting in an ongoing cohort study (Maule Cohort-MAUCO). Material and Method: Cross-sectional design in 3,465 adults (40-74 years) with PEFR ATS standard records (Mini-Wright flowmeter) using Gregg and Nunn values. PEFR ≤ 80% predicted were considered decreased. Sex, age, educational level, physical activity and tobacco smoking were obtained. Body mass index (BMI) was calculated based on anthropometrical measurements. Main Results: Sample had 63.9% of women; mean age of 55 (± 9) years, schooling of 9 (± 4) years. Overweight and obesity were 43.1% and 41.5%. Physical inactivity was 81.5% and 29.4% were current smokers. PEFR mean value was 330 (± 80) L/min (women) and 460 (± 119) L/min (men): Decreased PEFR was 50.6% ([48.9-52.3]) with significant differences by age, schooling, BMI and physical activity. Conclusions: High prevalence of decreased PEFR was observed: PEFR showed variations according to sex, age, schooling, BMI and physical activity. As other Latin-American studies show, low values were also highly prevalent, suggesting that Gregg & Nunn overestimated PEFR values. These results suggests the convenience of studying local environmental factors.

Abstract Introduction: The peak expiratory flow (PEF) is a practical method that evaluates the caliber of the proximal airways. The ease in handling and the low cost of portable PEF meters made their use frequent. However, few studies have examined the correlation between the different equipment and compared their reference values. Objective: Analyze the correlation of three portable meters of PEF and check if the reference values are applicable in the studied population. Methods: 168 healthy individuals, of both genders, ranging in age from 20 to 80 years, had their PEF measured in Mini-Wright®, Assess® and AirZone® and the values obtained were compared to those predicted by Leiner et al. (1963) and Nunn and Gregg (1989). Statistical analysis was performed by the Kolmogorov-Smirnov tests, ANOVA, Wilcoxon test, Spearman correlation and analysis of agreement of Bland-Altman (p< 0.05). Results: There was significant difference in the values of PEF obtained between the Mini-Wright® and Assess® meters in both genders, between the AirZone® and Assess® only in men and between the Mini-Wright® and AirZone® only in women. The predicted values in the three meters have overestimated the obtained in both genders; there was no correlation between the values obtained from three meters due to the great variation of agreement limits and large interindividual variation. Conclusion: The values obtained in the three meters are not interchangeable and the predicted values were unsuitable for the sample.


Resumo Introdução: O pico de fluxo expiratório (PFE) é um método acessível que avalia o calibre das vias aéreas proximais. A facilidade no manuseio e o baixo custo dos medidores portáteis de PFE tornaram seu uso frequente, entretanto, poucos estudos analisaram a concordância entre os diferentes equipamentos e compararam seus valores de referência. Objetivo: Analisar a concordância de três medidores portáteis de PFE e verificar se os valores de referência são aplicáveis na população estudada. Métodos: 168 indivíduos saudáveis, de ambos os gêneros, na faixa etária de 20 a 80 anos, realizaram medidas de PFE nos aparelhos Mini-Wright®, Assess® e AirZone® e comparou-se os valores obtidos aos previstos por Leiner et al. (1963) e Nunn e Gregg (1989). A análise estatística foi realizada pelos testes Kolmogorov-Smirnov, ANOVA, teste de Wilcoxon, correlação de Spearman e análise de concordância de Bland-Altman (p<0,05). Resultados: Constatou-se diferença significativa nos valores de PFE obt idos entre os medidores Mini-Wright® e Assess® em ambos os gêneros, entre o AirZone® e o Assess® apenas nos homens e entre o Mini-Wright® e o AirZone® somente nas mulheres, sendo que os valores previstos nos três aparelhos superestimaram os obtidos em ambos os gêneros; não se verificou concordância entre os valores obtidos dos três medidores devido à grande variação dos limites de concordância e grande variação interindividual. Conclusão: Os valores obtidos nos três medidores não são intercambiáveis e os valores previstos se mostraram inadequados para a amostra estudada.

Introducción. El entrenamiento muscular respiratorio es el método más utilizado para revertir la debilidad muscular respiratoria; no obstante, el efecto de protocolos basados en maniobras no respiratorias no ha sido suficientemente estudiado en población pediátrica. El objetivo de este estudio fue determinar el efecto del entrenamiento de la musculatura abdominal sobre la fuerza de los músculos respiratorios y los flujos espiratorios forzados en adolescentes sanos. Métodos. Estudio cuasiexperimental. La muestra estuvo integrada por adolescentes sanos divididos en dos grupos: un grupo experimental, quienes realizaron 8 semanas de entrenamiento de la musculatura abdominal activo, y un grupo control equivalente. Se midió la fuerza de los músculos abdominales, la presión inspiratoria máxima, la presión espiratoria máxima (PeMax), el flujo espiratorio máximo y el flujo máximo de tos antes y después del protocolo. Se consideró significativo un valor de p < 0,05. Resultados. Todas las variables estudiadas aumentaron significativamente en el grupo experimental; sin embargo, solo la PeMax se incrementó en el grupo control. Adicionalmente, el grupo experimental presentó una mayor PeMax que el grupo control al final del protocolo, así como también una mayor ganancia en la PeMax y flujo espiratorio máximo. Finalmente, el incremento en la PeMax se asoció con el aumento en el flujo máximo de tos en el grupo experimental, lo que no fue observado en el grupo control. Conclusión. En 8 semanas de entrenamiento de la musculatura abdominal, se logró incrementar la PeMax y el flujo espiratorio máximo en adolescentes sanos (sedentarios). Estos efectos se asociaron a incrementos en el flujo máximo de tos inducidos por la intervención.


Introduction. Respiratory muscle training is the most commonly used method to revert respiratory muscle weakness; however, the effect of protocols based on non-respiratory maneuvers has not been adequately studied in the pediatric population. The objective of this study was to establish the effect of abdominal muscle training on respiratory muscle strength and forced expiratory flows in healthy adolescents. Methods. This was a quasi-experiment. The sample was made up of healthy adolescents divided into two groups: an experimental group who completed eight weeks of active abdominal muscle training, and an equivalent control group. The following indicators were measured: abdominal muscle strength, maximal inspiratory pressure, maximal expiratory pressure (MEP), peak expiratory flow, and peak cough flow, before and after protocol completion. A value of p < 0.05 was considered significant. Results. All studied outcome measures increased significantly in the experimental group but only MEP increased in the control group. In addition, a higher MEP was observed in the experimental group compared to the control group at the end of the protocol, together with a greater increase in MEP and peak expiratory flow. Finally, the increase in MEP was associated with an increase in cough peak flow in the experimental group but not in the control group. Conclusion. After eight weeks of abdominal muscle training, MEP and peak expiratory flow increased in healthy (sedentary) adolescents. Such effects were associated with intervention-induced increases in cough peak flow.
